Resume Introduction Les patients candidats a une chirurgie bariatrique beneficient frequemment d’un depistage de syndrome d’apnees du sommeil (SAS) qui fait partie des recommandations alors que les autres troubles du sommeil, notamment l’insomnie et la privation chronique de sommeil, sont negliges. Methodes Etude transversale evaluant la prevalence et le retentissement de l’insomnie chronique et d’une privation du sommeil, associees ou non a un SAS, sur la qualite de vie et du sommeil de patients adresses en chirurgie bariatrique. Resultats Au total, 88 patients (74 % de femmes, âge median 41 [33,5 ; 50] ans, IMC median a 42 [39,2 ; 45,7] kg/m2) ont ete inclus. La prevalence de l’insomnie chronique etait de 31 % pour une prevalence du SAS de 87 %. 27 % des patients SAS presentaient egalement une insomnie chronique. L’insomnie chronique etait associee a une qualite de vie alteree (EVA EQ5D mediane : 60 [50 ;70] p = 0,04) et a une mauvaise qualite sommeil (Pittsburgh moyen : 8 [6 ; 11] p Conclusion L’insomnie chronique et la privation de sommeil ont des repercussions cliniques pejoratives et meritent d’etre recherchees avant chirurgie bariatrique. L’evolution post-chirurgie de ces sous-groupes de patients necessite d’etre evaluee dans des etudes ulterieures.

Continuous positive airway pressure (CPAP) is the most efficient treatment of obstructive sleep apnea (OSA). Little is known about the impact of spousal relationship profiles on CPAP adherence. We aimed to identify clusters of couples of OSA patients, and their association with CPAP adherence 120 days after CPAP initiation. In a multicenter prospective study, OSA patients recently prescribed CPAP were enrolled with their spouses. Data about spousal relationships were collected at inclusion and at day 120. Latent class analysis was performed to determine homogeneous groups of spousal relationships. The 290 participants were predominantly males (77%), median age was 53 years and interquartile range (IQR) 46&ndash, 62, median body mass index (BMI) was 32 kg/m², (IQR: 28.6&ndash, 35.9) and median apnea&ndash, hypopnea index: 43 events per hour (IQR: 33&ndash, 58). Three couple clusters were identified: 1) older retired couples, 2) young working couples, and 3) mature active couples. Patients in the older retired couples cluster presented the highest CPAP adherence (p <, 0.01) independently of initial complaints, OSA severity, and degree of improvement under CPAP. In a large cohort of OSA patients in whom clusters of couples were determined, there was a significant difference in CPAP adherence at day-120 after CPAP initiation.

Resume Depuis mi-mars 2020, l’epidemie de COVID-19 a conduit a la suspension de la prise en charge des patients presentant des troubles du sommeil et de la vigilance. Bien qu’ayant rarement un caractere urgent, cette prise en charge ne peut etre repoussee indefiniment. Des propositions visant a etablir les modalites de reprise progressive des activites de medecine du sommeil ont ete elaborees grâce a la methodologie CORE qui permet de degager rapidement un consensus d’experts lorsque les donnees probantes sont insuffisantes. La realisation d’explorations diagnostiques du sommeil et de la vigilance doit etre limitee aux patients pour lesquels la balance benefice–risque est favorable de maniere indiscutable et lorsque les resultats sont susceptibles d’avoir un impact decisif sur une strategie therapeutique, en privilegiant la teleconsultation et les enregistrements ambulatoires. Le strict respect de la distanciation physique et des autres mesures barriere, le port d’equipement de protection par le personnel en fonction des tâches, l’utilisation de materiel a usage unique si possible, et des procedures rigoureuses de nettoyage et de desinfection de l’equipement et des locaux doit permettre de limiter les risques de transmission du SARS-CoV-2 entre le patient et les soignants. Les propositions du groupe d’experts sont valables au moment de leur publication mais seront bien entendu reevaluees et completees tres regulierement en fonction de l’evolution des connaissances scientifiques et des recommandations des autorites de sante basees sur l’evolution de l’epidemie.

Objectif L’objectif de l’etude est d’identifier des clusters de couples de patients ayant un SAOS et les liens entre ces profils et l’observance a la PPC, 120 jours apres l’initiation du traitement. Methodes Les patients ayant un SAOS et une prescription initiale pour une PPC ont ete inclus dans le cadre d’une etude prospective multicentrique avec leur conjoint. Les donnees concernant les relations au sein du couple ont ete recueillies a l’initiation du traitement et 120 jours apres le debut du traitement. Une analyse en classe latente a ete realisee pour identifier des groupes homogenes de couples de patients SAOS. Resultats Les 290 patients inclus etaient majoritairement de sexe masculin (77 %) avec un âge median de 53 ans (interquartile range 46–62), un indice de masse corporelle de 32 kg/m2 (IQR : 28,6–35,9) et un index apnees-hypopnees median de 43 evenements par heure (IQR : 33–58). Trois profils de couples ont ete identifies : 1) couples retraites âges, 2) jeunes couples actifs et 3) couples murs et actifs. Les patients appartenant au profil de couple âges et retraites presentaient l’observance a la PPC la plus elevee (p Conclusion Dans cette cohorte prospective multicentrique de 290 couples avec un patient SAOS, il y a une difference significative de l’observance a la PPC a 120 jours apres le debut du traitement selon le profil de couple des patients.

Objectif Les patients candidats a une chirurgie bariatrique beneficient systematiquement d’un depistage de SAOS alors que les autres troubles du sommeil ne sont habituellement pas recherches, en particulier la privation de sommeil et l’insomnie chronique. Methodes Notre etude observationnelle prospective evalue la prevalence de la privation de sommeil, de l’insomnie chronique associees ou non a un SAOS dans une cohorte de patients obeses candidats a une chirurgie bariatrique. Elle precise leurs caracteristiques respectives et leurs consequences sur la qualite de vie et de sommeil des patients. Resultats Quatre-vingt-huit patients sont inclus dans l’etude. Soixante-six pour cent des patients sont en dette de sommeil et 23,8 % declarent un temps sommeil inferieur a 6 heures. L’insomnie chronique a une prevalence de 30,6% et le SAOS de 87 %. Parmi les patients, 53,4 % (47/88) beneficient d’une ventilation par PPC (dont 18 patients avant l’inclusion dans l’etude) en raison d’un SAOS modere a severe. L’association SAOS et insomnie chronique est retrouvee pour 27,2 % des sujets. Conclusion En comparaison avec les autres pathologies, l’insomnie chronique est associee a une somnolence plus severe, une qualite de vie diminuee et une mauvaise perception de qualite sommeil (p

Patients9 expectations for a new lightweight portable noninvasive ventilator for shortness of breath. Introduction: The VitaBreath (Philips-Respironics, Murrysville, PA) is a portable noninvasive ventilator developed to provide short-term relief of dyspnea associated with physical exertion in COPD patients. Objectives: 1) to evaluate patients’ expectation for usefulness of VitaBreath in daily life; 2) to identify the clinical profile of patients who had the most positive anticipation of usefulness. Methods: In this multi-center prospective survey, the VitaBreath was introduced to COPD patients by using a short video; patients could handle the device, but did not use it. The perception of usefulness was rated using a 10-item scale (0=not useful at all; 10=very useful). Anthropometrics, medical history, respiratory function and quality of life (QoL) were collected at inclusion. Results: 137 patients were included in the analysis (63% male; mean (SD) age= 67(9) years; FEV1=0.96(0.4)L); CAT score= 19.9(6.9); 71% had a MRC-scale ≥3/4. 57% of the patients graded the usefulness of VitaBreath ≥9/10. In multivariate analysis, a high usefulness perception(≥9/10) was independently associated with a FEV1/FVC Conclusion: COPD patients with severe respiratory impairment, but with relatively well preserved QoL expressed the highest expectations for usefulness of VitaBreath. These data will help to better define the appropriate COPD population for future interventional trial

International audience; Background: Continuous positive airway pressure (CPAP) is the first line therapy for obstructive sleep apnea (OSA) but its effectiveness requires high adherence. We aimed to assess the impacts of the spouse's/partner's involvement and the quality of the couple's relationship on CPAP adherence. Methods: In a multicenter prospective study conducted in France, patients reported their subjective views regarding their partner's engagement in their CPAP treatment and the quality of their marital relationship using the Quality of Marriage Index. A hierarchical linear model was built to assess the predictors of CPAP adherence at day 120. Structural equation modeling was performed to evaluate the direct and indirect effects of the spouse's/partner's engagement and the quality of the couple's relationship on CPAP adherence. Results: The 290 OSA patients were predominantly male (77%), with a median age of 53 years IQR: [46; 62], median BMI: 32 kg/m2 [28.6; 35.9] and median apnea + hypopnea index: 43/per hour [33; 58]. Independent factors for CPAP adherence at day 120 were the partner's encouragement of CPAP usage and a stable relationship exceeding 30 years, although emotional support or collaboration were not associated with CPAP adherence. Structural equation modeling demonstrated that spouse's/partner's engagement is directly related to CPAP adherence and improvement of symptoms, and that CPAP adherence is a mediator of disease-specific health-related quality of life. Marital quality was a significant moderator of these interactions meaning that a spouse's/partner's engagement improved adherence only when the quality of marriage index was high. Conclusion: Future research and integrated OSA management should systematically include and document the role of the spouse/partner in CPAP adherence.

Resume Les resultats preliminaires de l’etude SERVE-HF ont donne lieu a la diffusion d’une information de securite conduisant a la contre-indication de l’utilisation d’une ventilation auto-asservie (VAA) pour le traitement des apnees centrales chez les patients avec insuffisance cardiaque systolique symptomatique chronique avec fraction d’ejection du ventricule gauche (FEVG) ≤ 45 %. L’objectif de cet article est de faire le point sur ces resultats et d’apporter une argumentation reposant sur les donnees de la litterature en faveur de la poursuite de l’utilisation de cette ventilation dans des indications differentes telles que : insuffisance cardiaque a FEVG conservee, syndrome des apnees du sommeil (SAS) complexe, SAS central induit par les opiaces, SAS central idiopathique, SAS central consequence d’un accident vasculaire cerebral. Ces differents elements nous conduisent a proposer la mise en place de registres dedies aux patients desappareilles de leur VAA d’une part, et d’autre part dans le cadre des prochaines mises en place de ce type de ventilation dans ces indications specifiques afin d’assurer la securite des patients et permettre des decisions argumentees concernant l’utilisation de la VAA.

Objectif Etudier l’impact de la qualite du mariage, de l’engagement du conjoint et des caracteristiques du couple sur l’utilisation de la PPC dans une population de patients SAOS vivant en couple depuis plus d’un an. Methodes Etude multicentrique prospective evaluant des patients SAOS sous PPC vivant en couple depuis plus d’1 an. A JO et a J45 sont mesurees, la qualite de sommeil du patient (Questionnaire de Quebec), l’implication du conjoint (echelle de coping dyadique) et la qualite du mariage (Echelle de Norton). L’observance est mesuree a J120. Les facteurs influencant l’observance sont evalues par analyse longitudinale et uni-variee. L’etude de l’impact de l’implication du conjoint, de la qualite de mariage et de la qualite du sommeil sur l’observance est valide par un modele d’equations structurelles (SEM logiciel AMOS). Resultats Au total, 290 patients SAOS sont inclus : 77 % hommes, âge 53(46–62), IMC 32(28,6–35,9), IAH 43(33–58). A J120, l’observance a PPC est de 359 min(307–425). Les facteurs independants ameliorant l’observance sont l’implication du conjoint par sa collaboration et la duree du couple (> 30 ans) (p = 0,01). Le modele SEM montre que : l’implication du conjoint ameliore l’observance (p Conclusion L’implication du conjoint et la qualite de relation du couple conditionnent l’observance a la PPC.

Baclofen, a gamma-aminobutyric acid-B agonist with muscle-relaxant properties, is widely used in patients with severe spasticity. In animals, baclofen has been shown to decrease respiratory drive. In humans, however, use of baclofen at the standard dose did not significantly impair sleep-disordered breathing in a susceptible population of snorers. Recently, there has been increasing interest in the role of baclofen for the treatment of alcohol dependence. We describe severe central sleep apnea (CSA) in four patients with none of the conditions commonly associated with CSA who were receiving chronic baclofen therapy for alcohol withdrawal. In one patient, baclofen withdrawal was associated with a complete resolution of CSA. Three patients were treated by adaptive servo-ventilation while continuing their treatment with baclofen. Given the increasing number of patients receiving baclofen for alcohol withdrawal treatment, physicians should be aware that these patients might be affected by severe CSA. Future studies are required to determine the mechanisms, prevalence, and treatment modalities of sleep-disordered breathing associated with baclofen usage.

18. Does CPAP Use in the First 15 Days Predict its Use after 4 Months? A Prospective French Cohort Study

Objective: Early prediction of which patients with obstructive sleep apnea will not adhere with CPAP can trigger physicians to correct possible causes or offer alternative therapies. This study aims to determine the ability of early CPAP use to predict long-term adherence with CPAP independent from its definition and to test previously described predictors of adherence. Method: This is a prospective, observational cohort study that was performed in nine sleep disorder centers in France. Patients were newly diagnosed with obstructive sleep apnea and a CPAP was prescribed for the first time. We collected socioeconomic, medical, sleep, and psychological variables to be associated with adherence with CPAP. The patients were evaluated 15 days after CPAP set up and monthly for 4 months until they were adherent. We assessed potential predictors of CPAP adherence at 4 months with emphasis on CPAP use at 15 days. CPAP adherence was defined as an average daily use of at least 3, 4, 5, and 6 hours. Results: We enrolled 420 patients. Early CPAP use and CPAP use at 4 months were similar. At 4 months, 94% of patients used a CPAP ≥3 hours and 49% of patients used it ≥6 hours. The area under the ROC curve for early CPAP use predicting adherence at 4 months was ≥0.85 for all adherence definitions. The optimal threshold for early CPAP use to predict adherence increased from 3.2 to 6.4 hours as the definition of CPAP adherence increased from 3 to 6 hours. CPAP use at 4 months was higher in older patients. Conclusions: Early CPAP use was the single best predictor of CPAP adherence and was independent of how adherence was defined. Physicians must assess CPAP use no later than 2 weeks to address its causes or prescribe an alternative therapy.

19. La reprise d’activité physique en centre d’obésité : le point de vue du patient

Introduction La reprise d’activite physique est primordiale dans la prise en charge multidisciplinaire de l’obesite qui est actuellement un probleme de sante publique majeur en France (15 % de la population). Notre etude vise a evaluer les caracteristiques demographiques et socioprofessionnelles de patients obeses, et a sonder leur motivation a la reprise d’une activite physique au travers d’un programme de readaptation. Materiels et methodes Etude descriptive d’une population de patients integrant notre centre de prise en charge de l’obesite. Pendant six mois, ces patients repondent, sur la base du volontariat et de facon anonyme, a un questionnaire comprenant des donnees anthropometriques, socioprofessionnelles et motivationnelles. Resultats Cent trente et un patients, a dominance feminine (86 %), jeunes (âge moyen 39,6 ans), avec une obesite severe (IMC moyen : 38,7 kg/m 2 ) ont complete le questionnaire. Soixante-dix-huit pour cent des patients ont une activite professionnelle, 53 % d’entre eux travaillant plus de 35 heures par semaine. Une absence complete d’activite physique est decrite dans 58 % des cas. A la question « vous sentez vous en bonne forme physique ? » seulement 15 % repondent « oui ». 80 % des patients sont interesses par une reprise d’activite physique. Un programme de reentrainement encadre par un coach (84 %) et individuel (70 %) a la preference. En majorite, les patients acceptent de participer financierement a leur reentrainement avec un apport minimal de 100 Euros. Conclusion Les patients obeses ayant une activite professionnelle sont demandeurs d’integrer un programme de remise en forme physique conjointement a leur prise en charge nutritionnelle et sont prets a y consacrer du temps et de l’argent.

age, 55 11 years; mean body mass index, 31 6 kg/m 2 ; mean apnea-hypopnea index, 44 21 events/h) were randomly assigned to receive 3 months of treatment with CPAP (108 patients) or C-Flex (110 patients). Objective compliance, generic quality-of-life questionnaire (SF-36) scores, disease-specific quality-of-life questionnaire (Grenoble Sleep Apnea Quality of Life [GrenobleSAQOL]) scores, and visual analog scales for CPAP comfort and side effects were determined at baseline and after 3 months. After 3 months, patients in the CPAP arm were moved to the C-Flex arm for 3 additional months (open study). Results: An intention-to-treat analysis demonstrated that there were no differences at 3 months between C-Flex and CPAP use in terms of compliance, the rate of side effects, and comfort. Low compliers receiving CPAP therapy (< 4 h of use) significantly improved this outcome during the open study (p 0.04). There was a significant improvement in six of eight of the SF-36 domain scores and in all of the domains of the GrenobleSAQOL scores in both groups using either CPAP or C-Flex. Conclusion: In unselected sleep apnea patients, C-Flex was associated with similar outcomes to standard CPAP. Low compliers receiving CPAP therapy improved their adherence when moving to C-Flex. Objectif La PR1 REMstar auto avec le mode de confort P-Flex est une machine de ventilation en pression positive continue (PPC) qui s’adapte aux besoins de pression du patient. Autrement dit des la detection de l’inspiration, la machine accompagne l’inspiration pour atteindre la pression requise puis en debut expiration, le mode P-Flex applique une reduction de pression proportionnelle au debit du patient ainsi qu’au niveau de pression. Contrairement au mode de confort A-Flex qui applique en fin d’expiration une reduction de pression qui n’est proportionnelle qu’au debit patient. Le but de cette etude est d’evaluer l’efficacite, le confort, le niveau moyen de pression delivre par la PR1 REMstar Auto en utilisant le mode de confort P-Flex chez des patients presentant un syndrome d’apnee hypopnee obstructif du sommeil (SAHOS) severe. Methodes Notre etude prospective a inclus des patients somnolents (Epworth > 10) porteurs d’un SAHOS severe. Une polysomnographie est realisee lors du bilan initial et a 3 mois de traitement par la PPC. L’observance est mesuree via la carte memoire de la machine. La pression moyenne delivree par la PPC auto est comparee a celle mesuree au masque soit la pression correspondant a l’effet du mode P-Flex. Resultats Trente-deux patients (24 hommes) (IMC = 31,7 ± 4,5 kg/m 2 , âge = 52,4 ± 11 ans, IAH = 49,9 ± 18/h) sont inclus. A 3 mois, l’IAH-PSG est reduit de maniere significative (4,3 ± 4,8/h), de meme que l’index de desaturation (31,6 ± 27,2 a 4,8 ± 5,7/h), l’indice de micro-eveils (39,4 ± 18,1 a 2,6 ± 3,4/h) et la somnolence (Epworth = 13,8 ± 2,9 a 6,6 ± 4,3) ( p 2 O). Conclusion Chez des patients avec un SAHOS severe, la PR1 REMstar auto avec le mode de confort P-Flex est bien toleree et traite efficacement les evenements respiratoires et les symptomes des troubles respiratoires du sommeil avec un niveau moyen de pression plus bas.

Dermatomyositis, an inflammatory muscle disease probably related to dysimmunity, is associated with character skin eruptions. Dermatomyositis is often associated with cancer (15 to 40% of cases depending on the series). All histological types and all cancer localizations observed in the general population can be associated with dermatomyositis. We report the case of a patient with dermatomyositis who developed small-cell lung cancer. There have been few descriptions of this association in the literature. Certain clinical features of dermatomyositis would be predictive of its paraneoplastic nature. Prognosis is very poor. Treatment is basically dictated by the underlying neoplasia.

SummaryThe preliminary results of the SERVE-HF study have led to the release of safety information with subsequent contraindication to the use of adaptive servo-ventilation (ASV) for the treatment of central sleep apnoeas in patients with chronic symptomatic systolic heart failure with left ventricular ejection fraction (LVEF) ≤ 45%. The aim of this article is to review these results, and to provide more detailed arguments based on data from the literature advocating the continued use of ASV in different indications, including heart failure with preserved LVEF, complex sleep apnoea syndrome, opioid-induced central sleep apnea syndrome, idiopathic central SAS, and central SAS due to a stroke. Based on these findings, we propose to set up registers dedicated to patients in whom ASV has been stopped and in the context of the next setting up of ASV in these specific indications to ensure patient safety and allow reasoned decisions on the use of ASV.

29. Investigating associations between social determinants, self-efficacy measurement of sleep apnea and CPAP adherence: the SEMSA study.

Study Objectives: The prospective Self-Efficacy Measure for Sleep Apnea study (SEMSAS) is investigating thresholds for health literacy, self-efficacy and precariousness at obstructive sleep apnea (OSA) diagnosis to predict CPAP adherence. This paper describes the study protocol and presents baseline data from the ongoing study., Methods: Eligible individuals had confirmed OSA and were referred to a homecare provider for continuous positive airway pressure (CPAP) therapy initiation. Data on patient characteristics and comorbidities were collected, along with baseline evaluations of self-efficacy [15-item Self-Efficacy Measure for Sleep Apnea tool (SEMSA-15)], precariousness [Deprivation in Primary Care Questionnaire (DipCareQ)], and health literacy (Health Literacy Questionnaire). CPAP adherence over 12 months of follow-up will be determined using remote monitoring of CPAP device data. The primary objective is to define an optimal SEMSA-15 score threshold to predict CPAP adherence at 3- and 12-month follow-up., Results: Enrollment of 302 participants (71% male, median age 55 years, median body mass index 31.6 kg/m
2 ) is complete. Low self-efficacy (SEMSA-15 score ≤ 2.78) was found in 93/302 participants (31%), and 38 (12.6%) reported precariousness (DipCareQ score > 1); precariousness did not differ significantly between individuals with a SEMSA-15 score ≤ 2.78 versus >2.78. Health literacy was generally good, but was significantly lower in individuals with versus without precariousness, and with low versus high self-efficacy., Conclusion: SEMSAS is the first study using multidimensional baseline assessment of self-efficacy, health literacy and precariousness, plus other characteristics, to determine future adherence to CPAP, including CPAP adherence trajectories. - Abstract
Introduction: Investigation for obstructive sleep apnea syndrome (OSAS) is mandatory before bariatric surgery. Data regarding chronic insomnia and chronic sleep deprivation are scarce in this population., Methods: A cross-sectional study assessing the prevalence of chronic insomnia, OSAS and chronic sleep privation in an obese population referred for bariatric surgery., Results: In all, 88 patients (74% women, median age 41 [33.5-50] years and median body mass index 42 [39.2-45.7] kg/m
2 ) were included. The prevalence of chronic insomnia was 31% in the 87% suffering from OSAS that required continuous positive airway pressure therapy. Comorbid insomnia and sleep apnoea (COMISA) were found in 27% of our population. Chronic insomnia was associated with a lower quality of life (median EQ5D analogue visual scale: 60 [50-70] P=0.04) and a poor sleep quality (median Pittsburgh sleep quality index (PSQI): 8 (6-11 P<0.01) The deleterious combination of sleep privation and insomnia had a higher impact in terms of impairment of quality of life and sleep quality (median EQ5D analogue visual scale: 50 [40-65] P=0.02 et median PSQI: 11 [9-14, P<0.01) CONCLUSION: Chronic insomnia and sleep privation have synergistic deleterious effects in candidates for bariatric surgery. Most of the continuous positive airway pressure (CPAP) devices currently in use allow telemonitoring of observance, leaks and the apnoea-hypopnoea index (AHI). 32. Multidimensional Evaluation of Continuous Positive Airway Pressure (CPAP) Treatment for Sleep Apnea in Different Clusters of Couples.

Continuous positive airway pressure (CPAP) is the most efficient treatment of obstructive sleep apnea (OSA). Little is known about the impact of spousal relationship profiles on CPAP adherence. We aimed to identify clusters of couples of OSA patients, and their association with CPAP adherence 120 days after CPAP initiation. In a multicenter prospective study, OSA patients recently prescribed CPAP were enrolled with their spouses. Data about spousal relationships were collected at inclusion and at day 120. Latent class analysis was performed to determine homogeneous groups of spousal relationships. The 290 participants were predominantly males (77%), median age was 53 years and interquartile range (IQR) 46-62, median body mass index (BMI) was 32 kg/m² (IQR: 28.6-35.9) and median apnea-hypopnea index: 43 events per hour (IQR: 33-58). Three couple clusters were identified: 1) older retired couples, 2) young working couples, and 3) mature active couples. Patients in the older retired couples cluster presented the highest CPAP adherence ( p < 0.01) independently of initial complaints, OSA severity, and degree of improvement under CPAP. In a large cohort of OSA patients in whom clusters of couples were determined, there was a significant difference in CPAP adherence at day-120 after CPAP initiation.

Independent factors for CPAP adherence at day 120 were the partner's encouragement of CPAP usage and a stable relationship exceeding 30 years, although emotional support or collaboration were not associated with CPAP adherence. Structural equation modeling demonstrated that spouse's/partner's engagement is directly related to CPAP adherence and improvement of symptoms, and that CPAP adherence is a mediator of disease-specific health-related quality of life. Marital quality was a significant moderator of these interactions meaning that a spouse's/partner's engagement improved adherence only when the quality of marriage index was high., Conclusion: Future research and integrated OSA management should systematically include and document the role of the spouse/partner in CPAP adherence., (Copyright © 2018. - Abstract
Baclofen, a gamma-aminobutyric acid-B agonist with muscle-relaxant properties, is widely used in patients with severe spasticity. In animals, baclofen has been shown to decrease respiratory drive. In humans, however, use of baclofen at the standard dose did not significantly impair sleep-disordered breathing in a susceptible population of snorers. Recently, there has been increasing interest in the role of baclofen for the treatment of alcohol dependence. We describe severe central sleep apnea (CSA) in four patients with none of the conditions commonly associated with CSA who were receiving chronic baclofen therapy for alcohol withdrawal. In one patient, baclofen withdrawal was associated with a complete resolution of CSA. Three patients were treated by adaptive servo-ventilation while continuing their treatment with baclofen. Given the increasing number of patients receiving baclofen for alcohol withdrawal treatment, physicians should be aware that these patients might be affected by severe CSA. Future studies are required to determine the mechanisms, prevalence, and treatment modalities of sleep-disordered breathing associated with baclofen usage., (Copyright © 2016 American College of Chest Physicians. - Abstract
The preliminary results of the SERVE-HF study have led to the release of safety information with subsequent contraindication to the use of adaptive servo-ventilation (ASV) for the treatment of central sleep apnoeas in patients with chronic symptomatic systolic heart failure with left ventricular ejection fraction (LVEF) ≤ 45%. The aim of this article is to review these results, and to provide more detailed arguments based on data from the literature advocating the continued use of ASV in different indications, including heart failure with preserved LVEF, complex sleep apnoea syndrome, opioid-induced central sleep apnea syndrome, idiopathic central SAS, and central SAS due to a stroke. Based on these findings, we propose to set up registers dedicated to patients in whom ASV has been stopped and in the context of the next setting up of ASV in these specific indications to ensure patient safety and allow reasoned decisions on the use of ASV., (Copyright © 2015 SPLF. Background: Continuous positive airways pressure (CPAP) is the accepted therapy for obstructive sleep apnoea (OSA), but compliance is variable. We hypothesised that an auto bi-level device with pressure relief during exhalation (auto bi-level) would treat OSA as well as CPAP and that transitioning non-compliant CPAP patients without modifiable causes of poor compliance to this device would improve compliance and clinical outcomes., Materials and Methods: OSA patient's on positive airways pressure therapy with compliance below 4 h of use on ≥70% of nights over the past 3 months despite having no modifiable causes of poor compliance were transitioned onto an auto bi-level device for 10 weeks. Patients completed an Epworth sleepiness scale and Functional Outcomes of Sleep Questionnaire (FOSQ) at 15 days and 10 weeks and had their compliance and therapy data downloaded. Additionally, patients underwent polysomnography on their auto bi-level device at week 10., Results: Thirty-five patients were included. The apnoea-hypopnoea index, arousal index, sleep efficiency, total sleep time and sleep stage distribution were similar at baseline and week 10. Compliance, excessive daytime sleepiness and several FOSQ domains improved significantly at day 15 and week 10. Patients requiring an effective pressure ≥10 cmH(2)0 during the lead-in period on CPAP experienced greater significant improvements compliance than those requiring an effective pressure <10 cmH(2)0., Conclusions: Auto bi-level with pressure relief during exhalation treats OSA as effectively as CPAP without inducing additional arousals. Transitioning non-compliant CPAP patients without modifiable causes of poor compliance from their CPAP to this new device improves compliance and clinical outcomes over a 10-week period.

Introduction: This French, multicenter, randomized double-blind controlled trial tested the hypothesis that pressure reduction during exhalation (C-Flex; Respironics; Murrysville, PA) would improve continuous positive airway pressure (CPAP) compliance, comfort, and quality of life., Methods: Two hundred eighteen newly diagnosed sleep apnea patients (seven centers; mean [+/- SD] age, 55 +/- 11 years; mean body mass index, 31 +/- 6 kg/m(2); mean apnea-hypopnea index, 44 +/- 21 events/h) were randomly assigned to receive 3 months of treatment with CPAP (108 patients) or C-Flex (110 patients). Objective compliance, generic quality-of-life questionnaire (SF-36) scores, disease-specific quality-of-life questionnaire (Grenoble Sleep Apnea Quality of Life [GrenobleSAQOL]) scores, and visual analog scales for CPAP comfort and side effects were determined at baseline and after 3 months. After 3 months, patients in the CPAP arm were moved to the C-Flex arm for 3 additional months (open study)., Results: An intention-to-treat analysis demonstrated that there were no differences at 3 months between C-Flex and CPAP use in terms of compliance, the rate of side effects, and comfort. Low compliers receiving CPAP therapy (< 4 h of use) significantly improved this outcome during the open study (p = 0.04). There was a significant improvement in six of eight of the SF-36 domain scores and in all of the domains of the GrenobleSAQOL scores in both groups using either CPAP or C-Flex., Conclusion: In unselected sleep apnea patients, C-Flex was associated with similar outcomes to standard CPAP. Low compliers receiving CPAP therapy improved their adherence when moving to C-Flex., Trial Registration: ISRCTN Register Identifier: 08065291.

Objectives: a) To evaluate in septic patients the blood levels of endocan, a circulating proteoglycan, which regulates leukocyte function-associated antigen-1/intercellular adhesion molecule-1 interactions in vitro; b) to determine whether endocan could be used as a diagnostic and prognostic marker in sepsis in the intensive care unit; and c) to study kinetics of endocan secretion by endothelial cells in vitro after stimulation by soluble mediators involved in sepsis., Design: Prospective observational study., Setting: Intensive care unit of the University Hospitals of Lille, France, and Geneva, Switzerland., Patients: All patients admitted to the intensive care unit over a 6-month period with clinical evidence of severe sepsis or septic shock., Interventions: None., Measurements and Main Results: In vitro, we showed a sustained endocan secretion by endothelial cells after stimulation by lipopolysaccharide and tumor necrosis factor-alpha. Circulating levels of endocan measured in 63 patients admitted to the intensive care unit with sepsis were significantly elevated compared with 20 healthy donors and seven patients with systemic inflammatory response syndrome: 2.71 +/- 4.88 ng/mL vs. 0.77 +/- 0.44 ng/mL vs. 0.68 +/- 1.03 ng/mL (median +/- interquartile range, p < .001). Endocan levels were higher in patients with septic shock (6.11 +/- 12.99 ng/mL, n = 22) than in patients with severe sepsis (1.97 +/- 7.8 ng/mL, n = 12) or sepsis (1.95 +/- 1.63 ng/mL, n = 29). Measurement of endocan at intensive care unit admission revealed higher levels in nonsurvivors (n = 12) than in patients still alive 10 days later (n = 51, 6.98 +/- 13.8 vs. 2.45 +/- 4.09, p < .01)., Conclusions: These results suggest that in septic patients, endocan blood level is related to the severity of illness and the outcome of the patient and may represent a novel endothelial cell dysfunction marker.

- Abstract
Endocan is a proteoglycan specifically secreted by endothelial cells. Through its glycan domains, endocan binds to hepatocyte growth factor and increases its mitogenic activity. Here, we show that human embryonic kidney 293 cells, which have been genetically engineered to overexpress endocan, form tumors when injected s.c. in SCID mice. Both the glycan and a phenylalanine-rich region of endocan are necessary for mediating tumor growth activity. Blocking the phenylalanine-rich region with a monoclonal antibody results in a marked reduction of tumor growth. Finally, we report that circulating levels of endocan are increased in mice with the endocan-expressing human embryonic kidney 293 cell tumors and in a series of adult patients with lung cancer. Taken together, these results suggest that (a) endothelial-derived endocan induces tumor growth, (b) antibodies to endocan may have therapeutic potential, and (c) circulating levels of endocan may eventually represent a novel marker for cancer.

- Abstract
Eosinophil, basophil, and T helper 2 (TH2) cell recruitment into tissues is a characteristic feature of allergic diseases. These cells have in common the expression of the chemokine receptor CCR3, which may represent a specific pathway for their accumulation in vivo. Although animal models of allergic reactions are available, findings cannot always be extrapolated to man. To overcome these limitations, we have developed a humanized mouse model of allergic cutaneous reaction using severe combined immunodeficiency mice engrafted with skin and autologous peripheral blood mononuclear cells from allergic donors. Intradermal injection of the relevant allergen into human skin xenografts from allergic individuals induced a significant recruitment of human CD4(+) T cells, basophils, and TH2-type cytokine mRNA-expressing cells, as well as murine eosinophils. Human skin xenografts, atopic status, and autologous peripheral blood mononuclear cell reconstitution were all mandatory to induce the allergic reaction. Next, we addressed the role of CCR3 in the endogenous mechanisms involved in the inflammatory cell recruitment in this experimental model of allergic cutaneous reaction. In vivo administration of an anti-human CCR3-blocking antibody selectively reduced accumulation of eosinophils but not that of CD4(+) cells, basophils, or cells expressing mRNA for TH2-type cytokines. These findings establish a new in vivo model of humanized allergic reaction and suggest that eosinophil migration is mediated mainly through CCR3. Finally, these results suggest that this model might be useful to test human-specific antiallergic modulators.

Proteoglycans that modulate the activities of growth factors, chemokines, and coagulation factors regulate in turn the vascular endothelium with respect to processes such as inflammation, hemostasis, and angiogenesis. Endothelial cell-specific molecule-1 is mainly expressed by endothelial cells and regulated by pro-inflammatory cytokines (Lassalle, P., Molet, S., Janin, A., Heyden, J. V., Tavernier, J., Fiers, W., Devos, R., and Tonnel, A. B. (1996) J. Biol. Chem. 271, 20458-20464). We demonstrate that this molecule is secreted as a soluble dermatan sulfate (DS) proteoglycan. This proteoglycan represents the major form either secreted by cell lines or circulating in the human bloodstream. Because this proteoglycan is specifically secreted by endothelial cells, we propose to name it endocan. The glycosaminoglycan component of endocan consists of a single DS chain covalently attached to serine 137. Endocan dose-dependently increased the hepatocyte growth factor/scatter factor (HGF/SF)-mediated proliferation of human embryonic kidney cells, whereas the nonglycanated form of endocan did not. Moreover, DS chains purified from endocan mimicked the endocan-mediated increase of cell proliferation in the presence of HGF/SF. Overall, our results demonstrate that endocan is a novel soluble dermatan sulfate proteoglycan produced by endothelial cells. Endocan regulates HGF/SF-mediated mitogenic activity and may support the function of HGF/SF not only in embryogenesis and tissue repair after injury but also in tumor progression.

- Abstract
ICAMs are ligands for LFA-1, a major integrin of mononuclear cells involved in the immune and inflammatory processes. We previously showed that endothelial cell specific molecule-1 (ESM-1) is a proteoglycan secreted by endothelial cells under the control of inflammatory cytokines. Here, we demonstrate that ESM-1 binds directly to LFA-1 onto the cell surface of human blood lymphocytes, monocytes, and Jurkat cells. The binding of ESM-1 was equally dependent on Ca(2+), Mg(2+), or Mn(2+) divalent ions, which are specific, saturable, and sensitive to temperature. An anti-CD11a mAb or PMA induced a transient increase in binding, peaking 5 min after activation. Direct binding of ESM-1 to LFA-1 integrin was demonstrated by specific coimmunoprecipitation by CD11a and CD18 mAbs. A cell-free system using a Biacore biosensor confirmed that ESM-1 and LFA-1 dynamically interacted in real time with high affinity (K(d) = 18.7 nM). ESM-1 consistently inhibited the specific binding of soluble ICAM-1 to Jurkat cells in a dose-dependent manner. These results suggest that ESM-1 and ICAM-1 interact with LFA-1 on binding sites very close to but distinct from the I domain of CD11a. Through this mechanism, ESM-1 could be implicated in the regulation of the LFA-1/ICAM-1 pathway and may therefore influence both the recruitment of circulating lymphocytes to inflammatory sites and LFA-1-dependent leukocyte adhesion and activation.

- Abstract
Dermatomyositis, an inflammatory muscle disease probably related to dysimmunity, is associated with character skin eruptions. Dermatomyositis is often associated with cancer (15 to 40% of cases depending on the series). All histological types and all cancer localizations observed in the general population can be associated with dermatomyositis. We report the case of a patient with dermatomyositis who developed small-cell lung cancer. There have been few descriptions of this association in the literature. Certain clinical features of dermatomyositis would be predictive of its paraneoplastic nature. Prognosis is very poor. Treatment is basically dictated by the underlying neoplasia.
