Abstract

Outre l’utilisation appropriée des moyens de protection et la fidélité réciproque à son partenaire, la lutte contre le sida passe nécessairement par la connaissance de son statut sérologique. Mais comment faire « impunément » la démarche de connaître son statut dans un contexte dans lequel l’absence ou l’insuffisance d’offre thérapeutique aggrave les comportements de demande ? Le présent article traite des résultats d’une recherche entreprise de juin à août 2004 à Douala, capitale économique du Cameroun. Il examine l’attitude générale très négative des jeunes camerounais face au test du VIH. Par le biais des discussions de groupe, ces jeunes nous livrent dans un discours franc, qui fait parfois fi des conventions et de la censure, ce qui constitue pour eux des pesanteurs à la pratique du test du VIH. In addition to the appropriate use of protective means and mutual fidelity to one’s partner, the fight against HIV requires from everyone to be aware of his serology. However, in such a context whereby the absence or lack of therapeutic supply worsens demand behaviour, how can one be bold enough to do a deliberate HIV test? This article deals with the results of a research work carried out from June to August 2004 in Douala, the economic capital of Cameroon. It examines the general negative attitude of young Cameroonians concerning HIV screening. Through focus group discussions, these young people deliver in an open talk, which sometimes ignore censorship and social conventions, what is identified by them as the main barriers to the deliberate action to do an HIV screening.

Abstract

This paper aims to show how complex the relation of Cameroonian youths is to the West and how this influences their relation to HIV risk and partly induces their exposure to the risk. This work is the result of anthropological fieldwork carried out in Cameroon between 2004 and 2006. Information was got using focus group discussion and in-depth interviews among young people aged 15-24 both in urban and rural areas. In fact, one observes among these youths a relative westernization, which coexists with a violent relation, somewhat fantastical, made up of admiration, great fascination, suspicion and mistrust, adoption and rebuttal of what comes from the West. This strong and complex relation to the West acts at various levels. It leads to victimization trough a process of selective past and current information that would partly explain the state of lethargy in which Africa is nowadays: In this process of victimization, there is a high tendency of releasing from individual responsibility, which enables rationalization and justification of casual sex. It also reinforces the notion of otherness of risk through the construction of the risk as being a far away fact. Living in a context sandwiched between tradition and modernity places individuals in a state of global tension. They are roughly under the influence of five discourses: biomedical discourse, religious discourse, traditional medicine discourse, traditionalist opinion leader's discourse, conspiracy discourse on topics related to the sale of condoms, the origins of HIV, and research on efficient therapy and vaccine. Abstract

Background: The use of telemonitoring to manage renal function in patients with chronic kidney disease (CKD) is recommended by health authorities. However, despite these recommendations, the adoption of telemonitoring by both health care professionals and patients faces numerous challenges., Objective: This study aims to identify barriers and facilitators in the implementation of a telemonitoring program for patients with CKD, as perceived by health care professionals and patients, and to explore factors associated with the adoption of the program. This study serves as a process evaluation conducted alongside the cost-effectiveness NeLLY (New Health e-Link in the Lyon Region) trial., Methods: A mixed methods approach combining a quantitative questionnaire and semistructured interviews was conducted among nurses, nephrologists, and patients with stages 3 and 4 CKD across 10 renal care centers in France that have implemented telemonitoring. The Technology Acceptance Model (TAM) and the Consolidated Framework for Implementation Research (CFIR) were used to design the questionnaires and interview guides. The dimensions investigated included ease of use, perceived usefulness, and intention to use (TAM), as well as characteristics of the intervention, local and general context, individual factors, and processes (CFIR). The adoption of telemonitoring was assessed based on the frequency with which patients connected to the telemonitoring device. Determinants of telemonitoring use were analyzed using nonparametric tests, specifically the Wilcoxon-Mann-Whitney and Kruskal-Wallis tests. Thematic analysis was conducted on the transcriptions of semistructured interviews. Both quantitative and qualitative results, including data from patients and professionals, were integrated to provide a comprehensive understanding of the factors associated with the use of remote monitoring in CKD., Results: A total of 42 professionals and 128 patients with CKD responded to our questionnaire. Among these, 11 professionals and 13 patients participated in interviews. Nurses, who were responsible for patient follow-up, regularly used telemonitoring (8/13, 62%, at least once a month), while nephrologists, who were responsible for prescribing it, were primarily occasional users (5/8, 63%, using it less than once a month). Among professionals, the main obstacles identified were the heavy workload generated by telemonitoring, lack of training, and insufficient support for nurses. Among the 128 patients, 46 (35.9%) reported using the application at least once a week. The main barriers for patients were issues related to computer use, as well as the lack of feedback and communication with health care professionals. The main facilitators identified by both professionals and patients for using telemonitoring were the empowerment of patients in managing their health and the reduction of the burden associated with CKD., Conclusions: Improving adherence to telemonitoring in the context of CKD requires collaborative efforts from both professionals and patients.
