The use of bioactive glass as a mineralization agent in treating dentin hypersensitivity has been established for almost two decades where the melt-quench-derived silicate glass, 45S5, may be regarded as the gold standard formulation. Compared to silicate glasses, borate glasses are less chemically durable and have the potential to mineralize (i.e., convert to apatite) more rapidly. This study compared the apatite forming capacity of a sol-gel-derived calcium-borate glass, 40CaO–60B 2 O 3 (SGB60), to that of 45S5, on demineralized dentin discs and its ability to occlude tubules on non-demineralized dentin discs, in vitro. Moreover, it investigated the vapour sorption reactivity of the glasses to explore the potential correlation between their immediate aqueous interactions and bioactivity in remineralization solution. Compared to 45S5, SGB60 demonstrated more rapid calcium-phosphate layer formation on demineralized dentin surfaces. On the other hand, after 24 h immersion in remineralization solution, the 45S5-induced calcium-phosphate layer occlusion of tubules in non-demineralized dentin surfaces appeared to be qualitatively greater than that induced by SGB60. Moreover, vapour sorption analysis under either air or nitrogen gas, demonstrated that the reactivity of 45S5 with vapour varied under the different purge gasses, with significant carbonate formation under air. In contrast, SGB60 showed a more rapid rate of initial weight increase with vapour under both purge gasses, and this vapour reactivity may be correlated to its more rapid calcium-phosphate layer formation on demineralized dentin surfaces when immersed in remineralization solution. Objective: The use of immediate dentin sealing (IDS) following tooth preparation for indirect restorations is well‐supported by laboratory studies. The inclusion of IDS as a mandatory clinical step, it must be firmly supported by evidence from randomized clinical trials (RCT). The objective of this study is to assess whether IDS should be added as a mandatory procedure in all indirect restorations, taking into consideration clinically significant outcomes for the patient, such as restoration longevity and hypersensitivity. Materials and Methods: A bibliographic search was carried out in the PubMed. In addition, a manual search was conducted in the references of literature reviews. Results: Data from four RCT were included. Two studies assessed preparations for full crowns, and two assessed preparations for partial restorations. Data from full crown preparations were subjected to a meta‐analysis, revealing a reduction in hypersensitivity incidence in the IDS group 1 week after restoration cementation. With respect to hypersensitivity, IDS seems to offer advantages exclusively during the provisional phase and up to 1 week following the cementation of the final restoration. One study has shown no difference on longevity. Conclusion: The use of IDS should be considered as an elective clinical step during the rehabilitation with indirect restorations. This research evaluated the dentinal tubule occlusion capabilities of titanium dioxide-doped phosphate glass (TDPG) paste, with and without adjunctive Nd:YAG laser treatment in comparison to nanohydroxyapatite (n-HA) that also contains calcium phosphate. In total, forty etched dentin (ED) discs were allocated into eight groups: untreated ED (G1), laser-treated ED (G2), TDPG paste-applied ED (G3), TDPG paste followed by laser (G4), TDPG microspheres paste-applied ED (G5), microspheres paste followed by laser (G6), n-HA paste-applied ED (G7), and n-HA paste followed by laser (G8). The interventions were assessed using scanning electron microscopy (SEM) for morphological changes, counting opened dentinal tubules and elemental analysis. Additionally, the study incorporated an evaluation of radiological properties, specifically the materials' mass attenuation coefficients, effective atomic numbers, and exposure buildup factors, to ascertain their compatibility with X-ray imaging modalities. Findings indicated that paste application alone created a layer of precipitated crystals, effectively occluding dentinal tubules. Subsequent laser treatment enhanced occlusion by reducing opened dentinal tubules by approximately 50%, created a dense layer of altered TDPG or n-HA crystals with modified phosphorus and calcium composition. The inclusion of radiological assessment suggested that these materials, particularly when combined with laser, have potential not only for treating dentin hypersensitivity but also compatible with radiographic diagnostic processes. Background: Keratin pearls are foci of central keratinization within concentric layers of squamous cells that can form under the clitoral prepuce and cause pain (clitorodynia); in-office removal of keratin pearls may reduce clitoral pain and improve sexual function. Aim: This study aims to investigate clitoral pain and sexual function in women with partial clitoral phimosis and keratin pearls before and after in-office lysis of clitoral adhesions with keratin pearl excision (LCA-KPE). Methods: A pre-post interventional study evaluated patients who underwent LCA-KPE between January 2017 and February 2023 in 2 metropolitan gynecology clinics specializing in vulvar pain. Patients presenting with keratin pearls and partial clitoral phimosis identified through retrospective chart review were asked to complete postprocedure questionnaires and provide subjective responses on clitoral discomfort, sexual function, sexual distress, and their experience with in-office LCA-KPE. Bivariate analyses with paired t tests were conducted to determine the effect of LCA-KPE. Qualitative data were analyzed with thematic coding. Outcomes: An 11-point pain visual analog scale was utilized to determine pre- and postprocedure clitoral discomfort and difficulty with orgasm. Female sexual dysfunction was measured with the Female Sexual Function Index (FSFI) and Female Sexual Distress Scale–Revised. Results: A total of 32 of 74 patients who met inclusion criteria completed postprocedure surveys (43% response rate). Mean clitoral pain for respondents was 6.91 at baseline and 2.50 after LCA-KPE (P <.001). Mean difficulty with orgasm was significantly decreased from 5.45 at baseline to 3.13 after LCA-KPE (P <.001). Participants had a mean FSFI total score of 17.68 after treatment compared with a mean total baseline FSFI of 12.12 (P =.017). The mean FSFI score for pain was 2.43 at follow-up compared with 1.37 at baseline (P =.049). There was no significant difference in the mean Female Sexual Distress Scale–Revised score before vs after the procedure (P =.27). Qualitative themes described the procedure as painful but worthwhile, with 77% of participants reporting the overall experience as positive. Recurrence rate overall was 28%, with a median of 2 repeat procedures. Clinical Implications: Recognizing keratin pearls as a structural cause of clitoral pain and offering in-office treatment is an important tool in addressing clitorodynia and improving sexual function. Strengths and Limitations: This is the largest study to date documenting the occurrence, identifying associated pain conditions, and evaluating procedural outcomes for clitoral keratin pearls. Objective: This randomized controlled trial aimed to evaluate the equivalence in the color change, adverse effects, self-perception (AS) and the impact on oral condition (IO) of participants submitted to different application protocols of in-office dental bleaching. Materials and methods: 165 participants were bleached with a 35% hydrogen peroxide gel (Total Blanc Office One-Step, DFL), according to the following protocols: (1) 2 applications of 20-min each (2 × 20 min); (2) 1 × 40-min and; (3) 1 × 30-min. The color change was evaluated with the Vita Easyshade spectrophotometer, Vita Classical and Vita Bleachedguide scales. The intensity and risk of tooth sensitivity (TS) and gingival irritation (GI) were recorded using a 0–10 visual analogue scale (VAS). AS and IO was assessed before and after the bleaching procedure using the Orofacial Aesthetic Scale and Oral Health Impact Profile-14, respectively. Results: Equivalent color change were observed (p < 0.001), with no significant difference between groups. The group 2 × 20 min presented the highest risk of TS (76%, 95% CI 63 to 85), compared to the 1 × 30 min (p < 0.04). The intensity of TS and GI and the risk of GI was similar between groups (p > 0.31). Irrespectively of the group (p = 0.32), significant improvements were observed for all items of AS and IO after bleaching (p < 0.02). Conclusions: The 1 × 30 min protocol produced equivalent color change to the other bleaching protocols with reduced risk of TS and shorter application time. Clinical relevance: A more simplified application regimen of a single application of 30 min yields effective bleaching and patient satisfaction while minimizing undesirable side effects and improving patient satisfaction. The authors evaluated the synergistic analgesic effect of preemptive administration of 400 mg of ibuprofen (IBU) and potassium fluoride 2% (KF2), both in isolation or combination, on the painful perception of tooth sensitivity after tooth bleaching. Fifteen patients participated in this triple-blind, randomized, placebo-controlled clinical trial. The study used a crossover design for drug administration and a split-mouth design for desensitizer agent. Four paired groups were formed: IBU plus KF2 (IBU + KF2); IBU (IBU + placebo KF); KF2 (placebo IBU + KF2), and placebo (placebo IBU + placebo KF). The outcome measure was the perception of tooth sensitivity, assessed using a visual analog scale. Data were collected at 4 different times: immediately after tooth bleaching (baseline) and after 6, 30, and 54 hours. Statistical analysis was performed using the Friedman test and relative risk. IBU plus KF2 was found to be more effective in reducing tooth sensitivity immediately after bleaching (baseline) compared with the placebo group (P <.05). The risk of experiencing moderate or severe tooth sensitivity was approximately 4 times higher in the placebo group than in the IBU plus KF2 group (relative risk, 4.00; 95% CI, 1.01 to 15.81; P =.025). The combined use of 400 mg of IBU and KF2 appears to be beneficial in managing postbleaching tooth sensitivity. It provides a superior analgesic effect compared with placebo. The preemptive administration of IBU plus KF2 reduces tooth sensitivity after tooth whitening when compared with placebo. Background: Atrophic scars are white, dermal depressions, caused by the destruction of collagen fibers and decrease in epidermal cells, following inflammation after different types of trauma. They lead to significant physical, aesthetic and psychological barriers and their treatment remain a therapeutic challenge for dermatologists. Microneedling has been shown to improve scars by stimulating angiogenesis and neocolagenesis and the combination of anti‐fibrotic drugs could potentialize the results. Methods: We present 8 cases of patients with linear scars, successfully treated with two sessions of a new Microneedling technique, using a tattoo machine, associated with drug delivery of 5‐FU. Results: A marked improvement in scar pigmentation and texture were noted by patients and doctors, 6 months following the sessions of MMP and drug delivery with 5‐FU, in different body sites. We also showed that the assessment scores of at least one of the professionals with those of the patient had significant correlations with each other, which shows consistency between the qualitative assessment instruments. We also showed that the cause of the injury can influence joint assessment scores (physicians plus patient) or those exclusive to professionals trained for the assessments, generating evidence that the cause of the injury can influence the treatment outcome itself. Conclusions: We present an inexpensive and promising approach that can be easily done as an in‐office procedure. Objectives: This study aimed to investigate the effect of antioxidant (sodium ascorbate) on tooth sensitivity after two in‐office bleaching techniques (light‐activated and chemical bleaching). Materials and Methods: Ten patients aged 18–25 were bleached in four groups according to bleaching materials and/or antioxidant used. Group A: Maxillary right quadrant received light‐activated bleaching with antioxidant. Group B: Maxillary left quadrant received light‐activated bleaching without antioxidant. Group C: Mandibular right quadrant received chemical bleaching without antioxidant. Group D: Mandibular left quadrant received chemical bleaching with antioxidant. The tooth sensitivity was recorded using Numerical Rating Scale (NRS) and Schiff Scale (SS) immediately after treatment, 1 day, 1 week, 2 weeks, and 1 month follow‐up periods. Results: Group B exhibited higher sensitivity values than Group A. This difference was statistically significant on the first day after treatment (p = 0.027* in NRS and p = 0.046* in SS). Furthermore, antioxidant incorporation in Group D led to a reduction in teeth sensitivity values compared to Group C. This disparity was highly significant on the first day after treatment (p = 0.001* in NRS and p < 0.001** in SS). Conclusions: The antioxidant (10% sodium ascorbate) reduces the intensity of tooth sensitivity at the different follow‐up periods, especially after 1 day from bleaching. Objective Materials and Methods Results Conclusion Clinical Significance To compare the color change, the risk and intensity of tooth sensitivity (TS), and gingival irritation (GI) of at‐home bleaching applied on the buccal surface only or the buccal and lingual surfaces.Sixty patients with canines A2 or darker were selected and their superior arches were randomized in two groups: at‐home bleaching on the buccal‐only or on the buccal and lingual surfaces, with 7.5% hydrogen peroxide, for 1 h daily/2 weeks. The color change was evaluated at baseline, 7, 14 days, and 1 month after bleaching using shade guides scales (ΔSGU) and a spectrophotometer (ΔEAB, ΔE00, and ΔWID). Risk and intensity of TS and GI were recorded daily using visual analogic scale (0–10). Patient satisfaction was evaluated with the orofacial esthetics. Paired t‐test, McNemar's, and Wilcoxon signed‐rank test were used for data analysis (α = 5%).Neither the color change nor the risk/intensity of TS was statistically different between groups (p > 0.05). Patient satisfaction increased after bleaching for both groups (p < 0.05).The addition of one contact surface does not result in an increased whitening degree compared to bleaching applied solely on the buccal surface.Understanding the influence of surfaces interacting with the bleaching agent is crucial for comprehending the bleaching mechanism and avoiding unnecessary material expenses. Direct pulp capping (DPC) is a conservative approach for preserving tooth vitality without requiring more invasive procedures by enhancing pulp healing and mineralized tissue barrier formation. We investigated the effectiveness of Platelet Rich Plasma (PRP) vs. Mineral Trioxide Aggregate (MTA) as a DPC agent. Forty-two teeth from three mongrel dogs were divided into two equal groups. After three months, the animals were sacrificed to evaluate teeth radiographically using cone-beam computerized tomography, histopathologically, and real-time PCR for dentin sialophosphoprotein (DSPP), matrix extracellular phosphoglycoprotein (MEPE), and nestin (NES) mRNA expression. Radiographically, hard tissue formation was evident in both groups without significant differences (p = 0.440). Histopathologic findings confirmed the dentin bridge formation in both groups; however, such mineralized tissues were homogenous without cellular inclusions in the PRP group, while was osteodentin type in the MTA group. There was no significant difference in dentin bridge thickness between the PRP-capped and MTA-capped teeth (p = 0.732). The PRP group had significantly higher DSPP, MEPE, and NES mRNA gene expression than the MTA group (p < 0.05). In conclusion, PRP enables mineralized tissue formation following DPC similar to MTA, and could generate better cellular dentinogenic responses and restore dentin with homogenous architecture than MTA, making PRP a promising alternative DPC agent. Aim: This study aimed to know the potential of diode light amplification by stimulated emission of radiation (LASER) and desensitizing agents on blocking the dentinal tubules, which may further help in reducing the most common dental problem named dentinal hypersensitivity (DH). Materials and Methods: Sixty human permanent bicuspids were selected, and preparation was performed in the cervical region measuring 2.5 mm × 2.5 mm × 2.5 mm, followed by acid etching for 15 seconds. These are then divided according to the groups mentioned: group 1—nano-hydroxyapatite (HA) was applied for 15 minutes; group 2—biosilicate was applied for 15 minutes; group 3—diode LASER application was performed twice for 60 seconds in noncontact mode; group 4—nano-HA plus diode LASER application; and group 5—biosilicate plus diode LASER application. The same procedure was followed on the 1st, 7th, and 14th days and then rinsed and stored in artificial saliva, and the saliva was changed every 24 hours. The dentinal tubular occlusion was observed under a scanning electron microscope (SEM) after the 14th day. Result: The maximum dentinal tubular occlusion was observed in group 4, where nano-HA and diode LASER application was performed. Conclusion: All the investigated treatment groups have promising occluding potential, but the maximum was found in group 4 >group 5 >group 3 >group 1 >group 2. Aim: Although dentine hypersensitivity is widespread, can cause substantial pain and impact quality of life, it is not routinely discussed during dental consultations. This qualitative study aimed to develop an understanding of the barriers and facilitators to these discussions. Materials and Methods: Using the Theoretical Domains Framework to shape the topic guide, N = 7 online focus groups were organized with a total N = 40 participants comprising experienced dentists, dental foundation trainees and dental care professionals. Inductive and deductive thematic analyses of the anonymized, transcribed focus group conversations were undertaken. Results: An attitude–behaviour gap was observed in dental teams' accounts. Although they saw it as part of their professional role to routinely discuss sensitivity, and believed that such conversations were 'an easy win', in practice they experienced several behavioural barriers that hindered these conversations from taking place. These included competing priorities, a perceived lack of seriousness and assessment of dentine hypersensitivity and practical issues such as time. Conclusions: Systemic (e.g., lack of time and training, professional culture) and behavioural (e.g., dental teams' belief that conversations should take place only with patients likely to be adherent) barriers to dentine hypersensitivity conversations explain why these conversations do not routinely take place. A prospective observational case series included six patients who presented with discoloured upper and lower teeth extending from the right second premolar to the left second premolar. The photoactivation dosimetry and treatment protocol were as follows: λ 450 nm, 1 W, CW; flattop beam profile; 1 cm2; 15 J/spot; 10 irradiated spots; an irradiation time of 15 s/spot; three whitening cycles in a single session. Blanc One ULTRA+ was the bleaching agent. A visual analogue scale (VAS) was utilised to evaluate the pain intensity and dental hypersensitivity during treatment immediately after complete treatment (T1), 24 h (T2), and 8 h (T3) postoperatively, and at an 8-month follow-up timepoint (T4), whereas the dental colour shade change was assessed using the VITA colour shade guide pre-treatment (T0), T1, and T4. The Gingival index and modified Wong Baker faces scale were utilised to evaluate gingival inflammation and patients' treatment satisfaction, respectively. Our findings revealed a reduction in the dental colour shade of the six cases between 2 and 10- fold (average of 3.5-fold) at T1 and maintained at T4, indicating significant improvement in the colour shade change with optimal outcomes. The percentage of this improvement for all the patients was ranged between 16.6% and 33.3%. At all timepoints, a "0" score was provided for pain intensity, dental hypersensitivity, and gingival inflammation. Our study demonstrates the feasibility and safety of a λ 450 nm laser delivered with a flattop handpiece to achieve optimal whitening outcomes without adverse effects. This offers a useful guide for dental clinicians for vital in-office tooth whitening. Extensive clinical studies with large data are warranted to validate our study protocol. [ABSTRACT FROM AUTHOR]

The aim of this study was to assess the impact of molar-incisor hypomineralisation (MIH) on oral health-related quality of life (OHRQoL) in children and adolescents, including information on restorative care, tooth sensitivity, as well as sociodemographic factors. Thirty-five patients aged between 7 and 17 years underwent a comprehensive oral examination. Severity of MIH was graded using the MIH Treatment Need Index (MIH-TNI), OHRQoL using the Child Oral Health Impact Profile (COHIP-19). Clinical quality of restorations was assessed according to modified FDI-criteria, tooth sensitivity using the Schiff Cold Air Sensitivity Scale (SCASS). The mean age was 11.3 ± 3.0 years, 34% were female. On average, 6.9 ± 2.8 teeth were affected, 62,9% had hypersensitive teeth (SCASS ≥ 1). Eighty-nine percent of patients had received restorative care, with a mean of 3.3 ± 2.1 teeth restored, most often with composite, followed by fissure sealing. Nine percent of restorations failed by the FDI-criteria. Mean estimated survival times for success were 4.9 years (95% CI 3.5; 6.2) and 5.6 years (95% CI 5.0; 6.3) for fissure sealants and composite restorations, respectively. The mean COHIP-19 score was 64.3 ± 8.2 (max. possible score = 76). A higher severity of MIH-TNI correlated significantly with impaired OHRQoL (rs = − 0.38, p = 0.013). However, this was not mirrored in multiple regression analysis. Despite the high rate of restorative treatment with an acceptable failure rate, OHRQoL is reduced in children with MIH. Background Primary palmar hyperhidrosis causes a lot of problems for patients and negatively affects their quality of life. Currently, iontophoresis with tap water and aluminum chloride hexahydrate is used for primary palmar hyperhidrosis. Yet, little evidence exists about iontophoresis with aluminum chloride hexahydrate in the form of gel. This study investigated the effect of aluminum chloride hexahydrate gel iontophoresis compared to tap water iontophoresis on primary palmar hyperhidrosis. Methods In this randomised controlled trial study, 32 patients with primary palmar hyperhidrosis were divided randomly into two groups (n = 16). Participants received 7 sessions of iontophoresis with aluminum chloride hexahydrate gel or tap water every other day on the dominant hand. The sweating rate was measured by gravimetry and iodine–starch tests before and after the last treatment session. Results Following the iontophoresis, the rate of sweating in both hands in the two groups was significantly reduced (P < 0.001). However, the sweating rate in the treated hand and the non-treated hand showed no significant difference. There was no significant difference observed in sweating rate reduction between both groups over time, but the larger effect size values observed in the aluminum chloride hexahydrate gel iontophoresis group may suggest the superiority of this gel over tap water in reducing the rate of sweating. Limitations Further investigations with longer follow-up are needed to confirm the hypothesis regarding the effectiveness of aluminum chloride hexahydrate gel iontophoresis over other types of iontophoresis. In addition, contraindications of iontophoresis such as pregnancy, pacemakers, and epilepsy should be considered. Conclusion The present study provides preliminary evidence suggesting that aluminum chloride hexahydrate gel iontophoresis is an effective alternative treatment to decrease sweating rate in extended areas with fewer side effects in patients with primary palmar hyperhidrosis. Introduction: This in vitro study aimed to evaluate the effect of three different chemical agents on stain removal and mineral uptake of artificial dentin caries (ADC) lesions treated with silver diamine fluoride (SDF). Methods: Baseline L*a*b* values were determined in polished human permanent dentin blocks, and ADC lesions were induced with an acid gel for 1 week. Samples were assigned to four groups; in three groups, half of each sample received SDF (30% SDF for 3 min), while the other half received SDF followed by a bleaching treatment protocol (garlic extract, bentonite, or 35% hydrogen peroxide). The fourth group had one SDF-treated half and one half without SDF. Color changes (ΔE) were measured by spectrometry, and transversal microradiography was used to quantify integrated mineral loss (ΔZ) 24 h after treatment (SDF or SDF + bleaching). A two-way mixed ANOVA was applied to thirty percent. Results: SDF application increased mineral uptake by ADC (p = 0.001). The type of chemical agent evaluated (p < 0.0001), time (p = 0.01), and their interaction (p < 0.0001) bleached the ADC treated with SDF. However, 35% hydrogen peroxide was the only compound with a bleaching effect (p < 0.001), without returning to baseline color. None of the compounds altered the mineral uptake effect of SDF (p = 0.30). Conclusion: This in vitro study showed mineral uptake effect in ACD within 24 h after SDF application and the ability of hydrogen peroxide to partially remove (reduction of 24%) the staining caused by SDF without affecting its mineral uptake effect. Dentinal hypersensitivity (DHS) affects about 1.3%-92.1% of the population globally and has a significant impact on the quality of life of the affected individuals. Desensitizing agents that directly block the dentinal tubules or cause chemical blocking of the pulpal nerve endings are commonly recommended for the management of DHS. Potassium oxalate (KO) has been used as a desensitizing agent in the management of DHS. This narrative review aimed to assess the literature related to the efficacy of KO as a desensitizing agent, with a focus on mouth rinses containing 1.4% KO. A comprehensive search of the literature was carried out using PubMed and Google Scholar with relevant keywords. The results from the retrieved articles were assessed, and the findings regarding the efficacy of KO in managing DHS were analyzed. Studies suggest that the usage of 1.4% KO mouth rinses results in the occlusion of dentinal tubules by the formation of acid-resistant calcium oxalate crystals within dentinal tubules and on the dentine surface. These calcium oxalate crystals tend to form a strong bond with the inner surface of the dentinal tubules and lower the hydraulic conductivity of the dentine. (1) Background: Congenital erythropoietic porphyria (CEP), named Günther's disease, is a rare recessive type of porphyria, resulting from deficient uroporphyrinogen III synthase (UROS), the fourth enzyme of heme biosynthesis. The phenotype ranges from extremely severe perinatal onset, with life-threatening hemolytic anaemia, to mild or moderate cutaneous involvement in late-onset forms. This work reviewed the perinatal CEP cases recorded in France in order to analyse their various presentations and evolution. (2) Methods: Clinical and biological data were retrospectively collected through medical and published records. (3) Results: Twenty CEP cases, who presented with severe manifestations during perinatal period, were classified according to the main course of the disease: antenatal features, acute neonatal distress and postnatal diagnosis. Antenatal symptoms (seven patients) were mainly hydrops fetalis, hepatosplenomegaly, anemia, and malformations. Six of them died prematurely. Five babies showed acute neonatal distress, associated with severe anemia, thrombocytopenia, hepatosplenomegaly, liver dysfunction, and marked photosensitivity leading to diagnosis. The only two neonates who survived underwent hematopoietic stem cell transplantation (HSCT). Common features in post-natal diagnosis (eight patients) included hemolytic anemia, splenomegaly, skin sensitivity, and discoloured teeth and urine. All patients underwent HSCT, with success for six of them, but with fatal complications in two patients. The frequency of the missense variant named C73R is striking in antenatal and neonatal presentations, with 9/12 and 7/8 independent alleles, respectively. (4) Conclusions: The most recent cases in this series are remarkable, as they had a less fatal outcome than expected. Investigation the protective effect of transient receptor potential channel modulator 2-Aminoethoxydiphenyl Borate (2-APB) on aminoglycoside nephrotoxicity caused by reactive oxygen species, calcium-induced apoptosis and inflammation was aimed. Forty Wistar rats were divided (n=8) as follows: Control group; DMSO group; 2-APB group; Gentamicin group (injected 100 mg/kg gentamicin intramuscularly for 10 days); Gentamicin+ 2-APB group (injected 2 mg/kg 2-APB intraperitoneally, then after 30 minutes 100 mg/kg gentamicin was injected intramuscularly for 10 days). Blood samples were collected for biochemical analyses, kidney tissue samples were collected for light, electron microscopic and immunohistochemical investigations. In gentamicin group glomerular degeneration, tubular dilatation, vacuolization, desquamation of tubular cells and hyaline cast formation in luminal space and leukocyte infiltration were seen. Disorganization of microvilli of tubular cells, apical cytoplasmic blebbing, lipid accumulation, myelin figure like structure formation, increased lysosomes, mitochondrial swelling and disorganization of cristae structures, apoptotic changes and widening of intercellular space were found. TNF-α, IL-6 and caspase 3 expressions were increased. BUN and creatinine concentrations were increased. Increase in MDA levels and decrease in SOD activities were determined. Even though degeneration still continues in gentamicin+2-APB treatment group, severity and the area it occupied were decreased and the glomerular and tubule structures were generally preserved. TNF-α, IL-6, caspase 3 immunoreactivities and BUN, creatinine, MDA concentrations were reduced and SOD activities were increased markedly compared to gentamicin group. In conclusion, it has been considered that 2-APB can prevent gentamicin mediated nephrotoxicity with its anti-oxidant, anti-apoptotic and anti-inflammatory effects. [ABSTRACT FROM AUTHOR]

This study aims to identify the two-dimensional and three-dimensional analyses and evaluate the loss of tooth structure in the tooth's different constituent elements and the ceramic antagonist's surface. Methods: In this study, three groups (n = 10) represented by different ceramic systems (lithium disilicate [GDis], lithium silicate reinforced with zirconia [GSil], and monolithic zirconia [GZir]) were evaluated. Each group obtained ten ceramic blocks and submitted them to the sintering/crystallization process. To carry out the wear test, healthy mandibular premolars were used as the specimens and the ceramic blocks as antagonists. The premolars were submitted to two-dimensional and three-dimensional analyses using a computerized microtomography (µTC) before and after the wear test. The wear test was performed with 30 N and 300,000 cycles load, with occlusion, laterality, and disocclusion movements. Results: The statistical analysis comparing the loss of two-dimensional tooth structure showed a statistically significant difference among all groups (p < 0.05). Statistical analysis comparing the percentage of loss of three-dimensional tooth structure showed a statistically significant difference between groups GDis and GSil and between groups GDis and GZir. However, when comparing GSil with GZir, no statistically significant difference was found. The qualitative analysis of the teeth showed that GDis showed considerable enamel loss and dentin exposure, GSil showed enamel wear with flattening the cusp without dentin exposure, and GZir showed minimal enamel wear without dentin exposure. In the qualitative analysis of ceramic antagonists, more significant wear of the ceramic material for GDis was observed, followed by the GSil and GZir groups, respectively. Significance: The use of the lithium disilicate should be cautious, restricting it to areas with lower masticatory forces. Areas of higher masticatory forces showed a large amount of antagonist wear, with dentin involvement. This can lead to dentinal hypersensitivity, risk of compromising patients' occlusion, by harming group disocclusion guides, causing pain and temporomandibular disorders. Background: Microlaryngoscopy (MLscopy) and direct laryngoscopy have been the traditional choice for diagnosing laryngeal masses among ENT surgeons, whereas transnasal fiber-optic bronchoscopy (FOB) is preferred by pulmonologists for various in-office endobronchial pathologies. The comparative accuracy and cost-effectiveness of these methods in diagnosing laryngeal masses have remained uncertain. Objective: The study aims to compare the histopathologic accuracy, diagnostic effectiveness, and cost-effectiveness of in-office FOB with MLscopy and direct laryngoscopy in patients with laryngeal masses. Methods: This study involved 70 patients with suspicious laryngeal or vocal cord masses. Patients with significant comorbidities underwent FOB, while others underwent MLscopy. FOB was performed under local anesthesia in an office setting, and MLscopy was conducted under general anesthesia with required hospitalization. The study compared the histopathologic accuracy, duration of hospital stay, and costs between the two methods. Results: FOB proved to be less time-consuming and more cost-effective, with patients spending about three hours for the procedure at a cost of 4600, as compared to those undergoing MLscopy, who were hospitalized for at least 12 hours at a cost of 22000. While both methods showed high histopathologic accuracy, FOB was slightly less accurate than MLscopy. Conclusion: Both in-office FOB and MLscopy are effective in diagnosing laryngeal masses. However, FOB offers a more cost-effective and efficient alternative, particularly suitable for patients with comorbidities or in settings where reduced hospital stay and cost are priorities. The study underscores the importance of adopting patient-friendly, cost-effective diagnostic procedures in healthcare, especially in resource-constrained settings like India. Social jetlag is associated with physical and mental health problems. With the increased popularity of telework, we investigated a specific form of social jetlag that we termed "telework jetlag". This study aimed to clarify the relationship between telework jetlag—the difference in sleep and wake-up times between in-office and telework days—and mental health problems among Japanese hybrid workers. A cross-sectional study was conducted with 1789 participants from October to December 2021 using an online-based questionnaire. Telework jetlag, defined as the difference in the midsleep point between in-office and telework days, was investigated using two groups according to telework jetlag—those lagging <1 h versus ≥1 h. We used the six-item Kessler Scale as a nonspecific psychological distress scale for the outcome. Telework jetlag was significantly associated with psychological distress, and the ≥1 h group had a higher risk (odds ratio: 1.80) of developing high psychological distress (HPD) than the <1 h group in the multivariate analysis. Since most teleworkers are forced to have a hybrid work style that mixes going to work and teleworking, telework jetlag must be addressed to maintain the health of teleworkers. Enamel tissue, the hardest body tissue, which covers the outside of the tooth shields the living tissue, but it erodes and disintegrates in the acidic environment of the oral cavity. On the one hand, mature enamel is cell-free and, if damaged, does not regenerate. Tooth sensitivity and decay are caused by enamel loss. On the other hand, the tissue engineering approach is challenging because of the unique structure of tooth enamel. To develop an exemplary method for dental enamel rebuilding, accurate knowledge of the structure of tooth enamel, knowing how it is created and how proteins interact in its structure, is critical. Furthermore, novel techniques in tissue engineering for using stem cells to develop enamel must be established. This article aims to discuss current attempts to regenerate enamel using synthetic materials methods, recent advances in enamel tissue engineering, and the prospects of enamel biomimetics to find unique insights into future possibilities for repairing enamel tissue, perhaps the most fascinating of all tooth tissues. Cognitive processing therapy (CPT) is an effective treatment for posttraumatic stress disorder (PTSD); however, some patients do not improve to the same extent as others. It is important to understand potential factors that can be modified for better patient outcomes. This clinical trial implemented a three‐arm, equipoise‐stratified randomization design to allow for the accommodation of patient preference before randomization to one of three CPT treatment modalities: in‐home, in‐office, or telehealth. This study examined whether satisfaction with the modality, perceived stigma, expectations of therapy, and credibility of the therapist differed between modalities and whether these factors impacted treatment outcomes. We hypothesized that the contributions of these variables would depend upon whether participants opted out of any treatment arms and that these factors would predict treatment outcomes. Participants who endorsed less perceived stigma demonstrated larger reductions in PTSD symptom severity than those with similar levels of perceived stigma in the telehealth and in‐office conditions, η2 =.12–.18. Participants who endorsed lower satisfaction with their treatment modality and were assigned to the in‐home condition experienced larger PTSD symptom reductions than those with similar dissatisfaction in the telehealth and in‐office conditions, η2 =.20. The results show the robustness of evidence‐based therapies for PTSD given that dissatisfaction did not impede treatment success. In addition, they demonstrate that it is important for clinicians to address stigma before initiating evidence‐based therapies for PTSD. Strategies to address these factors are discussed. Objective: To evaluate the clinical relevance of using the XP-endo Finisher as a supplementary tool to improve the success of root canal treatment of posterior teeth with apical periodontitis, as assessed by 1-year follow-up. Methods: A randomized clinical trial was conducted with 92 posterior teeth with apical periodontitis. Root canal treatment was performed using a single reciprocating file, with or without the supplementary use of the XP-endo Finisher. The status of apical periodontitis was assessed using the periapical index (PAI) at baseline and 1 year follow-up. Changes on PAI indicated that the lesions were healed, healing, or not healed. Successful treatments were defined as healed or healing lesions without clinical symptoms. Chi-square analysis and logistic regression were used for data analysis (α = 0.05). Results: There was no significant difference in the distribution of healing status between the XP-endo Finisher group and the control group (p = 0.690). The success rates were also similar, with 81% in the XP-endo Finisher group and 78% in the control group. However, gender had a significant impact on success rates, with higher rates observed in females. Conclusions: The use of the XP-endo Finisher file as a supplementary tool did not affect the success rate of root canal treatment in posterior teeth with apical periodontitis. The findings indicate that the XP-endo Finisher file has limited clinical relevance in improving treatment outcomes for root canal treatment in posterior teeth with apical periodontitis. Objectives: To assess whether in animals or patients with ≥ 1 tooth extracted, hyaluronic acid (HyA) application results in superior healing and/or improved complication management compared to any other treatment or no treatment. Materials and methods: Three databases were searched until April 2022. The most relevant eligibility criteria were (1) local application of HyA as adjunct to tooth extraction or as treatment of alveolar osteitis, and (2) reporting of clinical, radiographic, histological, or patient-reported data. New bone formation and/or quality were considered main outcome parameters in preclinical studies, while pain, swelling, and trismus were defined as main outcome parameters in clinical studies. Results: Five preclinical and 22 clinical studies (1062 patients at final evaluation) were included. In preclinical trials, HyA was applied into the extraction socket. Although a positive effect of HyA was seen in all individual studies on bone formation, this effect was not confirmed by meta-analysis. In clinical studies, HyA was applied into the extraction socket or used as spray or mouthwash. HyA application after non-surgical extraction of normally erupted teeth may have a positive effect on soft tissue healing. Based on meta-analyses, HyA application after surgical removal of lower third molars (LM3) resulted in significant reduction in pain perception 7 days postoperatively compared to either no additional wound manipulation or the application of a placebo/carrier. Early post-operative pain, trismus, and extent of swelling were unaffected. Conclusions: HyA application may have a positive effect in pain reduction after LM3 removal, but not after extraction of normally erupted teeth. Clinical relevance: HyA application may have a positive effect in pain reduction after surgical LM3 removal, but it does not seem to have any impact on other complications or after extraction of normally erupted teeth. This study aimed to investigate the effectiveness of erbium-doped yttrium garnet (Er:YAG) laser and GLUMA desensitizer for dentin hypersensitivity in teeth affected by Molar-Incisor Hypomineralization (MIH). One hundred twenty children were randomly allocated to four groups: the control (Co) group, the desensitizer (De) group, the laser (La) group, and the laser + desensitizer (La + De) group. Outcome measures included Visual Analogue Scale (VAS) and 14-item Oral Health Impact Profile (OHIP-14) evaluation. For mean VAS scores, a significant reduction was found over time in all groups. Co and De groups, Co and La groups, Co and La + De groups, De and La + De groups, and La and La + De groups differed significantly (p < 0.05). For mean scores in all dimensions of OHIP-14 after treatment 6 months, the La + De group was significantly lower (p < 0.001). The La + De groups and the La groups as well as the La + De groups and the De groups differed significantly in total OHIP, functional limitation, physical disability, and psychological disability (p < 0.05). Physical pain between the La + De groups and the La groups and handicap between the La + De groups and De groups differed significantly (p < 0.05). The mean values of each dimension differed significantly between the group Co and the La + De group (p < 0.0001). Combination therapy of Er:YAG laser and GLUMA desensitizer had greater desensitizing effects and oral health-related quality improvement of life, which might be an effective alternative treatment in dentin hypersensitivity in MIH children. Dentin hypersensitivity is an oral health concern affecting a large percentage of the world's adult population. Occlusion of the exposed dentinal tubules is among the treatment options available, and silver diammine fluoride (SDF) is an occluding agent used for interrupting or dampening the stimulus of the dental pulp nerves that produce pain. In addition to dentin permeability testing, the evaluation of desensitizing agents occluding dentinal tubules strongly relies on microscopic techniques, such as scanning electron microscopy (SEM). Limitations of SEM are that it provides only surface images that lack detailed information on the depth of penetration and amount of material present within the treated specimen, and it is prone to sample preparation artifacts. Here, we present high-resolution X-ray computed tomography (nano-CT) as a potential method for investigating dentin specimens with occluded tubules. We studied human dentin treated with SDF as an exemplary dentinal occlusion treatment option. We evaluated the silver deposits formed on the dentin surface region near the dentinal tubules and in the tubular regions using cross-section SEM, Energy Dispersive X-ray (EDX) analysis, and nano-CT. The resulting images obtained by SEM and nano-CT had comparable resolutions, and both techniques produced images of the tubules' occlusion. Nano-CT provided three-dimensional images adequate to quantitate tubule size and orientation in space. Moreover, it enabled clear visualization of dentinal tubules in any virtual plane and estimation of the amount and depth of occluding material. Thus, nano-CT has the potential to be a valuable technique for evaluating the occluding effects of virtually any material applied to dentinal tubules, supporting deciding between the best occluding treatment options. To evaluate the biomimetic remineralization capabilities of Moringa oleifera leaves (MOL) extract on coronal dentin and acellular cementum, two different concentrations (50 and 200 mg/ml) of MOL extract loaded in plain varnish (M1 and M2 groups respectively) were compared to fluoride varnish (FL group) and native surface (C group). Eighty sound premolar teeth were collected. Forty teeth (10 teeth in each group) were used for coronal dentin testing while the other forty (10 teeth in each group) were used for acellular cementum testing. Teeth in M1, M2, and FL groups were etched for 30 s and then received the specific varnish treatment. All samples were immersed in artificial saliva for 14 days and then collected, dried, and examined by scanning electron microscopy and energy dispersive X-ray spectroscopy (EDX). Histologically, FL group showed mineral deposition as discrete vesicular granules of various sizes on the surface of both coronal dentin and acellular cementum. Mineral deposition only occurred on some DTs openings while opened tubules remained. The surface of the acellular cementum revealed regular grooves, micro-fissures, and cracks. In the M1 and M2 groups, mineral deposition appeared as a homogenous continuous layer on coronal dentin and acellular cementum. Only a few DTs and cementum fissures were not filled completely. In L.S. sections of the coronal one-third, the DTs appeared almost sealed with varying lengths of mineral deposition. EDX results statistical analysis showed that the M2 group had the highest phosphate ions (P−) and calcium ions (Ca+2) at%. MOL has an extraordinary effect on the remineralization of coronal dentin and acellular cementum. As cementation represents the last stage of the work involved in making various indirect restorations (metal ceramic crowns and bridges, full ceramic crowns and bridges, inlays, onlays, and fiber posts), its quality significantly contributes to the clinical success of the therapy performed. In the last two decades, the demand for ceramic indirect restorations in everyday dental practice has considerably increased primarily due to the growing significance of esthetics among patients, but also as a result of hypersensitivity reactions to dental alloys in some individuals. In this context, it is essential to ensure a permanent and reliable adhesive bond between the indirect restoration and the tooth structure, as this is the key to the success of aesthetic restorations. Resin-based luting materials benefit from excellent optical (aesthetic) and mechanical properties, as well as from providing a strong and durable adhesive bond between the restoration and the tooth. For this reason, resin cements are a reliable choice of material for cementing polycrystalline ceramic restorations. The current dental material market offers a wide range of resin cement with diverse and continually advancing properties. In response, we wish to note that the interest in the properties of resin-based cements among clinicians has existed for many years. Yet, despite extensive research on the subject and the resulting continued improvements in the quality of these materials, there is still no ideal resin-based cement on the market. The manuscript authors were guided by this fact when writing the article content, as the aim was to provide a concise overview of the composition, properties, and current trends, as well as some future guidelines for research in this field that would be beneficial for dental practitioners as well as the scientific community. Tooth sensitivity is a common side effect of whitening treatment. The popularity of alternative and complementary therapies such as auriculotherapy has grown steadily. The purpose of this research is to assess the efficacy of acupuncture as an integrative technique for reducing sensitivity during and after the whitening operation. The study is a clinical trial with two study groups: group 1 (n=3) volunteers who got auriculotherapy before and during the whitening operation, and group 2 (n=3) participants who did not get auriculotherapy, for a total of (N=6) volunteers (pilot study). The volunteers were males between the ages of 18 and 30, chosen based on inclusion and exclusion criteria. The auriculotherapy technique was performed by batches. Six envelopes with the process to be performed, auricle without crystal, merely micropore, and auricle with crystal on the predefined locations were separated. Two sessions were performed, one 7 days before whitening and the second shortly before whitening, to replace the crystals in the participants chosen for this operation. Only the micropore was altered in the case of the others. Sensitivity was assessed using the Wong-Baker Face Scale in conjunction with the Visual Analog Scale. It was discovered that the group that used auriculotherapy had lower sensitivity than the group that did not. Gingival recession, particularly in the anterior teeth, can lead to functional and aesthetic concerns. It can cause aesthetic deterioration, dentin hypersensitivity, and difficulties in maintaining proper oral hygiene. Multiple surgical methods have been utilized to treat gingival recession. This article presents a case where the lateral pedicle technique combined with a Subepithelial Connective Tissue Graft (SCTG) was successfully utilized to cover the exposed root surface of a single tooth. The adjacent soft tissue was repositioned over the recession defect, resulting in the establishment of an aesthetically pleasing and healthy periodontium, with positive patient satisfaction. Background: Various similarities have been observed between gingival crevicular fluid (GCF) and peri-implant sulcular fluid (PISF). This has resulted in research that has evaluated similar biological fluid markers that are similar to those present within the gingival sulcus. These biomarkers have high sensitivity and are a reliable biological tool when compared to clinical and/or radiographic examination and aid in diagnosis as well as monitoring the progression of periodontal disease surrounding teeth as well as the implants. Aim: The study aimed to compare the effectiveness of periostin in peri-implant sulcular and gingival crevicular fluids. Materials and Methods: This experimental prospective in vitro analysis was done following clearance by the institutional ethical committee. A total of 100 patients were selected. They were categorized into two groups: (I) Group A patients had peri-implant disease (n = 50), whereas (II) Group B patients had periodontitis (n = 50). Clinical loss of attachment score was noted in six sites around natural teeth and four sites around the implants. Presterilized filter paper strips were inserted within the sulcus/pocket till pressure was felt for 60 s. Periostin concentration levels in GCF and PISF samples were measured by the enzyme-linked immunosorbent assay technique. Statistical analysis of data collected was performed using Shapiro–Wilk statistical tool for normally distributed numerical data. Results: Mean ± standard deviation concentration of periostin in gingival crevicular fluid from periodontitis cases was recorded as 20.15 ± 2.76 ng/30sn, whereas in PISF was 19.23 ± 1.89 ng/30sn. On statistical analysis, no statistically significant differences were seen after comparing the concentration of periostin in periodontitis as well as peri-implantitis groups (P > 0.05). Conclusion: The present study analyzed periostin levels in gingival crevicular fluid obtained from patients diagnosed with periodontitis and sulcular fluid obtained from the sulcus around implants. Introduction: Dentinal hypersensitivity is a prevalent and distressing dental condition characterised by acute, transient pain or discomfort arising from exposed dentin in response to external stimuli, such as variations in temperature, mechanical stimuli, or consumption of acidic or sweet foods. Dentinal Hypersensitivity (DH) impacts daily activities, such as eating, drinking, and oral hygiene practices. Dentifrices are commonly used in the treatment of hypersensitivity, as they relieve sensitivity by obstructing nerve pathways through the formation of compounds. Aim: The aim of the study is to evaluate and compare the efficacy of two types of desensitising toothpastes, Vantej and Dente 91, in treating DH. Materials and Methods: A randomised controlled clinical trial was conducted with 40 patients who had a minimum of two hypersensitive teeth. The clinical investigation took place in a dental set-up in Bhubaneswar, Odisha, from July to August 2022. The patients were randomly allocated to two groups (20 in each group) in a 1:1 ratio for the Vantej and Dente 91 groups. Over a four-week period, the patients were instructed to use the prescribed toothpaste twice daily. DH assessment techniques included a Verbal Response Scale (VRS), air blast stimulation, and cold stimulation, both at baseline and after the four-week duration. Statistical analyses were performed using the Mann-Whitney U test and Wilcoxon signed-rank test, with the significance level set at p<0.05. Results: The fourth-week scores compared to baseline scores for the verbal scale, air blast stimulus, and cold sensitivity methods in both groups showed a reduction in DH. In the inter group comparison, the Dente 91 group exhibited greater effectiveness compared to the Vantej group, with a significant reduction in VRS (p=0.02) and air blast stimulus (p=0.01), but not in cold sensitivity (p=0.09). Conclusion: Both Vantej and Dente 91 toothpastes significantly reduced dentinal hypersensitivity. However, the percentage reduction was higher in the latter group, suggesting more effective management of DH. To evaluate the influence of a glutaraldehyde-based desensitizer (GL) on postoperative sensitivity (POS) in posterior bulk-fill resin composite restorations using an adhesive applied in the self-etch (SE) and etch-and-rinse (ER) strategies; and to assess the clinical performance of the restorations. Posterior resin composite restorations (n = 228) at least 3 mm deep were inserted in 57 subjects using a split-mouth design. The adhesive was applied with/without prior application of a GL. A resin composite was used for all restorations. Spontaneous POS (risk and intensity), as well as POS caused by stimulation with an air blast and assess the response to horizontal and vertical percussion was assessed using two scales in the baseline and after 7, 14, and 30 days. In addition, some parameters were evaluated using FDI criteria up to 24 months of clinical service. No significant POS was observed (p > 0.05). A higher absolute risk and intensity of spontaneous POS was observed within 7 days (35.1%), without statistically significant differences among groups. At 24 months 5 restorations were considered clinically unsatisfactory, and 73 restorations showed minor discrepancies in adaptation, with no significant differences between groups (p > 0.05). Objectives: To evaluate noncarious cervical lesions (NCCL) in older adults by analyzing each lesion's morphology, dimension, and occlusal risk factors using intraoral scanning and clinical evaluation. Materials and methods: Individuals over 60 treated at the School of Dentistry in Minas Gerais, Brazil, were invited to participate in this cross-sectional study. We clinically evaluated 1245 teeth for the presence of NCCL and dentin hypersensitivity (DH) in 53 individuals. Three-dimensional models were obtained using Omnicam, and occlusal risk factors were evaluated through the presence of wear facets, intense occlusal contacts, and interferences. The 3Shape 3DViewer software was used to classify NCCL morphology and to measure their height. NCCL depth, width, and area measurements were determined using ImageJ. Descriptive analyses were performed. The Kolmogorov-Smirnov test showed that quantitative data exhibited non-normal distribution. For bivariate analyses of quantitative data, the Mann-Whitney test was employed. The results were reported with mean, standard deviation, median, minimum, and maximum. For bivariate analyses of categorical data, the Pearson chi-square test was used. The results were reported with frequencies (counts) and percentages. A regression model evaluating the association between occlusal risk factors and the absence/presence of NCCL was built. Results: Most participants were female (58.5%), with a mean age of 66. The frequency of NCCL was 28.27%, corresponding to 352 teeth. Of these, 68.18% exhibited saucer morphology and 31.82% wedge-shaped morphology. The frequency of DH was 13.92% in teeth with NCCL. Saucer NCCL exhibited significantly greater height (p = 0.02), while wedge-shaped lesions showed greater depth (p < 0.001). Also, teeth with NCCL had a higher proportion of wear facets (66.8%; p < 0.001), intense occlusal contacts (39.8%; p = 0.008), and occlusal interferences (21.9%; p = 0.05). The regression model showed that teeth with wear facets were 1.50 times more likely to exhibit NCCL than teeth without wear facets (p < 0.001). Teeth with intense occlusal contact were 1.22 times more likely to exhibit NCCL than teeth without intense occlusal contact (p = 0.031). Conclusions: NCCL is a common dental condition in the older adults evaluated in this study. Wedge-shaped NCCL exhibited greater depth, while saucer NCCL had more expressive height. Occlusal risk factors may represent a crucial mechanism for the occurrence of NCCL in this population. Clinical relevance: Understanding the mechanisms involved in developing NCCL is helpful in diagnostic and preventive practices. Objectives: There is a lack of studies evaluating the accuracy of the 2009 American Association of Endodontists (AAE) diagnostic criteria for diagnosing pulpal health in primary teeth. This study aimed to estimate and correlate the diagnostic accuracy of clinical diagnosis of reversible and irreversible pulpitis using the 2009 AAE criteria with histological findings in primary teeth. Methods: Eighty primary teeth that were clinically diagnosed with normal pulp (n = 10), reversible pulpitis (n = 30), irreversible pulpitis (n = 30) and pulp necrosis (n = 10) were collected. The teeth were histo-processed, and pulp tissues were diagnosed histologically as uninflamed pulp, reversible or irreversibly inflamed and necrosis based on previously proposed criteria. Results: The clinical diagnosis of pulp necrosis (sensitivity 70%, specificity 96%) and normal pulp (sensitivity 91%, specificity 100%) matched the histological diagnosis of necrosis and uninflamed pulp in 70% and 100%, respectively. The clinical diagnosis of irreversible pulpitis (sensitivity 64%, specificity 72%) matched the histological diagnosis of irreversible pulp inflammation for 47% of teeth evaluated. For the clinical diagnosis of reversible pulpitis (sensitivity: 65%, specificity: 86%), 80% matched the histological diagnosis of reversible pulp inflammation. Teeth with histologically diagnosed irreversible pulp inflammation were more likely to have lingering (OR 5.08; 95% CI 1.48–17.46, P = 0.010) and nocturnal tooth pain (OR 15.86; 95% CI 1.57–160.47, P = 0.019) when compared to teeth with reversible pulp inflammation. Using the classification and regression tree model, the presence of widened periodontal ligament space and nocturnal tooth pain were useful predictors of irreversible pulp inflammation with an accuracy of 78%. Purpose: This study evaluates the effects of a novel nanohydroxyapatite gel and Er: YAG laser on the surface roughness, surface morphology, and elemental content after dentin hypersensitivity treatments. Methods: Dentin discs (2 × 3 × 3 mm3) were prepared from 75 human molars. Out of 75 human molars, 50 were used to evaluate surface roughness and randomly divided into five groups: Group ID (intact dentin), Group DD (demineralized dentin), Group BF (fluoride varnish/Bifluorid 10), Group Lsr (Er: YAG laser-50 mJ, 0.50 W, 10 Hz), and Group NHA (nanohydroxyapatite-containing gel). Dentin hypersensitivity was stimulated by 35% phosphoric acid for 1 min (except Group ID). The surface roughness (Ra, μm) was measured via contact profilometry (n = 10). Out of the 75 sound human molars, 25 were used to evaluate the surface morphology and elemental content using scanning electron microscopy and energy-dispersive X-ray spectroscopy (n = 5). The data were statistically analyzed using Welsch ANOVA, Games–Howell, Kruskal–Wallis, and Dunn tests (p < 0.05). Results: Group Lsr showed significantly lower surface roughness than Group NHA and Group BF (p < 0.05). The SEM analysis indicated that most of the dentinal tubules were obliterated for Group NHA. Precipitant plugs with partially occluded dentinal tubules were observed for Group BF, while partially or completely occluded tubules with a melting appearance were detected for Group Lsr. The EDS analysis revealed that Group NHA and Group Lsr presented similar calcium and phosphorus amounts to Group ID. All dentin hypersensitivity treatment methods could provide promising results in terms of tubular occlusion efficiency. Introduction: To identify gender-based differences in pulpal sensory thresholds in human teeth utilizing various conducting media. Materials and Methods: The study involved 50 participants of both genders equally distributed. The maxillary central incisors were the teeth selected for the intervention. The three conducting media that were put to the test were Colgate Sensitive Pro-Relief Enamel Repair Toothpaste (CS), Himalaya herbals sensitivity toothpaste (HS), and Lignox 2% gel (LG). Subject's sensory threshold readings as well as the Visual analogue scale (VAS) scores were noted at the tiniest sensation they experienced. Following an ANOVA, a post hoc analysis was performed to confirm the significance between the three media. Results: Lox 2% Jelly significantly reduced sensory threshold values compared to the other conducting media (P < 0.001). Comparing the sensory threshold values of the sexes showed that men have greater values. Male and female readings differed significantly in the Lox 2% Jelly group (P = 0.021), whereas Colgate Sensitive Pro-Relief Enamel Repair Toothpaste, Himalaya herbals sensitivity toothpaste (P < 0.001) groups showed highly significant variances. All the three conducting media had a highly significant variance for the VAS scores. (P < 0.001). Aim: to evaluate, through a clinical trial, the effectiveness and sensitivity of tooth whitening techniques using violet light emitting diodes (LED), comparing with other whitening techniques. Methodology: 75 patients were selected to undergo tooth whitening, randomly distributed into five different treatment groups: G1 (35% hydrogen peroxide), G2 (35% hydrogen peroxide + Green LED), G3 (35% hydrogen peroxide at 35% + Blue LED), G4 (35% hydrogen peroxide + Violet LED) and G5 (Violet LED). Sensitivity measurement was recorded using the Visual Analogue Scale (VAS) from 0 to 10; tooth color measurement was performed before (T0) and after 30 days of whitening treatment (T1) using Easyshade Advance 4.0 Vita—Wilcos® dental spectrophotometer. Statistical analysis was performed using the Fisher and Kruskal–Wallis exact test at significance level of p < 0.05. Results: the enamel whitening effect was observed in all groups, with significant changes in G1. With regard to sensitivity, no statistically significant differences were found between groups. Conclusion: In G5, no participant reported post-whitening sensitivity, suggesting that treatment with violet LED showed the best sensitivity outcomes, changing the enamel color one more tone on the color scale. Dentin hypersensitivity is an oral health concern affecting a large percentage of the world's adult population. Occlusion of the exposed dentinal tubules is among the treatment options available, and silver diammine fluoride (SDF) is an occluding agent used for interrupting or dampening the stimulus of the dental pulp nerves that produce pain. In addition to dentin permeability testing, the evaluation of desensitizing agents occluding dentinal tubules strongly relies on microscopic techniques, such as scanning electron microscopy (SEM). Limitations of SEM are that it provides only surface images that lack detailed information on the depth of penetration and amount of material present within the treated specimen, and it is prone to sample preparation artifacts. Here, we present high-resolution X-ray computed tomography (nano-CT) as a potential method for investigating dentin specimens with occluded tubules. We studied human dentin treated with SDF as an exemplary dentinal occlusion treatment option. We evaluated the silver deposits formed on the dentin surface region near the dentinal tubules and in the tubular regions using cross-section SEM, Energy Dispersive X-ray (EDX) analysis, and nano-CT. The resulting images obtained by SEM and nano-CT had comparable resolutions, and both techniques produced images of the tubules' occlusion. Nano-CT provided three-dimensional images adequate to quantitate tubule size and orientation in space. Moreover, it enabled clear visualization of dentinal tubules in any virtual plane and estimation of the amount and depth of occluding material. Thus, nano-CT has the potential to be a valuable technique for evaluating the occluding effects of virtually any material applied to dentinal tubules, supporting deciding between the best occluding treatment options. BACKGROUND: Nano hydroxyapatite is an excellent bioactive material, which has a certain protective effect on demineralized enamel and dentin, but the effect and mechanism of its solution on remineralization of demineralized dentin are not clear. OBJECTIVE: To explore the feasibility of nano hydroxyapatite in the prevention and treatment of dentin sensitivity and dentin caries. METHODS: Abandoned human bicuspid specimens were collected, and 72 dentin slice samples were prepared from isolated bicuspids. The samples were randomly divided into 6 groups, with 12 samples per group: After demineralization, each group was immersed in 10% nano hydroxyapatite solution (group A), 6% bioactive glass solution (group B), 10% nano hydroxyapatite and 6% bioactive glass composite solution (group C), 0.05% sodium fluoride (group D), artificial saliva (group E), and deionized water (group F). After 14 days of treatment, working surfaces of samples were observed by scanning electron microscope. The surface composition was detected with energy spectrum analyzer. Dentin Rhodamine B staining was detected under a confocal laser microscope. RESULTS AND CONCLUSION: (1) Scanning electron microscopy: The dentinal tubules orifice in group A was completely sealed, and the surface of the dentin was uniformly covered by a layer of tightly connected granular material. The dentinal tubules in group C were partially sealed. A small amount of scattered particles were found on the dentin surface and in the dentinal tubules of group B, group D and group E, and the dentinal tubules were open. In group F, there was no particulate matter deposition on the dentin surface or in the dentinal tubules, and the dentinal tubules were open. (2) X-ray energy dispersive spectrocopy results: The Ca/P ratio of group A was 1.61, which was closest to the theoretical value of healthy dentin; the Ca/P ratio was higher in group A than that in the groups B, E, and F (P < 0.05). There was no significant difference in Ca/P ratio between group A and group C (P > 0.05). (3) Laser confocal microscopy test results showed that the fluorescence intensity and fluorescence area of group A were the smallest, and the mean fluorescence intensity was smaller in group A than that in other five groups (P < 0.05). The mean fluorescence intensity of group C was lower than that of groups B, D, E and F (P < 0.05). (4) It is indicated that nano hydroxyapatite has a good remineralization effect on demineralized dentin. The combination of nano hydroxyapatite and bioactive glass can also deposit on the dentin surface, but there is no synergistic effect. Objectives: Dentine hypersensitivity (DH) is a common concern in dentistry that has the potential to restrict daily activities and harm a person's quality of life. In this study, the remineralization characteristics of nano-hydroxyapatite (nHAp) extracted from waste eggshells and fish scales were comparatively assessed. Materials and methods: The extraction methods used to obtain nHAp from both fish scales and eggshells are also described. The effect of the extraction process and bio-waste source on the physicochemical characteristics of the nHAp such as Ca/P ratio, functional groups, crystallinity and phase change, and surface morphology are presented in the study. The remineralization properties were evaluated using dentine models (n = 15). A field scanning electron microscope was used to evaluate the effectiveness of the dentine tubules occlusion. The percentage occluded area for all the specimens was evaluated statistically using a one-way analysis of variance (α = 0.05). Results: The results showed that there were variations in the physicochemical characteristics of the nHAp extracted, including the crystallinity, particle size, and surface morphology, and buffering effects against citric acids. The EnHAp extracted from eggshells had higher crystallinity, superior buffering effects, and smaller particle size compared to the nHAp extracted from fish scales, making it a more favourable material for remineralization of teeth. The statistical evidence showed that there were statistically significant differences in the dentine occluding properties measured in the nHAp (p < 0.001). The highest mean % occluded area was measured with the EnHAp group. Objective: This controlled randomized clinical trial determined the whitening efficacy and the intensity and absolute risk of tooth sensitivity in dual whitening when prefilled at‐home whitening trays were used between in‐office whitening intervals. Materials and Methods: An in‐office whitening agent containing 35% hydrogen peroxide was used. A prefilled tray with a whitening agent containing 6% hydrogen peroxide was used for at‐home whitening. Sixty‐six subjects were randomly assigned to three groups. Group I: at‐home whitening was performed 10 times between the in‐office whitening treatments. Group II: at‐home whitening was performed five times between the in‐office whitening treatments. Group III: only in‐office whitening was performed. The tooth color changes were evaluated using a spectrophotometer. A visual analog scale was used to express the pain intensity. Results: All the groups showed increased ΔE*ab, ΔE00, and ΔWID with increased whitening sessions. Group I at the 3rd whitening session showed significantly higher ΔE*ab, ΔE00, and ΔWID than group III. Tooth sensitivity showed higher values up to 24 h after whitening. Conclusions: Although dual whitening with the prefilled tray and in‐office whitening had higher whitening ability than in‐office whitening alone, the intensity and absolute risk of tooth sensitivity was similar. Hypertension is an important morbidity factor. The prognostic consequences of the white-coat effect have been studied extensively. The repercussion on the circadian rhythm of urinary water and salt excretion in the same subgroup remain, conversely, among the open topics. Postulating an impaired diurnal sodium and volume excretion we decided to investigate both, in subjects with or without a white-coat effect, in the general population. A sample of 1023 subjects, has been considered. We collected 24-h urine samples, divided in day and night, and we measured the blood pressure with an Ambulatory Blood Pressure Monitoring (ABPM). ABPM values were then compared with physician collected in-office values to assign subjects to the group with or without the white-coat effect. Concerning the circadian pattern of urinary sodium excretion, we found no significant differences between the groups. There was instead in the white-coat effect group a higher night/day ratio of urinary water excretion. The white-coat effect, has been considered a potential hypertension precursor, and its consequent handling could be prospectively relevant in hypertension prevention. The absence of repercussions on the urinary circadian sodium excretion pattern and on the potentially related risk factors in subjects with a white coat effect is reassuring. Background: The coronavirus 2019 (COVID-19) pandemic resulted in rapid implementation of telemedicine. Little is known about the impact of telemedicine on both no-show rates and healthcare disparities on the general primary care population during the pandemic. Objective: To compare no-show rates between telemedicine and office visits in the primary care setting, while controlling for the burden of COVID-19 cases, with focus on underserved populations. Design: Retrospective cohort study. Setting: Multi-center urban network of primary care clinics between April 2021 and December 2021. Participants: A total of 311,517 completed primary care physician visits across 164,647 patients. Main Measures: The primary outcome was risk ratio of no-show incidences (i.e., no-show rates) between telemedicine and office visits across demographic sub-groups including age, ethnicity, race, and payor type. Results: Compared to in-office visits, the overall risk of no-showing favored telemedicine, adjusted risk ratio of 0.68 (95% CI 0.65 to 0.71), absolute risk reduction (ARR) 4.0%. This favorability was most profound in several cohorts with racial/ethnic and socioeconomic differences with risk ratios in Black/African American 0.47 (95% CI 0.41 to 0.53), ARR 9.0%; Hispanic/Latino 0.63 (95% CI 0.58 to 0.68), ARR 4.6%; Medicaid 0.58 (95% CI 0.54 to 0.62) ARR 7.3%; Self-Pay 0.64 (95% CI 0.58 to 0.70) ARR 11.3%. Limitation: The analysis was limited to physician-only visits in a single setting and did not examine the reasons for visits. Conclusion: As compared to office visits, patients using telemedicine have a lower risk of no-showing to primary care appointments. This is one step towards improved access to care. The aim of this study is to evaluate intratubular crystal formation from the experimental material consisting of dicalcium silicate (C2S) and tricalcium silicate (C3S) with nano-scaled particle size. A total of twenty-four specimens were made by isolating 8 mm of the cervical part centered at the cementoenamel junction of extracted premolars. Twelve specimens were not treated and considered as control. The experimental material was applied to the other twelve specimens by brushing for 10,000 strokes. Each group was randomly divided into four subgroups according to the period of immersion in phosphate buffer saline (PBS) for 1, 30, 60, and 90 days each. The specimens were sectioned longitudinally and examined with scanning electron microscopy and energy dispersion X-ray spectroscopy. The intratubular crystal were formed in PBS and densely filled the dentinal tubules over time. The crystal formation occurred at a depth of more than 50 μm from the dentin surface. The Ca/P ratio of formed intratubular crystals was 1.68 after 3 months. The experimental material consisting of C2S and C3S with a nanoscale particle size can form hydroxyapatite-like crystals in dentinal tubules in PBS, and there is a possibility of reducing dentin hypersensitivity by blocking the dentinal fluid flow. Background: Deep neck space infections are uncommon in infants. Retropharyngeal abscess (RPA) is a deep space neck infection that can present with subtle signs and symptoms. Delay in diagnosis can lead to life-threatening complications. Here we describe life-threatening complication of retropharyngeal abscess. Case presentation: We report a life-threatening complication of retropharyngeal abscess in 10-month old Asian infant weighing 8.2 kg. The patient presented with fever, right-sided neck swelling, hoarseness of voice, and respiratory distress. The clinical and radiological findings were suggestive of airway obstruction complicated by retropharyngeal abscess. The patient was urgently taken to the operating room and underwent (grade 3) intubation. After stabilization, the patient underwent endoscopic trans-oral incision and drainage, during which 5 cc pus was aspirated. Antibiotics were prescribed for 2 weeks following the procedure based on the sensitivity result of the pus culture. Conclusion: In retropharyngeal abscess, a delay in diagnosis can result in life-threatening complications. This report highlights the importance of prompt recognition of a threatened airway and the management of retropharyngeal abscess by emergency physicians. If an emergency physician suspects RPA with airway obstructions, the airway should be immediately secured in a secure environment by otolaryngologist and an anesthesiologist. [ABSTRACT FROM AUTHOR]
