1. Evidence-based Hand Hygiene. Can You Trust the Fluorescent-based Assessment Methods?

Abstract

Healthcare-Associated Infections present a major threat to patient safety globally. According to studies, more than 50% of HAI could be prevented by proper hand hygiene. Effectiveness of hand hygiene is regularly evaluated with the fluorescent method: performing hand hygiene with a handrub containing an ultra violet (UV) fluorescent marker. Typically, human experts evaluate the hands under UV-A light, and decide whether the applied handrub covered the whole hand surface. The aim of this study was to investigate how different experts judge the same UV-pattern, and compare that to microbiology for objective validation. Hands of volunteer participants were contaminated with high concentration of a Staphylococcus epidermidis suspension. Hands were incompletely disinfected with UV-labeled handrub. Four different UV-box type devices were used to take CCD pictures of the hands under UV light. Size of inadequately disinfected areas on the hands were determined in two different ways. First, based on microbiology; the areas where colonies were grown were measured. Second, four independent senior infection control specialists were asked to mark the missed areas on printed image, captured under UV light. 8 hands of healthy volunteers were examined. Expert evaluations were highly uncorrelated (regarding interrater reliability) and inconsistent. Microbiology results weakly correlated with the expert evaluations. In half of the cases, there were more than 10% difference in the size of properly disinfected area, as measured by microbiology versus human experts. Considering the result of the expert evaluations, variability was disconcertingly high. Evaluating the fluorescent method is challenging, even for highly experienced professionals. A patient safety quality assurance system cannot be built on these data quality.

14. Microanatomical Properties of Energy Willow (Salix spp.) Leaves after Exposure to Potentially Toxic Elements from Wastewater Solids and Wood Ash.

Abstract

This open-field small-plot long-term experiment was set up between 2011 and 2021 with willow (Salix triandra × S. viminalis 'Inger'), grown as a short rotation coppice energy crop in Nyíregyháza, Hungary. The sandy loam Cambisol was treated with wastewater solids (WS) in the form of municipal sewage sludge compost (MSSC, 2011, 2013, and 2016), municipal sewage sediment (MSS, 2018), and with willow ash (WA, 2011, 2013, 2016, and 2018). Control plots remained untreated since 2011. All soil treatments significantly enhanced the uptake or accumulation of potentially toxic elements (PTEs) in the leaves of willows. During June 2019, 53 weeks after the last soil treatments, MSSC + MSS-, WA-, and MSSC + MSS + WA-treated willows leaves had 14–68% more As, 17–48% more Ba, 31–104% more Cr, 4–12% more Cu, 6–15% more Mn, 18–218% more Pb, and 11–35% more Zn compared to the untreated control. Significantly higher Mn and Zn concentrations were measured in the MSSC + MSS + WA treatments than in the MSSC + MSS treatments. The assumption that WA reduces the accumulation of PTEs in willow leaves when applied together with MSSC and MSS was therefore only partially confirmed. The hypothesis of this study was that PTEs accumulated in the leaves would affect the microanatomical parameters of the leaves. Numerous positive changes were observed with the combined application of WS and WA. MSSC + MSS + WA treatment reduced the thickness of the mesophyll less than MSSC + MSS or WA treatments alone; the size of the cells building the palisade and spongy parenchyma and the extent of the main vein significantly increased. In the case of the combined treatment, the extent of the sclerenchymatous stock was smaller than in the control but larger than in WS- or WA-treated willow. The extent of the collenchymatous stock significantly increased compared to the control. Increases in the thickness of the adaxial epidermis and the number of stomata were statistically significant. However, the extent of the increases did not reach the extent of the increase experienced in the case of WS treatment, as the size of the stomata did not significantly decrease. Abstract

Soil acidity (pH <5.5) limits agricultural production due to aluminum (Al) toxicity. The primary target of Al toxicity is the plant root. However, symptoms can be observed on the shoots. This study aims to determine the potential use of chlorophyll fluorescence imaging, multispectral imaging, and 3D multispectral scanning technology to quantify the effects of Al toxicity on corn. Corn seedlings were grown for 13 days in nutrient solutions (pH 4.0) with four Al treatments: 50, 100, 200, and 400 μM and a control (0 μM AlCl3 L−1). During the experiment, four measurements were performed: four (MT1), six (MT2), 11 (MT3), and 13 (MT4) days after the application of Al treatments. The most sensitive traits affected by Al toxicity were the reduction of plant growth and increased reflectance in the visible wavelength (affected at MT1). The reflectance of red wavelengths increased more significantly compared to near‐infrared and green wavelengths, leading to a decrease in the normalized difference vegetation index and the Green Leaf Index. The most sensitive chlorophyll fluorescence traits, effective quantum yield of PSII, and photochemical quenching coefficient were affected after prolonged Al exposure (MT3). This study demonstrates the usability of selected phenotypic traits in remote sensing studies to map Al‐toxic soils and in high‐throughput phenotyping studies to screen Al‐tolerant genotypes. Abstract

Electroencephalography (EEG) functional connectivity (FC) estimates are confounded by the volume conduction problem. This effect can be greatly reduced by applying FC measures insensitive to instantaneous, zero‐lag dependencies (corrected measures). However, numerous studies showed that FC measures sensitive to volume conduction (uncorrected measures) exhibit higher reliability and higher subject‐level identifiability. We tested how source reconstruction contributed to the reliability difference of EEG FC measures on a large (n = 201) resting‐state data set testing eight FC measures (including corrected and uncorrected measures). We showed that the high reliability of uncorrected FC measures in resting state partly stems from source reconstruction: idiosyncratic noise patterns define a baseline resting‐state functional network that explains a significant portion of the reliability of uncorrected FC measures. This effect remained valid for template head model‐based, as well as individual head model‐based source reconstruction. Abstract

The use of sewage sludge on arable land has been widespread for many years. This by-product, treated as waste, can provide valuable nutrients to the soil, but the applied amount of sewage sludge to arable land is limited. The possibility of application of sewage sludge is essentially determined by the composition of the sludge. The goal of the experiment was to demonstrate that the physiological, morphological, and biochemical parameters of maize (Zea mays L. cv. Armagnac) linearly change with increasing concentrations of sewage sludge (25%, 50%, and 75% as m/m%). The experiment was set up in a glasshouse. The following parameters were investigated: plant height, relative chlorophyll content, photosynthetic pigments (chlorophyll-a, chlorophyll-b, carotenoids), and leaf proline and malondialdehyde (MDA) content, and PS II quantum efficiency in the 3-leaf stages of the plants. Sewage sludge applied in lower doses had a beneficial effect on the initial growth of maize. The relative chlorophyll content was significantly higher in all treatments compared to the control. There was no significant difference in the maximum quantum efficiency of PS II reaction centers among the treatments. In this experiment, different concentrations of sewage sludge treatments had different impacts on the MDA and proline content of maize leaves. Abstract

Our auditory sense allows us to constantly monitor our surroundings for potentially hazardous events. It may be for that reason that listeners are particularly sensitive to sounds indicating looming auditory objects. Researchers have elicited and studied the phenomenon of looming bias by creating moving stimuli through changes in either broadband or spectral intensity, thereby exposing this bias across various species and ages during attentive listening. For effective hazard protection, however, the bias should not require the listener's attention. In our present study, we directly compared the two cue types used to elicit the looming bias and looked into the role of attention: twenty-eight normal-hearing listeners were initially passively exposed to the stimuli, being directed to focus on a muted and dubbed movie. Subsequently, in a second condition, they were asked to discriminate sound motion by keypress. In both conditions, neural activity was recorded through high-density electroencephalography (EEG). Cortical source activity was localized based on individual brain anatomies and electrode positions. Behaviourally, we verify the presence of the looming bias and its manifestation in higher accuracies for spectral cues and faster responses for broadband cues. Neurally, we see the emergence of the bias as early as 80 ms after the change. The bias is evoked later but stronger for changes in broadband versus spectral intensity and, most importantly, present in both passive and active listening modes. At the level of the primary auditory cortex (PAC), the bias occurs most consistently on the right hemisphere.
