Abstract

Background/Objectives: Taste disorders have a negative impact on meal enjoyment, which is essential for maintaining adequate nutrition and quality of life. Japan is a rapidly aging society with an increasing number of individuals with taste disorders. However, despite the increasing prevalence of taste disorders, the correlation between oral frailty and taste sensitivity remains largely unknown. The objective of this study was to assess the relationship between oral health status and taste sensitivity among the Japanese population. Methods: Participants were recruited from Kanagawa Dental University Hospital Medical–Dental Collaboration Center between 2018 and 2021. The exclusion criteria were severe systemic infections, pregnancy, or lactation. Clinical examinations, oral function assessments, and taste tests were conducted using tap water and 1% sweet, 0.3% salty, 0.03% umami, and 0.1% umami tastants. The relationships between oral function, systemic indicators, and taste sensitivity were statistically evaluated. Results: Of the 169 participants included in this cross-sectional study, 39.6% were male and 60.4% were female (median age, 68 years). Participants with low taste sensitivity showed a decline in tongue–lip motor function, independent of age, sex, or smoking status. A multiple logistic regression analysis conducted using two age categories—younger than 65 years and older than 65 years—revealed an association between tongue–lip motor function and taste sensitivity among participants younger than 65 years. Conclusions: Decreased taste sensitivity is associated with tongue–lip motor function. Abstract

Aim To examine the relationship between the number of present and functional teeth at baseline and future incidence of loss of independence. Methods Participants were community-dwelling older individuals who participated in a comprehensive geriatric health examination conducted in Kusatsu town, Japan, between 2009 and 2015. The primary endpoint was the incidence of loss of independence among participants, defined as the first certification of long-term care insurance in Japan. The numbers of present and functional teeth at baseline were determined via an oral examination. Demographics, clinical variables (e.g., history of chronic diseases and psychosocial factors), blood nutritional markers, physical functions, and perceived masticatory function were assessed. Results This study included 1121 individuals, and 205 individuals suffered from loss of independence during the follow-up period. Kaplan–Meier estimates of loss of independence for participants with smaller numbers of present and functional teeth were significantly greater than for those with larger numbers of teeth. Cox proportional hazard analyses indicated that a smaller number of present teeth was not a significant risk factor after adjusting for demographic characteristics. However, the number of functional teeth was a significant risk factor after the adjustment (hazard ratio: 1.975 [1.168–3.340]). Additionally, higher hazard ratios were observed in other adjusted models, but they were not statistically significant. Conclusions The number of functional teeth may be more closely related to the future incidence of loss of independence than the number of present teeth. This novel finding suggests that prosthodontic rehabilitation for tooth loss possibly prevents the future incidence of this life-event.

8. Influence of Meal Sequence and Number of Teeth Present on Nutrient Intake Status: A Cross-Sectional Study.

Abstract

Intake of fiber, as well as protein, and lipid preloading help to control postprandial glycemic elevation in people with type 2 diabetes and in healthy individuals. However, there are few studies on the awareness of meal sequence and nutrient intake status that consider oral conditions. This cross-sectional study aimed to determine the effects of meal sequences on nutrient intake status and whether these relationships were related to the number of teeth present. The subjects were recruited from the Medical and Dental Collaboration Center of Kanagawa Dental University Hospital between 2018 and 2021. Medical and dental examinations were performed, and a questionnaire was used to determine whether the diet consisted of vegetables, meat or fish, and carbohydrates in that order. Nutrient intake status was assessed using the brief-type self-administered diet history questionnaire. Data were collected from 238 participants. The group with awareness of meal sequence ingested increased nutrients such as n-3 fatty acids, total dietary fiber, calcium, and vitamin C. Saturated fatty acid intake increased in those with fewer teeth, while it was not significantly related to meal sequence. In conclusion, our results showed that meal sequence was associated with nutrient intake status. Abstract

高齢者における顎関節症の有病率の傾向を明らかにする目的で、関節円板の形態や位置、および下顎頭の形態や変化を評価した。教育実習用の解剖遺体17体(女性14体、死亡時平均82.8歳)を調査対象とし、両側の関節円板や下顎頭の形態を肉眼解剖学的に、およびレントゲン写真にて評価した。その結果、関節円板については3例で転位が、6例で穿孔が認められた。下顎頭については29例で変形が生じていた。下顎頭に変形がみられない集団では変形が存在する集団に比べ残存歯数が多く、Eichnerの分類にて評価した咬合関係もより良好であることが示された。以上の所見から、高齢者において顎関節の形態学的特徴や骨構造にはばらつきがみられることが明らかになった。

Results Kaplan–Meier analysis, followed by a log‐rank test, revealed that fewer PT (P < 0.001) and FT (P = 0.002) were significantly associated with a reduced survival rate. Cox's proportional hazard analysis indicated that the number of FT, but not the number of PT, was a significant independent mortality risk factor after adjusting for demographics, clinical variables, nutrient markers and physical functioning (P = 0.036, hazard ratio: 2.089). Conclusions Current results suggest that the number of FT more strongly predicts all‐cause mortality than the number of PT among community‐dwelling older adults. Further studies are necessary to consider the confounding of socioeconomic status and disability status.

Abstract

Purpose: A cohort of Japanese patients diagnosed with occlusal dysesthesia (OD) was clinically analyzed for psychosomatic background, management, and treatment outcome.Materials and Methods: The study group comprised 61 patients (17 men and 44 women) who met the OD criteria. Treatment outcomes were categorized as improvement, interruption, and transfer to another department.Results: The diagnosed OD was resolved in 25 patients (41%), 20 patients (33%) discontinued treatment, 13 (21%) were referred or transferred to other specialties such as psychiatry, and 3 (5%) continued to receive treatment following an engagement period of 3 months, 2 years, and 5 years, respectively. Among the 20 patients who discontinued treatment, complaints persisted for 10 and they did not comply with treatment, 1 had immodithymia characterized by adherence to symptoms, 3 had depressive states, 2 were suspected to have schizophrenia, and 2 were suspected to have so-called phantom bite syndrome.Conclusion: This study suggests that OD treatment should take into account the underlying psychiatric disorder manifesting as physical complaints. Abstract

Abstract Background Sleep bruxism has been described as a combination of different orofacial motor activities that include grinding, clenching and tapping, although accurate distribution of the activities still remains to be clarified. Methods We developed a new system for analyzing sleep bruxism to examine the muscle activities and mandibular movement patterns during sleep bruxism. The system consisted of a 2-axis accelerometer, electroencephalography and electromyography. Nineteen healthy volunteers were recruited and screened to evaluate sleep bruxism in the sleep laboratory. Results The new system could easily distinguish the different patterns of bruxism movement of the mandible and the body movement. Results showed that grinding (59.5%) was most common, followed by clenching (35.6%) based on relative activity to maximum voluntary contraction (%MVC), whereas tapping was only (4.9%). Conclusion It was concluded that the tapping, clenching, and grinding movement of the mandible could be effectively differentiated by the new system and sleep bruxism was predominantly perceived as clenching and grinding, which varied between individuals.

26. Effects of Wearing Complete Dentures During Sleep on the Apnea-Hypopnea Index.

Abstract

Purpose: The aim of this study was to investigate whether wearing complete dentures during sleep can improve the apnea-hypopnea index (AHI). Materials and Methods: A total of 34 edentulous patients (16 men and 18 women, mean age: 72.5 ± 8.8 years) completed the study. Portable sleep recording was performed for two nights in patients' homes. All patients wore complete dentures one night and slept without dentures the other night. Information about medical history and denture use was obtained for all patients. The occurrence of obstructive sleep apnea syndrome (OSAS) was calculated in edentulous patients, and the effect that wearing complete dentures during sleep had on the AHI was evaluated. Results: Twenty-seven of the 34 patients suffered from OSAS with an AHI = 5. The mean AHI in patients sleeping with dentures was lower than that of those without dentures (13.3 ± 10.0 versus 17.7 ± 14.6, P= .022). Nineteen of these 27 patients showed a decrease in AHI while eight showed an AHI increase, of whom four showed increases of more than 5 points when wearing dentures during sleep. Conclusions: Wearing complete dentures during sleep improves the AHI of most edentulous OSAS patients. In contrast, some patients suffer from AHI increases due to the use of complete dentures. Also in some patients, any significant change in AHI associated with the usage of complete dentures went unnoticed. Abstract

This position paper reviews physical and mechanical properties of thermoplastic resin usedfor non-metal clasp dentures, and describes feature of each thermoplastic resin in clinicalapplication of non-metal clasp dentures and complications based on clinical experience ofexpert panels. Since products of thermoplastic resin have great variability in physical andmechanical properties, clinicians should utilize them with careful consideration of thespecific properties of each product. In general, thermoplastic resin has lower color-stabilityand higher risk for fracture than polymethyl methacrylate. Additionally, the surface ofthermoplastic resin becomes roughened more easily than polymethyl methacrylate. Studiesrelated to material properties of thermoplastic resin, treatment efficacy and follow-up areinsufficient to provide definitive conclusions at this time. Therefore, this position papershould be revised based on future studies and a clinical guideline should be provided.
