17. An investigation of the association between catastrophic thoughts and anxiety in people with respiratory diseases

Abstract

The purpose of this study was to investigate whether the cognitive model of panic may explain the high levels of anxiety that have been reported amongst people with Chronic Obstructive Pulmonary Disease (COPD) (Carr et al, 1994). In previous research, cognitive variables, assessed on general measures of catastrophizing (such as the Agoraphobic Cognitions Questionnaire), have been shown to be strongly predictive of anxiety in people with COPD, regardless of severity or duration of the respiratory disease. One of the aims of this study was to develop a questionnaire to elicit specific cognitions in response to a range of COPD-related symptoms presented in different situations. Favourable reliability and validity were demonstrated for the questionnaire, and its completion by 37 people with COPD produced a range of cognitions of differing severity. High levels of anxiety (assessed using the Hospital Anxiety and Depression scale) were found in the group (more than 27%). More severe catastrophic thoughts were found to be strongly predictive of greater anxiety, particularly for specific anxiety ratings on the new questionnaire, and this relationship was more significant when cognitions and anxiety wore elicited by symptoms presented in 'unsafe' situations. Severity and duration of illness were not significant predictors of anxiety, but social support was strongly predictive of generalized anxiety. In the light of the adverse consequences of anxiety with regards to appropriate coping with respiratory disease, an exploration of the use of cognitive techniques in therapy is suggested.

32. The development of coping resources in adulthood

Abstract

We examined three community samples to determine whether stressful episodes form a context for the development of coping resources in adulthood. The first study found that 81.9% of a sample of 845 older men reported drawing upon prior experiences in coping with a recent problem. Content analysis revealed that only 22.7% drew upon similar stressful episodes; the rest drew upon problems from work, the military, illnesses, deaths, etc. The second study replicated the earlier findings in 102 men and women, ages 24 to 84, who reported on a recent low point in semistructured interviews. In addition, 75% reported long-term effects, equally split between negative, positive, and mixed effects. Those individuals who perceived advantages from the low point were significantly more likely to report positive long-term effects. The third study replicate the findings from the first two studies in a sample of 941 men and women ages 23 to 62. Path analyses showed that coping strategies differentially predicted perceived positive or negative outcomes, which in turn predicted current mastery and depression levels. While the findings are cross-sectional and causality cannot be inferred, they are nonetheless supportive of the effects of stress and coping personality.

Abstract

Controversies exist concerning the influence of age on the stress and coping process, in part due to differences in methods across studies. We examined age differences in stress, appraisal, and coping, using both semistructured interview questions and a coping checklist in middle-aged, young-old, and old-old men. Despite extensive probing, nearly a quarter of the old-old reported having had no problems and they expended less coping effort even when they did have problems. The types of problems reported varied systematically with age. Middle-aged men were more likely to appraise their problems both as challenges and as annoyances than the older men. Different age patterns emerged from the coping interviews vs the checklists, but controlling for type of problem significantly attenuated age differences. However, there were no age differences in perceived stressfulness of the problem, appraisals of harm/loss, or helpless appraisals, number of emotions reported, or coping efficacy. One interpretation of these results is that the nature of stress changes with age, from episodic to chronic, which in turn affects appraisal and coping processes.

Abstract

The outbreak of the novel coronavirus (COVID-19) has resulted in upward of 14 million confirmed cases and >597,000 deaths worldwide as of July 19, 2020. The current disruption in sports activities caused by COVID-19 presents a challenge to physicians, coaches, and trainers in discerning best practices for a safe return to sport. There is a distinct need to develop and adopt consistent measures for resumption of sports activities, including training and competition, in a way that places the health and well-being of athletes at the forefront while also protecting coaches, allied staff, and spectators. This article provides an overview of the effects of COVID-19 in the athletic population and presents considerations for training during the pandemic, as well as guidelines for return to sports as restrictions are lifted. Abstract

Background: Despite the common nature of orthopaedic injuries in equestrian sports, there is no published review to specifically characterize orthopaedic injuries in equestrian athletes. Purpose: To characterize orthopaedic injury patterns in equine sports–related injuries and their treatment. Study Design: Systematic review; Level of evidence, 4. Methods: This review was performed through a PubMed, EMBASE, and Scopus query (from 1978 to June 2014) in the English literature using search terms “(equine-related OR equestrian-related OR horse-related OR equestrian OR equestrians) AND (injury OR injuries).” Only full-text studies reporting on orthopaedic injury patterns pertinent to equestrian sports in the United States (US) and the United Kingdom (UK) were included. Orthopaedic injuries were defined as those resulting in a fracture or dislocation. In all, 182 studies were excluded, leaving a total of 27 studies for evaluation. The studies included were analyzed for demographic and epidemiological data for orthopaedic injuries, including fractures and dislocations. Cranial and facial injuries were excluded from analysis. Results: The majority of those injured in the US were female (64.5%). The leading cause of injury in the US was falling from a horse. The use of protective equipment seemed to vary widely, with helmet use ranging from less than 6% up to 66.7%. In the UK, fractures were found to account for 17.4% of reported injures, compared with 33.6% of injuries in the US. The majority of fractures in US riders occurred in the upper extremities (50.7%). Conclusion: This review helps characterize the epidemiology of equestrian injuries based on currently available data.
