Background: The use of myocardial cytoprotectors (meldonium) in patients with exertional angina is a scientific-practical dilemma. Material and methods: An open randomized clinical trial was conducted involving 160 patients with chronic heart faliure (117 men and 43 women) aged 37 to 81 years. Of them, 142 patients had angina pectoris of stable effort from different functional classes, and 21 – unstable angina pectoris. Study groups were comparable according to the frequency of indication of background drugs and meldonium. Results: The number of patients with normal diastolic function in both groups, but with a net superiority to meldonium combination administration, has considerably increased: 41 patients (91.11%) in group II vs 33 (58.93%) in the group treated with basic treatment after 9 months of medication; 43 patients (95.56%) group II vs 36 (64.29%) in group I at 12 months of medication. During this period, no patients with pseudonormal type of diastolic dysfunction were registered, these passing into a ”more” favorable category – delayed relaxation. Conclusions: The data obtained confirmed the benefit of using cardiocitoprotection in inducing the reverse-remodelling of the myocardium of left ventricle regardless of the initial ventricular geometric pattern, but the administration of mildronate combination demonstrated a significantly superior efficiency to the basic treatment in hypertrophy of left ventricle regression, an event notable towards the end of the research period.

Background: The aim of the study was to breakdown the relationship and its nature between the two pathological entities arterial hypertension (AHT) and SARS-CoV-2 infection. The analysis of existing literature on both topics has been conducted. Sources including journals, books, existing publications and online platforms like Google Academic, PubMed, HINARI were used as search engines. Additionally, guidelines and circulars from European Society of Cardiology, American Heart Association and other respective bodies were also referred to. Data from large meta-analyses and clinical studies were included to bring out the relationship study. Pathophysiological breakdown of the two entities, contradicting proposals regarding treatment, new treatment modalities and de novo onset of AHT in post-Covid infection were included to delineate the relationship between the two pathological entities. Conclusions: Findings emphasize that the role of arterial hypertension in SARS-CoV-2 infection is mediated through its effect on the regulation of Renin Angiotensin Aldosteron System (RAAS), inflammation and immune responses. De-novo arterial hypertension was also reported in post SARS-CoV-2 infection patients. Though some initial studies hypothesized that RAAS inhibitors may add to clinical adversities, most studies afterward disproved the same and in fact revealed a protective role of the same. Angiotensin Converting Enzyme 2 (ACE2) is proposed as a treatment option in SARS-CoV-2 infection.

Background: Dyslipidaemia and obesity are recognized modifiable cardiovascular risk factors, a major health challenge of the 21st century. Youth obesity and lipid abnormalities are insufficiently documented. Asymptomatic young people tend not to appeal to health services, but they may be at high cardiovascular risk. În pofida progreselor semnificative în promovarea sănătății cardiovasculare, în Republica Moldova, bolilor cardiovasculare li se atribuie 59% în structura mortalității generale. Dislipidemia și obezitatea sunt recunoscute ca factori de risc cardiovascular modificabili, astfel impactul impetuos al acestora poate fi influențat. Delimitarea timpurie a grupurilor de pacienți cu risc cardiovascular sporit va optimiza diagnosticul la etapele presimptomatice, ceea ce conferă importanță pentru strategiile individualizate de profilaxie în rândul tinerilor. Scopul. Estimarea ratei obezității și a dislipidemiei în lotul femeilor tinere din Republica Moldova. Materiale și metode. Studiul transversal a fost realizat cu participarea a 302 femei tinere (17-29 ani), aparent sănătoase, înmatriculate la Universitatea de Stat de Medicină și Farmacie „N. Testemițanu“ în 2011. S-au analizat următorii parametri biochimici și antropometrici: HDL colesterol, colesterol total, trigliceride, non-HDL colesterol, indicele masei corporale, circumferința abdominală. Rezultate. Rezultatele studiului au demonstrat că peste 47% (143 subiecți) din populația tânără studiată au prezentat cel puțin un parametru lipidic modificat (hipertrigliceridemie (23%), hipercolesterolemie (28,6%), concentrația serică de non-HDL colesterol mărită (23%) și reducerea concentrației serice a HDL colesterolului (57%)). Obezitatea de tip abdominal a fost documentată la 86 de participante de sex feminin (28,5%), iar 19,5% au fost supraponderale / obeze conform parametrului IMC (criterii OMS), dintre care 15 tinere au fost obeze. Concluzii. În rândul femeilor tinere din Republica Moldova s-a constatat o rată alarmantă a dislipidemie și a obezității asimptomatice. Constatările noastre susțin necesitatea aplicării precoce a unor strategii personalizate în acest grup de vârstă.

Diabetic cardiomyopathy (DCM) is a distinct condition with a pathogenesis that is independent from coronary artery disease and can lead to heart failure in diabetic patients. In some patients, diastolic dysfunction may progress to heart failure with reduced ejection fraction and results in systolic heart failure. Since DCM is highly prevalent in the asymptomatic diabetic patients, screening for its presence and the diagnosis of functional myocardial abnormalities (echocardiography and other cardiac imaging techniques) in diabetic populations can have a favorable impact on the progression to chronic heart failure.
