Staphylococcus aureus - both meticillin-resistant S. aureus (MRSA) and meticillin-susceptible S. aureus (MSSA) - is a major cause of neonatal infections. Infection control measures have not lowered the incidence of MSSA infections to the same degree as that of MRSA infections.To investigate the transmission pathway of MSSA in neonatal intensive care unit (NICU) using genetic analysis.Neonatal patients, their parents, and healthcare workers were swab-tested in the NICU at our hospital at the time of hospitalization and then every month thereafter from October 1There were 16 MSSA-positive patients. Four MSSA-positive patients shared strains from the same phylogenetic groups as those of healthcare workers. One presented the same strain as the parent. MSSA-positive twin neonates shared the same strain. Ten had sporadic strains; 32 of the 97 tested healthcare workers were MSSA positive.The findings of this study suggest that the route of transmission of MSSA in NICU may be through MSSA in the hospital environment in addition to horizontal transmission via healthcare workers. Along with hand hygiene with alcohol, thorough environmental maintenance and parental education are important for infection control in NICUs targeting MSSA.

We analyze Ha or CO rotation curves (RCs) extending out to several galaxy effective radii for 100 massive, large, star-forming disk galaxies (SFGs) across the peak of cosmic galaxy star formation (z~0.6-2.5), more than doubling the previous sample presented by Genzel et al. (2020) and Price et al. (2021). The observations were taken with SINFONI and KMOS integral-field spectrographs at ESO-VLT, LUCI at LBT, NOEMA at IRAM, and ALMA. We fit the major axis kinematics with beam-convolved, forward models of turbulent rotating disks with bulges embedded in dark matter (DM) halos, including the effects of pressure support. The fraction of dark to total matter within the disk effective radius ($R_e ~ 5 kpc$), $f_DM (R_e)=V_{DM}^2 (R_e)/V_{circ}^2 (R_e)$, decreases with redshift: At z~1 (z~2) the median DM fraction is $0.38\pm 0.23$ ($0.27\pm 0.18$), and a third (half) of all galaxies are "maximal" disks with $f_{DM} (R_e)2.5 M_{\odot} yr^{-1} kpc^{-2}$) and galaxies with massive bulges ($M_{bulge}>10^{10} M_{\odot}$). A combination of stellar or active galactic nucleus (AGN) feedback, and/or heating due to dynamical friction, either from satellite accretion or clump migration, may drive the DM from cuspy into cored mass distributions. Background It was reported that glucagon-like peptide-1 has cardio-protective effects, however, the occurrence of heart failure was increased with DPP4 inhibitor in some large clinical trials. Purpose The purpose of the present work was to clarify whether DPP4 inhibitor had any effects on chronic, dilated heart failure and to identify the mechanisms. Methods Heart / Muscle-specific Manganese superoxide dismutase (Mn-SOD)-deficient mice, (H/M-Sod−/−) which exert dilated cardiomyopathy were randomized to receive low dose (1 mg/kg) linagliptin, HF-L group; high dose (10 mg/kg) linagliptin, HF-H group; mixed with food, or normal food, HF group; for eight weeks. Then, evaluation was performed in cardiac function and pathophysiological changes. Result Interestingly, the mice treated with linagliptin treatment exhibited shorter life span and greater heart/body weight ratio. Echocardiographic studies showed decreased contractility in the HF-H group with compared to the HF control group. In Sirius Red staining, severer fibrosis was also observed in the linagliptin groups. HbA1c levels did not change among three groups, however, glycogen content in the liver was significantly decreased in the linagliptin groups. Similarly, indirect calorimetry showed decreased carbohydrate consumption in the HF-H group compared to the HF control group, suggesting worsening of heart failure was due to unavailability of carbohydrates as an energy source. Therefore, we fed a diet containing 33 wt% glucose to the HF-H group as complementary experiments. As a result, adequate glucose supplementation reduced cardiac fibrosis and cardiac function, while worsened life span, HbA1c levels and CHO consumption were not changed. Therefore, we hypothesized that there might be a change in the absorption of glucose in intestine or utilization ability of glucose in body another experiments were performed. 13C, which was released from mice after the oral administration of 13C glucose and measured by respiratory gas analysis, was significantly reduced in HF-H mice. The measurements the amount of 13C in feces revealed increased levels of unabsorbed glucose by linagliptin treatment in HF. In addition, GLUT2, a glucose transporter, in small intestine, was downregulated in the HF-H and the HF-HG groups. On the other hand, the cardiac uptake of Deoxy-D-glucose, 2-[1,2-3H(N)] (3H-2DG), which was injected from mice tail vein, was not different between in the HF-H group and in the HF control group. Taken together, linagliptin treatment may inhibit glucose transport in intestine in HF. Conclusions Linagliptin treatment exacerbated heart failure, resulting in shortened life span, worsened cardiac function, and fibrosis of the myocardium. DPP4 inhibitor may promote the cardiac cachexia and exacerbate heart failure at least, in part, through modification of glucose absorption and utilization. Background Atrial fibrillation (AF) is a risk factor of Regurgitation of Atrioventricular Valve and Regurgitation of Atrioventricular Valve is improved after ablation for AF. However, Mechanism of improvement of Tricuspid regurgitation (TR) and Mitral regurgitation (MR) after ablation for AF were unclear. Methods The one hundred fifty-seven cases consecutive patients who were underwent ablation for persistent AF were examined in this study. These patients were performed Holter ECG and Echocardiography 6 months after ablation for persistent AF. The area of the Right Atrium (RA), Left Atrium (LA), diameter of the tricuspid valve anulus and Mitral valve annulus, were measured by echocardiography at before and 6 months after ablation for AF. The reverse remodeling index (RRI) was defined as divided post area of RA and LA by pre area of RA and LA. The Improved group was defined as one or more than regurgitation grade improvements. Results TR improved in 56 cases (35.7%) of 157 cases and MR improved in 25 (16.6%) cases of 157 cases. The ratio of improved TR was significantly higher than ratio of improved MR (p Conclusion Reverse remodeling of RA was higher than Reverse remodeling of LA among patients who were able to maintain sinus rhythm after ablation for persistent AF and Reverse remodeling of RA and LA were important factors of improvement of TR and MR after ablation for persistent AF. OBJECTIVE Gamma Knife radiosurgery (GKRS) is an established surgical option for the treatment of trigeminal neuralgia (TN), particularly for high-risk surgical candidates and those with recurrent pain. However, outcomes after three or more GKRS treatments have rarely been reported. Herein, the authors reviewed outcomes among patients who had undergone three or more GKRS procedures for recurrent TN. METHODS The authors conducted a multicenter retrospective analysis of patients who had undergone at least three GKRS treatments for TN between July 1997 and April 2019 at two different institutions. Clinical characteristics, radiosurgical dosimetry and technique, pain outcomes, and complications were reviewed. Pain outcomes were scored on the Barrow Neurological Institute (BNI) scale, including time to pain relief (BNI score ≤ III) and recurrence (BNI score > III). RESULTS A total of 30 patients were identified, including 16 women and 14 men. Median pain duration prior to the first GKRS treatment was 10 years. Three patients (10%) had multiple sclerosis. Time to pain relief was longer after the third treatment (p = 0.0003), whereas time to pain recurrence was similar across each of the successive treatments (p = 0.842). Complete or partial pain relief was achieved in 93.1% of patients after the third treatment. The maximum pain relief achieved after the third treatment was significantly better among patients with no prior percutaneous procedures (p = 0.0111) and patients with shorter durations of pain before initiation of GKRS therapy (p = 0.0449). New or progressive facial sensory dysfunction occurred in 29% of patients after the third GKRS treatment and was reported as bothersome in 14%. One patient developed facial twitching, while another experienced persistent lacrimation. No statistically significant predictors of adverse effects following the third treatment were found. Over a median of 39 months of follow-up, 77% of patients maintained complete or partial pain relief. Three patients underwent a fourth GKRS treatment, including one who ultimately received five treatments; all of them reported sustained pain relief at the extended follow-up. CONCLUSIONS The authors describe the largest series to date of patients undergoing three or more GKRS treatments for refractory TN. A third treatment may produce outcomes similar to those of the first two treatments in terms of long-term pain relief, recurrence, and adverse effects.

To view a copy of this licence, visit http://creativecommons.org/licen ses/by/4.0/, Our knowledge about the solar chromosphere and transition region (TR) has increased in the last decade thanks to the huge scientific return of space-borne observatories like SDO, IRIS, and Hinode, and suborbital rocket experiments like CLASP1, CLASP2, and Hi-C. However, the magnetic nature of those solar regions remain barely explored. The chromosphere and TR of the Sun harbor weak fields and are in a low ionization stage both having critical effects on their thermodynamic behavior. Relatively cold gas structures, such as spicules and prominences, are located in these two regions and display a dynamic evolution in high-resolution observations that static and instantaneous 3D-magnetohydrodynamic (MHD) models are not able to reproduce. The role of the chromosphere and TR as the necessary path to a (largely unexplained) very hot corona calls for the generation of observationally based, time-dependent models of these two layers that include essential, up to now disregarded, ingredients in the modeling such as the vector magnetic field. We believe that the community is convinced that the origin of both the heat and kinetic energy observed in the upper layers of the solar atmosphere is of magnetic origin, but reliable magnetic field measurements are missing. The access to sensitive polarimetric measurements in the ultraviolet wavelengths has been elusive until recently due to limitations in the available technology. We propose a low-risk and high-Technology Readiness Level (TRL) mission to explore the magnetism and dynamics of the solar chromosphere and TR. The mission baseline is a low-Earth, Sun-synchronous orbit at an altitude between 600 and 800 km. The proposed scientific payload consists of a 30 cm aperture telescope with a spectropolarimeter covering the hydrogen Ly-alpha and the Mg II h&k ultraviolet lines. The instrument shall record high-cadence, full spectropolarimetric observations of the solar upper atmosphere. Besides the answers to a fundamental solar problem the mission has a broader scientific return. For example, the time-dependent modeling of the chromospheres of stars harboring exoplanets is fundamental for estimating the planetary radiation environment. A 66 year-old man with a previous history of Sjögren's syndrome was admitted with anastomotic pseudoaneurysm and aortic dissection in the ascending aorta, which developed after Bentall's surgery, a procedure that has severe complications and high mortality. Using the reverse extra-anatomical aortic arch debranching technique, zone 0 thoracic endovascular aortic repair was performed emergently. The postoperative course was uneventful. Twenty months later, computed tomography showed remodeling of the dissection, resolution of the pseudoaneurysm, and patency of the reverse extra-anatomical aortic arch debranching graft.

Funding Acknowledgements Type of funding sources: None. Background Anthracycline cardiotoxicity is an important prognostic determinant in childhood cancer survivors (CCSs). That has been reported to improve with early therapeutic intervention, and because it is dose-dependent and progressive, early diagnosis and long-term follow-up are important. In adult survivors of cancer, the longitudinal strain (LS) is useful as a sensitive index of cardiac function. In childhood cancer survivors, strain abnormalities are also observed at both short-term and long-term follow-up. Global longitudinal strain (GLS) abnormalities are common during or early after chemotherapy, whereas changes in global circumferential strain (GCS) are more significant and consistent on long-term follow-up. Purpose In this study, we aimed to conduct a cross-sectional study using strain analysis in childhood cancer survivors of a wide age range to clarify the mode of progression of anthracycline-induced cardiotoxicity and to identify useful indicators for long-term follow-up. Methods In total, 116 patients (median age: 15.5 [range: 4.7-40.2] years) with childhood cancer who had passed at least 1 year after chemotherapy with anthracycline, and 116 control patients of similar age. Strain measurements were assessed for longitudinal strain (LS) and circumferential strain at the apical, papillary and basal levels using speckle tracking imaging. Estimated value at 5, 15, 25 and 35 years old were mathematically calculated. Results Results were shown in tables and figures. Most of conventional echocardiographic parameters were not significantly different between CCCs and controls. LS, papillary CS and basal CS in CCCs decreased compared to normal controls at all age. The difference of estimated value between CCCs and normal controls in LS at all age were relatively constant. However, those in basal CS tended to increase with aging. Furthermore, basal CS in CCCs decreased with aging (r = 0.212, p < 0.001) and the duration after completion of anthracycline treatment (r = -0.244, p < 0.008). Conclusions In childhood cancer survivors, strain analysis is a more sensitive indicators of cardiac function than conventional parameters. In addition, basal CS may decrease most markedly over time and could be useful indicator in long-term follow-up. Longitudinal studies should be conducted in the future to improve the accuracy of predicting anthracycline cardiotoxicity. Polycyclic Aromatic Hydrocarbons (PAHs) are carbon-based molecules that are ubiquitous in a variety of astrophysical objects and environments. In this work, we use JWST/MIRI MRS spectroscopy of three Seyferts to compare their nuclear PAH emission with that of star-forming regions. This study represents the first of its kind using sub-arcsecond angular resolution data of local luminous Seyferts (Lbol>10^44.46 erg/s) on a wide wavelength coverage (4.9-28.1 micron). We present an analysis of their nuclear PAH properties by comparing the observed ratios with PAH diagnostic model grids, derived from theoretical spectra. Our results show that a suite of PAH features is present in the innermost parts (~0.45 arcsec at 12 micron; in the inner ~142-245 pc) of luminous Seyfert galaxies. We find that the nuclear regions of AGN lie at different positions of the PAH diagnostic diagrams, whereas the SF regions are concentrated around the average values of SF galaxies. In particular, we find that the nuclear PAH emission mainly originates in neutral PAHs. In contrast, PAH emission originating in the SF regions favours ionised PAH grains. The observed PAH ratios in the nuclear region of AGN-dominated galaxy NGC 6552 indicate the presence of larger-sized PAH molecules compared with those of the SF regions. Therefore, our results provide evidence that the AGN have a significant impact on the ionization state (and probably the size) of the PAH grains on scales of ~142-245 pc., 12 pages, 10 figures. The CLASP2 (Chromospheric LAyer SpectroPolarimeter 2) sounding rocket mission was launched on 2019 April 11. CLASP2 measured the four Stokes parameters of the Mg II h & k spectral region around 2800 Angstroms along a 200 arcsecond slit at three locations on the solar disk, achieving the first spatially and spectrally resolved observations of the solar polarization in this near ultraviolet region. The focus of the work presented here is the center-to-limb variation of the linear polarization across these resonance lines, which is produced by the scattering of anisotropic radiation in the solar atmosphere. The linear polarization signals of the Mg II h & k lines are sensitive to the magnetic field from the low to the upper chromosphere through the Hanle and magneto-optical effects. We compare the observations to theoretical predictions from radiative transfer calculations in unmagnetized semi-empirical models, arguing that magnetic fields and horizontal inhomogeneities are needed to explain the observed polarization signals and spatial variations. This comparison is an important step in both validating and refining our understanding of the physical origin of these polarization signatures, and also in paving the way toward future space telescopes for probing the magnetic fields of the solar upper atmosphere via ultraviolet spectropolarimetry., 14 pages, 5 figures, to be published in the Astrophysical Journal (ApJ), initially submitted May 2022, revised submission July 2022
