Abstract

We study the electronic structure of bulk 1T-TaSe$_2$ in the charge density wave phase at low temperature. Our spatially and angle resolved photoemission (ARPES) data show insulating areas coexisting with metallic regions characterized by a chiral Fermi surface and moderately correlated quasiparticle bands. Additionally, high-resolution laser ARPES reveals variations in the metallic regions, with series of low-energy states, whose energy, number and dispersion can be explained by the formation of quantum well states of different thicknesses. Dynamical mean field theory calculations show that the observed rich behaviour can be rationalized by assuming occasional stacking faults of the charge density wave. Our results indicate that the diverse electronic phenomena reported previously in 1T-TaSe$_2$ are dictated by the stacking arrangement and the resulting quantum size effects while correlation effects play a secondary role., Comment: Main text including Supplemental Material

Abstract

The fate of the Fermi surface in bulk electron-doped Sr$_{2}$IrO$_{4}$ remains elusive, as does the origin and extension of its pseudogap phase. Here, we use high-resolution angle-resolved photoelectron spectroscopy (ARPES) to investigate the electronic structure of Sr$_{2-x}$La$_{x}$IrO$_{4}$ up to $x=0.2$, a factor of two higher than in previous work. Our findings reveal that the Fermi surface evolves smoothly with doping. Notably, the antinodal pseudogap persists up to the highest doping level, while nodal quasiparticle coherence increases monotonously. This demonstrates that the sharp increase in Hall carrier density recently observed above $x^{*}=0.16$ [Y.-T. Hsu et al., Nature Physics 20, 1596 (2024)] cannot be attributed to the closure of the pseudogap. Further, we determine a temperature boundary of the pseudogap of $T^{*}\simeq~200~\textrm{K}$ for $x=0.2$, comparable to cuprates. Our results suggest that pseudogaps are a generic feature of doped quasi-2D antiferromagnetic Mott insulators, likely related to short range magnetic correlations., Comment: 8 pages, 3 figures. Abstract

Recently, growing health awareness, novel methods allow individuals to monitor sleep at home. Utilizing sleep sounds offers advantages over conventional methods like smartwatches, being non-intrusive, and capable of detecting various physiological activities. This study aims to construct a machine learning-based sleep assessment model providing evidence-based assessments, such as poor sleep due to frequent movement during sleep onset. Extracting sleep sound events, deriving latent representations using VAE, clustering with GMM, and training LSTM for subjective sleep assessment achieved a high accuracy of 94.8% in distinguishing sleep satisfaction. Moreover, TimeSHAP revealed differences in impactful sound event types and timings for different individuals.

11. Feasibility and Reliability of the Pandemic-Adapted Online-Onsite Hybrid Graduation OSCE in Japan

Abstract

Objective structured clinical examination (OSCE) is widely used to assess medical students' clinical skills. Virtual OSCEs were used in place of in-person OSCEs during the COVID-19 pandemic; however, their reliability is yet to be robustly analyzed. By applying generalizability (G) theory, this study aimed to evaluate the reliability of a hybrid OSCE, which admixed in-person and online methods, and gain insights into improving OSCEs' reliability. During the 2020-2021 hybrid OSCEs, one examinee, one rater, and a vinyl mannequin for physical examination participated onsite, and a standardized simulated patient (SP) for medical interviewing and another rater joined online in one virtual breakout room on an audiovisual conferencing system. G-coefficients and 95% confidence intervals of the borderline score, namely border zone (BZ), under the standard 6-station, 2-rater, and 6-item setting were calculated. G-coefficients of in-person (2017-2019) and hybrid OSCEs (2020-2021) under the standard setting were estimated to be 0.624, 0.770, 0.782, 0.759, and 0.823, respectively. The BZ scores were estimated to be 2.43-3.57, 2.55-3.45, 2.59-3.41, 2.59-3.41, and 2.51-3.49, respectively, in the score range from 1 to 6. Although hybrid OSCEs showed reliability comparable to in-person OSCEs, they need further improvement as a very high-stakes examination. In addition to increasing clinical vignettes, having more proficient online/on-demand raters and/or online SPs for medical interviews could improve the reliability of OSCEs. Reliability can also be ensured through supplementary examination and by increasing the number of online raters for a small number of students within the BZs.

Abstract

Co3Sn2S2 has been reported to be a Weyl semimetal with broken time-reversal symmetry with c axis ferromagnetism (FM) below a Curie temperature of 177 K. Despite the large interest in Co3Sn2S2, the magnetic structure is still under debate and recent studies have challenged our understanding of the magnetic phase diagram of Co3Sn2S2 by reporting unusual magnetic phases including the presence of exchange bias. Understanding the magnetism of Co3Sn2S2 is important since its electronic band structure including the much-celebrated flat bands and Weyl nodes depend on the magnetic phase. In this work, using X-ray Magnetic Circular Dichroism (XMCD), we establish that the magnetic moment in Co arises from the spin, with negligible orbital moment. In addition, we detect an in-plane AFM minority phase in the sea of a FM phase using spatially-resolved angle-resolved photoemission spectroscopy ({\mu}-ARPES) combined with density functional theory (DFT) calculation. Separately, we detect a sharp flat band precisely at the Fermi level (EF) at some regions in the sample, which we attribute to a surface state. The AFM phase survives even to the low temperature of 6 K. This example of entirely different magnetic ground states in a stoichiometric intermetallic invites further efforts to explore the observed AFM phase and understand the origin and nature of the magnetic and electronic inhomogeneity on the mesoscale and the interface between the AFM and FM phases.

19. Controlling crystal cleavage in Focused Ion Beam shaped specimens for surface spectroscopy

Abstract

Our understanding of quantum materials is commonly based on precise determinations of their electronic spectrum by spectroscopic means, most notably angle-resolved photoemission spectroscopy (ARPES) and scanning tunneling microscopy (STM). Both require atomically clean and flat crystal surfaces which traditionally are prepared by in-situ mechanical cleaving in ultrahigh vacuum chambers. We present a new approach that addresses three main issues of the current state-of-the-art methods: 1) Cleaving is a highly stochastic and thus inefficient process; 2) Fracture processes are governed by the bonds in a bulk crystal, and many materials and surfaces simply do not cleave; 3) The location of the cleave is random, preventing data collection at specified regions of interest. Our new workflow is based on Focused Ion Beam (FIB) machining of micro-stress lenses in which shape (rather than crystalline) anisotropy dictates the plane of cleavage, which can be placed at a specific target layer. As proof-of-principle we show ARPES results from micro-cleaves of Sr$_2$RuO$_4$ along the ac plane and from two surface orientations of SrTiO$_3$, a notoriously difficult to cleave cubic perovskite.

20. Curative Resection of T1a Inlet Patch Adenocarcinoma With Traction-Assisted Endoscopic Submucosal Dissection.

Abstract

Esophageal inlet patch (EIP) adenocarcinoma is extremely rare. We present a case of a 58-year-old man who underwent a diagnostic esophagogastroduodenoscopy for dysphagia and found to have a 2 cm polypoid mass arising from an EIP. Biopsies and staging were consistent with T1aN0M0 EIP adenocarcinoma. While surgical resection was the main method of treatment of these lesions, very few case reports have shown that endoscopic resection can successfully remove these lesions. After multidisciplinary discussion, the patient underwent curative traction-assisted endoscopic submucosal dissection-which is the first known case report to highlight the success of this technique.

Abstract

Abstract Background Endoscopic ultrasound-guided transmural drainage (EUS-TD) is widely performed to treat postoperative peripancreatic fluid collection (POPFC). Recent reports on EUS-TD lack a consensus on stent selection. This study aimed to assess the efficacy of EUS-TD for POPFC using an external drainage-based approach. Methods We retrospectively examined the medical records of patients with POPFC treated with EUS-TD using external drainage from October 2016 to July 2024. Technical success was defined as successful placement of the external drainage. Clinical success was defined as the reduction in fluid collection, as evidenced by follow-up computed tomography 1 week post-procedure. Results This study included 14 patients. The median duration from surgery to endoscopic treatment was 13 (range: 11–26) days. The median procedural time was 26 (range: 13–35) min. The technical success rate was 100%, and 6 Fr endoscopic nasocystic drainage was performed in all patients. The clinical success rate was 100%, and no adverse events were observed. One patient experienced self-removal and required repuncture. Conclusions EUS-TD for POPFC with an external drainage-based approach is safe and effective, with a short procedure time. However, this was a retrospective study with a small sample size, suggesting that future prospective studies are warranted.

Abstract

ABSTRACT Aims/Introduction This study evaluated the risk factors for insulin therapy before 24 gestational weeks (early insulin therapy) in pregnant women with gestational diabetes diagnosed before 24 gestational weeks (E‐GDM). Materials and Methods This study included 530 singleton mothers with E‐GDM who underwent a 75 g oral glucose tolerance test (OGTT) in the first trimester at Keio University Hospital between January 2013 and December 2021. E‐GDM can be classified according to its management into only diet therapy until delivery (Diet E‐GDM), insulin therapy started before 24 gestational weeks (EarlyIns E‐GDM), and insulin therapy started after 24 gestational weeks (LateIns E‐GDM). We analyzed the risk factors for EarlyIns E‐GDM. Results Patients with EarlyIns E‐GDM had a significantly higher maternal age at delivery, pre‐pregnancy BMI, first trimester hemoglobin A1c, 1 h plasma glucose levels (1 h‐PG), and 2 h‐PG, as well as a more pronounced initial increase and subsequent decrease, compared with those in the Diet E‐GDM group. However, the Apgar scores at both 1 and 5 min were significantly lower in patients with EarlyIns E‐GDM than in those with Diet E‐GDM. The number of abnormal values in the OGTT showed the largest area under the receiver operating characteristic curve (AUC) for predicting EarlyIns E‐GDM (0.83, 95% confidence interval [CI]: 0.79–0.86), followed by the 1 h‐PG value (AUC: 0.81, 95% CI: 0.77–0.85). The initial increase showed the third largest AUC (0.78, 95% CI: 0.74–0.82). Conclusions Although further research is needed, our data suggest the importance of early insulin therapy in cases of E‐GDM with multiple abnormal OGTT values, especially with high 1 h‐PG levels and initial increase.

Abstract

Introduction: Preoperative estimations of blood loss are important when planning surgery for cervical spine injuries in older adults. The association between ankylosis and blood loss in perioperative management is of particular interest. This multicenter database review aimed to evaluate the impact of ankylosis on surgical blood loss volume in elderly patients with cervical spine injury. Methods: The case histories of 1512 patients with cervical spine injury at among 33 institutions were reviewed. After the exclusion of patients without surgery or whose blood loss or ankylosis status was unclear, 793 participants were available for analysis. Abstract

We sought to clarify the musculoskeletal ultrasound (MSUS) findings and serum concentrations of cytokines/growth factors in glucocorticoid-resistant polymyalgia rheumatica (GC-R PMR). Patients with PMR admitted to Nagasaki University Hospital (n = 41) were enrolled. MSUS of both shoulder joints was performed in 36 patients. Patients’ sera were analyzed for cytokines/growth factors using multiplex assay kits. We examined the cases of 17 males and 24 females (median age 70 years). There were 27 GC-R patients and 14 non-GC-R patients. PD signals were detected in significantly more GC-R patients (65.2%) than non-GC-R patients (23.1%) (p = 0.035). The median serum level of vascular endothelial growth factor (VEGF) was significantly higher in the GC-R group at 871.4 pg/mL than the non-GC-R group (422.8 pg/mL) (p = 0.03) and significantly higher in the PD-positive versus PD-negative patients (p = 0.04). A multivariate analysis revealed that a high serum VEGF level was an independent variable contributing to GC resistance (p = 0.017, odds ratio: 10.34). These results suggest that a high serum VEGF level is a biomarker of GC resistance in PMR, which might be explained by PD-positive neovascularization in inflamed joints of PMR.

Abstract

This paper describes an overview of the techniques of Hibikino-Musashi@Home, which intends to participate in the domestic standard platform league. The team has developed a dataset generator for the training of a robot vision system and an open-source development environment running on a human support robot simulator. The robot system comprises self-developed libraries including those for motion synthesis and open-source software works on the robot operating system. The team aims to realize a home service robot that assists humans in a home, and continuously attend the competition to evaluate the developed system. The brain-inspired artificial intelligence system is also proposed for service robots which are expected to work in a real home environment.

Abstract

We study the temperature evolution of quasiparticles in the correlated metal Sr$_2$RuO$_4$. Our angle resolved photoemission data show that quasiparticles persist up to temperatures above 200~K, far beyond the Fermi liquid regime. Extracting the quasiparticle self-energy we demonstrate that the quasiparticle residue $Z$ increases with increasing temperature. Quasiparticles eventually disappear on approaching the bad metal state of Sr$_2$RuO$_4$ not by losing weight but via excessive broadening from super-Planckian scattering. We further show that the Fermi surface of Sr$_2$RuO$_4$ - defined as the loci where the spectral function peaks - deflates with increasing temperature.
