From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions. © Copyright.

Contains fulltext : 206133.pdf (publisher's version ) (Open Access), Objectives: The aims of the present study were to assess the association and interactions of physical workload and poor health with health-related job loss (HRJL) among older workers, and the association and interactions of occupational social class and poor health with HRJL. Methods: Data were used from an existing prospective cohort study, Health and Employment after Fifty, where employed or self-employed workers aged 50–64 years (n=4909) were followed-up between 2014 and 2016. Associations between potential determinants (self-perceived health status, physical workload and occupational social class) and 2-year HRJL were examined by Cox regression analyses. To study whether physical workload or occupational social class moderates the influence of poor health on HRJL, additive and multiplicative interactions were calculated. Results: Older workers with poor self-perceived health status had increased risk of HRJL during the 2-year follow-up period (men: HR 2.57 (95%CI: 1.68 to 3.92); women: HR 3.26 (95%CI: 2.33 to 4.55)). Furthermore, men with high physical workload were at increased risk for HRJL (HR 1.63 (95%CI: 1.09 to 2.43)). No significant interactions (p<0.05) were identified between poor health and high physical workload, nor between poor health and lower occupational social class. Conclusion: Our study indicates that older workers in poor health, and older workers with a physically demanding job, are at increased risk of HRJL. Having a physically demanding job or working in routine/manual occupations does not moderate the association between poor health and HRJL.

23. Identification of risk factors for hospital admission using multiple-failure survival models: a toolkit for researchers

- Abstract
Background: The UK population is ageing; improved understanding of risk factors for hospital admission is required. Linkage of the Hertfordshire Cohort Study (HCS) with Hospital Episode Statistics (HES) data has created a multiple-failure survival dataset detailing the characteristics of 2,997 individuals at baseline (1998-2004, average age 66 years) and their hospital admissions (regarded as ‘failure events’) over a 10 year follow-up. Analysis of risk factors using logistic regression or time to first event Cox modelling wastes information as an individual’s admissions after their first are disregarded. Sophisticated analysis techniques are established to examine risk factors for admission in such datasets but are not commonly implemented.Methods: We review analysis techniques for multiple-failure survival datasets (logistic regression; time to first event Cox modelling; and the Andersen and Gill [AG] and Prentice, Williams and Peterson Total Time [PWP-TT] multiple-failure models), outline their implementation in Stata, and compare their results in an analysis of housing tenure (a marker of socioeconomic position) as a risk factor for different types of hospital admission (any; emergency; elective; >7 days). The AG and PWP-TT models include full admissions histories in the analysis of risk factors for admission and account for within-subject correlation of failure times. The PWP-TT model is also stratified on the number of previous failure events, allowing an individual’s baseline risk of admission to increase with their number of previous admissions. Results: All models yielded broadly similar results: not owner-occupying one’s home was associated with increased risk of hospital admission. Estimated effect sizes were smaller from the PWP-TT model in comparison with other models owing to it having accounted for an increase in risk of admission with number of previous admissions. For example, hazard ratios [HR] from time to first event Cox models were 1.67(95%CI: 1.36,2.04) and 1.63(95%CI:1.36,1.95) for not owner-occupying one’s home in relation to risk of emergency admission or death among women and men respectively; corresponding HRs from the PWP-TT model were 1.34(95%CI:1.15,1.56) for women and 1.23(95%CI:1.07,1.41) for men. Conclusion: The PWP-TT model may be implemented using routine statistical software and is recommended for the analysis of multiple-failure survival datasets which detail repeated hospital admissions among older people.

24. Global variation in grip strength: a systematic review and meta-analysis of normative data

Background: weak grip strength is a key component of sarcopenia and is associated with subsequent disability and mortality. We have recently established life course normative data for grip strength in Great Britain, but it is unclear whether the cut points we derived for weak grip strength are suitable for use in other settings. Our objective was to investigate differences in grip strength by world region using our data as a reference standard.Methods: we searched MEDLINE and EMBASE for reporting age- and gender-stratified normative data for grip strength. We extracted each item of normative data and converted it on to a Z-score scale relative to our British centiles. We performed meta-regression to pool the Z-scores and compare them by world region.Findings: our search returned 806 abstracts. Sixty papers met inclusion criteria and reported on 63 different samples. Seven UN regions were represented, although most samples (n = 44) were based in developed regions. We extracted 726 normative data items relating to 96,537 grip strength observations. Normative data from developed regions were broadly similar to our British centiles, with a pooled Z-score 0.12 SDs (95% CI: 0.07, 0.17) above the corresponding British centiles. By comparison, normative data from developing regions were clearly lower, with a pooled Z-score of −0.85 SDs (95% CI: −0.94, −0.76).Interpretation: our findings support the use of our British grip strength centiles and their associated cut points in consensus definitions for sarcopenia and frailty across developed regions, but highlight the need for different cut points in developing regions.Our search returned 806 abstracts. 26. Social inequalities in musculoskeletal ageing among community dwelling older men and women in the United Kingdom

The population of the United Kingdom (UK) is ageing; the already substantial burden of musculoskeletal disorders on health and social care systems will increase over time as the population ages. Social inequalities in health are well documented for the UK in general but little is known about social inequalities in musculoskeletal ageing. Using data from the 3,225 ‘young-old’ (age 59 to 73 years) community dwelling men and women who participated in the Hertfordshire Cohort Study, this thesis has explored social inequalities in musculoskeletal ageing: specifically, loss of muscle strength and physical function (PF); falls; Fried frailty; and osteoporosis. Socioeconomic position was characterised by age left full-time education, parental social class at birth and own social class in adulthood, and current material deprivation by housing tenure and car availability. Not owning one’s home was associated with lower grip strength and increased frailty prevalence among men and women and with poorer self-reported short-form 36 (SF-36) PF among men. Reduced car availability was associated with lower grip strength and poorer SF-36 PF among men and women and with increased falls and frailty prevalence among men. There was no convincing evidence for social inequalities in fracture, dual-energy x-ray absorptiometry (DXA) total femoral bone mineral density (BMD) and bone loss rate, or peripheral quantitative computed tomography (pQCT) strength strain indices for the radius or tibia. This thesis has argued that social variations in height, fat mass, diet and physical activity are likely to have mediated these results. Moreover, evidence for a social gradient in grip strength but not BMD is consistent with ageing skeletal muscle remaining highly responsive to physical activity in later life in a way that ageing bone does not; the impact of lifecourse customary and occupational physical activity on social inequalities in musculoskeletal ageing merits further research. The results presented in this thesis suggest that any clinical interventions designed to reduce the loss of muscle mass and function with age should be targeted proportionately across the social gradient; strategies to reduce fracture and osteoporosis should continue to have a universal population focus. Finally, this thesis suggests that there exists a subgroup of older men and women in the UK who face the multiple jeopardy of increased levels of material deprivation combined with greater loss of muscle strength and physical function; these men and women urgently need the government to commit to reform of the funding system for adult care and support.

Objectives: To evaluate the acceptability of grip strength measurement among older people in different healthcare settings. Design: A cross-sectional study with quantitative and qualitative data collection. Setting: Four healthcare settings in one town in southern England. Participants: 101 community hospital rehabilitation inpatients, 47 community physiotherapy referrals, 57 patients attending a Parkinson’s clinic at the hospital and 100 residents in care homes. Measurements: Grip strength, Barthel score, Mini Mental State Examination and outline questions on the grip measurement process were assessed on all participants. In-depth semi-structured interviews ascertained the views of a sub-sample of 20 participants on grip strength measurement. Results: The instructions were easily understood, most participants did not find the measurement painful or tiring, and almost all were prepared to repeat the assessment. Participants felt that this could be a useful and acceptable routine assessment, which some thought could be an opportunity to improve their health, while others were uncertain whether it would be helpful to be told that they were becoming weaker. Participants were generally accepting of medical assessments and felt that grip measurement was easy, unless there was a problem with an individual’s hand. Conclusions: This is the first study to demonstrate that grip strength measurement is acceptable to older people undergoing rehabilitation, living with a chronic neurological condition or resident in care homes. The high level of acceptability found among older people in different healthcare settings in this study supports the use of grip strength measurement in routine clinical practice.

29. Social inequalities in musculoskeletal ageing among community dwelling older men and women in the United Kingdom

- Abstract
The population of the United Kingdom (UK) is ageing; the already substantial burden of musculoskeletal disorders on health and social care systems will increase over time as the population ages. Social inequalities in health are well documented for the UK in general but little is known about social inequalities in musculoskeletal ageing. Using data from the 3,225 ‘young-old’ (age 59 to 73 years) community dwelling men and women who participated in the Hertfordshire Cohort Study, this thesis has explored social inequalities in musculoskeletal ageing: specifically, loss of muscle strength and physical function (PF); falls; Fried frailty; and osteoporosis. Socioeconomic position was characterised by age left full-time education, parental social class at birth and own social class in adulthood, and current material deprivation by housing tenure and car availability. Not owning one’s home was associated with lower grip strength and increased frailty prevalence among men and women and with poorer self-reported short-form 36 (SF-36) PF among men. Reduced car availability was associated with lower grip strength and poorer SF-36 PF among men and women and with increased falls and frailty prevalence among men. There was no convincing evidence for social inequalities in fracture, dual-energy x-ray absorptiometry (DXA) total femoral bone mineral density (BMD) and bone loss rate, or peripheral quantitative computed tomography (pQCT) strength strain indices for the radius or tibia. This thesis has argued that social variations in height, fat mass, diet and physical activity are likely to have mediated these results. Moreover, evidence for a social gradient in grip strength but not BMD is consistent with ageing skeletal muscle remaining highly responsive to physical activity in later life in a way that ageing bone does not; the impact of lifecourse customary and occupational physical activity on social inequalities in musculoskeletal ageing merits further research. Aims/hypothesis. Previous studies have suggested that the high hone density often observed in type 2 diabetic patients may he explained by insulin resistance. We explored this hypothesis in the Herfordshire Cohort Study. Methods. A total of 465 men and 444 women aged 59 to 71 years and with no prior diagnosis of diabetes at- tended a clinic where a glucose tolerance test was performed and hone density measured at the femoral neck and lumbar spine. Biochemical markers of hone turnover (serum osteocalcin and urinary mean c-terminal cross-linking telopeptide of type II collagen) were measured in 163 men. Results. According to WHO criteria, 83 men and 134 women were diagnosed with impaired glucose tolerance and a further 33 men and 32 women were diagnosed as having type 2 diabetes. Bone density was higher in newly diagnosed diabetic subjects, with relationships stronger in women (p<0.01) than men (p<0.05) and attenuated by adjustment for body mass index. In both sexes, we observed positive correlations between the total femur and femoral neck bone mineral density with measures of insulin resistance (r=0.17-0.22), with stronger results observed in women. These relationships did not apply after adjustment for body mass index. Glucose status did not lead to differences in osteocalcin level or c-terminal cross-linking telopeptide of type II collagen levels. Conclusions/interpretation. Small body size at birth and during infancy is associated with an increased risk of adult osteoporosis and cardiovascular disease. Fetal programming of the growth hormone–insulin-like growth factor (GH-IGF) axis may provide a mechanism for these epidemiological findings. To determine whether measurements of GH and IGF-I in late middle age were related to size at birth and in infancy. Overnight urinary GH excretion and fasting serum IGF-I were measured in 309 men and 193 women from Hertfordshire (born 1920–1930) for whom birthweight and weight at 1 year were recorded. Serum IGF-I was measured in men and women from Preston ( n = 254, born 1935–1943) and Sheffield ( n = 215, born 1939–1940) whose birthweight and other birth measurements were recorded. Urinary GH and serum IGF-I were not related to birthweight, other measurements at birth, or weight at 1 year. In contrast to previous studies in children or young adults, these data do not support the hypothesis that IGF-I concentrations are programmed by intra-uterine events, as assessed by birthweight, in late middle age. BACKGROUND: Comparable global data on health and nutrition of school-aged children and adolescents are scarce. We aimed to estimate age trajectories and time trends in mean height and mean body-mass index (BMI), which measures weight gain beyond what is expected from height gain, for school-aged children and adolescents., METHODS: For this pooled analysis, we used a database of cardiometabolic risk factors collated by the Non-Communicable Disease Risk Factor Collaboration. We applied a Bayesian hierarchical model to estimate trends from 1985 to 2019 in mean height and mean BMI in 1-year age groups for ages 5–19 years. The model allowed for non-linear changes over time in mean height and mean BMI and for non-linear changes with age of children and adolescents, including periods of rapid growth during adolescence., FINDINGS: We pooled data from 2181 population-based studies, with measurements of height and weight in 65 million participants in 200 countries and territories. In 2019, we estimated a difference of 20 cm or higher in mean height of 19-year-old adolescents between countries with the tallest populations (the Netherlands, Montenegro, Estonia, and Bosnia and Herzegovina for boys; and the Netherlands, Montenegro, Denmark, and Iceland for girls) and those with the shortest populations (Timor-Leste, Laos, Solomon Islands, and Papua New Guinea for boys; and Guatemala, Bangladesh, Nepal, and Timor-Leste for girls). In the same year, the difference between the highest mean BMI (in Pacific island countries, Kuwait, Bahrain, The Bahamas, Chile, the USA, and New Zealand for both boys and girls and in South Africa for girls) and lowest mean BMI (in India, Bangladesh, Timor-Leste, Ethiopia, and Chad for boys and girls; and in Japan and Romania for girls) was approximately 9–10 kg/m2. In some countries, children aged 5 years started with healthier height or BMI than the global median and, in some cases, as healthy as the best performing countries, but they became progressively less healthy compared with their comparators as they grew older by not growing as tall (eg, boys in Austria and Barbados, and girls in Belgium and Puerto Rico) or gaining too much weight for their height (eg, girls and boys in Kuwait, Bahrain, Fiji, Jamaica, and Mexico; and girls in South Africa and New Zealand). In other countries, growing children overtook the height of their comparators (eg, Latvia, Czech Republic, Morocco, and Iran) or curbed their weight gain (eg, Italy, France, and Croatia) in late childhood and adolescence. When changes in both height and BMI were considered, girls in South Korea, Vietnam, Saudi Arabia, Turkey, and some central Asian countries (eg, Armenia and Azerbaijan), and boys in central and western Europe (eg, Portugal, Denmark, Poland, and Montenegro) had the healthiest changes in anthropometric status over the past 3·5 decades because, compared with children and adolescents in other countries, they had a much larger gain in height than they did in BMI. The unhealthiest changes—gaining too little height, too much weight for their height compared with children in other countries, or both—occurred in many countries in sub-Saharan Africa, New Zealand, and the USA for boys and girls; in Malaysia and some Pacific island nations for boys; and in Mexico for girls., INTERPRETATION: The height and BMI trajectories over age and time of school-aged children and adolescents are highly variable across countries, which indicates heterogeneous nutritional quality and lifelong health advantages and risks., Wellcome Trust, AstraZeneca Young Health Programme, EU., peer-reviewed
