RESUMO O objetivo deste estudo foi produzir análise multimodal exploratória sobre a atenção compartilhada. Utilizou-se a ferramenta ELAN (EUDICO Language Annotator), software com recursos para sincronização temporal e especiais para modalidades verbal e não verbal, que facilitam a visualização e anotação de contextos interacionais. Foram analisados e transcritos trechos de dez minutos de sessão de avaliação fonoaudiológica de uma criança em investigação para transtorno do espectro do autismo (TEA) e outra, com desenvolvimento típico, pareadas por faixa etária e gênero. Foram investigadas e quantificadas as ocorrências espontâneas de direcionamento do olhar das crianças para: os olhos do interlocutor; os brinquedos e/ou brincadeiras; os olhos do interlocutor – brinquedos (atenção compartilhada). As ocorrências de direcionamento do olhar diferiram entre as crianças de forma quantitativa e qualitativa. A criança com suspeita de TEA não produziu episódio de atenção compartilhada, nem direcionou seu olhar para o interlocutor. Foram 56 ocorrências de direcionamentos de olhar apenas para os brinquedos. A criança em desenvolvimento típico produziu 18 ocorrências de atenção compartilhada, sendo que, em todas as vezes que direcionou o olhar para os olhos do interlocutor, o fez para compartilhar o brinquedo ou brincadeira. Observou-se, ainda, 37 ocorrências de direcionamento do olhar para o brinquedo. A partir da análise exploratória produzida pela ferramenta ELAN, foi possível observar que houve diferença em número de ocorrência e trajetória do olhar entre as duas crianças e verificar que a atenção compartilhada esteve ausente na criança com risco para TEA. ABSTRACT The aim of this study was to produce exploratory multimodal analysis on joint attention. We used the ELAN tool: software with resources for temporal and special synchronization for verbal and non-verbal modalities that facilitate the visualization and annotation of interactional contexts. Excerpts of ten minutes of speech and language pathology evaluation of the child with suspected ASD and of another child with typical development matched by age group and gender, were analyzed and transcribed. Spontaneous occurrences of the child's gaze were investigated: a) Towards the interlocutor's eyes. b) Towards toys or play. c) Towards the interlocutor's eyes and toys (Joint Attention). The occurrences of look direction differed between children in a quantitative and qualitative way. The child with suspected ASD did not produce an episode of joint attention, nor did he direct his gaze to the interlocutor. There were 56 occurrences of gazing towards the toys. The typical child produced 18 occurrences of joint attention, and whenever he directed his eyes to the interlocutor's eyes, he did so with the intention of sharing the toy or play. We also observed 37 occurrences of gazing towards the toy or play. From the exploratory analysis produced by ELAN tool, it was possible to observe that there were differences in the number of occurrences and look direction between the evaluated children and to verify that the joint attention was absent in the child with ASD risk.

RESUMO Objetivo Sabe-se que alguns autistas são considerados não verbais, uma vez que não são hábeis para utilizar o código linguístico. E tampouco usam gestos para compensar a ausência de fala. Sendo assim, a habilidade comunicativa desses indivíduos pode ser beneficiada pelo uso do sistema de comunicação alternativa Picture Exchange Communication System – PECS. O objetivo deste estudo foi verificar os vocábulos mais frequentemente utilizados na implementação do PECS em crianças autistas. E, de forma complementar, analisar a correlação entre a frequência destes vocábulos e o índice de comportamentos não adaptativos. Método Trata-se de um estudo transversal. A amostra foi constituída por 31 crianças autistas, sendo vinte e cinco meninos e seis meninas, na faixa etária de 5 a 10 anos. Para identificação dos vocábulos mais frequentemente utilizados no período inicial de implementação do PECS, utilizamos a Planilha de Seleção de Vocabulário. E, para obtermos o índice de comportamentos não adaptativos, aplicamos o Autism Behavior Checklist (ABC). Resultados Houve predomínio significativo de itens na categoria alimentos, seguido de atividade e bebidas. Não houve correlação entre o total de itens identificados pelas famílias com o índice de comportamentos não adaptativos. Conclusão Foi possível identificar as categorias de vocábulos mais mencionados pelas famílias e verificar que o índice de comportamentos não adaptativos não interferiu diretamente na elaboração da planilha de seleção de vocábulos das crianças estudadas. ABSTRACT Purpose It is known that some autistic individuals are considered non-verbal, since they are unable to use verbal language and barely use gestures to compensate for the absence of speech. Therefore, these individuals’ ability to communicate may benefit from the use of the Picture Exchange Communication System – PECS. The objective of this study was to verify the most frequently used words in the implementation of PECS in autistic children, and on a complementary basis, to analyze the correlation between the frequency of these words and the rate of maladaptive behaviors. Methods This is a cross-sectional study. The sample was composed of 31 autistic children, twenty-five boys and six girls, aged between 5 and 10 years old. To identify the most frequently used words in the initial period of implementation of PECS, the Vocabulary Selection Worksheet was used. And to measure the rate of maladaptive behaviors, we applied the Autism Behavior Checklist (ABC). Results There was a significant prevalence of items in the category “food”, followed by “activities” and “beverages”. There was no correlation between the total amount of items identified by the families and the rate of maladaptive behaviors. Conclusion The categories of words most mentioned by the families could be identified, and it was confirmed that the level of maladaptive behaviors did not interfere directly in the preparation of the vocabulary selection worksheet for the children studied.

Purpose: To compare abilities of imitating generic and sequential motion gesture schemes in family routines among children with Autism Spectrum Disorder (ASD) and Specific Language Impairment (SLI) and to analyze the relation between imitation index and verbal production in the ASD group. Methods: The sample was constituted by 2:1 pairing of 36 children, according to gender and age. All of them were diagnosed by a multidisciplinary team as belonging to the ASD group (n=24) or SLI group (n=12) and were under direct and indirect intervention in a school clinic. We have used the stage of imitation of the Assessment of Symbolic Maturity, which entails the imitation of nine generic and three sequential motion gesture schemes. Results: There was a tendency to a better performance of the SLI group at imitating both generic and sequential gesture schemes. As we have related the ability of imitation to the verbal production in the ASD group, a direct relation between the production of phrases and the imitation of sequential schemes was detected. Conclusion: The ability to imitate gesture and sequential schemes could be compared, and a more prominent impairment was identified in children with autism. Among them, a direct significant relationship between the ability of imitating sequential gesture schemes in family routine and verbal production of words and sentences was verified. Objetivo: Comparar as habilidades de imitação de esquemas gestuais simples e de sequências de ações em rotinas familiares de crianças com Transtorno do Espectro do Autismo (TEA) e com Transtorno Específico de Linguagem e Fala (TEL) e analisar a relação entre o índice de imitação e a produção verbal das crianças do Grupo TEA. Métodos: A amostra foi constituída pelo pareamento 2:1 de 36 crianças, de acordo com gênero e faixa etária. Todas foram diagnosticadas por equipe multidisciplinar como pertencentes ao Grupo TEA (n=24) ou grupo TEL (n=12) e atendidas em intervenção direta e indireta em clínicaescola. Utilizamos a etapa de imitação da Avaliação da Maturidade Simbólica, que compreende a imitação de nove esquemas gestuais simples e de três esquemas gestuais sequenciais. Resultados: Observamos tendência de melhor desempenho do Grupo TEL na imitação de esquemas gestuais tanto simples como sequenciais. Ao relacionarmos a habilidade de imitação à produção verbal do Grupo TEA, verificamos relação direta entre produção de frases e imitação de esquemas sequenciais. Conclusão: Foi possível comparar a habilidade de imitação gestual e de esquema sequencial e verificar maior comprometimento de desempenho nas crianças com diagnóstico compatível com o Espectro do Autismo. Dentre estas, verificouse relação direta significativa entre a habilidade de imitação de sequências de rotinas familiares e a produção verbal de palavras e frases.

Objetivo Identificar um parâmetro de tempo de intervenção fonoaudiológica para crianças com Transtorno do Espectro do Autismo, assistidas tanto pela associação de ações diretas e indiretas, quanto apenas por ações indiretas. Métodos A amostra foi constituída por 11 meninos, de 4 a 10 anos de idade, pertencentes ao espectro do Autismo. As crianças foram divididas, aleatoriamente, em dois grupos: Grupo GT, formado por seis crianças assistidas em intervenção terapêutica fonoaudiológica direta (sessão individual com a criança) e indireta (sessão de orientação aos pais); Grupo GO, com cinco crianças assistidas apenas indiretamente. Utilizamos o Autism Behavior Checklist (ABC) para entrevistar as mães e as avaliações de Interação e do Comportamento Vocal para avaliar e mensurar o tempo de evolução da criança, em três momentos: tempo zero, após seis meses (tempo 1) e após 12 meses (tempo 2). Resultados Nos primeiros seis meses, a extensão e a velocidade do processo evolutivo tornaram-se evidentes, especialmente no Grupo GT, tanto nos valores totais do ABC, quanto nas áreas que o compõem. Nos itens da Avaliação da Interação e do Comportamento Vocal também identificamos decréscimo das médias, em ambos os grupos, especialmente entre os tempos 0 e 1. Conclusão O intervalo de tempo de seis meses tornou-se uma base de evidência confiável, tanto para ações indiretas, quanto para a associação de ações diretas e indiretas. Portanto, pode servir de parâmetro para intervenção terapêutica fonoaudiológica direcionada aos Transtornos do Espectro do Autismo. Purpose To identify a time parameter of speech and language therapy for children with Autistic Spectrum Disorders assisted by the association of both direct and indirect actions, as only by indirect shares. Methods The design of this study is the Clinical Trial pilot. The sample was composed of 11 children with autism spectrum disorders, 4-10 years old. These children were randomly divided into two groups: six were receiving both direct (individual session) and indirect intervention (parent orientation session) (Therapy Group - TG), and five were receiving exclusively indirect intervention (Orientation Group - OG). We used the Autism Behavior Checklist (ABC) to interview the mothers; Interaction Assessment (IA) and Sample of Vocal Behavior (SVB) to assess and to measure the evolution of children on three occasions: at the beginning, six months later and 12 months later. Results We observed there was greater evolution, in velocity and extension, in the first semester of TG in the Autism Behavior Checklist. In the Interaction Assessment and Sample of Vocal Behavior there were decreased values, specially between 0 and 1 time. Conclusion The six month period is strong evidence for indirect action and for the association between indirect and direct actions. This measure of time can be a parameter for speech and language therapy for children with autism spectrum disorders.

OBJETIVO: analisar a quantidade e qualidade dos gestos produzidos por uma criança com Transtorno Autista. PROCEDIMENTOS: trata-se de estudo de caso. Foram utilizados critérios para análise quanto à forma, função e tipo de gestos. RESULTADOS: após seis meses de intervenção terapêutica fonoaudiológica foi observado o aumento de gestos com funcionalidade em relação ao período inicial e após um ano, ausência de movimentos aleatórios. CONCLUSÃO: verificou-se aumento na qualidade gestual do caso em questão. PURPOSE: to analyze the quantity and quality of gestures produced by an autistic child. PROCEDURES: case report. Criteria were used for analyzing the form, function and type of gestures. RESULTS: after 6-month therapy, the analyzed subject obtained gestures with functionality above the therapy initiation state. After a year of therapy the subject has not produced more random movements. CONCLUSION: improvement was verified in gestural quality in this case report.

OBJECTIVE: To determine the agreement between responses on the Autism Behavior Checklist by mothers and healthcare professionals. METHOD: Twenty-three mothers of children from the autism spectrum (DSM-IV-TR, 2002) were interviewed. The children were part of an educational program of the Autistic Friends Association-SP. The healthcare professionals in charge of the children at the institution filled out a questionnaire regarding the method for observing behavior. For comparison purposes, an additional 15 mothers of children from the autism spectrum were interviewed at the speech therapy clinic of Universidade Federal de São Paulo, and the speech therapists responsible for the children filled out the questionnaire on the observation method. The Kappa index was employed to obtain the frequency of agreement between mothers and healthcare professionals' observations on the overall Autism Behavior Checklist, as well as its individual components. The Student's t-test was used to assess differences or similarities between the Kappa results. RESULTS: The agreement index was low for the total score, with a statistically significant difference between both groups (p < 0.001). Agreement was also low for each Autism Behavior Checklist components, but there was greater agreement in the group treated at the public service in the Language, Body and Use of Objects components (p < 0.001). CONCLUSION: There was disagreement between the observations in both intervention contexts, but agreement was better at the public service than at Autistic Friends Association. OBJETIVO: Verificar a concordância das respostas no Inventário de Comportamentos Autísticos de mães e de profissionais de crianças pertencentes ao espectro autístico. MÉTODO: Entrevistou-se 23 mães de crianças do Espectro Autístico (DSM-IV-TR, 2002) inseridas em programa educacional da Associação dos Amigos do Autista-SP. O profissional da instituição responsável pelas crianças preencheu o questionário sob forma de observação de comportamentos. Para comparação, entrevistou-se 15 mães de crianças do Espectro Autístico (DSM-IV-TR, 2002) do ambulatório de fonoaudiologia da Universidade Federal de São Paulo e os fonoaudiólogos responsáveis pelas crianças preencheram o questionário sob forma de observação. Empregou-se o Kappa para obter a freqüência de concordância entre as observações das mães e do profissional no total e nas áreas do Inventário de Comportamentos Autísticos, e teste T-student para avaliar diferenças ou igualdades entre os resultados de Kappa. RESULTADOS: No escore total, o índice de concordância foi baixo, com diferença estatisticamente significante entre os dois grupos (p < 0,001). Para cada uma das subáreas do Inventário de Comportamentos Autísticos, o índice de concordância também foi baixo, mas teve maior concordância no grupo atendido no serviço público, nas áreas de Linguagem, Corpo e Uso de Objeto, e Relacionamento (p < 0,001). CONCLUSÃO: Houve discordância entre as observações nos dois contextos de intervenção; porém, no serviço público, a concordância foi melhor do que na Associação dos Amigos do Autista.
