1. The histone demethylase KDM4B regulates peritoneal seeding of ovarian cancer

Abstract

Epithelial ovarian cancer (EOC) has poor prognosis and rapid recurrence because of widespread dissemination of peritoneal metastases at diagnosis. Multiple pathways contribute to the aggressiveness of ovarian cancer, including hypoxic signaling mechanisms. In this study, we have determined that the hypoxia-inducible histone demethylase KDM4B is expressed in ∼60% of EOC tumors assayed, including primary and matched metastatic tumors. Expression of KDM4B in tumors is positively correlated with expression of the tumor hypoxia marker CA-IX, and is robustly induced in EOC cell lines exposed to hypoxia. KDM4B regulates expression of metastatic genes and pathways, and loss of KDM4B increases H3K9 trimethylation at the promoters of target genes like LOXL2, LCN2 and PDGFB. Suppressing KDM4B inhibits ovarian cancer cell invasion, migration and spheroid formation in vitro. KDM4B also regulates seeding and growth of peritoneal tumors in vivo, where its expression corresponds to hypoxic regions. This is the first demonstration that a Jumonji-domain histone demethylase regulates cellular processes required for peritoneal dissemination of cancer cells, one of the predominant factors affecting prognosis of EOC. The pathways regulated by KDM4B may present novel opportunities to develop combinatorial therapies to improve existing therapies for EOC patients.

4. Machine learning model to predict mental health crisis from electronic health records

Abstract

Timely identification of patients who are at risk of mental health crises opens the door for improving the outcomes and for mitigating the burden and costs to the healthcare systems. Due to high prevalence of mental health problems, a manual review of complex patient records to make proactive care decisions is an unsustainable endeavour. We developed a machine learning model that uses Electronic Health Records to continuously identify patients at risk to experience a mental health crisis within the next 28 days. The model achieves an area under the receiver operating characteristic curve of 0.797 and an area under the precision-recall curve of 0.159, predicting crises with a sensitivity of 58% at a specificity of 85%. The usefulness of our model was tested in clinical practice in a 6-month prospective study, where the predictions were considered clinically useful in 64% of cases. This study is the first one to continuously predict the risk of a wide range of mental health crises and to evaluate the usefulness of such predictions in clinical settings.

Abstract

The interaction of water flow, ice, and structures is common in fluvial ice processes, particularly around Ice Control Structures (ICSs) that are used to manage and prevent ice jam floods. To evaluate the effectiveness of ICSs, it is essential to understand the complex interaction between water flow, ice and the structure. Numerical modeling is a valuable tool that can facilitate such understanding. Until now, classical Eulerian mesh-based methods have not been evaluated for the simulation of ice interaction with ICS. In this paper we evaluate the capability, accuracy, and efficiency of a coupled Computational Fluid Dynamic (CFD) and multi-body motion numerical model, based on the mesh-based FLOW-3D V.2023 R1 software for simulation of ice-structure interactions in several benchmark cases. The model’s performance was compared with results from meshless-based models (performed by others) for the same laboratory test cases that were used as a reference for the comparison. To this end, simulation results from a range of dam break laboratory experiments were analyzed, encompassing varying numbers of floating objects with distinct characteristics, both in the presence and absence of ICS, and under different downstream water levels. The results show that the overall accuracy of the FLOW-3D model under various experimental conditions resulted in a RMSE of 0.0534 as opposed to an overall RMSE of 0.0599 for the meshless methods. Instabilities were observed in the FLOW-3D model for more complex phenomena that involve open boundaries and a larger number of blocks. Although the FLOW-3D model exhibited a similar computational time to the GPU-accelerated meshless-based models, constraints on the processors speed and the number of cores available for use by the processors could limit the computational time.

Abstract

BACKGROUND: Cognitive impairment is common in haemodialysis (HD) patients and is associated independently with depression and mortality. This association is poorly understood, and no intervention is proven to slow cognitive decline. There is evidence that cooler dialysis fluid (dialysate) may slow white matter changes in the brain, but no study has investigated the effect of cooler dialysate on cognition. This study addresses whether cooler dialysate can prevent the decline in cognition and improve quality of life (QOL) in HD patients. METHODS: This is a multi-site prospective randomised, double-blinded feasibility trial. SETTING: Four HD units in the UK. PARTICIPANTS AND INTERVENTIONS: Ninety HD patients randomised (1:1) to standard care (dialysate temperature 36.5 °C) or intervention (dialysate temperature 35 °C) for 12 months. PRIMARY OUTCOME MEASURE: Change in cognition using the Montreal Cognitive Assessment (MoCA). SECONDARY OUTCOME MEASURES: Recruitment and attrition rates, reasons for non-recruitment, frequency of intradialytic hypotension, depressive symptom scores, patient and carers burden, a detailed computerised cognitive test and QOL assessments. ANALYSIS: mixed method approach, utilising measurement of cognition, questionnaires, physiological measurements and semi-structured interviews. DISCUSSION: The results of this feasibility trial will inform the design of a future adequately powered substantive trial investigating the effect of dialysate cooling on prevention and/or slowing in cognitive decline in patients undergoing haemodialysis using a computerised battery of neuro-cognitive tests. The main hypothesis that would be tested in this future trial is that patients treated with regular conventional haemodialysis will have a lesser decline in cognitive function and a better quality of life over 1 year by using cooler dialysis fluid at 35 °C, versus a standard dialysis fluid temperature of 36.5 °C. Abstract

The Chaudière River in Quebec, Canada, is well known for its frequent ice jam flooding events. As part of a larger watershed research program, an extensive field campaign has been carried out during the 2018–2019 and 2019–2020 winter seasons to quantify the spatiotemporal characteristics of the break-up processes along the Chaudière River. The results showed that mid-winter ice jams have formed in the Intermediate Chaudière and persisted until spring break-up. Spring break-ups were initiated in the Upper Chaudière, and then, almost simultaneously, in the Intermediate and Lower Chaudière reaches. The break-up in the Intermediate Chaudière usually lasts longer than the rest of the river since the slope is much milder, and the occurrence of mid-winter ice jams has been seen to delay the ice clearing. A reach-by-reach characterization of the cumulative degree day of thawing and discharge thresholds for the onset of break-up has been identified. During the field campaign, 51 ice jams were documented together with their location, length, date of formation, and the morphological feature triggering jam formation. Break-up patterns, hydrometeorological thresholds of ice mobilization, and ice jam sites identified in this study can serve as a basis for the implementation of an early warning system. HIGHLIGHTS Effect of mid-winter jamming and the importance of monitoring the entire ice season has been highlighted.; A reach-by-reach break-up sequence of the entire Chaudière River has been documented.; The dependency of the onset of break-up on threshold levels of cumulative degree day of thawing and discharge was assessed.; Fifty-one ice jams were characterized together with the geomorphological features that initiated their formation.;

Abstract

Importance: Early cardiovascular and bleeding events after coronary stenting are associated with high risk of morbidity and mortality. Objective(s): To assess the prognosis of cardiovascular and bleeding events occurring beyond 1 year after coronary stenting. Design, Setting, and Participant(s): This secondary analysis is derived from data from the Dual Antiplatelet Therapy (DAPT) Study, a multi center trial involving 220 US and in ternational clinical sites from 11 countries. The study dateswere August 2009 to May 2014. Individuals who underwent coronary stenting and completed 12 months of thienopyridine plus aspirin therapy without ischemic or bleeding events remained on an aspirin regimen and were randomized to continued thienopyridine therapy vs placebo for 18 additional months. Individuals were then followed up for 3 additional months while receiving aspirin therapy alone. The analysis began in August 2015. Abstract

Introduction: Mild cognitive impairment (MCI) is a term used to describe a level of decline in cognition which is seen as an intermediate stage between normal ageing and dementia, and which many consider to be a prodromal stage of neurodegeneration that may become dementia. That is, it is perceived as a high risk level of cognitive change. The increasing burden of dementia in our society, but also our increasing understanding of its risk factors and potential interventions, require diligent management of MCI in order to find strategies that produce effective prevention of dementia. Aim: To update knowledge regarding mild cognitive impairment, and to bring together and appraise evidence about the main features of clinical interest: definitions, prevalence and stability, risk factors, screening, and management and intervention. Methods: Literature review and consensus of expert opinion. Results and conclusion: MCI describes a level of impairment in which deteriorating cognitive functions still allow for reasonable independent living, including some compensatory strategies. While there is evidence for some early risk factors, there is still a need to more precisely delineate and distinguish early manifestations of frank dementia from cognitive impairment that is less likely to progress to dementia, and furthermore to develop improved prospective evidence for positive response to intervention. An important limitation derives from the scarcity of studies that take MCI as an endpoint. Strategies for effective management suffer from the same limitation, since most studies have focused on dementia. Behavioural changes may represent the most cost-effective approach. Abstract

Background and Objectives:\ud \ud The provision and quality of mental health services in Acute General Hospitals is a growing concern. Developing research to elicit the views of patients and staff will offer insights into service improvements. The Rapid Assessment, Interface and Discharge service (RAID) developed in an Acute General Hospital to deliver a rapid-response, 24-hour, 7-day- a- week, age-inclusive intervention was evaluated for its impact on staff satisfaction, with emphasis on staff training; and patient satisfaction, with emphasis on the differences in satisfaction between working age (under 65 years) and older adults (over 65 years).\ud \ud Population:\ud \ud Staff working in acute hospital for patients with mental health needs, and patients presenting to acute hospitals, requiring clinical input for their mental health.\ud \ud Method:\ud \ud Data on patient satisfaction was collected through a structured telephone questionnaire including fixed and open-ended questions. Data related to staff satisfaction regarding the service provided was collected by a semi-structured interview administered face-to-face with staff from wards referring to the team. Training was evaluated using open-ended, Likert-scale and open-ended questionnaires.\ud \ud Results:\ud \ud Results show that the majority of working age patients rated the service as ‘good’ (42.2%), felt that the team was helpful in their care (84.8%), met their mental health needs (69.7%), and treated them with respect (96.1%). Overall, older adults rated the service as ‘excellent’ (58.3%), felt that the team was helpful in their care (85.7%), met their mental health needs (85.7%), treated them with respect (92.9%) and stated that they were seen in good time (100%). The difference in satisfaction levels between patients of working age and older patients was statistically significant.\ud \ud Common aspects staff rated as most helpful were advice on managing patients (12.0%), support of staff (11.0%) and advice on medication (11.0%). The majority of staff surveyed felt that their practice would be improved following the training, and rated it as either excellent (61.6%) or good (36.3%).\ud \ud Interpretation and Conclusion:\ud \ud This study highlighted the benefits of providing support and training to staff working directly with patients with mental health needs. It is more challenging to measure the satisfactory effect of older people who continue to give favourable answers on satisfaction questionnaires.

Abstract

Climate change affects many meteorological parameters that could result in spatiotemporal variations of the hydrological cycle. These variations can affect local rainfall intensities or design storms; therefore, it is necessary to assess the local effects of climate change in different areas. Therefore, the current research aims at evaluating the accuracy of the precipitation data of the most recent Coupled Model Intercomparison Project phases 5 and 6 (CanESM2 from CMIP5 and CanESM5 from CMIP6 models), over a historical period from 1953 to 2010, as well as the predicted data for the future between 2010 and 2050 for the Quebec City rain gauge Station (Jean Lesage Intl). In this regard, precipitation data were analyzed using a statistical index to find the most accurate model for the study area. The results of this evaluation showed that CanESM5 is more accurate than CanESM2 for most of the evaluation indices. However, both of these models did not perform well since the precipitation prediction for CanESM5 (as the accurate model) R index was 0.48 for the monthly and was 0.75 in the seasonal scale. In addition, the Bias index revealed that both models underestimated rainfall prediction with negative index values for both scales and models. The trend of future precipitation under socio–economic scenarios (4.5 (pessimistic) and 8.5 (optimistic)) shows that the changes in future precipitation are not significant. In addition, for scenario 4.5, the trend of precipitation decreases for almost half of the year, while for scenario 8.5, the magnitude of the decrease and the number of months with a decreasing trend of precipitation are significantly reduced when compared to scenario 4.5.

Abstract

Rebar corrosion in reinforced concrete bridge decks is a major problem. Most technical solutions forthe problem attempt to slow the corrosion rate or prevent corrosion, but an alternate approach isreplacing the steel with a more durable material such as fiber reinforced polymer (FRP).The University of Sherbrooke, Quebec, Canada, recently conducted a field project in which FRP rebar and grids were used to reconstruct portions of a concrete deck slab that had severely deteriorated when the reinforcing steel corroded. The project included analysis and design of the bridge structure, construction of the concrete deck slab, and field performance monitoring of the completed construction using fiber optic sensors (FOSs). The integration of FOSs to monitor long-term performance of RC structures is a fairly recent development that deserves more attention from practicing engineers and field personnel.

Abstract

Background: There is little available literature on the effect of suicide methods on brain weight.Aims: To explore variations in post-mortem brain weight in different methods of fatal self-harm (FSH) and in deaths from natural causes.Method: A review of a sample of coroners' records of elderly persons (60 and above). Verdicts of suicide, misadventure and open verdicts were classified as FSH. Post-mortem brain weight for 142 FSH victims and 150 victims of unexpected, sudden or unexplained death due to natural causes, and from various methods of FSH, were compared.Results: Brain weight of victims of FSH was significantly higher than of those who died of natural causes (P < 0.01); brain weights in both groups were within the normal range for this age group. There was no significant difference in brain weight between different methods of FSH (P > 0.05).Conclusions: The findings require critical examination and further research, to include data from younger age groups. A regional or national suicide neuropathological database could be set up if all victims of FSH underwent routine neurohistochemical post-mortem examination. [ABSTRACT FROM AUTHOR]

Abstract

Introduction and ObjectivesThe typical profile of patients with Chronic Obstructive Pulmonary Disease (COPD) includes a wide range of co-morbidity, with increasing evidence demonstrating an association between COPD and cognitive impairment. This cross-sectional study aims to demonstrate domain specific cognitive function and deficit in patients with mild-moderate COPD using the comprehensive Addenbrookes Cognitive Examination-III assessment (ACE-III). It also aims to compare the nature of cognitive impairment in patients with COPD to patients with Alzheimer’s type dementia (AD).MethodsWe enrolled 44 patients with mild or moderate COPD, 17 patients with mild or moderate AD and 28 control patients with a similar age profile and gender distribution. We analysed the difference in ACE-III total scores and domain specific scores between the groups and additionally performed logistic regression analyses to account for cardiovascular co-morbidity and smoking exposure.ResultsPatients with COPD had significantly lower total ACE-III scores (p<0.001) compared to the control group with 47.5 percent of patients demonstrating scores below the recognised cut-off score for cognitive impairment of 88 out of 100. This association remained even when accounting for cardiovascular co-morbidity and smoking exposure. There was no significant difference in the visuospatial cognitive domain (p=0.062), however, compared to the control group patients with COPD demonstrated significantly lower scores in memory (p<0.01), attention (p=0.013) fluency (p=0.002) and language (p=0.004) domains. Interestingly the pattern of cognitive impairment in patients with COPD was similar but less severe compared to patients with AD who demonstrated significantly lower total ACE-III scores (p<0.001).ConclusionsThis study demonstrates a high burden of cognitive impairment in patients with COPD. Interestingly the cognitive domains of memory, attention, fluency and language seem to be predominantly affected in this population. These findings further our understanding of cognitive impairment in patients with COPD, with patients exhibiting a similar but less severe pattern of cognitive impairment to that seen in the AD group.
