Objective: The epidemiological characteristics, risk factors, complications, recurrence status, clinical and laboratory features, and treatment methods of the patients who admitted to our Pediatric Cardiology Outpatient Clinic with a pre-diagnosis of acute rheumatic fever (ARF) were evaluated. Materials and Methods: The data of 166 patients who admitted with a pre-diagnosis of ARF and were diagnosed with ARF, and the data of 51 patients who were not diagnosed with ARF, were retrospectively analyzed. Results: The patients with ARF were between the ages of 5 and 18. Most of the patients with ARF attack admitted in December (15.6%), January (13.8%), and February (13.2%). The most common complaints of the patients diagnosed with ARF were isolated joint pain and/or swelling, at 50.6%. While 91.5% of the patients were diagnosed for the first time, 8.5% had ARF recurrence. It was seen that the most common major criterion was carditis (94.6%). The severity of valve regurgitation and the rates of monoarthritis were significantly higher in patients with recurrence (P < .05). Non-compliance with prophylaxis was observed in 10 (71.4%) of 14 patients with recurrence, and in 43 (28.2%) of 152 patients without recurrence. Anti-streptolysin O was lower (P = .021) and alanine transaminase (ALT) was higher (P = .019) in the recurrence group. Conclusion: Our study showed that in patients with a pre-diagnosis of ARF, a differential diagnosis should be made with other diseases. Especially in patients with joint complaints as the only major symptom, a differential diagnosis should be made. ARF recurrence is associated with non-compliance with prophylaxis, and both the severity of valve regurgitation and monoarthritis rates are higher in patients who develop recurrence. Alanine aminotransferase is significantly higher in patients with ARF recurrence.

Objective: The World Health Organization labelled the COVID-19 outbreak a pandemic on March 11, 2020, the first day COVID-19 cases were diagnosed in Turkey. Since then, people's main concerns have been whether their lives or the lives of their relatives would be at risk if they became infected. The objective of this study was to determine the risk factors that increase mortality in COVID-19 patients treated in ICUs and to facilitate the appropriate precautions. Methods: In this study, patients who were diagnosed with COVID-19 [positive on a polymerase chain reaction (PCR) test] and monitored in the Pandemic Hospital at Diyarbakır Dicle University between 15 March 15 and 15 May, 2020, were evaluated for risk factors for mortality. Results: A total of 50 ICU patients were included in the study. Of them, 52% were males, and the mean age was 63.8±17.6 years. A comparison of deceased and surviving patients showed that being female, older than 62, and a smoker and having diabetes mellitus, hypertension, and/or coronary artery disease significantly increased mortality. An investigation of the findings from the patients' lung tomography examinations revealed that relatively prominent pulmonary involvement and localization did not affect mortality. Conclusions: Because patients who are advanced in age, female and smokers and have comorbidities are in the high-risk group, measurements related to treating the COVID-19 disease should be improved. Laboratory parameters are useful for the evaluation of mortality and morbidity, while mechanical ventilation increased mortality. Giriş: Dünya Sağlık Örgütü tarafından COVID-19 hastalığı pandemi olarak ilan edildiği 11 Mart 2020 tarihinde Türkiye'de de ilk vakalar görülmeye başlandı. O tarihten beri insanlarda paniğe neden olan düşüncelerin başında hastalığa yakalanmasıyla beraber kendi ve yakınlarının hayatının tehlikeye girip girmeyeceği oldu. Bu çalışmanın amacı COVID-19 hastalığına bağlı yoğun bakımda takip edilen hastalarda mortaliteyi arttıran risk faktörlerin tespit edilip bu yönde önlem alınmasını sağlamaktır Yöntemler: Bu çalışmada, Diyarbakır Dicle Üniversitesi Pandemi Hastanesine 15 Mart 2020 ve 15 Mayıs 2020 tarihleri arasında COVID-19 Yoğun bakımlarında yatan ve polymerase chain reaction (PCR) ile COVID-19 tanısı konan yoğun bakım olguları değerlendirildi. Sonuçlar: Çalışmaya alınan 50 yoğun bakım hastasının %54'ü erkekti. Hastaların yaş ortalaması 63.8 ± 17.6 yıl olarak hesaplandı. Ölen hastalarda sağ kalanlara göre kadın cinsiyet, 62 yaş üzeri hastalar, sigara kullanımı, diyabetes mellitus, hipertansiyon ve koroner arter hastalığının varlığının mortaliteyi istatistiksel olarak arttırdığı bulundu. Tomografi bulguları karşılaştırıldığında, akçiğer tutulumun fazla olması ile mortalite arasında doğru orantı olmadığı bulundu. Tartışma: İleri yaş, kadın cinsiyet, sigara kullanımı ve komorbid hastalıkları olanlar yüksek risk grubunda olduğundan dolayı COVID-19 hastalığından korunmak için önlemleri arttırmak gerekiyor. Akciğer tomografi bulguları tanısal olarak anlamlı olsa da hastalığın sürveyansında anlamlı olmayabilir. Laboratuvar bulguları mortalite ve morbidite üzerinde etkili parametreler olup, olup mekanik ventilatöre bağlananlarda mortalite oranı yüksek olarak rapor edildi.

Amaç: Hipoksikiskemikensefalopati (HİE), perinatal dönemde hipoksiye bağlı olarak görülen ağır morbidite ve mortalite ile sonuçlanabilen klinik bir durumdur. Bu çalışmanın amacı, hipoksik iskemik ensefalopatide kardiyak biyomarkerlerden N-Terminal pro Brain Natriüretik Peptid (NTproBNP) ve Troponin I’nın rolünü değerlendirmektir. Yöntemler: Bu çalışmada, Ocak 2019 ve Mayıs 2021 tarihleri arasında Yenidoğan Yoğun Bakım Ünitesinde HİE tanısı ile takip ve tedavileri yapılmış olan hastaların Troponin I ve NTproBNP düzeyleri geriye dönük olarak incelendi. Bulgular: Bu çalışma %44,5 (45)’i kız ve %55.5(56)’sı erkek olmak üzere 101 olgudan oluştu. Normal spontan vajinal yol ile doğum oranı %57,8 (63) iken Sezaryenle doğum oranı %42,2 (46) idi. Olgularda HİE evre 1 oranı %31.7 (32), HİE evre2 oranı %31,7 (32) ve HİE evre 3 oranı%36,6 (37) olarak bulundu. Ortalama doğum ağırlığı 3139.95±499.02 g, ortalama gestasyon haftası 38.39±1.69 hafta bulundu. NTproBNP; ortalama 10099.49±12023.03 pg/mL ve Troponin I; 122.575±188.5287 ng/L bulundu. Hipoksik iskemik ensefalopatinin şiddetinin artması ile Troponin I arasında istatistiksel olarak anlamlı bir fark bulunurken, NTproBNP ile HİE’nin şiddeti arasında herhangi bir farklılık bulunmadı. Sonuç: Yenidoğan HİE’li bebeklerde NTproBNP ve Troponin I düzeylerinde artış vardır. Objective: Hypoxic ischemic encephalopathy (HIE) is a clinical condition that can result in severe morbidity and mortality due to hypoxia in the perinatal period. The aim of this study is to evaluate the role of cardiac biomarkers NTerminal pro Brain Natriuretic Peptide (NTproBNP) and Troponin I in hypoxic ischemic encephalopathy. Methods: In this study, Troponin I and NTproBNP levels of patients who were followed up and treated with the diagnosis of HIE in the Neonatal Intensive Care Unit between January 2019 and May 2021 were evaluated retrospectively. Results: This study consisted of 101 subjects, 44.5% (45) female and 55.5% (56%) male. While the rate of normal spontaneous vaginal delivery was 57.8% (63), the rate of cesarean delivery was 42.2% (46). In the cases, the rate of HIE stage 1 was 31.7% (32), the rate of HIE stage 2 was 31.7% (32) and the rate of HIE stage 3 was 36.6% (37). Mean birth weight was 3139.95±499.02 g, mean gestational week was 38.39±1.69 weeks. NTproBNP; mean 10099.49±12023.03 pg/mL and Troponin I; 122.575±188.5287 ng/L was found. While there was a statistically significant difference between the increase in the severity of hypoxic ischemic encephalopathy and Troponin I, there was no difference between the severity of NTproBNP and HIE. Conclusion: There is an increase in NTproBNP and Troponin I levels in infants with neonatal HIE. In addition, a significant correlation was found between Troponin I levels and the severity of neonatal hypoxic ischemic encephalopathy.

Objective: The aim of this study was to measure serum lipid levels and epicardial adipose tissue thickness in patients determined with subclinical hypothyroidism. Methods: The study included 61 paediatric patients with a diagnosis of subclinical hypothyroidism and a control group of 61 healthy children. The thyroid hormone levels, lipid parameters and epicardial adipose tissue thickness were examined in all the patients. Results: The mean epicardial adipose tissue thickness of the subclinical hypothyroidism patients was higher than that of the control group but not at a level of statistical significance (4.15±0.91 vs 4.06±0.99, p=0.598). The mean high-density lipoprotein cholesterol level of the subclinical hypothyroidism group was statistically lower than that of the control group (p=0.040). Conclusion: The results of this study showed a significant decrease in the high-density lipoprotein cholesterol levels of children with subclinical hypothyroidism. No significant increase was seen in the epicardial adipose tissue thickness of the children with subclinical hypothyroidism. This is the first study to have examined epicardial adipose tissue thickness in children with subclinical hypothyroidism.

Amaç: Çocuk kardiyoloji polikliniğimize kardiyak üfürüm nedeni ile gönderilen hastaların kardiyak üfürüm ve ekokardiyografi bulgularının değerlendirilmesi amaçlanmıştır. Materyal ve Metod: Hastaların üfürümleri masum ve patolojik olmak üzere ikiye ayrıldı. Hastalara elektrokardiyografik ve ekokardiyografik değerlendirme yapıldı. Bulgular: Çalışmamıza üfürüm ile başvuran 219 hasta alındı. Hastaların yaş ortalaması 21,61 ± 34,3 ay. Masum üfürümü olan 138 hastada şu an için tedavi gerektirmeyen patent foramen ovale (40 hasta), küçük sekundum atriyal septal defekt (1 hasta) saptandı. Giriş: Çarpıntı; ritm bozukluğu saptanan hastaların en sık ve en önemli semptomudur. Ritm bozuklukları sadece elektrokardiyografi ile saptanamayabilir Amaç: Çalışmamızın amacı çarpıntı şikayeti ile başvuran çocukların elektrokardiyografi ve 24 saatlik Holter elektrokardiyografi monitörizasyonu sonuçlarını değerlendirip ritim bozukluklarını ve sıklığını saptamak, Holterin tanı ve tedavideki yerini vurgulamaktır. Gereç ve Yöntem: Hastanemize çarpıntı şikayeti ile başvuran 320 çocuğun yüzeyel elektrokardiyografi, 24 saatlik Holter monitörizasyonu ve ekokardiyografi verileri değerlendirildi. Bulgular: Holter monitörizasyonu ile değerlendirilen hastaların 163’ü kız (%50,9) iken, 157’si erkek (%49,1) idi. Hastaların 246’sında (%76,9) izole çarpıntı, 55’inde (%17,2) çarpıntı ve göğüs ağrısı, 18’inde (%5,6) çarpıntı ve senkop, birinde ise (%0,3) çarpıntı ve nefes darlığı şikayeti vardı. Elli hastada (%15,6) elektrokardiyografide, 182 hastada (%56,8) Holterde aritmi saptandı. Holterde en sık saptanan aritmiler; 63 hastada supraventriküler ekstrasistol (% 19,7) ve 58 hastada ventriküler ekstrasistol (%18,2) idi. Hastaların 63’ünde ciddi aritmi saptandı. İzole çarpıntı ile gelen hastalardaki ciddi aritmi oranı diğer gruplara göre istatistiksel olarak anlamlı şekilde yüksek saptandı . Sonuç: Elektrokardiyografisi normal olan çocuk hastaların Holterlerinde aritmi saptanması sadece elektrokardiyografi ile hastaların aritmilerinin atlanabileceğini göstermektedir. İzole çarpıntı ile başvuran hastalarda çarpıntı ile beraber başka şikayetleri olan hastalara göre ciddi aritmilerin daha fazla olması izole çarpıntı ile gelen hastalara aritmi açısından dahadikkatli olmamız gerektiğini gösteriyor olabilir. Ancak bunu desteklemek için daha fazla çalışmaya ihtiyaç olduğunu düşünüyoruz. Objective:Palpitations are the most common and most important symptoms of patients with rhythm disturbances. Rhythm disturbances may not be detected by electrocardiography.The aim of our study is to evaluate the results of electrocardiography and 24-hour holter electrocardiography monitoring in children presenting with the complaint of palpitations, to determine the rhythm disturbances and their frequency, and to emphasize the role of holter in diagnosis and treatment. Material and Methods: Electrocardiography, 24-hour holter monitoring and echocardiography data of 320 children admitted to our hospital with the complaint of palpitations were evaluated. Results: One hundred-sixty three of the patients were female (50.9%). Isolated palpitations in 246 (76.9%) patients, palpitations and chest pain in 55 (17.2%), palpitations and syncope in 18 (5.6%), one patient had (0.3%) palpitations and shortness of breath. The most common arrhythmias were supraventricular extrasystole in 63 patients (19.7%) and ventricular extrasystole in 58 patients (18.2%) in holter. Severe arrhythmia was detected in 63 of the patients. The rate of severe arrhythmia in patients presenting with isolated palpitations was found to be statistically significantly higher than the other groups. Conclusion: Detection of arrhythmias in the holters of pediatric patients with normal electrocardiography shows that the arrhythmias of the patients can be overlooked only with electrocardiography. The higher rate of serious arrhythmias in patients presenting with isolated palpitations compared to patients with other complaints may indicate that we should be more careful in terms of arrhythmia in patients presenting with isolated palpitations.

Bizim olgumuz rutin prenatal ultrasonografi ile tespit edilmiş artmış ense kalınlığı, hipoplastik sol kalp sendromu ve serebral anomaliler gibi pek çok anomalilere sahiptir. Olgumuzun karyotipi de novo kromozomal yeniden düzenlenmeye bağlı 46,XX,der(4)(l7qter-->l7q21, Partial trisomy of distal 17q is rare. Until now, 32 cases of partial trisomy for the dis— tal region of 17q were reported. Our case has multiple anomalies, such as increased nuchal translucency, hypoplastic left heart syndrome and cerebral anomalies by routine prenatal ultrasonografic screen. Between January 1991 and December 1993, 50 hip hemiarthroplasties were done using moduler head and straight stem endoprosthesis for femoral neck fractures in SSK Göztepe Hospital; Of these, 23 were male and 26 were female. The average age of the patients were 64.5 (38-92). A Mean follow up was 18 (2-34) months. The patients were evaluated by Harrisi functional scoring system. 38 (%86) cases are found satisfactory. It's concluded that straight stem prostheses were succussfuly applied for femoral fractures in the elderly patients., Ocak 1991 ve Aralık 1993 tarihleri arasında SSK Göztepe Hastanesi II. Ort. ve Trav. Kliniğinde, femur boynu kırığı olan 23 erkek 26 kadın hastanın toplam 50 kalçasına modüler başlı düz saplı parsiyel endoprotez ameliyatı yapıldı. Hastaların ortalama yaşı 64.5 (38-92) idi. Ortalama 18 ay (2-34) takip edilen vakalar, postoperatif Harris'in fonksiyonel skorlama sistemine göre değerlendirildiğinde 38 (%86) inde tatminkar sonuç alındı. Çalışmamızın sonunda yaşlılarda femur boynu kırıklarının düz saplı protezlerle başarılı olarak tedavi edildiği sonucuna varıldı.

Bu araştırmada, özel eğitim alanında çalışan psikolojik danışmanlık ve rehberlik bölümü ile psikoloji bölümü mezunu bireysel eğitimcilerin tükenmişlik düzeyleri araştırılmıştır. Araştırmada demografik özelliklerin belirlenebilmesi için araştırmacı tarafından geliştirilmiş “Kişisel Bilgi Formu” ile tükenmişlik düzeyini ölçmeye yarayan “Maslach Tükenmişlik Envanteri” kullanılmıştır. Araştırmanın bağımsız değişkenleri psikolojik danışmanlık ve rehberlik ile psikoloji bölümü mezunu bireysel eğitimcilerin cinsiyet ve özel eğitim alanında çalışma yılları, bağımlı değişkenleri ise tükenmişlik düzeyleri şeklinde belirlenmiştir. Araştırmada elde edilen verilerin çözümünde iki ya da daha fazla değişken arasındaki ilişkinin anlamlılığını ölçmek için Pearson Koreleasyonu; bir değişkenin, iki ya da daha fazla grup üzerinde, gruplar arası anlamlı bir fark oluşturup oluşturmadığını ölçmek için Tek Yönlü Varyans Analizi (ANOVA); bir değişkenin, gruplar arasında oluşturduğu farkın anlamlılığını ölçmek için ise Bağımsız Değişkenli t Testi analizlerinden faydalanılmıştır. Cinsiyet değişkenine göre ise erkek ve bayan deneklerin duyarsızlaşma düzeyleri arasında anlamlı bir ilişkinin olduğu; duygusal tükenme ve kişisel başarı düzeyleri yönünden herhangi bir anlamlılığa rastlanmadığı bulgulanmıştır. Erkek deneklerin duyarsızlaşma düzeyleri daha yüksek çıkmıştır., This research aims to investigate the burnout levels of the individual educators who graduated from the department of psychological counseling and guidance and the department of psychology in special education. At least one year experienced 72 female and 59 male of 131 participants who work in special education centers connected to Ministry of National Education placed on different locations of İstanbul, had joined to this research. A ?Personal Information Form? that was prepared by the researcher to determine the demographic specialities and also ?Maslach Burnout Inventory? measuring the burnout level, were used in the research. The non-dependent variables of the research were gender and the total years of employment in special education of the individual educators who graduated from the department of psychological counseling and guidance and the department of psychology, the dependent variable was the burnout levels. Pearson Correlation Analysis was used for analyzing the relationship between variables and Non-dependent t Test and One-Way Analysis of Variation (ANOVA) were employed for analyzing the significant differences between the groups. As a result of the research, it has been found that there is a significant relationship between the total years of employment and levels of emotional exhaustion, depersonalization and personal accomplishment of the individual educators. Also male educators experienced a higher level of depersonalization than female educators did.

Mozaik trizomi 17 nadir görülen otozomal trizomilerden olup literatürde sadece 30 olguda bildirilmiştir. Olguların büyük çoğunluğu “chorionic villus sampling” (CVS) ve amniyosentez materyallerinden yapılan prenatal genetik analizlerde saptanmış olup klinik bulgular son derece değişkendir. Burada sunduğumuz olgunun prenatal dönemde ultrasonografik takibinde lateral ventriküllerde sınırda ventrikülomegali saptandı ve amniyosentez yapıldı. Sitogenetik analiz ile karyotip 47,XX,+17[5]/46,XX[35] olarak saptandı. Yapılan FISH analizi ile mozaik trizomi 17 karyotip bulgusu doğrulandı (nuc ish(RARA×3,PML×2)[30/100], (TP53×3)[35/100]). Termde spontan vajinal yolla doğan olgunun fizik muayenesinde ve transfontanel ultrasonografisinde herhangi bir dismorfik bulgu ya da konjenital anomali saptanmadı. Periferik kandan postnatal olarak yapılan kromozom analizinde karyotip 46,XX olarak saptandı. Periferik kandan ve yanak mukozasından lokus spesifik problar ile yapılan FISH analizinde de mozaisizm saptanmadı (nuc ish(RARA,PML)×2[200],(TP53×2)[200]). Prenatal tanıda nadir görülen mozaik trizomi 17 saptanan olgumuz, ileride karşılaşılabilecek benzer olgularda ailelere verilecek genetik danışmanlık açısından katkı sağlayacaktır., Mosaic trisomy 17 is rarely observed and about 30 cases have been reported so far. Most of the cases have been identified by genetic diag-nostic analysis of chorionic villus sampling (CVS) and amniotic fluid in the prenatal period. Clinical manifestations are extremely variable. In the case presented here, due to the size of lateral ventricles was seen at the upper limit of normal range in the prenatal ultrasonographic investigation, amniocynthesis was performed and the karyotype was found as 47,XX,+17[5]/46,XX[35]. This mosaicism was confirmed by FISH analysis also (nuc ish(RARA×3,PML×2)[30/100],(TP53×3)[35/100]). The baby was born by spontaneous vaginal delivery at term. No dysmorphic feature or congenital anomaly has been observed in the physical examination or in trans-fontanel ultrasonographic examination. Peripheral blood karyotype was found 46,XX. The mosaicism was not diagnosed in FISH analysis of peripheral blood and buccal mucosa samples using spesific probes, too (nuc ish(RARA,PML)×2[200],(TP53×2)[200]). This case report demonstrating uncommon mosaic tri-somy 17 would be useful to give genetic counseling to the families having similar cases in the future.

Amaç: Bu çalışma ile hışıltılı çocuklar ve kontrol grubunda serum D-Vitamini, Folikasit ve B12 düzeyi bakılarak iki grup arasında anlamlı fark olup olmadığının belirlenmesi amaçlandı.Yöntem: Çalışmaya en az üç kez hışıltı geçirme öyküsü ile başvuran 0-3 yaş arasındaki 107 çocuk ile genel çocuk polikliniğine başvuran sağlıklı 96 çocuk alındı. Bazı kanser türlerinde yatkınlık ile fibroblast büyüme faktörü reseptörü-4 (FGFR-4) genin polimorfizmleri arasında ilişkiyi gösteren çalışmalar vardır. Bu çalışmada, primer akciğer kanseri (PAK) gelişimin ile FGFR-4 (Gly388Arg) gen polimorfizmi arasındaki ilişkiyi araştırmayı amaçladık.Çalışmamıza PAK tanısı almış 124 hasta ve herhangi bir kanser hikayesi olmayan 100 kişi alındı.
