Abstract

This paper describes the joint effort of Brno University of Technology (BUT), AGH University of Krakow and University of Buenos Aires on the development of Automatic Speech Recognition systems for the CHiME-7 Challenge. We train and evaluate various end-to-end models with several toolkits. We heavily relied on Guided Source Separation (GSS) to convert multi-channel audio to single channel. The ASR is leveraging speech representations from models pre-trained by self-supervised learning, and we do a fusion of several ASR systems. In addition, we modified external data from the LibriSpeech corpus to become a close domain and added it to the training. Abstract

Abstract Childhood adversity is associated with various clinical dimensions in psychosis; however, how genetic vulnerability shapes the adversity-associated psychopathological signature is yet to be studied. We studied data of 583 First Episode Psychosis (FEP) cases from the EU-GEI FEP case-control study, including Polygenic risk scores for major depressive disorder (MDD-PRS), bipolar disorder (BD-PRS) and schizophrenia (SZ-PRS); childhood adversity measured with the total score of the Childhood Trauma Questionnaire (CTQ); and positive, negative, depressive and manic psychopathological domains from a factor model of transdiagnostic dimensions. Genes and environment interactions were explored as a departure from a multiplicative effect of PRSs and total CTQ on each dimension. Analyses were adjusted for age, sex, 10 PCA, site of recruitment and for medication. A childhood adversity and PRS multiplicative interaction was observed between A) the CTQ and MDD-PRS on the predominance of positive (β = 0.42, 95% CI = [0.155, 0.682], p = 0.004); and depressive (β = 0.33, 95% CI = [0.071, 0.591], p = 0.013) dimensions; B) between the CTQ and BD-PRS on the positive dimension (β = 0.45, 95% CI = [0.106, 0.798], p = 0.010), and C) with the CTQ and SZ-PRS on the positive dimension (β = −0.34, 95% CI = [−0.660, −0.015], p = 0.040). Bonferroni corrected p-value of significance was set at 0.0125. In conclusion, despite being underpowered, this study suggests that genetic liability for MDD and BD may have a moderating effect on the sensibility of childhood adversity on depressive and positive psychotic dimensions. This supports the hypothesis of an affective pathway to psychosis in those exposed to childhood adversity.

5. Development of Hybrid Laboratory Sessions during the COVID-19 Pandemic

Abstract

This paper presents the hybrid delivery method of a laboratory experiment at the Stability Wind Tunnel of Virginia Tech to some 170 students during April 2020, which can be considered the early stages of the COVID-19 induced lockdown. The steps of converting the hands-on labs to hybrid labs are presented in detail. Namely, a videoconferencing tool was used to: (1) stream the instrumentation used; (2) provide live video feed; and (3) to interact with the students. Labs began with a remote tour of the facility, whilst the presence of an expert-at-a-distance added key value to the labs as it enhanced students conceptual understanding via verbal interaction. The experiments were then performed by laboratory personnel while student's engagement was kept high via the teleconferencing session. At the end of the two-week laboratory campaign, the students provided feedback of the laboratory sessions via an open-ended and closed-ended survey. They highlighted the added value of expert-at-distance, the live video feed, and the ability of working with instructors. While their feedback was rather positive, students showed a strong preference toward hands-on laboratories. Overall, the methodologies presented here can be considered a relatively low-cost method to upgrade hands-on laboratories to hybrid or remote labs.

Abstract

Personal assistants, automatic speech recognizers and dialogue understanding systems are becoming more critical in our interconnected digital world. A clear example is air traffic control (ATC) communications. ATC aims at guiding aircraft and controlling the airspace in a safe and optimal manner. These voice-based dialogues are carried between an air traffic controller (ATCO) and pilots via very-high frequency radio channels. In order to incorporate these novel technologies into ATC (low-resource domain), large-scale annotated datasets are required to develop the data-driven AI systems. Two examples are automatic speech recognition (ASR) and natural language understanding (NLU). In this paper, we introduce the ATCO2 corpus, a dataset that aims at fostering research on the challenging ATC field, which has lagged behind due to lack of annotated data. The ATCO2 corpus covers 1) data collection and pre-processing, 2) pseudo-annotations of speech data, and 3) extraction of ATC-related named entities. The ATCO2 corpus is split into three subsets. 1) ATCO2-test-set corpus contains 4 hours of ATC speech with manual transcripts and a subset with gold annotations for named-entity recognition (callsign, command, value). 2) The ATCO2-PL-set corpus consists of 5281 hours of unlabeled ATC data enriched with automatic transcripts from an in-domain speech recognizer, contextual information, speaker turn information, signal-to-noise ratio estimate and English language detection score per sample. Abstract

AimThe anticholinergic properties of medications are associated with poorer cognitive performance in schizophrenia. Numerous scales have been developed to assess anticholinergic burden and yet, there is no consensus indicating which anticholinergic burden scale is more relevant for patients with schizophrenia. We aimed to identify valid scales for estimating the risk of iatrogenic cognitive impairment in schizophrenia.MethodsWe identified 27 scales in a literature review. The responses to neuropsychological tests of 839 individuals with schizophrenia or schizoaffective disorder in the FACE-SZ database were collected between 2010 and 2021. We estimated the association between objective global cognitive performance and the 27 scales, the number of psychotropic drugs, and chlorpromazine and lorazepam equivalents in bivariable regressions in a cross-sectional design. We then adjusted the bivariable models with covariates: the predictors significantly associated with cognitive performance in multiple linear regressions were considered to have good concurrent validity to assess cognitive performance.ResultsEight scales, the number of psychotropic drugs, and drug equivalents were significantly associated with cognitive impairment. The number of psychotropic drugs, the most convenient predictor to compute, was associated with worse executive function (Standardized β = −0.12, p = .004) and reasoning (Standardized β = −0.08, p = .037).ConclusionAnticholinergic burden, the number of psychotropic drugs, and drug equivalents were weakly associated with cognition, thus suggesting that cognitive impairment in schizophrenia and schizoaffective disorder is explained by factors other than medication. The number of psychotropic drugs was the most parsimonious method to assess the risk of iatrogenic cognitive impairment.

Abstract

Air traffic control (ATC) relies on communication via speech between pilot and air-traffic controller (ATCO). The call-sign, as unique identifier for each flight, is used to address a specific pilot by the ATCO. Extracting the call-sign from the communication is a challenge because of the noisy ATC voice channel and the additional noise introduced by the receiver. A low signal-to-noise ratio (SNR) in the speech leads to high word error rate (WER) transcripts. We propose a new call-sign recognition and understanding (CRU) system that addresses this issue. The recognizer is trained to identify call-signs in noisy ATC transcripts and convert them into the standard International Civil Aviation Organization (ICAO) format. By incorporating surveillance information, we can multiply the call-sign accuracy (CSA) up to a factor of four. Abstract

Background/Objectives: Melanoma is an aggressive skin malignancy, and the majority of deaths associated with melanoma result from malignant skin lesions. Our study aims to evaluate the expression of the markers CD133 and NANOG, associated with tumor stem cells, and to analyze their link with epidemiological and histological parameters, thus contributing to early diagnosis and the development of targeted therapies. Methods: We performed a retrospective study in the Mureș Clinical County Hospital, Romania, which included 66 cases of melanoma: 50 primary cutaneous melanomas, 10 metastases, and 6 local recurrences. CD133 and NANOG marker expression was assessed by immunohistochemistry and quantified using the H score. Statistical analyses were applied to determine the correlations between marker expression and clinicopathological parameters. Results: CD133 expression was identified in six cases (12%) of primary melanoma, with a mean H-Score of 29, and was associated with an increased Breslow index and a higher number of mitoses. NANOG expression was positive in 30 cases (60%) of primary melanoma, with a median H-Score of 15 and with increased expression observed in cases with pagetoid migration and lesions in situ. In metastases, eight cases (80%) were positive for NANOG and four (40%) for CD133. Local recurrences showed positive expression for NANOG in four cases (66%). Conclusions: The expression of CD133 and NANOG markers highlights the role of tumor stem cells in melanoma progression. Early identification of these markers could improve diagnosis and treatment, including the application of targeted therapies.

Abstract

Abstract Schizophrenia is characterized by the most salient medication adherence problems among severe mental disorders, but limited prospective data are available to predict and improve adherence in this population. This investigation aims to identify predictors of medication adherence over a 1-year period in a large national cohort using clustering analysis. Outpatients were recruited from ten Schizophrenia Expert Centers and were evaluated with a day-long standardized battery including clinician and patient-rated medication adherence measures. A two-step cluster analysis and multivariate logistic regression were conducted to identify medication adherence profiles based on the Medication Adherence rating Scale (MARS) and baseline predictors. A total of 485 participants were included in the study and medication adherence was significantly improved at the 1-year follow-up. Higher depressive scores, lower insight, history of suicide attempt, younger age and alcohol use disorder were all associated with poorer adherence at 1 year. Among the 203 patients with initially poor adherence, 86 (42%) switched to good adherence at the 1-year follow-up, whereas 117 patients (58%) remained poorly adherent. Targeting younger patients with low insight, history of suicide, alcohol use disorder and depressive disorders should be prioritized through literacy and educational therapy programs. Adherence is a construct that can vary considerably from year to year in schizophrenia, and therefore may be amenable to interventions for its improvement. However, caution is also warranted as nearly one in five patients with initially good adherence experienced worsened adherence 1 year later.

Abstract

Air traffic management and specifically air-traffic control (ATC) rely mostly on voice communications between Air Traffic Controllers (ATCos) and pilots. In most cases, these voice communications follow a well-defined grammar that could be leveraged in Automatic Speech Recognition (ASR) technologies. The callsign used to address an airplane is an essential part of all ATCo-pilot communications. We propose a two-steps approach to add contextual knowledge during semi-supervised training to reduce the ASR system error rates at recognizing the part of the utterance that contains the callsign. Initially, we represent in a WFST the contextual knowledge (i.e. air-surveillance data) of an ATCo-pilot communication. Then, during Semi-Supervised Learning (SSL) the contextual knowledge is added by second-pass decoding (i.e. lattice re-scoring). Results show that `unseen domains' (e.g. data from airports not present in the supervised training data) are further aided by contextual SSL when compared to standalone SSL. For this task, we introduce the Callsign Word Error Rate (CA-WER) as an evaluation metric, which only assesses ASR performance of the spoken callsign in an utterance. We obtained a 32.1% CA-WER relative improvement applying SSL with an additional 17.5% CA-WER improvement by adding contextual knowledge during SSL on a challenging ATC-based test set gathered from LiveATC., Comment: Presented at: Interspeech conference 2021 (Brno, Czechia, August 30 - September 3)

Abstract

As the ‘WHO Traditional Medicine Strategy: 2014–2023’ is entering its final phase, reflection is warranted on progress and the focus for a new strategy. We used WHO documentation to analyse progress across the objectives of the current strategy, adding the role of traditional, complementary and integrative healthcare (TCIH) to address specific diseases as a dimension absent in the current strategy. Our analysis concludes on five areas. First, TCIH research is increasing but is not commensurate with TCIH use. TCIH research needs prioritisation and increased funding in national research policies and programmes. Second, WHO guidance for training and practice provides useful minimum standards but regulation of TCIH practitioners also need to reflect the different nature of formal and informal practices. Third, there has been progress in the regulation of herbal medicines but TCIH products of other origin still need addressing. A risk-based regulatory approach for the full-range of TCIH products seems appropriate and WHO should provide guidance in this regard. Fourth, the potential of TCIH to help address specific diseases is often overlooked. The development of disease strategies would benefit from considering the evidence and inclusion of TCIH practices, as appropriate. Fifth, inclusion of TCIH in national health policies differs between countries, with some integrating TCIH practices and others seeking to restrict them. We encourage a positive framework in all countries that enshrines the role of TCIH in the achievement of universal health coverage. Finally, we encourage seeking the input of stakeholders in the development of the new WHO Traditional Medicine Strategy.

Abstract

Background/Objectives: Melanoma is an aggressive skin malignancy, and the majority of deaths associated with melanoma result from malignant skin lesions. Our study aims to evaluate the expression of the markers CD133 and NANOG, associated with tumor stem cells, and to analyze their link with epidemiological and histological parameters, thus contributing to early diagnosis and the development of targeted therapies. Methods: We performed a retrospective study in the Mureș Clinical County Hospital, Romania, which included 66 cases of melanoma: 50 primary cutaneous melanomas, 10 metastases, and 6 local recurrences. CD133 and NANOG marker expression was assessed by immunohistochemistry and quantified using the H score. Statistical analyses were applied to determine the correlations between marker expression and clinicopathological parameters. Results: CD133 expression was identified in six cases (12%) of primary melanoma, with a mean H-Score of 29, and was associated with an increased Breslow index and a higher number of mitoses. NANOG expression was positive in 30 cases (60%) of primary melanoma, with a median H-Score of 15 and with increased expression observed in cases with pagetoid migration and lesions in situ. In metastases, eight cases (80%) were positive for NANOG and four (40%) for CD133. Local recurrences showed positive expression for NANOG in four cases (66%). Conclusions: The expression of CD133 and NANOG markers highlights the role of tumor stem cells in melanoma progression.
