4. Exploratory study on the impact of intraesophageal pressure on quality of life in patients following total gastrectomy: a retrospective cohort study.

Abstract

Background: The usefulness of high-resolution impedance manometry (HRIM) in patients who underwent total gastrectomy with Roux-en-Y (R-Y) anastomosis has never been well validated. This study aimed to investigate whether intraesophageal pressure affects quality of life in patients who underwent total gastrectomy with R-Y anastomosis. Methods: The participants comprised 12 patients who underwent total gastrectomy for gastric cancer between October 2014 and July 2022 and underwent a postsurgical HRIM examination. The association between the HRIM data and Postgastrectomy Syndrome Assessment Scale-37 (PGSAS-37) questionnaires was analyzed. Results: Esophageal body motility was normal in almost all patients. The anastomosis shape (circular stapler and overlap method with linear stapler) did not influence intraesophageal pressure. The integrated relaxation pressure and lower esophageal sphincter (LES) residual pressure during swallowing-induced relaxation were involved in "diarrhea subscale" scores (p = 0.0244 and p = 0.0244, respectively). The average maximum intrabolus pressure was not involved in postgastrectomy symptom. The contractile front velocity correlated with the "indigestion subscale," "diarrhea subscale," and "constipation subscale" (p = 0.0408, p = 0.0143, and p = 0.0060, respectively). The distal latency, i.e., the time from upper esophageal sphincter relaxation to contractile deceleration, was also associated with the "abdominal pain subscale" (p = 0.0399). LES pressure and esophageal body motility affected patients' quality of life after total gastrectomy. Conclusions: HRIM for the evaluation of intraesophageal pressure is useful for the functional assessment of esophagojejunostomy with the R-Y reconstruction after total gastrectomy. Abstract

ω3 Polyunsaturated fatty acids are currently obtained mainly from fisheries; thus, sustainable alternative sources such as oleaginous microorganisms are required. Here, we describe the isolation, characterization, and application of 3 novel ω3 desaturases with ω3 polyunsaturated fatty acid–producing activity at ordinary temperatures (28 °C). First, we selected Pythium sulcatum and Plectospira myriandra after screening for oomycetes with high eicosapentaenoic acid/arachidonic acid ratios and isolated the genes psulω3 and pmd17, respectively, which encode ω3 desaturases. Subsequent characterization showed that PSULω3 exhibited ω3 desaturase activity on both C18 and C20 ω6 polyunsaturated fatty acids while PMD17 exhibited ω3 desaturase activity exclusively on C20 ω6 polyunsaturated fatty acids. Expression of psulω3 and pmd17 in the arachidonic acid–producer Mortierella alpina resulted in transformants that produced eicosapentaenoic acid/total fatty acid values of 38% and 40%, respectively, at ordinary temperatures. These ω3 desaturases should facilitate the construction of sustainable ω3 polyunsaturated fatty acid sources.

Abstract

Introduction: To prevent port‐site hernia, we established a simple and low‐cost closure method that uses a venous catheter needle and suture, without any other special devices. Materials and Surgical Technique: We used the inner needle of a 16‐G venous catheter and a 2‐0 absorbable bladed suture. To evaluate the efficacy of this technique, the procedure time was noted and compared among three operators (an experienced surgeon and two inexperienced surgeons). Discussion The median suturing time was 60.5 seconds (range, 26‐130 seconds) per incision. Even an unexperienced surgeon can close the fascia safely and quickly after some experience with the procedure. [ABSTRACT FROM AUTHOR]

38. Accuracy of reaction rates in the accelerator-driven system with 14MeV neutrons at the Kyoto University Critical Assembly

Abstract

Reaction rate experiments on the accelerator-driven system (ADS) are conducted by combining a critical assembly of a solid-moderated and -reflected core with a pulsed neutron generator. Neutrons (14 MeV) generated from the accelerator are injected into a subcritical system and the reaction rates are measured by the foil activation method to obtain neutronic spectrum data. The numerical calculations are executed by MCNPX with ENDF/B-VI.8, JENDL-3.3 and JENDL/D-99 libraries to evaluate the reaction rates of activation foils set in the center of the core. For the ADS experiments with 14 MeV neutrons, the C/E values between the experiments and the calculations are found to be well within the relative difference of about 30% in all foils up to subcriticality 1.05%Δk/k. The reaction rates do not depend on the subcriticality level in cases of [115]In, [56]Fe (purity 99.99%), [27]Al, whereas subcriticality dependence is observed in [93]Nb. In the critical experiments carried out in the A, B and C cores, special mention should be made of the remarkable effect of the composition rate of [56]Fe material. Thus a remarkable improvement is observed in the accuracy of experimental and numerical reaction rates, demonstrating the importance of material impurity for subcritical experiments.
