Introduction: The aim of present study was to estimate effective dose from most common procedures performed in nuclear medicine departments of Golestan province. Methods: Data of nuclear medicine procedures performed in 2 nuclear medicine departments in Golestan province were collected during 4 years. Effective dose, collective effective dose and effective dose per examination were calculated using standard dosimetry tables. Results: Based on the data of this study, results of 10437 nuclear medicine procedures performed during 4 years have lead to 3.97 mSv as average effective dose per examination and 10.37 human-Sv as mean collective effective dose. It was also revealed that Tc-99m was the main source of effective dose (98.3%), bone scan was the most common procedure (25.9%) and cardiac scan (MIBI-rest) has the highest collective effective dose (33.5%) during 4 years. Conclusion: Beside the cardiac scan which was the most common nuclear medicine procedure and the main contributor of effective dose in patients, due to geographical condition of the northeast of Iran, bone scan was the highest performed nuclear medicine examination in the Golestan province. (186)Re-1-hydroxyethylidene-1,1-diphosphonate (HEDP) is an attractive radiopharmaceutical for the treatment of bone pain arising from skeletal metastatic lesions. Currently, (186)Re-HEDP is most commonly used in European countries. The aim of this study was to investigate the palliative efficacy and adverse effects of (186)Re-HEDP in patients with different types of cancers and skeletal bone pain. METHODS: Nineteen (8 male, 11 female) patients with various cancers (breast, prostate, renal cell carcinoma, colon, and neuroendocrine tumors) and painful bone metastases were included in the study. A dose of 1,480-3,330 MBq (40-90 mCi) of (186)Re-HEDP was administered intravenously. The patients' level of pain relief was assessed by the Visual Analog Scale for 8 wk after treatment and by a weekly blood cell count to evaluate for hematologic toxicity. RESULTS: The overall response rate was 89.5%, and the mean pain score assessed by the Visual Analog Scale was reduced from 9.1 to 5.3 after 1 wk (P = 0.003). No adverse effects were reported by patients during intravenous administration or for up to 24 h after administration. A flare reaction was seen in 63.2% of patients, mainly during days 1-3, and lasted for 2-4 d. There was no significant correlation between the response to therapy and the flare reactions (P > 0.05). The nadir of platelet reduction occurred at the fourth or fifth week and led to platelet infusion in only 4 patients with a low baseline platelet count and diffuse skeletal metastases. Bone marrow suppression occurred in patients receiving higher doses, but no clinical problems were seen except in 2 patients who required packed cell transfusion similar to their prior transfusions. Radioiodine (131I iodide) has long been a safe, effective and widely used treatment in the management of differentiated thyroid cancer (DTC). Concerns regarding stochastic radiogenic risks have led to regulatory criteria for the release from medical confinement of patients who receive such radionuclide therapy. Over a 6-y period, the external whole-body dose rates at 1 m from 562 DTC patients were measured with an ionisation chamber calibrated in microsieverts per hour out to 5-d post-administration. Patients were stratified into four administered activity groups: 3.7 GBq (36.8 %), 5.55 GBq (47.3 %), 7.4 GBq (12.8 %) and 9.25 GBq (3 %). Consistent with previously published data, the current study demonstrated that a bi-phasic model accurately described 131I-iodide kinetics up to at least 5-d post-administration in DTC patients, providing data that would be useful in formulating radiation safety guidelines for staff and other individuals coming into contact with such patients after treatment. Three days following administration of radioactive iodine in 38 patients, exposure rates such as dose rate close to the dress (D0), dose rate of dress at 1 m (D1), dose rate close to the bedsheet (B0), dose rate of bedsheet at 1 m (B1), dose rate of patient at 1 m (P1) and also sum of D1 and B1 (S1) were measured using a radiation detection survey metre with a calibrated ionisation chamber. The patient population consisted of 36 females and 2 males with mean age 38.92 ± 15.76 y. A considerable correlation was found between age and P1 (r = 0.37 and p = 0.02), and administered dose and P1 (r = 0.43 and p = 0.006). It is recommended that a document should be prepared for practitioners and also all individuals coming in contact with older patients are to be made aware of the contamination and radiation hazards.
