Spontaneous intracranial hypotension (SIH) is a syndrome that was unknown until the advent of magnetic resonance imaging (MRI). It is a cause of orthostatic headache, which remains underdiagnosed and, rarely, can result in several complications including dural venous sinus thrombosis, subdural hematoma and subarachnoid hemorrhage. Some of these complications are potentially life-threatening and should be recognized promptly, mainly by imaging studies. We reviewed the MRI of nine patients with SIH and describe the complications observed in three of these patients. Two of them had subdural hematoma and one had a dural venous sinus thrombosis detected by computed tomography and MRI. We concluded that MRI findings are of great importance in the diagnosis of SIH and its complications, which often influence the clinical-surgical treatment of the patient. RESUMO Hipotensão Intracraniana Espontânea (HIE) é uma síndrome desconhecida até o advento das imagens de Ressonância Magnética (RM). É uma causa de cefaleia ortostática que permanece subdiagnosticada e raramente resulta em complicações, como trombose de seios venosos durais, hematoma subdural e hemorragia subaracnoidea. Algumas dessas complicações são potencialmente ameaçadoras à vida e devem ser prontamente reconhecidas pelos estudos de imagem. Nós revisamos as RM de 9 pacientes com HIE e descrevemos as complicações observadas em 3 casos. Dois deles tiveram hematoma subdural e um teve trombose de seio venoso dural detectados por tomografia computadorizada e RM. Concluímos que achados de RM são de grande importância no diagnóstico de HIE e suas complicações, frequentemente influenciando o tratamento clínico-cirúrgico do paciente.

OBJECTIVE: To evaluate, in a population-based approach, the association of extreme sleep duration with sociodemographic factors, health, and well-being. METHODS: We analyzed the data from the 2014/2015 Health Survey in the city of Campinas, State of São Paulo, Brazil (ISACamp), performed with 1,969 individuals (≥ 20 years old). Associations between the independent variable and short (≤ 6 hours) and long (≥ 9 hours) sleep were determined using the Rao-Scott chi-square test. The analyses were adjusted with multinomial logistic regression models. RESULTS: Men, individuals aged 40 to 59, those with higher schooling, those who have one (OR = 1.47, 95%CI 1.02–2.12), two (OR = 1.73, 95%CI 1.07–2.80), or three or more (OR = 1.62, 95%CI 1.16–2.28) chronic diseases, and those with three or more health problems (OR = 1.96, 95%CI 1.22–3.17) were more likely to have a short sleep. The chance of long sleep was higher in widowers and lower in those who have more years of schooling, with higher income, worked, lived with more residents at home, and reported three or more diseases (OR = 0.68, 95%CI 0.48–0.97) and health problems. The chance of either short (OR = 2.41, 95%CI 1.51–3.87) or long sleep (OR = 2.07, 95%CI 1.23–3.48) was higher in unhappy individuals. CONCLUSIONS: These findings highlight the higher chance of short sleep duration among men, among persons in productive age, and among those with a higher level of schooling in a Brazilian city. The association of short sleep with comorbidities and the association of happiness with extremes of sleep duration were also important results to understand the relation of sleep duration with health and well-being.

Objective To evaluate the risk factors and comorbid conditions associated with the development of pneumonia in patients with acute stroke. To determine the independent predictors of pneumonia. Method Retrospective study from July to December 2011. We reviewed all medical charts with diagnosis of stroke. Results 159 patients (18-90 years) were admitted. Prevalence of pneumonia was 32%. Pneumonia was more frequent in patients with hemorrhagic stroke (OR: 4.36; 95%CI: 1.9-10.01, p < 0.001), higher National Institute of Health Stroke Scale (NIHSS) (p = 0.047) and, lower Glasgow Coma Score (GCS) (p < 0.0001). Patients with pneumonia had longer hospitalization (p < 0.0001). Multivariable logistic regression analysis identified NIHSS as an independent predictor of pneumonia (95%CI: 1.049-1.246, p = 0.002). Conclusion Pneumonia was associated with severity and type of stroke and length of hospital stay. The severity of the deficit as evaluated by the NIHSS was shown to be the only independent risk factor for pneumonia in acute stroke patients.

SummaryObjectiveTo compare the efficacy of medical and surgical treatment for refractory mesial temporal lobe epilepsy associated with hippocampal sclerosis (MTLE).MethodsA prospective controlled non-randomized study of 26 patients with MTLE who underwent surgical treatment and 75 patients with MTLE who underwent medical treatment between August 2002 and October 2004. All patients failed to achieve seizure control with at least two first line antiepileptic drugs (AED) for partial seizures before entering the study. OBJECTIVE: To investigate the value of leaving seizure-free patients on low-dose medication. METHOD: This was an exploratory prospective randomized study conducted at our University Hospital. We evaluated the frequency of seizure recurrence and its risk factors following complete or partial antiepileptic drug (AED) withdrawal in seizure free patients for at least two years with focal, secondarily generalized and undetermined generalized epilepsies. For this reason, patients were divided into two groups: Group 1 (complete AED withdrawal), and Group 2 (partial AED withdrawal). Partial AED withdrawal was established as a reduction of 50% of the initial dose. Medication was tapered off slowly on both groups. Follow-up period was 24 months. RESULTS: Ninety-four patients were followed up: 45 were assigned to complete (Group 1) AED withdrawal and 49 to partial (Group 2) AED withdrawal. Seizure recurrence frequency after two years follow-up were 34.04% in group 1 and 32.69% in Group 2. Survival analysis showed that the probability of remaining seizure free at 6, 12, 18 and 24 months after randomization did not differ between the two groups (p = 0.8). Group 1: 0.89, 0.80, 0.71 and 0.69; group 2: 0.86, 0.82, 0.75 and 0.71. The analysis of risk factors for seizure recurrence showed that more than 10 seizures prior to seizure control was a significant predictive factor for recurrence after AED withdrawal (hazard ratio = 2.73). CONCLUSION: Leaving seizure free patients on low AED dose did not reduce the risk for seizure recurrence. That is, once the decision of AED withdrawal has been established, it should be complete. OBJETIVO: Investigar o valor da manutenção de baixas doses de medicação em pacientes com epilepsia controlada. MÉTODO: Trata-se de um estudo prospectivo aleatorizado exploratório, realizado em nosso Hospital Universitário. Nós avaliamos a freqüência de recorrência de crises e os fatores de risco associados após a retirada parcial ou completa da droga antiepiléptica (DAE), em pacientes com crises controladas por pelo menos dois anos. Os pacientes foram divididos em dois grupos: Grupo 1 (retirada completa da DAE) e Grupo 2 (retirada parcial da DAE). Retirada parcial da DAE foi estabelecida como uma redução de 50% da dose inicial. A medicação foi retirada lentamente nos dois grupos. O período de seguimento foi de 24 meses. RESULTADOS: Noventa e quatro pacientes foram seguidos: 45 foram selecionados para a retirada completa (Grupo 1) e 49 para a retirada parcial (Grupo 2). As taxas de recorrência de crises após dois anos de seguimento foram 34,04% para o Grupo 1 e 32,69% para o Grupo 2. A análise de sobrevivência mostrou que a probabilidade de permanecer livre de crises aos 6, 12, 18 e 24 meses após a aleatorização não diferiu entre os dois grupos (p = 0,8). Grupo 1: 0,89, 0,80, 0,71 e 0,69; Grupo 2: 0,86, 0,82, 0,75 e 0,71. A análise dos fatores de risco para a recorrência de crises demonstrou que mais que 10 crises antes do controle foi significante para a recorrência após a retirada da DAE (razão de risco = 2,73). CONCLUSÕES: A manutenção de baixas doses de medicação em pacientes com epilepsia controlada não reduz o risco de recorrência de crises. Portanto, caso a decisão de retirar a DAE tenha sido estabelecida, a retirada deve ser completa.

OBJECTIVE: To evaluate the clinical features and seizure control of epilepsy related to neurocysticercosis. METHOD: 18 patients with partial epilepsy and neurocysticercosis were treated with albendazol or praziquantel and followed from 3 months to 12 years. We analyzed results from the CSF exam, interictal electroencephalogram (EEG), head computerized tomography and/or magnetic resonance imaging. RESULTS: The patients' mean age was 36.4 years. The mean duration of epilepsy was 16 years. 83% patients had simple partial seizures ; 17% had complex partial seizures . All patients underwent routine EEGs: 62% had abnormalities and 38% were normal. A relationship was observed between focal EEG abnormality and the location of cyst in 28% of the patients. The CSF exams showed pleocytosis in 33% of the patients, and 28% had elevated protein levels. Only 22% of patients had positive titer for cysticercosis in the CSF. In all patients who had somatosensory and special sensory seizures there was a relationship between location of the cysts and seizure semiology (n=11). After cysticidal therapy, 83% patients had a significant improvement in controlling seizures. CONCLUSION: In this group, we found a predominance of simple partial seizures and a relationship between somatosensory and special sensory seizures and the location of the cysts. Cysticidal therapy was effective in controlling seizures in these patients and should be considered for patients with partial seizures and semiology related to cyst location. OBJETIVO: Avaliar as características e o controle das crises em pacientes com epilepsia associada à neurocisticercose. MÉTODO: Foram avaliados 18 pacientes tratados com albendazol ou praziquantel, com seguimento após tratamento variando de 3 meses a 12 anos. Todos pacientes tinham LCR, EEG interictal, tomografia computadorizada e/ou ressonância magnética de crânio. RESULTADOS: A idade média foi de 36,4 anos. O tempo de epilepsia foi em média 16,2 anos; 83% dos pacientes apresentavam crises parciais simples e 17%; crises parciais complexas. O EEG foi normal em 38%. Em apenas 28% dos pacientes houve relação entre alterações focais do EEG e a localização do cisto. Em relação ao LCR; 33% apresentavam pleiocitose, 28% hiperproteinorraquia e apenas 22% apresentavam imunologia positiva para cisticercose . Foi observada relação entre localização do cisto e semiologia das crises em todos os pacientes com crises de início sensitivo-motor e sensorial especial (n= 11). Após tratamento cisticida, 83% tiveram redução de pelo menos 50% das crises . CONCLUSÃO: Na epilepsia relacionada a neurocisticercose predominam crises parciais simples e observa-se relação entre crises sensitivo-motoras ou sensoriais especiais e localização do cisto. O tratamento cisticida deve ser considerado nos pacientes com crises parciais com semiologia relacionada à localização do cisto.

This manuscript contains the conclusion of the consensus meeting on the diagnosis of narcolepsy based on the review of Medline publications between 1980-2010. Narcolepsy is a chronic disorder with age at onset between the first and second decade of life. Essential narcolepsy symptoms are cataplexy and excessive sleepiness. Cataplexy is defined as sudden, recurrent and reversible attacks of muscle weakness triggered by emotions. Accessory narcolepsy symptoms are hypnagogic hallucinations, sleep paralysis and nocturnal fragmented sleep. The clinical diagnosis according to the International Classification of Sleep Disorders is the presence of excessive sleepiness and cataplexy. A full in-lab polysomnography followed by a multiple sleep latency test is recommended for the confirmation of the diagnosis and co-morbidities. The presence of two sleep-onset REM period naps in the multiple sleep latency test is diagnostic for cataplexy-free narcolepsy. A positive HLA-DQB1*0602 with lower than 110pg/mL level of hypocretin-1 in the cerebrospinal fluid is required for the final diagnosis of cataplexy- and sleep-onset REM period -free narcolepsy.

Este artigo relata as conclusões da reunião de consenso com médicos especialistas sobre diagnóstico de narcolepsia baseada na revisão dos artigos sobre narcolepsia listados no Medline entre 1980 e 2010. A narcolepsia é uma doença crônica de início entre a primeira e segunda décadas de vida do indivíduo. Os sintomas essenciais são cataplexia e sonolência excessiva. A cataplexia é definida como episódios súbitos, recorrentes e reversíveis de fraqueza da musculatura esquelética desencadeados por situações de conteúdo emocional. Os sintomas acessórios são alucinações hipnagógicas, paralisia do sono e sono fragmentado. Critérios de diagnóstico clínico de acordo com a Classificação Internacional dos Transtornos do Sono são de sonolência excessiva e cataplexia. Recomenda-se a realização de polissonografia seguida do teste de latência múltipla do sono em um laboratório de sono para confirmação e diagnóstico de comorbidades. Quando não houver cataplexia, deve haver duas ou mais sonecas com sono REM no teste de latência múltipla do sono. Tipagem HLA-DQB1*0602 positiva com níveis de hipocretina-1 abaixo de 110pg/mL devem estar presentes para o diagnóstico de narcolepsia sem cataplexia e sem sonecas com sono REM. A epilepsia generalizada idiopática (EGI) frequentemente não é diagnosticada corretamente em adultos, com sérias consequências para os pacientes. O objetivo deste estudo foi avaliar os fatores mais frequentemente associados a dificuldades no diagnóstico diferencial entre epilepsias parciais e generalizadas em adultos. Avaliamos 41 pacientes com diagnostico de crises parciais complexas com elementos de anamnese e EEG indicando um possível diagnóstico diferencial. Foi possível a mudança do diagnóstico de epilepsia parcial para EGI em 25 pacientes: 22 (88%) com EMJ; um com ausência juvenil, um com síndrome de ausências com mioclonias periorais e um com ausência com mioclonias palpebrais. Mioclonias, uma das características da EMJ e outras formas de EGI, geralmente não eram espontaneamente relatadas pelos pacientes. Abalos mioclônicos unilaterais eram confundidos com crises parciais motoras. Ausências breves e pouco frequentes e anormalidades focais no EEG contribuíram para o não reconhecimento de EGI. Todos os 25 pacientes apresentavam crises sem controle adequado antes da revisão diagnóstica. Após o diagnóstico correto e mudança para monoterapia com acido valpróico ou valproato de sódio, 19 (76%) ficaram livre de crises e seis (24%) dos 25 pacientes apresentaram melhora significativa. A associação de lamotrigina em três destes pacientes propiciou redução significativa da frequência de crises. Em conclusão, anamnese detalhada e questionamento direcionado para determinar a presença de mioclonias e crises tipo ausência e a sua interpretação no contexto clínico são fundamentais para o diagnóstico correto das EGI em adultos.
