Diabetes mellitus is a chronic disease which may lead to degenerative complications in long-term. Effective glycemic control can prevent the occurence of complications. The aim of this study was to evaluate the effect of repeated diabetes education programme given in long term on glucose and lipid metabolisms in type 2 diabetes mellitus patients under insulin treatment. One hundred thirty patients without chronic complications were retrospectively evaluated. Fifty three patients completed the education and therapy programmes (education group) while 77 only joined the therapy programme (control group). The age, gender, diabetes duration, baseline blood pressure (BP) and metabolic parameters of the patients in both groups were similar. A1C, fasting and post-prandial glucose levels and systolic BP values of the patients in the education group significantly decreased after the education while systolic BP value increased in the control group. In the education group HDL-chol levels increased from 40 ± 9 mg/dl to 49 ± 14 mg/dl at 6th month. Our study suggests that a comprehensive and frequently repeated diabetes education programme is effective in the achievement of metabolic control., Diabetes mellitus uzun dönemde dejeneratif komplikasyonlara yol açabilen kronik bir hastalıktır. Glisemik kontrol komplikasyonların gelişimini engelleyebilir. Bu çalışmada insülin kullanan tip 2 diabetes mellituslu hastalarda sık tekrarlı diyabet eğitiminin uzun dönemde glukoz ve lipid metabolizması üzerine etkisini değerlendirmeyi amaçladık. Kronik komplikasyonsuz 130 hasta retrospektif olarak incelendi. 53 hastanın eğitim ve tedavi programını tamamladığı (eğitim gurubu), 77'sinin sadece tedaviye devam ettiği saptandı (kontrol gurubu). Her iki guruptaki hastaların yaş, cinsiyet, diyabet süreleri, başlangıç kan basıncı (KB) ve metabolik değerleri benzerdi. Eğitim gurubundaki hastaların eğitim sonrası A1C, açlık ve tokluk kan şekeri ve sistolik KB değerleri anlamlı düşerken kontrol gurubunda sistolik KB değeri anlamlı arttı. Eğitim gurubunda HDL-kol düzeyleri 6. ayda 40 ± 9 mg/dl'den 49 ± 14 mg/dl'ye yükseldi. Çalışmamız kapsamlı ve sık tekrarlı bir diyabet eğitim programının metabolik kontrolün sağlanmasında etkili olduğunu düşündürmektedir.

We aimed to evaluate our therapy results in acromegalic patients for last ten years. Although 52 acromegalic patients admitted to our center, only 30 (M/F: 13/17) patients whose follow-up data were available enrolled into the study. Their mean age was 43 ± 38 (28-64) years, mean age of diagnosis was 39 ± 9 (26-60) years and mean follow-up period was 42 ± 38 (5-127) months. Basal or nadir postglucose GH level less than 2 ng/ml accepted as cure criteria. After diagnosis, all patients operated transsphenoidally in different medical centers by different surgeons and the cure rate of first operation was 33 % (10/30). The cure rates were 63 % and 15 % in patients with microadenomas (n=11) and macroadenomas (n=19) respectively. And the difference was statistically significant (pIn conclusion cure rate probability was found higher in patients with microadenomas after transsphenoidal operation while it was very low in patients with macroadenomas. Our low cure rates could be attributed to the fact that the operations have been performed by inexperienced surgeons. While cure probability decreases after the second operation, complication risk increases. Octreotide therapy, which could be used as an alternative choice to the surgery, revealed high success rates in microadenomas and macroadenomas., Akromegali tanılı hastaların 10 yıllık süre içinde merkezimizdeki tedavi sonuçlarını değerlendirmeyi amaçladık. Merkezimize başvuran toplam 52 akromegali hastası olmasına karşın klinik izlem bilgilerine ulaşılabilen 30 hasta (E/K: 13/17) çalışmaya alındı. Yaş ortalamaları 43 ± 10 (28-64) yıl, akromegali tanısı aldıklarındaki yaş ortalamaları 39 ± 9 (26-60) yıl ve ortalama izlem süreleri 42 ± 38 (5-127) ay idi. Kür olma kriteri serum GH seviyesinin bazal veya glukoz yüklemesi sonrası 2 ng/ml altına inmesi olarak kabul edildi. Tanı sonrası tüm hastaların değişik merkezlerde ve değişik cerrahlar tarafından transsfenoidal hipofizektomi operasyonu geçirdiği ve birinci operasyonda % 33 (10/30) oranında kür elde edildiği görüldü. Operasyon öncesi mikroadenomu olan hastalardaki (n=11) kür oranı % 63, makroadenomu olan hastalardaki (n=19) kür oranı % 15 idi. Aralarındaki fark istatistiksel olarak anlamlı bulundu (pSonuç olarak mikroadenomu olan hastalarda transsfenoidal operasyon sonrası kür olasılığı yüksek iken makroadenomlularda bu olasılık çok düşük bulundu. Bizim elde ettiğimiz kür oranlarının yayınlardan daha düşük olmasının nedeni operasyonların tecrübeli sayılabilecek cerrahlar tarafından yapılmamış olmasına bağlandı. İkinci operasyon sonrasında kür elde etme olasılığı çok daha azalırken, komplikasyon görülme olasılığının arttığı görüldü. Makroadenom ve mikroadenomlularda uygulanan oktreotid tedavisinin büyüme hormonu seviyesini güvenli sayılan seviyelere baskıladığı ve operasyona seçenek olabileceği saptandı.

Medullary thyroid carcinomas (MTC) constitute 5-10 % of all thyroid cancers. Total thyroidectomy and lymph node dissection are the proposed treatment options, while external radiotherapy, radionuclide treatment and chemotherapy are reported to be useful for palliative purposes. Noncurative chemotherapy regimens are generally used in progressive medullary thyroid carcinoma cases. We tried to evaluate the role of chemotherapy in the treatment of MTK using our chemotherapy results obtained from two different patients, with the diagnosis of sporadic MTK and MEN 2B MTK, to whom we had to use chemotherapy due to the rapid progression of their diseases., Medüller tiroid kanserleri (MTK) tüm tiroid kanserlerinin %5-10'unu oluşturmaktadır. MTK'lerinin tedavisinde total tiroidektomi ve lenf nodu diseksiyonu önerilmekte, eksternal radyoterapi, radyonüklid tedavi ve kemoterapinin palyatif amaçlı kullanılabileceği bildirilmektedir. Küratif olmayan kemoterapi rejimleri progresif MTK'ler için kullanılmaktadırlar. Hızlı progresyon nedeniyle kemoterapi uyguladığımız biri sporadik, diğeri MEN 2B MTK tanılı iki olgumuzdaki sonuçlarımızla kemoterapinin MTK'deki yerini gözden geçirmeyi amaçladık.
