Abstract

Increasing evidence suggests that early neurodevelopmental defects in Huntington's disease (HD) patients could contribute to the later adult neurodegenerative phenotype. Here, by using HD-derived induced pluripotent stem cell lines, we report that early telencephalic induction and late neural identity are affected in cortical and striatal populations. We show that a large CAG expansion causes complete failure of the neuro-ectodermal acquisition, while cells carrying shorter CAGs repeats show gross abnormalities in neural rosette formation as well as disrupted cytoarchitecture in cortical organoids. Gene-expression analysis showed that control organoid overlapped with mature human fetal cortical areas, while HD organoids correlated with the immature ventricular zone/subventricular zone. We also report that defects in neuroectoderm and rosette formation could be rescued by molecular and pharmacological approaches leading to a recovery of striatal identity. These results show that mutant huntingtin precludes normal neuronal fate acquisition and highlights a possible connection between mutant huntingtin and abnormal neural development in HD.

Abstract

Objective: To assess the value of opportunistic biomarkers derived from chest CT performed at hospital admission of COVID-19 patients for the phenotypization of high-risk patients. Methods: In this multicentre retrospective study, 1845 consecutive COVID-19 patients with chest CT performed within 72 h from hospital admission were analysed. Clinical and outcome data were collected by each center 30 and 80 days after hospital admission. Patients with unknown outcomes were excluded. Chest CT was analysed in a single core lab and behind pneumonia CT scores were extracted opportunistic data about atherosclerotic profile (calcium score according to Agatston method), liver steatosis (≤ 40 HU), myosteatosis (paraspinal muscle F < 31.3 HU, M < 37.5 HU), and osteoporosis (D12 bone attenuation < 134 HU). Differences according to treatment and outcome were assessed with ANOVA. Prediction models were obtained using multivariate binary logistic regression and their AUCs were compared with the DeLong test. Results: The final cohort included 1669 patients (age 67.5 [58.5–77.4] yo) mainly men 1105/1669, 66.2%) and with reduced oxygen saturation (92% [88–95%]). Pneumonia severity, high Agatston score, myosteatosis, liver steatosis, and osteoporosis derived from CT were more prevalent in patients with more aggressive treatment, access to ICU, and in-hospital death (always p < 0.05). A multivariable model including clinical and CT variables improved the capability to predict non-critical pneumonia compared to a model including only clinical variables (AUC 0.801 vs 0.789; p = 0.0198) to predict patient death (AUC 0.815 vs 0.800; p = 0.001). Conclusion: Opportunistic biomarkers derived from chest CT can improve the characterization of COVID-19 high-risk patients. Abstract

The aim of the study was to evaluate the effect of type of irrigation and of amount of water supplied on vine growth, yield and fruit quality at harvest and after 2 and 4 months of cold storage. The study was carried out in 2019, in northern Italy, on kiwifruit Gold 3®(A. chinensis var. chinensis) grafted onto micro-propagated 'Hayward' (A. chinensis var. deliciosa) rootstocks in 2012, planted at a distance of 4.5×2 m apart. The investigated irrigation systems included: 1) control (T0); 2) drip irrigation with a single line (T1); 3) drip irrigation with double lines (T2); 4) sprinkler (T3); 5) drip irrigation with double lines + sprinkler (T2+T3). Water management in the control treatment was carried out with drip irrigation with a single pipeline according to the advisory service, based on daily evapotranspiration only. In T1, T2, T3, and T2+T3 water was applied according to the soil water content measured by potentiometric probes, located at two depths: 0.20 and 0.60 m, and two distances from the line of the trees: 0.20 and 1 m. Irrigation started when soil water content got down below the field capacity (-0.03 MPa) and returned the same amount of water lost the day before and measured by a PAN evaporimeter. Compared to the control, T1 reduced the volume of water applied by 34%, T2 by 3%, while T3 increased the irrigation volume by 14% and T2+T3 by 17%. Sprinkler application induced an excess of water in the inter row, where soil structure was impaired by the frequent passes of machines, reducing soil permeability. No effect of treatments on vine yield was observed, however, fruit soluble solid concentration and fruit dry matter at harvest was increased by T2 compared to the other treatments. A similar response was also found after 2 months of cold storage, but disappeared after 4 months. The incidence of storage break down (SBD) was higher in fruits of the control and T3 (22 and 40% of the fruits, respectively), while T2 and T2+T3 irrigation strategies showed the lowest percentage (5-8%, respectively). Finally, the incidence of SBD was negatively correlated with the fruit dry matter (r=-0.88; P≤0.05).

Abstract

Purpose: To develop and validate an effective and user-friendly AI platform based on a few unbiased clinical variables integrated with advanced CT automatic analysis for COVID-19 patients’ risk stratification. Material and Methods: In total, 1575 consecutive COVID-19 adults admitted to 16 hospitals during wave 1 (February 16-April 29, 2020), submitted to chest CT within 72 h from admission, were retrospectively enrolled. In total, 107 variables were initially collected; 64 extracted from CT. The outcome was survival. A rigorous AI model selection framework was adopted for models selection and automatic CT data extraction. Model performances were compared in terms of AUC. A web–mobile interface was developed using Microsoft PowerApps environment. The platform was externally validated on 213 COVID-19 adults prospectively enrolled during wave 2 (October 14-December 31, 2020). Results: The final cohort included 1125 patients (292 non-survivors, 26%) and 24 variables. Logistic showed the best performance on the complete set of variables (AUC = 0.839 ± 0.009) as in models including a limited set of 13 and 5 variables (AUC = 0.840 ± 0.0093 and AUC = 0.834 ± 0.007). For non-inferior performance, the 5 variables model (age, sex, saturation, well-aerated lung parenchyma and cardiothoracic vascular calcium) was selected as the final model and the extraction of CT-derived parameters was fully automatized. The fully automatic model showed AUC = 0.842 (95% CI: 0.816–0.867) on wave 1 and was used to build a 0–100 scale risk score (AI-SCoRE). The predictive performance was confirmed on wave 2 (AUC 0.808; 95% CI: 0.7402–0.8766). Conclusions: AI-SCoRE is an effective and reliable platform for automatic risk stratification of COVID-19 patients based on a few unbiased clinical data and CT automatic analysis.

Abstract

Critical levels (CL) of available potassium (K) in soils and leaves and maximum technical efficiency (MTE) doses are relevant information to define the best K fertilization strategies. The aim of this study was to determine reference values of K in leaves and soil, using Bayesian analysis, in order to maximize grape yield and must quality of ‘Chardonnay’ and ‘Pinot Noir’ grown in a subtropical climate. The vines were subjected to applications of 0, 20, 40, 60 and 80 kg K2O ha−1 year−1. The CL and MTE doses of K, in leaves and soils, were determined using Bayesian hierarchical models. The range of probability of the occurrence of CL in leaves was 15.8 to 18.9 g K kg−1 in ‘Chardonnay’ and 16.8 to 19.1 g K kg−1 in ‘Pinot Noir’ at flowering. Moreover, the range was 15.3 to 18.1 g K kg−1 in ‘Chardonnay’ and 16.1 to 18.2 g K kg−1 in ‘Pinot Noir’ at veraison. In the soil, the CL of available K for ‘Chardonnay’ was 27.4 mg K kg−1 and for ‘Pinot Noir’ it was 23.2 mg K kg−1. The increase of K in leaves and soil increased the TSS, while the TTA decreased in both cultivars after sufficiency rate. The MTE was estimated in ‘Pinot Noir’. The proposition of CL and/or MTE doses of K can help increase the efficiency of K fertilization in vineyards.

Abstract

Recent clinical and demographical studies on COVID-19 patients have demonstrated that men experience worse outcomes than women. However, in most cases, the data were not stratified according to gender, limiting the understanding of the real impact of gender on outcomes. This study aimed to evaluate the disaggregated in-hospital outcomes and explore the possible interactions between gender and cardiovascular calcifications. Data was derived from the sCORE-COVID-19 registry, an Italian multicentre registry that enrolled COVID-19 patients who had undergone a chest computer tomography scan on admission. A total of 1683 hospitalized patients (mean age 67±14 years) were included. Men had a higher prevalence of cardiovascular comorbidities, a higher pneumonia extension, more coronary calcifications (63% vs.50.9%, p<0.001), and a higher coronary calcium score (391±847 vs. 171±479 mm3, p<0.001). Men experienced a significantly higher mortality rate (24.4% vs. 17%, p=0.001), but the death event tended to occur earlier in women (15±7 vs. 8±7 days, p= 0.07). Non-survivors had a higher coronary, thoracic aorta, and aortic valve calcium score. Female sex, a known independent predictor of a favorable outcome in SARS-CoV2 infection, was not protective in women with a coronary calcification volume greater than 100 mm3. There were significant differences in cardiovascular comorbidities and vascular calcifications between men and women with SARS-CoV2 pneumonia. The differences in outcomes can be at least partially explained by the different cardiovascular profiles. However, women with poor outcomes had the same coronary calcific burden as men. The presumed favorable female sex bias in COVID-19 must therefore be reviewed in the context of comorbidities, especially cardiovascular ones.

Abstract

Background and aims: The potential impact of coronary atherosclerosis, as detected by coronary artery calcium, on clinical outcomes in COVID-19 patients remains unsettled. We aimed to evaluate the prognostic impact of clinical and subclinical coronary artery disease (CAD), as assessed by coronary artery calcium score (CAC), in a large, unselected population of hospitalized COVID-19 patients undergoing non-gated chest computed tomography (CT) for clinical practice. Methods: SARS-CoV 2 positive patients from the multicenter (16 Italian hospitals), retrospective observational SCORE COVID-19 (calcium score for COVID-19 Risk Evaluation) registry were stratified in three groups: (a) “clinical CAD” (prior revascularization history), (b) “subclinical CAD” (CAC >0), (c) “No CAD” (CAC = 0). Primary endpoint was in-hospital mortality and the secondary endpoint was a composite of myocardial infarction and cerebrovascular accident (MI/CVA). Results: Amongst 1625 patients (male 67.2%, median age 69 [interquartile range 58–77] years), 31%, 57.8% and 11.1% had no, subclinical and clinical CAD, respectively. Increasing rates of in-hospital mortality (11.3% vs. 27.3% vs. 39.8%, p < 0.001) and MI/CVA events (2.3% vs. 3.8% vs. 11.9%, p < 0.001) were observed for patients with no CAD vs. subclinical CAD vs clinical CAD, respectively. The association with in-hospital mortality was independent of in-study outcome predictors (age, peripheral artery disease, active cancer, hemoglobin, C-reactive protein, LDH, aerated lung volume): subclinical CAD vs. No CAD: adjusted hazard ratio (adj-HR) 2.86 (95% confidence interval [CI] 1.14–7.17, p=0.025); clinical CAD vs. No CAD: adj-HR 3.74 (95% CI 1.21–11.60, p=0.022). The recent severe acute respiratory syndrome-related coronavirus 2 (SARS-CoV-2) pandemic has highlighted the importance of pulmonary computed tomography (CT) for diagnosis and prognostic stratification of this new viral pneumonia. 1370 lung CT scans (performed at the time of admission) of consecutive patients hospitalized for SARS-CoV-2 in Northern Italy during the first epidemic wave were analyzed by a radiological CoreLab. The presence of pleural effusion on pulmonary CT scan was present in 188 patients (13.3% of the population) and identified a population with more comorbidities. Patients with pleural effusion had more cardio-respiratory complications with higher mortality. Pleural effusion was an independent predictor of death on multivariate analysis with an HR of 1.4 (95% confidence interval 1-1.9). Pulmonary CT pleural effusion was an independent predictor of mortality.

Background Coronary access after transcatheter aortic valve implantation (TAVI) with supra-annular self-expandable valves may be challenging or un-feasible. There is little data concerning coronary access following transcatheter aortic valve-in-valve implantation (ViV-TAVI) for degenerated surgical bioprosthesis Aims To evaluate the feasibility and challenge of coronary access after ViV-TAVI with the supra-annular self-expandable ACURATE neo valve. Methods Sixteen patients underwent ViV-TAVI with the ACURATE neo valve. Post-procedural computed tomography (CT) was used to create 3D-printed life-sized patient-specific models for bench-testing of coronary cannulation. Primary endpoint was feasibility of diagnostic angiography and PCI. Secondary endpoints included incidence of challenging cannulation for both diagnostic catheters (DC) and guiding catheters (GC). The association between challenging cannulations with aortic and transcatheter/surgical valve geometry was evaluated using pre and post-procedural CT scans. Results Diagnostic angiography and PCI were feasible for 97% and 95% of models respectively. All non-feasible procedures occurred in ostia that underwent prophylactic ‘chimney’ stenting. Abstract

Obiettivo dello studio è stato di stabilire la dinamica di assorbimento dei nutrienti minerali e la loro ripartizione tra i diversi organi della pianta di kiwi a polpa gialla. La prova è stata condotta a Brisighella, in un frutteto impiantato nel 2014 su un terreno franco-argilloso di buona fertilità. Dal mese di maggio sono stati determinati periodicamente la lunghezza dei germogli e il numero di foglie, la crescita del frutto (diametro, peso e sostanza secca), mentre ogni quattro settimane sono stati prelevati campioni di foglie e frutti per la determinazione della composizione minerale. A fine stagione, infine, sono state raccolte e pesate le foglie abscisse e il legno di potatura, anch’essi analizzati per macro e micronutrienti. Alla raccolta, nei frutti erano presenti rispettivamente 77, 10, 74, 11, 5 e 6 kg ha-1 di N, P, K, Ca, Mg e S. Boro e Fe sono risultati i micronutrienti più presenti nei frutti alla raccolta (circa 100 g ha-1). L’accumulo di N, P e K nelle foglie e nei frutti è aumentato linearmente fino alla raccolta; l’accumulo di Ca nei frutti si è interrotto ad agosto, mentre nelle foglie è continuato fino alla senescenza fogliare. Tenuto conto dei diversi organi della pianta (frutti diradati, legno di potatura, frutti raccolti e foglie abscisse), le quantità di N, P, K, Ca e Mg assorbite durante la stagione sono state rispettivamente pari a 143, 16, 105, 202 e 38 kg ha-1.

Abstract

Doped calcite (Fe-EDDHSA/CaCO3) was experimentally produced. The hypothesis of the present experiment is that, when roots get in contact with Fe-EDDHSA/CaCO3, the extrusion of H+ decreases the pH and dissolves calcite with subsequent release of Fe that becomes available for roots. The aim of the experiment was to determine whether doped calcite might represent a slow-release Fe source for in-vitro grown kiwifruit plantlets. The root elongation media used in the experiment had pH 8.0 and differed from each other for Fe supply as follow: Control medium that contained complete Murashige and Skoog salt mixture, including FeSO4 and Na2EDTA; calcite medium enriched with Fe-EDDHSA/CaCO3 as the only Fe source; −Fe medium without Fe. The absence of FeSO4 in the medium caused a reduction of plantlet growth. The final pH was higher with calcite medium than in control and −Fe. The addition of Fe-EDDHSA/CaCO3 increased Fe shoot concentration when compared with the −Fe medium. The data of the present experiment show the potential Fe slow release ability of Fe-EDDHSA/CaCO3; however, further investigation on Fe containing fertilizers should be conducted on potted plants to validate our results.

Abstract

Severe mitral regurgitation represents the second most common indication for valve surgery in Europe. In the last decade, transcatheter mitral valve repair has become an established effective and safe treatment option for patients at high or prohibitive operative risk. A relevant proportion of these patients develop significant tricuspid regurgitation. Once considered a benign and reversible finding, nowadays increasing attention is being paid to the coexistence of this valvular disease, as a growing body of evidence points towards its negative prognostic role. Over the last few years, transcatheter repair has been performed successfully also for the treatment of significant tricuspid regurgitation in patients at high surgical risk. Therefore, combined transcatheter repair is emerging as a treatment strategy for high surgical risk patients with both severe mitral and tricuspid regurgitation. In this review, we provide a comprehensive summary of the epidemiology, pathophysiology, and prognostic role of the coexistence of severe mitral and tricuspid regurgitation. We then discuss the evidence in favor of combined surgical treatment and the emerging data supporting combined transcatheter treatment, with attention to available treatment technologies. The paper is further enriched with three practical case reports and with the suggestion of a decision-making algorithm for the management of this complex subset of patients.

Abstract

Background: Previous randomized controlled trials demonstrated a higher rate of stent thrombosis with bioresorbable vascular scaffold (BVS) implantation as compared with second-generation drug-eluting stent in selected patients/lesions. However, long-term outcomes of BVS implantations that utilize an optimized technique (OIT) in unselected patients/lesions are lacking. The aim of this study was to assess the real-world, long-term clinical outcomes of BVS (Absorb; Abbott Vascular) with OIT. Methods and results: In a cohort of 156 patients, a total of 347 BVS devices (435 lesions) were implanted, with intravascular ultrasound (IVUS) guidance utilized in 303 (87.3%) of the scaffolds. The primary efficacy endpoint was target-lesion revascularization (TLR) and the primary safety endpoint was scaffold thrombosis. Despite performing routine high-pressure postdilation, postintervention IVUS detected BVS underexpansion/malapposition in 53 scaffolds (28.7%), requiring further postdilation. At a median follow-up of 60 months (interquartile range, 45-73 months), TLR and scaffold thrombosis occurred in 16 patients (10.3%) and 1 patient (0.6%), respectively. Abstract

Background: Coronavirus disease 2019 (COVID-19) has spread worldwide determining dramatic impacts on healthcare systems. Early identification of high-risk parameters is required in order to provide the best therapeutic approach. Coronary, thoracic aorta and aortic valve calcium can be measured from a non-gated chest computer tomography (CT) and are validated predictors of cardiovascular events and all-cause mortality. However, their prognostic role in acute systemic inflammatory diseases, such as COVID-19, has not been investigated. Objectives: The aim was to evaluate the association of coronary artery calcium and total thoracic calcium on in-hospital mortality in COVID-19 patients. Methods: 1093 consecutive patients from 16 Italian hospitals with a positive swab for COVID-19 and an admission chest CT for pneumonia severity assessment were included. At CT, coronary, aortic valve and thoracic aorta calcium were qualitatively and quantitatively evaluated separately and combined together (total thoracic calcium) by a central Core-lab blinded to patients’ outcomes. Results: Non-survivors compared to survivors had higher coronary artery [Agatston (467.76 ​± ​570.92 vs 206.80 ​± ​424.13 ​mm2, p ​< ​0.001); Volume (487.79 ​± ​565.34 vs 207.77 ​± ​406.81, p ​< ​0.001)], aortic valve [Volume (322.45 ​± ​390.90 vs 98.27 ​± ​250.74 mm2, p ​< ​0.001; Agatston 337.38 ​± ​414.97 vs 111.70 ​± ​282.15, p ​< ​0.001)] and thoracic aorta [Volume (3786.71 ​± ​4225.57 vs 1487.63 ​± ​2973.19 mm2, p ​< ​0.001); Agatston (4688.82 ​± ​5363.72 vs 1834.90 ​± ​3761.25, p ​< ​0.001)] calcium values. Coronary artery calcium (HR 1.308; 95% CI, 1.046–1.637, p ​= ​0.019) and total thoracic calcium (HR 1.975; 95% CI, 1.200–3.251, p ​= ​0.007) resulted to be independent predictors of in-hospital mortality. Conclusion: Coronary, aortic valve and thoracic aortic calcium assessment on admission non-gated CT permits to stratify the COVID-19 patients in-hospital mortality risk.

Abstract

Objectives Enlarged main pulmonary artery diameter (MPAD) resulted to be associated with pulmonary hypertension and mortality in a non-COVID-19 setting. The aim was to investigate and validate the association between MPAD enlargement and overall survival in COVID-19 patients. Methods This is a cohort study on 1469 consecutive COVID-19 patients submitted to chest CT within 72 h from admission in seven tertiary level hospitals in Northern Italy, between March 1 and April 20, 2020. Derivation cohort (n = 761) included patients from the first three participating hospitals; validation cohort (n = 633) included patients from the remaining hospitals. CT images were centrally analyzed in a core-lab blinded to clinical data. The prognostic value of MPAD on overall survival was evaluated at adjusted and multivariable Cox's regression analysis on the derivation cohort. The final multivariable model was tested on the validation cohort. Results In the derivation cohort, the median age was 69 (IQR, 58-77) years and 537 (70.6%) were males. In the validation cohort, the median age was 69 (IQR, 59-77) years with 421 (66.5%) males. Enlarged MPAD (>= 31 mm) was a predictor of mortality at adjusted (hazard ratio, HR [95%CI]: 1.741 [1.253-2.418], p < 0.001) and multivariable regression analysis (HR [95%CI]: 1.592 [1.154-2.196], p = 0.005), together with male gender, old age, high creatinine, low well-aerated lung volume, and high pneumonia extension (c-index [95%CI] = 0.826 [0.796-0.851]). Model discrimination was confirmed on the validation cohort (c-index [95%CI] = 0.789 [0.758-0.823]), also using CT measurements from a second reader (c-index [95%CI] = 0.790 [0.753;0.825]). Conclusion Enlarged MPAD (>= 31 mm) at admitting chest CT is an independent predictor of mortality in COVID-19.

Abstract

Background: Since December 2019, the global population has been experiencing an unprecedented challenge due to Corona virus disease (COVID-19). A pandemic was declared by the World Health Organization on March 11th 2020, with an escalation of new cases worldwide. Dermatology units experienced a reorganization of regular activity, also providing clinical diagnosis and medical assistance to COVID-19-positive patients who developed cutaneous manifestations. Objective: To evaluate the impact of the COVID-19 pandemic on Italian dermatologic clinical practice. Materials & Methods: This was a prospective online survey, consisting of a questionnaire with 35 multiple-choice questions uploaded on the website of the Italian Society of Dermatology and Venereology — SIDeMaST. Results: A total of 136 dermatologists, 78 women (57%) and 58 men (43%), participated in the survey. The mean age was 58 ± 14 years. In total, 60% of participants reported an impact of the pandemic on their practice, in most cases consisting of a remarkable reduction in routine clinical activity (58%). Concern regarding possible infection was evaluated with a score ranging from 0 (no concern) to 5 (extremely concerned): the fear of becoming infected was high (≥3 in 40%), as was the fear of infecting families, colleagues or patients (≥3 points in 45%). Conclusion: The COVID-19 pandemic is having a strong impact on dermatology practice in Italy. The identification of critical points may help scientific societies to improve the clinical scenario and create specific strategies to overcome the emergency.

Abstract

In soils rich in organic matter (OM), such as those in the elevations of South Brazil, the mineralized N from organic matter does not always meet the N demands of grapevines, thus, N applications are necessary during vine growth. However, the optimal N rate to achieve satisfactory yield and desirable grape enological composition is not known. The study aimed to evaluate yield and must composition of grapevine subjected to N applications, in relation to temperature and rainfall. The experiment was conducted in a vineyard planted in 2003 in the city of Água Doce, located in the Midwest region of the state of Santa Catarina, southern Brazil. The cultivar ‘Chardonnay’ grafted on rootstock 1103 Paulsen, planted in clay, high-OM soil was trained as a spur pruned cordon system. From 2011 to 2014, the grapevines were submitted to the application of N as urea at the following rates: 0, 20, 40, 80 and 120 kg N ha-1 split at bud sprout and full bloom. Leaves, collected at flowering and veraison, were dried, milled and analyzed for total N. At harvest, the number of clusters per plant and yield were recorded. Total soluble solids, titratable acidity, pH and tartaric acid were evaluated on must. Temperature and rainfall data were collected throughout the experiment. The application of N increased N concentration in leaves but had little effect on yield and must composition. According to principal component analysis, a negative correlation between rainfall and N in must was found. The highest N concentration in must was observed in crop season 2011/2012 with low rainfall; the opposite occurred in 2013/2014.
