Background/aim: It is still not known how an immunosuppressive state affects the response to coronavirus disease 2019 (COVID-19) in children and adolescents. The aim of this study was to evaluate clinical characteristics, outcomes, and follow-up results of COVID-19 in pediatric patients with a history of immunocompromise or malignancy, retrospectively. Materials and methods: Patients with a diagnosis of COVID-19 who were under 18 years of age and had a history of immunosuppressive chronic disease or under immunosuppressant treatment were included in the study. Patients were applied to our outpatient clinic or consulted to our department in a tertiary center during the first year of the pandemic. Results: We evaluated 18 patients with a median age of 15.0 (0.6???17.8) years. Twelve patients (66.6%) were tested because of a symptom and the most common symptom was fever (44.4%, n = 8). Ten of the symptomatic patients (55.5% of all cohort) had a mild disease, the remaining two patients (11.1%) with an end-stage malignancy had critical diseases. Twelve patients (66.7%) were managed on an outpatient basis and were followed up at home, while the remaining six (33.3%) required hospitalization. One patient, who had Ewing sarcoma, died during the follow-up in the intensive care unit, and others were recovered without any morbidities. Lymphocyte (LYM) counts were significantly lower, C-reactive protein (CRP), and ferritin levels were higher in the individuals that needed hospitalization (p = 0.039, 0.027, and 0.039, respectively). Conclusion: Immunocompromised children and adolescents with COVID-19 should be monitored closely, especially those with an end-stage malignancy, low LYM count, or high CRP and ferritin levels.

Objective: Immunoglobulin A (IgA) vasculitis (IgAV), also known as Henoch-Schönlein purpura (HSP), is a vasculitis characterized by the accumulation of IgA in the vessel walls. In this study, we purposed to evaluate the demographics, clinical and laboratory findings, and treatment and treatment responses of patients diagnosed with IgAV/HSP in our center. Methods: The records of 201 IgAV/HSP patients who were followed up in the pediatric nephrology-rheumatology clinic were evaluated retrospectively. Results: It was seen with the equal frequency between girls and boys. While all patients had purpura, other findings were gastrointestinal, joint, renal, subcutaneous edema, and testicular involvement, in order of frequency. The rate of patients who developed intussusception was 2.5%, and none required surgical treatment. Biopsy was performed in patients with persistent proteinuria or hematuria. Histopathological diagnoses were mesangial proliferation, crescent, and minimal change, respectively. While the rate of renal involvement was high in cases with rash and relapse (p=0.046), there was no difference in gastrointestinal and joint involvement. In the histopathological findings of the boys, the crescent was higher than in the girls (p=0.017). Conclusion: IgAV/HSP generally has a good prognosis, but some patients suffer from renal involvement. In our study, renal histopathology in cases with renal involvement showed milder findings in girls than in boys, but there was no difference in other findings. Renal involvement was higher in relapsed patients.

Amaç:Hipertansiyon (HT) yaygın görülen ve hedef organ hasarıyla ilişkili olan bir sağlık problemidir. Fazla kilolu olmak, obezite, erkek cinsiyet, yüksek sodyum alımı primer HT için etiyolojik risk faktörleridir. Vücut kitle indeksi (VKİ) obeziteyi tahmin etmenin en pratik ve yaygın kullanılan yöntemidir. Ancak abdominal obezite ve bel çevresinin VKİ’den bağımsız olarak HT ile ilişkili olduğu ve VKİ’nin vücut yağ dağılımını dikkate almadığı gösterilmiştir. Vücut yağ yüzdesi (VYY)HT gibi obezite ilişkili hastalıkların gelişimi açısından tek başına yüksek VKİ’den daha önemlidir. Ayrıca sıvı ve iyonik denge ile kan basıncı (KB) arasında bir ilişki vardır. Biyoelektrik impedans analizi (BİA), VYY ve vücut su dağılımını belirlemek için en yaygın kullanılan yöntemdir. Çalışmamızın amacı çocuklarda KB ile vücut kompozisyonu arasındaki ilişkiyi açıklamaktır.Gereç ve Yöntem:Ayaktan kan basıncı monitörizasyonu (AKBM) ile çocuklar hipertansif (Grup 1, n:46) ve normotansif (Grup 2, n:31) olarak gruplandırıldı. Tüm hastalara BİA (Bodystat Qudscan 4000, Bodystat Limited, British Isles) yapıldı ve VKİ, VYY, vücut yağ kitle indeksi, yağsız kitle indeksi, total vücut suyu (TVS), hücre içi sıvısı (HİS), hücre dışı sıvısı (HDS), bazal metabolizma hızı, tahmini günlük kalori ihtiyacı ve impedans değerleri kaydedildi. Gruplar antropometrik ve demografik özellikler, laboratuvar parametreleri ve vücut kompozisyonu açısından karşılaştırıldı. Ayrıca bu değişkenlerin ortalama kan basıncı (KB) (sistol, diyastol ve toplam) ve KB yükleri (gece ve gündüz) ile korelasyonu değerlendirildi.Bulgular:Grup 1’de 46 Grup 2’ de 31 çocuk vardı. Her iki grupta da yaş ve cinsiyet benzerdi. Vücut ağırlığı, VKİ, VKİ SDS, bel çevresi, kalça çevresi, bel/kalça oranı Grup 1’ de Grup 2’ ye göre yüksekti (sırasıyla p=0,012, p=0,000, p=0,000, p=0,000, p=0,005, p=0,007). Kan üre azotu ve ürik asit Grup 1’de Grup 2’ ye göre fazlaydı (sırasıyla p=0,028, p=0,003). VYY, vücut yağ kütlesi, vücut yağ kitle indeksi, yağsız kitle indeksi, TVS, bazal metabolizma hızı ve tahmini günlük kalori ihtiyacı Grup 1’ de Grup 2’ ye göre yüksekti (sırasıyla p=0,030, p=0,000, p=0,040, p=0,011, p=0,013, p=0,020, p=0,045). HDS ise Grup 1’ de Grup 2’ den küçüktü (p=0,000). Ayrıca sistolik gündüz ve gece ortalama KB değerleri ile vücut yağı kütlesi yağsız vücut kütlesi, yağsız kitle indeksi, kuru ağırlık, TVS, HDS, HİS, bazal metabolizma hızı arasında pozitif korelasyon belirlendi (pSonuç:Vücut ağırlığı, VKİ, bel çevresi, kalça çevresi, VYY, yağ kitle indeksi ve yağsız kitle indeksi, TVS, HDS, HİS arttıkça sistolik KB ortalama değerleri de artmaktadır.Anahtar Kelimeler:Sistolik ve diyastolik kan basıncı, vücut kompozisyonu&nbsp

All rights reserved.Anetoderma is a rare cutaneous disorder characterized by the loss of normal elastic tissue, clinically manifested as atrophic patches in the body and extremities. Immunological mechanisms may play a role in this process. Here, we report an 11-year-old boy with systemic lupus erythematosus who presented with numerous well-circumscribed atrophic lesions on the trunk and limbs. Histopathological examination showed that elastic fibers were sparse in the superficial dermis and almost completely absent in the mid dermis. This finding was consistent with anetoderma. There were not antiphospholipid antibodies, but D-dimer level was high. We think that anetoderma originated from a developing microthrombosis process, especially as supported by the increased D-dimer.
