Nepageidaujamų įvykių ir neatitikčių pranešimas ir registravimas yra sveikatos priežiūros saugos kultūros ir kokybės pagrindas. Tyrimo tikslas - remiantis naujausiomis mokslinėmis publikacijomis, apžvelgti priežastis, galinčias trukdyti pranešti ir registruoti nepageidaujamus įvykius ir neatitiktis sveikatos priežiūroje. Tyrimo metodas. Tyrimui atlikti buvo taikyta sisteminė mokslinės literatūros apžvalga, remiantis PRISMA metodu. Mokslinės literatūros paieška buvo vykdoma naudojant PubMed, Google Scholar duomenų bazes. Pagal pasirinktus įtraukimo kriterijus ir paieškos žodžius išanalizuotos 5 mokslinės publikacijos. Rezultatai. Išanalizuotoje literatūroje pateikiamos galimos kliūtys: emocinės ir psichologinės priežastys; žinių trūkumas, techninės kliūtys. Išvados. 1. Atlikus naujausių mokslinių publikacijų analizę, išsiaiškinta, kad sveikatos priežiūros specialistai, susidūrę su nepageidaujamais įvykiais ir neatitiktimis, gali delsti apie juos pranešti ar registruoti. Pagrindinės priežastys: emocinės, psichologinės kliūtys, kai patiriamos neigiamos emocijos, baiminamasi drausminių priemonių, žinių trūkumas, techninės kliūtys, kai trūksta informacijos apie nepageidaujamų įvykių ir neatitikčių registravimą. 2. Norint sumažinti psichologinę ir emocinę baimę pranešti ir registruoti nepageidaujamus įvykius ir neatitiktis, svarbu užtikrinti registruojančių asmenų anonimiškumą, suteikti pakankamai informacijos apie registraciją, užtikrinti atgalinį ryšį ir tinkamai apibrėžti su pacientų sauga susijusias sąvokas: nepageidaujamas įvykis, neatitiktis, žala, medicininė klaida. The reporting and recording of adverse events and non-conformities is at the heart of safety culture and quality in healthcare. The aim of this study is to review, on the basis of recent scientific publications, possible reasons that may hinder the reporting and recording of adverse events and non-conformities in healthcare. The method of the study was a systematic review of the scientific literature based on the PRISMA method. The literature search was conducted using PubMed, Google scholar databases. 5 scientific publications were analysed according to the selected inclusion criteria and search terms. Results. The analysed literature presents possible barriers: emotional and psychological reasons; lack of knowledge; technical barriers. Conclusions. 1.The analysis of recent scientific publications has shown that healthcare professionals may delay reporting or recording adverse events and non-compliances. The main reasons for this are: emotional and psychological barriers, where negative emotions are experienced, fear of disciplinary measures and lack of knowledge, and technical barriers, where there is a lack of information about recording adverse events and discrepancies. 2. To reduce the psychological and emotional fear of reporting and recording adverse events and non-conformities, it is important to ensure the anonymity of the recording persons. It is also important to provide sufficient information about the registration, to ensure feedback and to properly define the patient safety related terms: adverse event, non-compliance, harm, medical error. Įvadas. Psichikos sveikatos raštingumas (PSR) – tai psichikos sveikatos būklių supratimas, padedantis jų prevencijai, atpažinimui ir gydymui. Dėl nepakankamo visuomenės išmanymo apie psichikos sutrikimus, jų atpažinimas, kreipimasis pagalbos ir gydymas vėluoja. Pavyzdžiui, JAV depresiją atpažįsta

1920 m. rudenį Lietuvos kariuomenėje buvo suformuoti keturi „skrajojantieji“, t. y. mobilūs, sanitarijos būriai. Jų sudėtyje buvo gydytojų, medicinos seserų ir sanitarų (felčerių). Būrių tikslas – nemokamai teikti medicinos pagalbą Vilniaus krašto gyventojams. Susirūpinimą kėlė regione plitusios infekcinės ligos, tačiau iš raportų galima susidaryti įspūdį, kad lygiagrečiai būta ir kitų tikslų. Siekta, teikiant medicinos pagalbą, išžvalgyti krašte padėtį, apklausti žmones ir susidaryti įspūdį, kokios nuotaikos (prolenkiškos, prolietuviškos ar prosovietinės / prorusiškos) krašte tvyrojo. Lygiagrečiai buvo siekiama formuoti kuo pozityvesnį vietos gyventojų požiūrį į Lietuvos valstybę kaip į galimą politinės ateities alternatyvą. Taigi mobilūs medikų būriai, vykdami į Rytų Lietuvos regioną, turėjo kompleksą užduočių: a) susipažinti su sanitarine ir epidemiologine padėtimi; b) nemokamai teikti medicinos paslaugas; c) rinkti informaciją apie padėtį regione, politines nuotaikas, tautinę sudėtį, požiūrį į Lietuvos valstybę; d) teikti materialinę pagalbą išskirtinai skurstantiems (tam kiekvienas būrys iš Lietuvos Raudonojo Kryžiaus buvo gavęs po 10 tūkst. auksinų), nes vargingai gyveno dauguma lankyto regiono gyventojų; e) formuoti kuo palankesnį požiūrį į Lietuvos valstybę. Platesnės apimties ir išsamesnių tyrimų, skirtų mirties reiškiniui Lietuvos kariuomenėje 1919–1940 m., kol kas nėra atlikta. Lietuvos istoriografijoje apsiribojama tarpukario (tarp 1926 ir 1937 m.) Lietuvos kariuomenės arba jos buvusių karininkų paskelbtais karo nuostolių skaičiais. Teiginys apie 1,4 tūkst. žuvusiųjų Lietuvos nepriklausomybės kare 1919–1920 m. istoriografijoje buvo virtęs savotiška aksioma. Atskirą tyrimą galima rengti apie tai, kaip plito 1937 m. Petro Rusecko paskelbtas aukų skaičius (su vardiniu sąrašu) skirtingų autorių darbuose (panašūs skaičiai tarpukariu buvo publikuojami ir kituose leidiniuose), nors istorikas Paulius Šležas dar tais pačiais 1937 m. P. Rusecko duomenis sukritikavo. Esminė minėtų duomenų spraga, kad juose į vieną sąrašą ir apibendrintą statistinį skaičių sujungti žuvę Lietuvos kariai nuo 1919 m. iki 1928 m., – aktyvūs karo veiksmai jau buvo pasibaigę de jure 1920 m. (remiantis Lietuvos kariuomenės įsakymu) arba de facto 1923 m., nors pavienių incidentų pasienyje su Lenkija pasitaikė visą tarpukarį. Kitas šios problemos aspektas, kad į žuvusiųjų sąrašą P. Ruseckas įtraukė ir tuos, kurių mirtys tiesiogiai su karo veiksmais nebuvo susijusios. Tarp karo aukų yra savižudžių, skenduolių, nelaimingų atsitikimų aukų. Be to, į aukų sąrašus nepateko dalis mirusiųjų, kadangi sąrašas buvo sudaromas pirmiausia remiantis palaidojimais, nes karo medicinos tarnyba mirčių neregistravo, tik vedė sužeistųjų ir susirgusiųjų statistiką. Į apskaitą nepateko ir dalis tų, kurie buvo išvežti palaidoti į šeimos kapavietes, ir kitais atvejais. Šis tyrimas siekia aptarti karių mirčių, tiesiogiai nesusijusių su kariniais veiksmais, priežastis Lietuvos kariuomenėje tarpukariu. Įgyvendinant tai, iš karių mirčių statistikos pirmiausia reikėjo išskirti tuos, kurie mirė taikos metu. Toliau bandyta sugrupuoti mirčių priežastis pagal kategorijas. Galiausiai, remiantis vieno kariuomenės dalinio, 7-ojo pėstininkų pulko, pavyzdžiu, straipsnyje atskleidžiamas atskirų mirčių priežasčių kategorijų dažnumas ir pasiskirstymas pagal metus. Vykdant tyrimą, duomenys apie mirusius karius buvo imami iš penkių šaltinių: 1) iš duomenų apie kariuomenės netektis – duomenis 1937 m. paskelbė P. Ruseckas; 2) iš karių kapų registro, kuris paskelbtas 2003 m. atlase; 3) iš karo aukų sąrašo, kuris iškaltas Vytauto Didžiojo karo muziejaus kriptoje esančiose lentose; 4) iš Lietuvos centriniame valstybės archyve saugomų dokumentų; 5) iš kariams skirto savaitinio žurnalo Karys, kurį Lietuvos kariuomenė leido 1920–1940 m. Pašalinus pasikartojančius atvejus, suvesti duomenys leido sudaryti maksimaliai išsamų mirusių karių sąrašą, apimantį beveik 3,5 tūkst. Lietuvos karių mirčių atvejų. Sąrašas nėra baigtinis, nes pavieniai mirčių atvejai fiksuojami ir toliau. Dėl šios priežasties straipsnyje susitelkta ne į kiekybinių duomenų pristatymą, o į bandymą identifikuoti ir sugrupuoti mirčių priežastis taikos metu. Šias priežastis ne tik bandyta apibendrinti visos Lietuvos kariuomenės mastu. Vieno kariuomenės dalinio pavyzdžiu straipsnyje pateikiama ir kiekybinė mirties priežasčių charakteristika. 7-asis pėstininkų Žemaičių kunigaikščio Butegeidžio pulkas tam pasirinktas dėl kelių priežasčių. Pirmoji, tai faktas, kad šis pulkas egzistavo nuo 1919 m. iki pat 1940 m., t. y. jis nebuvo išformuotas 1923 m. ar 1926 m., o vėliau (1934 m.) atkurtas, kaip kai kurie kiti kariuomenės pulkai. Tai leidžia disponuoti duomenimis apie visą laikotarpį. Antroji priežastis – šis pulkas karo metu aktyviai dalyvavo kovos veiksmuose, skirtingai nei dalis kitų Lietuvos kariuomenės vienetų, kurie Nepriklausomybės karo metais buvo rezerve arba saugojo nepagrindinius fronto ruožus. Tai leidžia aiškiau išskirti taikos meto aukas iš bendro aukų skaičiaus. Galiausiai trečioji, ne mažiau svarbi, priežastis yra nuosekliai ir sistemingai pulke vestas mirusių karių sąrašas, kuris išliko ir šiuo metu saugomas archyve. Toks sąrašas leido analizuoti išsamius duomenis apie šio pulko mirusius karius. Tyrimo metu surinkti ir susisteminti duomenys leidžia teigti, kad, be karinių veiksmų, karių mirtis Lietuvos kariuomenėje tarpukariu lėmė virtinė kitų priežasčių. Straipsnyje pateiktas mirties priežasčių pristatymas ir vieno pulko atvejo tyrimas rodo, kad reikšmingą dalį mirties priežasčių sudarė įvairios ligos. Tarp jų išsiskiria vidurių šiltinė, gripas ir plaučių uždegimas. Rečiau identifikuoti tokie fataliniai susirgimai kaip tuberkuliozė, rožė, apendicitas, leukemija, meningitas, paratifas ir kt. Pulko atvejo analizė rodo, kad tarp jaunų vyrų dažniausia medicinine mirties priežastimi tapdavo infekcinės ligos, ypač vidurių šiltinė ir plaučių uždegimas. Tokioms ligoms plisti palankią terpę sudarydavo tarnybos sąlygos ir higiena, ypač gyvenamųjų patalpų būklė. Infekcinės ligos plito ne vien dėl ankštos kasdienės karių aplinkos, bet dėl naudojimosi tais pačiais vandens šaltiniais ir kita infrastruktūra, kuria miestuose su įgulomis naudojosi ir civiliai gyventojai. Tą ypač aiškiai parodo Tauragės atvejis: ligos plitimą įguloje ten lėmė miesto infrastruktūra, o didelį mirtingumą – kareivinių uždarumas. Between the First World War and the Second World War, approximately 280 thousand men served in the Lithuanian armed forces. This is a significant figure for a country that only had a population of two to 2.5 million. Even though the Lithuanian armed forces were only involved in active military operations and low-intensity fighting from 1919 to 1923, servicemen died during the entire period up to 1940. The numbers of deaths during the Wars of Independence are well known; however, the causes of death in the Lithuanian armed forces, both from 1919 to 1920 and in later years, have not been investigated in great depth. We understand that one cause of death in an active army is active combat. In the study of war, deaths in action are further classified into deaths caused by artillery fire, machine gun and rifle fire, bayonets, etc. This allows scholars to determine the effectiveness of weapons systems and tactical elements used on the battlefield. However, the focus of this article is the causes of soldiers’ deaths that are not directly combat related. In the first section, I discuss causes of death in the Lithuanian armed forces during periods of war and peace, and provide a host of examples, which include deaths caused by disease, accidents, homicide and suicide. In the last section, I present the results of quantitative analysis. The quantitative analysis is a case study of a single regiment that demonstrates the predominant causes of death from 1919 to 1940, with a separate analysis of causes of death for the period 1919 to 1920. Darbuotojų pasitenkinimui darbo aplinka bei darbo rezultatams įtakos turi mikroklimatinės darbo aplinkos sąlygos. PSO duomenimis, 30 proc. visų pastatų turi vidaus patalpų kokybės problemų. ES valstybėse 14 proc., Lietuvoje 17 proc. darbuotojų yra nepatenkinti darbo aplinkos mikroklimato parametrais. Siekiant užtikrinti darbuotojų saugą ir sveikatą darbe, atliekami objektyvūs patalpų mikroklimato parametrų matavimai bei išklausoma darbuotojų nuomonė, nustatomos probleminės darbo zonos, padedančios sukurti veiksmingą prevencijos priemonių planą, kaip išvengti arba sumažinti veikiančių veiksnių įtaką darbuotojų sveikatai. Šiame tyrime buvo atliktas sveikatos priežiūros įstaigos darbo vietų rizikos vertinimas. Apklausos metu buvo siekiama išsiaiškinti darbuotojų nuomonę apie mikroklimatinių darbo sąlygų sąsajas su darbuotojų sveikata. Apklausoje dalyvavo X sveikatos priežiūros įstaigos visi darbuotojai, atsako dažnis 90,5 procento. Nustatyta, kad darbuotojai skundėsi sausu oru, gryno oro trūkumu, triukšminga ir per šilta darbo aplinka bei darbo aplinkos temperatūros svyravimu. Moterys dažniau nei vyrai jautė darbo aplinkos pokyčius, daugiau nusiskundimų darbo aplinka turėjo 40–60 m. amžiaus grupės respondentai, turintys 6–15 metų darbo stažą. Visą darbo dieną patalpose praleidžiantys darbuotojai dažniau skundėsi temperatūrų pokyčiais, gryno oro trūkumu, per šiltomis patalpomis, akių sausumu bei nuovargiu, galvos skausmu. Respondentai, nurodę jog jų patalpose yra kompiuterinė ir (ar) medicininė technika, dažniau skundėsi temperatūrų pokyčiais, elektros iškrovomis bei per šiltomis patalpomis. Nustatyta, kad darbuotojų sveikatos nusiskundimai (galvos skausmas, nuovargis, dėmesio koncentracijos stoka, akių sudirgimas, sausumas bei nuovargis, užsikimšusi nosis, sausa veido ir rankų oda) yra susiję su šiais darbo aplinkos veiksniais: per aukšta patalpų temperatūra ir jos svyravimai, sausas oras, gryno oro trūkumas, nepakankamas apšvietimas bei triukšminga aplinka. Employees’ satisfaction with the work environment and work results are influenced by the microclimatic conditions of the work environment. According to WHO, 30% of all buildings have indoor quality problems, 14% in EU countries, 17% in Lithuania employees are dissatisfied with the microclimate parameters of the work environment. In order to ensure the safety and health of employees at work, objective measurements of indoor microclimate parameters are carried out and the opinions of employees are listened to, problematic work areas are identified, which help to create an effective plan of prevention measures to avoid or reduce the influence of operating factors on the health of employees. In this study, a workplace risk assessment was carried out in a healthcare facility, and during the survey, the aim was to find out the opinion of employees about the links between microclimatic working conditions and the health of employees. All employees of health care facility X participated in the survey, the response rate was 90.5 %. It was found that the workers complained about dry weather, lack of fresh air, noisy and too warm working environment, and temperature fluctuation of the working environment. Women felt changes in the work environment more often than men, the 40–60 years old had more complaints about the work environment, respondents belonging to the age group with 6–15 years of work experience. Employees who spend the full working day indoors complained more often about temperature changes, lack of fresh air, too warm rooms, dry eyes and fatigue, headaches. Respondents who stated that their premises have computer and/or medical equipment more often complained about temperature changes, electric discharges and too warm premises. It has been established that the following factors of the working environment: too high temperature and its fluctuations, dry air, lack of fresh air, insufficient lighting and noisy environment, are related to the health complaints of workers (headache, fatigue, lack of concentration, eye irritation, dryness and fatigue, stuffy nose, dry skin of the face and hands). Straipsnyje tiriama ir analizuojama Klaipėdos miesto pirminės sveikatos priežiūros paslaugų kokybė. Tyrimo tikslas – išanalizavus sveikatos priežiūros paslaugų kokybės teorinius vertinimo metodus, įvertinti Klaipėdos miesto pirminės asmens sveikatos priežiūros teikiamų paslaugų kokybę paslaugų gavėjų ir jų teikėjų aspektais. Siekiant šio tikslo, analizuota mokslinė literatūra, atlikta anketinė apklausa, duomenys apdoroti taikant statistinius metodus, įvertinti rezultatai, suformuoti apibendrinimai. Kokybiška sveikatos priežiūros paslauga – tai visapusiškas paciento sveikatos poreikių tenkinimas, atsižvelgiant į jo lūkesčius ir prioritetus. Atlikus Klaipėdos miesto pirminės asmens sveikatos priežiūros paslaugų kokybės vertinimą ir išanalizavus gautus rezultatus, nustatyta, kad Klaipėdos sveikatos priežiūros įstaigų teikiamų paslaugų kokybė vertinama 8,47 balo iš 10. Šios paslaugos tenkina tiek paslaugų gavėjų, tiek jų teikėjų poreikius. Tyrimas leido nustatyti ir esminius trūkumus. Paslaugos gavėjai prasčiausiai įvertino vizito pas gydytoją laukimo laiką, konsultacijos laiko trukmę, personalo pasiekiamumą, nuotolinių paslaugų teikimą. Gydytojai nepatenkinti konsultacijos laiko trukme, per dideliu darbo krūviu. Jų manymu, turėtų būti daugiau šeimos gydytojų, didesnė slaugytojų kompetencija. The article investigates and analyses the quality of Klaipėda’s primary health-care services. The study aims to evaluate the quality of services provided by Klaipėda City Primary Health-Care Services based on the attitudes of service beneficiaries and service providers after analysing the theoretical methods of the quality of health-care services. The article analyses scientific literature, comparative analysis, generalisation, survey, descriptive statistical methods (data comparison, grouping, graphic portrayal), and secondary statistics (analysis of patients signed in medical institutions, visits, and preventive state-funded programmes). The following methods were used for data analysis: cross-analysis (x2), Spearman correlation, and non-parameter Mann-Whitney (Z) test. Cronbach’s alpha was estimated to assess the reliability of scales. Health care aims to meet patients’ health needs, manage health-care processes and structures, and apply the latest quality management principles and methods. A quality health-care service creates a value that satisfies both the supplier and the customer, and preserves customer loyalty. Models of general evaluation and ISO 9001 quality management assessment are most widely used to assess health-care services. With the systematisation of primary health-care quality theories and assessment models in health care, the theoretical evaluation model of primary health-care service quality was designed. The model is based on global and national health-care policies, strategic health documents, and theoretical assumptions and models of primary health-care quality dimensions. To ensure health-care service quality, it is recommended to assess: 1) patients’ perceived quality, 2) the quality of health-care services provided, and 3) management quality. These three directions of quality-ofservice improvements include leading health-care factors: availability, reliability, timeliness, consistency, tangibility, safety, justice, performance and patient orientation. In terms of recipients, health-care services are provided with good quality and were evaluated at 8.47 out of 10. The best assessment of health-care service quality was for safety requirements, discriminatory properties, patient respect and privacy, communication, personnel professionalism, and the institution’s environment. The worst assessment was for waiting times for the doctor’s visit, the duration of the consultation, the accessibility of the staff, and the provision of remote services. Service providers (doctors) evaluated the quality of health-care services even better, at 8.93 out of 10. According to the doctors, patients are not discriminated against, treatment is conducted safely, privacy is respected, and confidentiality is ensured. In terms of the institution, doctors evaluate well the cleanliness and the environment, the working hours, and the range of services; but are dissatisfied with the duration of the consultation, the excessive workload, and the number of patients, which affects the patient’s consultation and examination. They think health-care institutions should have a greater number of family doctors and more responsibilities for nurses. It also highlights the salary and the workload, and would like a better system of organisation. Comparing the results obtained from the patient and the physician, it was found that the quality of Klaipėda’s primary health-care services is good. Patients estimated safety best at 4.22 and availability worst at 3.93. Doctors estimated availability best at 4.35 and timeliness worst at 3.75. SummaryDuring various emergencies, especially pandemics, there is a heavy burden on healthcare workers and pharmacists. Organizational support plays a significant role in protecting their mental health. Though the study aimed analyze the subjectively perceived difficulties and challenges of healthcare workers related to organizing work in the context of a pandemic.MethodsTwenty seven subjects (20 women, 7 men) participated in the qualitative research 30–45 min. Duration semi-structured interviews were performed, and thematic data analysis was applied.ResultsDuring the first wave of the COVD-19 pandemic, research participants experienced an avalanche of change in all significant areas of life: experienced general overall uncertainty, confusion in working order, and intense changes in work functions, responsibilities, and workload. These changes reduced the scope for control and predictability, there was a lack of structure and clarity. The avalanche of change caused by the COVID-19 pandemic provoked a strong and controversial emotional response. The contradiction was revealed between helplessness, disruption, loss of control experienced by staff and the internal and external pressure to adapt as quickly as possible to the functions of caregivers. The threats posed by the pandemic reinforced the need for active and engaged leadership and highlighted the key features of an employee supporting organization.ConclusionSurviving the avalanche of change caused by the pandemic, healthcare workers and pharmacists emphasized the importance of management decisions about managing patient and employee health threats, clear work organization, active and inclusive leadership, change planning, and organizational concern for employee sustainability and emotional well-being. Regular, systematic, clear and understandable, timely, open and sincere, uncontroversial, and consistent communication of administration provides security for employees and can contribute to better physical and psychological well-being of employees.

Patient-centered care is considered to be one of the essential pillars of a modern healthcare system. Thus, quality assessment based on patients’ perceptions, views and experiences in their journey through the healthcare system is recognized as one of the key principles for quality improvement initiatives. Measuring patient satisfaction can be confounded by expectations and prior experiences, which can be at least partly overcome by evaluating patient-perceived healthcare quality (PPHQ). Understanding the principal constituents of PPHQ may aid healthcare professionals and decision makers in the healthcare management process and help in creating instruments to meaningfully measure patient feedback. Herein, we aimed to analyze the primary determinants of PPHQ and their interactions, with a focus on patient experiences and healthcare accessibility, using the example of Lithuanian primary healthcare. For this purpose, we conducted a cross-sectional representative telephone survey that included a total of 1033 respondents (48% male) who had encountered primary healthcare during last 3 years. Survey questions consisted of sociodemographic characteristics, patient perceptions of healthcare service provision, patient experiences, self-reported health status and overall PPHQ ranked with a 5-point Likert scale as the primary outcome. The classification-regression tree (CRT) technique was used to analyze the relationship between different explanatory variables and PPHQ, as well as their relative importance and interactions. The majority of respondents (89%) evaluated PPHQ as acceptable or good. CRT analysis identified staff behavior, organizational accessibility and financial accessibility as the most important factors affecting PPHQ. Importantly, the latter factors surpassed the effect of other known PPHQ determinants, such as sociodemographic characteristics or health status. Further analysis has revealed that the relative importance of staff behavior, including understanding, attention and empathy, increased when more problems with organizational accessibility were encountered. In conclusion, our study suggests that PPHQ in primary healthcare may primarily be determined by organizational and financial accessibility and staff behavior, which may also act as an important mediating factor. Background. Air pollution has a significant effect on human health and there is a broad body of evidence showing that exposure to air pollution is associated with an increased risk of adverse health effects. The main objective of this study was to assess the association of traffic-related air pollutants with fatal AMI during the ten-year period. Methods. The study was conducted in Kaunas city, where the WHO MONICA register included a total of 2273 adult cases of fatal AMI cases during the 10-year study period. We focused on the period between 2006 and 2015. The associations between exposure to traffic-related air pollution and the risk of fatal AMI were evaluated by using a multivariate Poisson regression model, RR presented per an increase in IQR. Results. It was found that the risk of fatal AMI was significantly higher in all subjects (RR 1.06; 95% CI 1.00–1.12) and women (RR 1.12; 95% CI 1.02–1.22) when the concentration of PM10 in the ambient air was increased 5–11 days before the onset of AMI, adjusting for NO2 concentration. The effect was stronger during spring in all subjects (RR 1.12; 95% CI 1.03–1.22), in men (RR 1.13; 95% CI 1.01–1.26), in younger-aged (RR 1.15; 95% CI 1.03–1.28), and in winter in women (RR 1.24; 95% CI 1.03–1.50). Conclusions. Our findings show that ambient air pollution increases the risk of fatal AMI, and this pertains to PM10 specifically. Healthcare workers (HCWs) often experience high levels of stress, anxiety, and depression due to high workloads and responsibilities in their professional activities. Therefore, recovery from work-related stress is highly important in HCWs. The Recovery Experience Questionnaire (REQ) is a 16-item self-reported measure covering four stress recovery domains: psychological detachment from work, relaxation, mastery, and control. The current study aimed to test the REQ’s psychometric properties in a sample of Lithuanian HCWs. In total, 471 HCWs from various healthcare institutions participated in this study. Confirmatory factor analysis (CFA) was used to test the structure of the REQ. We also used the Brief Patient Health Questionnaire (PHQ-4) and the World Health Organization Psychological Well-Being Index (WHO-5) to assess the mental health of the study participants. The CFA analysis supported the correlated four-factor structure of the REQ. Furthermore, we found significant correlations between the levels of REQ and anxiety, depression, and well-being. We conclude that the REQ is a valid measure that could be a useful tool in research on HCWs’ mental health. Kiekvienos organizacijos tikslas – darbuotoją motyvuoti ir skatinti iniciatyvą, užtikrinti geras darbo sąlygas, darbo priemones, gerą psichologinį mikroklimatą, orų atlyginimą ir kitus veiksnius, kurie sudarys sąlygas jaustis saugiai ir užtikrintai. Pasitenkinimas (nepasitenkinimas) darbu turi tiesioginę įtaką darbuotojų kaitai organizacijoje bei specialistų pasitraukimui iš profesijos. Tyrimo tikslas – nustatyti visuomenės sveikatos priežiūros specialistų, dirbančių bendrojo ugdymo įstaigose, pasitenkinimą darbu ir su juo susijusius veiksnius. Atliktas momentinis kiekybinis tyrimas, kurio metu anoniminės anketinės apklausos būdu apklausti bendrojo ugdymo įstaigose dirbantys visuomenės sveikatos specialistai. Remiantis 2022 metų visuomenės sveikatos priežiūros specialistų registro duomenimis, bendrojo ugdymo įstaigose dirbo 281 visuomenės sveikatos priežiūros specialistas. Tyrime dalyvavo 214 specialistų. Atsako dažnis – 76,2 procento. Tyrimo rezultatai atskleidė, kad 78,5 proc. apklausoje dalyvavusių sveikatos priežiūros specialistų darbas ugdymo įstaigoje patiko, 3,7 proc. apklaustųjų nurodė, kad darbas jiems nepatinka arba labai nepatinka. Pasitenkinimas darbu buvo reikšmingai susijęs su respondentų amžiumi. 28,5 proc. respondentų nurodė, kad jiems dirbti sunku arba labai sunku, kas penktas teigė, kad jam dirbti visai nesunku arba nesunku. 32,2 proc. apklaustųjų svarstė galimybę išeiti iš darbo. Tokią galimybę svarsčiusių apklaustųjų buvo statistiškai reikšmingai daugiau tarp jaunesnių ir trumpesnį darbo stažą turinčių visuomenės sveikatos specialistų. Daugiausia bendrojo ugdymo įstaigose dirbančių visuomenės sveikatos specialistų buvo nepatenkinti arba labai nepatenkinti gaunamu atlyginimu (31,8 proc.), karjeros galimybėmis (23,3 proc.), darbo krūviu (16,4 proc.). Daugiausia specialistų buvo patenkinti arba labai patenkinti aprūpinimu darbo priemonėmis, asmens apsaugos priemonėmis (80,8 proc.), galimybėmis kelti profesinę kvalifikaciją (80,4 proc.), santykiais su ugdymo įstaigos bendruomene (79,4 proc.). The goal of each organization is to motivate and encourage the employee to take the initiative, to ensure good working conditions, work equipment, a good psychological microclimate, decent pay and other factors that will make them feel safe and secure. Job satisfaction / dissatisfaction has a direct impact on employee turnover in the organization and the withdrawal of professionals from the profession. The aim of the study is to determine job satisfaction and related factors of public health care professionals working in general education institutions. A quantitative instantaneous survey was conducted, during which general education institutions public health specialists were interviewed by anonymous survey. Based on the 2022 data of the register of public health care specialists, 281 health care specialists worked in general education institutions. 214 specialists participated in the study. Response rate – 76.2 %. The results of the study revealed that 78.5 %of the health care professionals who participated in the survey really liked their job in the educational institution., 3.7 % respondents indicated that they did not like their job. Job satisfaction was significantly related to the age of the respondents. 28.5 percent of respondents indicated that it is difficult or very difficult for them to work, every fifth respondent said that it is not difficult or not difficult for them to work. 32.2 % of the respondents considered the possibility of leaving their job. There were statistically significantly more respondents who considered such a possibility among public health specialists who were younger and had a shorter working experience. The majority of public health specialists working in general education institutions were dissatisfied or very dissatisfied with the salary they received (31.8%), career opportunities (23.3%), workload (16.4%). The majority of specialists were satisfied or very satisfied with the provision work tools, personal protection tools (80.8%), opportunities to raise professional qualifications (80.4%), relations with the community of the educational institution (79.4%). COVID-19 pandemijos metu sutriko sveikatos apsaugos sektoriaus veikla, o atsiradę pokyčiai turėjo neigiamų padarinių vaikų sveikatos priežiūros paslaugų prieinamumui. Tyrimo tikslas – įvertinti COVID-19 pandemijos įtaką vaikų sveikatos priežiūros paslaugoms. Metodai. Remiantis Google Scholar ir Pubmed duomenų bazių šaltiniais, naudojant raktinius žodžius ir jų derinius atrinkta ir išanalizuota 14 mokslinių publikacijų pasirinkta tema. Išvados. COVID-19 infekcija ir jos valdymas turėjo neigiamų padarinių vaikų sveikatai. Pandemija sumažino sveikatos priežiūros paslaugų prieinamumą dėl ribotų žmogiškųjų ir finansinių išteklių paskirstymo bei pacientų baimės užsikrėsti COVID-19 infekcija gydymo įstaigose. COVID-19 infekcija lėmė kitų ligų pavėluotą diagnostiką ir gydymą, todėl padidėjo komplikacijų dažnis. Dėl pandemijos metu sutrikusios vaikų imunizacijos, tikėtina, kad ateityje vakcinomis kontroliuojamų ligų atvejų skaičius augs. The health care sector was disrupted during the COVID-19 pandemic, and the resulting changes had negative consequences for the availability of children’s health care services. The aim - to assess the impact of the COVID-19 pandemic on children’s healthcare services. Material and methods. Based on the sources of Google Scholar and Pubmed databases, the selected topic of 14 scientific publications was selected and analyzed according to the keywords and their combinations: COVID-19, pandemic, child, health care services, impact, changes. Conclusions. The infection and management of COVID-19 has had negative effects on children’s health. The pandemic has reduced the availability of healthcare services due to the limited allocation of human and financial resources and patients fear of contracting the COVID-19 infection in medical facilities. The infection of COVID-19 has led to delayed diagnosis and treatment of other diseases, and for these reasons the frequency of complications has increased. Moreover, due to the disruption of childhood immunization during the pandemic, it is likely that the number of cases of vaccine-controllable diseases will increase in the future. Tikslas − nustatyti tiriamųjų sveikatos raštingumo, fizinio aktyvumo ir streso lygį COVID-19 pandemijos metu bei ryšius tarp jų. Tyrimo metodai. Tyrime dalyvavo 123 respondentai (18 - 64 m. amžiaus). Tyrimas vyko nuo 2021 m. lapkričio mėn. iki 2022 m. vasario mėn. Parengta anketa, kurią sudarė Europos sveikatos raštingumo klausimyno sutrumpinta forma (HLS-EU-Q16), Šiaurietiška fizinio aktyvumo klausimyno sutrumpinta forma (NPAQ short form), Suvokiamo streso skalė (10 item Perceived Stress Scale) ir kiti klausimai. Tyrimo duomenų apdorojimas, analizė ir skaičiavimas buvo atliktas naudojant Microsoft Excel ir Rcommander programą. Buvo skaičiuojami gautų rezultatų vidurkiai, standartiniai nuokrypiai, medianos, mažiausios ir didžiausios reikšmės bei koreliaciniai ryšiai. Rezultatai. Apskaičiavus gautus tyrimo duomenis, probleminis sveikatos raštingumas nustatytas 69 proc. respondentų. Fizinio aktyvumo normų nesiekia 48 proc. respondentų. Sveikatos raštingumo rezultatų vidurkiai statistiškai reikšmingai skiriasi (p = 0,007) tarp fizinio aktyvumo grupių, tačiau raštingumo lygis išlieka probleminis abiejose grupėse. Apskaičiuoti koreliaciniai ryšiai parodė, kad yra silpna teigiama statistiškai reikšminga koreliacija (r = 0,283) tarp sveikatos raštingumo ir fizinio aktyvumo; silpnas neigiamas koreliacinis ryšys (r = -0,285) tarp sveikatos raštingumo ir suvokiamo streso bei silpnas neigiamas koreliacinis ryšys tarp fizinio aktyvumo ir suvokiamo streso (r = -0,277). Išvados. PSO fizinio aktyvumo rekomendacijų neįvykdo 48 proc. respondentų, o suvokiamo streso lygis prilygsta vidutiniam. PSO fizinio aktyvumo rekomendacijas atitinkančių respondentų raštingumo lygis yra reikšmingai didesnis, negu šių rekomendacijų neatitinkančių asmenų. Visi tyrime skaičiuoti koreliaciniai ryšiai buvo statistiškai reikšmingi, tačiau silpni. Jie parodė, jog kuo didesnis tiriamųjų sveikatos raštingumas, tuo didesnis fizinio aktyvumo lygis ir tuo mažesnis suvokiamo streso lygis; kuo didesnis fizinis aktyvumas, tuo mažesnis suvokiamo streso lygis. Materials and methods: Participants were 123 adults (18 - 64 years old). The study was started in November 2021 month and was ongoing until February 2022. The survey comprised a short form of European Health literacy questionnaire (HLS-EU-Q16), a short form of Nordic physical activity questionnaire (NPAQ short form), 10-item Perceived Stress Scale and some other additional questions. Statistical analysis was done by using “RCommander“ and “Microsoft Excel“ programs. The averages, standard deviations, medians, minimum and maximum values, and correlations were calculated. Results: 69 % of respondents have a problematic level of health literacy. 48 % of respondents do not meet the WHO physical activity norms. The average of health literacy between physical activity groups differs statistically significantly (p = 0,007), but the health literacy level remains problematic in both groups. The calculated correlations show that there is a weak positive statistically significant relation between health literacy and physical activity (r = 0,283); a weak negative correlation between health literacy and perceived stress (r = -0,285) and a weak negative correlation between physical activity and perceived stress (r = -0,277). Conclusions: 48 % of respondents do not follow the WHO recommendations for physical activity and the level of perceived stress is moderate. Participants, who met the WHO physical activity recommendations, health literacy level were significantly higher compared to those, who did not meet the WHO physical activity recommendations. All the correlations are statistically significant, but weak. Respondents with higher level of health literacy also have higher levels of physical activity and lower levels of perceived stress. Tyrimai rodo COVID-19 pandemijos ir karantino poveikį žmonių psichikos sveikatos būklei ir galimą savižudybės rizikos grėsmę. Lietuvoje didžiausi savižudybių rodikliai skaičiuojami regionuose ir, tikėtina, atspindi ten jau ir taip susidariusius psichosocialinius sunkumus, tad svarbu stebėti pagalbos sistemos veikimą šiuose regionuose per pandemiją. Šio tyrimo tikslas buvo išsiaiškinti, kokie yra specialistų, dirbančių regionuose ir dalyvaujančių savižudybių prevencijos sistemoje, darbo COVID-19 pandemijos metu ypatumai. Šiame tyrime naudoti rezultatai iš platesnio savižudybių prevencijos sistemos vertinimo projekto, vykusio 2020 m. Pusiau struktūruoto interviu būdu apie savižudybių prevencijos sistemos veikimą buvo apklausta 30 specialistų (10 savižudybių prevencijos sistemos koordinatorių, 20 tiesiogiai teikiančių pagalbą psichikos sveikatos specialistų). Šiame straipsnyje susitelkta į klausimus apie pandemijos poveikį pagalbos sistemai. Transkribuoti rezultatai išanalizuoti remiantis Boyatzis teminės analizės metodu. Rezultatuose išskirti šie kodai: pagalbos teikimas pandemijos metu (psichikos sveikatos specialistai ir įstaigos prisitaikė prie pandemijos sąlygų, nuotolinis darbas apsunkina pagalbos teikimą, dėl pandemijos pagalba sunkiau prieinama); kreipimasis pagalbos pandemijos metu (dėl pandemijos žmonės mažiau kreipiasi pagalbos, nuotoliniu būdu kreiptis pagalbos paprasčiau, pandemijos metu pagalbos kreipimosi dažnis nesikeitė); pandemijos poveikis ir valdymas (pandemija didina emocinius ir psichikos sunkumus, pandemijai pasibaigus gali blogėti žmonių psichikos būklė, pandemijos valdymas Lietuvoje galėtų būti sklandesnis). Tyrimas atskleidžia ne tik pozityvius psichikos sveikatos priežiūros įstaigų ir specialistų prisitaikymo prie krizinės situacijos aspektus, bet ir reikšmingas problemas, susijusias su nuotoliniu darbu ir pagalbos prieinamumu bei bendra gyventojų psichikos sveikata. The COVID-19 pandemic can influence the situation of suicide rates and mental health in rural regions even more than in major cities. The aim of the current study was to explore the functioning of mental health service provision during the COVID-19 pandemic through interviews with mental health professionals and other specialists who work with suicide prevention in rural areas. Thirty specialists were interviewed using a semi-structured interview format. The following codes were identified during the thematic analysis: providing help during the pandemic (mental health professionals and institutions adapted to the conditions of the pandemic, remote counselling makes providing help more difficult, the help is less reachable); help-seeking during the pandemic (people seek less help because of the pandemic, seeking remote help is easier, the frequency of help seeking didn’t change); the effects and governing of the pandemic situation (the pandemic can have negative effects on mental health; after the pandemic mental health might get worse; the governing of the pandemic situation in Lithuania could be more fluent). The current study reveals positive aspects of mental health professionals’ adaptivity during the pandemic, as well as severe problems which are related to the access to the mental health services during the COVID-19 pandemic.

Tyrimo tikslas – nustatyti narkotinių ir psichotropinių medžiagų vartotojų mirtingumą Lietuvoje ir išanalizuoti mirčių priežastis. Medžiaga ir tyrimo metodai. Atliktas perspektyvinis kohortinis tyrimas (angl. prospective study). Tyrimo tikslinė grupė – Lietuvos Respublikos piliečiai, kurie 2017 m. kreipėsi į Lietuvos asmens sveikatos priežiūros įstaigas dėl psichikos ir elgesio sutrikimų, kai vartoja narkotines ir psichotropines medžiagas. Tyrimo duomenų šaltiniai: Elektroninės sveikatos paslaugų ir bendradarbiavimo infrastruktūros informacinė sistema (e. sveikata). Papildomas duomenų apie asmenų narkotikų vartojimą šaltinis – asmenų, kurie kreipiasi į asmens sveikatos priežiūros įstaigas dėl psichikos ir elgesio sutrikimų, kai vartoja narkotines ir psichotropines medžiagas, stebėsenos informacinė sistema. Tyrimo rezultatų standartizavimo procedūrai atlikti naudojama kontrolinė kohorta – Europos standartinė populiacija. Rezultatai ir išvados. Kohortinio narkotikų vartotojų Lietuvoje mirtingumo tyrimo tiriamąją populiaciją sudarė 231 asmuo, iš jų 179 vyrai ir 52 moterys. Tiriamuoju laikotarpiu (2018–2020 m.) analizuota 14 tyrimo dalyvių mirčių atvejų: devyni atvejai vyrų ir penki moterų. Tai sudarė 6,1 proc. visų įtrauktų į tyrimą dalyvių – 5,0 proc. visų įtrauktų vyrų ir 9,6 proc. įtrauktų moterų, santykis 1,9 karto. Daugiausia mirusių tiriamųjų (9 asm., 64 proc.) buvo registruoti dėl psichikos ir elgesio sutrikimų, vartojant opioidus. Bendras mirusiųjų amžiaus vidurkis – 36,4 metų, vyrų – 38,2 metų, moterų – 33,2 metų (palyginimui vyrų vidutinė tikėtina gyvenimo trukmė Lietuvoje 2019 m. buvo 71,5 metų, o moterų – 81 metai). Per visą tyrimo laikotarpį Lietuvos tirtos kohortos mirtingumas buvo 19,7/1000 asm. Standartizavus pagal amžių, standartu imant Europos populiaciją, mirtingumas 21,9/1000 asmens metų, vyrų – 13,3/1000 asmens metų, moterų 30,5/1000 asmens metų. Material and research methods. A prospective cohort study was performed. The target group of the research is citizens of the Republic of Lithuania, who in 2017 applied to Lithuanian health care institutions for mental and behavioral disorders when using drugs and psychotropic substances. Research data sources: Electronic health services and cooperation infrastructure information system (e-health). An additional source of data on individuals' drug use is the Information System for Monitoring Individuals who Appear to Personal Health Care Institutions for Mental and Behavioral Disorders When Using Drugs and Psychotropic Substances. Performing standardization procedures for test results in a controlled location - European standard population. Outcome and conclusions. The study population of the cohort study of mortality of drug users in Lithuania consisted of 231 persons, of which 179 were older and 52 were female. During the study period (2018–2020), 14 deaths of study participants were analyzed: nine cases in men and five in women. This was 6.1 percent. 5.0% of all participants included in the study. of all men included and 9.6 percent. included women, the ratio was 1.9-fold. The highest number of deaths was due to mental and behavioral disorders due to opioid use - nine subjects (64%). The total average age of deaths was 36.4 years, for men - 38.2 years, for women - 33.2 years (comparing men on average only with life expectancy in Lithuania in 2019 was 71.5 years, and for women - 81 years). During the whole study period, the mortality of the studied cohort in Lithuania was 19.7 / 1000 persons. Mortality is 21.9 per 1000 person-years, for men 13.3 per 1000 person-years for men and 30.5 per 1000 personyears for women, standardized by age for the European. Tyrimo tikslas – išanalizuoti tėvų, auginančių proto negalią turintį vaiką, santykių su sveikatos specialistais patirtį. Metodika. Tyrimo metu apklausti 82 tėvai, auginantys proto negalią turinčius vaikus. Atsako dažnis 68,3 proc. Gauti duomenys buvo apdoroti ir analizuoti naudojant statistinių duomenų paketą SPSS 22. Skirtumas tarp kintamųjų, pasirinkus reikšmingumo lygmenį α=0,05, laikomas statistiškai reikšmingu ir patikimu, kai apskaičiuotoji p reikšmė yra ne didesnė nei 0,05 (p≤0,05). Rezultatai. Tyrime dalyvavo 82 respondentai, kurių daugumą sudarė moterys (89 proc.). Dažniausiai tėvai pagalbos ieškojo sveikatos priežiūros įstaigose (84,9 proc.). Tyrimo metu buvo nustatyta, kad 87,8 proc. apklaustųjų yra patenkinti šeimos gydytojo darbu su jų neįgaliu vaiku. Didžioji dauguma (78 proc.) apklaustųjų nurodė, kad nepatiria sunkumų hospitalizuojant ir gydant proto negalią turintį vaiką. Išvados. Dažniausiai tėvai, sužinoję apie vaiko negalią, kreipiasi į sveikatos priežiūros įstaigas, siekdami tęsti gydymą pas įvairius sveikatos priežiūros specialistus. Proto negalią turintys vaikai yra mažiau atsparesni rizikos veiksniams, todėl dažniau serga įvairiomis somatinėmis (virškinamojo trakto, kvėpavimo takų, širdies ir kraujagyslių) ligomis. Dauguma tėvų dėl savo vaiko negalios patiria sunkumų, lankydamiesi pas gydytoją odontologą, okulistą ir LOR. Raktažodžiai: proto negalia, sveikatos priežiūra, neįgalumas. The aim - to analize experience of parents, having children with mental illness, regarding healthcare system. Method: During the research, a total of 82 parents who have rising kids with mental disability were surveyed. The response rate was 68,3 proc. Results were processed and analyzed using statistical data packet SPPS 22. Results: Examine involved 82 respondents, which most of them were women‘s (89 percent). Parents most often seek help in health care institutions (84.9 percent). The investigation revealed that 87.8 percent of respondents are satisfied with the work carried out by the family doctor of their disabled child. Also, most majority (78 percent) of the respondents indicated they haven‘t any difficulty hospitalization and treatment of mental disability having child. Conclusions: 1. In most cases parents become aware of the child‘s disability appeals to health care institutions with the purpose to continue treatment at various health care professionals. 2. With an intellectual disability children were less resistant to risk factors, and generally suffer from various somatic (digestive tract, respiratory tract, cardiovascular) disease. According to The Joint Commission, a culture of safety is a key component for achieving sustainable and safe health care services, and hospitals must measure and monitor this achievement. Promoting a patient safety culture in health services optimally includes midwifery and nursing. The first aim of this study is to assess the University Perinatal Center’s staff members’ perceptions of safety culture. A second aim is to identify how the perceptions of safety culture actors are related to the socio-demographic characteristic of the respondents. Methods: A descriptive, cross-sectional, correlational design was applied in this study. Registered nurses and midwives were recruited from the University Perinatal Center in Lithuania (N = 233). Safety culture was measured by the Safety Attitudes Questionnaire (SAQ). Results: The mean scores of the responses on the 6 factors of the SAQ ranged from 3.18 (0.46) (teamwork climate) to 3.79 (0.55) (job satisfaction) points. The percentage of positive responses to the SAQ (4 or 5 points on the Likert scale) ranged from 43.2% to 69.0%. Straipsnyje pristatomas tyrimas atskleidžia pacientų ligoninės pasirinkimo proceso sudėtingumą ir tyrimo duomenų kontekste leidžia įvertinti pacientų ligoninės rinkimosi veiksnius. Ligoninės pasirinkimo klausimas aktualus ne tik Lietuvoje, bet ir kitose ES bei pasaulio šalyse. Šiame sudėtingame procese svarbu įvertinti pasirinkimo veiksnių daugiafunkciškumą: paciento ligos pobūdį ir sveikatos būklę, ligoninės charakteristikas, interaktyvų ligoninės darbuotojų, ligonio artimųjų, internetinės prieigos galimybių tarpusavio santykį. Tyrimo imtį sudarė 477 dalyviai nuo 18 iki 89 metų amžiaus, kurie gydėsi įvairiose šalies ligoninėse. Remiantis straipsnyje pateikiamo tyrimo rezultatais, galima teigti, kad didžiausią įtaką pacientų atitinkamos ligoninės pasirinkimui daro artimųjų ir draugų, šeimos gydytojo patarimai ir asmeninė patirtis. The article presents the research that reveals the complexity of the patient hospital choice process and, in the context of the research data, enables the evaluation of the patient hospital choice factors influencing their decision. The problem of hospital choice is topical both in Lithuania and other EU and world countries. It is a complex process that requires the evaluation of the poly-functionality of the choice factors: the character of the patient disease and their state of health, the characteristics of the hospital, the interactive relationship between the hospital staff, the patient’s relatives, and Internet access. The research sample consisted of 477 participants aged between 18 and 89, treated in various hospitals throughout the country. Based on the results of the research presented in the article, one can argue that the factors having the greatest impact on patients’ choice of hospital include the advice of friends and significant others, personal experience, and the advice of their family doctor. The main prerequisites for choosing a hospital named by the patient respondents were the qualification of the medical staff, the quality of health care, communication between doctors and patients, the geographical location of the hospital, and its accessibility.

Dental anxiety (DA) is a prevalent public health issue. However, there is a lack of selfadministered DA interventions. The aim of this study was to evaluate the short-term effects of web-based interventions aiming to reduce DA in adults in two European countries. A pretest posttest design was used. Tailor-made websites were developed in Lithuania and Norway. Volunteers who self-reported DA were invited to participate. DA levels measured by the Modified Dental Anxiety Scale (MDAS) were assessed at baseline and after two weeks via online questionnaires. The interventions were completed by 34 participants in Lithuania and 35 participants in Norway. Sveikatos priežiūros įstaigos (SPĮ) yra ganėtinai uždaros, kompleksinės įstaigos. Čia komunikaciniai ryšiai tarp nutolusių skyrių, įvairių profesijų atstovų ir hierarchinių grandžių yra sudėtingi, kyla konfliktų, nesusikalbama, iškraipoma informacija. Nesprendžiamų vidinės komunikacijos barjerų pasekmės – ne tik kilusi įtampa tarp darbuotojų, darbo kokybės prastėjimas, bet nukenčia ir įstaigos reputacija, galima neatitaisoma žala paciento sveikatai. Siekiant veiksmingos komunikacijos, reikia tam tikrų įgūdžių, abipusio supratimo, pagarbos tiek iš organizacijos vadovų, tiek iš darbuotojų. Straipsnyje analizuojamos SPĮ vidinės komunikacijos barjerų atsiradimo priežastys bei jų šalinimo būdai. Remiantis tyrimo rezultatais, nustatyta, kad analizuojama SPĮ neturi vidinės komunikacijos strategijos, vidinė komunikacija nepakankamai kontroliuojama, yra fragmentiška. Personalas susiduria su organizacijos struktūros, fizinio, psichologinio ir technologinio pobūdžio vidinės komunikacijos barjerais. Siekiant juos eliminuoti, pateikiama rekomendacijų, kaip gerinti SPĮ vidinę komunikaciją. Health Care Institutions (HCI) are rather closed and complex institutions. Here, the communication links between the remote departments, the various professions and the hierarchical chains are complex, the conflicts, miscommunication, and information distortion arises. The consequences of unresolved internal communication barriers – the tension between employees, the decrease in the quality of work, the negative impact on the reputation of the institution. Effective communication requires certain skills, mutual understanding and respect both from the heads of organization and from the staff sides. The article raises problematic questions: What are the barriers of internal communication in a health care institution? How can internal communication barriers be removed? Objective: to investigate the barriers of internal communication of the health care institution and to provide recommendations for their elimination. • To analyse internal communication barriers in theory. • To investigate the internal communication barriers of health care institutions. • To provide recommendations for the elimination of internal communication barriers in the healthcare institutions. Methods: analysis of scientific literature, analysis of internal documents of HCI, questionnaire survey, semi-structured interview. Data processed using SPSS software package. Based on the study results, it was found, that one of the main internal communication gaps in the HCI is the hierarchy of duties between employees. This issue is also sharpened by the opinion of the research participants on the lack of respect, equivalence, evaluation and shortage of attitude as a team member. This leads to conflicts, distrust of managers and lack of feedback. Other significant internal communication barriers: lack of communicative skills, lack of informal forms of communication, outdated IT, insufficient IT application in internal communication, excessive flow of information, lack of time, physical distance between sections and departments. The study found, that gender and cultural differences, constant change of employees and forms of informal communication (rumours and informal celebrations) do not fundamentally affect the dissemination of information in the institution. The investigation revealed, that the staff of the HCI felt quite free to communicate on various topics, express their opinions and ideas. In most cases, this happens in informal discussions, which are positively evaluated by respondents and are a valuable tool for information exchange. Meanwhile, the content of transmitted information and the performance of tasks are not affected by the rumours. The results of the research revealed, that employees focus more on the means of transmission of formal communication and their quality, and consider the value of informal communication only as much as it helps to meet the necessary social needs, but without the need for greater efforts. Recommendations have been made for the elimination of diagnosed communicative process deficiencies. It is recommended the audit of institution’s internal communication, and based on its findings, establish an internal communication strategy and a plan for its implementation, linked to the overall organization strategy. It is also proposed to apply an internal communication efficiency model, that includes the concept of HCI team formation. Its main objective is directed to create a positive, open, motivating, supportive internal communication. The development of IT would help reduce the information distance between different positions, save time and improve the regulation of information flows.

Straipsnyje nagrinėjamos technologinių inovacijų, skirtų sveikatos priežiūrai, sampratos, inovacijų rūšys pagal technologijas ir jų pagrindu sukuriamus produktus. Taip pat apžvelgiama dabartinė technologinių inovacijų poveikio sveikatai ir gyvenimo trukmei, kaip dažniausiai ekonomistų naudojamam sveikatos rodikliui, matavimo praktika. Remiantis esamais empirinių tyrimų duomenimis, autorės pasiūlo modelį, skirtą analizuoti skirtingų veiksnių grupių reikšmingumą vidutinei tikėtinai gyvenimo trukmei gimimo metu, ir patikrina modelį atlikdamos apklausą, siekiant patvirtinti modelio turinį ir kiekvienos modelio veiksnių grupės įtakos svarbą gyvenimo trukmei. The article explores the concept of technological innovations in healthcare, types of innovations by technology and its output. It also overviews the existing practice of the technological innovations’ impact measurement on health and life expectancy as the most common metric, used by the economist. Based on the existing empirical research evidence the authors propose a model to analyze the impact of different groups of factors on the life expectancy at birth. The proposed model is validated by running a survey to confirm its content and weight of impact of each group of the factors on life expectancy. The paper briefly describes the current EU legal framework in the healthcare field and assess to which extent it could apply to the provision of cross border eHealth services. In particular, it analyses the provision of such services from the free movement angle, i.e. whether EU law guarantees to health professionals access to and exercise of activiti es in the 3rd type of situations, as well as whether the insured persons have the right to reimbursement of costs of such healthcare from their social security system.

Vaikų sveikatos priežiūros poreikių pokyčiai skatina Europos šalis peržiūrėti vaikų sveikatos priežiūros sistemas, kad visiems vaikams būtų prieinama efektyvi ir tinkama sveikatos priežiūra. Atlikta atrinktų mokslinių publikacijų, rekomendacijų ir studijų apžvalga parodė Europos šalių vaikų sveikatos priežiūros sistemų skirtumus, ypač pirminės vaikų sveikatos priežiūros organizavimo. Išskiriamos trys vaikų pirminės sveikatos priežiūros sistemos: pediatrinė, šeimos gydytojo ir mišri. Ne visos šalys vadovaujasi PSO Europos regiono biuro parengta Vaikų ir paauglių sveikatos strategija 2015-2020 m., skirta padėti šalims įdiegti įrodymais pagrįstą sistemą, siekiant patobulinti vaikų ir paauglių poreikius atitinkančią sveikatos priežiūrą. Pastebima, kad vaiko centriškumo principas nėra įdiegtas ne tik daugelio šalių sveikatos priežiūros sistemose, bet ir kitose gyvenimo srityse. Modeliuojant bendrą sveikatos priežiūros sistemą, neišskiriant vaikų, dažniausiai remiamasi didžiosios gyventojų dalies – suaugusiųjų poreikiais, nuvertinant vaikų poreikius. Vaikystės sveikatos problemos gali turėti įtakos visam gyvenimui. Sveikatos priežiūros netolygumų problemos sprendimas, mažinant socialinių veiksnių įtaką sveikatai, apsaugotų vaikų populiaciją. Kūdikių mirtingumo rodikliai žemesni tose Europos šalyse, kuriose geresnė ekonominė situacija ir didesnis dėmesys skiriamas gyventojų gerovei. Augančio vaiko sveikatos priežiūros poreikiai yra specifiniai daugelyje sričių, pradedant specifinėmis žiniomis, kurių reikia vaikų ligų gydymui ir baigiant vaikų atstovavimo ypatumais, todėl sveikatos priežiūros paslaugos turi būti pritaikytos vaikams, o specialistai turi turėti specifinių kompetencijų. The changing needs of children’s health care encourage European countries to review their children’s healthcare systems to make effective and appropriate healthcare available to all children. An overview of selected scientific publications, recommendations and studies showed differences between countries in European children’s healthcare systems, especially in the organization of primary health care. Three primary care systems for children are distinguished: pediatric, family doctor and mixed. Not all countries follow the WHO European Office’s Children’s and Adolescent Health Strategy 2015-2020 to help countries put in place an evidence-based system to improve health care that meets the needs of children and adolescents. It is noted that the principle of centricity of the child is not introduced only in healthcare systems in many countries but also in other areas of life. When modeling the overall healthcare system without excluding children, the majority of the population is based on the needs of adults to underestimate children’s needs. Childhood health problems can have an impact throughout their lives. Addressing health inequalities by reducing the impact of social factors on health would protect the pediatric population. Infant mortality rates are lower in European countries with a better economic situation and greater focus on the well-being of the population. The health care needs of a growing child are specific in many areas, from the specific knowledge needed to treat children’s diseases to the peculiarities of their representation. Therefore, health care services need to be adapted to children and professionals need to have specific competencies. Augantis paciento kaip pagrindinio paslaugų kokybės vertintojo vaidmens pripažinimas kokybės gerinimo procese ir nuolatinis neigiamos gimdymo patirties (būklės, susijusios su neigiama patirtimi, pvz., pogimdyminė depresija ar kitokia našta) eskalavimas žiniasklaidoje tiek atskiriems individams, tiek visuomenei skatina plačiau nagrinėti su akušerinių paslaugų teikimo kokybe susijusius, ją formuojančius aspektus. Akušerinių paslaugų sektoriaus vaidmuo pabrėžiamas Lietuvos sveikatos programoje, kurioje numatomos ilgalaikės gimdyvėms teikiamų paslaugų kokybės tobulinimo kryptys. Akušerinių paslaugų kokybės užtikrinimo tematika yra viena prioritetinių Pasaulio sveikatos organizacijos veiklos krypčių. Šio tyrimo tikslas – įvertinti pacienčių (gimdyvių) požiūrį į teikiamų akušerinių paslaugų kokybę. Tretinio lygio akušerines paslaugas teikiančioje sveikatos priežiūros įstaigoje buvo atliktas kiekybinis anketinės apklausos tyrimas. Tyrime dalyvavo 309 pagimdžiusios moterys, joms pateikta 50 klausimų paslaugų kokybės vertinimo anketa. Suminis vidinis klausimyno patikimumas – 0,966. Nustatyta, kad nors tiriamos įstaigos paslaugų kokybė vertinama kaip aukšta, tačiau moterų poreikis aktyviai dalyvauti savo sveikatos priežiūros procese realizuojamas nepakankamai, tobulintini komunikaciniai paslaugų teikimo aspektai. Išskirtos pacienčių grupės, kurių priežiūrai, siekiant gerinti teikiamų paslaugų kokybę iš paciento perspektyvos, skirtinas ypatingas personalo dėmesys, pirmiausia realizuojant efektyvios komunikacijos tarp paciento ir paslaugas teikiančio personalo poreikį. Tai natūraliais gimdymo takais gimdžiusios moterys, kurioms atlikta epiziotomija, ir moterys, kurioms nebuvo užtikrintas ankstyvas kontaktas su naujagimiu iškart pagimdžius. Vertinant greitą paslaugų kokybę apibūdinančių veiksnių kaitą, atsirandant naujų kokybišką paslaugų teikimą apibrėžiančių faktorių, tampa akivaizdu, kad standartinės kokybės vertinimo anketos, skirtos vertinti akušerinių situacijų paslaugų kokybei, turi būti nuolat atnaujinamos, o kokybės vertinimas turi būti atliekamas periodiškai, pasitelkus vertinimo klausimynus, orientuotus į akušerijos kokybės specifiką. The growing recognition of patients’ roles as key evaluators of service quality in the healthcare quality improvement process – as well as periodic media coverage of adverse birth experiences, such as postpartum depression, that place burdens on mothers and society in general – encourages a broader examination of the aspects that form the quality of midwifery services. The relevance of this topic is also determined by the emphasis on the role of the obstetric service sector in the Lithuanian Health Programme, which provides long-term directions for improving the quality of services provided to mothers. Quality assurance of maternity services is also one of the main scopes of the World Health Organization’s activity. The aim of this study is to evaluate the attitudes of patients towards the quality of obstetric services provided. A quantitative survey was conducted at a tertiary level obstetric facility unit involving 309 women who had given birth. Respondents participated in the study by submitting a service quality evaluation questionnaire, and the total internal reliability of the questionnaire was 0.966. The research revealed that, although the quality of maternity services in the institution where the research was conducted was assessed as high, the need for women to actively participate in their health care process is not ensured sufficiently and communicative aspects of the provision of services can be improved. Women who undergo vaginal delivery and have episiotomies, as well as women who are not guaranteed early contact with the newborn immediately after giving birth, require special attention on behalf of the medical staff. This is first ensured by realizing the need for effective communication between the patient and healthcare specialists in order to improve the quality of the services provided. Assessing rapid changes in the healthcare service quality field and the constant emergence of new factors defining quality of services, it becomes obvious that standard quality assessment questionnaires for obstetric situations must be constantly reviewed, and quality assessment must be carried out periodically using questionnaires focused specifically on obstetrical practice. Sveikatos priežiūros organizaciniai pokyčiai neįsivaizduojami be e. sveikatos technologinės pažangos. Jie yra sėkmingesni, kai kuo daugiau sveikatos priežiūros specialistų jaučiasi pasiruošę pokyčiams, pripažįsta pokyčio vertę ir kuo mažiau yra besipriešinančių pokyčiams. Tyrimo tikslas – išanalizuoti stacionarinės asmens sveikatos priežiūros įstaigos (ASPĮ) specialistų požiūrį į priešinimąsi mažinančius veiksnius ir priemones, diegiant ir plečiant e. sveikatos paslaugas. Tyrimo metodika – atliktas kiekybinis tyrimas – anketinė apklausa raštu. Anketa sudaryta atsižvelgiant į mokslininkų, nagrinėjusių e. sveikatą, tyrimus. Statistiniai duomenys apdoroti IBM SPSS 23.0 (Statistical Package for the Social Sciences) programiniu paketu. Rezultatai. Nustatyta, kad naudojantis e. sveikata yra greitesnis, patogesnis informacijos prieinamumas, jos gavimas. Tyrimų rezultatų gavimas iš laboratorijos tiesiogiai gydančiam gydytojui, kitos informacijos gavimas, jos panaudojimas ir keitimasis mažina priešinimąsi e. sveikatos diegimui. Aukštąjį universitetinį išsilavinimą turintys specialistai statistiškai reikšmingai dažniau nurodo, kad e. sveikata suteikia galimybę greičiau ir patogiau keistis informacija. Jaunesni nei 50 metų specialistai statistiškai dažniau sutinka, kad priešinimąsi e. sveikatos diegimui mažina laiko trumpėjimas popierinės dokumentacijos pildymui. Pirmos amžiaus grupės respondentai statistiškai reikšmingai dažniau nurodė, kad sėkmingai e. sveikatos plėtrai labiausiai priimtinas apsikeitimas gerąja praktika. Didžioji dalis darbuotojų, turinčių aukštąjį universitetinį išsilavinimą, ir vadovai statistiškai reikšmingai dažniau nurodė, kad darbuotojų kvalifikacijos tobulinimas, sklandus informacinių technologijų infrastruktūros diegimas yra reikšmingos priemonės sėkmingai e. sveikatos sistemos plėtrai. Išvados. Informacinių technologijų panaudojimas ir plėtra e. sveikatoje yra reikšmingas veiksnys plėtojant mokslinius tyrimus ir gerinant sveikatos priežiūros paslaugų kokybę. E. sveikatos diegimas ir plėtra sveikatos priežiūros specialistų suvokiama kaip teigiamas ir naudingas pokytis. Tyrimo metu nustatyti veiksniai, mažinantys priešinimąsi e. sveikatos sistemos diegimui ir plėtrai: greitesnis, patogesnis informacijos prieinamumas, jos gavimas, panaudojimas ir keitimasis, tiesioginis tyrimų rezultatų gavimas iš laboratorijos gydančiam gydytojui. Priemonės, mažinančios priešinimąsi: apsikeitimas gerąja praktika, darbuotojų kvalifikacijos tobulinimas ir sklandus informacinių technologijų infrastruktūros diegimas. Organizational changes in healthcare are unimaginable without e. health technological advances, but they are more successful when as many healthcare professionals feel ready for change and recognize the value of change, and as few are resistant to change The aim of this work – to analyse stationary personal healthcare institution specialists’ approach to resistance reduction factors for installing and expanding services of e. health system. Methods – Quantitative research – questionnaire in written form. Questionnaire was concluded taking into account researches that has been carried out by other scientists. Statistical data was processed using IBM SPSS 23.0 (Statistical Package for the Social Sciences) program package. Results. It was found that using e. health is faster, more convenient access to information, obtaining it. Obtaining test results from the laboratory directly to the treating physician, obtaining, using, and exchanging other information reduces resistance to e. for the implementation of health. Specialists with university education indicate statistically significantly more often that e. health makes it possible to exchange information faster and more conveniently. Professionals under the age of 50 statistically more often agree that opposition to e. health implementation is reduced by reducing the time required to complete paper documentation. The respondents of the first age group statistically significantly more often indicated that exchange of „good practices” in e. health is the most acceptable for health development. The majority of employees with university education and managers indicated statistically significantly more often that improving the qualifications of employees, smooth implementation of information technology infrastructure are significant means for successful development of the e. health system. Conclusions. Use and development of information technologies in e. health is a significant factor in developing scientific research and improving the quality of healthcare services. The implementation and development of e-health is perceived by healthcare professionals as a positive and beneficial change. Research showed factors that reduce resistance to e. for the implementation and development of the health system: faster, more convenient access to information, its acquisition, use and exchange, direct receipt of research results from the laboratory to the attending physician. Measures to reduce resistance: exchange of “good practice”, improvement of employee qualifications and smooth implementation of information technology infrastructure. Per Abiejų Tautų Respublikos padalijimus ir po jų XVIII–XIX amžių sandūroje Lietuvos elitas patyrė daug permainų: ėmė ryškėti takoskyra tarp Apšvietos idėjų šalininkų ir konservatyviosios aristokratijos. Pirmieji siekė reformų ir valstybės savarankiškumo bei visuomenės progreso. Antrieji nepalaikė pertvarkų ir siekė išsaugoti buvusią visuomenės sanklodą, neretai siekė Rusijos imperijos politikų paramos. Būtina tyrinėti ne tik to laikotarpio elito veiksmus politikos srityje, bet ir jų buitį bei gyvenimo būdą. Straipsnyje nagrinėjama, ką galima sužinoti apie veiksnius, galėjusius padaryti įtaką Liudviko Skumino Tiškevičiaus (1751–1808) – vieno svarbesnių XVIII–XIX amžių sandūros Lietuvos politinių veikėjų – sveikatai. Pagrindinis straipsnio herojus istoriografijoje vertintas kaip vienas iš konservatyvių pažiūrų didikų, didžiausią dėmesį skyręs savo materialinių gėrybių kaupimui. Ludwik Skumin Tyszkiewicz (1751–1808), the last Grand Marshal of the Duchy of Lithuania and First Marshal of the Nobles of the Lithuanian Governorate in Tsarist Russia, was not known as a gifted politician. He supported the conservatives in the Polish-Lithuanian Commonwealth before the Third Partition in 1795, and he was among the nobles who were supported by Russia and who were fundamentally hostile to reforms and the Constitution of 3 May 1791. In the Russian Empire, he was quite loyal to the new government, with no higher ambitions. Contemporaries and historians often emphasised Skumin Tyszkiewicz’s pursuit of wealth and luxury as troubling. Could it have directly or indirectly influenced his health? The article deals with this issue. The main sources were the accounts of Tyszkiewicz’s Vilnius treasury and provincial estates, and other informative financial documents, which tell us about the nobleman’s diet and his purchases of medication. Tyszkiewicz’s personal and financial correspondence, his memoirs and his funeral sermon, were used as well. During the early period of his life, references to Tyszkiewicz’s health problems are not very numerous. In fact, they are rather fragmentary and unspecific. However, at the beginning of the 19th century, Tyszkiewicz’s health was seriously undermined, he suffered from gout and other health problems, and he very rarely left his Garodnia estate. Knowledge about the aristocrat’s kitchen is indeed plentiful. Meat was served in abundance: beef (which probably predominated), lamb, pork, turkey, chicken and goose meat were all used in Skumin Tyszkiewicz’s kitchen. In addition, we know that wild animals were served (hare, roe deer, beaver, elk), wild birds (hazel grouse, partridge, western capercaillie, black grouse, common snipe), and small birds (such as nightingale, Bohemian waxwing, and even European bee-eater). There was also no shortage of fish, both local (pike, perch, tench, eel, European cisco and others), and imported (sturgeon, anchovy, herring). The nobleman used eggs, local and imported cheeses, rye bread and wheat bread, and rice. There was no shortage of vegetables either (carrots, lettuce, parsley, dill, cabbage, kohlrabi, potatoes). Tyszkiewicz’s kitchen was full of spices (black peppers, allspice, cinnamon, ginger, nutmeg). Chocolate, coffee, cream, sugar and honey were used as well. We also know quite a lot about local and imported alcohol for the nobleman: Burgundy, Hungarian wines, Champagne, regular and strong beer, porter, English ale, and French and cinnamon vodka. It can be concluded from the accounts that he consumed a lot of alcohol, especially beer. Research into the diet of his last year shows that although he used alcoholic beverages, sugar and a large quantity of flour dishes, Tyszkiewicz’s diet really did not lack healthy products, such as vegetables, greens, lean meat and fish. His death, however, was hardly unexpected, according to the memoirs of his daughter Anna Potocka. Presenting the results of the autopsy in his memoirs, Joseph Frank wrote that Skumin Tyszkiewicz must have felt unwell, as he was found with a completely ossified aorta, huge kidney stones and stony lung calcifications, a consequence of gout. It is difficult to judge whether the pursuit of luxury and abundant consumption led to the death of Skumin Tyszkiewicz. He lived as was usual in his epoch. The lifestyles of many other nobles were hardly different. But a deeper comparative analysis, requiring future research, may answer this question. Tarpukariu Lenkijos sveikatos apsaugos sistemoje universitetai atliko patariamąjį vaidmenį, taip pat padėjo įgyvendinti sveikatos politiką. Vilniaus Stepono Batoro universiteto Medicinos fakulteto Higienos katedros vedėjas profesorius Kazimierzas Karaffa-Korbuttas (1878–1935) ir jo studentai dalyvavo šviečiant visuomenę higienos klausimais. Mūsų tyrimo metu surinkti pirminiai duomenys rodo, kad ryškiausių veiksmų šioje srityje ėmėsi vyresnioji asistentė ir adjunktė daktarė Janina Bortkiewicz-Rodziewiczowa (1892–?). Vietinėje ir valstybės spaudoje ji paskelbė daugiau kaip 100 populiarių straipsnių įvairiomis higienos švietimo temomis, pavyzdžiui, apie mikrobus, klaidingus liaudies įsitikinimus ir prietarus, slaugą namuose, infekcines ligas apskritai, konkrečias infekcines (socialines) ligas, asmens higieną, taip pat buto ar darbo vietos higieną, profesinės sveikatos problemas ir kt. Janinos Bortkiewicz viešosios higienos klausimams skirtos publikacijos buvo tiesiogiai susijusios su sveikatos apsaugos iššūkiais to meto Vilniaus krašte ir oficialios sveikatos politikos gairėmis, t. y. kova su socialinėmis ir užkrečiamosiomis ligomis, tokiomis kaip tuberkuliozė, gripas, skarlatina, reumatas, darbininkų ligos ir kt. Tai, jog Janina Bortkiewicz, kaip gydytoja, dirbo vietos ligoninėse ir ambulatorijose, taip pat Vilniaus Stepono Batoro universiteto Higienos katedroje, leidžia teigti, kad ji buvo gerai susipažinusi su vietos sveikatos apsaugos situacija. Ji taip pat dalyvavo sveikatos priežiūros specialistų ir neprofesionalų švietimo veikloje. Tačiau užuot pasirinkusi formalų stilių ir pamokslavusi, savo populiariose publikacijose ji aiškiai ir kūrybiškai kreipėsi į skaitytojus, stengdamasi paskatinti jų smalsumą ir norą bendradarbiauti. Tikriausiai tai buvo susiję su jos gydytojos praktika vietos ligoninėse ir ambulatorijose. Būdama mokslininkė, ji stengėsi kuo paprasčiau paaiškinti to meto visuomenės sveikatos problemas ir jų sprendimo būdus, kad tai suprastų paprasti žmonės. Tačiau kartu jos straipsniai buvo informatyvūs, apgalvoti ir konstruktyvūs. Juose buvo pateikiama tikslinga informacija, kuri galėjo sėkmingai padėti spręsti sveikatos problemas ir numatyti galimas grėsmes. Autorė stengėsi paaiškinti daugelį socialinių reiškinių, glaudžiai susijusių su šiomis ir kitomis socialinėmis ligomis. Mokslininkė stengėsi įtikinti skaitytojus pasikliauti mokslu grįsta medicina. Jos stilius buvo paprastas ir aiškus, tačiau jam taip pat būdingas gausus žodynas ir spalvingi posakiai, kurie ne visada pasiduodavo tiksliam vertimui į kitas kalbas. Straipsniai turėjo aiškų didaktinį tikslą. Mokslininkė nebijojo oponuoti klaidingiems paprastų žmonių įsitikinimams ar tiesiog griauti populiarius mitus. Tačiau kartu ji nesistengė pamokslauti, teisti ar primygtinai reikalauti savo nuomonės pripažinimo. Savo straipsniuose ji tik išdėstydavo tuo metu mokslui žinomus faktus. Kai kurie iš jų šiandienos mokslo kontekste jau yra pasenę, tačiau daugelis autorės praktinių patarimų ir jų pateikimo būdas tebėra aktualūs ir šiandien. Todėl jos straipsniai yra geras pavyzdys ir šiandienos mokslininkams, siekiantiems populiarinti savo mokslo srities žinias. Populiariuose Janinos Bortkiewicz straipsniuose pateikti aprašymai daugiausia autobiografiniai, paremti jos pačios praktika, išsamūs, vaizdžiai atskleidžiantys to meto visuomenės pažiūras, todėl leidžia suprasti visuomenės medicininio raštingumo, higienos lygį. Juose taip pat pateikiama nemažai universalių samprotavimų apie socialinę ligų kilmę, jie ir šiandien lieka aktualūs, ypač kalbant apie mažiau išsilavinusias visuomenės grupes. Because General Żeligowski’s troops occupied Vilnius in the autumn of 1920 and Poland annexed it two years later, the health-care system that operated in Poland at the time began to be introduced in the city and the region. The official guidelines for health policy in Poland derived from the concept of hygiene proposed by Tomasz Janiszewski, the founder of the health system in the country, which focused on social hygiene. Universities played an advisory role in the Polish health system and were involved in educating the public on hygiene issues. In interwar Vilnius, the most prominent figure in this field was Janina Bortkiewicz-Rodziewiczowa, a researcher and senior assistant in the Department of Hygiene of the Faculty of Medicine at Stephen Bathory University. This article analyses her publications aimed at promoting science. It examines the means by which BortkiewiczRodziewiczowa conveyed specific medical knowledge to a lay audience. It also discusses what topics she emphasised most and what reasons led to her choices, and how this correlated with priorities in medical science and health policy at that time. Finally, it touches on an interesting practical aspect, namely what public education strategies applied at the time can still be applied today. Straipsnyje analizuojami 1944–1947 m. įvykiai Lietuvoje, kai sovietų valdžia buvo priversta kovoti su dideliu atvykstančių elgetų, vaikų bei suaugusiųjų iš Sovietų Sąjungos ir Rytų Prūsijos antplūdžiu. Tiriama šios socialinės problemos mastas ir priežastys, įvairiataučių elgetų maršrutai, valdžios institucijų pastangos deportacijomis suvaldyti elgetų antplūdžio krizę, susijusią su šiltinės plitimu. Pokariu Lietuvą ištikęs elgetų antplūdis istoriografijoje ir viešojoje erdvėje (literatūroje, kine, muziejų rengiamose parodose), net teisėje įsitvirtino vien vilko vaikų likimo temomis. Turtingi ir intensyviai tyrėjų bei menininkų eksploatuojami šaltiniai – vokiečių vaikų liudijimai – tapo vieninteliu atminties klodu, užgožusiu kitus, ne mažiau dramatiškus pirmųjų pokario metų siužetus, visų pirma – žinias apie iš Sovietų Sąjungos atvykstančių elgetų bangas. Jų pirmoji pasiekė Lietuvą dar 1944 m., o dėl 1946–1947 m. bado atsirito ir antroji. Tuo metu elgetų iš SSRS (neįgaliųjų, „maišelininkų“, vaikų) atvykdavo daug daugiau nei iš Rytprūsių. Be jų, išmalda vertėsi ir lietuviai, po šalį klajojo čigonai (romai). Pokario šaltinių analizė nepatvirtina vien vilko vaikų patirtimi grįstų istoriografijos teiginių apie pokario elgetų tautinę sudėtį, amžių ir Lietuvos gyventojams dėl vokiečių globos esą taikytas represijas. Sovietų valdžia netoleravo elgetaujančių ir valkataujančių žmonių, ne vien vokiečių ir ne vien vaikų. Jie persekioti pirmiausia dėl nusikalstamumo ir infekcinių ligų protrūkių. Elgetų antplūdžio į Lietuvą krizė (didžiausio antplūdžio metu kasdien atvykdavo apie tūkstantį žmonių) baigėsi 1947–1948 m. sandūroje. Ją įveikti padėjo kova su šiltinės epidemija: milicijos ir saugumo pareigūnai 1947 m. gegužę–birželį blokavo elgetų migraciją, atvykėliai iš kitų regionų buvo grąžinami į kilmės vietas. Vėlesniais metais svetimtaučių elgetų antplūdis mažėjo. Tai lėmė ekonominė bei politinė situacija kaimyniniuose regionuose ir pačioje Lietuvoje. Čia slopinant partizaninį karą, prasidėjus masiniams trėmimams, nusavinus nuosavybę steigiant kolūkius situacija tik blogėjo. Prievarta primestos sovietinės reformos skurdino ir kaimo, ir miesto gyventojus. Tuo metu pokarį Rytų Prūsijoje išgyvenę vokiečiai naudojosi atsiradusia galimybe ištrūkti į Rytų Vokietiją, o nacių okupaciją patyrusiame SSRS pakraštyje socialinė bei ekonominė situacija šiek tiek normalizavosi. Veikė nemažai atsakingų institucijų, o ir baudžiamųjų priemonių elgetavimui ir ypač valkataujančių nepilnamečių nusikalstamumui suvaldyti. This article presents an analysis of events in Lithuania in the years 1944 to 1947, when the Soviet authorities were forced to stem the large numbers of beggars, both children and adults, flooding into the country. The scale of the social problem is examined, as well as the reasons, the routes taken by beggars of various nationalities (Russian-speakers, Germans, Roma (Gypsies) and the local population), and the efforts taken by government institutions to use deportation as a way of managing the crisis associated with the spread of typhus. The author reviews the stereotypes, well established in historiography, alleging that only German children begged after the war, and that members of the Lithuanian population who took them in were therefore subject to repressions. Asmens sveikatos priežiūros įstaigų darbuotojai susiduria su sudėtingomis psichosocialinio darbo sąlygomis. Ypač daug įtampos ir streso profesinėje veikloje patiria bendrosios praktikos slaugytojai. Jie darbe susiduria ir su įvairiomis psichosocialinėmis problemomis: patiriama psichologinė ar net fizinė agresija, dėl didelio krūvio trūksta personalo darbuotojų, didelis slaugomų pacientų skaičius, darbas su sunkiais pacientais, neišvengiamas susidūrimas su slaugomų pacientų mirtimi, rizika užsikrėsti infekcinėmis ligomis ir pan. Straipsnyje pristatomo tyrimo tikslas – atskleisti bendrosios praktikos slaugytojų darbo vietos psichosocialinį klimatą. Tyrime dalyvavo 300 Kauno miesto asmens sveikatos įstaigose dirbančių bendrosios praktikos slaugytojų. Pasirinkta kiekybinio tyrimo strategija. Tyrimo duomenų rinkimui taikyta anketinė apklausa. Duomenų analizei naudota aprašomosios statistikos skaičiavimai ir statistinių duomenų apdorojimo programa „Microsoft Office Excel 2019“ programa. Tyrimo rezultatai atskleidė, kad dauguma slaugytojų darbe susiduria su veiksniais, turinčiais neigiamą poveikį jų fizinei sveikatai. Šie veiksniai susiję su dideliu darbo krūviu, sparčiu darbo tempu, keliamais aukštais reikalavimais. Penktadalis tyrimo dalyvių teigia dirbantys viršvalandžius. Didžioji dalis slaugytojų teigia dirbantys ergonomiškoje aplinkoje, tačiau trečdalis jų nėra patenkinti darbo aplinka – mažos palatos, senos, nefunkcionalios lovos ir pan. Daugiau nei pusė slaugytojų darbinėje veikloje yra turėję nesaugų kontaktą su biologinėmis medžiagomis, daugiau nei trečdalis jų yra patyrę neigiamą dezinfekcinių priemonių poveikį – rankų odos išsausėjimą, sutrūkinėjimą, kvėpavimo takų dirginimą, nosies užgulimą, čiaudulį ir akių graužimą. Labiausiai slaugytojams psichologinę įtampą keliantys veiksniai susiję su mirštančių ligonių slaugymu, bendravimu su pacientais ir jų artimaisiais, medicininės klaidos baimė, žemas darbo atlygis ir neužtikrintumas dėl ateities. Health care professionals face difficult psychosocial work conditions. By working intensively round-the-clock, they are expected to make the right decisions that affect the patient’s health, so the psychological and physical workload of health care staff is significantly higher than expressed. Particularly, it is general practice nurses who experience a lot of tension and stress in their professional activities. They face various psychosocial issues at work, including psychological or even physical aggression, high workload due to lack of staff, a large number of the patients to look after, treating difficult patients, the inevitable death of patients, and the risk of infectious diseases (Kaburi et al., 2019; Starc, 2018). The study aims to reveal the psychosocial climate of the workplace of general practice nurses. The objectives are to identify potential factors impacting nurses’ physical and psychological health. The study was conducted in July-August 2021. The research target group was 300 general practice nurses working in health care institutions in the city of Kaunas. An online questionnaire was used to collect the survey data. Descriptive statistics calculations and statistical data processing program Microsoft Office Excel 2019 were used for data analysis. The study results revealed that most nurses are affected by the factors having a negative impact on their physical health. They are related to the high workload, fast-paced work, and high demands. One-fifth of the survey participants say they work overtime. Most nurses note that they work in an ergonomic work environment. However, a third of the respondents are dissatisfied with the work environment because of the small wards and old, non-functional beds. More than half of nurses have had unsafe contact with biological agents at work, and more than a third have experienced adverse effects of disinfectants, such as dry skin, cracking, respiratory tract irritation, nasal congestion, sneezing, and eye corrosion. Nurses’ most stressful psychosocial factors include caring for dying patients, interacting with patients and their relatives, fear of medical errors, low salary, and uncertainty about the future.

Lietuviška spauda (knygos, laikraščiai, žurnalai, kalendoriai etc.) XX a. 1–4-uoju dešimtmečiais intensyviai švietė savo skaitytojus užkrečiamųjų ligų prevencijos ir gydymo klausimais. Vis dėlto nurodytu laikotarpiu dažnos įvairios epidemijos kelia klausimą, ar ši informacija pasiekdavo adresatą, ypač kai provincijoje, kaip rodo tautosakos šaltiniai, tebebuvo smarkiai kliaujamasi liaudies medicina. Straipsnyje šis klausimas sprendžiamas kaip pavyzdį pasirinkus choleros ligą. Lietuviškoje periodikoje ir specialiuose leidiniuose pateikti apsisaugojimo nuo šios ligos bei jos gydymo metodai yra lyginami su archyvuose surinktais liaudies medicinos tekstais. Analizuojant surinktą medžiagą, taip pat išryškinama liaudiškoji limpamos ligos samprata (taip tuo metu vadintos užkrečiamosios ligos), galėjusi turėti įtakos XX a. pirmosios pusės gydytojų rekomendacijų recepcijai plačiojoje to meto visuomenėje. In the first half of the 20th century, the number of people suffering from infectious diseases in Lithuanian society was high. Cholera, all variations of typhus (louse-borne typhus, relapsing fever, typhoid fever), smallpox, diphtheria, tuberculosis, scarlet fever, dysentery, influenza, malaria, venereal diseases and trachoma were the main infectious diseases that plagued the Lithuanian population during this period. These and several other illnesses were also classified as limpamos ligos (communicable diseases) by folklore collectors who were interested in folk medicine and compiled lists of diseases in the late 19th and early 20th centuries. The press of the period contains no shortage of information on how quickly these diseases spread, the reactions of those in the health system, and the behaviour of the community. However, the spread of the diseases was also due to other problems, such as a shortage of doctors, which was compensated for in the provinces by ‘traditional’ healers, such as spell-casters, herbalists, sorcerers, and the like. An attempt was made to bridge the gap between urban areas and the provinces with literature promoting medical knowledge. By 1922 alone, more than 200 publications had come out in Lithuanian. The books and brochures were not only aimed at health education and the importance of hygiene in the home, but also at informing people about various diseases, their causes, their causative agents, the course of illnesses, and methods of treatment. The aim of this paper is to examine closely the relationship between the descriptions and treatments of infectious diseases found in the press on one hand, and what is found in folk medicine on the other. To this end, the article chooses one disease, cholera, which is examined here as an example. In the early 20th century, this infectious disease, first described in Lithuania in 1831 and recorded until 1921, had already been responsible for several pandemic waves of high mortality worldwide. Various information publications had been published since the middle of the 19th century to inform the public and help them protect themselves from contracting cholera. They explained the causes, symptoms and course of the disease, recommended treatment, and gave advice to home carers and how to deal with the death of a sick person. When cholera epidemics broke out in urban areas, efforts were made to inform the public rapidly, isolate and quarantine those in contact with the sick, disinfect premises and courtyards, and initiate vaccination against the disease. However, according to Lithuanian folklore records from the first half of the 20th century, in Lithuanian villages, cholera was usually treated with plants such as garden angelica (Angelica archangelica), popularly known as dzingelis or cingelis in Lithuanian. The root of the plant was soaked in vodka for the relief of the symptoms, crushed leaves were scattered on the streets, and pieces of the root were worn as amulets. In fact, wild angelica (Angelica sylvestris) may also have been called dzingelis. Bulbous chervil (Chaerophyllum bulbosum), another plant similar to garden angelica and wild angelica, is also mentioned in folklore records as an effective treatment for cholera. People also used blueberries. In addition, other plants are mentioned which are difficult to identify. Sometimes dangerous, even poisonous substances, such as carbolic acid, may also have been used to treat cholera. There are several possible reasons why the information given in the press in the first half of the 20th century on how to prevent cholera and folk medical knowledge recorded at a similar time do not overlap. Firstly, the lack of doctors in the provinces may have limited the dissemination of up-to-date information in rural communities. Secondly, economic inequality prevented some from seeking professional help. Thirdly, the dissemination of medical knowledge may have been hampered by literacy levels: according to the first general population census in 1923, the literacy rate in Lithuania was 35.1% of the total population, while the semi-literacy rate (those who could only read) was 20.8%, thus the total literacy rate was 55.9% (in 1897, the literacy rate plus the semi-literacy rate was 41.9%). Therefore, although useful information was being printed and disseminated, it was not accessible to everyone. Fourthly, there was a difference between concepts of illness, and more specifically, a difference between medical systems, official and populist, whereby illness was perceived as a sovereign being in the popular imagination, occupying a high place in the hierarchy of mythological beings. That this attitude persisted into the first half of the 20th century is evidenced by beliefs recorded in the period between the two world wars. The continued association of disease with the supernatural is also evidenced by the fact that even during the epidemics of the Second World War, crosses continued to be erected and special cloths continued to be woven, a method that was used until the 18th century to defend against the plague, and was later adapted to cholera. It could be argued that, for all the above-mentioned reasons, folklore records do not reflect the transfer of information from the press. Asmens sveikatos priežiūra - viena iš svarbiausių visuomenės sričių, apimanti itin jautrią ir asmeninę informaciją apie asmenį. Pandemijos metu sveikatos priežiūros paslaugas pradėjus teikti nuotoliniu būdu, pastebėta, kad tokio pobūdžio paslaugų teikimas Lietuvoje - pakankamai nauja sritis ir mažai nagrinėta mokslinėje literatūroje. Pagrindinė problema, su kuria susiduriama nagrinėjant nuotolinių asmens sveikatos priežiūros paslaugos atitiktį asmens duomenų apsaugos reglamentui - asmens duomenų ir kitos konfidencialios informacijos, susijusios su sveikata, saugumas. Straipsnyje analizuojama, kaip užtikrinti, kad, teikiant nuotolines asmens sveikatos priežiūros paslaugas, nebūtų pažeidžiami asmens duomenų apsaugos reglamento keliami reikalavimai. Siekiant užsibrėžto tikslo, vertinama nuotolinės asmens sveikatos priežiūros paslaugos samprata, požymiai bei teisinis reglamentavimas Lietuvoje ir pasirinktose užsienio valstybėse. Taip pat nagrinėjama asmens sveikatos duomenų samprata ir požymiai, siekiant atskleisti, kas konkrečiai patenka į šias sritis bei kaip reglamentuojama tokių duomenų apsauga. Išanalizavus teisės aktus, teismų praktiką, mokslinę literatūrą, Lietuvoje esantį teisinį reglamentavimą, įvertinus nuotolinės asmens sveikatos priežiūros paslaugos ir asmens duomenų sampratas bei išnagrinėjus asmens duomenų apsaugai keliamus reikalavimus, nustatyta, kad Lietuvoje teisinis reglamentavimas iš esmės atitinka asmens duomenų apsaugos reikalavimus, tačiau teisės aktuose nedetalizuojami reikalavimai paciento sutikimo formai, kas atskirais atvejais gali pažeisti asmens duomenų apsaugos reikalavimus. Teisiniame reguliavime paliekama didelė diskrecija sveikatos priežiūros įstaigoms pasirinkti asmens identifikavimo būdus, dėl ko kyla rizika netinkamai nustatyti asmens tapatybę ir pažeisti asmens duomenų saugumą ir konfidencialumą. Bent vienos sveikatos priežiūros įstaigos tvarkoje nurodoma, kad nuotolinės konsultacijos privalo būti įrašomos, kai tokius veiksmus draudžia NGT4 - tvarka turėtų būti koreguojama. Personal healthcare is one of the most important areas of the society that involves highly sensitive and personal information about an individual. The introduction of remote healthcare services (telemedicine) during the pandemic revealed that remote provision of the services of such type in Lithuania was a relatively new area that had not been adequately addressed in the scientific literature. The main issue encountered in the analysis of the compliance of telemedicine services with the General Data Protection Regulation (GDPR) is the security of personal data and other confidential health-related information. The article analyses the ways to prevent breaches of the requirements set by the General Data Protection Regulation in the context of provision of the telemedicine services. The concept, features, and legal regulation of telemedicine services in Lithuania and selected foreign countries are assessed to achieve the goal of the study. In addition, the concept and features of personal health data are examined with a view to revealing what exactly falls within these areas and how the protection of such data is regulated. The analysis of legislation, case-law, scientific literature, legal framework applicable in Lithuania, the assessment of the concepts of telemedical services and personal data and the examination of the requirements for the protection of personal data revealed that the legal framework in Lithuania fundamentally complies with the requirements of personal data protection, however, the legal acts do not specify the requirements for the patients consent form, which in individual cases may break the requirements of personal data protection. The legal framework leaves considerable discretion for health care institutions to choose methods of personal identification, which creates the risk of improper identification of a person and violate the security and confidentiality of the personal data. The procedure of at least one health care institution states that remote consultations must be recorded when such actions are prohibited by the order of the Ministry of Health of the Republic of Lithuania - the procedure should be adjusted. Kintant ir plečiantis požiūriui į mokymosi aplinkos kūrimą, kaip vieną esminių mokymosi proceso organizavimo kokybės veiksnių, straipsnyje atskleidžiamos sąsajos, pagrindžiančios andragogo bendrųjų kompetencijų svarbą kuriant transformuojančią mokymosi aplinką. Mokslinės problemos aktualumą išryškina tyrimo teorinės ir empirinės andragogo bendrųjų kompetencijų, sudarančių palankias prielaidas besimokančiojo įgalinimo asmenybės transformacijai, paieškos. Tyrimo dalyviai – įvairių Lietuvos regionų sveikatos priežiūros įstaigų darbuotojai, dalyvavę kvalifikacijos tobulinimo renginiuose Klaipėdos universiteto Socialinių ir humanitarinių mokslų fakulteto Tęstinių studijų centre. Sveikatos priežiūros specialistų požiūrio analizė dėl andragogo bendrųjų kompetencijų poreikio kuriant transformuojančią mokymosi aplinką išryškino tiesioginę amžiaus rodiklio įtaką skirtingiems poreikiams. Tyrimo rezultatai atskleidė, kad andragogo bendrosios kompetencijos padeda kurti transformuojančią mokymosi aplinką, kurioje intelektinės, emocinės ir praktinės dinamikos visuma įgalina besimokančiojo transformaciją, energinį potencialą nukreipiant į su asmenybės augimu, vystymusi ir veiklos tobulinimu susijusių asmeninių savybių, gebėjimų, įgūdžių, nuostatų bei pozityvaus požiūrio ir nusiteikimo nuolat tobulėti plėtojimą. Pateikiama siūlymų andragogams, kaip veiksmingai skatinti besimokančių suaugusiųjų pokyčius, nukreiptus į asmenybės kaitą, kuriant transformuojančią mokymosi aplinką. With the changing and expanding approach to the creation of a learning environment as one of the essential factors in the quality of the organisation of the learning process, the article reveals the links that underpin the importance of an andragogue’s general competencies for the development of a transformative learning environment. The relevance of the scientific problem is highlighted by the theoretical and empirical search for the general competencies of the andragogue, which creates favourable preconditions for the transformation of the learner’s empowerment personality. The participants in the research were employees of health-care institutions in various regions of Lithuania who have participated in in-service training events at the Centre for Continuing Studies of the Faculty of Social Sciences and Humanities at Klaipėda University. The analysis of the need for health-care professionals due to the importance of an andragogue’s general competencies in creating a transformative learning environment has highlighted the direct impact of age on different needs. The results of the research show that an andragogue’s general competencies help to create a transformative learning environment, in which the totality of intellectual, emotional and practical dynamics enables the learner’s transformation, focusing on personal qualities, abilities, skills, attitudes and positive attitudes, and the development of a mindset for continuous improvement. Suggestions are provided to andragogues on how to streamline changes in adult learning that focus on personality change by creating a transformative learning environment.

Pastarųjų metų įvykiai – COVID-19 pandemija, tarptautiniai kariniai konfliktai – išryškino susiformavusių ir nusistovėjusių sveikatos sistemų struktūrinius, organizacinius, technologinius trūkumus bei parodė, kad sveikatos priežiūros paslaugų teikimo ateitis priklauso nuo asmens sveikatos priežiūros įstaigų gebėjimo dinamiškai reaguoti į nenumatytus ekstremalius įvykius ir kuo sklandžiau veikti naujomis sąlygomis. Straipsnyje analizuojama kritinėms situacijoms, kurias gali sukelti gamtos jėgos (žemės drebėjimai, klimato atšilimo pokyčiai, pandemijos) arba žmonių pastangos (kariniai konfliktai, kibernetinės atakos ir pan.), atsparios asmens sveikatos priežiūros įstaigos samprata, aptariant teorines įžvalgas moksliniuose straipsniuose dėl atsparumo sąvokos, jos sampratos elementų, atsparumo didinimo priemonių. Tyrimo problema: siekiant atskleisti asmens sveikatos priežiūros įstaigos atsparumo kritinėms situacijoms koncepciją, tikslinga apibrėžti nelaimėms atsparios ligoninės sąvoką, jos elementus bei nurodyti galimas atsparumo stiprinimo priemones. Tyrimo objektas – nelaimėms atsparios asmens sveikatos priežiūros įstaigos samprata. Tyrimo tikslas – identifikuoti nelaimėms atsparios asmens sveikatos priežiūros įstaigos sampratą ir jos elementus. Tyrimo uždaviniai: 1) apibrėžti mokslinėje literatūroje vartojamą nelaimėms atsparios asmens sveikatos priežiūros įstaigos sąvoką; 2) nustatyti reikšmingus šios sampratos elementus; 3) nustatyti asmens sveikatos priežiūros įstaigos atsparumą didinančias priemones. Tyrimo metodai: problemai tirti taikyta sisteminė ir lyginamoji mokslinės literatūros turinio analizė. Atliekant tyrimą apibrėžiama asmens sveikatos priežiūros įstaigų atsparumo sąvoka, mokslininkų požiūriai, sąvokos turinio elementai, nurodomos atsparumo didinimo priemonės. Padarytos esminės išvados, kad nelaimėms atsparios asmens sveikatos priežiūros įstaigos sąvoka apima dvipolį įstaigos (sveikatos priežiūros įstaiga apima sveikatos paslaugų teikimą, sveikatos priežiūros darbuotojus, sveikatos informacijos valdymo sistemą, medicinos produktus, sveikatos finansavimą ir sveikatos vadovavimą bei valdymą) gebėjimą minimaliomis išteklių sąnaudomis išlaikyti funkcionalumą ir sumažinti šoko tikimybę, ištikus įvairioms nelaimėms, greitai atsigauti po nelaimės sukelto šoko, patirtų nuostolių, atkuriant pradinę būseną arba prisitaikant prie naujos susiklosčiusios situacijos. Mokslinėje literatūroje nustatyti bendri organizacijų atsparumui būdingi tipai: konstrukcinis, infrastruktūrinis ir administracinis; atsparumo raiškos lygiai, priklausantys nuo konteksto, sutrikdymo, gebėjimo susidoroti su sutrikdymu ir reakcijos į sutrikdymą. Asmens sveikatos priežiūros įstaigos atsparumui priskiriamos spartos, tvirtumo, pertekliškumo, išradingumo, sąmoningumo, įvairovės, savireguliacijos, integruotumo, prisitaikymo ypatybės, kurios gali būti reikšmingos skirtingose nelaimės fazėse: iki pavojaus, ištikus pavojui ir atsigavimo. Nuo nelaimės tipo (žemės drebėjimas, klimato pokyčiai, pandemijos) priklauso ir asmens sveikatos priežiūros įstaigų atsparumo didinimo priemonės. Siekiant didinti asmens sveikatos priežiūros įstaigos atsparumą žemės drebėjimams, didžiausias dėmesys turėtų būti skiriamas pastatų ir komunalinių paslaugų infrastruktūrai, užkrečiamųjų ligų protrūkių atvejais – personalo mokymo, paslaugų saugumo užtikrinimo klausimams. Atsižvelgiant į tai, asmens sveikatos priežiūros įstaigos atsparumo nelaimėms temą ateityje būtina plėtoti, skiriant pagal skirtingo pobūdžio nelaimes. This article analyses the concept of hospital resilience to natural disasters (earthquakes, global warming, pandemics, and man-made disasters such as war, conflict and cyber-attacks) in the context of theoretical insights in scholarly articles on the concept of resilience, and elements of its perception and other aspects. The research conducted identified that the concept of a resilient hospital encompasses its ability to maintain functionality at minimal resource costs, and reduce the likelihood of shock in the event of various disasters. This capacity to withstand hazard can be of various types, for example, constructive, infrastructural or administrative. The following four elements are most commonly identified levels of resilience: context, disturbance, capacity to deal with disturbance, and reaction to disturbance. A resilient hospital maintains the following characteristics: rapidity, robustness, redundancy, resourcefulness, awareness, diversity, self-regulation, unity and adaptiveness. A variety of means are used to amplify resilience to distinct disasters and it is relevant to manage different types of resilience, but the human factor is indisputably essential in this framework. Sveikatos priežiūros kokybė, ypač ligoninėse – pagrindinis sveikatos priežiūros sistemų veiklos tikslas. Jis tampa dar aktualesnis COVID-19 pandemijos metu. Tyrimo tikslas – apžvelgti, įvertinti ir palyginti sveikatos priežiūros kokybę iki COVID-19 pandemijos ir pandemijos laikotarpiais ligoninėse. Atliekant tyrimą, analizuotos įvairių šalių autorių mokslinės publikacijos, apklausti pacientai, gydęsi ikikovidiniu ir pokovidiniu laikotarpiais ligoninėje. Respondentai vertino sveikatos priežiūros ligoninėje kokybę. Nustatyta, kad daugelio šalių ligoninėse sveikatos priežiūros kokybė COVID-19 pandemijos metu pablogėjo. Pagal kai kuriuos rodiklius sveikatos priežiūros kokybė, pacientų nuomone, kiek pablogėjo ir tirtoje ligoninėje. Absoliuti dauguma pacientų sveikatos priežiūros kokybę tiek ikikovidiniu, tiek COVID-19 pandemijos metu ligoninėje vertino gerai ir labai gerai. It becomes even more relevant during a COVID-19 pandemic. The aim of this article – to review, evaluate and compare prepandemic and COVID-19 pandemic health care quality in hospitals. Material and methods. Scientific publications of authors from various countries were analyzed during the research. The to patients who were treated in the Klaipeda university hospital and filled out the assessment of health care and its quality questionnaire in 2018-2019 years (69365) and 2020-2021 years (40514) were surveyed. The research data were analyzed and compared in two periods: pre-pandemic and COVID-19 pandemic. Analysis of statistical data was done using software SPSS version 23. Results. It has been found that the quality of healthcare in hospitals in most countries has deteriorated during the COVID-19 pandemic. In the opinion of in patients, according to some indicators the quality of health care has deteriorated slightly in the studied hospital. Conclusions. The causes of negative changes in health care quality during the COVID-19 pandemic are the deterioration of hospital financing, changes in the behavior of health care workers and patients, psychological and emotional stress, increased workloads, worsening working conditions, external pressure on healthcare workers from politicians and patients, and others. The absolute majority of surveyed patients assessed the health care quality in the hospital during the pre- pandemic and COVID- 19 pandemic periods as good and very good. Slight negative changes in some health care quality indicators in the hospital, in the opinion of patients, reflect trends in the world and European countries and are related to common causes caused by the COVID-19 pandemic, primarily to changes in patient behavior. Vaiko negalė yra išbandymas visiems šeimos nariams. Vakarų šalyse socialinės, medicininės ir edukologinės pagalbos negalei sistema gyvuoja jau ne vieną dešimtmetį. Lietuvoje sukurta reikiama teisinė bazė, tačiau tokio pobūdžio pagalbos tradicijos tik formuojasi. Vienas veiksmingiausių būdų, gerinantis tėvų, artimųjų fizinę ir emocinę sveikatą, padedantis mažinti nuovargį, stresą yra konsultavimas ir psichosocialinio pobūdžio parama. Vienas iš efektyviausių metodų, leidžiančių pasiekti emocinės ir fizinės pusiausvyros, yra panašaus pobūdžio gyvenimo išbandymus patiriančių asmenų savitarpio pagalbos grupės. 2018-2019 m. Kauno kolegija kartu su Kauno rajono socialinių paslaugų centru įgyvendino Visuomenės sveikatos stiprinimo fondo finansuotą projektą „Vaikų, turinčių kompleksinę negalią, sveikatos stiprinimas fiziniu ir psichosocialiniu požiūriu“. Projektu siekta prisidėti prie negalią turinčių vaikų ir jų artimųjų fizinės ir emocinės sveikatos gerinimo, socialinės įtraukties didinimo. Projekto veiklos rūšys aprėpė neįgalių vaikų tėvų mokymą ir konsultavimą fizinio aktyvumo stiprinimo, sveikatos priežiūros klausimais, edukacines keleto dienų stovyklas, artimųjų savitarpio pagalbos grupių organizavimą. Projekto tikslinė grupė – negalią turintys vaikai (N=60) ir jų tėvai, broliai ar seserys (N=120). Įgyvendinant projektą, remtasi socialine paradigma „Kokie yra vaiko sveikatos šaltiniai?“ „Kaip juos stiprinti?“ Vaiko sunkumai buvo sprendžiami orientuojantis į jo galias, gebėjimus, panaudojant ne tik jo, bet ir jį supančios aplinkos išteklius. Tyrimu siekta atskleisti tėvų nuomonę apie projekto veiklos rūšių bei taikytų priemonių poreikį ir naudą neįgaliems vaikams ir patiems tėvams. Tyrime dalyvavo 54 tėvai, auginantys negalią turinčius vaikus. Tyrimo tikslui pasiekti vykdyta apklausa raštu. Klausimai suskirstyti į uždaro ir atviro tipo (kokybinius). Tyrimo rezultatai atskleidė palankų visų projekto veiklos rūšių vertinimą. Tėvai nurodė, kad dalyvaudami projekte jie įgijo naudingų žinių vaikų burnos higienos, mitybos, pirmosios medicinos pagalbos, lytinio brendimo ir kt. klausimais, patobulino gastrostomų, tracheostomų priežiūros įgūdžius, išmoko masažo pagrindų. Tėvai taip pat įžvelgė psichologinę ir socialinę dalyvavimo projekte naudą. Jie teigė pagerinę emocinę sveikatą bei patobulinę bendravimo su vaikais ir kitais šeimos nariais įgūdžius. Child disability is a challenge for all family members. In Western countries the system of social, medical and educational assistance for disability has been in existence for decades. Although the necessary legal framework is developed in Lithuania, this type of assistance is still shaping its traditions. Counseling and psychosocial support is one of the most effective ways to improve the physical health of parents, relatives and relieve fatigue. The most effective methods for achieving an emotional, physical balance are support groups that involve individuals with similar life challenges. In 2018 - 2019 the project “Improving the physical and psychosocial health of children with complex disabilities” (funded by the Ministry of Health of the Republic of Lithuania, Public Health Promotion Fund) was implemented by the Faculty of Medicine, Kaunas University of Applied Sciences together with Kaunas District Social Services Center. The project was aimed to contribute to the improvement of the physical and emotional health and social inclusion of children with disabilities and their relatives. Project activities included parents’ teaching and counseling on enhancement and health care of disabled children, educational camps, organization of parents supporting groups. The project target group - children with disabilities (N = 60) and their parents, brothers / sisters (N = 120). The age of the children was from 2 to 17 years old. The main disabilities of the children were: autism spectrum disorder, Down syndrome, paralysis, myasthenia, rare genetic diseases. The project was based on the social paradigm “What are the child’s health sources?” “How to strengthen them?”. In this way, the child’s difficulties were solved by focusing on the child’s powers and abilities, using not only his or her resources but also the surrounding environment. The study aimed to reveal parents’ views on the need and benefits of the activities and measures implemented in the project for their children with disabilities and for themselves. The study included 54 parents of children with disabilities. To achieve the research objective a survey including closed and open type (qualitative) questions was conducted. The results of the survey revealed a favorable evaluation of all the activities carried out in the project. Parents claim that by participating in the project activities they have acquired useful knowledge in children’s oral hygiene, nutrition, first aid, sexual maturation, improved gastrostomy, tracheostomy care skills, learned the basics of massage. Parents also see the psychological and social benefits of participating in the project. They claim to have improved emotional health and communication skills with children and other family members.

Straipsnyje, remiantis mokslinės literatūros apžvalga bei dokumentų analize nagrinėjama, kokia visuomenės dalyvavimo su sveikata susijusių sprendimų priėmime samprata buvo formuluojama pagrindiniuose sveikatos politikos dokumentuose pirmąjį dešimtmetį po Lietuvos Nepriklausomybės atkūrimo. Straipsnyje analizuojama 16 sveikatos politikos dokumentų: Nacionalinė sveikatos koncepcija (1991), Lietuvos sveikatos programa (1998) ir 1994–2002 m. priimti sveikatos sistemą reglamentavę įstatymai, kuriuose kalbama apie visuomenės dalyvavimą. Daromos išvados, jog nors visuomenės dalyvavimo svarba pagrindiniuose sveikatos politikos dokumentuose buvo pabrėžiama, tačiau galimybės, kaip visuomenė konkrečiai galėtų įsitraukti į su sveikatos priežiūra susijusių sprendimų priėmimą ir ypač į sveikatos politikos procesus, daugumoje dokumentų nebuvo aiškiai apibrėžtos. Daugelyje nacionalinės svarbos sveikatos politikos dokumentų visuomenė matoma kaip poveikio objektas, dominuoja dalyvavimo kaip pacientų įsitraukimo priimant savo sveikatai svarbius sprendimus samprata. The article focuses on the concept of public participation in key national health policy documents that were adopted during the first decade of Lithuania’s Independence (1991–2004). Seventeen health policy documents are analyzed: National Health Conception (1991), Lithuanian Health Program (1998) and major health system related laws that defined public participation. It is concluded that although the importance of public participation in key health policies has been highlighted, the possibilities for public involvement in health policy or health and healthcare decision-making in most of these documents were not clearly defined. Public participation is often seen not as involvement of the public in the processes of shaping or implementing health policies, but as participation of patients in their healthcare through granting them certain rights to make personally significant decisions. The public is also often treated as the object in health or healthcare-related decision-making.

Purpose Knowledge sharing across health ecosystems is extremely fuzzy because of knowledge asymmetries, barriers and diverse types and sources of knowledge, all of which together affect patient decision making and value creation. The purpose of this study is to identify core knowledge mediators across ecosystem with the focus on a patient as a central decision maker in their own health management to ensure smooth knowledge flows across actors. Design/methodology/approach To understand the knowledge flows in the health ecosystem, a phenomenological approach was applied in this study. Based on case study research. The analysis is based on the patient-centric approach and draws on qualitative, semi-structured interviews. Moreover, a knowledge-creating community approach (Paavola et al., 2004) is applied in which various stakeholders create and share knowledge of clinical and social domain, which together contribute to patient value creation. Findings Knowledge socialization and development starts within very close and trusted community members. Trust, validity, reliability and responsibility of knowledge have emerged as full mediators for knowledge absorption. Thus, health communities and knowledge ecosystems need safe places for “unverified” knowledge to ensure that the important trends and unresolved questions are not missed. Originality/value This study proposes a new health knowledge management approach for communities, which is more than clinical decisions and formal medical knowledge and embraces varieties of knowledge and information sources and types. At the end, the identified barriers and mediators can be used for serving the main goal of patient value increase because it responds to the need for a systematic approach in encouraging patients to play a more active role in their own health management.

Darbo tikslas. Įvertinti optometrinės priežiūros paslaugų organizavimo ir prieinamumo Lietuvoje ypatumus. Darbo metodika. Tyrimui atlikti naudotas instrumentas – elektroninis klausimynas, į kurį respondentams buvo išsiųsta nuoroda elektroniniu paštu. Tiriamųjų imtį (134 respondentai) sudarė optometrininkai, registruoti Lietuvos optometrininkų asociacijoje. Iš viso buvo užpildytos ir pateiktos analizei 86 respondentų anketos, t.y. grįžtamumas sudarė 64,2%. Pusės respondentų (48,8%) nurodytas darbo stažas buvo 6– 10 m. Daugumos respondentų (82,6%) nurodyta pagrindinė darbo vieta – didmiestis (Vilnius, Kaunas, Klaipėda), o daugiau nei 95% – teikia paslaugas privačiame sektoriuje (t.y. optikos salonuose). Asmens sveikatos priežiūros įstaigų žalos Privalomojo sveikatos draudimo fondo biudžetui nustatymo ir įrodinėjimo ginčuose tarp teritorinių ligonių kasų ir asmens sveikatos priežiūros įstaigų tema moksliniame lygmenyje nagrinėjama mažai. Dėl mišraus santykių tarp teritorinių ligonių kasų ir asmens sveikatos priežiūros įstaigų reglamentavimo, praktikoje ilgą laiką buvo neaiškus šių ginčų nagrinėjimo procesas. Straipsnyje, remiantis teisės aktų nuostatomis, išsamia teismų praktikos analize, siekiama išanalizuoti teritorinių ligonių kasų patikrinimo dokumentų teisinę reikšmę ir su ja susijusius civilinės atsakomybės kilimo sąlygų įrodinėjimo aspektus. Object. The object of analysis here of is the legal value of final documents of control procedures, i. e. expert reports of the territorial health insurance funds and the aspects concerning the settlement of legal disputes; the latter aspects are determined in the legal acts and the case law of the Republic of Lithuania. Methodology. In order to achieve the goal, the essential features of relations between the territorial health insurance funds and personal health care institutions are distinguished, the legal value of inspection certificate of territorial health insurance funds in the legal acts of the Republic of Lithuania is analysed, and the systematic analysis of judicial practice in Lithuania is covered. The method of qualitative content analysis is used in the article. Conclusions. The relationship between the territorial health insurance funds and personal health care institutions implies the composite character. The final document of control procedure, i. e. an expert report accepted by the territorial health insurance funds is neither an administrative act nor an expert’s opinion. It is a statement of facts. Expert reports submitted to the courts by the territorial health insurance funds are judged on together with other evidence presented in the case. To be awarded an indemnification of CHIF budget, the territorial health insurance fund must address the court of general jurisdiction to prove the conditions of civil liability in accordance with the Civil Code of the Republic of Lithuania and the Code of Civil Procedure of the Republic of Lithuania. A personal health care institution which challenges civil liability conditions set out in the expert report has the right and duty to provide evidence including the findings of independent experts, to deny the non-existence of unlawful actions or actual damage to the CHIF budget.

Background: Given that approximately one-third of individuals with coronary artery disease (CAD) remain severely fatigued after completion the cardiac rehabilitation, it is necessary to identify reliable intervention targets aimed at reducing fatigue. Perceived social support is closely linked to health outcomes and depressive symptoms in individuals with CAD. However, to our knowledge, the relationship between subjective fatigue levels and social support in those with CAD has not been analyzed.Objective: We aimed to examine the associations between perceived social support and subjective fatigue levels in individuals with CAD with and without depression symptoms.Methods: This cross-sectional study was comprised of 1,036 participants with CAD (57±9years, 77% men) 1–2weeks after acute coronary syndrome (ACS). Participants completed the Hospital Anxiety and Depression scale (HADS), Multidimensional Fatigue Inventory-20 (MFI-20), and the Multidimensional Scale of Perceived Social Support (MSPSS).Results: In total, 12% (n=129) of study participants had elevated depression symptoms (HADS score≥8). In individuals with CAD and depressive symptoms, after adjustment for sex, age, New York Heart Association (NYHA) functional class, and anxiety, linear regression analyses showed significant inverse associations between higher social support from others and general, physical fatigue as well as reduced activity and motivation (pp’sp’sppConclusion: The results of this study suggest that fatigue and its features could be associated by the perceived social support in individuals with CAD following ACSs. While in individuals with CAD and depressive symptoms, greater subjective fatigue is associated with less perceived social support from others, higher levels of subjective fatigue in non-depressed individuals with CAD are significantly associated with reduced perceived social support from friends.

In the setting of disasters, people seek information as they hope that knowledge will provide security. This makes the media a critical source of crisis exposure. The aim of the study described in this article was to analyze COVID-19 pandemic-related psychological aspects of media use by healthcare and pharmacy workers in Lithuania and to reveal the subjective effects of media consumption on their psychological well-being. 967 healthcare workers and pharmacists in Lithuania completed an electronic survey, which consisted of questions about the changes in well-being experienced since the beginning of the lockdown and media use in the search for information on COVID-19. It was found that communication might have ambiguous effects on psychological well-being. Excessive, unreliable, misleading, contradictory information and “catastrophizing” were subjectively related to impaired psychological well-being. Objective and reliable, relevant, clear, timely, hopeful and supportive information had a subjective positive effect. Seeking COVID-19-related information many times a day was associated with increased fear of becoming infected with COVID-19, feeling unable to control the risk of contracting COVID-19, fear of infecting relatives with COVID-19 and feeling that other people would avoid interaction with healthcare workers and pharmacists because of their job. General browsing was not consistently associated with COVID-19-related fears.

Aim: The aim of the present study was to describe the association between primary healthcare (PHC) nurses' perceived stress and organizational culture and climate in a team context. Design: A descriptive, cross-sectional correlation study. Methods: 29 teams in 18 PHC centers situated in one Lithuanian county participated in the study. A total of 187 nurses completed the Expanded Nursing Stress Scale, while 344 healthcare professionals (including nurses) responded to the Organizational Social Context questionnaire. Results: Absolute values of Spearman's rho correlation coefficients between 0.19-0.24 (p < 0.01) were found when examining the association of nurses' stress with organizational culture and climate across PHC teams. Whether the culture was resistant or proficient seemed to have associations with some of the stress factors experienced by nurses. Additionally, how the whole team reported stress or functionality in their organizational climate seemed to be associated with perceived stress of nurses. Conclusion: Investigating the organizational culture and climate experienced by PHC teams may help identify manageable problems and decrease stress experienced by nurses. The study also allows the possibility of nursing scholars using the study's designs and instruments for further investigation of teams.

Background Migraine is a primary headache disorder which affects all aspects of life. The financial burden of migraine imposed on the society might be substantial. This study aims at estimating the economic cost of migraine in Latvia and Lithuania, including both direct and indirect costs. Direct costs encompass the costs of migraine-related health care resource utilization. Indirect costs are related to productivity loss, the potential or expected earnings lost due to migraine. Methods Direct cost is assessed by using the prevalence method, a widely used cost-of-illness approach. The prevalence rate of migraine and the migraine-related health care resource utilization are proxied from the literature, whereas unit cost of medical services and procedures are retrieved from national databases and providers. For estimating the indirect cost of migraine, we follow the human capital approach. We quantify three components of indirect costs: reduced labour force participation, absence from work and reduced productivity while at work. The number of unemployed migraineurs, days missed from work and days lost due to impairment while at work are drawn from the literature. Unemployment rate and average income in Latvia and Lithuania are then inserted to assess indirect costs. Results We find that the mean per-person total cost of migraine is €801 annually in Latvia, and €721 in Lithuania. In both countries around 30% of total cost is direct cost; cost related to a wide array of migraine-related medical services and interventions. The total cost of migraine is €112.26 million in Latvia, corresponding to 0.42% of Latvia’s GDP. The total cost of migraine is €149.62 million in Lithuania, corresponding to 0.35% of Lithuania’s GDP. In both countries two thirds of total cost is related to lost workdays due to absenteeism and presenteeism. Conclusions The financial burden of migraine imposed on the society is substantial in Latvia and Lithuania. Improvements in care for patients with migraine, such as easier access to structured headache assessment services, wider availability of various procedures and preventive medications would significantly increase direct costs. Nevertheless, this cost increase might be far outweighed by lower migraine-related productivity loss, especially as the prevalence of migraine is the highest in the most productive years of life.

Akušerijos, ginekologijos ir neonatologijos paslaugas teikiančiose gydymo įstaigose gydomos labai pažeidžiamos pacientų grupės, todėl ypač svarbu užtikrinti puikią sveikatos priežiūros kokybę ir pacientų saugumą. Nuo organizacinės kultūros gydymo įstaigoje tipo priklauso organizacijos galimybės tobulėti, pasiekti geresnių rezultatų, daryti pokyčius, diegti naujas metodikas. Organizacinės kultūros įvertinimas šiose gydymo įstaigose padėtų suprasti ir pagerinti pacientų saugumą, jų pasitenkinimą sveikatos priežiūros paslaugomis, sveikatos priežiūros įstaigų veiklos efektyvumą. Darbo tikslas: nustatyti organizacijos kultūrą akušerijos, ginekologijos ir neonatologijos paslaugas teikiančioje gydymo įstaigoje, vertinant slaugytojų ir akušerių nuomonę. Tyrimo metodika. Naudojant organizacijos kultūros vertinimo klausimyną (OCAI) buvo vertinama slaugytojų ir akušerių nuomonė apie organizacinę kultūrą. Atsako dažnis 100 % (n=233. Rezultatai. Tiriamuoju laikotarpiu vyrauja hierarchijos kultūra. Analizuodami pagrindinių charakteristikų, organizacijos lyderytės, organizacijos vienybės, strateginių akcentų komponentus nustatėme, kad tiriamuoju laikotarpiu akušerijos, ginekologijos ir neonatologijos skyriuose vyrauja įvairūs kultūros tipai, dominuojant hierarchinei (D) kultūrai. Analizuodami darbuotojų administravimo ir sekmės kriterijų komponentus, nustatėme kad tiriamuoju laikotarpiu vyrauja įvairūs kultūros tipai, dominuojant Klano (A) kultūrai. Komponentų pagrindinių charakteristikų, organizacijos lyderytės, strateginių akcentų ir sėkmės kriterijų hierarchinės kultūros teiginio "norimas" ir "dabar" vidurkių pokyčiai buvo reikšmingai didesni naujagimių skyriuose. Išvados. Slaugos personalo nuomone, akušerijos, ginekologijos ir neonatologijos paslaugas teikiančioje gydymo įstaigoje dominuoja hierarchinės kultūros tipas. Tačiau respondentai vertintų tendenciją į draugiškesnę (klano kultūra) ir kūrybiškesnę (inovacijų kultūra) darbo aplinką. Background. Institutions that provide services in the fields of obstetrics-gynecology and neonatology treat highly vulnerable patient groups, and thus assurance of the quality of healthcare and the safety of the patients is of utmost importance. The type of the organizational culture at a healthcare institution determines the organization’s abilities to develop, achieve better results, initiate changes, and implement new methods. The evaluation of organizational culture in such institutions would help to understand and improve patient safety and satisfaction with the healthcare services as well as the effectiveness of the activity of healthcare institutions. The aim of the study was to identify the type of the organizational culture at a healthcare institution that provides services in the fields of obstetrics-gynecology and neonatology based on the evaluation of nurses’ and midwives’ opinions. Summary of the work. The evaluation of the nurses’ and midwives’ opinions about the organizational culture was performed by using the Organizational Culture Assessment Instrument (OCAI). The overall response rate to the survey was 100% (n=233). Summary of the results. The results of the study showed that during the studied period, the Hierarchy culture predominated. The analysis of the components Dominant Characteristics, Organizational Leadership, Organization Glue, and Strategic Emphases revealed the presence of various types of organizational culture, with the Hierarchy culture (D) being dominant. The analysis of the components Management of Employees and Criteria of Success showed that during the studied period, various culture types existed in the studied institution, with the Clan culture (A) being dominant. In the Hierarchy culture, the analysis of the components Dominant Characteristics, Organizational Leadership, Strategic Emphases, and Criteria of Success revealed that significantly greater differences between the Preferred and Now mean evaluations were in the neonatal departments. Conclusion. According to the nursing staff, the Hierarchy culture type was predominant in the studied healthcare institution that provides services in the fields of obstetrics-gynecology and neonatology. However, the respondents also would prefer a trend towards a friendlier (the Clan culture) and more creative (the Adhocracy culture) work environment.

Tyrimo tikslas - aptarti teorinius ir praktinius pacientų edukacijos aspektus kaip integralią krūties vėžiu sergančių pacienčių sveikatos priežiūros sąlygą. Mokslinės literatūros studijos atskleidė, kad pacientų, sergančių navikinėmis ligomis, edukacija yra labai aktuali problema visuomenėje. Onkologiniams ligoniams būdingų fizinių, socialinių ir psichologinių problemų kontekste informacijos poreikis apie ligą ir pagalbos galimybes yra vienas iš svarbiausių. Ypatingas dėmesys turėtų būti skiriamas krūties vėžiu sergančioms moterims, kadangi tai dažniausiai diagnozuojama jų onkologinė liga, ir moterims kyla didesnė rizika patirti stiprų nerimą ar netgi susirgti depresija ligos gydymo laikotarpiu. Tuo tarpu mokslinių tyrimų rezultatai parodė, kad edukacinė veikla teikia įvairialypę naudą pirmiausia patiems pacientams bei jį gydantiems specialistams. Edukacinės intervencijos paneigia klaidingus įsitikinimus, „stigmas“, kuriuos pacientai neretai sužino naršydami internete, paskatina aktyviau dalyvauti savo ligos gydyme, gerina pacientų gyvenimo kokybę ir prisideda prie sklandesnių ligos gydymo procesų užtikrinimo. Education of patients with diagnosed cancer is a major issue in our modern society. Multiple studies show that patients experience multiple social, physical and psychological problems, therefore, the demand of information is high. Special attention should be paid to women diagnosed with breast cancer, as it is the most common cancer disease among them. Consequently, the risk of developing anxiety and depression during treatment is high. Firstly, education is beneficial for both: patients and their doctors. Educational interventions help deny false information and “stigma” which patients learn from the Internet. Furthermore, it encourages people to actively participate in their own treatment planning and helps to increase the quality of life. Aim of this study: to discuss theoretical and practical aspects of patient education as an integral part of the health care provision for patients with breast cancer.

Įvadas. Pastarąjį dešimtmetį sergančiųjų išsėtine skleroze (IS) darbingumo lygiui nustatyti taikoma biopsichosocialiniu požiūriu paremta Tarptautinė funkcionavimo, neįgalumo ir sveikatos klasifikacija (TFK) ne tik prisideda prie sergančiųjų būklės nustatymo, bet ir siūlo priemones geresnei jų įtraukčiai į visuomenę. Visgi Lietuvoje TFK principai darbingumo lygiui nustatyti taikomi fragmentiškai, dažniau remiamasi biologiniais ir fiziniais asmens sveikatos būklės parametrais. Šio darbo tikslas – atlikti išsamų Lietuvos sergančiųjų IS biopsichosocialinį vertinimą ir nustatyti, kaip jų darbingumo lygis siejasi su TFK įvairių kategorijų rezultatais. Tiriamieji ir tyrimo metodai. Atliktame skerspjūvio tyrime dalyvavo 184 asmenys, sergantys IS: 157 pacientai, kurie kreipėsi į Neįgalumo ir darbingumo nustatymo tarnybą prie Socialinės apsaugos ir darbo ministerijos (NDNT) ir jiems buvo nustatytas darbingumo lygis, ir 27 sergantys IS asmenys, kurie į NDNT dar nesikreipė. Tiriamieji buvo apklausti, taikant trumpąjį TFK kategorijų rinkinį išsėtinei sklerozei, surinkti jų sociodemografiniai ir ligos duomenys. Rezultatai. Net 66 % sergančiųjų IS nurodė, kad jiems labai sunku judėti kitais nei ėjimas būdais (TFK kategorija d455), 32 % sunkumas yra mokamas darbas (d850). Net 81 % sergančiųjų IS aukštesniosios pažinimo funkcijos (b164) nesudaro sunkumų, 63 % pacientų sunkumų nesudaro dienos režimo laikymasis (d230), 57 % – problemų sprendimas (d175). 7–10 % susiduria su nepalankiomis šeimos narių ar sveikatos priežiūros specialistų nuostatomis jų atžvilgiu ir tai jiems yra kliuvinys. Asmenys, kurių darbingumo lygis siekia tik 0–25 %, patiria didesnių sunkumų beveik visose TFK kategorijose, nei tie, kurių darbingumas yra didesnis. Pacientai, kurių darbingumo lygis prilygsta 30–40 %, turi daugiau sunkumų dėl regos, šlapinimosi, raumenų jėgos funkcijų, ėjimo, judėjimo kitais nei ėjimas būdais, nei tie tiriamieji, kurių darbingumo lygis siekia 45–55 %, ir patiria daugiau sunkumų dėl judėjimo funkcijos bei turi labiau pakitusią galvos smegenų struktūrą nei tie, kurių darbingumo lygis yra 60–100 %. Išvados. Ne visų TFK kategorijų ryšiai su sergančiųjų IS darbingumo lygiu yra vienodi. Kuo didesnis asmens darbingumo lygis, tuo mažiau kliuvinių TFK kategorijų vertinime nurodo sergantieji IS. Background. During the re cent decade, the working capacity of patients with multiple sclerosis (MS) in many countries is determined by International Classification of Functioning, Disability and Health (ICF) based on biopsychosocial view. It not only identifies the condition of health of patients, but also suggests measures to improve their inclusion into society. However, in Lithuania, the ICF principles for determining the level of working capacity are applied in a fragmentary way, and more of ten the biological- physical parameters of a person’s health status are used. Thus, the establishment of relationship between ICF and working capacity level would create assumptions to legitimize the use of this classification that subsequently would con tribute to the well-being of people with MS. The aim of this research is to carry out a comprehensive biopsychosocial assessment of Lithuanian MS patients and to determine how their level of working capacity is related to the results of various categories of ICF. Subjects and methods. The cross-sectional study involved 184 people with MS including 157 people who had applied to the Disability and Working capacity Assessment Office under the Min is try of Social Security and Labour of the Republic of Lithuania (DWCAO) to determine their working capacity level and 27 people with MS who had n’t yet ap plied to DWCAO. The subjects were interviewed using a brief ICF core set for MS; sociodemographic and disease data were also collected. Results. 66% of patients with MS re ported that they had a complete difficulty of moving around by means other than walking (ICF cat e gory d455), 32% – remunerative employment (d850). 81% of patients with MS had no difficulties in higherlevel cognitive functions (b164), 63% in carrying out daily routine (d230), 57% – in solving problems (d175). 7–10% of patients faced negative attitudes of their family members or health care specialists and they considered it as a difficulty. Those with working capacity level of 0–25% faced higher difficulties in al most all ICF categories, com pared to people with higher working capacity. MS patients with working capacity level of 30–40% had more difficulties of seeing, urination, muscle power functions, walking and moving around by means other than walking than MS patients with working capacity level of 45–55%; they also faced more difficulties in moving around functions and had a different brain structure than those with working capacity level of 60–100%. Conclusions. The value of ICF categories for assessment of the working capacity level of people with MS is different. The higher the level of per son’s working capacity, the less restrictions patients with MS indicate in assessing ICF categories.

Public satisfaction with healthcare systems is an important dimension of healthcare legitimacy. The paper analyzes how satisfaction with healthcare systems depended upon the economic situation of our respondents during and after the economic recession in the Baltic states. The results show that there were no differences in public satisfaction with healthcare between better and worse off people in Estonia (except in 2009). In Lithuania, however, satisfaction among the economically better off was higher compared to other groups between 2008 and 2014. In Latvia, inequality in satisfaction between groups in different economic situations became significant from 2010.

Darbo tikslas: išanalizuoti sveikatos priežiūros paslaugų kokybę pacientų požiūriu Vilniaus miesto ligoninės kardiologijos centre. Tyrimo medžiaga ir metodai. Anketinė apklausa. Imtis – 200 pacientų. Vertinti pacientų lūkesčiai ir įspūdžiai apie ligoninės kardiologijos centre teikiamas sveikatos priežiūros paslaugas. Trust –a critical mechanism to manage vulnerability amidst uncertainty– may greatly influence healthcare practices, and consequently, its objectives. Building upon the work of Jurgen Habermas and the framework of trust chains, the aim of this article is to unpack how trust dynamics between the state, the provider, and the service user shape the functioning of mental healthcare in one of the former Soviet states –Lithuania. The case is of interest to medical sociology due to the region's historical and contemporary context. By drawing on in-depth interviews with healthcare providers and users, I demonstrate how the chains of reciprocal distrust underpin the workings of the mental health system and how the actors in turn employ a range of responses to such distrust. The instances of trusting relations nevertheless demonstrate how trust might facilitate the strive for mental healthcare that is more accessible, efficient, and of higher quality.

Straipsnyje pristatomos problemos, kylančios asmens duomenų apsaugai sveikatos apsaugos srityje. 2019 metais prasidėjusios koronaviruso infekcijos (toliau – ir COVID-19) pandemijos metu asmens duomenų apsaugos užtikrinimo klausimai visuomenėje tapo dar aktualesni. Ekstremali situacija Lietuvoje ir visame pasaulyje, susiklosčiusi dėl spartaus infekcijos plitimo, sveikatos priežiūros paslaugų teikimas nuotoliniu būdu, sergančių asmenų duomenų konfidencialumo garantavimas tapo dideliu iššūkiu tiek valstybinėms institucijoms, tiek gydymo įstaigoms. Straipsnyje atskleidžiamos asmens duomenų, asmens sveikatos duomenų sampratos, pristatomi pagrindiniai principai ir reikalavimai, taikomi asmens sveikatos duomenų tvarkymui, aptariamas asmens sveikatos duomenų konfidencialumas ir jų teikimo tretiesiems asmenims ypatumai. Taip pat pristatomos problemos, susijusios su asmenų, sergančių ŽIV ir AIDS duomenų apsauga. The article presents the problems arising from the protection of personal data in the field of health care. During the coronavirus infection (COVID-19) pandemic that started in 2019, the issues of ensuring the protection of personal data in the society became even more relevant. The extreme situation in Lithuania and around the world, due to the rapid spread of the infection, the provision of health care services at a distance, and guaranteeing the confidentiality of the data of sick people has become a major challenge for both state institutions and medical institutions. The article reveals the concepts of personal data and personal health data, presents the main principles and requirements applicable to the processing of personal health data, discusses the confidentiality of personal health data and the peculiarities of their provision to third parties. Issues related to data protection for people living with HIV and AIDS are also presented.
