Abstract

Ly49 receptor genes are expressed by subsets of natural killer (NK) cells in an overlapping fashion, accounting for the capacity of NK subsets to attack host cells that have selectively downregulated self-major histocompatibility complex (MHC) class I molecules. It was shown previously that most NK cells express only one or the other allele of a given Ly49 gene, while a smaller population expresses both alleles. However, the methods used to detect monoallelic and biallelic cells were nonquantitative. Here, new allele-specific antibodies were used to provide the first quantitative examination of biallelic and monoallelic expression of Ly49A and Ly49G2. The results demonstrate conclusively that most Ly49A(+) and Ly49G2(+) NK cells express the corresponding gene in a monoallelic fashion, with a smaller subset expressing both alleles. Unexpectedly, biallelic Ly49A(+) NK cells were more numerous than predicted by completely independent allelic expression, suggesting some heterogeneity among NK progenitors in the potential to express a given Ly49 gene. The data also show that cells expressing one allele of Ly49G2 may express Ly49A from the same or opposite chromosome with equal likelihood, indicating that the expressed allele is chosen independently for different Ly49 genes. Finally, the data demonstrate that biallelic expression of Ly49A or Ly49G2 occurs least frequently in mice that express ligands for these receptors (H-2(d) mice), and most frequently in class I-deficient mice. Thus, biallelic expression of Ly49 genes is regulated by interactions of NK cell progenitors with MHC class I molecules.

Abstract

Objective: To investigate the safety and effectiveness of direct oral anticoagulants (DOAC) versus vitamin K antagonists (VKA) after recent stroke in patients with atrial fibrillation (AF) aged ≥85 years. Methods: Individual patient data analysis from seven prospective stroke cohorts. We compared DOAC versus VKA treatment among patients with AF and recent stroke (≥85y = 0.65, 95%-CI [0.52, 0.81]) and < 85 years (HR = 0.79, 95%-CI [0.66, 0.95]) in simple (p interaction = 0.129), adjusted (p interaction = 0.094) or weighted (p interaction = 0.512) models. Analyses on recurrent stroke, ICH and death separately were consistent with the primary analysis, as were sensitivity analyses using age dichotomized at 90 years and as a continuous variable. DOAC had a similar net clinical benefit in patients aged ≥85 (+1.73 to +2.66) and < 85 years (+1.90 to +3.36 events/100 patient-years for ICH-weights 1.5 to 3.1). Interpretation: The favorable profile of DOAC over VKA in patients with AF and recent stroke was maintained in the oldest old. ANN NEUROL 2021.

Abstract

Facioscapulohumeral muscular dystrophy (FSHD) is one of the most prevalent forms of muscular dystrophy. The objective of this study is to establish a national patient registry for FSHD as part of Remudy, a nationwide registry for neuromuscular diseases in Japan, with the intention of facilitating future clinical trials for FSHD. Methods: A variety of clinical data and genetic analysis results, including haplotype information, were collected. Results: A total of 145 patients (66 males and 79 females) were enrolled from September 2020 to October 2023. Of these, 140 had FSHD1 and 5 had FSHD2. The mean age at the time of registration and onset were 42.0 and 15.5 years, respectively. The proportion of non-ambulatory patients was 26.9%, with a mean age of 37.4 years at which they became non-ambulatory. Respiratory dysfunction was observed in 47%, with 29% requiring mechanical ventilation. The majority of FSHD patients (n=136) had D4Z4 repeat units shortened to 1-6 repeats and 4qA haplotypes. Specifically, three patients had one repeat unit, 20 patients had two, 41 patients had three, 32 patients had four, 24 patients had five, and 16 patients had six. The median age of disease onset was 10, 8, 10, 12, 19, and 19.5 years, respectively. The percentage of patients who lost ambulation was 60.9% of those with one to two repeats, 27.4% of those with three to four repeats, and 10.0% of those with five to six repeats. Retinopathy was observed in 5 (3%) and hearing loss in 22 (15%) FSHD1 patients, both with 3 or fewer repeats. Among all adult female patients with FSHD, a history of pregnancy was reported in 30.7% of those with one to three repeats, 51.4% of those with four to six repeats, and 80% of those with seven or more repeats. Conclusion: In FSHD1, motor function, retinopathy, and hearing loss were influenced by the number of D4Z4 repeats. Abstract

Experiments have become part and parcel of today’s economics. Experimental evidence is used to update and to refute economic theories but also helps in formulating new theories or policies. Experimental evidence and (quasi)experimental designs are becoming increasingly popular in many fields of applied economics. The consequences of this transformation are deep enough to prompt methodological reflection at least as wide-ranging and radical, as the change itself. In this discussion, seemingly outdated distinctions may gain new significance and inspire more general questions on the structure and perspectives of current economic science.

Abstract

Neuromuscular and neurodegenerative diseases are mostly modeled using genetically modified animals such as mice. However, animal models do not recapitulate all the phenotypes that are specific to human disease. This is mainly due to the genetic, anatomical and physiological difference in the neuromuscular systems of animals and humans. The emergence of direct and indirect human somatic cell reprogramming technologies may overcome this limitation because they enable the use of disease and patient-specific cellular models as enhanced platforms for drug discovery and autologous cell-based therapy. Induced pluripotent stem cells (iPSCs) and urine-derived stem cells (USCs) are increasingly employed to recapitulate the pathophysiology of various human diseases. Recent cell-based modeling approaches utilize highly complex differentiation systems that faithfully mimic human tissue- and organ-level dysfunctions. In this review, we discuss promising cellular models, such as USC- and iPSC-based approaches, that are currently being used to model human neuromuscular and neurodegenerative diseases.

Abstract

In contrast to the majority of allergic or hypersensitivity conditions, worldwide anaphylaxis epidemiological data remain sparse with low accuracy, which hampers comparable morbidity statistics. Data can differ widely depending on a number of variables. In the current document we reviewed the forms on which anaphylaxis has been defined and classified; and how it can affect epidemiological data. With regards to the methods used to capture morbidity statistics, we observed the impact of the anaphylaxis coding utilizing the World Health Organization's International Classification of Diseases. As an outcome and depending on the anaphylaxis definition, we extracted the cumulative incidence, which may not reflect the real number of new cases. The new ICD-11 anaphylaxis subsection developments and critical view of morbidity statistics data are discussed in order to reach new perspectives on anaphylaxis epidemiology. © 2018 The Author(s).
