2. Contribution of AcrAB efflux pump to ciprofloxacin resistance in Klebsiella pneumoniae isolated from burn patients

Abstract

[english] Resistance to fluoroquinolones has been recently increased among bacterial strains isolated from outpatients. Multidrug-resistant is one of the major organisms isolated from burn patients and the AcrAB efflux pump is the principal pump contributing to the intrinsic resistance in against multiple antimicrobial agents including ciprofloxacin and other fluoroquinolones. Fifty-two isolated from burn patients in Shahid Motahari hospital and confirmed by conventional biochemical tests. Antimicrobial susceptibility testing was done according to CLSI 2011 guidelines, to determine the antimicrobial resistance pattern of isolates. A gene was detected among ciprofloxacin-resistant isolates by PCR assay. MICs to ciprofloxacin were measured with and without carbonyl cyanide . Forty out of the 52 isolated from burn patients in Shahid Motahari hospital were resistant to ciprofloxacin according to breakpoint of CLSI guideline. PCR assay for A gene demonstrated that all ciprofloxacin-resistant isolates harbored A gene coding the membrane fusion protein AcrA and is a part of AcrAB efflux system. Among these isolates, 19 strains (47.5%) showed 2 to 32 fold reduction in MICs after using CCCP as an efflux pump inhibitor. The other 21 strains (52.5%) showed no disparity in MICs before and after using CCCP. In conclusion, the AcrAB efflux system is one of the principal mechanisms contribute in ciprofloxacin resistance among isolates but there are some other mechanisms interfere with ciprofloxacin resistance such as mutation in target proteins of DNA gyrase of topoisomerase IV enzymes.

Abstract

This study was designed to determine the molecular characteristic and antimicrobial resistance of enterococcal strains isolated from patients in an Iranian Hospital. Enterococcal strains were isolated from the burn patients. All strains were screened for genes encoding resistance to aminoglycoside [ aac(6')-Ie-aph(2'')-Ia, aph (3'), ant (4')], resistance to vancomycin (vanA, vanB), resistance to tetracycline (tetK, tetL, tetM, tetO), and resistance to erythromycin (ermA, ermB, ermC) by PCR and multiplex PCR–based methods. Genetic diversity was evaluated using Random Amplified Polymorphic DNA (RAPD)-PCR. All enterococcal isolates showed complete sensitivity to vancomycin with MIC ≤ 0.5µg/ml. Resistance to gentamicin, tetracycline, erythromycin, ciprofloxacin or quinopristin-dalfopristin was detected, whilst more than 96.2% of isolates were high-level gentamicin-resistant (HLGR) and multiresistant. The most prevalent aminoglycoside resistance gene was aac(6')-Ie-aph(2'')-Ia, found in 96.2% (26/27) of the isolates. The most prevalent tetracycline resistance gene was tetM, found in 85.1% (23/27) of the isolates followed by tetL and tetO found in 7.4% (2/27) of the isolates. The ermA and ermB genes were detected in 33.3% (9/27) and 44.4% (12/27) of the isolates respectively. Random Amplified Polymorphic DNA (RAPD)-PCR analysis yielded 17 distinct profiles among 27 investigated isolates. One clusters of isolates shared the same RAPD patterns, while 16 isolates had unique RAPD patterns. we found that over the examined time period one RAPD genotype were common type and were disseminated among patients in the burn unit. Abstract

Objectives: Today, the emergence of antibiotic-resistant strains and the acquisition of antibiotic resistance have caused many problems in the treatment of Staphylococcus aureus infection and is one of the most important health issues. At present, nanotechnology has a significant impact on the various fields including pharmacology, health, medicine, and food. This study aimed to synthesize the titanium dioxide nanoparticles (Tio2NPs) and its antibacterial effect on S. aureus compared with conventional antibiotics. Materials and Methods: In this study, Tio2NPs were tested on S. aureus compared with a number of antibiotics. First, Tio2NPs were synthesized. The shape and size of the particles as well as the synthesis quality were investigated using scanning electron microscopy (SEM), atomic force microscopy (AFM), dynamic light scattering, zeta potential and X-ray diffraction, and Fourier-transform infrared spectroscopy. The antimicrobial effect of Tio2NPs on S. aureus strain (ATCC 12600) was then studied by disc diffusion method. Antibiotics of ampicillin, colistin, and ertapenem were used as control group. Results: Based on the results and given that the polydispersity index of the samples was below 0.5 (0.466), the other sample was estimated at 347.9 nm. The zeta potential of Tio2 sample was estimated at −9.48 indicating the stability of the nanoparticle and its suspension in a suitable amount per unit of time in the solution. The results of the AFM showed that the lower mean value obtained for the Tio2NPs was 0.5 nm, and the growth of the nanoparticles was noticeable in some regions and uniform and low in some others. The result of SEM showed that the size of the nanoparticle was 28.45–34.14 nm. The best inhibition zone diameter was obtained for ampicillin (30.66 mm), followed by ertapenem (15 mm) and Tio2(10 mm). Colistin without inhibition zone was identified as ineffective group. Conclusion: Due to the development of drug resistance to antibiotics, the production and synthesis of antimicrobial agents is one of the requirements of the current time. Due to the excellent synthesis of Tio2 and the presence of very fine nanoparticles, it can be used as a strong antimicrobial compound, especially on S. aureus infection. [ABSTRACT FROM AUTHOR]
