Background Macrotextured breast implants are associated with double capsules. There is little agreement as to what defines double capsules, how they present, and whether different degrees of double capsule exist. Objectives This study aimed to define double capsules and report an association between double-capsule type and degree of tissue adherence. Methods Consecutive aesthetic patients undergoing explantation of Biocell (Allergan, Inc., Irvine, CA) implants between May 2018 and November 2018 were included if they were found to have double capsules intraoperatively. Patient demographics, implant characteristics, explantation reason, implant adherence, and intraoperative findings were recorded. Both adherent and double capsules were histologically examined. Data were analyzed by descriptive statistics. Results Eleven patients had 22 Biocell implants explanted during the study period. The average explantation time was 8.0 years. Sixteen implants were found to have some degree of nonadherence, and all areas of nonadherence were associated with double-capsule formation. Double capsules were either partial or complete. The architecture of the inner layer of double capsules varied between an organized capsular layer and a thin area of nascent capsule. Histologically, all capsular specimens demonstrated an overall hypocellular fibrous capsule with scattered chronic inflammatory infiltrates. Synovial metaplasia was present in all capsule types and spaces/cracks in the capsule were disproportionately represented in partially adherent capsules. Conclusions This is the first study to identify a clinical and pathological correlation between double capsules and failed tissue adherence. Double capsules represent a spectrum of inner capsule formation ranging between nascent capsular tissue to a mature inner capsular layer. Level of Evidence: 4

Purpose To evaluate capsules formed by microtextured silicone implants with and without Parietex® mesh coverage histologically. Methods Sixty Wistar rats were divided in two groups (meshed and unmeshed). Each group was, then, divided into two subgroups for evaluation at 30 and 90 days. Capsules were analyzed based on hematoxylin and eosin (HE) and picrosirius staining. Results The number of fibroblasts, neutrophils and macrophages was similar among all subgroups. There was a higher lymphocyte reaction in the 30-day meshed group (p = 0.003). Giant cell reaction, granulation tissue and neoangiogenesis were similar among the subgroups. Synovial metaplasia was milder at 90-day in the unmeshed (p = 0.002) and meshed group (p < 0.001). Capsular thickness was significantly greater in the meshed samples (30-day p < 0.001 and 90-day p < 0.001). There was a similar amount of collagen types I and III in both groups. Conclusions The mesh-covered implants produced capsules similar to the microtextured ones when analyzing inflammatory variables. Synovial metaplasia was milder at 90 than at 30 days, and the capsular thickness was significantly greater in the meshed group. A similar amount of collagen types I and III was observed. Due to these characteristics, the mesh coverage did not seem to significantly affect the local inflammatory activity.

Synovial chondromatosis is a rare condition consisting of metaplasia of the synovial tissue that usually presents in large joints such as the knee and hip. The reported occurrence of synovial chondromatosis in the foot and ankle joints is rare in the literature. In this case report, the successful surgical management of two patients presenting with this condition using open ankle arthrotomy & synovectomy is described. These two cases are presented to provide added depth to the current literature, as well as presenting a review of published literature to further guide clinicians in the management of this rare condition. Background: Capsular contracture remains one of the major complications after breast implantation surgery. The extent of capsular contraction is scored using the Baker scale. The aim of this study was to compare intra-individual Baker-I with Baker-IV capsules, and in particular the prevalence and histological properties of the inner capsule layer. Methods: Twenty capsules from ten patients were included after bilateral explantation surgery due to unilateral capsular contracture (Baker-IV) after cosmetic augmentation with textured implants. All capsules underwent (immune-)histochemical analysis: haematoxylin–eosin (morphology), CD68 (macrophages), cytokeratin (epithelial cells) and vimentin (fibroblasts), and were visually scored for cell density and the presence of an inner layer and measured for thickness. Results: Baker-IV (n = 10) capsules were significantly thicker compared to Baker-I (n = 10) capsules (P = 0.004). An inner layer was present in 8 Baker-I capsules. All Baker-I capsules were vimentin and CD68-positive and cytokeratin-negative. Positive vimentin was seen throughout the inner layer, and CD-68 staining was observed adjacent to the intermediate capsule layer. In contrast, only 2 Baker-IV capsules had an inner layer, of which only 1 showed the same profile as Baker-I capsules (P = 0.016). No cytokeratin positivity was seen in any capsule. In Baker-IV capsules, outer layers showed more positivity for both vimentin and CD68. Conclusion: The inner layer is morphologically consistent with synovial metaplasia and is more prevalent in healthy, uncontracted Baker-I capsules. This inverse relation between the presence of the inner layer and higher Baker classification or pathological contracture could indicate a protective role of the inner layer against capsular contracture formation. Traumatic implantation of foreign material in the oral mucosa is unusual and may occur. In this article, we report the presence of an unusual foreign object associated with a cyst-like inflammatory response in a 50-year-old male. The patient presented with a painless recurrent soft tissue swelling in the anterior vestibule of maxilla of at least 2-year duration. The lesion was surgically excised and a retained shrapnel-like object within a cyst-like and thick-walled structure was obtained. Histopathologically, the cyst-like structure exhibited a lymphohistiocytic infiltrate in the wall and a luminal synovial metaplasia-like change, predominantly composed of histiocytes. Immunohistochemical stain of CD163 highlighted the histiocytes. The patient stated a history of traumatic implantation of shrapnel fragments in the area that occurred 30 years ago due to an explosion in the Iran-Iraq war. Die primare synoviale Chondromatose ist eine seltene, zumeist monoartikular auftretende Metaplasie der Synovia mit einer hohen Rezidivneigung. In seltenen Fallen kann es zu einer malignen Entartung kommen, wobei die histologische und rontgenmorphologische Unterscheidung von einem „Low-grade“-Chondrosarkom mit Schwierigkeiten verbunden ist. Wir berichten von einem 32-jahrigen Patienten mit rezidivierender Chondromatose im rechten Tarsometatarsalbereich, einer seltenen Lokalisation.

Resumen Introduccion Presentamos un caso excepcional de metaplasia sinovial localizada en la capsula producida como respuesta a protesis mamaria de silicona como consecuencia de un raro tumor recidivante. Material y metodos Mujer de 31 anos a la que se realiza mastectomia e implante de protesis de silicona, por tumor mixoide recidivante diagnosticado como histiocitoma fibroso maligno, variante mixoide. Por dos veces se implanto protesis de silicona y ambas produjeron una capsula integra fibrosa. Resultados El estudio histopatologico evidencio que la membrana estaba revestida por una verdadera metaplasia sinovial con dos tipos celulares. Las tecnicas de inmunohistoquimica fueron positivas para CD-68, vimentina y proteina S-100. Esto se correlaciono con la ultraestructura donde se ponen de manifiesto dos tipos bien diferenciados de celulas. Ademas se realizo microestudio de particulas y se pudo observar presencia de silicona en la superficie de la membrana. Conclusiones Los resultados presentados nos manifiestan que la silicona es el material desencadenante de la metaplasia sinovial y no las maniobras quirurgicas como estan descritas en otras localizaciones. Se trata de metaplasia sinovial verdadera siendo menos de cinco los casos publicados en la literatura medica.

We report a case of hypertrophic synovial metaplasia in the third metatarsophalangeal joint of a young fit male. Clinical features were vague and indistinct, with morphological and histopathological features being difficult to interpret. Eventual diagnosis was made on correlation of painful symptoms, MRI findings, and histological features. Treatment consisted of excision of the metaplastic tissue and eventual excision arthroplasty of the MTP joint.

Synovial metaplasia is a change seen most frequently in the tissues surrounding silicone breast prostheses and in healing tissue adjacent to joint prostheses. It has also been described in skin and soft tissues, most frequently in healing or healed traumatic or surgical wounds. We report a case of synovial metaplasia occurring in a hitherto unreported location, namely, adjacent to a silicone low-pressure voice prosthesis. A review of cases of synovial metaplasia reported in the literature revealed that in most cases, spaces that form adjacent to foreign material (most commonly silicone breast prostheses) and the smooth gliding surfaces of the foreign material that resist penetration by fibroblast processes are frequent associated findings that precede the occurrence of synovial metaplasia. Thus, synovial metaplasia might represent a specialized form of healing in cases that have this combination of physical features.

Both giant synovial osteochondroma and parosteal osteosarcoma are rare musculo-skeletal tumors, often localized in the vicinity of the knee. Misdiagnosis of a malignant bone tumor can entail fatal consequences. Etiology of giant synovial osteochondroma is widely unsolved but is believed to originate from synovial chondromatosis, a mostly benign metaplasia of the synovial membrane. Parosteal osteosarcoma is a low-grade surface osteosarcoma with a propensity of local recurrence and the potential of distant metastasis and therefore requiring a different therapeutical approach. We report the case of a popliteal giant osteochondroma mimicking a parosteal osteosarcoma. Relevant facts of this rare entity regarding pathogenesis, treatment, and differential diagnoses will be discussed.

La condromatosis sinovial es una lesión poco frecuente caracterizada por la metaplasia cartilaginosa de la membrana sinovial de pequeñas y grandes articulaciones. Se suelen observar múltiples nódulos cartilaginosos y un grado variable de destrucción articular. Las manifestaciones clínicas son variables y dependen de la localización y el estadio de la enfermedad. Nuestra paciente concurrió con una tumoración en la planta del pie a nivel de la raíz del segundo y tercer dedo, de meses de evolución. Luego de los estudios por imágenes correspondientes y evidenciar la extensión dorsal de la lesión, se decidió realizar la resección en bloque a través de doble abordaje; el análisis anatomopatológico informó condromatosis sinovial. La ausencia de extensión intrarticular fue constatada en la cirugía. Este caso representa una manifestación poco frecuente de la patología no solo por su localización, sino también por su ubicación extrarticular. Según nuestra búsqueda bibliográfica, no se han publicado reportes de casos con condromatosis sinovial extrarticular avanzada (grado 3) en el antepié y creemos que debería ser considerada dentro de las lesiones tumorales de aspecto cartilaginoso en esta localización.

Saline-filled prostheses are currently the only type of prostheses available for cosmetic use in the United States because of concerns raised about the possibility of systemic toxicity of silicone-filled artificial mammary implants. Although the approved implants are saline-filled, their potential to release silicone particles from the shells has not been systematically evaluated. We performed microscopic examination of the pericapsular tissue of 54 patients with textured-surface implants and compared these with 51 patients with smooth-walled implants over a 2-year period. The capsules that had formed around virtually all textured-surface implants had silicone fragments present either in extracellular spaces, in vacuolated histiocytes, or in the form of foreign-body granulomas in surrounding fibroadipose tissue but not in capsules associated with smooth-walled implants. In 87 percent of samples of pericapsular tissue from textured saline implants, the contact surface displayed exuberant reactive synovial metaplasia, a histologic pattern not previously described with these devices. Our findings suggest that smooth-walled prostheses are associated with less silicone fragmentation than textured devices in the peri-implant tissue capsules that tend to form around artificial surfaces used for this purpose.

Following a statement by the UK Medical Devices Agency (MDA), soya-oil-filled Trilucent implants for breast augmentation were withdrawn from sale in March 1999. The most recent report on the toxicity of the Trilucent implant suggests that one of the breakdown products of the filler is an aldehyde with an explicit risk of genotoxic and teratogenic effects. Explantation of all these prostheses was advised by the MDA in June 2000. No guidance regarding capsulectomy was put forward by any publication. The operation of capsulectomy confers an increased morbidity compared with simple prosthetic replacement. This study looks at the capsules of 18 consecutive patients who had received Trilucent implants for cosmetic breast augmentation, and who had undergone explantation and capsulectomy. The mean duration of implantation was 3 years (range: 2 years to 4 years 9 months). The capsules were examined histologically. The significant features of all the capsules included a florid foreign-body type reaction, synovial metaplasia, a villous hyperplasia and the presence of refractile material within the substance of the capsule. It is postulated that the refractile material is filler material that has bled through the wall of the intact Trilucent implant, and as such would represent a potential genotoxic hazard. Until such time as this potential hazard has been investigated fully and a conclusion reached, we recommend acceptance of the increased morbidity, and the performance of capsulectomies when explanting Trilucent implants.

We present a unique case of a lipoma with widespread synovial metaplasia. A 52-year-old woman had a recurrence 1 year after excision of a subcutaneous lipoma of the neck. Histologically, the primary tumor was an ordinary lipoma. The recurrent tumor was a myxoid lipoma with synovial metaplasia. The synovial metaplastic process manifested as labyrinthlike clefts, which were lined by one or more synovial-like cell layers. Ultrastructurally, the synovial metaplastic cell had secretory, phagocytic, and fibroblastlike features.

A 43-year-old woman underwent two-stage bilateral reconstruction mammoplasty using silicone-filled, polyurethane foam-covered breast implants. The permanent implants were later removed. The periprosthetic capsule demonstrated synovial metaplasia by light and electron microscopy. Synovial metaplasia is believed to be induced by the physical and chemical characteristics of the prostheses and may have important clinical implications in reducing capsular contracture and increasing host acceptance of implantable biomaterials.

Synovial chondromatosis is a rare disease, especially in children. We report on a 12-year old girl who suffered twice a knee trauma. She then developed an arthritis of her right knee joint and finally showed all signs of synovial chondromatosis arthroscopically. Treatment consisted of removal of loose as well as stalked chondromatous bodies by means of arthroscopy. According to Milgram (1975) the evolution of the disease progresses through three stages, starting with synovial metaplasia producing chondromas which are then shed into the joint. Kay, Freemont and Davies (1989) doubt this theory.

The metaplastic synovial cyst (synovial metaplasia of the skin) is a recently described intradermal cyst lined by a membrane that resembles hyperplastic synovium. All of the reported cases thus far have occurred in adults after surgical incision. A case of metaplastic synovial cyst after partial excision of nevus sebaceus in a 7-year-old girl is herein reported. Pathogenesis and light-microscopic and immunohistochemical features of this case are discussed.

Synovial chondromatosis is a rare condition in which osteocartilaginous nodules are formed by synovial metaplasia and become intra-articular loose bodies. It is usually monarticular, most commonly affecting the knee and is regarded as invariably benign. There are reports of malignant change, but only the two case studies of Mullins, Berard and Eisenberg (1965) and of Dunn et al. (1974) fully document the development of chondrosarcoma in this condition. We report a patient with a long history of histologically proven synovial chondromatosis in whom the condition became clinically aggressive and underwent malignant transformation to chondrosarcoma.
