Abstract

Aim: The aim of this study was to determine distribution of serum cholinesterase activity according to age and sex and serum cholinesterase activity of patients who were operated in Malatya Turgut Ozal Medıcal Center. Material and Method: In this study, serum cholinesterase activity of 96 (44.2%) male and 121 (55.8%) female patients who were operated in Turgut Özal Medical Center was determined by enzyme-spectrophotometric method. Results: Enzyme activities were 7092±226 U/ml and 6804±207 U/ml in males and females, respectively (p=0.58). The enzyme activity in the 0-15 years age group (8680±525 U/ml) was significantly higher that in the over 41 years age group (6567±330 U/ml) of males (p=0.01). However, there were no significant differences among age groups in females (p=0.06). The enzyme activity in the 16-40 years age group of females (6335±308 U/ml) was significantly lower that in the 16- 40 years age group (7316±334 U/ml) of males (p=0.04). Enzyme activity was found to be under the normal range (, Amaç: Malatya Turgut Özal Tıp Merkezi'ne ameliyat olmak için başvuran hastalarda, serum kolinesteraz düzeylerini ve enzim düzeylerinin yaş ve cinsiyete göre dağılımını belirlemektir. Materyal ve Metod: Bu çalışmada, Turgut Özal Tıp Merkezi'nde ameliyat olan, 96 (%44.2) erkek ve 121 (%55.8) kadın hastanın serum kolinesteraz aktiviteleri enzim-spektrofotometrik yöntemle tayin edildi. Bulgular: Enzim düzeyi, erkeklerde 7092±226 U/ml, kadınlarda 6804±207 U/ml olarak bulundu (p=0.58). 0-15 yaş grubu erkek hastalardaki enzim aktivitesi (8680±525 U/ml), 40 ve üzeri yaş grubuna (6567±330 U/ml) göre anlamlı düzeyde yüksekti (p=0.01). 41. Pentoksifillinin spinal kord iskemisindeki etkisinin araştırılması (Deneysel bir çalışma)

Abstract

ÖZET Anestezi ve cerrahi teknikteki ilerlemeye rağmen torakal ve torakoabdominal anevrizma ameliyatlarından sonra postoperatif nörolojik defisit halen önemli bir komplikasyon olarak görülmektedir. Erken ve özellikle geç tip paraplejinin patofizyolojisi tam olarak anlaşılamamıştır. Bu çalışmada, invitro nötrofil aktivasyonunu inhibe eden ve hayvanlarda serebral iskemi modelinden sonra serebral fonksiyonlarda düzelme sağladığı gösterilmiş olan pentoksifilinin spinal kord korunmasındaki etkisi araştırıldı. Spinal kord iskemi modeli için 24 beyaz Yeni Zelanda tavşanı kullanıldı. İnfrarenal aortaya oklüzyon sağlayacak düzenek yerleştirildi. 48 saat sonra tavşanlar, çalışma için rastgele seçildi. PTL grubuna (n=12) İV bolus 40 mg/kg ve idamede 0,2 mg/kg/dk PTL verildi. Kontrol grubuna serum fizyolojik verildi. Her iki gruptaki tavşanlara anestezik ajan verilmeden 0-24 dk geçici spinal kord iskemisi oluşturuldu. Operasyondan sonra spinal kord fonksiyonları 6, 12, 24, 48 ve 72 saat sonra değerlendirildi ( skor 5 = normal zıplıyor, skor 0 = hiç haraket yok). Histolojik inceleme, akut parapejiden hemen, geç tip paraplejiden sonraki 24 saat içinde spinal kord çıkarılarak yapıldı. Aort oklüzyonu sırasında distal aort basınçları her iki gruptada aynıydı (PTL grubu: 14,92 ±3,78 mmHg; kontrol grubu: 17,42 ±3,2). 72. saatte PTL ve kontrol grupları arasındaki skorlamada bir fark bulunmadı (PTL grubu 1,58±2,11; kontrol grubu 0,83±1,95) (p=0.817). Her iki gruptada 3'er tavşanda akut parapleji gelişti. Geç tip parapleji PTL grubunda 6 (%50), kontrol grubunda 7 (%58) tavşanda gözlendi. Spinal kordun morfolojik incelemesinde her iki grupta iskemik değişiklikler gözlendi. Akut parapleji gelişen kontrol grubunda nötrofil lökositler, geç tip paraplejigelişen kontrol grubunda ise makrofaj infiltrasyonu tespit edilirken PTL grubunda lökosit ve makrofaj sekestrasyonu tespit edilmedi. Spinal kord iskerni ve reperfüzyonundan sonra oluşan nörolojik komplikasyonlardan kan orjinli fagositik hücreler direkt olarak sorumlu değildir ve bu hücrelerin fonksiyonlarının inhibe edilmesi nörolojik sonucu değiştirmez. 31 43
