Abstract

İşsiz genç kadınlar emek piyasasındaki açık pozisyonlar için eşit olarak mücadele edemezler. Bir tarafta işsiz genç kadın, toplumsal cinsiyet rolleri çerçevesinde erkeklerin, sermayenin ve devletin hakimiyetini sarsmayacak şekilde belirlenmiş kriterlere göre iş arar. Diğer tarafta işveren, işe alma ve işten çıkarma pratikleri ile bu kriterleri ve bu kriterlerin bir sonucu olan devletin, erkeklerin ve sermayenin çıkarlarını yeniden üretir. Bu eşitsiz pazarlık veya mücadelenin taraflarını genç kadın ve işveren olarak ele almak genç kadın işsizliğinin nedenlerini irdelemede eksik bir resim ortaya koymaktadır. Devlet yeni-sağ uygulama ve düzenlemeleri ile, emek piyasası 80 sonrası sürecin getirdiği neo-liberal işleyişi ile, aile ataerkil sistemin kendisine sunduğu çeşitli mekanizmalar ile bu pazarlık sürecinin tarafı ve belirleyicisidir. Genç kadın işsizliği makro düzeyde aile, devlet ve emek piyasası arasındaki ataerkil kapitalist eklemlenme süreçlerinin mikro düzeyde genç kadın ve işveren arasındaki pazarlığa yansımasının bir sonucudur, Unemployed young women cannot struggle equally for open positions in the labour market. On the one side, unemployed young woman seeks a job through the criteria determined in a way that it shall not constitute any challenge for the dominance of men, capital and state. On the other side, by his/her hiring and firing practices, employers reproduces both these criteria and state’s, men’s and capital’s interests consequent of these. Considering young woman and employer, as the sole parts of this unequal bargain or struggle will generate inadequate picture in examining the reasons of young women’s unemployment. State with its new right applications and regulations, labour market with its neoliberal operation ensuing from the process since 1980s, family by mechanisms provided by patriarchal system are a party to and identifiers of this bargaining process. Young women’s unemployment is the consequence of the reflection of the patriarchal capitalist articulation mechanisms among family, state and labour market in macro level on the bargain between young women and employer in micro level

Abstract

One of the main objectives of the Spanish Sleep Society is to promote healthy sleep in both the general population and in health professionals. This document aims to conduct a review of the current scientific literature on sleep habits that can serve as the basis on which to establish a set of general recommendations, regarding healthy sleep, for use by the general population in Spain as well as to identify the main challenges faced by research into sleep habits. The document has been developed by a multidisciplinary team made up of members of the Spanish Sleep Society who are experts in paediatric sleep medicine, clinical neurophysiology, pulmonology, neurology, chronobiology, physiology and psychology. The existing scientific literature dealing with sleep habits in the general population was reviewed, and the following aspects were addressed: the current state of sleep habits in the Spanish population; a generic review of the optimum number of hours of sleep; the impact of the environmental setting (noise, temperature, illumination, etc.), hours of sleep, diet and sport, together with several specific sections for children and teenagers, shift-workers and drivers of different vehicles. The conclusions from all the aspects addressed in this document have resulted in a set of final general recommendations that will serve as a guide for the general population and health professionals. Likewise, the principal environmental challenges and future lines of research are also discussed.

Abstract

We have investigated the autonomic function of 95 patients with Parkinson disease (PD) by testing their cardiovascular reflexes and compared the results with those of 53 healthy volunteers. 51.1% of the patients were on antiparkinsonian therapy. The patients showed a smaller heart rate response to deep breathing with a mean exhalation-inspiration difference of 87.73 +/- 7 (p: 0.0005), a smaller heart rate response to standing with a mean 30: 15 index of 1.18 +/- 0.25 (p: 0.01), and a smaller blood pressure rise to handgrip with a mean rise of 9.53 +/- 8.76 mmHg (p: 0.005). We found a higher percentage of patients with established sympathetic lesion (41% vs 34%) or atypical patterns of global autonomic function involvement (33% vs 26%), but the percentage of subjects with parasympathetic lesion was similar in both, patients and controls (16% vs 15%). We found no significant difference on cardiovascular performance between treated and untreated patients. The patients with autonomic impairment were older than the patients with normal autonomic function. Our findings suggest that patients with PD have a true autonomic dysfunction.

6. [A study of parasomnias in the prepubertal age]

Abstract

Parasomnias have been little studied in prepubertal age children. When it happens often they can alter the continuity of the sleep and reduce the value restorer of this affecting the academic functioning and daytime behavioural in younger children. We study the total frequency of the parasomnias, the individual frequency of every parasomnia and the association between them in primary education children.We distribute to children's parents (358) of third through sixth grade of Primary Education of one college of Alcoy the Owens Children's Sleep Habits Questionnaire of for parents to answer.Rate of response was 58 %. Total frequency of the parasomnias was 6.2 %. The most frequent parasomnia of the research was the periodic limb movements (PLM) (7.7 %) followed of bruxism (6.7%), nightmares (1.9%), enuresis (1.9%) and sleepwalking (1.4%). We did not find any case of night terror. There is a major frequency of enuresis, sleepwalking and nightmares in male children and of PLM in the girls though we have not obtained statistical significance. The PLM associated with nightmares (75%, p0.005), sleepwalking (66%, p0.05) and bruxism (40%, p0.005).Parasomnias happen with a frequency raised in the children in prepubertal age. Following the repercussion that a parasomnia can suppose to the child as well as to his environment, it is important to detect it as soon as possible in order to prevent its consequences and not let them continua once the child gets older.

7. Disparities in particulate matter (PM10) origins and oxidative potential at a city scale (Grenoble, France) – Part 1: Source apportionment at three neighbouring sites

Abstract

A fine-scale source apportionment of PM10 was conducted in three different urban sites (background, hyper-center, and peri-urban) within 15 km of the city in Grenoble, France using Positive Matrix Factorization (PMF 5.0) on measured chemical species from collected filters (24 h) from February 2017 to March 2018. To improve the PMF solution, several new organic tracers (3-MBTCA, pinic acid, phthalic acid, MSA, and cellulose) were additionally used in order to identify sources that are commonly unresolved by classic PMF methodologies. An 11-factor solution was obtained in all sites, including commonly identified sources from primary traffic (13 %), nitrate-rich (17 %), sulfate-rich (17 %), industrial (1 %), biomass burning (22 %), aged sea salt (4 %), sea/road salt (3 %), and mineral dust (7 %), and the newly found sources from primary biogenic (4 %), secondary biogenic oxidation (10 %), and MSA-rich (3 %). Generally, the chemical species exhibiting similar temporal trends and strong correlations showed uniformly distributed emission sources in the Grenoble basin. The improved PMF model was able to obtain and differentiate chemical profiles of specific sources even at high proximity of receptor locations, confirming its applicability in a fine-scale resolution. In order to test the similarities between the PMF-resolved sources, the Pearson distance and standardized identity distance (PD-SID) of the factors in each site were compared. The PD-SID metric determined whether a given source is homogeneous (i.e., with similar chemical profiles) or heterogeneous over the three sites, thereby allowing better discrimination of localized characteristics of specific sources. Overall, the addition of the new tracers allowed the identification of substantial sources (especially in the SOA fraction) that would not have been identified or possibly mixed with other factors, resulting in an enhanced resolution and sound source profile of urban air quality at a city scale.

However, in the case where the array elements are fractals, the design of a fractal array may have some problems. Moreover, array feeder bandwidth is usually limited, not allowing a real multiband performance. This abstract proposes a solution to get a real multiband bandwidth. Criteria for the design techniques are here described and results from theoretical analysis using MoM are presented.

Abstract

Introduction RLS affects 5–10% of the general population, but it affects 25% of pregnant women. Sleep disorders are common in RLS. The aim of this study was to determine the frequency of RLS in pregnancy, its characteristics and associated sleep complaints. Materials and methods 50 pregnant women followed at the gynecology department of Vistahermosa Hospital. They completed Stop, Berlin and Pittsburgh questionnaires. The IRLSSG criteria were investigated by self-administered questionnaire and by telephone interview by a sleep specialist. Polysomnograms were obtained with a recording of the movements of both legs. Results 35 of 50 patients (70%) completed all questionnaires and polysomnograms was recorded in 100%. Middle age was 34 ± 4. 20% of the patients meeting IRLSSG criteria, 71% reporting the beginning of the symptoms at the end of the second trimester of pregnancy and 42% described sporadic symptoms before pregnancy (p 5 were found in 14%. Patients with RLS were younger (⩽35 years) (p Conclusion RLS is frequent during pregnancy affecting 20% of pregnant woman especially in the second half and usually resolving after delivery. We want to highlight that a considerable number of patients reported RLS symptoms just before pregnancy and a family history was present. Our patients described symptoms mainly as moderate-high intensity, they didn’t complain of sleep disturbances, perhaps because 50% of them reported a low frequency of symptoms (less than 2–5 nights/week). Although iron, genetics and central nervous dopamine have been shown to play a major role in RLS unrelated to pregnancy, it’s etiology in pregnancy remains unclear. The aim of this study was to determinate risk factors associated to RLS in pregnancy. Materials and methods Healthy pregnant women treated at the Vistahermosa Hospital Gynecology Department were asked to complete IRLSSG criteria by self-administered questionnaire and a sleep specialist made the clinical diagnosis of RLS by telephone interview. PCR was registered with a recording of the movements of both legs. Ferritin levels were determined. Results Fifty patients were registered and 35 (70%) completed all questionnaires and everyone was studied by PCR. Average age was 34 ± 4.20% were in the first trimester, 37% in the second and 43% in the third. Ferritin levels were determined in 66% of cases. 20% of the patients met IRLSSG criteria, when asked by both a self assessment questionnaire and when they had a telephone interview with a sleep specialist. 85% of them had iron supplementation during pregnancy. We found low ferritin levels (⩽40 ng/dl) in a high percentage of pregnant women (75%) and 83% of RLS symptomatic patients had low ferritin levels. The difference was not significant. Family history was positive in 43% of the cases. Of the patients who had delivered at the time 57% reported resolution of the symptom, 75% immediately after delivery and in one case two weeks after it. Conclusion Pregnancy is a period with special conditions affecting the development of RLS but it has not been well investigated. We have found a family history of RLS in almost 50% of the patients, results indicate a genetic predisposition maybe manifested for first time during pregnancy. We also investigated ferritin levels, and although low ferritin levels were frequent during pregnancy in our sample it was not significantly associated with RLS symptoms. Both results and the fact that symptoms usually resolved in the days after delivery, suggests in our opinion that there must be other mechanisms implicated. Abstract

A door-to-door survey of transient ischemic attack was conducted in Muro de Alcoi using a randomized sample. We randomly choose 646 people of the 4.525 included in the census. The prevalence ratio found for TIA and stroke were 12.3 per 1,000 people (95% CI 3.9-20.7). The age-specific prevalence rates for male and for women were 12.4 per 1,000 (95% CI 0.3-24.5) and 12.3 per 1,000 (95% CI 0.4-24.2) respectively. The work suggests that methodology of ascertainment strongly influences in the obtained rates, and reinforces the data of our previous whole population work in the same area.

Abstract

We evaluated cerebrovascular risk factors and the values of cholesterol and triglicerides in the survey in Muro d'Alcoi using a door-to-door randomised interview. We found 14.9% hypertensives, 5.5% persons with diabetes, 1.2% with heart diseases and 42.1 persons with tabaccoism into the 646 persons evaluated. The Serum Cholesterol levels were high and we observed that 62% of males and 63.8% of females had values higher than 200 mg/dl. Our study showed that 24.8% of males and 12% of females presented values of triglicerides higher than 150 mg/dl. The data of risk factors were comparable with the findings described in Beniarres, Lorcha and Planes, located in the same healthy area.

Abstract

The primary sugar compounds (SCs, defined as glucose, arabitol, and mannitol) are widely recognized as suitable molecular markers to characterize and apportion primary biogenic organic aerosol emission sources. This work improves our understanding of the spatial behavior and distribution of these chemical species and evidences their major effective environmental drivers. We conducted a large study focusing on the daily (24 h) PM10 SC concentrations for 16 increasing space scale sites (local to nationwide), over at least 1 complete year. These sites are distributed in several French geographic areas of different environmental conditions. Our analyses, mainly based on the examination of the short-term evolutions of SC concentrations, clearly show distance-dependent correlations. SC concentration evolutions are highly synchronous at an urban city scale and remain well correlated throughout the same geographic regions, even if the sites are situated in different cities. However, sampling sites located in two distinct geographic areas are poorly correlated. Such a pattern indicates that the processes responsible for the evolution of the atmospheric SC concentrations present a spatial homogeneity over typical areas of at least tens of kilometers. Local phenomena, such as the resuspension of topsoil and associated microbiota, do no account for the major emissions processes of SC in urban areas not directly influenced by agricultural activities. The concentrations of SC and cellulose display remarkably synchronous temporal evolution cycles at an urban site in Grenoble, indicating a common source ascribed to vegetation. Additionally, higher concentrations of SC at another site located in a crop field region occur during each harvest periods, indicating resuspension processes of plant materials (crop detritus, leaf debris) and associated microbiota for agricultural and nearby urbanized areas. Finally, ambient air temperature, relative humidity, and vegetation density constitute the main effective drivers of SC atmospheric concentrations.

Abstract

One of the main objectives of the Spanish Sleep Society is to promote healthy sleep in both the general population and in health professionals. This document aims to conduct a review of the current scientific literature on sleep habits that can serve as the basis on which to establish a set of general recommendations, regarding healthy sleep, for use by the general population in Spain as well as to identify the main challenges faced by research into sleep habits. The document has been developed by a multidisciplinary team made up of members of the Spanish Sleep Society who are experts in paediatric sleep medicine, clinical neurophysiology, pulmonology, neurology, chronobiology, physiology and psychology. The existing scientific literature dealing with sleep habits in the general population was reviewed, and the following aspects were addressed: the current state of sleep habits in the Spanish population; a generic review of the optimum number of hours of sleep; the impact of the environmental setting (noise, temperature, illumination, etc.), hours of sleep, diet and sport, together with several specific sections for children and teenagers, shift-workers and drivers of different vehicles. The conclusions from all the aspects addressed in this document have resulted in a set of final general recommendations that will serve as a guide for the general population and health professionals. Likewise, the principal environmental challenges and future lines of research are also discussed.

Abstract

The need to guarantee equitable access to health regardless of geographic, economic, or technological barriers motivated the Member States of the Organization of American States to create the Telehealth Network of the Americas, coordinated by the Inter-American Telecommunication Committee. The Network focuses on the use of new information and communications technology applied to health, based on the values of respect, equity, and solidarity and mandated by the philosophy of primary health. Its members include government agencies, nongovernmental organizations, university forums, hospital federations, and telecommunications companies, and it has already extended its reach to other continents and to different fields in which telemedicine is being used. Among its first achievements, it has implemented an innovative tool to be used in cases of disaster or limited geographic access. This mobile telemedicine station is housed in a portable case that includes a computer, various digital devices (otoscope, ophthalmoscope, microscope, dermatoscope), a high-resolution digital camera, an X-ray film viewer, and a satellite antenna. With this tool, it is possible to provide specialized support for rural physicians and primary health care workers located far from large urban centers.

Introduction: Sleep deficit in children has been associated with psychological and behavioral health alterations as well as an increased incidence of obesity and accidents. Several studies have assessed the effects of poor sleep patterns on adolescents and young adults; however few studies have been conducted in children and pre-teenagers. The main objective of this study was to examine sleep habits in primary school children., Methods: The parents of primary school children in Alcoy (Spain) kept a sleep diary for their children for 1 week. Parents were asked to record the time their children went to bed, woke up and got out of bed, as well as sleep latency and total sleep time on school days and weekends. The data were analyzed and differences according to grade and gender were noted., Results: Three hundred twenty-one sleep diaries were studied; 53% were from boys and 47% from girls. As grade increased, children went to bed later and slept less (p<0.001). Children slept more hours on weekend than during the week. There was a substantial gender difference at the highest grade level: girls slept 23min more than boys on weekends (p<0.001). Sleep latency was greater than 20min in 19% of children, primarily in girls 10-12 years of age. There were a considerable percentage of children in each group who slept less than average for their age., Conclusion: Sleep patterns in primary school children have been underreported. There appears to be insufficient sleep and delayed sleep-phase patterns among pre-teenagers.

Introduction and Aims: Parasomnias have been little studied in prepubertal age children. When it happens often they can alter the continuity of the sleep and reduce the value restorer of this affecting the academic functioning and daytime behavioural in younger children. We study the total frequency of the parasomnias, the individual frequency of every parasomnia and the association between them in primary education children., Patients and Methods: We distribute to children's parents (358) of third through sixth grade of Primary Education of one college of Alcoy the Owens Children's Sleep Habits Questionnaire of for parents to answer., Results: Rate of response was 58 %. Total frequency of the parasomnias was 6.2 %. The most frequent parasomnia of the research was the periodic limb movements (PLM) (7.7 %) followed of bruxism (6.7%), nightmares (1.9%), enuresis (1.9%) and sleepwalking (1.4%). We did not find any case of night terror. There is a major frequency of enuresis, sleepwalking and nightmares in male children and of PLM in the girls though we have not obtained statistical significance. The PLM associated with nightmares (75%, p < 0.005), sleepwalking (66%, p < 0.05) and bruxism (40%, p < 0.005)., Conclusions: Parasomnias happen with a frequency raised in the children in prepubertal age. Following the repercussion that a parasomnia can suppose to the child as well as to his environment, it is important to detect it as soon as possible in order to prevent its consequences and not let them continua once the child gets older.
