Bevezetés: A terhességi cholestasis a terhességek 1%-ában előforduló kórkép. Fennállása nagymértékben növeli a súlyos magzati szövődmények kialakulásának, így a koraszülésnek és a terminusközelben váratlanul bekövetkező, méhen belüli magzati elhalásnak a rizikóját. Célkitűzés: A hazánkban a klinikum számára a közelmúltban elérhetővé vált szérumösszepesavszint-méréssel kapcsolatos eddigi tapasztalataink összegzése és a terhességi cholestasis ellátása debreceni gyakorlatának bemutatása. Betegek és módszer: Retrospektív esetsorozatban vizsgáltuk a debreceni Szülészeti és Nőgyógyászati Klinikán 2020. szeptember és 2021. szeptember között kezelt súlyos cholestasisos várandósok adatait. Meghatároztuk az epesav-, a májenzim- és a bilirubinszintek statisztikai korrelációját is. Eredmények: 758 várandós 1258 szérummintájából történt epesav-meghatározás. 5 esetben (az összes eset 0,7%-a, a cholestasisos esetek 6,4%-a) súlyos (epesav 40–99 μmol/l), 7 esetben (0,9%, illetve 9,0%) igen súlyos (epesav ≥100 μmol/l) epepangás volt igazolható. A 12 súlyos cholestasisos várandós átlagéletkora 30,6 (21–43) év volt, közülük 7 volt primigravida. A betegek közül 5-nél állt fenn cholestasisra hajlamosító előzményi betegség. 6 beteg részesült urzodezoxikólsav-kezelésben, amely az epesavszintek jelentős csökkenését eredményezte. Súlyos cholestasisban az epesav–GOT (R2 = 0,14) és az epesav–GPT (R2 = 0,17) korreláció gyengének bizonyult (n = 45). Az epesavszintek postpartum hamar szignifikánsan javultak. A 12 beteg közül eddig 11 szült meg, és 13 újszülött született, 2/12 volt ikerterhesség. Az átlagos gestatiós kor a szüléskor 37 (33–40) hét volt. 3/11 koraszülés történt (27%). A terminusközeli szülések közül 7/8 (88%) szülésindukciót követően zajlott. Egyszer sem volt szükség elektív császármetszésre, és a vajúdás során mindössze 2/11 esetben kellett császármetszést végezni (18%). Intrauterin magzati elhalás nem következett be. Következtetés: A szérumepesav-mérés hatékony eszköz a terhességi cholestasis diagnosztikájában és követésében, a protokollalapú szülészeti ellátáshoz pedig nélkülözhetetlen. Összefoglaló. Bevezetés: Az endokrin betegségekhez társuló ovulációs diszfunkció gyakori vezető vagy társuló oka a női infertilitásnak, de optimális reprodukciós korban oki vagy ovulációindukciós kezeléssel többnyire rendezhető a fertilitás. Az in vitro fertilizációs (IVF-) kezelések vezető indikációi jelenleg az andrológiai és a női életkorból adódó, petefészek-eredetű infertilitás, de egyéb endokrin diszfunkció társulása befolyásolja a kezelési eredményeket. Célkitűzés: Az endokrin betegségek gyakoriságának vizsgálata az IVF-programban részt vevő párok nőtagjainál. Módszer: Az IVF-program előtti alkalmassági vizsgálatok során a vezető indikációtól függetlenül részletes endokrinológiai kivizsgálást végeztünk 231 nőnél (átlagéletkor 34 év). A vizsgálatok kiterjedtek a hypothalamus- és petefészek-funkcióra, a pajzsmirigyfunkcióra és pajzsmirigy-autoimmunitásra, a mellékvese-funkcióra, a szénhidrát-anyagcserére és az inzulinrezisztenciára. Az egyes endokrin betegségek előfordulásán túl azok társulási gyakoriságát is elemeztük. Eredmények: Az IVF vezető javallatainak megoszlása megfelelt a nemzetközi tendenciáknak, endokrin jellegű volt 87 esetben (37,6%; csökkent petefészek-tartalék: 55 eset és krónikus anovuláció: 32 eset). Társuló endokrin eltérést 141 esetben találtunk, így összesen 161 nőt érintett az endokrin diszfunkció (69,7%; átlagéletkor 35 év). Az endokrin diszfunkciók előfordulása gyakorisági sorrendben: pajzsmirigy-diszfunkció (32,5%), csökkent petefészek-tartalék (23,8%), pajzsmirigy-autoimmunitás (22,5%), polycystás ovarium szindróma (15,6%), inzulinrezisztencia (22,5%), elhízás (23,8%), hyperprolactinaemia (13,4%). Az endokrin betegségek társulása minden kórkép esetében fellelhető volt. Hypogonadotrop hypogonadismus 2, congenitalis adrenalis hyperplasia 1 esetben fordult elő. Nem találtunk endokrin eltérést 70 esetben (30,3%). Következtetés: Vizsgálatunk igazolja az endokrin diszfunkciók halmozott megjelenését és gyakori társulását az IFV-program résztvevőinél, bármely indikáció esetén. A részletes endokrin kivizsgálás és az IVF-kezelést végzők endokrin jártassága hozzájárulhat az IVF-kezelés sikerességéhez. Orv Hetil. 2022; 163(18): 712–719. Summary. Introduction: Ovulatory dysfunction associated with endocrine diseases is a common leading or associated cause of female infertility, but at optimal reproductive age, causal or ovulation-induction treatment can usually settle fertility. The leading indications for in vitro fertilization (IVF) treatments are currently andrological and originated from age-related ovarian infertility, but other accompanying endocrine dysfunctions affect treatment outcomes. Objective: To investigate the incidence of endocrine diseases in female members of couples participating in IVF program. Method: During aptitude tests prior to the IVF program, from the leading indication independently, a detailed endocrinological examination was performed in 231 women (mean age: 34 years). The studies of hypothalamic and ovarian function, thyroid function and thyroid autoimmunity, adrenal function, carbohydrate metabolism and insulin resistance were covered. In addition to the incidence of each endocrine disease, the frequency of their association was analyzed. Results: The distribution of IVF lead indications was in line with the international trends, it was endocrine nature in 87 cases (37.6%; decreased ovarian reserve in 55 cases and chronic anovulation in 32 cases). Associated endocrine abnormalities were found in 141 cases, and a total of 161 women was affected by endocrine dysfunction (69.7%; mean age: 35 years). Endocrine dysfunction incidences in order of frequency were thyroid dysfunction (32.5%), diminished ovarian reserve (23.8%), thyroid autoimmunity (22.5%), polycystic ovarian syndrome (15.6%), insulin resistance (22.5%), obesity (23.8%), hyperprolactinemia (13.4%). The endocrine disease associations were found in all of the cases above. Hypogonadotropic hypogonadism occurred in 2 cases, congenital adrenal hyperplasia occurred in 1 case. No endocrine abnormalities were found in 70 cases (30.3%). Conclusion: Our study confirms the cumulative appearance of endocrine dysfunctions and frequent association in IVF participants with any lead indication. Összefoglaló. Bevezetés: Az új koronavírus (SARS-CoV-2) okozta pandémia számos változást eredményezett életünk minden területén, így a debreceni Szülészeti és Nőgyógyászati Klinikán is. Célkitűzések: A koronavírus-járvány többek között a terminus körüli elektív szülésindukciók gyakorlatának megváltoztatását igényelte. A bevezetett új szakmai eljárásrendeknek, járványügyi intézkedéseknek, a megyei kórházi funkciót ellátó debreceni Kenézy Kórház Szülészeti Osztálya és a Szülészeti Klinika összevonásának, illetve a megváltozott jogi környezetnek a szülészeti ellátásra kifejtett együttes hatásait elemezzük. Módszerek: Helyi protokollokat és részletes eljárásrendeket készítettünk és vezettünk be. 1. Szülésindukció. 2. Szülésindukció cervixérlelést igénylő esetekben. 3. A szülés módjának megválasztása előzményi császármetszés után. 4. A császármetszés utáni hüvelyi szülést támogató, terminus körüli gondozási protokoll. A protokollok bevezetését követő első 9 hónap szülészeti mutatóit (szülésszám, császármetszések aránya, perinatalis kimenetel, szülésindukciók aránya és sikeressége) vizsgáltuk és hasonlítottuk össze a megelőző időszak debreceni adataival, illetve az országos szülészeti mutatókkal. Eredmények: Klinikánkon a császármetszés aránya 2020-ban 33,5%-ra csökkent, a protokollok bevezetése óta pedig 30,7%, miközben a 2020. évi magyarországi arány 40,3%. Az összes szülés 20%-a volt 2020-ban szülésindukció, melyek közül 74% végződött hüvelyi szüléssel, míg ugyanez 2019-ben 11% indukcióból 47%-nak adódott. A havi szülésszám 2020. január és 2021. január között folyamatosan emelkedett (250-ről 450-re), a havi császármetszési arány pedig 41%-ról 25%-ra csökkent. Mindezen változások mellett 2019-ről 2020-ra csökkent mind a perinatalis mortalitás (6,3‰-ről 4,2‰-re), mind a szülés után a Neonatalis Intenzív Centrumba történő felvételek aránya (14,8%-ról 13,5%-ra). Következtetések: A bemutatott tényezők együttes hatásaként – elsődlegesen a megfelelő és következetesen betartott protokolloknak köszönhetően – a szülésindukciók aránya és sikeressége jelentősen növekedett, a császármetszések aránya szignifikánsan csökkent, javuló perinatalis morbiditási és mortalitási mutatók mellett. Introduction: The pandemic caused by the new coronavirus (SARS-CoV-2) has catalized several changes in many fields of our lives, and also at the Department of Obstetrics and Gynecology of the University of Debrecen, Hungary. Objectives: We wanted to analyse the compound effect of our new local protocols regarding elective labour inductions at term, the coronavirus pandemic and the resulting infection control measures, the merging of the Obstetrics and Gynecology Ward of the Kenézy County Hospital of Debrecen and the University Department of Obstetrics and Gynecology, and also the change of the legal environment. Methods: Local protocols were introduced: 1. Labour induction. 2. Cervical ripening in labour induction. 3. Choosing the route of delivery after cesarean. 4. Management of pregnancy around term in the case of planned trial of labour after cesarean. We compared the obstetrical data (number of deliveries, cesarean section rate, perinatal outcome and the rate and success rate of labour inductions) before and after the implementation of the protocols. The results were also compared to the Hungarian national database. Results: The annual cesarean rate at our department dropped to 33.5% in 2020. In the first 9-month period, after the introduction of the new reforms, the cesarean rate decreased to 30.7%, whereas the Hungarian national rate was 40.3% in 2020. At our department, 20% of all the deliveries were induced and 74% of them led to vaginal deliveries in 2020, while in 2019 only 11% of deliveries were labour inductions, and 47% of these cases were vaginal deliveries. The monthly number of deliveries was rising constantly between January 2020 (250 deliveries) and January 2021 (450 deliveries), and the monthly cesarean rate decreased from 41% to 25%. Comparing the data of 2019 and 2020, the annual perinatal mortality rate dropped from 6.3‰ in 2019 to 4.2‰ in 2020. Neonatal morbidity, as measured by admissions to the neonatal intensive care unit, also decreased (14.8% in 2019 and 13.5% in 2020). Conclusions: As a compound result of the described factors, but mainly due to the new protocols, both the rate and the success rate of labour inductions increased significantly, while the cesarean rate decreased with improving perinatal mortality and morbidity. Absztrakt: A Nemzeti Véradó és Vérmentő Program célja a vérkészítmények észszerű és indokolt alkalmazásának támogatása és az irracionális transzfúziós gyakorlat megszüntetése, ezáltal a betegbiztonság növelése. Az általános elvek mellett a programnak vannak speciális szülészeti vonatkozásai is. A szülészeti, különösen a postpartum vérzések ugyanis világszerte az anyai halálozás vezető okai közé tartoznak, előfordulásuk pedig a fejlett országokban emelkedő tendenciát mutat. A szülésre való felkészülés fontos elemei a vérkép rendezése, a rutinszerű profilaktikus vagy terápiás vaspótlás és a szülészeti vérzés tekintetében nagy rizikóval rendelkező gravidák időben történő kiszűrése, gondozása. A peripartum vérzések legfőbb okai az abruptio placentae, placenta praevia, uterusatonia, szövetretenció a méh üregében, sérülés, a haemostasisrendszer zavarai és ezek kombinációi. A postpartum vérzés megelőzése érdekében a szülés során fontos a lepényi szak aktív vezetése, melynek alapvető eleme az uterotonikumok profilaktikus alkalmazása. A vérmentő technikák használata – megfelelő indikációval – császármetszés és postpartum vérzés során is megfontolandó. A szülészeti vérzések kezelése során a legfőbb prioritás a lumenes, sebészi vérzés szülész által történő ellátása. A masszív vérzéshez társuló szekunder coagulopathia megszüntetése érdekében a faktorkoncentrátum-alapú, viszkoelasztikus teszttel monitorozott, egyénre szabott kezelési mód preferált, de ilyenkor a terhesség során érvényes – és a nem terhes állapottól eltérő – referencia- és célértékeket kell alkalmaznunk. A szülészeti vérzések részben a megelőzhető halálokok közé tartoznak. A Nemzeti Véradó és Vérmentő Program szülészeti alkalmazásával reményeink szerint tovább javíthatók a szülészeti vérzésekből származó morbiditási és mortalitásai adatok. Introduction: Intrahepatic cholestasis of pregnancy complicates 1% of pregnancies. It increases the risk of severe fetal complications significantly, including preterm delivery and stillbirth. Objective: To summarize our experience with serum total bile acid level measurement that has recently become available for clinical routine in Hungary, and to present the way of gestational cholestasis care at our university. Patients and method: In a retrospective case series, we analyse the data of 12 patients suffering from severe cholestasis of pregnancy treated between September 2020 and September 2021 at the Department of Obstetrics and Gynecology, University of Debrecen. We also determine the statistical correlation between bile acid, transaminase and bilirubin levels in severe cholestasis. Results: 1258 serum samples of 758 patients were measured. 5 of them (0.7% of all cases, 6.4% of cholestasis cases) had severe (total bile acid 40-99 mu mol/L), 7 (0.9% of all cases and 9.0% of cholestasis cases) had very severe (total bile acid= 100 mu mol/L) disease. The average age of the 12 cases was 30.6 (21-43) years, 7 of them were primigravid. 5 of the patients had a predisposing disease in their history. 6/12 patients received ursodeoxycholic acid treatment, resulting in significant decrease in the bile acid concentrations. Bile acid and GOT (R-2 = 0,14) and bile acid and GPT (R-2 = 0,17) correlations were found to be week in severe cholestasis (n = 45). Postpartum bile acid levels showed rapid improvement. So far, 11 of the patients have delivered and 13 neonates were born, 2/12 were multiple pregnancies. Average gestational age at delivery was 37 (33-40) weeks. 3/11 (27%) were preterm deliveries. 7/8 (88%) of term deliveries were induced. Elective cesarean delivery was not indicated in any of the cases, and in only 2/11 (18%) of the cases did emergency cesarean sections become necessary during labour. No stillbirth occurred. Conclusion: Serum total bile acid measurement is an effective tool in the diagnosis and follow-up of intrahepatic cholestasis of pregnancy, and is inevitable for the protocoll-based obstetrical management of patients. Reviewing historical and recently collected material of family Aciculidae from Albania and the neighbouring region, we synonymized two species and one subspecies, namely Platyla corpulenta Subai, 2009 under Platyla procax Boeters, E. Gittenberger & Subai, 1989; Platyla ceraunorum A. Reischütz, N. Steiner-Reischütz & P. L. Reischütz, 2016 under Platyla similis (Reinhardt, 1880) and Renea kobelti albanica Boeters, E. Gittenberger & Subai, 1989 under Renea kobelti (A. J. Wagner,1910). Four aciculid species are first mentioned for Albania: Platyla similis and Platyla procax that replace their junior synonyms; and Platyla feheri Subai, 2009 and Platyla gracilis (Clessin, 1877) that are new to the country. Furthermore, new distribution data are provided for Platyla albanica Subai, 2012, Platyla banatica (Rossmässler, 1842), Platyla wilhelmi (A. J. Wagner, 1910) and Renea kobelti (A. J. Wagner, 1910). In the light of new distribution data, their conservation statuses according to IUCN criteria are assessed at the national level and, when necessary, are re-assessed at global level.

Rapid advance in oncology leads to increasing survival of oncologic patients. More and more of them live long enough to reach either the natural age of menopause or, as a side effect of their oncotherapy, experience the cessation of gonadal function, leading to premature ovarian insufficiency, with disturbing vasomotor symtoms and long-term negative cardiovascular and skeletal effects. Thus, an ever increasing number of cancer survivors search endocrinologic help in the form of hormone replacement therapy (HRT). The misinterpretation of the WHI (Women's Health Initiative) Study has lead to an irrational fear of female hormone replacement, both by the general population and medical professionals. It has seemed the logical and safe conclusion to many physicians to avoid HRT, supposing that this attitude definitely causes no harm, whereas the decision of prescribing estrogen alone or with progestins might bear oncologic and thromboembolic risks and may even lead to litigation in case of a potentially related complication. However, it was known even before the WHI results that premature menopause and hypogonadism decreases the life expectancy of women by years through its skeletal and cardiovascular effects, and this negative effect correlates with the length of the hypoestrogenaemic period. Therefore, the denial of HRT also needs to be supported by evidence and should be weighed againts the risks of HRT. Yet, the oncologic risk of HRT is extremely difficult to assess. In this work we review the latest evidence from in vitro experiments to clinical studies, regarding HRT in survivors of gynecologic and non-gynecologic cancers. Based on our literature research, we group tumours regarding the oncologic risk of properly chosen female hormone replacement therapy in cancer survivors as follows: ’HRT is advanageous’ (e.g. endometrial cancer type I, cervical adenocarcinoma, haematologic malignancies, local cutaneous malignant melanoma, colorectal cancer, hepatocellular cancer); ’HRT is neutral’ (e.g. BRCA 1/2 mutation carriers without cancer, endometrial cancer type II, uterinal carcinosarcoma and adenosarcoma, certain types of ovarian cancer, cervical, vaginal and vulvar squamous cell carcinoma, prolactinoma, kidney cancer, pancreatic cancer, thyroid cancer); ’HRT is relatively contraindicated’ for various reasons (e.g. leiomyosarcoma, certain types of ovarian tumours, brain tumours, advanced metastatic malignant melanoma, lung cancer, gastric cancer, bladder cancer); ’HRT is diasadvantageous and thus contraindicated’ (e.g. breast cancer, endometrial stroma sarcoma, meningioma, glioma, hormone receptor positive gastric and bladder cancer).

Orv Hetil. 2021; 162(21): 811-823.The pandemic caused by the new coronavirus (SARS-CoV-2) has catalized several changes in many fields of our lives, and also at the Department of Obstetrics and Gynecology of the University of Debrecen, Hungary.We wanted to analyse the compound effect of our new local protocols regarding elective labour inductions at term, the coronavirus pandemic and the resulting infection control measures, the merging of the Obstetrics and Gynecology Ward of the Kenézy County Hospital of Debrecen and the University Department of Obstetrics and Gynecology, and also the change of the legal environment.Local protocols were introduced: 1. Labour induction. 2. Cervical ripening in labour induction. 3. Choosing the route of delivery after cesarean. 4. Management of pregnancy around term in the case of planned trial of labour after cesarean. We compared the obstetrical data (number of deliveries, cesarean section rate, perinatal outcome and the rate and success rate of labour inductions) before and after the implementation of the protocols. The results were also compared to the Hungarian national database.The annual cesarean rate at our department dropped to 33.5% in 2020. In the first 9-month period, after the introduction of the new reforms, the cesarean rate decreased to 30.7%, whereas the Hungarian national rate was 40.3% in 2020. At our department, 20% of all the deliveries were induced and 74% of them led to vaginal deliveries in 2020, while in 2019 only 11% of deliveries were labour inductions, and 47% of these cases were vaginal deliveries. The monthly number of deliveries was rising constantly between January 2020 (250 deliveries) and January 2021 (450 deliveries), and the monthly cesarean rate decreased from 41% to 25%. Comparing the data of 2019 and 2020, the annual perinatal mortality rate dropped from 6.3‰ in 2019 to 4.2‰ in 2020. Neonatal morbidity, as measured by admissions to the neonatal intensive care unit, also decreased (14.8% in 2019 and 13.5% in 2020).As a compound result of the described factors, but mainly due to the new protocols, both the rate and the success rate of labour inductions increased significantly, while the cesarean rate decreased with improving perinatal mortality and morbidity. The aims of the National Blood Donation and Blood Saving Program are to support the rational and judicious utilization of blood products and abolish irrational transfusion policy to improve patient safety. In addition to the general principles, this program has got some special obstetrical aspects. Obstetrical, especially the postpartum haemorrhages belong to the leading causes of maternal mortality worldwide. In developed countries, a trend in increasing incidence can be observed. Preparing for delivery includes some important elements such as optimization of hemoglobin level, routinely applied prophylactic or therapeutic iron supplementation and early screening and comprehensive care of patients with high risk of obstetrical bleeding. The main causes of peripartum bleeding are abruptio placentae, placenta praevia, uterine atony, retained tissue in the uterus, trauma during delivery, and haemostatic disorders or their combinations. To prevent postpartum bleeding, it is important to use the active management of the third stage of labour including prophylactic utilization of uterotonics as an essential element. Utilization of blood salvage techniques with adequate indications may be considered in cases of cesarean section or postpartum haemorhage. In cases of obstetrical haemorrhage, management of surgical bleeding has the main priority by the obstetrician. Secondary coagulopathy associated with massive bleeding should be managed by viscoelastic test-guided, individualized and factor concentrate-based algorithm, however, pregnancy-specific reference and target ranges must be used that are different from the non-pregnancy values. Obstetrical bleedings belong to the potentially preventable causes of death. Hopefully, the implementation of the National Blood Donation and Blood Saving Program in the field of obstetrics can decrease the associated morbidity and mortality further. Existing data on the phylogeography of European taxa of steppic provenance suggests that species were widely distributed during glacial periods but underwent range contraction and fragmentation during interglacials into “warm-stage refugia.” Among the steppe-related invertebrates that have been examined, the majority has been insects, but data on the phylogeography of snails is wholly missing. To begin to fill this gap, phylogeographic and niche modeling studies on the presumed steppic snail Caucasotachea vindobonensis were conducted. Surprisingly, reconstruction of ancestral areas suggests that extant C. vindobonensis probably originated in the Balkans and survived there during the Late Pleistocene glaciations, with a more recent colonization of the Carpatho-Pannonian and the Ponto-Caspian regions. In the Holocene, C. vindobonensis colonized between the Sudetes and the Carpathians to the north, where its recent and current distribution may have been facilitated by anthropogenic translocations. Together, these data suggest a possible non-steppic origin of C. vindobonensis. Further investigation may reveal the extent to which the steppic snail assemblages consist partly of Holocene newcomers.

International audience; We present the reconstruction of mire vegetation changes and fire history recorded in a continuous sediment profile that spans the last 20,000 cal yr BP from the Late Pleniglacial to Holocene in North-eastern Hungary. We also aimed to reveal past climate changes by using ecological requirements of specific aquatic plants as summer temperature indicators. Our results suggest the formation of a mesotrophic mire around 20,000 cal yr BP with brown moss, Betula sp. and Selaginella selaginoides cover beside the occurrence of Phragmites australis, Typha latifolia and T. angustifolia suggesting base-rich fen and tundra-like wet-ground habitats on the lakeshore. This community shifted to reed dominated swamp at c. 18,300 cal yr BP with inferred min. July temperatures of 12–15.7°C. Pinus sp., Betula nana, B. pendula/pubescens, Hippuris vulgaris and P. australis dominated until 16,600 cal yr BP pointing to shallow muddy stagnant water and colder climatic conditions than in the preceding interval. The most warmth-demanding species, T. latifolia and T. angustifolia indicated July mean temperatures > 14-15.7°C soon after the LGM. The formation of biogenic carbonate also started at an early stage; major accumulation occurred between 15,200 and 10,000 cal yr BP. In the Early Holocene, environmental indicator species (e.g. Phragmites australis and T. latifolia) pointed to warmer and shallower conditions, while the late Holocene was characterised by strong eutrophication and reed swamp dominance on the lakeshore. Elevated macrocharcoal concentration, wood fragments and remains of Typha species suggested frequent local fires and dry mire surface conditions during the last 1700 years.

Polycystic ovary syndrome is the most common endocrine disease in women. Psychic consequences are significant; patients have difficulties to be pregnant and the disease disturbs the quality of life. Due to complications associated with polycystic ovary syndrome, studies on psychological state and disease perception of patients appear to be important.The aim of the authors was to assess how the disease influences quality of life and changes in body image.The authors used the Illness Intrusiveness Ratings Scale, Health-Related-Quality-of-Life Questionnaire for Women with Polycystic Ovary Syndrome and their own body image questionnaire. 121 women took part in the study.There was a negative correlation between illness intrusiveness and quality of life (r = -0.353) and a positive correlation between body image and quality of life (r = 0.614). A significant difference was observed in body satisfaction between patients who had visible body changes and those who had metabolic disturbances (p0.05). Overweight patients who lost weight with lifestyle changes had significantly better quality of life (p0.05).These results appear to be convincing in that in addition to gynecological care, psychotherapy could be offered to patients with polycystic ovary syndrome.Bevezetés: A polycystás ovarium szindróma a nők leggyakoribb endokrin betegsége. Pszichikai következményei megnehezítik a teherbe esést, s a betegség rontja az életminőséget, mivel megváltoztatja a külső megjelenést. A szövődmények miatt fontos a betegek pszichikai állapotának és a betegséghez kapcsolódó attitűdjének feltérképezése. Célkitűzés: A szerzők célul tűzték ki annak vizsgálatát, hogy a betegség hogyan hat az életminőségre és a testképpel való elégedettségre. Módszer: A Betegségteher-indexet, a Polycystás Ovarium Szindróma Kérdőívet és a szerzők által kidolgozott testképkérdőívet használták. A vizsgálatban 121 beteg vett részt. Eredmények: Negatívan korrelált a betegségteher és az életminőség (r = −0,353), míg pozitívan a testkép és az életminőség (r = 0,614). A testképpel való elégedettséget illetően szignifikáns különbséget (p0,05) találtak a külső elváltozásokat mutató és az anyagcserezavarral küzdő betegek csoportja között. Jobb volt azoknak a túlsúlyos betegeknek az életminősége, akik táplálkozási és testkontrollal megszabadultak súlyfeleslegüktől (p0,05). A betegség által érintett testrészeikkel elégedetlenebbekben az alkatilag meghatározott testjellemzők integrálódtak a testképhez (p0,05). Következtetések: Az eredmények meggyőzőek abból a szempontból, hogy indokolt lenne a betegek orvosi ellátása mellett a betegek differenciált pszichológiai ellátása is. Orv. In this paper two new localities of the hygromiid land snail Plicuteria lubomirski (Œlosarski, 1881) are reported from Romania (Suceava and Harghita Counties). Its presence at the Lacu Roou (Gyilkos-to) area represents the southeasternmost occurrence of the species. The only sample of P. lubomirski hitherto reported from Romania (leg. JICKELI in 1888 at Borsec Bai) seems to be lost from museum collections. The northern Carpathian distributional type is suggested for the species. The nomenclatural problem regarding the spelling of the specific name (i.e. lubomirskii or lubomirski) is discussed. The original, but less frequently used spelling (lubomirski) is suggested based on our understanding of the regulations of the ICZN.

Definiáljuk a fájdalmat, majd a szubjektív fájdalomérzet kialakulásában szerepet játszó neuroanatómiai struktúrák rövid ismertetése után utóbbiak ontogenezis során bekövetkező fejlődését ismertetjük. Ezt követően a noxa kiváltotta stresszválasz objektív (neuroendokrin-vegetatív, cardiovascularis, reflexes és viselkedési) komponensének intrauterin megfigyelhető bizonyítékait vesszük sorra.

Identical hemodynamic impedance and constant ratio of the fetal descending aorta and middle cerebral artery of uncomplicated pregnancies at term. Fetal aortic-cerebral Doppler resistance index ratio: An indicator of physiologic blood flow distribution. Objective To interpret the physiologic fetal arterial blood flow distribution by relating the vascular impedance of the fetal descending aorta (DA) and middle cerebral artery (MCA) and to establish the reference ranges for the aortic-cerebral Doppler resistance index ratio (ACRI). Study design Ninety-six patients with uncomplicated pregnancies were recruited for the cross-sectional assessment of the Doppler resistance index (RI) in the fetal DA and MCA between the 38rd and 40th weeks of gestation. The normal ranges of the ACRI were calculated. A cut-off value was designed to facilitate the clinical application of the ACRI. Results Between the 38th and 40th weeks of gestation in normal pregnancies the ACRI of healthy fetuses is constant, the overall mean is: 1.062 (+/– 0.087). A single cut-off value of 1.2 is recommended to assist separating normal and pathologic arterial blood flow patterns. Conclusion The normal ACRI reflects the identical vascular resistance of the descending aorta and the cerebral vessels, which maintains the physiologic fetal central arterial blood flow. Additional clinical studies are necessary to assess the diagnostic efficacy of the abnormal ACRI (>1.2) as a potentially useful marker of the centralized arterial circulation indicating the early stage of fetal hypoxemic jeopardy.

The Lozekia-Kovacsia species group comprises three species occurring in the Carpathian-Pannonian region, namely Lozekia transsilvanica (Westerlund, 1876), Lozekia deubeli (M. Kimakowicz, 1890) and Kovacsia kovacsi (Varga & L. Pinter, 1972). These species are known to be distinguishable by their genital morphology but, up to now, only a few populations have been examined anatomically. Due to their conchological similarity, distribution records that have been based on individuals of unknown anatomy are unreliable, and the ranges are far from being well defined. First we identified the type locality of L. transsilvanica and investigated the genital morphology of topotypical material in order to ensure the correct use of this name. Furthermore, we carried out a morphological analysis of Lozekia -Kovacsia material collected throughout the postulated range of this species group. Of the previously proposed species-specific features, the structure of the dart-sac complex and the number of digitiform glands were found to be the most reliable. Based on these genital characters, we re-defined the ranges of the three species; those of K. kovacsi and L. deubeli were found much larger than believed previously. Purinergic signal transduction mechanisms have been appreciated as a complex intercellular signalling network that plays an important regulatory role in both short- and long-term processes in practically every living cell. One of the most intriguing aspects of the field is the participation of ATP and other purine nucleotides in the determination of cell fate and the way they direct cells towards proliferation, differentiation or apoptosis, thereby possibly taking part in promoting or preventing malignant transformation. In this review, following a very brief introduction to the historical aspects of purinergic signalling and a concise overview of the structure of and signal transduction pathways coupled to P2 purinergic receptors, the current theories concerning the possible ways how extracellular ATP can alter the function of tumour cells and the effectiveness of anticancer therapies are discussed, including pharmacological, nutritional, vasoactive and 'anti-antioxidant' actions of the nucleotide. The effects of ATP on animals inoculated with human tumours and on patients with cancer are looked over next, and then an overview of the literature regarding the expression and presumed functions of P2 purinoceptors on tumour cells in vitro is presented, sorted out according to the relevant special clinical fields. The article is closed by reviewing the latest developments in the diagnostic use of P2 purinergic receptors as tumour markers and prognostic factors, while discussing some of the difficulties and pitfalls of the therapeutic use of ATP analogues.

Human herpesvirus 6 (HHV-6) frequently reactivates in human immunodeficiency virus 1 (HIV-1) infected patients, and is thought to be a cofactor in AIDS progression. Macrophages are targets and reservoirs of HIV-1 and HHV-6; hence, they have an important role in dissemination and pathogenesis of these viruses. The present study examined the effects of HHV-6 A variant on replication of R5 variants of HIV-1 in macrophages. For this purpose, HIV-1 replication was investigated in macrophages infected with HIV-1 alone or along with HHV-6A. Our results demonstrated that HHV-6A significantly suppressed HIV-1 replication in coinfected cultures. HHV-6A infection resulted in increased secretion of RANTES and IL-8. Experiments with exogenous RANTES and IL-8 revealed that these chemokines also significantly suppressed HIV-1 replication in infected macrophages. RANTES is able to induce desensitization and internalization of CCR5, the chemokine coreceptor of R5 variants. In addition, IL-8 receptor activation results in cross-desensitization and cross-internalization of CCR5. We found that CCR5 sensitivity and expression level is diminished in HHV-6A-infected macrophage cultures compared with uninfected cells. Taken together, our results indicate that HHV-6A infection decreases the susceptibility of macrophages to R5 variants of HIV-1 in which the HHV-6A induced RANTES and IL-8 may have importance.

Changes in intracellular calcium concentration ([Ca2+]i) evoked by prolonged depolarisation (120 mM KCl) or by the application of 15 mM caffeine were measured on skeletal muscle cells in primary culture. The extrusion rate (PVmax) of calcium from the myoplasm was determined, which in turn enabled the calculation of the calcium flux (Fl) underlying the measured calcium transients. PVmax was found to increase during differentiation, from 107 +/- 10 microM/s at the early myotube stage to 596 +/- 36 microM/s in secondary myotubes. This was paralleled by a decrease in resting [Ca2+]i from 99 +/- 4 to 51 +/- 2 nM. The depolarisation-evoked Fl rose to peak and then ceased despite the continuous presence of KCl. In contrast, the caffeine-induced Fl showed a peak and a clear steady-level with a peak-to-steady ratio of 5.6 +/- 1.2. Removal of external calcium suppressed the depolarisation--induced flux by 88 +/- 5% indicating that both an influx and a release from the SR underlie the K(+)-evoked calcium transients. Subsequent applications of caffeine resulted in essentially identical fluxes indicating an efficient refilling of the internal stores. Moreover, if a depolarisation-induced calcium transient preceded the second caffeine-evoked release, the latter was significantly larger than the first suggesting that much of the calcium that entered was stored in the SR rather than extruded.

Ezt követően a noxa kiváltotta stresszválasz objektív (neuroendokrin-vegetatív, cardiovascularis, reflexes és viselkedési) komponensének intrauterin megfigyelhető bizonyítékait vettük sorra. Ebben a második közleményben részletesen tárgyaljuk a korai fájdalom- és stresszélmény későbbi életkorban tapasztalható hosszú távú patológiás hatásait. Végül áttekintjük a magzatnak fájdalmat és stresszt okozni képes prae- és perinatalis beavatkozásokat, valamint az elméletileg lehetséges és a szülészeti gyakorlatban is alkalmazható intrauterin fájdalomcsillapító eljárások körét.

TPEN (N,N,N',N'-tetrakis(2-pyridylmethyl)-ethylenediamine) is a membrane-permeable heavy-metal ion chelator with a dissociation constant for Ca2+ comparable to the Ca2+ concentration ([Ca2+]) within the intracellular Ca2+ stores. It has been used as modulator of intracellular heavy metals and of free intraluminal [Ca2+], without influencing the cytosolic [Ca2+] that falls in the nanomolar range. In our previous studies, we gave evidence that TPEN modifies the Ca2+ homeostasis of striated muscle independent of this buffering ability. Here we describe the direct interaction of TPEN with the ryanodine receptor (RyR) Ca2+ release channel and the sarcoplasmic reticulum (SR) Ca2+ pump (SERCA). In lipid bilayers, at negative potentials and low [Ca2+], TPEN increased the open probability of RyR, while at positive potentials it inhibited channel activity. On permeabilized skeletal muscle fibers of the frog, but not of the rat, 50 microM TPEN increased the number of spontaneous Ca2+ sparks and induced propagating events with a velocity of 273 +/- 7 microm/s. Determining the hydrolytic activity of the SR revealed that TPEN inhibits the SERCA pump, with an IC(50) = 692 +/- 62 microM and a Hill coefficient of 0.88 +/- 0.10. These findings provide experimental evidence that TPEN directly modifies both the release of Ca2+ from and its reuptake into the SR.

N,N,N',N'-tetrakis(2-pyridylmethyl)-ethilenediamine (TPEN) is a membrane permeable heavy metal chelator that has been used to study intracellular calcium homeostasis but its exact mode of action is still unresolved. Here we examine the effects of TPEN on the Ca(2+) release from and the Ca(2+) uptake into the sarcoplasmic reticulum (SR) of cultured C2C12 skeletal muscle cells. Low concentrations (50 microM) of the drug evoked Ca(2+) transients in approximately 60% of C2C12 myotubes, while at high concentrations (500 microM) it significantly reduced the size of both depolarization-and caffeine-induced Ca(2+) transients, decreased the rate constant of decay and the calculated pump activity but failed to induce Ca(2+) transients. Experiments at low extracellular [Ca(2+)] revealed that it is the total rather than free TPEN concentration that is responsible for the observed effects. TPEN does not modify Ca(2+) release by Zn(2+) chelation, as evidenced by the unaltered effect seen after the removal of Zn(2+) from the extracellular space of the cells by chelating with EDPA. These findings provide experimental evidence that TPEN directly modifies both the release of Ca(2+) from the SR and its removal from the myoplasm.

Ca(2+)-signaling of human melanoma is in the focus of intensive research since the identification of the role of WNT-signaling in melanomagenesis. Genomic and functional studies pointed to the important role of various Ca(2+) channels in melanoma, but these data were contradictory. In the present study we clearly demonstrate, in a number of different ways including microarray analysis, DNA sequencing and immunocytochemistry, that various human melanoma cell lines and melanoma tissues overexpress ryanodine receptor type 2 (RyR2) and express P2X(7) channel proteins as compared to melanocytes. These channels, although retain some of their usual characteristics and pharmacological properties, display unique features in melanoma cells, including a functional interaction between the two molecules. Unlike P2X(7), RyR2 does not function as a calcium channel. On the other hand, the P2X(7) receptor has an antiapoptotic function in melanoma cells, since ATP-activation suppresses induced apoptosis, while knock down of the gene expression significantly enhances that.

Differentiation of skeletal muscle cells both in vivo and in vitro is accompanied by the development of voltage-dependent processes and alterations in purinergic signalling. To date at least two independent methods have been used to induce differentiation in primary cultures, namely, appropriate modification of culturing conditions and overexpression of specific protein kinase C (PKC) isoenzymes. Here we characterize and compare the development of purinergic and depolarization-dependent alterations using these two methods to induce differentiation in C2C12 cells. We demonstrate that depolarization- and ATP-evoked Ca(2+) responses underwent functional development during differentiation, and the characteristics of this progress were dependent on the actual differentiation-promoting stimulus. Overexpression of PKCalpha anticipated the appearance of robust increases in the intracellular calcium concentration upon ATP administration but failed to do so after depolarizing stimuli. Moreover, the first phase of the biphasic ATP-induced response observed in differentiated myotubes induced by culturing was not present in differentiated PKCalpha-overexpressing cells, suggesting that although purinergic signalling developed very early, purinergic stimuli failed to activate the voltage-dependent mechanisms of these cells even at subsequent stages of differentiation. Disruption of the coupling of purinergic signalling to depolarization-activated mechanisms may be explained by our observations that PKCalpha-overexpression changed the purinergic receptor pattern of immature myoblasts differently from what was seen in the course of culturing-induced differentiation. PKCalpha-specific alterations were characterized by the lack of increase in the expression of P2X(7) receptors and the failure of P2Y(4) receptors to appear and P2Y(2) receptors to disappear. The effects of PKCalpha-overexpression were proven to be specific since the overexpression of the hyperproliferative isoenzyme PKCdelta failed to induce any of the changes promoted by PKCalpha. Our data suggest that the method of inducing differentiation in skeletal muscle cells modifies not only the course of development but also the interaction of depolarization-dependent and purinergic pathways.

Although extracellular adenosine triphosphate (ATP) has been generally accepted as the regulator of cellular differentiation, the relative contribution of the various purinoreceptor subtypes to purinergic signalling at distinct stages of skeletal muscle differentiation is still poorly understood. Here we measured extracellular ATP-evoked changes in intracellular calcium concentration and surface membrane ionic currents (I (ATP)), calculated the calcium flux (FL) entering the myoplasmic space and compared these parameters at different stages of differentiation on cultured mouse myotubes. The ATP-evoked FL displayed an early peak and then declined to a steady level. With differentiation, the early peak became separated from the maintained component and was absent on mature myotubes. Repeated ATP applications caused desensitization of the response in both immature and differentiated myotubes, owing mainly to the reduction of the early peak of FL in the former and to a decline of both components in the latter group of cells. Depolarization of the cell or removal of external calcium suppressed the early peak. I (ATP) showed no inactivation, and its voltage dependence displayed strong inward rectification. The concentration dependence of I (ATP) can be fitted using a Hill equation, yielding an EC(50) of 56 microM. Results are consistent with the parallel activation of both P2X and P2Y receptors.
