Abstract

Background Two earthquakes on 6 February 2023 destroyed 10 cities in Türkiye. We report our experience with pediatric victims during these catastrophes, with a focus on crush syndrome related–acute kidney injury (Crush-AKI) and death. Method Web-based software was prepared. Patient demographics, time under rubble (TUR), admission laboratory data, dialysis, and kidney and overall outcomes were recorded. Results A total of 903 injured children (median age 11.62 years) were evaluated. Mean TUR was 13 h (interquartile range 32.5, max 240 h). Thirty-one of 32 patients with a TUR of >120 h survived. The patient who was rescued after 10 days survived. Two-thirds of the patients were given 50 mEq/L sodium bicarbonate in 0.45% sodium chloride solution on admission day. Fifty-eight percent of patients were given intravenous fluid (IVF) at a volume of 2000–3000 mL/m2 body surface area (BSA), 40% at 3000–4000 mL/m2 BSA and only 2% at >4000 mL/m2 BSA. A total of 425 patients had surgeries, and 48 suffered from major bleeding. Amputations were recorded in 96 patients. Eighty-two and 66 patients required ventilator and inotropic support, respectively. Crush-AKI developed in 314 patients (36% of all patients). In all, 189 patients were dialyzed. Age >15 years, creatine phosphokinase (CK) ≥20 950 U/L, TUR ≥10 h and the first-day IVF volume <3000–4000 mL/m2 BSA were associated with Crush-AKI development. Twenty-two deaths were recorded, 20 of 22 occurring in patients with Crush-AKI and within the first 4 days of admission. All patients admitted after 7 days survived. Conclusions These are the most extensive pediatric kidney disaster data obtained after an earthquake. Serum CK level was significantly associated with Crush-AKI at the levels of >20 950 U/L, but not with death. Adolescent age and initial IVF of less than 3000–4000 mL/m2 BSA were also associated with Crush-AKI. Given that mildly injured victims can survive longer periods in the disaster field, we suggest uninterrupted rescue activity for at least 10 days. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: With the premise that the demanding educational process and associated factors could influence the work‐related quality of life (WRQoL) of dental students, this study aimed to investigate the influence of diverse sociodemographic and health‐related factors, clinical education status, and sleep quality on the WRQoL of dental students. Materials and Methods: A three‐part questionnaire was administered to a group of preclinical and clinical dental students studying in different classes. The first part of the questionnaire included various sociodemographic and health‐related questions. In the second part, the multidimensional 23‐item WRQoL scale, with established validity and reliability, was applied to evaluate the WRQoL of the students. The last part of the questionnaire included the Pittsburgh Sleep Quality Index, comprising 19 items. General linear models were created to predict subscale scores using sociodemographic and health‐related characteristics, as well as univariate significant independent variables. Results: It was observed that the majority of the participants (78.2%) had poor sleep quality. Being clinical dental students had a significant and positive effect on the stress at work domain of WRQoL (p =.006), while it had a significant and negative effect on the home‐work interface domain (p <.001). The decrease in sleep quality had a significant and negative effect on all domains of WRQoL (p <.05) except for the stress at work domain. Conclusions: Clinical education status, regular smoking, and sleep quality significantly affect different domains of dental students' WRQoL. Abstract

Stres, vücudun hem fiziksel hem de psikolojik sınırlarını tehdit eden bir durumdur. Strese bağlı sağlık sorunları birey yaşamında etkili bir yere sahip olmakla birlikte hayatı fizyolojik ve psikolojik olarak etkilemektedir. Artan kalp hızı, çarpıntı, sıcak basması, nefes darlığı, hipertansiyon, diyabet gibi çeşitli faktörler stresin belirtileridir. Stresin üstesinden gelebilmek için yapılan çabalar genel olarak stresle başa çıkma şeklinde kavramsallaştırılmıştır. Stresle başa çıkma, kişide kaygı veya stres kaynaklarını azalttığı gösterilen zihinsel ve davranışsal tepki süreçleridir. Birey yaşamında çeşitli etkenlere bağlı olarak stres gelişebildiği gibi eğitim ve okul gibi etkenlere bağlı olarak da stres görülebilmektedir. Eğitim masrafları, sınavlar, okul arkadaşları ve eğitimciler eğitim ve okul yaşamı stres kaynakları arasında yer almaktadır. Bireylerde eğitim düzeyinin stres ile başa çıkma ile değişim gösterip göstermediğinin belirlenmesi amacıyla gerçekleştirilen bu araştırmada cinsiyet, yaş ve eğitim düzeyi ile stresle başa çıkma düzeyleri arası ilişki incelenmiştir. Araştırmada birden fazla kavram arasındaki ilişki incelendiğinden nicel araştırma kapsamında yer alan ilişkisel tarama modeli kullanılmıştır. Araştırmaya İstanbul ili Beylikdüzü ilçesinde ikamet eden 18 yaşından büyük 465 gönüllü katılım göstermiştir. Araştırmada Stresle Başa Çıkma Ölçeği kullanılmış ve ölçeğin Cronbach's Alpha değeri 0,763 olarak belirlenmiştir. Elde edilen veriler veriler SPSS for Windows 22.0 paket programıyla analiz edilmiştir. Araştırmada bireylerde stres ile başa çıkmanın cinsiyet, yaş ve eğitim gibi değişkenlere göre farklılık gösterdiği belirlenmiştir.

Abstract

Background/aim: There is limited data on COVID-19 disease in children with kidney disease. We aimed to investigate the characteristics and prognosis of COVID-19 in pediatric nephrology patients in Turkey. Materials and methods: This was a national, multicenter, retrospective cohort study based on an online survey evaluating the data between 11th March 2020 and 11th March 2021 as an initial step of a detailed pediatric nephrology COVID-19 registry. Results: Two hundred and three patients (89 girls and 114 boys) were diagnosed with COVID-19. One-third of these patients (36.9%) were between 10-15 years old. Half of the patients were on kidney replacement therapy: kidney transplant (KTx) recipients (n = 56, 27.5%), patients receiving chronic hemodialysis (n = 33, 16.3%) and those on peritoneal dialysis (PD) (n = 18, 8.9%). Fifty-four (26.6%) children were asymptomatic. Eighty-two (40.3%) patients were hospitalized and 23 (28%) needed intensive care unit admission. Fifty-five percent of the patients were not treated, while the remaining was given favipiravir (20.7%), steroid (16.3%), and hydroxychloroquine (11.3%). Acute kidney injury developed in 19.5% of hospitalized patients. Five (2.4%) had MIS-C. Eighty-three percent of the patients were discharged without any apparent sequelae, while 7 (3.4%) died. One hundred and eight health care staff were infected during the study period. Conclusion: COVID-19 was most commonly seen in patients who underwent KTx and received HD. The combined immunosuppressive therapy and frequent exposure to the hospital setting may increase these patients' susceptibility. Abstract

BackgroundCongenital anomalies of the kidney and urinary tract (CAKUT) are the predominant cause for chronic kidney disease below 30 years of age. Many monogenic forms have been discovered mainly due to comprehensive genetic testing like exome sequencing (ES). However, disease-causing variants in known disease-associated genes still only explain a proportion of cases. Aim of this study was to unravel the underlying molecular mechanism of syndromic CAKUT in two multiplex families with presumed autosomal recessive inheritance.Methods and ResultsES in the index individuals revealed two different rare homozygous variants inFOXD2,a transcription factor not previously implicated in CAKUT in humans: a frameshift in family 1 and a missense variant in family 2 with family segregation patterns consistent with autosomal-recessive inheritance. CRISPR/Cas9-derivedFoxd2knock-out (KO) mice presented with bilateral dilated renal pelvis accompanied by renal papilla atrophy while extrarenal features included mandibular, ophthalmologic, and behavioral anomalies, recapitulating the phenotype of humans withFOXD2dysfunction. To study the pathomechanism ofFOXD2-dysfunction-mediated developmental renal defects, in a complementary approach, we generated CRISPR/Cas9-mediated KO ofFoxd2in ureteric-bud-induced mouse metanephric mesenchyme cells. Transcriptomic analyses revealed enrichment of numerous differentially expressed genes important in renal/urogenital development, includingPax2andWnt4as well as gene expression changes indicating a cell identity shift towards a stromal cell identity. Histology ofFoxd2KO mouse kidneys confirmed increased fibrosis. Further, GWAS data (genome-wide association studies) suggests thatFOXD2could play a role for maintenance of podocyte integrity during adulthood.ConclusionsIn summary, our data implicate thatFOXD2dysfunction is a very rare cause of autosomal recessive syndromic CAKUT and suggest disturbances of the PAX2-WNT4 cell signaling axis contribute to this phenotype.

Abstract

Objective: we aimed to evaluate the approaches of pediatric nephrologists in our country to the management of childhood hypertension. Methods: the pediatric nephrologists in our country were invited to fill out an online questionnaire including 24 questions. The answers were compared between those working in the field for <= 10 years (Group 1, n =74) and >10 years (Group 2, n = 62). Results: of 136 participants (M/F = 101/35), 52% were following a single guideline [31% Fourth Report of 2004, 17% European Society of Hypertension in 2016, and 52% American Academy of Pediatrics in 2017], which is more common in Group 1 (P =.035). The most commonly used guideline was American Academy of Pediatrics of 2017 and Group 2 used Fourth Report of 2004 more commonly (P =.042). The most common choice to diagnose hypertension was office + home + ambulatory blood pressure monitoring (59%). The frequency of screening for end-organ damage at first evaluation was 96%. The time to wait for the effect of lifestyle modifications was 3 months in 52%. The first choice medication was angiotensin-converting enzyme inhibitors (49%) or calcium-channel blockers (48%) in non-obese and angiotensin-converting enzyme inhibitors (74%) in obese children. Calcium-channel blockers were more commonly prescribed as the first choice in non-obese children in Group 1 (P =.035). The most accessible emergency drug was esmolol.
