Abstract

背景と目的】菊地病は良性疾患であるが,若い人に多く発症するため,その臨床的所見を検討した.【対象と方法】 2004年以来 8年半の間に恵愛堂病院において菊地病と診断された 10例を対象とした.リンパ節は免疫学的,組織学的検討を行った.【結果】対象は男性 6例,女性 4例で年齢は 17歳から 45歳に分布し中央値は 32歳であった.表在性リンパ節腫脹は片側の頚部が 9例であった.発熱は 9例中 7例で確認されたが,リンパ節の自発痛は全例で認められなかった.検査所見では LDHは軽度上昇し, CRPは 10例中 8例が正常であった.病理組織学的には多数の大型のリンパ系芽球,アポトーシスに陥った核片を貪食した組織球に加え,形質細胞様樹状細胞や小リンパ球が見られたが,好中球は認められなかった.【結語】片側の頚部の表在性リンパ節腫脹と発熱を来たした若年者では菊地病も考慮して診察することが望ましいが,悪性リンパ腫を鑑別するためにはリンパ節の生検をすることが望ましい.

16. 胆嚢摘出術で注意すべき胆管走行異常(Cystohepatic duct)の1例

Abstract

論文(Article), A 74-year-old man presented with epigastralgia and was diagnosed as having cholelithiasis. Endoscopic retrograde cholangiopancreatography (ERCP) initially visualized the cystic duct with the Heister valve from the common bile duct, and then two intra-hepatic biliary ducts of segment 5 (B5) were visualized from the neck of the gallbladder. There was a contrast medium filling defect in B5, which was considered to be due to an incarcerated stone. Magnetic resonance cholangiopancreatography (MRCP) and three-dimensional computed tomography (CT) cholangiography showed similar findings, suggesting that the patient had a biliary anomaly of the cystohepatic duct in which two intrahepatic bile ducts (B5) flowed into the neck of the gallbladder and a stone incarcerated in the neck of the gallbladder. At surgery, during mobilization of the gallbladder, there was a thick string between the liver and the gallbladder, and this was considered to be the junction of B5 with the gallbladder. Therefore, the neck of the gallbladder was cut, and an incarcerated stone 10 mm in diameter was removed. Intraoperative cholangiography revealed that the cystohepatic ducts were preserved. The postoperative course was uneventful and there was no bile leakage or liver dysfunction. Although cystohepatic duct is a rare biliary anomaly, the surgeon should be alert for its possible presence during cholecystectomy. When cholecystectomy is scheduled, more than one preoperative examination by three-dimensional CT cholangiography, MRCP or ERCP should be performed, and the surgeon should be careful not to overlook any biliary anomaly. Abstract

Recently, some studies show various organic compounds such as pesticides and dyes degraded with the irradiation of ultraviolet light and visible light in the presence of oxalic acid and ferric ion (ferrioxalate/UVvis process). The process has much advance than other technologies because it can utilize the wavelength of 300∼450nm and also under the condition of neutral pH. Chlorinated organic compounds such as trichloroethene (TCE), which have caused ground water pollution on a lot of sites, have never been applied by photodegradation with this process. In this study, we showed the degradation of TCE in the presence of oxalic acid and iron ion and the change of pH, ferric and ferrous ion concentration during the photodegradation of TCE with ferrioxalate/UV-vis process. TCE was degraded in the presence of oxalic acid and iron ion. In the reactions, the equilibrium of oxalate ion and iron ion is important since it determines the amount of ferrioxalate complex which absorbs light and induces the reactions of the degradation of TCE. Thus, the pH value and iron ion concentration are the important factors which determine the amount of ferrioxalate complex. The pH is nearly constant during the photodegradation of TCE. The ferrous ion concentration was decreased as soon as beginning photodegradation of TCE, and then the ferrous ion concentration and ferric ion concentration became constant. © 2006 American Institute of Physics [ABSTRACT FROM AUTHOR]

Abstract

A heterotrophic bacterium, designated as HIB4, having an ability to oxidize ferrous iron was isolated from the sample of an enrichment culture with 9K medium, by using the modified WAYE (washed agarose/yeast extract) medium with ferrous sulphate. This isolate was identified as Alicyclobacillus disulfidooxidans from 16S rDNA sequence analysis. The isolate grew and oxidized ferrous iron in an inorganic medium containing 0.02 % (w/v) of yeast extract and Ferrous iron oxidation occurred at the almost end of its logarithmic phase. Yeast extract was an essential substrate for the isolate because the isolate could not grow or oxidize ferrous iron without yeast extract. However, higher concentration of yeast extract inhibited the growth of the isolate. On the other hand, it was confirmed that the isolate was able to grow without ferrous ion so that it did not get any energy by ferrous ion oxidation. Its optimum concentration of yeast extract was 0.02% (w/v) at the concentration of ferrous ion 0.08mol/liter. Its optimum pH was 1.5 and the optimum temperature was 30 °C These physiological characteristics were completely different from A. disulfidooxidans SD-11 which is the type strain. © 2006 American Institute of Physics [ABSTRACT FROM AUTHOR]

Abstract

To find an efficient bacterium, which has the strong capacity to produce hydrogen sulfide from polysulfide as the waste of process generating hydrogen from hydrogen sulfide by photocatalytic reaction using sun light, is very important for constructing hydrogen producing system. 10 strains of sulfate-reducing bacteria (SRB), which can reduce polysulfide directly, have been isolated from various natural samples such as TCE contaminated soil, soil and sludge around hot spring environment, and the cooling tower of a geothermal plant. This study describes physiological and phylogenetic characterization of SRB which can reduce polysulfide. All of isolates had the ability to reduce polusulfide but these reduction rates were difference depend on isolates. Phylogetetically, all of isolates located difference position for general SRB including Desulfovibrio desulfuricans, which is used standard strain in this study, so they do not belong to Proteobacteria. These have close relation to the genus Desulfotomaculum which can reduce elemental sulfur. It suggests that the ability of reducing elemental sulfur is important for reducing polysulfide to hydrogen sulfide. © 2006 American Institute of Physics [ABSTRACT FROM AUTHOR]
