1. Spectral Extremal Connectivity of Two-State Seizure Brain Waves

Abstract

Coherence analysis plays a vital role in the study of functional brain connectivity. However, coherence captures only linear spectral associations, and thus can produce misleading findings when ignoring variations of connectivity in the tails of the distribution. This limitation becomes important when investigating extreme neural events that are characterized by large signal amplitudes. The focus of this paper is to examine connectivity in the tails of the distribution, as this reveals salient information that may be overlooked by standard methods. We develop a novel notion of spectral tail association of periodograms to study connectivity in the network of electroencephalogram (EEG) signals of seizure-prone neonates. We further develop a novel non-stationary extremal dependence model for multivariate time series that captures differences in extremal dependence during different brain phases, namely burst-suppression and non-burst-suppression. One advantage of our proposed approach is its ability to identify tail connectivity at key frequency bands that could be associated with outbursts of energy which may lead to seizures. We discuss these novel scientific findings alongside a comparison of the extremal behavior of brain signals for epileptic and non-epileptic patients.

2. KenCoh: A Ranked-Based Canonical Coherence

Abstract

In this paper, we consider the problem of characterizing a robust global dependence between two brain regions where each region may contain several voxels or channels. This work is driven by experiments to investigate the dependence between two cortical regions and to identify differences in brain networks between brain states, e.g., alert and drowsy states. The most common approach to explore dependence between two groups of variables (or signals) is via canonical correlation analysis (CCA). However, it is limited to only capturing linear associations and is sensitive to outlier observations. These limitations are crucial because brain network connectivity is likely to be more complex than linear and that brain signals may exhibit heavy-tailed properties. To overcome these limitations, we develop a robust method, Kendall canonical coherence (KenCoh), for learning monotonic connectivity structure among neuronal signals filtered at given frequency bands. Furthermore, we propose the KenCoh-based permutation test to investigate the differences in brain network connectivity between two different states. Our simulation study demonstrates that KenCoh is competitive to the traditional variance-covariance estimator and outperforms the later when the underlying distributions are heavy-tailed. We apply our method to EEG recordings from a virtual-reality driving experiment. Our proposed method led to further insights on the differences of frontal-parietal cross-dependence network when the subject is alert and when the subject is drowsy and that left-parietal channel drives this dependence at the beta-band.

3. The South Atlantic Dipole via multichannel singular spectrum analysis.

Abstract

This study analyzes coupled atmosphere-ocean variability in the South Atlantic Ocean. To do so, we characterize the spatio-temporal variability of annual mean sea-surface temperature (SST) and sea-level pressure (SLP) using Multichannel Singular Spectrum Analysis (M-SSA). We applied M-SSA to ERA5 reanalysis data (1959-2022) of South Atlantic SST and SLP, both individually and jointly, and identified a nonlinear trend, as well as two climate oscillations. The leading oscillation, with a period of 13 years, consists of a basin-wide southwest-northeast dipole and is observed both in the individual variables and in the coupled analysis. This mode is reminiscent of the already known South Atlantic Dipole, and it is probably related to the Pacific Decadal Oscillation and to El Niño-Southern Oscillation in the Pacific Ocean. The second oscillation has a 5-year period and also displays a dipolar structure. The main difference between the spatial structure of the decadal, 13-year, and the interannual, 5-year mode is that, in the first one, the SST cold tongue region in the southeast Atlantics Cape Basin is included in the pole closer to the equator. Together, these two oscillatory modes, along with the trend, capture almost 40% of the total interannual variability of the SST and SLP fields, and of their co-variability. These results provide further insights into the spatio-temporal evolution of SST and SLP variability in the South Atlantic, in particular as it relates to the South Atlantic Dipole and its predictability.

9. Use of complexity theory to inform antimicrobial stewardship: a scoping review.

Abstract

Complexity theory has been used previously as a conceptual lens in human healthcare research. Antimicrobial stewardship (AMS) is an inherently complex healthcare intervention; however, the extent to which complexity has been operationalized in AMS is currently unclear. To investigate if, and how, complexity theory has been used to inform AMS in human health care. Scoping review methodology. Empirical research or policies specifically referencing complexity in relation to AMS were considered in any human healthcare setting and geographical location. Databases searched were Cinahl, Cochrane Library, Embase, Medline, National Institute for Health and Care Excellence, PsycInfo, Scopus and Web of Science from inception to June 2020. Grey literature and other databases searched were EVIPNet, Google, Mednar, Proquest Theses and the World Health Organization Library of National Antimicrobial Resistance Action Plans. Non-English language articles were excluded. Of 612 records retrieved, eight articles were included in this review. Heterogeneity in study design and geographical location were noted. Three interventional studies evaluated AMS in hospital (N =2) and long-term care (N =1) settings. The remaining studies were non-interventional and proposed AMS strategies conceptualized through complexity theory. The importance of close engagement between researchers or policy administrators and the target population was emphasized in all studies as a means of ensuring AMS relevance and success. There is a paucity of AMS research informed by complexity theory, and no policy documents could be located using complexity as a guiding theory. Mixed methods, informed by complexity theory, is a potentially suitable strategy to develop, implement and evaluate AMS as a complex intervention. [ABSTRACT FROM AUTHOR]

10. Examining Test Speededness by Native Language

Abstract

When power tests include a time limit, it is important to assess the possibility of "speededness" for examinees. Research on differential speededness in the past has included looking at gender and ethnic subgroups in the United States on paper and pencil tests. The needs of a global audience necessitated, and the availability of computer recorded response time data enabled this investigation of differential speededness by native language. Data from a computerized adaptive test administered globally in English were used to compare different measures of time pressure and speed for 15 language groups compared with native-English-speaking examinees. Results indicated that examinees whose native language was Korean had issues with speededness when many of the metrics were considered, but when ability was controlled, there were no relevant differences for any of the languages compared to English. Future research should include independent measures of ability and English proficiency as well as any possible effects of speededness on predictive validity. (Contains 6 figures and 3 tables.)

12. Metacognitive Protocols: A Qualitative Study of Perceptions of 'Smartness' of Adults and Children.

Abstract

Metacognition is a theoretical construct used to describe individuals' perceptions of their thinking processes and their own control over their thinking processes. This study examined the protocols of 78 undergraduates who responded to 3 questions from the Swanson Metacognitive Questionnaire: (1) What makes someone really smart? (2) How do children figure things out, like how to do something? and (3) Is there any reason why adults are smarter than children? Why? Two researchers examined the protocols independently and identified some themes. "Smartness" was largely described as having academic knowledge and common sense, with a genetic component. The dominant themes about children's intelligence were that they learn by observation, through asking questions, and by trial and error. Respondents indicated that adults had more life experiences and a larger knowledge base than children. These findings support suggestions in current psychological literature about differences in the thinking processes of children and adults. (Contains 3 tables and 11 references.) (Author/SLD)

14. Kids Having Kids: Perspectives of Adolescents and Young Adults of Los Baños, Laguna, Philippines on Teenage Pregnancy.

Abstract

The study focused on establishing the knowledge, attitude, and behavior of Grade 12 learners about teenage pregnancy (TP). The study was conducted among students from different high schools in Los Baños, Laguna. As a municipality, Los Baños has manifested a relatively high case of TP over the last 10 years from 2009 to 2018. A mixed method was used, which started with focus group discussions (FGD) that were geared towards survey implementation. Three FGDs were completed with a mixture of male and female participants. The survey had a total of 42 respondents. Results showed that students had high knowledge scores about TP and a mix of responses to the various attitude and behavior statements presented to them. The survey results and analysis show that knowledge scores and attitudes have a negative monotonic association. Based on the theory of reasoned action, the role of peers and the Internet are crucial to the attitude and behavior of the youth regarding teenage pregnancy. Institutional support is needed at the municipal and national levels to address the burgeoning concern on TP. A concerted effort of the family, the school, the community, the local government, and the youth community will also be needed to come up with cohesive and appropriate interventions that genuinely address this phenomenon. 17. Generalizability of GMAT[R] Validity to Programs outside the U.S.

Abstract

This study explores the predictive validity of GMAT[R] scores for predicting performance in graduate management programs outside the United States. Results suggest that the validity estimates based on the combination of GMAT[R] scores were about a third of a standard deviation higher for non-U.S. programs compared with existing data on U.S. programs. There also appeared to be differences for separate GMAT[R] sections such that verbal and writing scores had higher validity estimates for non-U.S. programs, but the validity estimates for quantitative scores were higher for U.S. programs. By meta-analytically combining data for more than 1,200 cases across six Western European programs, this study was able to explore the predictive validity of GMAT[R] scores for students with differing backgrounds, including gender, educational background, native language, and citizenship. The analyses show that predictive validity differences among groups have only a small impact on academic performance measured by grades. (Contains 6 tables.)

18. The Validity of Graduate Management Admission Test Scores: A Summary of Studies Conducted from 1997 to 2004

Abstract

The validity of Graduate Management Admission Test (GMAT) scores is examined by summarizing 273 studies conducted between 1997 and 2004. Each of the studies was conducted through the Validity Study Service of the test sponsor and contained identical variables and statistical methods. Validity coefficients from each of the studies were corrected for restriction of range on the predictors. The interquartile range of the validity coefficients for the combination of GMAT scores and undergraduate grades in predicting early graduate school performance was 0.45 to 0.63. Further examination of study findings showed that higher validity coefficients were observed for executive MBA programs compared to either full-time or part-time programs. There appeared to be no measurable differences in results for public versus private institutions. For programs not located at U.S. institutions, quantitative scores showed lower average validity coefficients compared to programs located in the United States. (Contains 4 tables.)

19. Evaluating the Predictive Validity of Graduate Management Admission Test Scores

Abstract

Admissions data and first-year grade point average (GPA) data from 11 graduate management schools were analyzed to evaluate the predictive validity of Graduate Management Admission Test[R] (GMAT[R]) scores and the extent to which predictive validity held across sex and race/ethnicity. The results indicated GMAT verbal and quantitative scores had substantial predictive validity, accounting for about 16% of the variance in graduate GPA beyond that predicted by undergraduate GPA. When these scores and undergraduate GPA were used together, they accounted for approximately 25% of the variation in first-year graduate GPA. Correcting correlations for restriction of range improved the predictive power. No statistical differences were found across examinee groups defined by race/ethnicity and sex, which suggests a lack of bias in these scores. The predictive utility of GMAT analytical writing scores was relatively low, accounting for only about 1% of the variation in graduate GPA, after accounting for undergraduate GPA and GMAT verbal and quantitative scores. Abstract

Acute appendicitis remains a common reason for hospital admission. Reports have suggested a reduction in patients attending emergency departments during the acute phase of the SARSCoV- 2 pandemic. Moreover, a global surge in conservative management of acute appendicitis has recently been registered by the Appy Study of the Association of Italian Surgeons in Europe (ACIE)1. This is a treatment option that has been known for some years, although quite seldom used before the pandemic2–4. As most countries are experiencing new waves of the pandemic, the attitude of surgeons towards SARS-CoV-2 screening policies and personal protective equipment (PPE) used during the management of patients with acute appendicitis need to be established. According to a subanalysis of the ACIE Appy Study, half of surgeons globally were testing patients for SARS-CoV-2 only when symptomatic or there was suspicion of infection; approximately 12 per cent did not test patients at all (Fig. 1 and Table S1). There were regional differences. In Europe, respondents tested all patients (50.8 per cent) or those with suspected infection (43.9 per cent), with only 5.3 per cent not being tested at all. In the USA, the majority of participants only tested patients with a suspected infection (65.6 per cent). A similar picture of testing only those with a suspected infection was also reported from Latin America (57.2 per cent), Asia/Middle East (76.8 per cent), and Africa (41.7 per cent). Even more worrisome, 58.3 per cent of respondents from Africa and 27.6 per cent from Latin America were not testing patients at all before appendicectomy. Concerning the screening modality, most respondents used PCR alone or in combination with chest imaging. Serology was rarely used overall and never in Africa (Fig. 1 and Table S2 ). It is now accepted that chest imaging is not routinely required and that PCR is an accurate screening modality. Serology might, however, be useful to shed light on the disease course and previous exposure to the virus, but respondents from some countries still have restricted access to this test. In terms of PPE during appendicectomy, most African respondents did not use different PPE compared with the prepandemic period in patients who tested negative for COVID-19. More concerning is that 58.3 per cent did not use different PPE in untested patients. This differed from other regions where the rate of those not considering a change of PPE in untested patients did not exceed 22 per cent. One in 10 respondents from Latin America also reported that they were not using different PPE compared with the prepandemic phase in patients who tested positive for COVID-19. These data, and taking into account the high prevalence of acute appendicitis, leads to the conclusion that omission of routine patient screening may have contributed to local clusters among patients and threatened the safety of healthcare workers5. In this respect, it is likely that limited access to PPE explains the attitude of surgeons towards patients with unknown SARSCoV- 2 status or those infected, raising ethical concerns about the safety of surgical staff. It is of outmost importance that, even during challenging times and stress on economic stability, industrialized countries make efforts to sustain low-income countries and those with limited resources. This would ensure equal working conditions, safer treatment for patients with acute appendicitis, and better control of the pandemic

Abstract

Background: Surgical strategies are being adapted to face the COVID-19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods: The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results: Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X-ray plus molecular testing (PCR) being the commonest (19·8 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6·6 and 2·4 per cent respectively before, but 23·7 and 5·3 per cent, during the pandemic (both P < 0·001). One-third changed their approach from laparoscopic to open surgery owing to the popular (but evidence-lacking) advice from expert groups during the initial phase of the pandemic. No agreement on how to filter surgical smoke plume during laparoscopy was identified. There was an overall reduction in the number of patients admitted with appendicitis and one-third felt that patients who did present had more severe appendicitis than they usually observe. Conclusion: Conservative management of mild appendicitis has been possible during the pandemic. The fact that some surgeons switched to open appendicectomy may reflect the poor guidelines that emanated in the early phase of SARS-CoV-2.

Abstract

High-efficiency particulate air supplied to a positive-pressure ventilation lobby (PPVL) in isolation rooms offers the dual advantage of protective and source isolation. This study demonstrates the in-use validity of PPVL rooms for protective isolation of patients. Of the 48 PPVL air samples investigated, Aspergillus fumigatus was detected from only one (2%) sample. Local and remote monitoring of the PPVL rooms is essential for the safety of patients and healthcare workers. Remote and point-of-use engineering controls are essential for ongoing ventilation monitoring, but this should be complemented by visual inspection of the isolation suite. Abstract

Purpose: This study was designed to evaluate the safety and efficacy of Stapled Trans-Anal Rectal Resection (STARR) in the treatment of Obstructed Defecation Syndrome ODS by the analysis of the data collected in the STARR Italian Registry (SIR) with a special emphasis on the analysis of symptoms and quality of life. Methods: Collected data included, preoperative tests findings, and the evaluation of symptoms; the latter was obtained by using dedicated tools such as the Obstructed Defecation Syndrome Score (ODS-S), the Severity Symptom Score (SSS), and the Continence Grading Scale (CGS). Data on the quality of life were collected by Patient Assessment of Constipation Quality of Life (PAC-QoL) and the Euro Quality of Life-5 Domains Visual Analogue Scale (EQ-5D VAS). The evaluation of the symptoms and the quality of life was repeated 6 and 12 months after surgery. Results: The SIR had collected data on 2171 patients (1653 females, 76.1%; mean age 56.2years; range 20-96 years). A significant improvement (P <.0001) was seen between preoperative and 12-month follow-up in all scores: ODS-S (16.7 vs 5.0), SSS (15.6 vs 2.6), CGS (2.0 vs 0.7), PAC-QoL (51.0 vs 22.1), and EQ-5D VAS (57.5 vs 85.7). Complications included defecatory urgency (4.5% at 12 months), bleeding (3.6%), perineal sepsis (3.4%), and one case of rectovaginal fistula (0.05%). Conclusion: The analysis of SIR data seems to confirm that STARR is a safe and effective procedure in the treatment of ODS. However, further studies are required to evaluate the long-term stability of results. © 2011 SAGE Publications

The association between healthcare-associated invasive aspergillosis and hospital construction/building works is well recognized. This infection can cause significant morbidity and mortality and imposes a substantial burden on the healthcare system. The population of patients at risk for this opportunistic infection has expanded and multi-triazole drug resistance has emerged globally. Hence the need for a multi-faceted approach to prevent acquisition of invasive aspergillosis in acute care settings. This article is a summary of the Irish National Guidelines for the prevention of healthcare-associated aspergillosis which is based on published reports, international clinical guidelines, official engineering standards, and technical guidelines. We discuss the key recommendations and strategies for the prevention of invasive aspergillosis from the planning/pre-construction, construction, and post-construction phases. Abstract

Aim This study evaluated the early results (with particular reference to complications) of stapled transanal rectal resection (STARR) carried out using the CCS-30 Contour® Transtar™ device. The procedure was performed in a single centre on patients with obstucted defecation caused by rectocele or rectal intussusception. Method From July 2007 to February 2009, 133 patients were treated. Preoperatively, all underwent clinical examination, transanal ultrasonography, anorectal manometry and cinedefaecography. Obstructed defaecation syndrome was assessed using the Cleveland Clinic Constipation Score (CCC-S). Early postoperative complications and those occurring within 6 months were recorded. Results The median follow-up period was 19 (range 12-30) months. The mean ± standard deviation preoperative CCC-S of 19.4 ± 7.1 decreased to 10.1 ± 9.0 postoperatively. The early complication rate was 15.7% and included rectovaginal fistula ( n = 1), rectal perforation ( n = 1), posterior dehiscence ( n = 4), further surgery for retained staples ( n = 2), postoperative bleeding ( n = 2) and postoperative impaired continence ( n = 11). Conclusions STARR using the Contour Transtar device seems to be effective for treating obstructed defaecation. However, serious complications may occur. [ABSTRACT FROM AUTHOR]

Abstract

Introduction:Esophageal-pericardial fistulas are rare complications of a variety of conditions ranging from benign esophageal disorders to iatrogenic etiologies such as radiofrequency catheter ablation. Here, we present a unique case of esophageal-pericardial fistula formation following esophageal stent migration in the setting of ulcerative esophagitis and benign esophageal stricture.Case Presentation:A 53-year-old woman with a history of ulcerative esophagitis and associated esophageal stricture was managed with esophageal stenting. Following stenting, her course was complicated by esophageal stent migration, resulting in severe dysphagia and multiple gastrointestinal bleeds requiring percutaneous endoscopic gastrostomy (PEG) tube placement. She presented with fatigue, melena, and dark output from her PEG tube and was hypotensive, tachycardic, and tachypneic. An echocardiogram (Figure 1A) showed hydro-pneumopericardium with cardiac tamponade physiology requiring emergent pericardiocentesis and aspirated fluid shown in Figure 1B. A subsequent computed tomography (CT) scan of the chest demonstrated a large complex pericardial effusion with air in the anterior pericardium, a large esophageal-pericardial fistula, and a moderate left hydropneumothorax (Figure 1C). Given the extent of her esophageal defect, the patient was taken for an esophagectomy with esophagostomy formation.Discussion:On review, we found only a few cases of hydro-pneumopericardium in the context of spontaneous pericardial herniation due to subcutaneous emphysema, radiofrequency ablation for atrial arrhythmias and esophagectomy for esophageal adenocarcinoma. However, there has been no report of esophageal-pericardial fistula due to esophageal stent migration.Conclusion:In an appropriate setting, clinicians should maintain a high index of suspicion for esophageal-pericardial fistulas, as delay in the management of these conditions could be fatal.

Abstract

t-chromosomes are natural polymorphisms in feral populations of mice that are thought to be descended from a single ancestral chromosome. They carry an inversion of at least 10 cM surrounding the major histocompatibility complex (MHC) that effectively prevents recombination between a t-bearing chromosome and wild type chromosomes. However, on the rare occasion when two different t-chromosomes meet in a wild female, recombination occurs at an apparently normal rate. Since they contain the highly polymorphic MHC, their limited origin and restricted chances for recombination make t-chromosomes a valuable tool for studying the relative contributions of mutation and recombination to the generation of diversity. Using 13 different serological reagents to class I antigens, and studying restriction enzyme polymorphisms detected with three molecular probes for class II genes examined with three endonucleases, we present data indicating that the major factor responsible for the diversity of class I antigens is recombination, but that for class II genes, mutation must play an important role in addition to recombination.

Abstract

The precise role of the granular enzyme A (granzyme A), a serine protease, in the lytic process of cytotoxic T lymphocytes (CTL) is not clear. We have recently constructed a CTL line transfected with the antisense gene of granzyme A (a-GrA). These a-GrA CTL had lower GrA activity as well as decreased lytic activities, as measured by51Cr and by DNA degradation assays. Furthermore, at low effector:target ratio (1:8) in prolonged lytic assays, they could not lyse targets as rapidly as the control CTL. When we examined their ability to exocytose BLT (CBZ-L-lys-thiobenzyl)-esterase in the presence of anti-CD3 antibody, the a-GrA CTL exocytosed poorly compared to the parental CTL or control transfectant with a CAT gene. Most strikingly, a-GrA cells could not release intracellular stores of Ca2+in response to anti-CD3 induction, although the Ca2+flux was normal when they were stimulated with ionomycin. When the parental CTL was treated with a specific benzyllactam inhibitor of BLT-esterase orN-tosyl-L-phenylalanylchloromethyl ketone, the Ca2+flux induced by anti-CD3 was also suppressed. We propose that granzyme A is involved in the signal transduction pathway that causes the rise of the intracellular calcium.

50. Voltage-gated potassium channels regulate calcium-dependent pathways involved in human T lymphocyte activation.

Abstract

The role that potassium channels play in human T lymphocyte activation has been investigated by using specific potassium channel probes. Charybdotoxin (ChTX), a blocker of small conductance Ca(2+)-activated potassium channels (PK,Ca) and voltage-gated potassium channels (PK,V) that are present in human T cells, inhibits the activation of these cells. ChTX blocks T cell activation induced by signals (e.g., anti-CD2, anti-CD3, ionomycin) that elicit a rise in intracellular calcium ([Ca2+]i) by preventing the elevation of [Ca2+]i in a dose-dependent manner. However, ChTX has no effect on the activation pathways (e.g., anti-CD28, interleukin 2 [IL-2]) that are independent of a rise in [Ca2+]i. In the former case, both proliferative response and lymphokine production (IL-2 and interferon gamma) are inhibited by ChTX. The inhibitory effect of ChTX can be demonstrated when added simultaneously, or up to 4 h after the addition of the stimulants. Since ChTX inhibits both PK,Ca and PK,V, we investigated which channel is responsible for these immunosuppressive effects with the use of two other peptides, noxiustoxin (NxTX) and margatoxin (MgTX), which are specific for PK,V. These studies demonstrate that, similar to ChTX, both NxTX and MgTX inhibit lymphokine production and the rise in [Ca2+]i. Taken together, these data provide evidence that blockade of PK,V affects the Ca(2+)-dependent pathways involved in T lymphocyte proliferation and lymphokine production by diminishing the rise in [Ca2+]i that occurs upon T cell activation.
