Objective:This study aimed to investigate the factors associated with pancreatic fistula (PF) development after pancreaticoduodenectomy (PD) at our clinic.Methods:Patients who underwent PD for periampullary tumours between 2010 and 2019 were included in the study and categorised into Group I (with PF) and Group II (without PF). The demographic and clinical characteristics, laboratory parameters, tumour characteristics and post-operative results were compared between the groups. Risk factors for PF were analysed by univariate analysis and multivariate logistic regression analysis.Results:In total, 155 patients participated in the study (Group I: n=31; Group II: n=124). The rate of PF was 20%. The two groups were comparable with regard to sex (p=0.348) and age (64.8 vs 66.9 years, p=0.916). Compared with Group II, Group I had a higher number of metastatic lymph nodes (1.61 vs 0.87, p=0.041), a higher number of post-operative complications (58.1% vs 21.8%, p=0.000) and a longer duration of post-operative hospital stay (25.25 vs 16.43 days, p=0.000). Haemoglobin (p=0.493) and albumin (p=0.698) levels were similar between the groups. Total survival duration was shorter in Group I (23.9 vs 38.18 months, p=0.024). In multivariate analyses, being ≥65 years (p=0.040), tumour localisation (p=0.021), lymph node stage (p=0.008) and performedtumour diameter ≥2 (p=0.021) were the independent risk factors for developing pancreatic fistula.Conclusion:In our study, tumour diameter, patient age and lymph node status were associated with PF development. The development of PF reduced expected survival. We believe that identifying the preoperative, intraoperative and post-operative factors related to PF formation may help decrease its development.

Aim:In this study, we aimed to share our surgical experiences and clinical results in non-traumatic small bowel perforations.Method:Patients who underwent surgical treatment for non-traumatic small bowel perforation between 2009-2019 were included in the study. Patients were divided into two groups according to postoperative 90-day mortality status: Group 1 (no mortality), Group 2 (mortality). The demographic, clinical features, treatment methods and results of the patients were compared between the groups.Results:Forty-two patients participated in our study. Group 1 consisted of 25, and Group 2 consisted of 17 patients. Male sex was dominant in Group 2 (48% vs 76.5%, p=0.062). Mean age was higher in Group 2 (54 vs 61, p=0.218). American Anesthesiology Association score (ASA) was statistically significantly higher in Group 2 (12% vs 58.8%). Forty-two percent of patients had abdominal surgery and 30% had a history of malignancy. The presence of electrolyte imbalance was similar in the groups (56% vs 76.5%, p=0.049). The days between the first complaint and laparotomy were similar between the groups (6.40 vs. 5.70 p=0.699). In perforated areas, jejunum was dominant in Group 2 with 64.7%, and ileum in Group 1 with 68%. The multiple perforation rate was higher in Group 2, but was not statistically significant (12% vs 23.5%, p=0.284). From postoperative complications, anastomosis leakage was higher in Group 1, but it was not statistically significant (12% vs 5.9%, p=0.501).Conclusion:Morbidity and mortality of non-traumatic small bowel perforations is high. While the ASA score and hypoalbuminemia were associated with postoperative mortality in non-traumatic small bowel perforations, we did not find the localization of the perforation and the time between the first complaint and laparotomy to be related to mortality.

© 2021, The Society for Surgery of the Alimentary Tract.Purpose: In this study, it was aimed to determine the predictors of clinically relevant postoperative pancreatic fistula (CR-POPF) following pancreaticoduodenectomy (PD) and clinical significance of mean platelet volume (MPV)/total platelet count ratio (MPR) as a new predictor for CR-POPF. Methods: A total of 105 patients who underwent PD consecutively due to periampullary located diseases were included in the study. Patients were divided into two groups as CR-POPF and no postoperative pancreatic fistula (No-POPF). Demographic parameters, preoperative serum-based inflammatory indicators, surgical procedures, intraoperative findings, and histopathological parameters were recorded retrospectively from prospectively recorded patient files and compared between the groups. Results: CR-POPF occurred in 16 (15.2%) patients: 8 (7.6%) were grade B and 8 (7.6%) were grade C according to the ISGPF classification. In univariate analysis, intraoperative blood loss > 580 mL (OR: 5.25, p = 0.001), intraoperative blood transfusion (OR: 5.96, p = 0.002), intraoperative vasoconstrictor medication (OR: 4.17, p = 0.014), benign histopathology (OR: 3.51, p = 0.036), and poor differentiation in malignant tumors (OR: 4.07, p = 0.044) were significantly higher in the CR-POPF group, but not significant in multivariate analysis. Soft pancreatic consistency (OR: 6.08, p = 0.013), pancreatic duct diameter < 2.5 mm (OR: 17.15, p < 0.001), and MPR < 28.9 (OR: 13.91, p < 0.001) were the independent predictors of CR-POPF according to multivariate analysis. Neoadjuvant treatment history and simultaneous vascular resection were less likely to cause CR-POPF development; however, they were insignificant. Conclusion: Soft pancreatic consistency, pancreatic duct diameter, and preoperative MPR were the independent predictors of CR-POPF following PD. Decreased MPR is a strong predictor for CR-POPF and should be considered when deciding treatment strategies.

Summary: Objective: The most common complication after thyroid surgery is hypocalcaemia and it sometimes leads to problems that are difficult to correct in patients. The present study was aimed to determine the risk factors causing the development of hypocalcaemia after thyroid surgery. Methods: 818 adult patients were included in the study. The data were recorded by examining the hospital automation system and patient files retrospectively. Patients' demographic characteristics, radiological imaging findings, serum biochemical parameters, extent of the performed surgery, histopathological diagnoses were recorded. Background: This study aimed to analyze the effects of quercitrin, which has anti-inflammatory properties, on bacterial translocation in inflammatory bowel diseases by using an experimental colitis model. Methods: Forty male Wistar-Albino rats were used in the study. Rats were divided into 4 groups (control, colitis, treatment 1 and 2 groups). The rats in the control group were given normal drinking water. In the colitis group, colitis was induced by 5% DSS in drinking water. The control and colitis groups underwent operation on Day 7. In the 2 treatment groups, 5% DSS was added to drinking water for the first 7 days and the groups were treated with quercitrin at the doses of 1 and 5 mg/kg/day for the following 10 days. Treatment groups operated on Day 18. Blood samples were taken for blood culture and left colectomy was performed. The inflammation in the colon was macroscopically and microscopically evaluated and graded. In this study, we aimed to evaluate patients who underwent curative surgical treatment for primary duodenal adenocarcinoma and to present our experience.Patients diagnosed with primary duodenal adenocarcinoma between 2006 and 2018 participated in the study. The demographic and clinical characteristics of the patients, details of the operation, pathological features of the tumors, short- and long-term follow-up results, and mean survival were evaluated retrospectively.Nine patients with a mean age of 54.7 participated in the study. 55% of the patients were male. The most common presenting symptom was abdominal pain (n: 6; 67%). The most common tumor localization was D2-3 (n: 7; 78%), and the most common surgical operation was pancreaticoduodenectomy (n: 7; 78%). There were no intraoperative complications in any patient. The mean tumor diameter was 3.5 cm. The mean number of lymph nodes dissected was 8.3 and the mean number of metastatic lymph nodes was 2. The most common postoperative complication was pancreatic fistula (n: 3; 33%). The mean length of stay was 21.8 days. One patient developed septic shock and mortality happened within the 30-day period. The most common cause of unplanned admission to the hospital within 90 days was wound infection (n: 2; 22%). One patient developed local recurrence and two patients had systemic metastasis. We found an average survival of 40 months.Pancreaticoduodenectomy is the most common approach in its curative surgery and it has a long survival despite the high postoperative complication rate. Lo studio è stato effettuato sui pazienti con diagnosi di adenocarcinoma duodenale primario trattati tra il 2006 e il 2018. Sono stati valutati retrospettivamente le caratteristiche demografiche e cliniche dei pazienti, i dettagli dell’operazione, le caratteristiche patologiche dei tumori, i risultati di follow-up a breve e lungo termine e la sopravvivenza media. Sono stati inclusi nello studio nove pazienti con un’età media di 54.7 anni: 55% dei pazienti era di sesso maschile. Il sintomo più comune al ricovero era il dolore addominale (n. 6; 67%). La localizzazione del tumore più comune è stata la D2-3 (n. 7; 78%) e l’operazione chirurgica più frequentemente eseguita è stata la pancreaticoduodenectomia (n. 7; 78%). Non ci sono state complicanze intraoperatorie in nessun paziente. Il diametro medio del tumore era di 3,5 cm. Il numero medio di linfonodi dissezionati era 8,3 e il numero medio di linfonodi metastatici era 2. La complicazione postoperatoria più comune era la fistola pancreatica (n. 3; 33%). La durata media della degenza è stata di 21,8 giorni. Un paziente è andato incontro a shock settico e mortalità seguita entro 30 giorni. La causa più comune di ricovero non programmato in ospedale entro 90 giorni è stata l’infezione della ferita (n. 2; 22%). Un paziente ha presentato recidiva locale e due pazienti hanno presentato metastasi sistemiche. Abbiamo registrato una sopravvivenza media di 40 mesi. La pancreaticoduodenectomia è l’approccio più comune quale chirurgia curativa con lunga sopravvivenza nonostante l’alto tasso di complicanze postoperatorie. Raccomandiamo dunque la resezione radicale nel trattamento chirurgico dell’adenocarcinoma duodenale primario.

Aim:We aimed to determine the clinical value of erythrocyte distribution width (RDW) and mean platelet volume (MPV) in the diagnosis of acute appendicitis (AA) and in determining the severity of the disease.Method:Patients who were operated between January 2013 and January 2020 with a preliminary diagnosis of AA were included in the study. The patients were divided into two groups as Group 1 negative appendectomy and Group 2 acute appendicitis. In addition, Group 2 is divided into subgroups as a:perforated and b: non-perforated. RDW and MPV were compared between the groups and subgroups.Results:A total of 861 patients participated in our study. Group 1 consisted of 144 patients and Group 2 consisted of 717 patients. The mean age was similar in both groups (33.02 vs 35.34, p=0.088), female sex was higher in Group 1 (52.8% vs 42.9, p=0.019), RDW was higher in Group 1 (14.03 vs 13.67, p=0.007), MPV was similar between the groups (8.81 vs 8.95, p=0.363). RDW was an independent risk factor in the diagnosis of AA in multivariate analysis (odds ratio 0.604, 95% confidence interval (minimum-maximum) 0.420-0.868, p=0.006). RDW and MPV were independent variables in the diagnosis of perforated appendicitis in multivariate logistic regression analysis.Conclusion:RDW is a useful parameter in the diagnosis of AA and in determining the severity of the disease. However, MPV value does not have sufficient diagnostic value.

Summary: Objective: The purpose of this study was to determine the risk factors that caused non-sentinel lymph nodes (nonSLNs) metastasis by considering the clinicopathological characteristics of patients who have 1–2 sentinel lymph node (SLN) metastasis in the clinical early stage (T1-2, N0) breast cancer. Methods: The demographic and clinicopathological characteristics of the patients were recorded retrospectively. Among these, age, size of the primary breast tumor, tumor localization and multifocality/multicentricity status, preoperative serum Neutrophil/Lymphocyte rate (NLR), c-erbB2/HER2-neu status, Estrogen Receptor (ER) and Progesterone Receptor (PR) status, primary tumor proliferation index (Ki-67), histopathological grade, molecular subtypes, histopathological subtypes, nipple/areola infiltration, Lymphatic Invasion (LI), Vascular Invasion (VI), Perineural Invasion (PNI), number of metastatic SLN m(SLN), mSLN diameter, SLN Extranodal Extension (ENE) status, and number of metastatic nonSLNs were recorded. Results: According to the univariate analysis, the HER2 positivity, Ki-67≥%20, mSLN diameter, LI, VI, PNI, ENE and molecular subtypes were found to be significant. However, the age, tumor localization, multifocality/multicentricity, T stage, ER and PR status, tumor size, histopathological grade and subtypes, nipple/areola infiltration and NLR were not found to be significant. In the multivariate analysis, significant independent predictors in nonSLN metastasis development were found to be HER2 positivity, PNI, mSLN diameter ≥10,5 mm and ENE. Conclusion: The HER2 positivity, ENE, PNI and mSLN diameter ≥10,5 mm were found to be very strong predictors in nonSLN metastasis development. The findings of this study have the potential to be a guideline for surgeons and oncologists when determining their patients' treatment plan. These components are candidates for inclusion among the clinicopathological factors that may be used in the new nomograms due to their higher sensitivity and specificity. The association of rectal prolapse and colorectal cancer is quite rare and only a few cases have been reported previously in the literature. It is unclear whether colorectal cancer triggers rectal prolapse.A 77-year-old male patient presented to our emergency department with complete rectal prolapse, and an anterior resection was performed after rectal digital examination revealed a mass. The pathology result came back as mucinous adenocarcinoma in the sigmoid colon and the postoperative period was uneventful.Considering the age group in which rectal prolapse is most commonly seen, and the change in bowel habits, chronic constipation and irritation chronic seen in rectal prolapse may be responsible for the development of rectum cancer, therefore endoscopic screening should not be overlooked in rectal prolapse cases.Anorectal emergencies, Colon cancer, Rectal prolapse.L’associazione tra prolasso rettale e carcinoma del colonretto è piuttosto rara e solo pochi casi sono stati riportati nella precedente letteratura. Non è chiaro se il tumore del colon-retto inneschi il prolasso rettale. Viene qui presentato il caso di un uomo di 77 anni giunto al nostro pronto soccorso con prolasso rettale completo e su cui si è eseguita una resezione anteriore dopo che l’esame digitale rettale aveva ha rivelato la presenza di una massa. Il referto patologico è stato di adenocarcinoma mucinoso nel colon sigmoideo. Il periodo postoperatorio è stato privo di eventi. Considerando la fascia di età in cui si osserva più comunemente il prolasso rettale con il cambiamento nelle abitudini intestinali, costipazione cronica e irritazione osservati nel prolasso rettale cronico possono essere responsabili dello sviluppo del cancro del retto, pertanto lo screening endoscopico non deve essere trascurato nel caso del prolasso rettale.

Although the surgical technique of laparoscopic appendectomy (LA) stump has been well understood, there are many alternative techniques in relation to torocar positioning and closure of appendicular stump. In recent times, Hem-o-lok polymer clips (HOLP) was implemented in several studies in the closure of appendicular stump because of its lower cost and easy implementation. The purpose of this study to investigate the safety, usefulness, and cost effectiveness of HOLP for the closure appendecular stumps in LA. The study was carried out between December 2011 and December 2013. Patients with acute appendicitis were included in the study. Two groups were defined as patients with the HOLP and patients with endoloop. The prospectively collected data, including age, sex, body mass index, operative time, hospital stay, cost effectiveness, intraoperative, and postoperative complications were retrospectively analyzed. A total of 66 (35 male, 31 female) LA were performed. The endoloop group consisted of 30 patients (16 male, 14 female, and mean age, 30.4 ± 1.8), while the HOLP group consisted of 36 patients (19 male, 17 female, and mean age, 28.6 ± 1.6). The mean operative time was 42.5 ± 1.3 in the HOLP group and 53.8 ± 1.5 in the endoloop group (P < 0.0001). The mean hospital stay was 2.1 ± 0.2 days in HOLP group and 2 ± 0.2 in the endoloop group (P = 0.73). Both patient groups had no intraoperative complication, and no cases were converted to open procedure. Total hospital cost was 1170.8 ± 6.3 dollars in the HOLP group and 1094 ± 6.9 dollars in the endoloop group (P < 0.0001). The use of HOLP for the appendicular stumps in LA is a feasible, safe, and cost-effective procedure in patients with uncomplicated acute appendicitis.

Background: Our aim was to investigate the pathologies in the hernia sac in adults, and the frequency of malignancy as well as to confirm the necessity of maintaining the current applications in histological examination of the hernia sac. Methods: Patients who were operated for hernia in our clinic from 2013 to 2019 were included in the study. Patient data were evaluated retrospectively. We divided the patients into four groups, according to the type of hernia. We evaluated the demographic characteristics of the patients, the pathologies within the hernia sac, histopathological examination outcomes of the hernia sac and clinical features of malignancy in patients with malignancy. Results: A total number of 556 adult patients underwent inguinal, femoral, umbilical or incisional hernia repair in our hospital. Nine patients (0.61%) had malignancy in the hernia sac. Three out of nine patients (33%) had no preoperative diagnosis of malignancy. Six patients (67%) had a known history of malignancy. Two tumors were located in the inguinal (22.0%), six tumors in the incisional (67%), and one in the umbilical (11%) hernia sacs. Among these, 56% were of gastrointestinal, 22% of gynecological, 11% of breast and 11% of epididymis origin. Most of the other pathologies found in the hernia sac were herniated bowel segments, lipomas and omentum. Conclusion: Since the hernia sac might be the first clue for an underlying cancer, if abnormal pathological findings are detected during surgery, histopathological examination should be performed to exclude malignancy. Aim: The purpose of this study was to determine the postoperative complications in total thyroidectomy patients based on the weight of the thyroid gland with retrospective screening.Material and Methods: A total of 263 adult patients, who underwent total thyroidectomy were included. The data automation system of the hospital and the files of the patients were examined retrospectively.Results: The total complication incidence was 24.3%, and these were determined as hypocalcemia, vocal cord paralyses (VCP), and hemorrhage and seroma at the surgery area. The rate of temporary hypocalcemia and permanent hypocalcemia rate was 20.1% and 1.5% respectively. The temporary VCP rate was 0.3% (n=1), and no permanent VCP was observed in the patients. The median thyroid weight was measured as 50 gr in patients without postoperative hypocalcemia, and as 40 g in the patients with hypocalcemia (p=0.283). There was no significant relation between the variability in the weight of the thyroid and postoperative hypocalcemia. However, the cervical lymph node dissection (LND) (p=0.006) and cervical dissection site (p=0.031) were significant in terms of postoperative complication development. In the multivariate analyses, it was found that female gender and LND were independent risk factors in the development of postoperative complications.Conclusion: It was determined that cervical LND and female gender were independent risk factors. Consistent with the literature findings, no significant results were found in the risk factors like heavy thyroid gland, presence of thyroid operation history, malignant thyroid pathology, retrosternal localization of the thyroid tissue, and hyperthyroidism.Keywords: Complications; hypocalcemia; thyroid weight; total thyroidectomy.

Aim: In this study, we aimed to investigate the factors related to postoperative mortality after pancreatoduodenectomy in our clinic. Materials and Methods: Patients who underwent pancreatoduodenectomy due to a periampullary region tumor between 2010 and 2019 were included in the study. Mortality that occurred within 30 days after PD was defined as postoperative mortality and groups were formed according to this definition Group 1(Postoperative Mortality) and Group 2(No mortality). The demographic and clinical features, laboratory parameters, and tumor features of the patients were compared between the groups. Risk factors for mortality were analyzed by univariate analysis and multivariate logistic regression analysis. Results: 155 patients participated in our study. We found our postoperative mortality rate as 11.6%. Accordingly, Group 1 consisted of 18 and Group 2 consisted of 137 patients. The sex was similar (p: 0.235). The average age was higher in Group 1 than 2 (71.2 vs 63.7, p:0.013). Tumor localizations were similar in groups (p:0.275). Lymph node positivity was similar in the groups (50% vs 41.6%, p:0.333). The pancreatic fistula was higher in Group 1, but not statistically significant (33% vs 18.2%, p:0.119). Preoperative white blood cell count (9490 mm3 vs 8050 mm3) and neutrophil count (6898 mm3 vs5442 mm3) were higher in Group 1. In multivariate analysis, no parameters were single-handedly risk factors. Conclusion: No factor was found to be effective alone in the development of mortality after pancreaticoduodenectomy. We think that postoperative mortality may decrease by revealing the factors in the preoperative, intraoperative and postoperative periods.

Aim: In this study, we aimed to determine the clinical value and prognostic significance of neutrophil / lymphocyte ratio in predicting pancreatic fistula in patients who underwent pancreatoduodenectomy due to periampullary tumor.Material and Methods: Patients who underwent pancreatoduodenectomy for periampullary tumor between 2010-2019 were included in the study. Group 1 (LowNLR) and Group 2 (HighNLR) were formed. Demographic and clinical characteristics, intraoperative and postoperative outcomes, and mean survival were compared between the groups. The significance of NLR in predicting pancreatic fistula at the cut off value was examined.Results: Patients were divided into two groups according to the 3.15 cut off value. Group 1 consisted of 61 patients and Group 2 consisted of 62 patients. In Group 2, albumin gr/dl value was lower (3.25 vs 3.70, p:000). in Group 2 pancreas tumors were more common (59.7% vs 42.6%, p: 0.041). The number of metastatic lymph nodes was higher in Group 1 (1.21 vs 0.63, p: 0.043), and the postoperative pancreatic fistula rate was similar (14.8% vs 21%, p: 0.254). Postoperative complications were higher in Group 2 (41.9% vs 14.8%, p: 0.032) NLR predicted pancreatic fistula with a sensitivity of 31.8% and specificity of 86.4%. In multivariate analysis, we found NLR to be a risk factor for survival (HR 1.760, 95% CI,1.179-2.627; p:0.006).Conclusion: Patients with a preoperative high NLR have a worse prognosis than patients with low NLR. It cannot be used alone to predict pancreatic fistula. NLR has the potential to be used in the management of patients.

OBJECTIVES: In this experimental study, it was aimed to assess the effects of erythropoietin (EPO) on bacterial translocation in a rat model of colitis. MATERIAL AND METHODS: The rats were randomly assigned into control, colitis and EPO-treated groups (n= 8 in each group). Saline solution (NS) was administered to control rats via rectal route. A trinitrobenzene sulfonic acid and ethanol mixture (TNBS-E) was used to induce colitis in the experiment groups. No treatment was administered to colitis group after induction. Starting at one day after induction of colitis with TNBS-E, EPO (1000 IU/kg) was administered subcutaneously for three days to the rats in the EPO-treated group. Colonic inflammation was assessed by gross and microscopic examination on day five. Blood samples were obtained to evaluate bacterial translocation while hepatic, mesenteric tissue samples and mesenteric lymph node (MLN) samples were collected for tissue culture. Aim: To compare therapeutic effectiveness of surgery alone with surgery plus corticosteroid in patients with granulomatous mastitis.Material and Methods: The study included 30 patients aged 23-60 years. The patients were randomly assigned into two groups by lot. In-group 1, the patients received 375 mg ampicillin-sulbactam (twice daily, p.o.) for a week; than 32 mg prednisolone was started which was gradually tapered (biweekly by 8 mg) over 8 weeks. Surgery was performed 2 weeks after tapering. In-group 2, the patients received 375 mg ampicillin-sulbactam (twice daily, p.o.) for a week and underwent surgery. During 2-years follow-up, all patients were assessed for etiological factors and recurrence.Results: No significant differences was found in demographic characteristics, smoking habits, oral contraceptive use, presenting complaints, physical examination findings, localization and diagnostic tools used between groups. During follow-up, recurrence was seen in one patient (6.7%) in surgery group and in 3 patients (20%) in steroid group but no significant difference was detected between groups (p>0.05). Conclusion: In patients with granulomatous mastitis, systemic steroid therapy is an effective modality in patients unresponsive to antibiotic therapy after ruling out tuberculosis.

Aim: Hydatid cyst (HC), which is localized in the liver and lungs in most cases, has been associated with a variety of hematologic and biochemical manifestations. Celiac disease (CD) is a small-intestinal malabsorption syndrome caused by hypersensitivity to gluten in subjects who carry the HLA haplotypes HLA DQ2 and DQ8. This study has attempted to show the connection between CD and HC. Materials-Methods: We prospectively analyzed data from 211 HC patients, 62 of whom had extrahepatic involvement of HC; in addition, we also classified the patients’ hydatid cysts by their radiologic features. All patients tested positive for HC by ELISA. Sera from the study population were also analyzed for IgA and IgG with ELISA using human recombinant tTG (AESKU. Diagnostics, Germany); the data were then analyzed statistically. Results: Twelve cases of seropositivity of TTG IgA were found among patients with HC. In the control group, the rate of TTG IgG seropositivity was only 2 out of 211 patients (~2%), which was lower than those with HC. In patients with HC, the mean WBC level was higher in patients with TTG IgA seropositivity compared with those without TTG IgA seropositivity. Younger ages were independently associated with TTG IgA seropositivity in the HC group. Conclusion: This study furthers the understanding of CD risk in HC. If confirmed by future studies, the study’s data will assist in developing optimal strategies for the detection of CD in patients with HC. Understanding the infectious factors involved in CD is important for identifying new approaches to the early detection of CD.
