Background/Aims: There is a paucity of evidence on how birth trauma can affect the mental health of fathers. This review aimed to determine the common psychological effects of witnessing traumatic events during birth for fathers. Methods: A narrative literature review of primary research and literature reviews was conducted using the British Nursing Index, CINAHL, Maternity and Infant Care, PubMed and EBSCO. A total of 6 research articles were included. Results: Data analysis generated three themes: toxic masculinity: the role of fathers in the birthing environment; unpreparedness: expectation vs reality; and relationships with partner and others. Conclusions: Traumatic events during labour and birth can have a profound impact on both mother and father. Fathers may experience poor mental health in the perinatal period, which can impact their relationship with their partner and the baby. It was noted that some healthcare professionals were more supportive than others. Effective communication is essential to quality care provision, which should help to manage poor paternal mental health in the perinatal period. Implications for practice: Services are needed to identify and address poor mental health in the perinatal period for both mothers and fathers. Antenatal education should include information about all potential outcomes during labour and birth. More healthcare professionals are needed with expertise in mental health, including the ability to assess and refer to specialist services where necessary. [ABSTRACT FROM AUTHOR]

There are several factors that influence women's childbirth experience, and personal interactions with health professionals are of particular significance. The main objective of this study was to present the validation of an abbreviated form of an existing questionnaire on attitudes about childbirth in medical and nursing students. We used a sample of 512 perinatal medicine and nursing students who received the original 52-item CAVE-st questionnaire to obtain a shorter version with proper psychometric properties. We used Cronbach's alpha coefficient to evaluate the new version's internal consistency. The Kaiser– Meyer–Olkin test and the Barlett sphericity test were performed to assess the suitability of exploratory factor analysis (EFA). Subsequently, confirmatory factor analysis (CFA) was performed using structural equation models in a second sample of 139 medical students. We obtained a 15-item version with a Cronbach's alpha of 0.82. The EFA revealed a four-dimensional structure, similar to the full 52-item version. In the CFA the adjustment indexes showed good model fitness, RMSEA= 0.046 [CI 0.00–0.07]; CFI = 0.978. We can conclude that the 15-item version is a valid tool for evaluating the attitude of students toward childbirth, pointing out the matters that should be improved in their training to avoid obstetric trauma by the promotion of a positive experience in women during childbirth. Aim: To determine the relationship between perceived satisfaction with childbirth and perceived autonomy in decision making during the childbirth process. Design: Cross-sectional study. Methods: The study took place within the international INTERSECT project. Data were collected in four hospitals in Slovakia. The research group consisted of 437 female respondents (average age 30.5 ± 4.8). Birth Satisfaction was measured by the Birth Satisfaction Scale – Revised (BSS-R). Level of autonomy in decision making during childbirth was measured by the Mothers’ Autonomy in Decision Making Scale (MADM). Results: Respondents who perceived a higher level of autonomy in decision making during childbirth perceived higher overall satisfaction with childbirth (p = 0.001, r = 0.416). Higher age of mothers was also associated with higher birth satisfaction (p = 0.004, r = 0.139). Almost 17% of respondents perceived their childbirth to be a traumatic event. Respondents described lower levels of satisfaction with childbirth (p = 0.001) and lower levels of autonomy in decision making during childbirth (p = 0.002) if they subjectively perceived their childbirth as traumatic. Significant associations for the BSS-R score were also demonstrated regarding parity (p = 0.001), mode of delivery (p = 0.001), and type of delivery (p = 0.001). Conclusion: The involvement of the mother in the decision-making process of her own birth is a fundamental aspect of healthcare during childbirth and overall satisfaction with the birth. Perinatal risk factors and a traumatic delivery experience can contribute to childbirth-related posttraumatic stress disorder (CB-PTSD). This study aimed to determine the prevalence of women who met the CB-PTSD diagnostic criteria in the postpartum period of 6 to 12 months and to examine the relationship between this prevalence, perceptions of traumatic birth experiences, and other associated factors. This online, cross-sectional, and analytical study included 402 Turkish women in the 6- to 12-month postpartum period between June 2021 and 2022 during the COVID-19 pandemic. The Personal Information Form, the Childbirth Perception Scale, and the City Birth Trauma Scale were used to collect data. Among the participants, 14.9% met the CB-PTSD diagnostic criteria according to DSM-5. Negative perceptions of childbirth, not working, perceptions of low socioeconomic status (SES), living in rural areas, a history of physical/verbal violence, stress during pregnancy, fear of childbirth, preterm delivery, the presence of intrapartum/postpartum maternal problems, and negative attitudes and behaviors of healthcare providers increased the risk of CB-PTSD, whereas early breastfeeding and skin-to-skin contact after birth decreased the risk (p < 0.05). Intrapartum negative perceptions of childbirth (odds ratio [OR]: 1.32; 95% confidence interval, 95% CI), low SES (OR: 0.42; 95% CI), and stress during pregnancy (OR: 2.94; 95% CI) are significant predictors of meeting the CB-PTSD diagnostic criteria. Healthcare providers engaged in perinatal care should provide women with proper care and counseling, and traumatic interventions should be avoided throughout the intrapartum period. Psychological birth trauma and childbirth-related posttraumatic stress disorder represent a substantial burden of disease with 6.6 million mothers and 1.7 million fathers or co-parents affected by childbirth-related posttraumatic stress disorder worldwide each year. There is mounting evidence to indicate that parents who develop childbirth-related posttraumatic stress disorder do so as a direct consequence of a traumatic childbirth experience. High-risk groups, such as those who experience preterm birth, stillbirth, or preeclampsia, have higher prevalence rates. The main risks include antenatal factors (eg, depression in pregnancy, fear of childbirth, poor health or complications in pregnancy, history of trauma or sexual abuse, or mental health problems), perinatal factors (eg, negative subjective birth experience, operative birth, obstetrical complications, and severe maternal morbidity, as well as maternal near misses, lack of support, dissociation), and postpartum factors (eg, depression, postpartum physical complications, and poor coping and stress). The link between birth events and childbirth-related posttraumatic stress disorder provides a valuable opportunity to prevent traumatic childbirths and childbirth-related posttraumatic stress disorder from occurring in the first place. Childbirth-related posttraumatic stress disorder is an extremely distressing mental disorder and has a substantial negative impact on those who give birth, fathers or co-parents, and, potentially, the whole family. Still, a traumatic childbirth experience and childbirth-related posttraumatic stress disorder remain largely unrecognized in maternity services and are not routinely screened for during pregnancy and the postpartum period. In fact, there are gaps in the evidence on how, when, and who to screen. Similarly, there is a lack of evidence on how best to treat those affected. Primary prevention efforts (eg, screening for antenatal risk factors, use of trauma-informed care) are aimed at preventing a traumatic childbirth experience and childbirth-related posttraumatic stress disorder in the first place by eliminating or reducing risk factors for childbirth-related posttraumatic stress disorder. Secondary prevention approaches (eg, trauma-focused psychological therapies, early psychological interventions) aim to identify those who have had a traumatic childbirth experience and to intervene to prevent the development of childbirth-related posttraumatic stress disorder. Tertiary prevention (eg, trauma-focused cognitive behavioural therapy and eye movement desensitization and reprocessing) seeks to ensure that people with childbirth-related posttraumatic stress disorder are identified and treated to recovery so that childbirth-related posttraumatic stress disorder does not become chronic. Adequate prevention, screening, and intervention could alleviate a considerable amount of suffering in affected families. In light of the available research on the impact of childbirth-related posttraumatic stress disorder on families, it is important to develop and evaluate assessment, prevention, and treatment interventions that target the birthing person, the couple dyad, the parent-infant dyad, and the family as a whole. Further research should focus on the inclusion of couples in different constellations and, more generally, on the inclusion of more diverse populations in diverse settings. The paucity of national and international policy guidance on the prevention, care, and treatment of psychological birth trauma and the lack of formal psychological birth trauma services and training, highlight the need to engage with service managers and policy makers. This study examines the relationship between birth memory and recall and the perception of traumatic birth in women who were a postpartum one-year period and the affecting factors. This descriptive and correlational study was conducted with 285 participants in the pediatric department of a state university medical school. Data were collected using a participant information form, Birth Memories and Recall Questionnaire, and Perception of Traumatic Childbirth Scale. In the study, it was determined that the women had a moderate level of birth memories and recall, and the rate of those with a "high" and "very high" perception of traumatic childbirth was 45.9%. According to path analysis, Birth Memories and Recall Questionnaire score and educational status (primary secondary school) have a positive and significant effect on the perception of traumatic birth. The perception of traumatic birth was a predictor that explained 17.3% of birth memories and recall. Nearly half of the study participants perceived the experience of giving birth as traumatic, and birth memories and recall were at a moderate level. Improving women's perception of education and traumatic birth will contribute to positive birth memories and to create positive emotions when they remember their birth. [ABSTRACT FROM AUTHOR]

Addressing psychological trauma after childbirth requires accurate measurement of its prevalence, risk factors, and outcomes using validated instruments that distinguish perceptions of traumatic birth, subclinical post-traumatic stress (PTS) symptoms, and symptoms meeting a diagnostic threshold. The purpose of this study was to review literature on psychological trauma following childbirth and appraise instruments that measure postpartum PTS. In January 2020, the authors searched for and evaluated peer-reviewed studies that quantitatively measured PTS following hospital-based live births in the United States, United Kingdom, Canada, Australia, Norway, Sweden, and Switzerland; 37 articles were selected and evaluated. Levels of post-traumatic stress disorder were most commonly measured, followed by PTS symptoms. Diagnostic instruments suggested lower PTS prevalence estimates than those screening for or assessing PTS symptoms. Community samples yielded lower prevalence estimates than samples recruited from the internet or settings specifically addressing mental health. Measurement sooner after birth yielded higher estimates. Study design, sample characteristics, instruments, and timing of measurements likely impact postpartum PTS prevalence estimates. Variation in these characteristics make it difficult to draw conclusions on the prevalence of postpartum PTS. Researchers should consider the appraisal of measurement tools presented here and use rigorous study methodology when studying traumatic birth experiences and evaluating interventions. Background: The research was carried out to examine the effect of a parenting preparation course given to midwifery students during an academic semester on the preferred mode of delivery, fear of childbirth, and traumatic birth perception. Methods: This was a quasi-experimental study with a pretest-posttest design that included 47 second-year students enrolled in the parenting preparation course. Students took the parenthood preparation course, 2 hours a week, for 14 weeks. Data were collected using a descriptive information form, the pre-pregnancy fear of birth scale, and the perception of traumatic birth scale. Paired t-test and chi-square test were used to evaluate intragroup and intergroup differences. The data were analyzed using SPSS 22.0 software at a statistical significance of 0.05. Results: The mean age of the students was 20.13±0.67 years. The mean score of pre-pregnancy fear of childbirth was 40.46±9.37 in the pretest and 23.61±6.79 in the posttest. In addition, the mean score of traumatic childbirth perception decreased from 77.34±25.15 in the pretest to 39.44±13.78 in the posttest. The number of students who preferred cesarean section decreased significantly, while the number of students who preferred vaginal delivery increased after the preparation course. Conclusion: Parenting preparation classes can contribute to the reduction of fear of childbirth and the perception of traumatic childbirth in women.

Plain English Summary This study shares information from qualitative interviews with Black women and women who are survivors of abuse regarding their choice to have a planned out-of-hospital birth, or to choose an unassisted birth (UAB) with no midwife or other professional birth attendant. Black women are less well represented among those who choose OOHB, and little is known about the reasons that they may choose OOHB. Previous studies show that women who have experienced childhood physical or sexual abuse may prioritize having a sense of control and autonomy during their birthing experiences; however, little is known about their specific choice for OOHB. Our study recruited 18 women who had an OOHB or UAB and who identified as either Black and/or survivors of trauma to participate in in-depth interviews concerning their choice to give birth out of hospital. Through qualitative research methods, we analyzed transcripts of these interviews and developed a theoretical model about women’s decision making related to OOHB or UAB. We found that women may choose OOHB or UAB because of a previous trauma, or because they feel discriminated against by healthcare professionals, either because of skin color, age, pregnancy, weight, or some other health condition, and that choosing OOHB or UAB allowed them to have more control during the process of giving birth. Understanding the role that previous trauma and experiences of discrimination play in birthplace choice may help birth professionals to consider bodily autonomy, physical and emotional safety, anti-racism, and independence as important factors in their clinical interactions with birthing women.

Framed by relational dialectics theory, the current investigation sought to understand women's meaning making processes in publicly shared stories about birth trauma. Contrapuntal analysis was used to identify culturally dominant systems of meaning embedded in women's talk about traumatic birth. Forty-one stories recounting traumatic births were analyzed within a variety of contexts (e.g., natural birth, cesarean, preemie). The dominant discourse of traumatic birth as incongruent with intensive motherhood informed much of women's communication about traumatic birth. Discursive interplay was also identified through the struggle between the discourse of traumatic birth as incongruent with intensive motherhood and the discourse of individualism. To describe expectant mothers' experiences of fear of childbirth after a previous traumatic birth. Qualitative, individual, in-depth interviews were conducted with eight expectant mothers between September and November 2020. Data were analysed using a descriptive phenomenological approach. We identified four interconnected constituents: Suboptimal midwifery care, loss of control and agency; insufficient time and capacity to process a traumatic birth experience; "The baby has to be delivered!", and finally, the path to a new childbirth with the hope of mastering fears. Findings reveal an association between a previous history of traumatic birth and a fear of childbirth in expectant mothers. The trauma they experienced whilst giving birth strongly impacted their subsequent pregnancy, making it difficult for them to deal with the new pregnancy and impending birth. Women who have experienced a traumatic birth need to have the opportunity to process the trauma. Routines must therefore be developed that identify, support and follow up with the women. If these women are identified and given the help they need, this may help prevent and/or alleviate fear of childbirth in their subsequent pregnancy. [ABSTRACT FROM AUTHOR]

Abstract Background High numbers of women experience a traumatic birth, which can lead to childbirth-related post-traumatic stress disorder (CB-PTSD) onset, and negative and pervasive impacts for women, infants, and families. Policies, suitable service provision, and training are needed to identify and treat psychological morbidity following a traumatic birth experience, but currently there is little insight into whether and what is provided in different contexts. The aim of this knowledge mapping exercise was to map policy, service and training provision for women following a traumatic birth experience in different European countries. Methods A survey was distributed as part of the COST Action “Perinatal mental health and birth-related trauma: Maximizing best practice and optimal outcomes”. Questions were designed to capture country level data; care provision (i.e., national policies or guidelines for the screening, treatment and/or prevention of a traumatic birth, service provision), and nationally mandated pre-registration and post-registration training for maternity professionals. Results Eighteen countries participated. Only one country (the Netherlands) had national policies regarding the screening, treatment, and prevention of a traumatic birth experience/CB-PTSD. Service provision was provided formally in six countries (33%), and informally in the majority (78%). In almost all countries (89%), women could be referred to specialist perinatal or mental health services. Services tended to be provided by midwives, although some multidisciplinary practice was apparent. Seven (39%) of the countries offered ‘a few hours’ professional/pre-registration training, but none offered nationally mandated post-registration training. Conclusions A traumatic birth experience is a key public health concern. Evidence highlights important gaps regarding formalized care provision and training for care providers.

Distressing or traumatic perinatal experiences and adverse health outcomes can lead to particularly challenging postpartum experiences for patients and families. By adopting a trauma-informed care approach, the health care team can provide much-needed support, prevent additional harm, and promote healing. We propose practical communication, behavioral, and procedural considerations for integrating trauma-informed care principles into routine postpartum care, with attention to populations that have been marginalized. Purpose To evaluate whether severe postpartum hemorrhage (PPH) is a risk factor for posttraumatic stress disorder (PTSD). Severe PPH can be experienced as a traumatic event. PTSD leads to negative mental health effects. Knowing risk factors for PTSD during childbirth offers opportunities for early interventions, which may prevent the development of PTSD. Materials and methods In this prospective study, we compared two groups of participants; women with ≥2000 mL of blood loss (severe PPH, patients) and women with ≤500 mL of blood loss (controls). Participants were screened for PTSD using the PCL-5 four to six weeks after delivery. Positive screening was followed by the CAPS-5 to diagnose PTSD. Results We included 187 PPH patients and 121 controls. Median PCL-5 scores were higher for PPH patients (5.0) than controls (4.0, p = 0.005). Thirteen PPH patients (7.0%) and two controls (1.7%) scored ≥32 on the PCL-5, indicative of probable PTSD (OR 4.45, 95% CI 0.99-20.06, p = 0.035). Significant more PPH patients than controls met criteria for a clinical diagnosis of PTSD on the CAPS-5 (n = 10, 5.6% vs n = 0, 0.0%; p = 0.007). Conclusions There is a significant and clinically relevant increased risk for developing PTSD after severe PPH. Gynecologists and midwives are advised to screen for PTSD at postpartum follow-up visits to prevent long-term negative mental health effects. Clinical Trial Registration NL50273.100.14

Background: About 1.6% of planned births in the United States occur out of hospitals. Studies indicate that planned out-of-hospital birth (OOHB) is safe and satisfying for women; however, there is great variation among ethnic groups, and Black women are underrepresented. A recent phenomenon is the choice to have an unassisted birth (UAB) with no midwife or other professional maternity care attendant. The purpose of this study is to fill a gap in understanding reasons for choosing OOHB or UAB for two clinically important sub-groups of women: Black women, and women who have experienced childhood physical or sexual abuse. Methods: This study recruited 18 women who had an OOHB or UAB and who identified as either Black or survivors of trauma to participate in in-depth qualitative interviews concerning their choice to give birth out of hospital. A grounded theory approach was utilized that involved a discursive process of data collection, coding textual passages to identify focused themes, memo writing to document analytic decision-making, and eventual conceptual modeling. Results: All 18 participants endorsed a history of trauma. Focused coding to identify inherent concepts led to the emergence of a theoretical model of the arc of decision-making around choice of place of birth and birth attendant, or lack thereof. Women may choose OOHB or UAB because of a previous trauma, or because they feel discriminated against by healthcare professionals, either because of skin color, age, pregnancy, weight, or some other health condition. Women may choose OOHB or UAB because it affords more control during the process of giving birth. Conclusion: Previous trauma and experiences of discrimination were influential factors for women in the study sample in their choice of birthplace setting and choice of provider. These findings can inform clinical understanding for birth professionals, including doctors, midwives, doulas, nurses, social workers, and psychologists, and contributes more broadly to the national conversation about birth choices in the USA. BackgroundThe subjective experience of giving birth to a child varies considerably depending on psychological, medical, situational, relational, and other individual characteristics. In turn, it may have an impact on postpartum maternal mental health and family relationships, such as mother–infant bonding. The objective of the study was to evaluate the relevance of the subjective birth experience (SBE) for mother–infant bonding difficulties (BD) in women with mental disorders.MethodsThis study used data from N = 141 mothers who were treated for postpartum mental disorders in the mother–baby day unit of the Psychosomatic University Clinic in Dresden, Germany. Patients' mental status at admission and discharge was routinely examined using a diagnostic interview (SCID I) and standard psychometric questionnaires (e.g., EPDS, BSI, PBQ). Both, the SBE (assessed by Salmon's Item List, SIL) as well as medical complications (MC) were assessed retrospectively by self-report. The predictive value of SBE, MC, as well as psychopathological symptoms for mother–infant BD were evaluated using logistic regression analyses.ResultsAbout half of this clinical sample (47.2%) reported a negative SBE; 56.8% of all mothers presented with severe mother–infant BD toward the baby. Mothers with BD showed not only significantly more depressiveness (EPDS: M = 16.6 ± 5.6 vs. 14.4 ± 6.2*), anxiety (STAI: M = 57.2 ± 10.6 vs. 51.4 ± 10.6***), and general psychopathology (BSI-GSI: M = 1.4 ± 0.7 vs. 1.1 ± 0.6**) compared to women without BD, but also a significantly more negative SBE (SIL: M = 79.3 ± 16.2 vs. 61.3 ± 22.9***). Moreover, the SBE was the most powerful predictor for BD in univariate and multiple logistic regression analyses [OR = 0.96*** (95% CI 0.94–0.98) vs. OR = 0.96** (95% CI 0.93–0.98)], even when univariate significant predictors (e.g., current psychopathology and MC during birth) were controlled.ConclusionsA negative SBE is strongly associated with mother–infant bonding in patients with postpartum mental disorders. It needs to get targeted within postpartum treatment, preferably in settings including both mother and child, to improve distorted mother–infant bonding processes and prevent long-term risks for the newborn. Furthermore, the results highlight the importance of focusing on the specific needs of vulnerable women prior to and during birth (e.g., emotional safety, good communication, and support) as well as individual factors that might be predictive for a negative SBE.

Purpose: To evaluate whether severe postpartum hemorrhage (PPH) is a risk factor for posttraumatic stress disorder (PTSD). Severe PPH can be experienced as a traumatic event. PTSD leads to negative mental health effects. Knowing risk factors for PTSD during childbirth offers opportunities for early interventions, which may prevent the development of PTSD.Materials and Methods: In this prospective study, we compared two groups of participants; women with ≥2000 mL of blood loss (severe PPH, patients) and women with ≤500 mL of blood loss (controls). Participants were screened for PTSD using the PCL-5 four to six weeks after delivery. Positive screening was followed by the CAPS-5 to diagnose PTSD.Results: We included 187 PPH patients and 121 controls. Median PCL-5 scores were higher for PPH patients (5.0) than controls (4.0, p = 0.005). Thirteen PPH patients (7.0%) and two controls (1.7%) scored ≥32 on the PCL-5, indicative of probable PTSD (OR 4.45, 95% CI 0.99-20.06, p = 0.035). Significant more PPH patients than controls met criteria for a clinical diagnosis of PTSD on the CAPS-5 (n = 10, 5.6% vs n = 0, 0.0%; p = 0.007).Conclusions: There is a significant and clinically relevant increased risk for developing PTSD after severe PPH. Gynecologists and midwives are advised to screen for PTSD at postpartum follow-up visits to prevent long-term negative mental health effects.Clinical Trial Registration: NL50273.100.14. Background: High numbers of women experience a traumatic birth, which can lead to childbirth-related post-traumatic stress disorder (CB-PTSD) onset, and negative and pervasive impacts for women, infants, and families. Policies, suitable service provision, and training are needed to identify and treat psychological morbidity following a traumatic birth experience, but currently there is little insight into whether and what is provided in different contexts. The aim of this knowledge mapping exercise was to map policy, service and training provision for women following a traumatic birth experience in different European countries.Methods: A survey was distributed as part of the COST Action "Perinatal mental health and birth-related trauma: Maximizing best practice and optimal outcomes". Questions were designed to capture country level data; care provision (i.e., national policies or guidelines for the screening, treatment and/or prevention of a traumatic birth, service provision), and nationally mandated pre-registration and post-registration training for maternity professionals.Results: Eighteen countries participated. Only one country (the Netherlands) had national policies regarding the screening, treatment, and prevention of a traumatic birth experience/CB-PTSD. Service provision was provided formally in six countries (33%), and informally in the majority (78%). In almost all countries (89%), women could be referred to specialist perinatal or mental health services. Services tended to be provided by midwives, although some multidisciplinary practice was apparent. Seven (39%) of the countries offered 'a few hours' professional/pre-registration training, but none offered nationally mandated post-registration training.Conclusions: A traumatic birth experience is a key public health concern. Traumatic birth is the occurrence of a situation that will threaten the baby to be born or the physical integrity of the woman at any stage of the fertility process or the act of giving birth, or the perception of it by the mother in this way. Women who have had a traumatic birth experience, describe the moment of birth; helplessness, the feeling of losing control, a strong fear and horror moment. In addition, these women may show posttraumatic stress symptoms such as frequent recollection of the event, having nightmares about the event, and re-experiencing the event. Mothers' perceptions of labor, their perspectives on the birth process and the events they experience in this process, and the meanings they attribute to them may differ. From this point of view, it is important to identify risky women in preconceptional and prenatal periods so that birth, which should be a normal, healthy, physiological and natural process in women's lives, is not perceived as traumatic. Risky women who are in interconceptional and postnatal follow-ups or in delivery rooms and obstetric services need to be diagnosed early and receive qualified care. Abstract Background Traumatic birth experience has undesirable effects on the life of the mother, child, family, and society. The identification of predictive factors can be useful in improving birth experiences among women. This study aimed to assess the prevalence of a traumatic birth experience and identify its predictors among a group primiparous women. Methods A cross-sectional study was conducted among 64 health centres in Tabriz, the second largest city in Iran. Cluster sampling was used to recruit 800 eligible women at one to 4 months postpartum. The Persian version of the Childbirth Experience Questionnaire was used to measure the womens’ birth experiences. Data were collected through face to face interviews and analysed mainly by multivariable logistic regression. Results The prevalence of traumatic birth experience was 37% in the study group. The independent predictors of the traumatic birth experience were related to antenatal and intrapartum factors. The antenatal predictor was the lack of exercise during pregnancy (OR = 2.81, CI 1.40–5.63, P = .003) and the intrapartum predictors were the absence of pain relief during labour and birth (OR = 4.24, CI 2.12–8.50, P

INTRODUCTION Midwives experiencing traumatic births are emotionally affected by this process, lose their self-confidence, and may intend to leave the profession. This study aims to carry out a meta-synthesis of current qualitative research exploring the experiences of midwives witnessing traumatic births. METHODS The meta-synthesis consisted of 18 full-text studies in English, obtained from PubMed, Scopus, Web of Sciences, Cumulative Index of Nursing and Allied Health Literature (CINAHL), EMBASE, and PsycINFO databases. The results of the studies were analyzed using the thematic analysis technique. The study includes qualitative, mixedmethod, and full-text studies published between 2000 and 2020 that explored the experiences of midwives and obstetric nurses witnessing birth trauma. RESULTS The thematic analysis identified seven themes: post-traumatic feelings, posttraumatic stress symptoms, the impact of trauma on professional values, social support, learning from experience, legal process, and reflection of emotions of women experiencing traumatic birth on the midwife. CONCLUSIONS Midwives who witnessed traumatic birth were mostly emotionally affected. They lost their self-confidence and intended to leave their profession. They emphasized the importance of peer support through which they could share their experiences after trauma. Psychological education should be provided to midwives who witness the trauma by specialists, and midwives should be strengthened against the effects of trauma in terms of both the institutional policies where the birth takes place and midwifery-specific legal policies. Fear of childbirth (FOC) may affect family planning in lesbian, bisexual and transgender (LBT) couples with two potential carriers of a pregnancy. FOC has previously been researched in heterosexual women, while experiences of LBT people have remained unattended. The choice of birth-giving partner in same-sex couples has gained some attention in previous research, but the potential complexities of the decision have not been studied. The aim is to explore how LBT people negotiate the question of who gives birth, in couples with two potential birth parents, and where one or both partners have a pronounced FOC. Seventeen self-identified LBT people were interviewed about their expectancies and experiences of pregnancy and childbirth. Data were analysed following a six-step thematic analysis. FOC was negotiated as one of many aspects that contributed to the decision of who would be the birth-giving partner. Several participants decided to become pregnant despite their fears, due to a desire to be the genetic parent. Others negotiated with their partner about who was least vulnerable, which led some of them to become pregnant despite FOC. Still other participants decided to refrain from pregnancy, due to FOC, and were delighted that their partner would give birth. Several participants described their partner's birth-giving as a traumatic experience for them, sometimes also when the birth did not require any obstetric interventions. The partner's experience was in some cases not addressed in postnatal care. It is important that healthcare staff address both partners' prenatal expectancies and postnatal experiences. Background: The incidence of Placenta Accreta Spectrum (PAS) is increasing. This study explores women's personal experience of a pregnancy, birth and early postpartum period complicated by PAS.Methods: A qualitative study involving seven women (age range: 33-44 years) following a pregnancy in the previous two years complicated by PAS was conducted. Using an Interpretative Phenomenological Analysis approach data were collected and analysed in 2019 and 2020 through face-to-face interviews. Interviews were transcribed verbatim.Results: Women described their experience from diagnosis up to two years following the birth and focussed on the strategies they used to cope with an unexpected diagnosis of PAS. Six main themes emerged. In pregnancy, women were focussed on the diagnosis and managing family life from hospital "it's not until you are told", "sad but safe", "relying on others can be unbearable". In terms of the birth a 'fear of dying' related to self and baby was central. Postnatally key themes related to the ongoing consequences of the condition as "no one believed I could be feeling so much pain" and "it's not all over when you go home".Discussion: Women who have experienced a pregnancy complicated by PAS experience significant challenges associated with long term hospitalization, post-operative pain and many express feelings of being generally worried lasting many months after their pregnancy. Objective: The psychological and emotional impact of a traumatic birth experience on clinicians is well‐established. It is also known that emotions can generally influence decisions. However, it is not clear whether experiencing a birth trauma can affect the professional behaviour and decision‐making of clinicians. This study explores the impact of birth trauma on clinical decision‐making of midwives. Data Sources: Four databases (Medline, Scopus, CINAHL and ProQuest) were searched to identify English language studies published from 1990 to 2018. Due to the lack of studies with specific focus on clinical decision‐making after birth trauma, we defined two main domains for our literature search. To be included, studies had to focus on either traumatic birth experience or clinical decision‐making in midwifery. The findings of the two domains were then integrated. Study Selection: Of a total 2104 studies identified, 70 received full‐text screening with 40 included in the review. Twenty‐two articles were about traumatic birth events and 18 examined decision‐making in midwifery. Data Extraction: Information were extracted on each article's purpose, study design, data collection, participants, definitions of birth trauma and the context in which clinical decisions were made. Results: Thematic analysis was conducted. The impact of birth trauma on midwives could be categorized into the following themes: psychological issues; professional concerns; changes in practice and positive impact. Review of literature indicated that clinical decision‐making could be influenced through all these themes. Conclusion: Decision‐making can be impacted by the midwife's affective state related to previous experience of birth trauma. The continuum of impact may vary from increased defensiveness to increased personal and professional growth. Objective: Planning to promote the health of mothers in postpartum is important in all countries. This study aimed to determine the effectiveness of two counseling method on prevention of post-traumatic stress after childbirth. Methods: In this clinical trial, 193 of mothers who had experienced a traumatic birth were randomly assigned to three groups. Participants were assessed using IES_R questionnaire at 4–6 weeks and 3 months after delivery. Results: Debriefing and brief cognitive behavioral counseling (CBC) significantly improved the symptoms of postpartum traumatic stress disorder. After 3 months, CBC had a significant effect on the symptoms. Conclusion: Screening of traumatic childbirth, implementation of supportive care, and early counseling prior to the initiation of post-traumatic stress are recommended. Background: A significant number of women experience childbirth as traumatic. These experiences are often characterized by a loss of control coupled with a perceived lack of support and inadequate communication with health care providers. Little is known about the choices women make in subsequent pregnancy(s) and birth(s), or why they make these choices. This study aimed to understand these choices and explore the reasons behind them.Methods: A longitudinal grounded theory methods study involving nine women was conducted. Over half of the participants had a formal diagnosis of post-traumatic stress disorder (PTSD) and/or PND related to the previous birth. Interviews were carried out at three timepoints perinatally. These findings are from the first interviews at 12–20 weeks.Results: From the first days of pregnancy, this cohort of women were focused on concerns that this birth would be a repeated traumatic experience. The women were deliberately searching out and analyzing information about their choices in this pregnancy and birth, and making plans which had two aims; firstly to avoid a repeat of their previous birth experience and secondly to avoid a loss of control to other people during the birth. The women considered a range of birth choices, from elective cesareans to freebirth. Some women felt well supported by those around them, including care providers, partners, friends, and family. Others did not feel supported and were anticipating conflict in trying to assert their birth choices. Many early relationships with healthcare professionals were characterized by fear and mistrust.Discussion: If women who have previously experienced a traumatic birth become pregnant again, they have a strong desire to avoid a repeat experience and to feel in control of their birth choices. Access to robust information appears to help reduce uncertainty and arm women in their discussions with professionals. Similarly making plans and seeking to have them agreed with care providers at an early stage is used a way to reduce the risk of having a further traumatic experience. Implications for practice include supporting women in formulating and confirming pregnancy and birth plans at an early stage to reduce uncertainty and foster a sense of control.

Background: The unique communication challenges faced by parents of infants undergoing therapeutic hypothermia have not been well characterized.Purpose: To develop awareness of communication challenges experienced by families of infants treated with therapeutic hypothermia.Methods: Semistructured interviews were conducted in a group setting with parents matched into groups according to the severity of the infant's presenting encephalopathy. The interviews were transcribed and coded into principal and additional subthemes.Results: Thirty adults were interviewed including 15 mothers, 12 fathers, 2 grandmothers, and 1 grandfather. The 15 infants were between 2 and 24 months of age at the time of the interviews. The principal theme of communication included the following 3 subthemes; transparency, consistency, and delivery style. Parents reported a strong desire for improved early and transparent communication about therapeutic hypothermia, particularly during transfer from an outside hospital. Parents also reported a preference for consistent communication and highlighted parental touch of the hypothermic infant, obstetrical nurse-to-neonatal intensive care unit nurse communication, and parent and visitor presence in the infant's room as areas in need of greater communication consistency. Parents valued direct and compassionate communication styles that excluded medical jargon.Implications For Practice: All providers can implement recommendations for communication to parents of infants treated with therapeutic hypothermia by increasing transparency, developing greater consistency in the communication delivered, and employing a direct and compassionate style to improve the parental experience of therapeutic hypothermia.Implications For Research: Further investigation is needed into the specific challenges parents face with a lack of transparent communication prior to the transfer of an infant for therapeutic hypothermia. Objective: This review aimed to identify the impact of childbirthrelated post-traumatic stress disorder (PTSD) or symptoms (PTSS) on a couple's relationship. Background: Childbirth can be psychologically traumatic and can lead to PTSD. There is emerging evidence that experiencing a traumatic birth can affect the quality of the couple's relationship. This is an important issue because poor-quality relationships can impact on the well-being of partners, their parenting and the welfare of the infant. Methods: A systematic search was conducted of Amed, CENTRAL, Cinahl, Embase, Maternity and Infant Care, Medline, MITCognet, POPLINE, PsycARTICLES, PsycBITE, PsycINFO, Pubmed and Science Direct. Additionally, grey literature, citation and reference searches were conducted. Papers were eligible for inclusion if they reported qualitative data about parents who had experienced childbirth and measures of PTSD or PTSS and the relationship were taken. Analysis was conducted using meta-ethnography. Results: Seven studies were included in the meta-synthesis. Results showed that childbirth-related PTSD or PTSS can have a perceived impact on the couple's relationship and five themes were identified: negative emotions; lack of understanding and support; loss of intimacy; strain on the relationship; and strengthened relationships. A model of proposed interaction between these themes is presented. Conclusions: The impact of childbirth-related PTSD or PTSS on the couple's relationships is complex. As the quality of the couple relationship is important to family well-being, it is important that healthcare professionals are aware of the impact of experiencing psychologically traumatic childbirth as impetus for prevention and support. Witnessing and/or providing care during a traumatic birth increases vulnerability to secondary traumatic stress, with potential effects on mental health of and attrition among the midwifery workforce. With the global shortage of the midwifery workforce, retention and peak functioning are critical to the well-being of childbearing women and their infants. PubMed, Scopus, CINAHL, and PsycINFO databases were searched to identify studies that examined secondary traumatic stress in both midwives and midwifery students. Fifteen studies were located from eight different countries. Four of these studies examined secondary traumatic stress in midwifery students and 11 in practicing midwives. This article discusses the limited research on secondary traumatic stress among midwives and students who have attended or witnessed traumatic births. The vulnerability and potential workforce attrition of midwives and students experiencing secondary traumatic stress are highlighted. The development of a comprehensive body of literature is needed on prevalence, impact, clinical implications, and healing strategies addressing this workforce issue. [ABSTRACT FROM AUTHOR]

The aim of this qualitative secondary data analysis was to identify the metaphors women used to describe their anniversary of their traumatic births. The existing data set that was reanalyzed was from a phenomenological study of the anniversary of birth trauma. Metaphor identification procedure was the method used to identify 8 different metaphors from the women's descriptions that provided a rich source of insight into their yearly struggles. These metaphors characterized the anniversary of traumatic birth as a great pretender, a lottery, a trigger, a clock watcher, a giant rubber band, a guilt trip, a sea of sadness, and bottled up anger. Background: Post-traumatic stress disorder is a Trauma- and Stressor-Related Disorder resulting from exposure to an event that is considered as traumatic. It is recognized in relation to traumatic childbirth that both patient, partner and health-care provider can develop a post-traumatic stress disorder. The most important risk factors in women are depression during the pregnancy, fear of childbirth, severe pre-eclampsia, preterm premature rupture of membranes, and severe neonatal complications. The prevalence rate in women in Western countries is estimated between 1 and 3%. The prevalence in partners witnessing childbirth is unknown. Post-traumatic stress disorder in relation to severe post-partum haemorrhage, in either women or partners, has not been extensively researched yet. Methods/design: This is a prospective cohort study in a hospital setting, with the objective to evaluate whether women and their partners have a higher risk to develop a post-traumatic stress disorder or symptoms, following a severe post-partum hemorrhage of 2.0 l or more, compared to a control group. The primary outcome variable is diagnosis of post-traumatic stress disorder. Secondary outcome variables are post-traumatic symptoms according to the post-traumatic stress disorder criteria, psychiatric co-morbidities and seeking psychological help. A total of at least 130 women and 130 partners, must be included according to power calculations. Patients, partners and controls are selected in eight hospitals through complication registers. They are asked to complete a digital questionnaire four to six weeks after delivery, to screen for a post-traumatic stress disorder or symptoms. Participants with scores above cut off values are asked to participate in a telephone interview. For secondary outcomes, risk factors will be evaluated by multivariable analysis. Discussion: This study is designed to give insight into the frequency of post-traumatic stress disorder and post-traumatic stress symptoms following a severe post-partum hemorrhage, both in patients and their partners. We strive to minimize the non-response bias, a common problem in this type or research, through early and active participant recruitment. Trial registration: NL50273.100.14.

OBJETIVO: Esta revisão da literatura tem como objetivo verificar a prevalência e os fatores de risco do parto traumático e do transtorno de estresse pós-traumático (TEPT) relacionado com o parto. MÉTODOS: Foi realizada uma pesquisa nos bancos de dados PubMed e BIREME, com as expressões "traumatic birth", "traumatic delivery", "postpartum posttraumatic stress disorder", "childbirth", "stress disorder" e avaliados estudos de 1994 a 2009. RESULTADOS: Três estudos qualitativos e quatro quantitativos sobre o parto traumático mostram que sua prevalência varia de 21,4% a 34% e que a mulher apresenta, durante o trabalho de parto ou o parto, medo intenso de sua morte ou do bebê, além de impotência, desamparo e horror. O parto traumático está relacionado a partos dolorosos, com procedimentos obstétricos de urgência e com assistência inadequada da equipe de saúde. Quanto ao TEPT relacionado com o parto, foram encontrados um estudo qualitativo e doze quantitativos e sua prevalência variou de 1,3% a 5,9%. Mulheres que apresentaram sintomas dissociativos ou emoções negativas no parto, que tiveram eventos traumáticos prévios, depressão na gestação e que tiveram pouco suporte social e pouco apoio da equipe de saúde são as mais vulneráveis para TEPT pós-parto. CONCLUSÃO: O parto traumático, apesar de pouco conhecido, não é um evento raro e traz consequências negativas para a vida da mulher, podendo inclusive ser sucedido de TEPT. A equipe de saúde que assiste mulheres no periparto deve estar preparada para prevenir e identificar esses casos.OBJECTIVE: The objective of this review was to examine the prevalence and risk factors of traumatic birth and childbirth-related posttraumatic stress disorder. METHODS: A literature search was carried out on the PubMed and BIREME databases using the search strings "traumatic birth", "traumatic delivery", "postpartum posttraumatic stress disorder", "childbirth" and "stress disorder". The search encompassed articles on prevalence and risk factors of traumatic delivery and childbirth-related posttraumatic stress disorder published between 1994 and 2009. RESULTS: Three qualitative and four quantitative studies on traumatic delivery revealed a rate ranging from 21.4% to 34%. Traumatic delivery is defined when, during labor or delivery, the mother presents intense fear of her own death or that of her child, besides feelings of impotence, helplessness and horror. Traumatic deliver is associated with painful delivery, emergency obstetric procedures and inadequate care from the health team. With regard to post partum PTSD, one qualitative and twelve quantitative studies were found, reporting a prevalence of 1.3% to 5.9%. Women who presented dissociation symptoms or negative emotions during delivery, or a history of traumatic events, depression in pregnacy, poor social support and a perception of a staff less suportive proved more vulnerable to post partum PTSD. CONCLUSION: Although not well understood, traumatic delivery a relatively common event which negatively impacts women's lives and may be a precursor to post partum PTSD. Health teams charged with caring for women during the peripartum period should be aware of this condition to allow identification and prevention of cases.
