1. SANITARY ASPECT OF NUTRITION CONTROL IN PUBLIC KITCHEN IN NIS

Abstract

The aim of the paper was to evaluate sanitary condition of food preparation and distribution in the "Public kitchen" in Nis, as well as the meals’ quality in order to assess the health risks. The local inspection of the objects and microbiological analysis of swaps were done according to standard methods. Energetical values and biochemical structure of the meals were assessed by bromatological methods and energetical density was calculated. The study was being conducted from 2002 – 2005. The results show that sanitary conditions in the object were not adequate, because of a high percentage of incorrect samples (13.4% from the total number of samples). Energetical values of meals were usually adequate (1157 – 1564 kcal, 4837 – 6581 kJ), but constantly decreased. The chemical structure of the meals was adequate too, meaning that meals were very well- planed. The energetic density of meals was high (1,18 – 1,42 kcal/g). Our conclusion is that there are no alternatives to continuous health care education of the medical stuff and regular sanitary hygienic control, all with the aim to accomplish complete social function of the public kitchen and to protect the health of its consumers.

2. THE INFLUENCE OF ARTERIAL HYPERTENSION ON THE LEFT VENTRICLAR DIASTOLIC FUNCTION

Abstract

The changes in the heart diastolic function are early functional disturbances in arterial hypertension and are present before the changes in the systolic function. The aim of this research was to study the influence of arterial hypertension on the left ventricular diastolic function. The research included a group of 126 subjects: 93 patients with arterial hypertension and 33 healthy subjects, who were the control group. The patients with arterial hypertension, compared to the control group, had a significantly higher: BMI (p < 0,001), left ventricular mass (p < 0,001), left ventricular mass index (p < 0,001) and the left atrium (p < 0,001). The parameters of the diastolic function in patients with arterial hypertension compared to the control group have shown statistically significant differences: IVRT (p < 0,001) and DT (p < 0,01) are significantly longer and the ratio E/A (p < 0,01) is significantly lower, which gives the image of delayed relaxation. The quotient of linear correlation of IVRT with the systolic and diastolic blood pressure has shown that IVRT statistically significantly correlates both with the systolic blood pressure (p < 0,005) and the diastolic blood pressure (p < 0,05). DT in correlations did not show the dependence on systolic and diastolic blood pressure. The quotient of the linear correlations of the E/A ratio with the systolic and diastolic blood pressure has shown that the E/A ratio, statistically significantly, negatively correlates with systolic blood pressure (p < 0,005) and diastolic blood pressure (p < 0,05). The changes in the left ventricular diastolic function in arterial hypertension are reversible and the condition for that is a good regulation of the blood pressure with hygienic and dietetic measures and medicaments.

Abstract

The prevalence of obesity is increasing in many developing countries and is followed by many cardiovascular disorders. The aim of the study was to investigate the effect of obesity on diastolic function in patients with arterial hypertension. Both obesity and hypertension have influence on diastolic function. The study group was comprised of 93 patients with arterial hypertension: 24 (25,80%) were obese, 69 (74,20%) were not obese. Obese patients with arterial hypertension and those patients that were not obese with arterial hypertension didn't significantly differ in terms of: age, gender distribution, systolic BP, diastolic BP, heart rate, left atrial volume, EF. Abstract

BACKGROUND There were gaps between guidelines and practice when surgery was the only treatment for aortic stenosis (AS). OBJECTIVES This study analyzed the decision to intervene in patients with severe AS in the EORP VHD (EURObservational Research Programme Valvular Heart Disease) II survey. METHODS Among 2,152 patients with severe AS, 1,271 patients with high-gradient AS who were symptomatic fulfilled a Class I recommendation for intervention according to the 2012 European Society of Cardiology guidelines; the primary end point was the decision for intervention. RESULTS A decision not to intervene was taken in 262 patients (20.6%). In multivariate analysis, the decision not to intervene was associated with older age (odds ratio [OR]: 1.34 per 10-year increase; 95% CI: 1.11 to 1.61; P = 0.002), New York Heart Association functional classes I and II versus III (OR: 1.63; 95% CI: 1.16 to 2.30; P = 0.005), higher age adjusted Charlson comorbidity index (OR: 1.09 per 1-point increase; 95% CI: 1.01 to 1.17; P = 0.03), and a lower transaortic mean gradient (OR: 0.81 per 10-mm Hg decrease; 95% CI: 0.71 to 0.92; P < 0.001). During the study period, 346 patients (40.2%, median age 84 years, median EuroSCORE II [European System for Cardiac Operative Risk Evaluation II] 3.1%) underwent transcatheter intervention and 515 (59.8%, median age 69 years, median EuroSCORE II 1.5%) underwent surgery. A decision not to intervene versus intervention was associated with lower 6-month survival (87.4%; 95% CI: 82.0 to 91.3 vs 94.6%; 95% CI: 92.8 to 95.9; P < 0.001). CONCLUSIONS A decision not to intervene was taken in 1 in 5 patients with severe symptomatic AS despite a Class I recommendation for intervention and the decision was particularly associated with older age and combined comorbidities. Abstract

Real-time systems are witnessing a significant increase in critical software's size, complexity, and performance needs, which can only be satisfied with high-performance hardware features. Cache memories, pervasively used to improve average performance, complicate Worst-Case Execution Time analysis: cache placement (i.e., how software objects are mapped to cache) during the testing phase does not only critically affect the observed performance, but also proves to be arduous to control and preserve up to operation. The probabilistic variant of Measurement-Based Timing Analysis (MBPTA) responds to this challenge by deploying time-randomized caches that naturally explore a different random cache placement in each run, relieving the user from producing tests that intercept relevant Cache Conflict Placements (CCP). Yet, to meet an adequate probabilistic CCP coverage, the user is required to collect a minimum number of measurements. We present two mechanisms, CCP-RM and CCP-HRP, to identify CCP with relevant probability of occurrence and large impact on execution-time, for the random modulo (RM) and hash-based random placement (HRP) policies. CCP-RM and CCP-HRP enable a reliable application of MBPTA by computing the number of runs R ' necessary to meet the desired CCP coverage. We exhaustively evaluate CCP-RM and CCP-HRP, showing their effectiveness on well-known benchmarks and a railway case study, on top of an accurate simulator and a concrete RTL implementation.

Abstract

Autonomous vehicles in embedded real-time systems increase critical-software size and complexity whose performance needs are covered with high-performance hardware features like caches, which however hampers obtaining WCET estimates that hold valid for all program execution paths. This requires assessing that all cache layouts have been properly factored in the WCET process. For measurement-based timing analysis, the most common analysis method, we provide a solution to achieve cache representativeness and full path coverage: we create a modified program for analysis purposes where cache impact is upper-bounded across any path, and derive the minimum number of runs required to capture in the test campaign cache layouts resulting in high execution times. Abstract

Measurement-Based Probabilistic Timing Analysis (MBPTA) has been shown to be an industrially viable method to estimate the Worst-Case Execution Time (WCET) of real-time program running on processors including several high-performance features. MBPTA requires hardware/software support so that program execution time, and so its WCET, has a probabilistic behaviour and can be modelled with probabilistic and statistic methods. MBPTA also requires that those events with high impact on execution time are properly captured in the (R) runs made at analysis time. Thus, a representativeness argument is needed to provide evidence that those events have been captured. This paper addresses the MBPTA representativeness problems caused by set-associative caches and presents a novel representativeness validation method (ReVS) for cache placement. Building on cache simulation, ReVS explores the probability and impact (miss count) of those cache placements that can occur during operation. ReVS determines the number of runs R, which can be higher than R, such that those cache placements with the highest impact are effectively observed in the analysis runs, and hence, MBPTA can be reliably applied to estimate the WCET. Abstract

The use of caches challenges measurement-based timing analysis (MBTA) in critical embedded systems. In the presence of caches, the worst-case timing behavior of a system heavily depends on how code and data are laid out in cache. Guaranteeing that test runs capture, and hence MBTA results are representative of, the worst-case conflictive cache layouts, is generally unaffordable for end users. The probabilistic variant of MBTA, MBPTA, exploits randomized caches and relieves the user from the burden of concocting layouts. In exchange, MBPTA requires the user to control the number of runs so that a solid probabilistic argument can be made about having captured the effect of worst-case cache conflicts during analysis. We present a computationally tractable Time-aware Address Conflict (TAC) mechanism that determines whether the impact of conflictive memory layouts is indeed captured in the MBPTA runs and prompts the user for more runs in case it is not. Abstract

Distal radius fractures are an increasingly prevalent upper extremity injury, especially among elderly patients. They represent approximately 3% of all upper extremity injures. Severity of these fractures is directly related to bone mineral density of the patient, and clinical results are dependent on this parameter as well. There is a bimodal distribution of these injuries, with a peak between 18 to 25 years of age, predominantly male population and a second peak in the elderly, older than 65 years, predominantly female population. Early reports of fractures of the distal radius considered these fractures to be group of injuries with a relatively good prognosis irrespective of the treatment given. When it comes to complex fractures, regardless of the method applied, major or minor functional invalidity persists. With that in mind fractures of the distal radius are medical, social and economic problems of modern society.

Abstract

Probabilistic timing analysis is a powerful approach to derive worst-case execution time (WCET) estimates, as needed in safety-critical systems, in the presence of high-performance hardware features (e.g., caches). To that end, the timing behavior of certain hardware resources, such as caches, is randomized. Time-randomized (TR) caches allow deriving hit/miss probabilities for each access and probabilistic WCET estimates for the overall program. However, the analysis of the average performance of TR caches, which is needed for lowly-critical high-performance tasks in mixed-criticality environments, has been neglected. So far, average performance of a TR cache can only be analyzed through simulation, whose accuracy strongly depends on carrying a large number of simulations. In this paper we address this challenge by proposing PACO, an accurate analytical approach to estimate cache hit/miss probabilities of full applications, parts of them and individual cache accesses at low cost for a wide variety of TR cache hierarchies and setups.

Abstract

Giovanni Battista Monteggia was born in Laverne on the 8th of August 1762. Monteggia started his education in the School of Surgery at the Hospital Maggiore in Milano in 1779. This hospital was called ?Big House? and it is one of the oldest medical institutions in Italy. He passed exam in surgery in 1781. Monteggia was promoted to assistant at surgery in Maggiore hospital in 1790. He was among the first who gave a complete clinical description of polio. He described traumatic hip dislocation and special forearm fracture which was named after him. Strictly speaking, a Monteggia fracture is a fracture of the proximal third of the ulna with an anterior dislocation of the radial head. Monteggia became a member of the renewed Institute of Science, Literature and Art in Milano in 1813.

Abstract

International audience; Probabilistic Timing Analysis (PTA) has emerged recently to derive trustworthy and tight WCET estimates. Computational costs due to the use of the mathematical operator called convolution used by SPTA – the static variant of PTA – and also deployed in many domains including signal and image processing, jeopardize the scalability of SPTA to real-size programs. We evaluate, qualitatively and quantitatively, optimizations to reduce convlution’s computational costs when it is applied to SPTA. We showthat SPTA specific optimizations provide the largest execution time reductions, at the cost of a small loss of precision.

Abstract

INTRODUCTION Left atrial size is increased in patients with arterial hypertension. Left atrial enlargement represents a risk factor of atrial fibrillation and stroke. Left atrial size depends on the effect of many other etiological factors, predominantly by body mass and the left ventricular mass. OBJECTIVE The objective of the study was to investigate the frequency of the left atrial enlargement in patients with arterial hypertension, in obese patients with arterial hypertension and in patients with arterial hypertension and left ventricular hypertrophy. In addition, the aim was to investigate the influence of diastolic function parameters on the left atrial dimension. METHOD The study included 93 patients with arterial hypertension (mean age of 46.9 ? 9.7 years, 50.5% of males) and 33 healthy subjects (mean age 45.6 ?10.6 yrs., 40% of males) who consisted the control group. There was no statistical difference of the age and sex between patients and healthy persons. All patients were examined by the echocardiographic ultrasound device HP Sonos 2500 by three echosonographers. Abstract

Therapy application during pregnancy is determined by various pathological conditions and the pregnant women's diseases. The most common occurences in pregnancy where the application of therapy is necessary, are anemia, hypertension, bleeding, etc. Exposure to air pollution during pregnancy can influence such conditions to occur, which entails a more frequent use of therapeutical preparations.In this respect, the aim of this research is to assess the frequency of the application of therapeutical preparations in pregnant women exposed to significantly different concentrations of harmful materials in the air, and also to determine a possible difference in the frequency of the application of certain therapeutical substances.The measurement of air pollutants, sulphur dioxide and soot in the air, was carried out daily at two measuring locations, in Nis and Niska Banja.The examinees were pregnant women, aged over 30, living in Nis, nonsmokers, of good physical condition, who hadn't before pregnancy and profesionally been exposed to harmful agents before pregnancy. Data on frequency of therapy application during pregnancy were obtained from the medical records of tested pregnant women, and were compared to the corresponding data from the control group of pregnant women living in Niska Banja.Research results have shown that the frequency of therapy application in pregnancy is higher in pregnant women exposed to higher concentration of sulphur dioxide and soot, as compared with pregnant women exposed to lower concentrations of these agents, and that the most common indications requiring therapy in all tested pregnant women were anemia, hypertension, and bleeding.

Abstract

Introduction. Arterial hypertension is a disease which has influence on the left ventricular diastolic function. It has been suggested that impairment of the left ventricular diastolic function in patients with diabetes mellitus is due to arterial hypertension and diffuse peripherial and coronary atherosclerosis which appear early in diabetic patients. Now, however, it is thought, that other mechanisms are responsable for the development of diabetic cardiomyopathy. The aim of the study was to investigate the effect of glycemia on the left ventricular diastolic function in patients with arterial hypertension. Methods. The study included 60 patients with arterial hypertension: 30 (50,00%) patients with diabetes mellitus and 30 (50,00%) with?out diabetes mellitus. The parameters of diastolic function were measured by the pulsed Doppler echocardiographic technique. Isovolumetric relaxation time, deceleration time, maximum velocity of the early stage of ventricular filling, maximum velocity of the late stage of ventricular filling and the early/late stage of ventricular filling ratio were used to assess the diastolic function. Abstract

The aim of this research was to investigate the potential relationship between the longterm exposure to air pollution, as a risk factor, and the development of allergic reactions in the population. The observed sample consisted of Nis citizens of different age groups who lived in areas with high concentrations of air pollutants (investigated group) such as the Square of the October Revolution and Sindjelic’s Square (investigated group), as well as the citizens of Niska Banja (control group) which is the zone with the lowest concentration of air pollution. The investigation was carried out in the Public Health Institute in the period between 1996 and 2000. A significance test was performed using a Mantel-Haenszel chi square test. This test was used to check for a statistically significant difference between the incidence of the obstructive lung disease between the investigated group and the control group across all age groups. The cross-risk and relative risk were determined. The highest value of the chi square test was determined in the age group 26-50 (RR=3.50), in the group up to 25, RR=2.25, and in the group above 51, RR=1.89. The obtained results prove that the incidence of the obstructive lung disease is significantly higher in the exposed population than in the non-exposed.
