A study of seasonal variation, sources and potential risks of organochlorine pesticides (OCPs) and polychlorinated biphenyls (PCBs) in open city drains in Makurdi, Central Nigeria was carried out. OCPs and PCBs were quantified using gas chromatograph-mass spectrometer. The total (∑8OCPs) concentrations (ngL−1) of OCPs in water was 2.99 with a mean ± SD of 0.75 ± 0.12 during wet season, while during dry season, the values were 11.43 and 2.86 ± 1.54 respectively. In sediment, the total concentration (ngg−1) of OCPs was 5270.66 with a mean ± SD of 1756.89 ± 450.01 during wet season and a total concentration of 5837.93 and the mean ± SD of 1945.98 ± 646.04, during dry season. Source apportionment of OCPs suggested historic application of the pollutants. The total (∑7PCBs) concentration (ngL−1) of PCBs in water was 0.24 with a mean ± SD of 0.03 ± 0.02 during wet season and a total concentration of 0.61 with a mean ± SD of 0.09 ± 0.11 during dry season. The total concentration (ngg−1) of PCBs in sediment was 37.88, mean ± SD of 5.41 ± 5.93 during wet season and a total of 47.07 and mean ± SD of 6.72 ± 7.27 during dry season. Ecological risk assessment based on effect range low (ERL) and effect range median (ERM) or threshold effect level (TEL) and probable effect level (PEL) that ecological risks were possible for some OCPs in this study, which calls for source control and remediation of the affected sites. Toxicity equivalency (TEQ) of PCB-118, the dioxin-like congener, indicated that it was most harmful to humans/mammals followed by birds, then fish.

Background. Preservation solutions are required for organ viability in deceased donor liver transplantation (LT). However, their role in live donor LT (LDLT) has not been standardized. Methods. Eighty adult recipients who underwent right lobe LDLT at the Department of Liver Transplantation Surgery, Gambat, Pakistan, were studied. Based on shorter cold ischemia time and no back table reconstruction work, recipients were assigned to receive “no preservation solution” (cases/non–histidine-tryptophan-ketoglutarate group; n = 40) or “HTK group” (controls; n = 40). Early allograft dysfunction (bilirubin, transaminases, and international normalized ratio), postoperative complications (biliary and vascular), hospital stay, and 1-y survival were reported. The direct cost was also reported. Results. Demographics and clinical characteristics were comparable in the 2 groups. Comparing cases versus controls, mean bilirubin, alanine aminotransferase, aspartate aminotransferase, and international normalized ratio on postoperative day 7 were similar in the 2 groups. Five (12.5%) cases and 4 (10%) controls developed early allograft dysfunction (P = 0.72). Post-LT complications (biliary leak 2.5% in cases versus 0 in control), strictures (15% in cases versus 17.5% in controls), hepatic artery thrombosis (2.5% versus 00%)‚ and portal vein thrombosis (0 versus 2.5%) were comparable. Mean hospital stay (10.80 + 2.36 and 11.78 + 2.91 d) and 30 d mortality (2.5% versus 5%) were also comparable. Finally, 1-y survival based on Kaplan-Meier analysis was comparable in both groups (ie, 92.5%; non-HTK group versus 90%; HTK group) (P = 0.71). The direct cost of using a non-HTK–based approach was less than the HTK solution. Conclusion. In a selected cohort of right lobe LDLT recipients, preservation solutions can be avoided safely with comparable outcomes.

The advance of carbon derivatives enhanced the performance of recently fabricated electrodes for energy storage devices. GO/Co3O4/NiO conductive single and composite electrodes were fabricated using hydrothermal technique and employed as electrodes for supercapacitor using three-electrode systems. The GO/Co3O4/NiO composite electrode demonstrates greater rate capability and excellent lasting stability. It also exhibits high energy density at comparatively high power density. The results presented a specific capacitance of the GO/Co3O4/NiO composite electrode of 1612 F/g at 1.0 A/g current density. GO/Co3O4/NiO composite electrode exhibited cycling stability of 88.6%. after 10, 000 cycles. Results indicate that the composites' electrode achieves improved specific capacitance than solitary metal oxide electrode and the addition of graphene oxide enhanced the performance. The results of the three samples were compared, the performance of the GO/Co3O4/NiO electrode enriched better than GO/Co3O4 and GO/NiO, which can be attributed to the synergistic effect of Co3O4, NiO, and GO.

Introduction and aim: Hepatic encephalopathy (HE) is a common complication in cirrhotics and is associated with an increased healthcare burden. Our aim was to study independent predictors of 30-day readmission and develop a readmission risk model in patients with HE. Secondary aims included studying readmission rates, cost, and the impact of readmission on mortality. Materials and methods: We utilized the 2013 Nationwide Readmission Database (NRD) for hospitalized patients with HE. A risk assessment model based on index hospitalization variables for predicting 30-day readmission was developed using multivariate logistic regression and validated with the 2014 NRD. Patients were stratified into Low Risk and High Risk groups. Cox regression models were fit to identify predictors of calendar-year mortality. Results: Of 24,473 cirrhosis patients hospitalized with HE, 32.4% were readmitted within 30 days. Predictors of readmission included presence of ascites (OR: 1.19; 95% CI: 1.06–1.33), receiving paracentesis (OR: 1.43; 95% CI: 1.26–1.62) and acute kidney injury (OR: 1.11; 95% CI: 1.00–1.22). Our validated model stratified patients into Low Risk and High Risk of 30-day readmissions (29% and 40%, respectively). BackgroundRabies remains endemic to the Harare Metropolitan Province of Zimbabwe, with a lack of public participations potentially contributing to the limited success in eliminating the disease. We hypothesized that rabies intervention campaigns were less successful than they could be as a result of poor understanding of the disease at the community level, and thus aimed to identify the knowledge, attitudes, and practices towards rabies in the province.MethodsA cross-sectional survey, using a semi-structured questionnaire, was implemented between January 2017 and June 2018 across the province and data were collected from 798 respondents. Frequency distributions and logistic regressions were undertaken to determine the factors associated with the adequacy of the prevailing rabies knowledge, pet ownership characteristics and the existing preventative practices.ResultsThe results of our study suggested that the majority of the respondents (92%) had heard of rabies. However, the level of rabies knowledge could be classified as "adequate" in only 36% of respondents. The multivariate logistic regression analysis indicated that pet ownership and type of occupation were statistically associated with a better understanding and knowledge of the disease. Off all the respondents, 49% owned at least one dog or cat and suburb density and occupation were statistically associated with owning a pet. Amongst the pet owners, 57% consulted an animal health practitioner at least once a year and 75% were aware of a rabies vaccine for their pets. The multivariate logistic regression analysis indicated that age, education and gender were statistically associated with pet owners taking their pet(s) to an animal clinic.ConclusionThis study showed that the majority of the respondents lacked comprehensive knowledge about rabies, with the knowledge pertaining to health seeking behaviour and the importance of rabies vaccination being the most lacking. Additional public education relying on key messages, aimed at the different target audiences, is required in the province.

Obesity is a major global health challenge and a risk factor for the leading causes of death, including heart disease, stroke, diabetes, and several types of cancer. Attempts to manage and regulate obesity have led to the implementation of various dietary regulatory initiatives to provide information on the calorie contents of meals. Although knowledge of the calorie content is useful for meal planning, it is not sufficient as other factors, including health status (diabetes, hypertension, etc.) and level of physical activity, are essential in the decision process for obesity management. In this work, we present an artificial intelligence- (AI-) based application that is driven by a genetic algorithm (GA) as a potential tool for tracking a user’s energy balance and predicting possible calorie intake required to meet daily calorie needs for obesity management. The algorithm takes the users’ input information on desired foods which are selected from a database and extracted records of users on cholesterol level, diabetes status, and level of physical activity, to predict possible meals required to meet the users need. The micro- and macronutrients of food content are used for the computation and prediction of the potential foods required to meet the daily calorie needs. The functionality and performance of the model were tested using a sample of 30 volunteers from the University of Ghana. Results revealed that the model was able to predict both glycemic and non-glycemic foods based on the condition of the user as well as the macro- and micronutrients requirements. Moreover, the system is able to adequately track the progress of the user’s weight loss over time, daily nutritional needs, daily calorie intake, and predictions of meals that must be taken to avoid compromising their health. The proposed system can serve as a useful resource for individuals, dieticians, and other health management personnel for managing obesity, patients, and for training students in fields of dietetics and consumer science.

Introduction. Cystoisospora belli (previously Isospora belli) is a parasitic protozoan of the human gastrointestinal system. It rarely causes symptoms in immunocompetent hosts but can cause severe diarrhea in immunocompromised patients, with a rate of recurrence and risk of dissemination. Gallbladder infections are however rare. The treatment of choice for symptomatic patients is a 7–10-day course of trimethoprim-sulfamethoxazole. Case. In this case, we report on an incidental finding of Cystoisospora belli organisms in the donor gallbladder following a transplant cholecystectomy. There was no report of symptoms in the donor. The recipient was treated with a course of trimethoprim-sulfamethoxazole, without evidence of cystoisosporiasis. Given the risk of recurrence in immunocompromised hosts, the patient will continue to be monitored for reactivation in the future. Conclusion. Despite advances in transplant protocols and screening, disease transmission from the donor to recipient still occurs in about 0.2% of all organ transplants. With the increased use of organs from drug overdose victims and other high-risk donors, practitioners (including pathologists, hepatologists, and surgeons) must maintain a high index of suspicion for such potentially harmful organisms.

A retrospective study of clinical bovine dermatophilosis outbreaks and cases for the period 1995–2014 was conducted, using data obtained from the Division of Veterinary Services (DVS). A total of 3856 outbreaks and 26 659 cases of dermatophilosis were reported countrywide during this period. The post rainy season accounted for 37.9% of the outbreaks followed by the rainy season (26.7%), cold dry season (22.1%) and the hot dry season (13.2%). A retrospective space–time scan statistic in SaTScanTM was used to detect clusters. From this study, it was evident that dermatophilosis was spreading from the north-west of Zimbabwe through the central to the north-east during the period 2010–2014. Five clusters were identified mainly in the central and north-western regions of Zimbabwe. The primary cluster was centred at Ungwe, Gokwe district in Midlands; the second, third, fourth and fifth likely clusters were centred at Bonga (Mashonaland Central), ARDA (Mashonaland West), Nsenga (Matabeleland North) and Zanda in Gokwe, respectively. The findings of this study suggest the continued spread of dermatophilosis across the country; as such the Department of Livestock and Veterinary Services are advised to develop measures aimed at managing this spread such as dipping, quarantine, movement control and raising farmer awareness.

There is a paucity of literature examining recipient-donor obesity matching on liver transplantation outcomes. The United Network for Organ Sharing database was queried for first-time recipients of liver transplant whose age was ≥18 between January 2003 and September 2013. Outcomes including patient and graft survival at 30 days, 1 year, and 5 years and overall, liver retransplantation, and length of stay were compared between nonobese recipients receiving a graft from nonobese donors and obese recipient-obese donor, obese recipient-nonobese donor, and nonobese recipient-obese donor pairs. 51,556 LT recipients were identified, including 34,217 (66%) nonobese and 17,339 (34%) obese recipients. The proportions of patients receiving an allograft from an obese donor were 24% and 29%, respectively, among nonobese and obese recipients.
