About half of the cases of obsessive-compulsive disorder (OCD) occurring in childhood/adolescence occur with similar symptoms both in childhood and adulthood. Immunologic stress is claimed to be a risk factor in the etiology of childhood onset OCD. Our aim was to elucidate the relationship between childhood onset OCD risk and MHC complex I and II alleles.MHC alleles of 49 OCD children together with 277 healthy children (aged 4-12) were analyzed by PCR. Results were evaluated by using univariate analysis and multivariate logistic regression analysis.A2, A29, C4, DRB3.1, and DRB1*16 alleles were found to increase the risk of OCD.The relationship found between DRB locus and OCD in this study was remarkable since there have been studies on different populations reporting similar relationship between DRB locus and rheumatoid arthritis, which is also an AID. MHC class I and class II alleles were found to increase the risk of OCD in our study, which serves as a suitable model for studies suggesting that MHC genes do not work completely independently. Even though the MHC class I and II genes are considered to have different roles in immune response, in fact they tend to work in cooperation. As in previous studies on AIDs, there is a linear relationship between MHC class II alleles and OCD risk.

3. The impact of maltreatment on symptom severity in patients with attention deficit hyperactivity disorder.

Purpose This study aimed to compare the impact of maltreatment on the subtype, comorbidity, sociodemographic characteristics, and psychometric characteristics of attention deficit hyperactivity disorder (ADHD). Materials and Methods: The study participants included 150 treatment-naïve children with ADHD with (n=75) and without (n=75) a history of maltreatment by parents. A sociodemographic form, the Conners Parent Rating Scale (CPRS), the Conners Teacher Rating Scale (CTRS), the Family Assessment Device (FAD), and the Stroop Form were applied. Results: Earlier referral, peer discord, and lower parental age were frequent in the maltreated group, while the maternal employment rates were significantly lower. ADHD-combined type and comorbid oppositional defiant disorder were significantly higher in the maltreated group. The CPRS and FAD scores were also significantly higher). Conclusion: A higher incidence of destructive behavioral patterns may increase the risk of maltreatment. Families of children with hyperactivity and defiant characteristics may have difficulties in managing behavioral problems and may exhibit domestic violence, with the Multidisciplinary approaches, including parental education programs and social investigation, should be considered. 9. Role of calcium metabolism in ADHD: The relationship between parathyroid hormone and ADHD symptom severity.

Background: Vitamin D has been found to be associated with the pathogenesis of attention deficit hyperactivity disorder (ADHD). However, the potential role of parathyroid hormone (PTH) is still unclear. Aim: We aimed to investigate the association between calcium metabolism and ADHD symptomatology. Methods: We included 106 participants aged between 7 and 13 years old (51 ADHD patients, mean age: 9.54 ± 1.77, 55 healthy controls mean age: 9.97 ± 0.94) to this study. K-SADS-PL and Conners' Parent/Teacher Rating Scales, Stroop Test were performed. Blood samples to measure serum levels of Vitamin D, PTH, calcium (Ca), magnesium (Mg), phosphorus (P), and alkaline phosphatase (ALP) were collected in the spring (March--April--May) to prevent seasonal variability. Results: PTH, P, and ALP values were significantly lower and Vitamin D, Ca, and Mg values were significantly higher in the ADHD group (P < 0.05, for all). Both groups had Vitamin D deficiency. Control group has lower Vitamin D levels than the ADHD group (respectively; 17.66 ± 9.07, 21.99 ± 10.99, P < 0.05). There was a negative correlation between PTH and CTRS hyperactivity, CGI-RI and CGI-EL sub-scores, CGI-Total, DSM-IV-Inattention, DSM-IV Hyperactivity/Impulsivity, DSM-IV-Total scores (P < 0.05, for all). Conclusions: We found lower PTH levels in ADHD patients and a strong and negative correlation between PTH and symptom severity. Future studies are needed to clarify if these findings are due to the key role of PTH in ADHD pathology or PTH's function in activating vitamin D. [ABSTRACT FROM AUTHOR]

Purpose: In this study, we aimed to investigate the sociodemographic and clinical characteristics of children and adolescents diagnosed with tic disorders. Materials and Methods: The study population included 187 children with tic disorders, aged between 4 and 18 years. The data were obtained by using Yale Global Tic Severity Scale (YGTSS), Screen for Child Anxiety and Related Disorders, Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version (K-SADS-PL). The demographic information, history of familial diseases were recorded. Results: The study included 37 girls (mean 10.9 ± 2.5 years) and 150 boys (mean 10.9 ± 2.7 years) with a mean age of 10.9 ± 2.6 years. Of all cases, 135 were classified as Tourette Syndrome (n=135; 72,2%), 50 as motor tics (n=50; 26,7 %) and 2 as others tic disorders (n=2; 1,1%) according to the diagnostic tic disorders subtype. There was no significant difference between the two groups in terms of mean age and first symptom age according to the subtype of tic disorder. In 91% of the cases, the age of onset of tics was 12 years or younger. The rate of having at least one comorbidity, mean number of comorbidities and comorbid ADHD rates were significantly higher among boys than girls. The most common comorbid diagnoses were ADHD (n=142, 75.9%), OCD (n= 08, 57.8%) and anxiety disorder (n=57, 30.4%). Conclusion: In our study, male cases, early onset and multiple comorbid diagnoses were found to be significantly higher in childhood tic disorders., Amaç: Bu çalışmada, tik bozukluğu tanısı alan çocuk ve ergenlerin sosyodemografik verileri, klinik özellikleri ve eşlik eden diğer ruhsal bozuklukların araştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmaya, Tik Bozukluğu tanısı olan 4-18 yaş aralığında 187 olgu alındı. Ruhsal belirtilerin taranması amacı ile Okul Çağı Çocukları İçin Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam boyu Şekli Türkçe uyarlaması ÇDŞG-ŞY (KSADS-PL), Tik belirti şiddeti için, Yale Genel Tik Ağırlığını Derecelendirme Ölçeği (YGTDÖ) kullanıldı. Olguların demografik bilgileri ve ailesel hastalık öyküleri kaydedildi. Bulgular: Çalışmaya, ortalama yaşı 10,9±2,6 yaş olan, 37 kız (ortalama 10,9±2,5 yaş) ve 150 erkek (ortalama 10,9±2,7 yaş) olgu alındı. Tik Bozukluğu alt tipine göre olguların 135’i (%72,2) Tourette Sendromu, %26,7’si (n=50) motor tik bozukluğu, 2’si (%1,1) bunların dışında bir tik bozukluğu tanısı aldı. Tik Bozukluğu alt tipine göre olguların ortalama yaşı, ilk belirti yaşı açısından iki grup arasında anlamlı fark yoktu. Olguların % 91’inde tiklerin başlangıç yaşı, 12 yaş ve altında idi. Kızlara göre erkekler arasında en az bir eş tanıya sahip olma oranı, ortalama eş tanı sayısı ve dikkat eksikliği hiperaktivite bozukluğu (DEHB) eş tanı oranı anlamlı biçimde daha fazlaydı. En sık görülen eş tanılar, DEHB (n=142, %75,9), obsesif kompulsif bozukluk (OKB) (n=108, %57,8) ve anksiyete bozukluğu (n=57, %30,4) idi. Sonuç: Çalışmamızda, çocukluk çağı tik bozukluklarında erkek olgu, erken başlangıç ve çoklu eş tanıların belirgin düzeyde yüksek olduğu bulunmuştur. And follicular fluid levels of BMP 2, BMP 4, BMP 7, GDF 9, and Emmprin (Basigin) were measured and compared for clinical pregnancy rates.ResultsFollicular levels of BMP 4 was significantly higher whereas Emmprin levels were lower in patients who had achieved clinically diagnosed pregnancy compared with those who did not achieve clinical pregnancy after ICSI procedure (P=0.007 and P=0.035, respectively). BMP 2, BMP 7, and GDF 9 levels were comparable for both groups.ConclusionClinical pregnancy rates after ICSI may be associated with follicular fluid levels of Emmprin and BMP 4. Follicular levels of Emmprin and BMP 4 can be used as a marker (as markers for predicting ICSI outcomes) for a better ICSI outcome.

Objective Methods Results Conclusions Pineal gland volume (PGV), which is associated with sleep and circadian rhythm, is known to be changed in some psychiatric disorders such as major depression, mood disorders and schizophrenia. This study aimed to compare the PGV of children with mild and moderate intellectual disability (ID) and healthy children.This multicentre retrospective study included 40 children with ID (patient group), aged 6–12 years and 40 age‐ and sex‐matched healthy children (control group). The children were examined for their sociodemographic characteristics and for PGV using magnetic resonance imaging.The PGV of the patient group was significantly larger than that of the controls (
p = 0.023). There was no statistically significant difference in PGV between mild and moderate ID. A moderate and positive correlation was found between Weschler Intelligence Scale for Children‐revised (WISC‐R) performance score and PGV (p = 0.049, r = 0.313) only in the patient group. In the receiver operating characteristic analysis, the area under the curve was 0.648, and the sensitivity was 70.0%, and the specificity was 60.0%.In conclusion, this study demonstrated that the increased PGV levels were associated with autism spectrum disorder (ASD) and PGV could be a risk factor in the aetiology of ID. Objective: Bullying that influences health and manyhealth related behaviors in adolescences can lead to negative changes on theirbehaviors even in the later periods of life. The aim of this study is toevaluate the cases of exposure to bullying, depression, anxiety state,relations with the family and school success among the students in a middleschool in Adana.Methods: This cross-sectional study was carried outamong middle school students at 6th, 7th and 8thgrade in the city of Adana. The survey form applied to the students includedsocio-demographic attributes, Child Depression Scale, STAI Scale, CaliforniaBullying Victimization Scale, Family Assessment Scale and Conners TeacherRating Scale.Results: The number of students at 6th, 7thand 8th grade was 53 (44,5%), 29 (24,4%) and 37 (31,1%) respectively. 33 of the students (27,7%) in the study were victims of peer bullying.Students who were subject to violence at home, at lower year levels and withnon-graduate mothers were more exposed to bullying. The ones exposed tobullying were detected to have high levels of depression anxiety andhyperactivity. Discussion: School bullying is an issue which iscommon all over the world and should not be underestimated. This study hasascertained that children exposed to bullying have high levels of anxiety andincidence of depression, also these children were found to be more hyperactive., Amaç: Ergenlerde sağlık ve birçoksağlıkla ilgili davranışları etkileyen zorbalık, yaşamın sonraki dönemlerindebile davranışlarda olumsuz değişikliğe neden olabilir. Bu çalışmanın amacı,Adana'daki ortaokul öğrencilerinde zorbalık, depresyon, kaygı durumu, aileyleilişkiler ve okul başarısı ile ilgili olguların değerlendirilmesidir.Yöntem: Kesitsel tipteki bu çalışma, Adanailinde 6., 7. ve 8. sınıflarda ortaokul öğrencileri arasında yürütülmüştür.Öğrencilere uygulanan anket formu, sosyodemografik özellikler, Çocuk DepresyonÖlçeği, Durumluk Kaygı Ölçeği, Kaliforniya Zorbalık Mağduriyet Ölçeği, AileDeğerlendirme Ölçeği ve Öğretmenlik Derecelendirme Ölçeği'dir.Bulgular: 6., 7. ve 8. sınıf öğrencilerininsayısı sırasıyla 53 (% 44,5), 29 (% 24,4) ve 37 (% 31,1) idi. Araştırmada 33öğrenci (% 27,7) akran zorbalığı kurbanıydı. Evde şiddete maruz kalan, dahadüşük sınıfta okuyan ve anneleri okulbitirmemiş olan öğrenciler, zorbalığa daha çok maruz kalmıştı. Zorbalığa maruzkalanlarda yüksek düzeyde depresyon, kaygı ve hiperaktivite saptanmıştır.Tartışma: Okul zorbalığı, tüm dünyada yaygınolan ve göz ardı edilmemesi gereken bir konudur. Bu çalışma, zorbalığa maruzkalan çocukların yüksek düzeyde kaygı ve depresyon sıklığı taşıdığını ve buçocukların daha hiperaktif olduklarını tespit etmiştir.

Background: The impaired regulation of emotional responses has significant social consequences for patients with attention deficit hyperactivity disorder (ADHD) and can be thought to increase the risk for technological addictions. Aim: Ditto objective of the present research is to investigate the relationship between technological addictions and trait emotional intelligence (EI) in adolescents with ADHD. Methods: This cross-sectional study was conducted in 150 treatment-naïve adolescents with ADHD, aged 12–18 years. The sociodemographic information form, the Emotional Quotient-Inventory: Youth Version (EQ-i: YV), Internet Addiction Test, Smartphone Addiction Scale, and Conners' Parent Rating Scales were used for data collection. Results: The findings revealed that ADHD-C and female patients have lower mean stress management scores on EQ-i: YV. Patients who have smartphone addiction (SA)/problematic internet usage have lower total EI and stress management scores. The oppositional, hyperactivity, and DSM-total scores were negatively correlated with stress management scores. Intrapersonal and stress management scores were negatively correlated to SA symptoms. Conclusion: The stress management dimension was the strongest factor related to ADHD and technological addictions. In adolescents with ADHD, stress management may be the key factor to cope with daily problems. Therefore, the interventions to develop EI can be a therapeutic option in ADHD and technological addictions. [ABSTRACT FROM AUTHOR]

ADHD is a treatable neuropsychiatric disorder which can continue till adulthood. There have been three subtypes including attention deficiency, hyperactivity and impulsivity. Its etiology remains unclear yet. It is hypothesized that there is an impairment of neurodevelopment in early developmental periods through the interactions between genetic and environmental processes. Corpus callosum provides an integration of cognitive, sensorial and motor functions of brain as a response to the impulses originating from the cortex. The damage of this region may result in deterioration of some cerebral functions such as cognitive functions, speaking, learning ability and impulse control. In this paper, we report two pediatric cases who had comorbid attention deficiency and corpus callosum agenesis without mental retardation., ÖZET: Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) erişkin hayatta da devam edebilen, tedavi edilebilir nöropsikiyatrik bir bozukluktur. Dikkat eksikliği, aşırı hareketlilik ve dürtüsellikle karakterize üç alt tipi bulunmaktadır. DEHB, etiyolojisi halen yeterince aydınlatılamamış bir alandır. DEHB de erken gelişim evrelerinde, çeşitli genetik ve çevresel etkenlerin etkileşimiyle, nörogelişimde bir aksama olduğu öne sürülmektedir. Korpus kallozum, korteksten köken alan uyaranları karşı taraf hemisfere bağlayarak beynin motor, duyusal ve kognitif performansını sağlamaktadır. Bu bölgede meydana gelen bir hasar kognitif beceriler, konuşma, ögrenme ve dürtü kontrol gibi beyin fonksiyonlarında bozulmaya neden olabilir. Bu yazıda; ilk ruhsal belirtileri dikkat eksikliği ve hiperaktivite olan iki çocuk olgudaki, korpus kallozum agenezisi ve DEHB birlikteliğinin olası patofizyolojik mekanizmaları tartışılmaktadır.

Aggressive behavior and hyperactivity are neurodevelopmental diseases with unknown pathogenesis. Pericentric inv(10)(p11.2;q21.2) mutation is frequently encountered in cytogenetic laboratories. This mutation is accepted as a polymorphic variant and is phenotypically silent, but in some cases it has been associated with neurodevelopmental diseases like autism. After blood culturing, standard chromosome obtaining procedure was applied to patients. In this study, clinical and cytogenetical findings of a boy with developmental delay, mental and motor retardation, attention deficit and hyperactivity have been reported. As a result of chromosome analysis, on chromosome 10, a large pericentric inversion between p11.1 and q22.q bands has been found. Karyotype analysis was also performed to mother, father and siblings of the patient and they have been found to have normal karyotype. It is plausible to consider a relation between inv(10) and some behavioral problems. Additionally, molecular studies targeting 10p-q critical region will be more informative for the true identification of this disease., Agresif davranış ve hiperaktivite, nedeni ve patogenezi bilinmeyen genetik nörogelişimsel hastalıklardır. Perisentrik inv(10)(p11.2;q21.2) mutasyonu sitogenetik laboratuarlarında sıklıkla tanımlanmaktadır. Bu olgu fenotipik olarak sessizdir ve polimorfik bir varyant olduğu düşünülmektedir ancak bazı hastalarda otizm gibi nörogelişimsel hastalıklarla ilişkilendirilmiştir. Bir erkek çocuğunun, G-bantlama boya tekniği kullanılarak karyotip incelemesi yapıldı. Bu çalışmada; gelişme geriliği, mental motor retardasyonu, dikkat eksikliği ve hiperaktivite hastalığı olan bir erkek çocuğunun klinik ve sitogenetik bulguları rapor edildi. Kromozom analizi sonucunda, 10. kromozomun p11.1 ve q22.1 bantları arasındaki bölgeyi kapsayan geniş bir perisentrik inversiyon saptandı. Probandın anne, baba ve kardeşlerinin karyotiplerinin normal olduğu bulundu. Bulgumuz, inv(10)"un davranış bozukluğu hastalıkları ile ilişkili olabileceğini düşündürmektedir. Bununla birlikte, moleküler çalışmalar, hastalığın patogenezine dahil olan 10p-q kritik bölgesinde yer alan genlerin bulunmasına ve hastalığın daha iyi tanımlanmasına yardımcı olacaktır.

Children regularly and voluntarily go to school in order to fulfill the expectations of society from them to continue their education or schooling. School continuation has been made compulsory by laws. Nonetheless, contrary to popular belief, for some children it is distressing to go to school. These children have difficulty continuing school and/or refuse to go to school. Today school refusal is defined as a child's inability to continue school for reasons, such as anxiety and depression. The prevalence of school refusal has been reported to be approximately 1% in school-age children and 5% in child psychiatry samples. The prevalence of school refusal is similar among boys and girls. School refusal can occur at any time throughout the child's academic life and at all socio-economic levels. School refusal is considered a symptom rather than a clinical diagnosis and can manifest itself as a sign of many psychiatric disorders, with anxiety disorders predominant. Separation anxiety disorder, generalized anxiety disorder, social phobia, specific phobia, and adjustment disorder with anxiety symptoms are the most common disorders co-occurring with school refusal. While separation anxiety disorder is associated with school refusal in younger children, other anxiety disorders, especially phobias, are associated with school refusal in adolescents. Children who have parents with psychiatric disorders have a higher incidence of school refusal, and psychiatric disorders are more frequently seen in adult relatives of children with school refusal, which supports a significant role of genetic and environmental factors in th etiology of school refusal. School refusal is a emergency state for child mental health. As it leads to detrimental effects in the short term and the long term, it should be regarded as a serious problem. The long-lasting follow-up studies of school refusing children have revealed that these children have a higher incidence of psychiatric disorders or that they are more likely to require psychological assistance. It is for these reasons that the treatment of school refusal is increasingly gaining impor-tance. The major aim of the treatment is to help the child return to school at the shortest time possible. The treatment should be carried out in cooperation with the child's parents and the school personnel. A widely accepted approach to the treatment of school refusal is one that is concerned with the application of a multi-faceted treatment. Psychosocial and psychopharmacological approaches constitute the crucial parts of the therapeutic process. Today, cognitive behaviour therapy and medication are the most frequently employed approaches in the treatment of school refusal., Toplumun eğitim ve okula devam etme gibi beklentilerini karşılamak için çocuklar düzenli ve gönüllü olarak okula gitmektedir. Okula devam etme yasalar ile de zorunlu hale getirilmiştir. Bununla birlikte, bazı çocuklar için okula gitmek beklenenin ötesinde sıkıntı verici bir durumdur. Bu çocuklar okula devam etmede güçlük çekmekte ve/veya okula gitmeyi reddetmektedir. Günümüzde okul reddi, çocukların anksiyete ve depresyon gibi duygusal sorunlar nedeni ile okula devam edememesi olarak tanımlanmaktadır. Okul reddinin yaygınlığı tüm okul çağındaki çocuklar için yaklaşık % 1 ve kliniğe başvuran tüm çocuklar için % 5 olarak bildirilmektedir. Okul reddi erkek ve kız çocuklarda eşit oranda görülmektedir. Okul hayatının herhangi bir döneminde ortaya çıkabilmektedir. Okul reddinin tüm sosyoekonomik düzeylerde görülebildiği bildirilmektedir. Okul reddi klinik bir tanı olmayıp bir belirti olarak değerlendirilmektedir. Anksiyete bozuklukları başta olmak üzere birçok ruhsal bozukluğun belirtisi olabilmektedir. Ayrılma anksiyetesi bozukluğu, yaygın anksiyete bozukluğu, sosyal fobi, özgül fobi ve anksiyete ile giden uyum bozukluğu okul reddi ile birlikte en sık görülen tanılardır. Tanılarda yaşa bağlı eğilimler görülmektedir. Ayrılma anksiyetesi bozukluğu genelde yaşça daha küçük çocuklardaki okul reddi ile ilişkilendirilirken, diğer anksiyete bozuklukları özellikle de fobiler ergenlerdeki okul reddi ile ilişkilendirilmektedir. Ruhsal bozukluğu olan ebeveynlerin çocuklarında okul reddi sık görülmektedir. Bu, genetik ve çevresel faktörlerin okul reddi gelişimindeki rolünü desteklemektedir. Okul reddi bir çocuk ruh sağlığı acilidir. Kısa ve uzun dönemde olumsuz sonuçlar doğurduğu için ciddi bir sorun olarak değerlendirilmektedir. Okul reddi olan çocukların uzun süreli izlem çalışmalarında, genel toplum ya da kontrol gruplarına göre daha yüksek oranda ruhsal bozuklukların bulunduğu ya da ruhsal yardıma gereksinim duyma olasılıklarının daha yüksek olduğu gösterilmiştir. Bu nedenlerden dolayı okul reddinin tedavisi çok önem kazanmaktadır. Tedavide temel amaç çocuğun en kısa süre içerisinde okula dönmesini sağlamaktır. Tedavi ebeveyn ve okul personeli ile işbirliği içinde yürütülmelidir. Tedavide genel kabul gören yöntem çok eksenli bir tedavi yaklaşımı uygulamaktır. Psikososyal ve psikofarmakolojik yaklaşımlar tedavinin önemli ayaklarını oluşturur. Günümüzde bilişsel davranışçı terapi ve ilaç sağaltımı tedavide en sık kullanılan yaklaşımlar olarak öne çıkmaktadır.

38. Executive Functions in Frontal Lob Syndrome: A Case Report.

Prefrontal cortex in the frontal lobe (FL) is the center of executive functions (EF). FL damage can lead to executive dysfunction by influencing frontal-subcortical circuits (dorsolateral, orbitofrontal, ventromedial). Damage to the dorsolateral prefrontal cortex (DLPFC) can lead to deterioration in EF, whereas damage to the orbitofrontal cortex (OFC) can lead to personality changes with characteristics of disinhibition and irritability. In addition, damage to the anterior cingulate cortex/medial prefrontal cortex (ACC/MPFC) can result in decreased spontaneity. Neuropsychological tests are important components in the assessment of EF including goal-directed behavior, decision-making, risk assessment, future plan management, prioritizing, and organization. Clinical conditions affecting frontal-subcortical connections outside of the FL can also lead to executive dysfunctions and frontal lobe syndrome (FLS). This case report is about an adolescent patient diagnosed with FLS. The clinical symptoms, assessment, and treatment processes of this case are discussed in this report. The case is a 15-year-old boy that was admitted to our clinic with behavioral problems that began after a car accident three years ago. A magnetic resonance imaging (MRI) study of the brain indicated hyper-intense signal increase in the periventricular deep white matter that was associated with traumatic brain damage. Neuropsychological test results (Stroop, Wisconsin Card Sorting Test, Serial Digit Learning Test, Judgement of Line Orientation Test, Verbal Memory Processes Scale) demonstrated impairment in the cognitive flexibility, verbal fluency, prioritizing, inappropriate response inhibition, sustained attention, planning, problem solving, organization, and subcortical memory functions. We thought that cognitive and behavioral symptoms of this case were associated with the dysfunctions of the frontal-subcortical circuits, independently from an obvious frontal lesion. FLS patients after head traumas, with sudden-onset behavioral and cognitive problems, should be kept in mind in differential diagnosis even in the absence of an obvious frontal lesion. Dikkatsizlik temel olarak okulda güçlük yaratır. Hiperaktif çocuk yaşıtlarına göre aşın hareketlidir. Ancak yapılandırılmış ortamlarda bunu göstermeyebilirler. Dürtüsellik, davranışın olumsuz sonucunu düşünmeden cevap vermedir. DEHB yüksek ailesel geçiş gösterir, ancak klinik olarak erken çocukluk çağında belirti veren, çoklu etkenlere bağlı bir bozukluktur. Eşlik eden bozukluklar sıklıkla görülür. Dikkatsizlik ve dürtüsellik tanısı alan olguların beyninde, dopaminerjik ve noradrenerjik yolaklarda, normal çocuklara göre bozukluk vardır. Belirtilerde, noradrenerjik ve dopaminerjik nörol yolaklarda değişiklik yapan bir çok ilaçlarla düzelme olduğu gösterilmiştir. VI ABSTRACT SOCIODEMOGRAPHIC CHARACTERISTICS, COMORBID DISORDERS AND TREATMENT OF CHILDREN DIAGNOSED WITH ATTENTION DEFICIT HYPERACTIVITY DISORDER Attention-deficit/hyperactivity disorder (ADHD) occurs in approximately 3-7% of school-age children. Inattention is mainly impaired inschool. Hyperactive child is more than excessive activity, although the latter has been demonstrated to be present during structured in-seat activity. Impulsivity refers to the inability to delay a response despite the anticipation of negative consequenses for the behavior. ADHD is considered a highly heritable but clinically heterogeneous disorder with onset of symptoms in early childhood. Patients are said to be `comorbid` when they have more than one psychiatric disorder. Its pathophysiology appears to involve alterations in dopaminergic and noradrenergic patways associated with control of attention and impulsivity. Symptoms have been shown to respond favorably to several pharmacological interventions, notably those that affect dopaminergic and noradrenergic neurotransmission. Objective: Inpatient rehabilitation and treatment systems, the Residential Treatment Centers (RTC) are widely used in the treatment of childhood sexual abuse (CSA) cases and offer the advantages of keeping a combination of several applications. „Oguz Kagan Koksal Social Care and Rehabitation Center' is the first RTC in Turkey providing services to girl CSA victims. In this study, demographic, clinical and follow-up findings of patients treated in this center since the establishment of the six year period will be presented. Methods: This study included 152 girls aged between 8-18 years who had histories of CSA, neglect or other kinds of adverse experiences, and in need for treatment or protecttion. Demographic and abuse-specific data were obtained from file records. The Child Beck Depression Inventory, the Trait-State Anxiety Inventory-1, -2, the Moudsley Obsessive Compulsive Questionnaire, and the Clinical Global Impression Scale were administered at the beginning and at the end of the treatment period. Results: A history of CSA was reported by 120 girls. Almost half of the cases were diagnosed as posttraumatic stress disorder, 23.7% as borderline personality disorder, 21.1% as destructive behavior disorders, and 9.2 as bipolar disorder. In the treatment, 95% of cases taken at least one psychotropic medication. The most commonly used drugs were antipsychotics, and given in treatment of 81.4% of girls. In addition, 51.7% of cases taken mood stabilizers, and 25.5% of them took antidepressants. Mean score of all psychometric tests were improved significantly at the end point compared to the baseline. Conclusion: Our findings support the usefulness of the system presented herein for child and adolescent victims of CSA. This study is important because it involves data from the first RTM in our country in this field. Comorbid psychiatric disorders, frequent hospitalization, multiple outpatient treatment, prior history of hypertension, obesity and lipid dysregulation are associated with higher risk of metabolic syndrome in children. Side effects of antipsychotic drugs and their management have recently become a major subject of research due to enhanced antipsychotic drug usage in child and adolescents. Prevention strategies are usually preferred to secondary or tertiary strategies in the management of metabolic syndrome associated with antipsychotic drugs. Clinicians should present multidisciplinary approach to endocrine and metabolic side effects due to antipsychotic use in pediatric patient groups and avoid multiple drug use in such patients. In this paper, we briefly reviewed metabolic side effects of second generation antipsychotic drugs in child and adolescent population, possible mechanisms of susceptibility to metabolic syndrome and pharmacological and non pharmacological treatment approach to prevention of weight gain. - Abstract
Assisted reproductive techniques are used in the treatment of infertility worldwide. Long-term and multidimensional effects of these techniques on the embryo, especially on genetic programming, are still unknown. Moreover, the difficult period before parenting may affect the attachment and, consequently, the child's mental health. The aim of this review is to discuss the neurodevelopmental and behavioral effects of assisted reproductive techniques from fetal and early childhood stages to adolescence. Follow-up studies, case reports, and meta-analyses investigating the neurological, psychiatric and genetic effects of in vitro fertilization techniques were reviewed. Animal studies suggest that the serum content, in which the embryo is preserved, may damage its genetic programming. Cerebral palsy, preterm delivery and low birth weight are the most frequent obstetric complications. Executive functions, memory and attention may be affected, especially in school-age children. Cases of autism have been reported as well. Preimplantation genetic screening has not been commonly used because of its high cost, complex technical procedures and ethical responsibilities. This paper suggests that multidisciplinary approach is needed in order to predict neurological and mental disorders. Parenting education and psychosocial support for infertile couples may facilitate the healthy attachment with their baby. [ABSTRACT FROM AUTHOR]
