BackgroundEnamel is highly transparent at short wavelength infrared imaging (SWIR) wavelengths allowing the detection of dental decay without the need for ionizing radiation. The purpose of this study was to use SWIR imaging methods including cross polarization optical coherence tomography (CP-OCT), occlusal transillumination (SWIR-OT), proximal transillumination (SWIR-PT), and occlusal reflectance (SWIR-R) to image interproximal lesions in vivo and compare the sensitivity with radiography.MethodsParticipants (n = 30) aged 18-80 each with a radiopositive interproximal lesion scheduled for restoration were enrolled in the study. Studies have shown that the opposing proximal surfaces across the contact will likely also have lesions. SWIR images were acquired of the adjoining teeth at each contact with an interproximal lesion scheduled for restoration. Lesion presence and depth were assessed on each side of the contact for radiography and each SWIR imaging method. Purpose: To describe the clinical and demographic characteristics and associated factors leading to bilateral acute iris transillumination (BAIT) syndrome. Methods: A retrospective review of patients with BAIT syndrome was performed. Results: Thirty-five patients with a diagnosis of BAIT were identified. The median age at presentation was 53 years; 80% of the patients were female. Twenty-six patients (74%) had recent histories of systemic antibiotic treatment. Of those with such a history, 24 patients (92%) had been receiving moxifloxacin. Two patients within our cohort were prescribed moxifloxacin prophylactically prior to a systemic surgical procedure and had no evidence of systemic illness or recent viral illness. Conclusions: Our data support the notion that moxifloxacin might be associated with the onset of BAIT syndrome. Notably, within our cohort, two patients received moxifloxacin as surgical prophylaxis and subsequently developed BAIT syndrome. This could suggest a potential association between moxifloxacin and the onset of BAIT, though further studies are needed to confirm this finding. [ABSTRACT FROM AUTHOR]

Introduction: Serendipitous findings are findings that were initially unsought but nevertheless contribute to the development of the discipline. This article reviews eight serendipitous findings in oto-rhino-laryngology important to its advancement. Method: The following serendipitous findings are discussed: the accidental discovery of the laryngeal mirror and indirect laryngoscopy by Garcia (1854), the invention of direct oesophagoscopy by Kußmaul (circa 1868), Czermák's (1863) development of diaphanoscopy, the unintentional emergence of bronchography from a clinical error made by Weingartner (1914), adenotomy by Meyer (1869), the discovery of the causes of unbalance related to the vestibular nerve by Flourens (1830), Bárány's (1914) finding that the semi-circular canal reflex is involved in equilibrium, and the relationship between gastroesophageal reflux and middle-ear infections by Poelmans and Feenstra (2002). Discussion: Based on these case studies we conclude that serendipity, defined as the art of making an initially unsought find, does not always appear out of nowhere. Often the researcher is already wrestling with a problem for which the serendipitous finding provides a solution. Sometimes the serendipitous finding enables the application of a known solution to a new problem. And sometimes a serendipitous finding is not recognized as such or considered unimportant. Bilateral acute iris transillumination (BAIT) is a clinical condition characterized by acute bilateral loss of iris pigment epithelium and iris transillumination. Because of the similar presentation, it is commonly confused with anterior uveitis, making it an important differential diagnosis for this condition. We report the case of a 60-year-old male patient who presented all the characteristics of BAIT after undergoing systemic treatment with moxifloxacin and that had the intraocular pressure (IOP) successfully controlled with selective laser trabeculoplasty (SLT). An epididymal cyst (EC) is a benign lesion. It arises due to cystic degeneration of the epididymis. The torsion of it is extremely rare. The usual presentation of the cyst is asymptomatic scrotal swelling. The swelling has a lobulated surface and feels like a bunch of grapes. The huge EC is rare and moreover, it is associated with torsion-rarest presentation.

Benign lesions involving the floor of the mouth are a rare encounter. In this case, a middle-aged female presented to the outpatient department with a swelling involving the floor of the mouth which occupied enough space to displace the tongue. There was difficulty in mastication, and on examination, it was brilliantly transilluminant. After sonography, the patient underwent an excision of the mass which on histopathology surprisingly was revealed as a lipoma. Having an idea about the complicated nearby anatomy, it is imperative to dissect accurately to prevent morbidity arising out of the complications associated with damage to the various structures.

Technological improvements have introduced significant innovations in dentistry and broadened the array of tools and techniques in dental care. One technological development that has been widely researched over the past 20 years is the use of Near-Infrared Transillumination (NIRT) imaging for the diagnosis of dental caries. This paper aims to introduce a comprehensive collection of NIRT images, intended as a reference tool for routine dental examinations, dental research, pedagogical activities, and forensic odontology. The collection presents pairwise clinical and NIRT images categorized as follows: (a) healthy teeth, (b) carious teeth, (c) restored teeth, (d) enamel defects, and (e) diverse findings. Background: The long-term safety and efficacy of repeated applications of subliminal transscleral cyclophotocoagulation (SL-TSCPC) with a focus on cumulative energy was evaluated in glaucoma patients. Methods: In this retrospective, multicentric study the data of a total of 82 eyes with various causes of glaucoma that were treated with a single or multiple applications of SL-TSCPC were collected. Treatments were performed under general or local anesthesia with an 810 nm diode laser. Power was 2000 mW; duty cycle, 31.3%; total treatment duration, 80–320 s; equaling a total energy of 50–200 J per treatment session. Fifty-five eyes (55 patients) presented for all follow-ups, and these eyes were selected for further statistical analysis. The mean age was 60.0 ± 17.1 years, and 22 (40%) of the patients were female. Intraocular pressure (IOP) and dependence on further glaucoma medication were evaluated at 12 months following the initial treatment. Results: Eyes underwent 1 or 2 consecutive SL-TSCPC treatments. Median (min–max) baseline IOP of 34 (13–69) decreased to 21.5 (7–61), 22 (8–68), 20 (9–68), and 19.5 (3–60) mmHg at the 1, 3, 6, and 12-month postoperative timepoints respectively. The mean (± SD) IOP decrease at 12 months was 26 ± 27%, 39 ± 32%, and 49 ± 33% in the low (below 120 J, n = 18), medium (120–200 J, n = 24), and high (above 200 J, n = 13) cumulative energy groups respectively. At the 12-month timepoint, oral carbonic anhydrase use was discontinued in ¾ of the cases. Conclusions: It was found that the repeated application of SL-TSCPC safely and efficiently decreases IOP in a Caucasian population with heterogenous causes of glaucoma, eyes with silicone oil responded to a greater extent. The intravenous (IV) injection procedure can be a challenging task, especially for individuals with thin veins, obesity, or patients with damaged and pigmented skin. Therefore, the IV procedure necessitates a portable medical device that can be used for academic demonstrations to train medical students or by health care professionals to perform venipuncture. Vein visualization with a vein detector is principally based on the interaction of blood components with wavelengths of the electromagnetic spectrum (EMS). In this paper, we first present the process of image formation in the spectral band of the near‐infrared region (NIR) of EMS. Then, we introduce the image acquisition system with image processing to extract the veins as a noninvasive vein detection method. A Raspberry Pi (Model 4B), along with a night vision camera, serves as an image acquisition tool to capture skin area illuminated by NIR. Following this, the data is transferred to the laptop where it can be filtered and processed using Python image processing tools before being viewed on the monitor. The results achieved through the device are quite encouraging, as the image recognition between veins and adjacent tissues from the skin sample can be clearly marked. The functionality, accuracy, and simplicity associated with this vein detection system make it a potential device for IV placement and the morphological study of disease detection. This systemic review focuses on the most recent trend in intrascleral intraocular lens (IOL) implantation in transconjunctival era, while also exploring the surgical issues associated with this method in diverse circumstances. Essentially, complicated cataract surgery, trauma, and/or genetic disorders may be accompanied by impairment of zonular and/or capsular support, for which several secondary IOL implantation options exist, including anterior chamber, iris-fixated, and scleral-fixated IOLs. Since the 1980s, when intrascleral IOL fixation made its debut, surgical techniques for implanting scleral-fixated IOLs have undergone multiple changes. Indeed, the efficacy of different surgical methods varies considerably and is greatly determined by the patient's prior ocular conditions as well as the surgeon's skill. Despite advances in surgical methods, studies are constrained by their retrospective nature, small sample sizes, and, most importantly, brief follow-up periods. Furthermore, the range of different procedures available, aside from transconjuctival intrasclear IOL fixation methods, suggests that the optimal approach to secondary IOL implantation in the absence of zonular or capsular support has yet to be determined. Background: Arterial punctures are a common procedure used to obtain blood samples for lab tests that guide treatment of neonatal patients. These punctures can be painful and have associated risks, emphasizing the importance of keeping attempts to a minimum. Purpose: The purpose of this quality improvement initiative was to determine whether using transillumination to visualize arteries before and while obtaining a blood sample would improve outcomes in neonatal patients. Methods: Quantitative data were collected pre- and postimplementation of education for the use of a transillumination device for arterial punctures on neonates in a level III neonatal intensive care unit (NICU). Outcomes followed included the success rate, average time to obtain a blood sample, and the average number of sticks to obtain a blood sample. Data were collected on 47 blood draw attempts in the control group (preimplementation) and 19 blood draw attempts in the experimental group (postimplementation). Results: A statistical analysis concluded that the use of the studied transillumination device does positively affect the incidence of successful blood draws, particularly for the novice NICU nurse. In addition, the average number of sticks to obtain a blood sample significantly decreased in the experimental group. Finally, the average time to obtain a blood sample using the device was not impacted in this study. Implications for Practice and Research: Use of a transillumination device does appear to result in improved outcomes for arterial punctures in neonatal patients. The aim of this clinical study was to compare the diagnostic performance of dual short wavelength infrared (SWIR) occlusal transillumination and reflectance multispectral imaging with conventional visual assessment and radiography for caries detection on premolars scheduled for extraction for orthodontics reasons. Polarized light microscopy (PLM) and micro-computed tomography (microCT) performed after tooth extraction were used as gold standards. The custom-fabricated imaging probe was 3D-printed and the imaging system employed a SWIR camera and fiber-optic light sources emitting light at 1300 nm for occlusal transillumination and 1600 nm for reflectance measurements. Teeth (n = 135) on 40 test subjects were imaged in vivo using the SWIR imaging prototype in the study and teeth were extracted after imaging. Our study demonstrates for the first time that near-simultaneous real-time transillumination and reflectance video can be successfully acquired for caries detection. Both SWIR imaging modalities had markedly higher sensitivity for lesions on proximal and occlusal surfaces compared to conventional methods (visual and radiographic). Reflectance imaging at 1600 nm had higher sensitivity and specificity than transillumination at 1300 nm. The combined SWIR methods yielded higher specificity but the combined sensitivity was lower than for each individual method.

A 78-year-old woman with an ocular history of cataract surgery with a diffractive intraocular lens (IOL) in each eye has developed fluctuating vision, greater in the right eye than the left eye, after 4 years. She has a history of inactive central serous retinopathy and a vision potential of 20/25 + 2 in the right eye and 20/25 in the left eye. She has well-controlled diabetes, hypertension, and hypercholesterolemia. She has enjoyed her spectacle independence for some time and wishes to have her vision restored. On examination, her uncorrected distance visual acuity (UDVA) was 20/50 in the right eye and 20/25 in the left eye and her uncorrected near visual acuity (UNVA) was J3 in the right eye and J1 in the left eye. Intraocular pressures (IOPs) measured 22 mm Hg in the right eye and 18 mm Hg in the left eye. Pupils had limited reactivity with irregularity in the right eye but no obvious relative afferent pupillary defect. Motility and confrontation visual fields were unremarkable in both eyes. Retinal acuity meter was 20/20 in both eyes, and manifest refraction was plano -1.25 x 105 20/40, J3 in the right eye and +0.50 x 20/25, J1 in the left eye. Pertinent findings on slitlamp examination included temporal iris atrophy and transillumination defects greater in the right eye than the left eye, peripupillary pseudoexfoliative changes in both eyes, significant inferior subluxation of a diffractive 3-piece posterior chamber IOL in the capsular bag with lens-pitting peripherally and few central, moderate pseudophacodonesis, and an open posterior capsule in the right eye. In the left eye, she had mild inferior subluxation of a single-piece acrylic diffractive IOL in the capsular bag with moderate pseudophacodonesis and an open posterior capsule (Figure 1). All other anterior segment findings were unremarkable. On dilated posterior examination, she had a cup-to-disc ratio of 0.50 in the right eye and 0.65 in the left eye without edema hemorrhage or pallor. There were attenuated vessels in both eyes, posterior vitreous detachment in both eyes, and a few small drusen peripherally in both eyes. There was retinal pigment epithelium irregularity and dropout parafoveal in the right eye and subfoveal in the left eye (Figure 2). There was no evidence of macular edema, subretinal fluid, choroidal thickening, or neovascular membranes. The periphery was unremarkable in both eyes. Recent studies in transillumination imaging for developing an optical computed tomography device for small animal and human body parts have used deep learning networks to suppress the scattering effect, estimate depth information of light-absorbing structures, and reconstruct three-dimensional images of de-blurred structures. However, they still have limitations, such as knowing the information of the structure in advance, only processing simple structures, limited effectiveness for structures with a depth of about 15 mm, and the need to use separated deep learning networks for de-blurring and estimating information. Furthermore, the current technique cannot handle multiple structures distributed at different depths next to each other in the same image. To overcome the mentioned limitations in transillumination imaging, this study proposed a pixel-by-pixel scanning technique in combination with deep learning networks (Attention Res-UNet for scattering suppression and DenseNet-169 for depth estimation) to estimate the existence of each pixel and the relative structural depth information. The efficacy of the proposed method was evaluated through experiments that involved a complex model within a tissue-equivalent phantom and a mouse, achieving a reconstruction error of 2.18 % compared to the dimensions of the ground truth when using the fully convolutional network. Furthermore, we could use the depth matrix obtained from the convolutional neural network (DenseNet-169) to reconstruct the absorbing structures using a binary thresholding method, which produced a reconstruction error of 6.82 %. Therefore, only one convolutional neural network (DenseNet-169) must be used for depth estimation and explicit image reconstruction. Therefore, it reduces time and computational resources. With depth information at each pixel, reconstruction of 3D image of the de-blurred structures could be performed even from a single blurred image. These results confirm the feasibility and robustness of the proposed pixel-by-pixel scanning technique to restore the internal structure of the body, including intricate networks such as blood vessels or abnormal tissues. Background: This study evaluates the efficacy and safety of micropulse transscleral cyclophotocoagulation (MP-CPC) in paediatric patients. Methods: This retrospective case-series recruited 44 eyes for glaucoma patients who were younger than 17 years and were treated with MP-CPC and followed for at least one year. Pre- and post-intervention intraocular pressure (IOP) was compared out to at least one year postoperatively. Success rates at 6 months and 1 year postoperatively were assessed. P < 0.05 was considered statistically significant. Results: There were 35 (79.5%) eyes with a history of glaucoma surgery. IOP decreased statistically significantly from a baseline of 32.7 (standard deviation:8.7 mmHg) to 23.2 (8.6) and 21.7 (7.9) mmHg at the 6 months and 1-year follow-up, respectively (P < 0.0001, all comparisons). Overall success was noted in 19 (47.5%) eyes at the 6 months follow-up, and 23 (53.5%) eyes at 1 year. Conclusions: MP-CPC reduces IOP and the burden of medications in paediatric patients with glaucoma. Background: Micropulse transscleral cyclophotocoagulation is a non-invasive, widely employed procedure that uses diode laser to target the ciliary body to lower the intraocular pressure. Despite its acknowledged efficacy, certain complications are recognized, with neurotrophic keratopathy being a rare yet serious consequence. This report seeks to shed light on a potential high-risk subgroup susceptible to neurotrophic keratopathy, exemplified by our patient with Marfan syndrome, a condition characterized by thin sclera. Case Report: Our patient, who was confirmed to have Marfan syndrome with pseudophakic glaucoma, underwent micropulse transscleral cyclophotocoagulation due to high intraocular pressure bilaterally and subsequently quickly manifested neurotrophic keratopathy in both eyes postoperatively. Swift initiation of management involved a comprehensive approach, including topical antibiotics, preservative-free lubrication, medroxyprogesterone acetate 1%, serum balanced salt solution (BSS) 50%, and the application of bandage contact lenses to expedite healing. Fortunately, the left eye demonstrated resolution within 10 days, while the right eye exhibited delayed healing, leading to subsequent scarring. Conclusions: This report highlights the critical importance of recognizing populations predisposed to neurotrophic keratopathy before subjecting them to micropulse transscleral cyclophotocoagulation. Such awareness allows for the fine-tuning of procedural parameters, offering a strategic approach to mitigate the risk of neurotrophic keratopathy development. By further exploring and recognizing potential risk factors, clinicians can enhance patient outcomes and refine the safety profile of micropulse transscleral cyclophotocoagulation. Purpose: In this study, we evaluated the possibility of precise intraoperative localization of the lateral genicular arteries by an orthopaedic surgeon using the transillumination method. Methods: Twelve patients underwent cemented TKA with patella-friendly Zimmer Biomet NexGen Legacy Posterior Stabilized prostheses (without patellar resurfacing), seven right knees and five left knees. The mean age of patients in the study group was 66.636 ± 7.003 years. The minimal follow-up period was 13 months (mean—16.363 ± 2.5 months). Functional outcomes were assessed using Knee Society and a specific patellar questionnaire—Kujala Score. Intraoperative detection of insufficient patellar stability and/or patellar maltracking was based on the no-thumb technique. In pre- and postoperative period X-ray investigation, standard standing X-ray and Merchant view were used to evaluate implant position and patellofemoral congruency. Results: In this study, ten out of twelve knee joints (83.3%) had at least one artery visible by the proposed method in the lateral parapatellar area. Five out of ten knee joints had more than one artery that could be visualized and identified as an arterial vessel. Postoperative Knee Society Score showed significant improvement from a mean 51.181 ± 3.868 to a mean 88.727 ± 3.663. Mean hospital length of stay is 8.545 ± 1.863 days. X-ray assessment using standard anteroposterior, lateral, and Merchant skyline views showed appropriate implant positioning and patellofemoral congruency. The mean Kujala Score in the postoperative period (3 and 6 months) was 67.3 ± 6.75 and 75.6 ± 6.42, respectively. Conclusions: Using the proposed transillumination method can help preserve the lateral blood supply to the patella and to avoid devascularized patella-related complications. Background: Despite the availability of current antibiotic and surgical treatment options, infective endocarditis (IE) remains associated with a high mortality rate. Even though two‐dimensional (2D) transesophageal echocardiography (TOE) is a major criteria in the diagnosis of IE, it is constrained by the single‐plane orientation. Since three‐dimensional (3D) TOE provides a comprehensive understanding of the cardiac architecture by allowing for a realistic visualization of the underlying structures in 3D space, it has attracted considerable interest in recent years. Aim: The purpose of this narrative review is to discuss the advantages and pitfalls of 3D TOE in patients with IE, as well as to address emerging photo‐realistic 3D techniques that have the potential to enhance the visualization of cardiac structures in this setting. Results: According to recent research, 3D TOE acquisitions outperform 2D acquisitions in terms of vegetation identification accuracy and embolism risk assessment. By reporting a variety of findings that are missed with 2D TOE, but which are validated by surgical examination, 3D TOE further improves the ability to identify endocarditis complications on both native and prosthetic valves. In addition to conventional 3D TOE, future developments in 3D technology led to the development of transillumination and tissue‐transparency rendering, which may improve anatomical understanding and depth perception. Due to the use of both conventional and novel 3D techniques, there are more patients who require surgical intervention, indicating that 3D TOE may have a clinical relevance on the surgical management. Bicuspid aortic valve is the most common congenital heart defect. Transthoracic echocardiogram is the initial tool to assess and diagnose this condition, however, transesophageal echocardiogram with 3D modalities, including transillumination have a better anatomical and functional evaluation of the valve, allowing to classify the bicuspid aortic valve according to the position of the raphe and assess the main vessels for complications or exclude other cardiovascular diseases. Introduction: We report a case of bilateral pigment dispersion syndrome after 13 years of uncomplicated implantable collamer lens (ICL) surgery. Case Presentation: A 53-year-old woman was referred from her optometrist to our glaucoma clinic due to early superonasal visual field loss in both eyes. She was asymptomatic with no changes in visual acuity and had undergone bilateral ICL implantation 13 years ago to correct her high myopia. Clinical examination revealed pigment deposition on the corneal endothelium, iris transillumination defects, and iris vaulting at the areas of contact with the ICL. Gonioscopy showed open angles with significant pigmentation of the trabecular meshwork. The diagnosis of pigment dispersion syndrome secondary to ICL implantation was made, and subsequent follow-up visits demonstrated normal intraocular pressure IOP and stable visual fields. Conclusion: Pigmentary dispersion syndrome can occur several years after ICL implantation. This case report emphasizes the need for long-term follow-up and monitoring after ICL surgery. The aim of this study is to compare the effectiveness of visual, radiologic, and near-infrared light transillumination caries detection methods on caries of dog teeth in-vitro. A total of 237 dog molar teeth were evaluated with three different methods; visio-tactile examination with a probe, radiographic assessment, and NIR-LT. Each tooth was evaluated with all of the methods; the absence or presence of occlusal caries was scored as either 0, caries not present, or 1, caries present. All caries detection methods yielded the same outcomes for each tooth. Among all caries detection methods positive Spearman's rho value (1=1) indicated that there was a strong positive correlation between the caries detection methods. Since NIR-LT is such a comfortable and easy caries diagnosis method, it can be used without sedation, especially in calm dogs. [ABSTRACT FROM AUTHOR]

Transillumination microscopes, often with a simple lens-free optical configuration, combined with lab-on-a-chip devices are useful tools for the characterisation of various biological samples. A key issue with these devices is light transparency across a lab-on-a-chip structure. In this work we achieved this by embedding a glass window in a silicon membrane. Despite light transmission, the membrane could be pressure actuated. A second key issue is software analysis of the images due to the holographic nature of the captured images. In this paper, the technology of the silicon/glass membrane and results of porcine oocyte imaging during deformation are presented and compared with our previous micro-electro-mechanical system cytometer working with a reflective microscope. The efficacy and safety of a single novel electronic pulp sensitivity tester with a transilluminator (PSTT) capable of providing 4 different tests was compared with gold standard (GS) pulp testing methods. Four hundred eighty teeth, including 3 from each quadrant and their contralateral and opposing teeth, were randomly assessed using the PSTT or GS methods. Seven days later, the same teeth were assessed using the method not used earlier so that all teeth were evaluated using both methods. Sixty previously root canal–treated teeth, serving as negative controls, were assessed identically to the experimental groups. Results were analyzed using IBM SPSS software (IBM Corp, Armonk, NY) (P <.05). A higher percentage of teeth responded to cold and electricity using GS methods compared with the PSTT (99.4 vs 93.1 and 99.6 vs 97.3, respectively). A lower percentage of teeth responded to heat using the GS method compared with the PSTT (50.0 vs 68.1). Chi-square tests determined differences in the proportion of sensitivity to temperature, and electric testing methods were statistically significant (P <.001 and P =.004, respectively). No difference in the proportion of sensitivity was observed for transillumination. The Wilcoxon signed rank test determined significantly shorter cold testing times using the GS method (P =.024). Shorter testing times were observed using the PSTT for heat (P <.001), electric pulp testing (P =.048), and transillumination (P =.001). The overall PSTT testing time was significantly shorter than the GS testing time (P =.03). Tissue injury was not observed. The Trichoscan method is particularly emphasized, as it allows for an accurate determination of hair density and hair root status. It is pointed out that successful treatment of hair loss often becomes visible only after several months, making the Trichoscan method important for monitoring progress and success. Proximal caries is a diagnostic and therapeutic challenge. Aim: to study the diagnostic accuracy of near-infrared transillumination (DIAGNOcam, KaVo) compared to digital bitewing radiography for initial proximal caries diagnosis. Materials and methods: 1600 proximal surfaces of premolars and molars of 50 patients were examined with DIAGNOcam and digital bitewing radiography, and the level of agreement between them was established. Results: for the diagnosis of proximal caries in dentin, a complete (100%) coincidence of the findings with the type of tooth and type of surface was established. For the diagnosis of proximal caries in enamel, a high level of agreement between the two methods was found, as transillumination with near-infrared light is more sensitive than digital bitewing radiography. Conclusion: DIAGNOcam is a promising screening device in clinical practice for early proximal caries detection.

Background Traditionally, the patient's head is placed in a neutral position with a chin-lift to facilitate light wand guided endotracheal intubation. However, our study found that the sniffing position was more effective. In this study, we aimed to compare the two positions of light wand guided endotracheal intubation. Methods Sixty adult patients were included in the study, after obtaining informed consent, and were randomly assigned to one of two groups: a control group in a neutral position with a chin-lift (group C, n = 30) and a sniffing position group (group S, n = 30). In group C, the anesthesiologist inserted a light wand after lifting the patient's mandible using the thumb of their non-dominant hand inside the patient's mouth. In group S, a light wand was inserted after the patient's head was flexed with the neck extended. We assessed variables such as light-search time, number of intubation attempts, time to achieve intubation, and side effects including blood tinge on the endotracheal tube, hoarseness, sore throat, and anesthesiologist satisfaction. Results The light-search and intubation times were shorter in group S than in group C. The incidence of blood tinge on the endotracheal tube was higher in group C than in group S. Anesthesiologist satisfaction was higher in group S than in group C. Conclusions The sniffing position was more effective in facilitating light wand guided endotracheal intubation than the neutral position with a chin-lift.

Introduction: This experimental study aimed to evaluate the efficacy of contrast medium-aided intraoral periapical radiographic imaging in the detection of cracked teeth and to determine optimal in-vitro compression force for crack detection. Methods: 27 extracted human lower premolar teeth were randomly divided into two groups: the control group, which was imaged using the conventional radiography and the experimental group which was imaged using a contrast medium (Iopamidol®). Both group were examined by transillumination to confirm the presence of a crack prior to imaging. A compression test using a Universal Testing Machine was used to induce the crack. Two dental clinicians blinded to the imaging technique evaluated the images and determined the presence or absence of cracks. Results: The force needed to produce a superficial crack ranges from 200N to 500N with a median compression force of 400N. The range of force to produce partial crack was 200N to 550N with the median force slightly higher than superficial crack which is 435N. The force to produce dentinal fracture ranges from 300N to 1100N with the highest median of 525N. The results showed that there were no visible cracks were detected in IOPA with/without contrast medium. Conclusions: The study did not find contrast-enhance IOPA to be effective in detecting cracks, but the study did establish the optimal median compression force values for inducing cracks which is between 400N to 525N. While numerical aperture of transillumination at or above 1.25 can be achieved with a substage oiled Abbe condenser, such immersion-capable condensers can be expensive limiting their use in resource poor settings. Also the measurement of numerical aperture generated by illuminators has received relatively little attention in the literature compared to methods for measuring the numerical aperture of acceptance by objectives. In this article, I show how an inexpensive paper diffuser with refractive-index coupling to the sample slide can generate illumination of a numerical aperture of over 1.4 at a small fraction of the cost of oiled dioptric condensers of comparable numerical aperture. In addition, I present two ways in which a diffuser may also be used to measure the numerical aperture generated by an illuminator using either a calibrated index-coupled paper diffuser to implement an interpolative variation of the Horsfall method or a diffuser as a detector screen coupled to a self-built microscope slide-based illumination system apertometer. Regulation of ion and water microcirculation within the lens is tightly controlled through aquaporin channels and connexin junctions. However, cataracts can occur when the lens becomes cloudy. Various factors can induce cataracts, including diabetes which is a well-known cause. The most common phenotype of diabetic cataracts is a cortical and/or posterior subcapsular opacity. In addition to the three main types and two subtypes of cataracts, a vacuole formation is frequently observed; however, their origin remains unclear. In this study, we focused on the aquaporins and connexins involved in diabetes-induced cataracts and vacuoles in Nile grass type II diabetes. The results showed that the expression of aquaporin 0 and aquaporin 5 increased, and that of connexin 43 decreased in diabetic rat lenses. Additionally, aquaporin 0 and 5 were strongly localized in peripheral of vacuoles, suggesting that aquaporins are involved in vacuoles formation. Transillumination photography revealed large vacuoles at the tip of the Y-suture in the anterior capsule of the diabetic lens, and several small vacuoles were observed in the posterior capsule. Within the vacuoles, cytoplasmic degradation and aggregation of fibrous material were observed. Our findings suggest that aquaporins are potential candidate proteins for preventing vacuole formation. Background: Ptosis causes an extra folding of tissues lateral to the eyebrows. Different techniques aimed to correct this with varying outcomes and limitations. We analyzed the long-term results of an office-based technique for deep-plane subgaleal lift under local anesthesia and assessed patient satisfaction. Methods: We retrospectively analyzed the long-term outcome of a modified temporal brow lift technique in patients with lateral eyebrow ptosis. The incision lines were marked 3–5 mm behind the temporal lines, and then at the deep temporal fascia layer was dissected. The three key steps in the procedure included (1) detecting the sentinel vein by transillumination; (2) cutting the lateral orbital ligament; and (3) releasing the superior temporal septum by blunt and sharp dissections. Finally, the tissues were fixed to the deep temporal fascia. After 1 and 12–15 months, the distance between the lateral brow and lateral canthus was measured. Results: We operated on 44 Iranian women (ranging from 28 to 51 years) between 2019 and 2021. The mean post-surgical brow elevation was 11.5 mm (95% CI 10.8–12.2; 133.4% improvement) after 1 month and 7.2 mm (6.4–8.1; 88.2% improvement) after 12–15 months. Minor complication (i.e., the stitch abscess) was observed in 6.8% of patients (three patients). Major complications included a complete relapse (two patients; 4.5%) and alopecia (one patient; 2.3%). Overall, 88.6% of patients (39 patients) were satisfied with the outcome. Conclusions: Lateral brow ptosis could be corrected with a modified temporal lift technique using transillumination for the detection of the sentinel vein and preservation of the temporal branch of the facial nerve. Ebola virus (EBOV) causes lethal disease in humans but not in mice. Here, we generated recombinant mouse-adapted (MA) EBOVs, including 1 based on the previously reported serially adapted strain (rMA-EBOV), along with single-reporter rMA-EBOVs expressing either fluorescent (ZsGreen1 [ZsG]) or bioluminescent (nano-luciferase [nLuc]) reporters, and dual-reporter rMA-EBOVs expressing both ZsG and nLuc. No detriment to viral growth in vitro was seen with inclusion of MA-associated mutations or reporter proteins. In CD-1 mice, infection with MA-EBOV, rMA-EBOV, and single-reporter rMA-EBOVs conferred 100% lethality; infection with dual-reporter rMA-EBOV resulted in 73% lethality. Bioluminescent signal from rMA-EBOV expressing nLuc was detected in vivo and ex vivo using the IVIS Spectrum CT. Fluorescent signal from rMA-EBOV expressing ZsG was detected in situ using handheld blue-light transillumination and ex vivo through epi-illumination with the IVIS Spectrum CT. Simple Summary: Advancements in optical technology have revitalized transillumination imaging for biomedical research. Using near-infrared (NIR) light, we have visualized internal structures in animals, even under conditions of heavy optical scattering. By extending the Beer–Lambert law's applicability through differentiation principles, we can observe real-time physiological changes noninvasively. A hurdle in transillumination is image blurring caused by scattering in body tissues. We have addressed this difficulty by extracting near-axis scattered components from diffuse light, and by employing software techniques such as PSF deconvolution and deep learning. This shift has enabled the clear 3D imaging of internal structures from blurred 2D images. Validation through experimentation with human and animal subjects has underscored the effectiveness of these techniques. Integrating transillumination with modern technology holds great promise for future biomedical applications. The classical transillumination technique has been revitalized through recent advancements in optical technology, enhancing its applicability in the realm of biomedical research. With a new perspective on near-axis scattered light, we have harnessed near-infrared (NIR) light to visualize intricate internal light-absorbing structures within animal bodies. By leveraging the principle of differentiation, we have extended the applicability of the Beer–Lambert law even in cases of scattering-dominant media, such as animal body tissues. This approach facilitates the visualization of dynamic physiological changes occurring within animal bodies, thereby enabling noninvasive, real-time imaging of macroscopic functionality in vivo. An important challenge inherent to transillumination imaging lies in the image blur caused by pronounced light scattering within body tissues. By extracting near-axis scattered components from the predominant diffusely scattered light, we have achieved cross-sectional imaging of animal bodies. Furthermore, we have introduced software-based techniques encompassing deconvolution using the point spread function and the application of deep learning principles to counteract the scattering effect. Finally, transillumination imaging has been elevated from two-dimensional to three-dimensional imaging. The effectiveness and applicability of these proposed techniques have been validated through comprehensive simulations and experiments involving human and animal subjects. The purpose of the study is to develop a novel peptide for caries management. Gallic-Acid-Polyphemusin-I (GAPI) was synthesised by grafting Polyphemusin I (PI) and gallic acid (GA). Biocompatibility was evaluated using a Cell Counting Kit-8 Assay. Antimicrobial properties were assessed using minimum inhibitory concentration (MIC) and minimum bactericidal/fungicidal concentration (MBC/MFC). The bacterial and fungal morphology after GAPI treatment was investigated using transmission electron microscopy (TEM). The architecture of a consortium biofilm consisting of Streptococcus mutans, Lacticaseibacillus casei and Candida albicans was evaluated using scanning electron microscopy (SEM) and confocal laser scanning microscopy. The growth kinetics of the biofilm was examined using a propidium monoazide–quantitative polymerase chain reaction. The surface and calcium-to-phosphorus molar ratio of GAPI-treated enamel after pH cycling were examined with SEM and energy-dispersive X-ray spectroscopy. Enamel crystal characteristics were analysed using X-ray diffraction. Lesion depths representing the enamel's mineral loss were assessed using micro-computed tomography. The MIC of GAPI against S. mutans, L. casei and C. albicans were 40 μM, 40 μM and 20 μM, respectively. GAPI destroyed the biofilm's three-dimensional structure and inhibited the growth of the biofilm. SEM showed that enamel treated with GAPI had a relatively smooth surface compared to that treated with water. The calcium-to-phosphorus molar ratio of enamel treated with GAPI was higher than that of the control. The lesion depths and mineral loss of the GAPI-treated enamel were less than the control. The crystallinity of the GAPI-treated enamel was higher than the control. This study developed a biocompatible, mineralising and antimicrobial peptide GAPI, which may have potential as an anti-caries agent. [ABSTRACT FROM AUTHOR]

Objective. To describe a single surgeon's experience utilizing prompt primary slow-burn transscleral cyclophotocoagulation (CPC) with prior or concurrent anti-VEGF and subsequent aqueous shunt as needed in NVG eyes with near-total synechial angle closure at presentation. Methods. Retrospective chart review of all NVG patients with uncontrolled IOP, active anterior segment NV, near-total synechial angle closure, and no contraindications to prompt anti-VEGF who received CPC within 3 days of presentation with at least 6 months of follow-up. Results. Eight patients with mean age 60.6 years were included. Underlying etiologies were CRVO (N = 3), PDR (N = 2), CRAO (N = 1), BRVO (N = 1), and chronic RD (N = 1). All eyes underwent CPC with intravitreal anti-VEGF within 3 days of presentation. Five patients did not require subsequent aqueous shunts through a mean follow-up of 15 months; most recent visual acuities ranged from 20/40 to LP, and IOPs ranged from 5 to 11 mmHg on 0 to 3 IOP-lowering medications. Three patients who required subsequent tubes had complete regression of active anterior segment NV at the time of surgery. Most recent visual acuities ranged from 20/100 to 20/125, and IOPs ranged from 8 to 14 mmHg on 0 meds at a mean follow-up of 10 months. No eyes developed uncontrolled inflammation, sympathetic ophthalmia, or phthisis. Conclusion. Prompt primary slow-burn CPC with prior or concurrent anti-VEGF may be an effective strategy to immediately lower IOP in acute NVG eyes with active anterior segment NV and near-total synechial angle closure. This in vitro study aimed to investigate the diagnostic potential of short-wave infrared transillumination (SWIRT) at 1050, 1200 and 1300 nm for the detection of proximal caries in molars and premolars. It was compared to the diagnostic performance of bitewing radiography (BWR) and micro-computed tomography (µCT) as the reference standard. 250 sound or decayed proximal surfaces of permanent posterior extracted teeth were examined using (1) SWIRT at 1050, 1200 and 1300 nm with two camera systems of different resolutions, (2) BWR and (3) µCT. Thresholds were defined for both test methods and the reference standard for caries in general, enamel caries and dentin caries. All images were assessed by two examiners twice, at an interval of two weeks. SWIRT at wavelengths of 1050, 1200 and 1300 nm achieved sensitivity values more than 2.5 times higher than BWR (enamel caries 3.2–4.4 times; dentin caries 3.25–4.25 times) for the detection of proximal caries. Sensitivity values of SWIRT improved with the higher wavelength. No significant difference was found in diagnostic quality between the two camera systems. SWIRT at 1300 nm imaged proximal enamel caries with the highest accuracy, while the physical optimum for transillumination in dentin was located at a lower wavelength (<1000 nm). The paper analyzes the spatial distribution of the average air density in a supersonic jet based on the results of laser transillumination. The algorithm to retrieve the average air density from transverse (to the jet axis) deviations of the transilluminating wavefront was tested in experiments on the Vertical jet setup of ITAM SB RAS. The results of retrieval are compared with the known literature data of contact measurements and with results of numerical simulation. The good sensitivity of local wavefront tilts to oscillations in the air density at frequencies of discrete acoustic tones has been demonstrated. Purpose: To evaluate the outcomes of micropulse transscleral laser therapy (MP-TLT) in patients with uncontrolled glaucoma and prior glaucoma aqueous tube shunt. Methods: In this single‑center, retrospective, interventional case series, eyes that underwent MP-TLT and had prior glaucoma aqueous tube shunt surgeries were included. The Cyclo Glaucoma Laser System (IRIDEX Corporation, Mountain View, CA, USA) with the MicroPulse P3 probe (version 1) was used. Post‑operative data were collected at day 1, week 1, and months 1, 3, 6, 12, 18, 24, 30 and 36. Results: A total of 84 eyes (84 patients) with mean age of 65.8 ± 15.2 years and with advanced glaucoma (baseline mean deviation -16.25 ± 6.80 dB and best-corrected visual acuity 0.82 ± 0.83 logMar) were included in the study. Baseline mean IOP was 19.95 ± 5.6 mm Hg with a mean number of medications 3.39 ± 1.02. There were statistically significant differences in IOP between baseline and all follow-up visits (p < 0.01 for all). The mean percentage of IOP reduction between baseline and different follow-up visits ranged from 23.4% to 35.5% (p < 0.01). There was a significant reduction of visual acuity (≥ 2-lines) at 1 year (30.3%) and 2 years (76.78%). There was a statistically significant reduction in the number of glaucoma medications between baseline and all follow-up visits after postoperative week 1 (p < 0.05 for all). No severe complications including persistent hypotony and related complications were observed. At the last follow-up visit, only 24 (28%) eyes out of 84 eyes remained in the study. Conclusion: MP-TLT is an effective treatment for reducing IOP and decreasing the number of medications in patients with advanced glaucoma and prior glaucoma aqueous tube shunt. Digital imaging fiber-optic transillumination (DIFOTI) devices have been used to detect caries, a technique without using X-rays. However, the effects of resin composites (RCs) shades on the images acquired with DIFOTI devices have not been investigated. Thus, this study aimed to elucidate the influence of RC shade on the images obtained with DIFOTI technique. Three shades (A1, A3, and Opaque) for each of four flowable RCs were filled on a cavity prepared in a left mandibular first premolar obtained from a donated body. Then, transmission images with a DIFOTI device (DIAGNOcam; KaVo, Biberach, Germany) were acquired, and the average lightness values of the images in the RC and enamel were used to calculate differences between those areas. To clarify the influence of the optical translucency and color on DIFOTI images, the color parameters (L*, a* and b*) of each RC were obtained with black and white backgrounds. The color differences between the backgrounds were calculated as transparency parameter (TP) values. The number of repetitions was set to 10. Differences in the lightness value of the shades varied in each RC. The difference in lightness was significantly associated with the TP value and color parameters of L* (p < 0.01), with negative (R = − 0.81) and positive (R = 0.84) correlations, respectively. Harnessing the power of light and its photonic energy is a powerful tool in biomedical applications. Its use ranges from biomaterials processing and fabrication of polymers to diagnostics and therapeutics. Dental light curable materials have evolved over several decades and now offer very fast (≤ 10 s) and reliable polymerization through depth (4–6 mm thick). This has been achieved by developments on two fronts: (1) chemistries with more efficient light absorption characteristics (camphorquinone [CQ], ~30 L mol-1 cm1 [ʎmax 470 nm]; monoacylphosphine oxides [MAPO], ~800 L mol-1 cm-1 [ʎmax 385 nm]; bisacylphosphine oxide [BAPO], ~1,000 L mol-1 cm-1 [ʎmax 385 nm]) as well mechanistically efficient and prolonged radical generation processes during and after light irradiation, and; (2) introducing light curing technologies (light emitting diodes [LEDs] and less common lasers) with higher powers (≤ 2 W), better spectral range using multiple diodes (short: 390–405 nm; intermediate: 410–450 nm; and long: 450–480 nm), and better spatial power distribution (i.e. homogenous irradiance). However, adequate cure of materials falls short for several reasons, including improper selection of materials and lights, limitations in the chemistry of the materials, and limitations in delivering light through depth. Photonic energy has further applications in dentistry which include transillumination for diagnostics, and therapeutic applications that include photodynamic therapy, photobiomodulation, and photodisinfection. Light interactions with materials and biological tissues are complex and it is important to understand the advantages and limitations of these interactions for successful treatment outcomes. This article highlights the advent of photonic technologies in dentistry, its applications, the advantages and limitations, and possible future developments.

The aim of this study was to compare the diagnostic performance of dual short-wavelength infrared (SWIR) transillumination and reflectance multispectral imaging devices for imaging interproximal lesions with radiography using extracted teeth that had been imaged with micro-computed tomography (microCT). Thirty-six extracted teeth with 67 lesions on the proximal surfaces were imaged using a newly fabricated SWIR multispectral proximal transillumination and reflectance imaging device along with an existing SWIR multispectral occlusal transillumination and reflectance device. The ability of SWIR imaging and radiography to detect lesions and accurately assess lesion dimensions were compared using microCT as a standard. Occlusal and proximal transillumination and occlusal reflectance performed best for imaging interproximal lesions while proximal reflectance was not useful for imaging interproximal lesions from tooth buccal and lingual surfaces. There was high correlation of the lesion dimensions measured in occlusal and proximal transillumination images compared to microCT and moderate correlation in occlusal reflectance images. The correlation between the lesion depth measured in radiographs and the lesion depth measured with microCT was not significant. This study demonstrates that SWIR occlusal and proximal transillumination and SWIR occlusal reflectance images are useful for imaging interproximal lesions and they provide more accurate measurements of lesion severity.

Simple Summary: Malignant bowel obstruction (MBO) requires adequate palliation in progressive ovarian cancer. Percutaneous endoscopic gastrostomy has limitations in 32.7% of patients with advanced/recurrent ovarian cancer due to anatomical constraints and a lack of transillumination. CT-guided gastrostomy is a safe and effective procedure, enabling rapid recognition of the anatomy, particularly in complex patients with peritoneal carcinomatosis and previous multivisceral surgeries. This technique should be particularly emphasized within the palliative management of MBO. A clinical registry should be implemented to evaluate the effectiveness of various treatment strategies in MBOs associated with gynaecological cancers. Peritoneal carcinomatosis-associated malignant bowel obstruction is a common feature that merits more attention in advanced and recurrent ovarian cancer. Decompressive gastrostomy is one of the most preferred methods to palliate distressing symptoms and maintain patients' quality of life. We retrospectively identified 31 patients with ovarian cancer-associated MBO, who underwent decompressive CT fluoroscopy-guided percutaneous gastrostomy (CT-PG) between September 2015 and April 2023 at our institution. A systematic literature review was conducted for CT-guided gastrostomy in ovarian cancer. Prior to CT-PG, 27 (87%) patients underwent unsuccessful attempts at endoscopic gastrostomy or surgery due to bowel obstruction; a total of 55% had received ≥3 lines of chemotherapy. CT-PG could be successfully inserted in 25 of 31 (81%) patients without grade 4–5 complications. CT-PG insertion was feasible in 76% of patients with previous unsuccessful attempts of endoscopic gastrostomy. A total of 80% of patients with a successful insertion had considerable symptom relief and could tolerate fluid intake. Mean survival after the procedure was 44.4 days. Chemotherapy could be administered in 7 of 25 (28%) patients following the CT-PG insertion. CT-guided percutaneous gastrostomy is a safe procedure that effectively manages intractable symptoms of bowel obstruction in ovarian cancer. The development of optical sensors, especially with regard to the improved resolution of cameras, has made optical techniques more applicable in medicine and live animal research. Research efforts focus on image signal acquisition, scattering de-blur for acquired images, and the development of image reconstruction algorithms. Rapidly evolving artificial intelligence has enabled the development of techniques for de-blurring and estimating the depth of light-absorbing structures in biological tissues. Although the feasibility of applying deep learning to overcome these problems has been demonstrated in previous studies, limitations still exist in terms of de-blurring capabilities on complex structures and the heterogeneity of turbid medium, as well as the limit of accurate estimation of the depth of absorptive structures in biological tissues (shallower than 15.0 mm). These problems are related to the absorption structure's complexity, the biological tissue's heterogeneity, the training data, and the neural network model itself. This study thoroughly explores how to generate training and testing datasets on different deep learning models to find the model with the best performance. The results of the de-blurred image show that the Attention Res-UNet model has the best de-blurring ability, with a correlation of more than 89% between the de-blurred image and the original structure image. This result comes from adding the Attention gate and the Residual block to the common U-net model structure. The results of the depth estimation show that the DenseNet169 model shows the ability to estimate depth with high accuracy beyond the limit of 20.0 mm. The results of this study once again confirm the feasibility of applying deep learning in transmission image processing to reconstruct clear images and obtain information on the absorbing structure inside biological tissue. To date, no scientific data is available regarding the development and radiographic assessment of approximal caries development after the insertion of 3D-printed, non-invasive veneers of different restoration thicknesses. For the present study, non-invasive veneers were fabricated from two different materials for printing and milling (Vita Enamic and VarseoSmile Crown plus). Three different restoration thicknesses (0.5, 0.7, and 0.9 mm) were selected. After digital design, leaving the approximal space free, and manufacturing of the restorations, adhesive insertion followed. All specimens were placed in a demineralizing solution for 28 days. Subsequently, a radiological and fluorescent examination was performed. The present study showed statistically significant interactions for the day (p < 0.0001) and manufacturing method (p < 0.0001) but not for restoration thickness. Additive manufactured restorations showed less radiological caries progression compared to subtractive manufactured restorations after 21 and 28 days (0.7 and 0.9 mm restoration thickness) (p < 0.0001). DIAGNOdent proved that the restoration thickness affected the caries progression within the subtractive group (p < 0.0001). Radiographic and fluorescence examination showed equivalent results regarding approximal caries assessment. Purpose: Late-onset retinal degeneration (L-ORD) is a rare retinal dystrophy with anterior segment (AS) abnormalities, including long anterior zonules (LAZ) and iris atrophy. This investigation evaluates AS changes in a L-ORD cohort. Methods: Prospective, longitudinal study including L-ORD individuals (Ser163Arg) with ocular exam and standard slit-lamp photographs between 2011 and 2022. AS images were merged and assessed for LAZ number and zonule-free zone (ZFZ) radius. Further clinical findings such as iris atrophy patterns were reported descriptively. Results: Twelve eyes of 6 patients (4 males, median age = 60.5 years) were included, showing a median of 160 (11–372) LAZs, mainly localized superiorly (39%) and inferiorly (24%). There was a high inter-ocular correlation (rs = 0.94, p < 0.01), no difference in LAZ count between eyes (p = 0.82), and an inverse relationship between LAZ and age (r = − 0.82; p < 0.05). The ZFZ had median 2.1 mm (1.3–5.4), with no inter-ocular difference (p = 0.31). Iris transillumination defects occurred in 11/12 eyes, with 4 major patterns identified: pupillary ruff rarefaction (10/12), patchy atrophy (6/12), notched defects (6/12), and radial streaks (2/12). In a short-term follow-up of 5.9 years, 4 eyes showed a reduction in LAZ count to median 139.5 (67–169) (p = 0.50) and a concomitant increase in ZFZ measurement to median 2.2 (1.7–2.6) (p = 0.17). Conclusion: This study confirms symmetric LAZs count and ZFZ in L-ORD, with ZFZ measurements smaller than in previous cohorts. A reduction in LAZs count and an increase in ZFZ with age were suggested longitudinally, yet findings need further evaluation as follow-up was limited to two cases. [ABSTRACT FROM AUTHOR]
