Abstract

Streptococcus pyogenes(group A Streptococcus, GAS) is a major human pathogen and causes every year over 600 millions upper respiratory tract onfections worldwide. Untreated or repeated infections may lead to post-infectional sequelae such as rheumatic heart disease, a major cause of GAS-mediated mortality. There is no comprehensive, longitudinal analysis of the M type distribution of upper respiratory tract strains isolated in Poland. Single reports describe rather their antibiotic resistance patterns or focus on the invasive isolates. Our goal was to analyse the clonal structure of the upper respiratory tract GAS isolated over multiple years in Poland. Our analysis revealed a clonal structure similar to the ones observed in high-income countries, with M1, M12, M89, M28, and M77 serotypes constituting over 80% of GAS strains. The M77 serotype is a major carrier of erythromycin resistance and is more often correlated with upper respiratory tract infections than other serotypes.

17. Health outcomes and unmet needs in patients with long-standing rheumatoid arthritis attending tertiary care in Greece: A cohort study

Abstract

Background: No previous studies have characterized a patient's experience of rheumatoid arthritis (RA) management in Greece and unmet needs may exist despite a broad range of available treatments. Therefore, we assessed quality of life (QoL), functional ability, and healthcare resource utilization in patients with established RA and receiving treatment in a tertiary care setting in Greece. Methods: This was a prospective, observational cohort of patients aged ≥18 years, receiving any type of treatment for RA, and followed for 12 months at 7 rheumatology referral centers across mainland Greece (NCT01001182). Patient data were collected at the initial visit and 3, 6, and 9 months. QoL was evaluated using the Euro Quality of Life-5 dimensions questionnaire (EQ-5D) and functional ability was evaluated using the Health Assessment Questionnaire (HAQ). Results: A total of 210 patients with RA were enrolled (76.7% women, mean ± standard deviation [SD] age: 59.1 ± 12.6 years, median [interquartile range] disease duration: 11.9 [5.0-16.0] years). Baseline mean ± SD EQ-5D and HAQ scores were 0.57 ± 0.32 and 0.75 ± 0.63, respectively, and remained largely unchanged throughout the study. Post-hoc comparison showed that patients receiving non-biologic disease-modifying antirheumatic drugs (non-bDMARDs) had significantly higher EQ-5D and lower HAQ-DI scores compared with those receiving biologic DMARDs. A majority of patients reported having difficulty doing housework or other duties (61.4 and 61.9%, respectively), and 55.2% reported needing external support for these tasks. Positive correlation was observed between QoL and functional ability. Hospitalization at least once during the study occurred in 9.5% of the patients, and 12.5% of these cases were due to exacerbation of RA. At baseline, 52.4% of the patients were retired, with 38.5% of retirees having retired early due to RA. Among the patients who were retired at baseline, the mean ± SD period from actual retirement to expected retirement age was 12.1 ± 8.1 years. Conclusion: QoL and functional ability were positively correlated in patients with long-standing RA, with a large proportion showing impairments in both. Timely, target-oriented treatment initiated as soon as possible after diagnosis may help to improve patient-reported outcomes and limit the burden of RA. Abstract

International audience; In POINT X, a study designed to reflect clinical practice and patient treatment choices, 254 European patients received open-label collagenase for Dupuytren's contracture. The most severely affected joint was treated first in 74% of patients. In total, 52%, 41%, 7%, and 1% of patients selected the little, ring, middle, and index finger, respectively; 79% had one or two joints treated. Only 9% of patients (n = 24) received 4 or 5 injections. The mean improvement in total passive extension deficit (TPED) was 34° on day 1, improving further by day 7 to 42°. This secondary improvement was maintained by day 90 and month 6. The mean number of injections/joint was 1.2 for the metacarpophalangeal joint and 1.25 for the proximal interphalangeal joint. Median time to recovery was 4 days; the mean improvement in hand function was clinically relevant as measured by the Unité Rhumatologique des Affections de la Main (URAM) score. In total, 87% and 86% of patients and physicians, respectively, were very satisfied or satisfied with treatment at month 6, although correlation between TPED and patient satisfaction was weak (Spearman -0.18, 95% CI -0.32 to -0.06). Collagenase was well tolerated, with 10 (3.9%) patients experiencing severe adverse events. As a real-world study, the POINT X findings can be generalized to the at-large population.

23. Eliciting health state utilities for Dupuytren's contracture using a discrete choice experiment

Abstract

Background and purpose An internet-based discrete choice\ud experiment (DCE) was conducted to elicit preferences for a wide\ud range of Dupuytren’s contracture (DC)-related health states. An\ud algorithm was subsequently developed to convert these preferences\ud into health state utilities that can be used to assess DC’s\ud impact on quality of life and the value of its treatments.\ud Methods Health state preferences for varying levels of DC\ud hand severity were elicited via an internet survey from a sample\ud of the UK adult population. Severity levels were deined using a\ud combination of contractures (0, 45, or 90 degrees) in 8 proximal\ud interphalangeal and metacarpophalangeal joints of the index,\ud middle, ring, and little ingers. Right-handed, left-handed, and\ud ambidextrous respondents indicated which hand was preferable\ud in each of the 10 randomly-selected hand-pairings comparing\ud different DC severity levels. For consistency across comparisons,\ud anatomically precise digital hand drawings were used. To anchor\ud preferences onto the traditional 0–1 utility scale used in health\ud economic evaluations, unaffected hands were assigned a utility\ud of 1.0 whereas the utility for a maximally affected hand (i.e., all\ud 8 joints set at 90 degrees of contracture) was derived by asking\ud respondents to indicate what combination of attributes and levels\ud of the EQ-5D-5L proile most accurately relects the impact of\ud living with such hand. Conditional logistic models were used to\ud estimate indirect utilities, then rescaled to the anchor points on\ud the EQ-5D-5L.\ud Results Estimated utilities based on the responses of 1,745\ud qualiied respondents were 0.49, 0.57, and 0.63 for completely\ud affected dominant hands, non-dominant hands, or ambidextrous\ud hands, respectively. Utility for a dominant hand with 90-degree\ud contracture in t h e metacarpophalangeal joints of the ring and\ud little ingers was estimated to be 0.89. Separately, reducing the\ud contracture of metacarpophalangeal joint for a little inger from\ud 50 to 12 degrees would improve utility by 0.02.\ud Interpretation DC is associated with substantial utility decrements.\ud The algorithms presented herein provide a robust and lexible\ud framework to assess utility for varying degrees of DC severity. \ud

Incidence of adverse events was numerically lower with CCH vs. equivalent complications from fasciectomy (median [range] incidence), including nerve injury (0% vs. 3.8% [0%-50+%]), neurapraxia (4.4% vs. 9.4% [0%-51.3%]), complex regional pain syndrome (0.1% vs. 4.5% [1.3%-18.5%]) and arterial injury (0% vs. 5.5% [0.8%-16.5%]). Tendon injury (0.3% vs. 0.1% [0%-0.2%]), skin injury (16.2% vs. 2.8% [0%-25.9%]) and haematoma (77.7% vs. 2.0% [0%-25%]) occurred at a numerically higher incidence with CCH than surgery. Adverse events in CCH trials not reported after fasciectomy included peripheral oedema; extremity pain; injection site pain, haemorrhage and swelling; tenderness; pruritus and lymphadenopathy. CCH-related adverse events were reported as predominantly injection-related and transient. These results may support clinical decision-making for treatment of Dupuytren's contracture.

31. The comparative immunogenicity of biologic therapy and its clinical relevance in psoriatic arthritis: a systematic review of the literature

Abstract

ABSTRACTIntroduction: Biologic agents have demonstrated efficacy in treating patients with psoriatic arthritis (PsA). Biologic agents also have an intrinsic capacity to induce an immune response in patients that could result in unwanted adverse events and/or treatment failure.Areas covered: In this systematic literature review, the authors document the incidence of immune responses, primarily anti-drug antibodies (ADA), to the biologic therapeutic agents currently in clinical practice for the treatment of PsA. The authors discuss the importance of these responses with respect to clinical practice.Expert opinion: Our evaluation of the published literature shows that the immune responses to the various biologic therapeutic agents currently being used to treat PsA are similar to those observed for these agents in other rheumatic diseases. Moreover, similar to observations in other rheumatic diseases, the incidence of ADA formation to biologic agents in patients with PsA is often decreased when patients are given concomitant treatment with disease-modifying anti-rheumatic drugs. These data strongly suggest that the immune response is a characteristic of the biologic agent. Using therapeutic drug monitoring may be an approach to assess the immune response to the agent and to mitigate the potential impact on efficacy and safety, and consequently optimize treatment.

Abstract

Two notable films set against the backdrop of wartime Poland and the Holocaust, released three years apart--Andrzej Wajda's Korczak and Steven Spielberg's Schindlers List--are compared. Numerous similarities are found, possibly because Spielberg was influenced by the earlier work. Both films are structured around their title characters--alike in some ways, profoundly different in others--the first a Polish-Jewish educator and orphanage director who remained faithful to his youthful charges even unto death at Treblinka, the second a German Nazi profiteer who repented and famously arranged to save the lives of his Jewish workers. Each is presented as a strong fatherly figure, although Korczak is presented as an idealist who stays true to his principles despite the war and Schindler as a man transformed for the better by the conflict. While there is strong reason to believe that Schindler offers an idealized picture of its protagonist, Wajda's portrayal of Korczak can be considered historically accurate. [ABSTRACT FROM AUTHOR]

Abstract

Od połowy lat 80. XX w. z wielu krajów zaczęły napływać doniesienia o ciężkich uogólniających się zakażeniach Streptococcus pyogenes, przebiegających z objawami wstrząsu toksycznego. Liczba tego typu zakażeń wzrasta i może stanowić poważny problem diagnostyczno-terapeutyczny. Ciężkie objawy, piorunujący przebieg, wysoka śmiertelność oraz możliwość transmisji inwazyjnego patogenu stały się powodem poszukiwań możliwości zapobiegania tym zachorowaniom. Większość tego typu zakażeń inwazyjnych ma charakter sporadyczny, a ponad 85% występuje w środowisku pozaszpitalnym. W pracy przedstawiono propozycje racjonalnych zasad postępowania profilaktycznego z osobami, które miały kontakt z chorym w środowisku domowym na podstawie najnowszych publikowanych danych.

43. Group A Streptococci from Invasive-Disease Episodes in Poland Are Remarkably Divergent at the Molecular Level

Abstract

ABSTRACTForty-one clinical isolates of group A streptococcus (GAS) were recovered in Poland from patients with severe invasive infections and were analyzed by phenotypic and genotypic techniques. All isolates were characterized by determining their susceptibilities to antimicrobial agents and by determining their types by pulsed-field gel electrophoresis, multilocus sequence typing, emmtyping, and the detection of five streptococcal pyrogenic exotoxin genes (speA, speB, speC, speF, ssa). The isolates studied were fully susceptible to penicillin G, levofloxacin, quinupristin-dalfopristin, and linezolid. Resistance to tetracycline, chloramphenicol, and erythromycin was detected in 46.3, 12.1, and 9.8% of the isolates, respectively. A total of 23 different emmsequence types were identified, of which emm1and emm12(19.5% each) were the most common, followed by emm81, emm44/61, and emm85. All the emm1isolates had the speA2allele. Twenty-three unrelated sequence types (STs) were identified, with the most frequent STs, ST28 and ST36, corresponding to emm1and emm12, respectively. Six newly found STs (STs 375, 376, 377, 378, 379, and 385) corresponded to emmtypes 74, 102, 77, 76, 84 and 63, respectively. The emm1type and the presence of speA2gene were associated with the severity of GAS infections. This work presents the first molecular study on Polish invasive GAS isolates.

45. Antibiotic susceptibility of bacterial strains isolated from urinary tract infections in Poland.

Abstract

Worldwide data show that there is increasing resistance among urinary tract pathogens to conventional drugs. The aim of this study was to obtain data on susceptibility patterns of pathogens responsible for urinary tract infections (UTIs) in Poland to currently used antimicrobial agents. A multicentre study of 141 pathogens from hospital-acquired infections and 460 pathogens from community-acquired infections was carried out between July 1998 and May 1999. The most prevalent aetiological agent was Escherichia coli (73.0%), followed by Proteus spp. (8.9%) and other species of Enterobacteriaceae (9.6%). Few community infections were caused by Gram-positive bacteria (2.2%). Gram-positive cocci were isolated more frequently from a hospital setting (14.1%) and the most common were Enterococcus spp. (8.5%). Pseudomonas aeruginosa was found only among hospital isolates and was responsible for 10.7% of infections. E. coli isolates from both community and hospital infections were highly susceptible to many antimicrobial agents with the exception of those isolates producing extended-spectrum beta-lactamases (ESBLs). Of all Enterobacteriaceae tested, 38 strains (6.9%) were capable of producing ESBLs.

The adsorption of metal ions at the stearic acid/electrolyte and nnonadccane-stearic acid mixture/electrolyte interface was investigated by means of the potentiometric titration, zeta potential and adsorption measurements. It was found that the studied colloidal suspensions exhibited an adsorption affinity towards multivalent metal ions. The adsorption of Ca2+, Cd2+ and Al3+ ions caused a strong decrease of surface charge density and zeta potential values in this systems. The adsorption reactions occur by way of cation exchange with protons from two surface carboxyl groups. Al high metal concentrations, in adsorption reactions there are involved also carboxyl groups from the subsurface layer. On the basis of the adsorption data, the cation surface complexation constants were calculated by Shindler's method.

Abstract

The description of electrical double layer at the metal oxide/electrolyte interface requires the determination of the constants of the ionization and complexation of surface groups. These constants allows to determine distribution of surface charge and a potential across the interface region. The different methods of the estimation of these constants at the metal oxide/electrolyte interface, including potentiometric titration, ion adsorption data and zeta potential are compared.
