Abstract

Background and purposeStent-assisted coil embolization using the new generation Neuroform Atlas Stent System has shown promising safety and efficacy. The primary study results of the anterior circulation aneurysm cohort of the treatment of wide-neck, saccular, intracranial, aneurysms with the Neuroform Atlas Stent System (ATLAS trial [Safety and Effectiveness of the Treatment of Wide Neck, Saccular Intracranial Aneurysms With the Neuroform Atlas Stent System]) are presented.MethodsATLAS IDE trial (Investigational Device Exemption) is a prospective, multicenter, single-arm, open-label study of wide-neck (neck ≥4 mm or dome-to-neck ratio 50%) at the target location. The primary safety end point was any major stroke or ipsilateral stroke or neurological death within 12 months. Adjudication of the primary end points was performed by an independent Imaging Core Laboratory and the Clinical Events Committee.ResultsA total of 182 patients with wide-neck anterior circulation aneurysms at 25 US centers were enrolled. The mean age was 60.3±11.4 years, 73.1% (133/182) women, and 80.8% (147/182) white. Mean aneurysm size was 6.1±2.2 mm, mean neck width was 4.1±1.2 mm, and mean dome-to-neck ratio was 1.2±0.3. The most frequent aneurysm locations were the anterior communicating artery (64/182, 35.2%), internal carotid artery ophthalmic artery segment (29/182, 15.9%), and middle cerebral artery bifurcation (27/182, 14.8%). Stents were placed in the anticipated anatomic location in all patients. The study met both primary safety and efficacy end points. The composite primary efficacy end point of complete aneurysm occlusion (Raymond-Roy 1) without parent artery stenosis or aneurysm retreatment was achieved in 84.7% (95% CI, 78.6%-90.9%) of patients. Overall, 4.4% (8/182, 95% CI, 1.9%-8.5%) of patients experienced a primary safety end point of major ipsilateral stroke or neurological death.ConclusionsIn the ATLAS IDE anterior circulation aneurysm cohort premarket approval study, the Neuroform Atlas stent with adjunctive coiling met the primary end points and demonstrated high rates of long-term complete aneurysm occlusion at 12 months, with 100% technical success and

Abstract

Background: Atrial fibrillation (AF) and congestive heart failure often coexist due to their shared risk factors leading to potential worse outcome, particularly cerebrovascular events. The aims of this study were to calculate the rates of ischemic and severe bleeding events in ischemic stroke patients having both AF and reduced ejection fraction (rEF) (⩽40%), compared to ischemic stroke patients with AF but without rEF.Methods: We performed a retrospective analysis that drew data from prospective studies. The primary outcome was the composite of either ischemic (stroke or systemic embolism), or hemorrhagic events (symptomatic intracranial bleeding and severe extracranial bleeding).Results: The cohort for this analysis comprised 3477 patients with ischemic stroke and AF, of which, 643 (18.3%) had also rEF. After a mean follow-up of 7.5 ± 9.1 months, 375 (10.8%) patients had 382 recorded outcome events, for an annual rate of 18.0%. While the number of primary outcome events in patients with rEF was 86 (13.4%), compared to 289 (10.2%) for the patients without rEF; on multivariable analysis rEF was not associated with the primary outcome (OR 1.25; 95% CI 0.84–1.88). At the end of follow-up, 321 (49.9%) patients with rEF were deceased or disabled (mRS ⩾3), compared with 1145 (40.4%) of those without rEF; on multivariable analysis, rEF was correlated with mortality or disability (OR 1.35; 95% CI 1.03–1.77).Conclusions: In patients with ischemic stroke and AF, the presence of rEF was not associated with the composite outcome of ischemic or hemorrhagic events over short-term follow-up but was associated with increased mortality or disability.

Abstract

Introduction: Ischaemic stroke patients with atrial fibrillation (AF) are at high risk of stroke recurrence despite oral anticoagulation therapy. Patients with cardiovascular comorbidities may take both antiplatelet and oral anticoagulation therapy (OAC/AP). Our study aims to evaluate the safety and efficacy of OAC/AP therapy as secondary prevention in people with AF and ischaemic stroke.Patients and methods: We performed a post-hoc analysis of pooled individual data from multicenter prospective cohort studies and compared outcomes in the OAC/AP cohort and patients on DOAC/VKA anticoagulation alone (OAC cohort). Primary outcome was a composite of ischaemic stroke, systemic embolism, intracranial bleeding, and major extracranial bleeding, while secondary outcomes were ischaemic and haemorrhagic events considered separately. A multivariable logistic regression analysis was performed to identify independent predictors for outcome events. To compare the risk of outcome events between the two cohorts, the relation between the survival function and the set of explanatory variables were calculated by Cox proportional hazard models and the results were reported as adjusted hazard ratios (HR). Finally another analysis was performed to compare the overall risk of outcome events in both OAC/AP and OAC cohorts after propensity score matching (PSM).Results: During a mean follow-up time of 7.5 ± 9.1 months (median follow-up time 3.5 months, interquartile range ±3), 2284 stroke patients were on oral anticoagulants and 215 were on combined therapy. The multivariable model demonstrated that the composite outcome is associated with age (OR: 1.03, 95% CI: 1.01–1.04 for each year increase) and concomitant antiplatelet therapy (OR: 2.2, 95% CI: 1.48–3.27), the ischaemic outcome with congestive heart failure (OR: 1.55, 95% CI: 1.02–2.36) and concomitant antiplatelet therapy (OR: 1.93, 95% CI: 1.19–3.13) and the haemorrhagic outcome with age (OR: 1.03, 95% CI: 1.01–1.06 for each year increase), alcoholism (OR: 2.15, 95% CI: 1.06–4.39) and concomitant antiplatelet therapy (OR: 2.22, 95% CI: 1.23–4.02). Cox regression demonstrated a higher rate of the composite outcome (hazard ratio of 1.93 [95% CI, 1.35–2.76]), ischaemic events (HR: 2.05 [95% CI: 1.45–2.87]) and bleeding outcomes (HR: 1.90 [95% CI, 1.06–3.40]) in OAC/AP cohort. After PSM analysis, the composite outcome remained more frequent in people treated with OAC + AP (RR: 1.70 [95% CI, 1.05–2.74]).Discussion: Secondary prevention with combination of oral anticoagulant and antiplatelet therapy after ischaemic stroke was associated with worse outcomes in our cohort.Conclusion: Further research is needed to improve secondary prevention by investigating the mechanisms of recurrent ischaemic stroke in patients with atrial fibrillation.

Abstract

Background: The aim of this study in patients with acute posterior ischemic stroke (PS) and atrial fibrillation (AF) were to evaluate the risks of recurrent ischemic event and severe bleeding and these risks in relation with oral anticoagulant therapy (OAT) and its timing.\ud \ud Methods: Patients with PS were prospectively included; the outcome events of these patients were compared with those of patients with anterior stroke (AS) which were taken from previous registries. The primary outcome was the composite of: stroke recurrence, TIA, symptomatic systemic embolism, symptomatic cerebral bleeding and major extracranial bleeding occurring within 90 days from acute stroke.\ud \ud Results: A total of 2,470 patients were available for the analysis: 473 (19.1%) with PS and 1,997 (80.9%) AS. Over 90 days, 213 (8.6%) primary outcome events were recorded: 175 (8.7%) in patients with AS and 38 (8.0%) in those with PS. In patients who initiated OAT within 2 days, the primary outcome occurred in 5 out of 95 patients (5.3%) with PS compared to 21 out of 373 patients (4.3%) with AS (OR 1.07; 95% CI 0.39-2.94). In patients who initiated OAT between days 3 and 7, the primary outcome occurred in 3 out of 103 patients (2.9%) with PS compared to 26 out of 490 patients (5.3%) with AS (OR 0.54; 95% CI 0.16-1.80).\ud \ud Conclusions: Patients with posterior or anterior stroke and AF appear to have similar risks of\ud ischemic or hemorrhagic events at 90 days with no difference concerning the timing of initiation of OAT.

Abstract Introduction Traumatic brain injury is a form of acquired brain injury that results from sudden trauma to the head. Specifically, mild traumatic brain injury is a clinical diagnosis that can have significant effects on an individual's life, yet is difficult to identify through traditional imaging techniques. Case presentation This is the case of a 68-year-old previously healthy African American woman who was involved in a motor vehicle accident that resulted in significant head trauma. After the accident, she experienced symptoms indicative of mild traumatic brain injury and sought a neurological consultation when her symptoms did not subside. She was initially evaluated with a neurological examination, psychological evaluation, acute concussion evaluation and a third-party memory test using software from CNS Vital Signs for neurocognitive function. A diagnosis of post-concussion syndrome was suggested. Diffusion tensor imaging revealed decreased fractional anisotropy in the region immediately adjacent to both lateral ventricles, which was used to confirm the diagnosis. Fractional anisotropy is a scalar value between zero and one that describes the degree of anisotropy of a diffusion process. These results are indicative of post-traumatic gliosis and are undetectable by magnetic resonance imaging. Our patient was treated with cognitive therapy. Conclusion Minor traumatic brain injury is a common injury with variable clinical presentation. The system of diagnosis used in this case found a significant relationship between the clinical assessment and imaging results. This would not have been possible using traditional imaging techniques and highlights the benefits of using diffusion tensor imaging in the sub-acute assessment of minor traumatic brain injury.

Abstract

Background: \ud In patients with acute ischemic stroke and atrial fibrillation, early anticoagulation prevents ischemic recurrence but with the risk of hemorrhagic transformation (HT). The aims of this study were to evaluate in consecutive patients with acute stroke and atrial fibrillation (1) the incidence of early HT, (2) the time to initiation of anticoagulation in patients with HT, (3) the association of HT with ischemic recurrences, and (4) the association of HT with clinical outcome at 90 days.\ud \ud Methods and Results: \ud HT was diagnosed by a second brain computed tomographic scan performed 24 to 72 hours after stroke onset. The incidence of ischemic recurrences as well as mortality or disability (modified Rankin Scale scores >2) were evaluated at 90 days. Ischemic recurrences were the composite of ischemic stroke, transient ischemic attack, or systemic embolism. Among the 2183 patients included in the study, 241 (11.0%) had HT. Patients with and without HT initiated anticoagulant therapy after a mean 23.3 and 11.6 days, respectively, from index stroke. At 90 days, 4.6% (95% confidence interval, 2.3–8.0) of the patients with HT had ischemic recurrences compared with 4.9% (95% confidence interval, 4.0–6.0) of those without HT; 53.1% of patients with HT were deceased or disabled compared with 35.8% of those without HT. On multivariable analysis, HT was associated with mortality or disability (odds ratio, 1.71; 95% confidence interval, 1.24–2.35).\ud \ud Conclusions: \ud In patients with HT, anticoagulation was initiated about 12 days later than patients without HT. This delay was not associated with increased detection of ischemic recurrence. HT was associated with increased mortality or disability.

Abstract

Objective: Tonic-clonic activity (TCA) at onset complicates 3% to 21% of cases of subarachnoid hemorrhage (SAH). The impact of onset TCA on in-hospital complications, including seizures, remains unclear. One study associated onset TCA with poor clinical outcome at 6 weeks after SAH, but to our knowledge no other studies have confirmed this relationship. This study aims to assess the impact of onset TCA on in-hospital complications, poor functional outcome, mortality, and epilepsy at 3 months. Methods: Analysis of a prospective study cohort of 1479 SAH patients admitted to Columbia University Medical Center between 1996 and 2012. TCA within 6 hours of hemorrhage onset was identified based on accounts of emergency care providers or family witnesses. Results: TCA at onset was described in 170 patients (11%). 32. Posterior Cerebral Artery Stroke

Abstract

A solid understanding of the pathophysiology of a posterior cerebral artery (PCA) stroke as well as the syndrome relating to it, requires adequate knowledge of the structures and vascular anatomy of the brain. Anterior and posterior circulations provide the primary blood circulation of the brain. Both circulations are connected by the posterior communicating arteries (PCOM), which make up the circle of Willis. When there is an occlusion in the cerebral vasculature, the circle of Willis, as well as collateral circulations, provide blood to the occluded areas. Posterior circulation is supplied by the vertebral arteries (VA), posterior inferior cerebellar arteries (PICA), basilar artery (BA), anterior inferior cerebellar arteries (AICA), pontine branches of the basilar artery, superior cerebellar arteries (SCA), PCA, and PCOM. The VAs arise from the subclavian arteries and fuse into the BA within the cranium. The BA typically divides into PCAs near the pituitary stalk at the pontomesencephalic junction. PCAs can originate from BA 70 percent of the time, 20 percent of the time from PCOMs, and 10 percent of the time from a mix of the two. The PCAs then give off branches to the midbrain, subthalamic nucleus, basal nucleus, thalamus, temporal, occipital, and occipitoparietal cortices (See Figure).  PCA is divided into four segments, P1 to P4. The segments can be further categorized into deep and superficial segments or proximal and distal, respectively. P1 and P2 segments are deep segments. The P1 segment is between the termination of the BA and the PCOM. The thalamic-subthalamic arteries derived from the P1 segment supply the paramedian parts of the upper midbrain and thalamus. The tuberothalamic arteries usually arise from the PCOM and supply the anterior and anterolateral parts of the thalamus. Both sides of the thalamus and midbrain can be supplied by an Artery of Percheron (AOP), which is a rare anatomic variation. AOP arises from proximal P1. The other branches of the P2 segment include thalamogeniculate arteries and the posterior choroidal arteries. The thalamogeniculate arteries supply the ventrolateral part of the thalamus. The posterior choroidal arteries supply the lateral geniculate body, pulvinar, posterior thalamus, hippocampus, and parahippocampal gyrus. . P3 and P4 are superficial segments. The P3 segment is the quadrigeminal segment. Anterior and posterior inferior temporal arteries arise from the P3 segment. The P4 segment is the cortical segment within the calcarine fissure and became the calcarine artery. Abstract

Intracranial hypertension (IH) is a clinical condition that is associated with an elevation of the pressures within the cranium. The pressure in the cranial vault is measured in millimeters of mercury (mm Hg) and is normally less than 20 mm Hg. The cranium is a rigid structure that contains three main components: brain, cerebrospinal fluid, and blood. Any increase in the volume of its contents will increase the pressure within the cranial vault. The Monroe-Kellie Doctrine states that the contents of the cranium are in a state of constant volume. That is, the total volumes of the brain tissues, cerebrospinal fluid (CSF), and intracranial blood are fixed. An increase in the volume of one component will result in a decrease in volume in one or two of the other components. The clinical implication of the change in volume of the component is a decrease in cerebral blood flow or herniation of the brain. CSF is a clear fluid found in the subarachnoid spaces and ventricles that cushions the brain and spinal cord. It is secreted by the choroid plexus in the lateral ventricles, travels to the third ventricle via the foramen of Monroe. From the third ventricle, CSF reaches the fourth ventricle through the aqueduct of Sylvius. 34. Lacunar Syndromes

Abstract

Lacunar syndromes are clinical manifestations of lacunar infarctions. Lacunar infarctions are defined as small subcortical lesions with a size of less than 15 mm in diameter caused by occlusion of a penetrating artery from a large cerebral artery, most commonly from the Circle of Willis. These penetrating arteries arise at sharp angles from major vessels and are thus, anatomically prone to constriction and occlusion. Other common sources of these penetrating arteries include the middle cerebral artery and the basilar artery. The term “lacune” was first described in the late 19th and early 20th century based on pathologic analysis and adapted after imaging technology confirmed the initial hypothesis in the 21st century. A lacune usually describes a small, chronic cavity that represents the healed phase of lacunar infarction. However, there have been cases reported in which a lacune results from a larger infarct or intracerebral hemorrhage. The anatomic distribution of lacunar syndromes and infarctions is most commonly the basal ganglia (globus pallidus, putamen, thalamus, and caudate), the pons, and the subcortical white matter structures (internal capsule and corona radiate). These anatomical sites correspond to lesions at the lenticulostriate arteries, the anterior choroidal artery, thalamoperforant arteries, paramedian branches of the basilar artery, and the recurrent artery of Heubner from the anterior cerebral artery. Lesions at these specific sites account for many of the symptoms of lacunar syndromes. 35. Basilar Artery Thrombosis

Abstract

The basilar artery is a vital vessel contributing to the posterior cerebral circulation. It is formed at the junction of the pons and medulla by the convergence of the dual vertebral arteries. The vertebral arteries join the basilar artery to form the vertebrobasilar system, which supplies blood to the posterior portion of the circle of Willis. Its crucial purpose is to serve the cerebellum, brainstem, thalamus, occipital, and medial temporal lobes of the brain with oxygen-rich blood. With an average diameter of 3 to 4 millimeters, it furnishes 20 paramedian and circumflex perforating arteries thus providing a robust collateral network to the pons and midbrain. Abstract

Central vertigo is a clinical condition in which an individual experiences hallucinations of motion of their surroundings, or a sensation of spinning, while remaining still, as a result of dysfunction of the vestibular structures in the central nervous system (CNS). The patient complains typically of dizziness with hallucination or sense of spinning. This is different from the light-headed "dizziness" which is more commonly secondary to global impairment of cerebral perfusion. The peripheral vestibular system consists of the saccule, utricle, and semicircular canals. The neuroepithelial hair cells within the peripheral vestibular apparatus send projections to the vestibular nuclei in the caudal pons and rostral dorsolateral medulla by way of the vestibular division of the VIIIth cranial (vestibulocochlear nerve). The vestibular nucleus on each side is divided into 4 sub-nuclei with three lateral nuclei and one medial nuclear column, they are labeled as the superior vestibular nucleus, lateral vestibular nucleus, medial vestibular nucleus, and descending vestibular nucleus. Some nuclei receive only primary vestibular afferents, but most receive afferents from the cerebellum, reticular formation, spinal cord, and contralateral vestibular nuclei. The projections from the vestibular nuclei extend to the cerebellum, extraocular nuclei, and spinal cord. With these neuroanatomic arrangements, it will be easy to understand the functions of the vestibular system i.e. maintaining visual fixation through the vestibulo-ocular reflex with changing head and body positions in space and extended or erect body posture. The maintenance of visual fixation also requires the normal function of the oculomotor central neural integrator which consists mainly of the medial vestibular nucleus and the nucleus prepositus hypoglossi. Cerebral venous thrombosis (CVT), which includes thrombosis of the cerebral veins and the dural sinuses, is a rare disorder that can lead to significant morbidity and mortality. Cerebral venous thrombosis can present with variable signs and symptoms that include a headache, benign intracranial hypertension, subarachnoid hemorrhage, focal neurological deficit, seizures, unexplained altered sensorium, and meningoencephalitis. The diversity of risk factors and variable presentation present challenges in diagnosing cerebral vein thrombosis. Delay in diagnosis is common, as the median delay from symptom onset to hospital admission is four days and from symptom onset to diagnosis is seven days. Abstract

Cerebrospinal fluid (CSF) has essential biochemical and mechanical functions. CSF is produced at a rate of around 500 ml/day; there are estimates that there is approximately 125 mL to 150 mL of CSF in the body at any given time. Depending on the rate of production and absorption (which varies individually), the supply of CSF can be replaced about every 7.5 hours. Most of this fluid is produced in the ventricles of the brain by the choroid plexus—however, the ependymal cells, which line the ventricles, produce a smaller portion. The choroid plexus arises from the walls of the lateral ventricles and the roofs of the third and fourth ventricles.  After production, CSF circulates through the ventricles, subarachnoid space, and parenchyma and is then ultimately reabsorbed into the venous circulation. It is reabsorbed into the blood through structures in the arachnoid mater called arachnoid villi (arachnoid granulations). Importantly, this fluid can be examined clinically through a lumbar puncture. A build-up of plaque in the carotid artery can lead to atherosclerosis and stenosis of the artery, which may or may not be symptomatic clinically. Such carotid artery disease increases a person’s risk for cerebrovascular disease and stroke. Carotid endarterectomy (CEA) is a surgery performed to decrease the risk of stroke in a patient with known cerebrovascular atherosclerotic disease. The procedure entails removing plaque from the common carotid artery and/or internal carotid artery to improve blood flow and remove potential embolic material, restoring more normal cerebral blood flow. Abstract

Wallenberg syndrome was first described in 1808 by Gaspard Vieusseux. However, a more detailed description given in 1895 by Adolf Wallenberg identified this condition as infarction of the lateral medulla oblongata following occlusion of the vertebral artery (VA) or Posterior inferior cerebellar artery (PICA). Wallenberg syndrome is also called lateral medullary or posterior inferior cerebellar artery syndrome. This neurological disorder is associated with various symptoms that result from damage to the lateral segment of the medulla posterior to the inferior olivary nucleus. Anatomy The posterior inferior cerebellar artery (PICA) usually originates from the vertebral artery at an average distance of approximately 16 or 17 mm below the vertebrobasilar junction. The vertebral artery, arising from the subclavian artery, is classically divided into four segments. The fourth segment of the vertebral artery gives off the PICA, the largest branch of the vertebral artery. The trunk of PICA is divided into five segments: 1. The anterior medullary segment begins at the origin of the PICA. It ends at the level of a rostrocaudal line that passes through the most prominent part of the inferior olive. 2. The lateral medullary segment extends from the level of the most prominent point of the olive to the level of the origin of the glossopharyngeal (CN IX), vagus (CN X), and accessory (CN XI) rootlets. 3. The tonsillomedullary segment begins where the PICA passes posterior to the CN IX-XI and ends at the midpoint of the PICA's ascent toward the roof of the fourth ventricle along the medial surface of the tonsil. 4. The telovelotonsillar segment begins where the PICA ascends to the mid-level of the medial surface of the tonsil and ends where the artery exits the fissures between the tonsil, vermis, and hemisphere to reach the suboccipital surface. 5. The cortical segment begins where the PICA leaves the groove between the vermis, tonsil, and hemisphere and includes the terminal cortical branches. The trunk of PICA gives rise to perforating, choroidal, and cortical branches. The PICA supplies the medulla, the choroid plexus, the tela choroidea of the fourth ventricle, the tonsils, the inferior vermis, and the inferior aspects of the cerebellar hemispheres. Furthermore, the PICA mostly supplies the choroid plexus on the roof and the median opening of the fourth ventricle and provides the majority of branches of the choroid plexus., (Copyright © 2022, StatPearls Publishing LLC.)

The autonomic nervous system (ANS) consists of general visceral efferent (GVE) fibers that create a motor response due to general visceral afferent (GVA) fiber stimulation. Although general visceral afferent fibers are part of the ANS, they are not classified as part of the sympathetic or parasympathetic system. However, these visceral sensory nerves often colocalize within sympathetic and parasympathetic nerves.[1] GVA fibers carry sensory impulses from internal organs to the central nervous system (CNS). Managing the core body temperature is essential to survival because enzymes do not operate optimally under temperatures outside a strict range. The naked human body prefers an ambient temperature of 20 to 25 degrees C. However, the temperature of the surroundings can vary widely, and yet the core body temperature can remain between 36.1 to -37.2 degrees C. Abstract

In the simplest terms, therapeutic exercise involves movement prescribed to correct impairments, restore muscular and skeletal function and/or maintain a state of well-being. The many benefits of exercise are too numerous to count in any one article, though therapeutic exercise involves a cavalcade of benefits for the restoration of function, quality of life, and overall health. Many people exist at a bare minimum of physical ability in terms of their daily lives and need only a minor illness to reduce function significantly. Exercise as a therapeutic approach involves repeated, regular activity in different modalities to increase a person's resistance to illness and length of recovery after such an illness. This article will explore important aspects of therapeutic exercise as pertaining to clinicians for the maintenance of health. The goals of rehabilitation are to restore function in the setting that further deterioration is unlikely, but there are also benefits when deterioration is inevitable (such as in the palliative care setting).
