Abstract

Objectives: Identifying patients with COVID-19 who are at risk of poor evolution is key to early decide on their hospitalization. We evaluated the combined impact of nucleocapsid (N)-antigenemia profiled by a rapid test and antibodies against the S1 subunit of the SARS-CoV S protein (S1) on the hospitalization risk of patients with COVID-19. Methods: N-antigenemia and anti-S1 antibodies were profiled at admission to the emergency department in 146 patients with COVID-19 using the Panbio® antigen Rapid Test and the SARS-CoV-2 immunoglobulin G II Quant/SARS-CoV-2 immunoglobulin G assay from Abbott. A multivariable analysis was used to evaluate the impact of these factors on hospitalization. Results: Patients with a positive N-antigen test in plasma and anti-S1 levels

Abstract

This report describes multidisciplinary care combining orthodontics and restorative dentistry for a patient with Class II occlusion and stained mandibular and maxillary resin composite veneers. The orthodontic treatment improved severe overjet and malocclusion prior to restorative care. Occlusal assessment was provided with a novel digital device (PlaneSystem, Zirkonzahn) that is integrated with digital workflows for the evaluation of the occlusal plane and condylar path inclination. Diagnostic digital impressions and digital wax-up for intraoral mock-ups led to the patient’s treatment acceptance. Minimally invasive tooth preparation, final digital impressions, and bonding under dental dam isolation fulfilled the patient’s esthetic and functional demands with all-ceramic restorations.

Abstract

Acne is a common chronic skin condition with serious physical and psychosocial consequences. In some cases, the appearance of pimples, whiteheads, or blackheads on the face, neck, and back may lead to scarring, disfiguring, depression, frustration, and anxiety in patients. Current treatments rely on antibiotics to eradicate Cutibacterium acnes (C. acnes), the bacterium responsible for this skin condition. However, these approaches do not scavenge the reactive oxidative species (ROS) generated during disease development and raise concerns about the increase in antimicrobial resistance. In this study, an environmentally friendly and cost-effective self-assembly nanoencapsulation technology based on zein, a bio-based hydrophobic protein, was employed to produce multifunctional essential oil (EO)-loaded nanocapsules (NCs) with superior antioxidant and bactericidal activity toward C. acnes. The NCs displayed “smart” release of the active cargo only under the conditions that were conducive to acne proliferation on skin. Once incorporated into creams, the EO-loaded NCs led to a complete inhibition of C. acnes and demonstrated the capacity to scavenge ROS, thus preventing damage to human skin cells. The in vitro permeation studies revealed that the nanoformulated EO was able to penetrate through the epidermis, indicating its potential for the treatment of skin diseases, such as acne.

Abstract

With its extreme density of stars and stellar remnants, dense young massive clusters, high specific star formation rate, intense radiation field, high magnetic field strength, and properties of the interstellar medium that resemble those in high redshift galaxies and starbursts, the Galactic Centre is the most extreme environment that we can observe in detail. It is also the only nucleus of a galaxy that we can observe with a resolution of just a few milli parsecs. This makes it a crucial target to understand the physics of galactic nuclei and star formation, as well as the connection between them. It enables studies of a large number of otherwise rare objects, such as extremely massive stars and stellar remnants, at a well-defined distance, thus facilitating the interpretation of their properties. The Galactic Centre has been and is being studied intensively with the most advanced facilities. In this White Paper, we advocate for a large-area, multi-wavelength survey with the Square Kilometre Array of an area of about 1.25x0.3 deg**2 (180x40 pc**2), centered on the massive black hole Sagittarius A* and for repeated deep observations of the nuclear star cluster over a decade, which will allow the community to address multiple science problems with a single data set.

Abstract

The most potent cell wall-derived inflammatory toxins (“pathogenicity factors”) of Gram-negative and -positive bacteria are lipopolysaccharides (LPS) (endotoxins) and lipoproteins (LP), respectively. Despite the fact that the former signals via toll-like receptor 4 (TLR4) and the latter via TLR2, the physico-chemistry of these compounds exhibits considerable similarity, an amphiphilic molecule with a polar and charged backbone and a lipid moiety. While the exterior portion of the LPS (i.e., the O-chain) represents the serologically relevant structure, the inner part, the lipid A, is responsible for one of the strongest inflammatory activities known. In the last years, we have demonstrated that antimicrobial peptides from the Pep19-2.5 family, which were designed to bind to LPS and LP, act as anti-inflammatory agents against sepsis and endotoxic shock caused by severe bacterial infections. We also showed that this anti-inflammatory activity requires specific interactions of the peptides with LPS and LP leading to exothermic reactions with saturation characteristics in calorimetry assays. Parallel to this, peptide-mediated neutralization of LPS and LP involves changes in various physical parameters, including both the gel to liquid crystalline phase transition of the acyl chains and the three-dimensional aggregate structures of the toxins. Furthermore, the effectivity of neutralization of pathogenicity factors by peptides was demonstrated in several in vivo models together with the finding that a peptide-based therapy sensitizes bacteria (also antimicrobial resistant) to antibiotics. Finally, a significant step in the understanding of the broad anti-inflammatory function of Pep19-2.5 was the demonstration that this compound is able to block the intracellular endotoxin signaling cascade.

9. Alpha hemolysin induces an increase of erythrocytes calcium: a FLIM 2-photon phasor analysis approach.

Abstract

α-Hemolysin (HlyA) from Escherichia coli is considered as the prototype of a family of toxins called RTX (repeat in toxin), a group of proteins that share genetic and structural features. HlyA is an important virulence factor in E. coli extraintestinal infections, such as meningitis, septicemia and urinary infections. High concentrations of the toxin cause the lysis of several cells such as erythrocytes, granulocytes, monocytes, endothelial and renal epithelial cells of different species. At low concentrations it induces the production of cytokines and apoptosis. Since many of the subcytolytic effects in other cells have been reported to be triggered by the increase of intracellular calcium, we followed the calcium concentration inside the erythrocytes while incubating with sublytic concentrations of HlyA. Calcium concentration was monitored using the calcium indicator Green 1, 2-photon excitation, and fluorescence lifetime imaging microscopy (FLIM). Data were analyzed using the phasor representation. In this report, we present evidence that, at sublytic concentrations, HlyA induces an increase of calcium concentration in rabbit erythrocytes in the first 10 s. Results are discussed in relation to the difficulties of measuring calcium concentrations in erythrocytes where hemoglobin is present, the contribution of the background and the heterogeneity of the response observed in individual cells.

28. Status of the SKA project and the SKA Regional Centre Network

Abstract

The aim of this talk is to present the status of the SKAO project and the SKA Regional Centre Network. The SKA project is an international effort to build two radio-interferometers that will be able to make revolutionary contributions to Astrophysics, Astrobiology, and Fundamental Physics. In January 2021 the SKA Observatory (SKAO) was established as the second intergovernmental organisation (IGO) in astronomy (after the ESO) in January 2021. SKA-Low will consist of 130.000 low-frequency dipoles in Australia, and SKA-Mid will consist of 133 SKA+64 MeerKAT dishes in South Africa. The construction phase commenced in July 2021 and the procurement and tender for the contracts have been initiated, and at the time of writing 5 construction contracts have been pre-allocated to Spain. In addition, the observatory is planning to start a development program for new instrumentation. The high data-rate and complexity of SKA will necessarily transform how scientists access, analyse and share data from observations, being considered a Big Data machine. A network of SKA Regional Centres (SRCs) is being established to take the science data products that will emerge from the SKAO to the final state required for science analysis. Those will constitute the core of SKA science, since analysis of SKA data will not be feasible out of them. The SRC Network will be part of an end-to-end system composed by the SKAO and the SRCs. The SRCs will provide the SKA community with access to data, computational resources and support to use the SKA and its data products, as well as a platform for collaborative science. To develop the SRC Network, several countries, including Spain, are building SRC prototypes that are the foundation of the work led by the SRC Steering Committee (SRCSC). In 2020, the SRCSC established seven working groups with wide participation from the Spanish community, and recently the SRCSC started a new phase to prototype specific components of the SRC Network.

Resumen: La violencia sexual es un problema de salud pública muy infradetectado, con importantes consecuencias a corto y largo plazo sobre la salud física, mental, social, sexual y reproductiva, que deben ser tenidas en cuenta desde los servicios de salud.Los sistemas de salud forman parte del conjunto de recursos necesarios para un abordaje integral desde el modelo ecológico: prevención y promoción de una sexualidad saludable y en igualdad, atención adecuada y coordinada ante una agresión sexual y apoyo posterior para la prevención de secuelas.Todas las violencias sexuales tienen consecuencias para la salud, incluso las que puedan parecer menos graves, como el acoso sexual o la ciberviolencia sexual.Debemos tener en cuenta las necesidades de la víctima y sus posibles reacciones emocionales. Se realizará una valoración de riesgos, se derivará a un servicio hospitalario cuando sea necesario y se priorizará una atención integral e integrada.La atención y el seguimiento deben realizarse con un enfoque centrado en la superviviente y por profesionales formados en trauma para comprender las consecuencias de la violencia sexual, ofrecer un entorno seguro y de confianza, y saber reforzar sus cualidades y apoyos. Abstract: Sexual violence is a very underdetected public health problem, with important short and long-term consequences on physical, mental, social, sexual and reproductive health, which must be taken into account by health services.Health systems are part of the set of resources necessary for a comprehensive approach from the ecological model: prevention and promotion of healthy sexuality with equality, adequate and coordinated care in the event of sexual assault and subsequent support to prevent sequelae.All sexual violence has health consequences, even those that may seem less serious such as sexual harassment or sexual cyberviolence.We must know the needs of the victim and their possible emotional reactions. A risk assessment will be carried out, the victim will be referred to a hospital if necessary and comprehensive and integrated care will be provided.Care and follow-up must focus on the survivor and with professionals trained in trauma to understand the consequences of sexual violence, offer a safe and trusting environment and know how to reinforce their qualities and support.

Abstract

Targeted bactericide nanosystems hold significant promise to improve the efficacy of existing antimicrobials for treatment of severe bacterial infections, minimizing the side effects and lowering the risk of the development of antibiotic resistance. In this work, we developed antibody-functionalized nanocapsules (NCs) containing antibacterial essential oil (EO) for selective and effective eradication of Staphylococcus aureus. Antibacterial EO NCs were produced via self-assembly nanoencapsulation in the plant-derived protein zein. The obtained EO NCs were decorated with aminocellulose to provide more reactive surface groups for carboxyl-to-amine immobilization of a antibody that is specific against S. aureus. The antibody-enabled EO NCs (Ab@EO NCs) demonstrated 2-fold higher bactericidal efficacy against the targeted bacterium compared to the pristine EO NCs at the same concentrations. The improved antibacterial effect of the Ab@EO NCs toward S. aureus was also confirmed in a real-time assay by monitoring bacterial cells elimination using a quartz crystal microbalance. Furthermore, the Ab@EO NCs selectively decreased the load and changed the cell morphology of the targeted S. aureus in a mixed inoculum with nontargeted Pseudomonas aeruginosa. Applying the nanoformulated antibacterial actives to an in vitro coculture model of the bacteria and skin fibroblasts resulted in suppression of S. aureus growth while preserving the human cells viability. The novel antibody-enabled antibacterial NCs showed potential for improving the treatment efficacy of staphylococcal infections, minimally affecting the beneficial microbial and human cells.

Abstract

Desmodus rotundus is one of the wild animal species that has benefitted by habitat alteration and its population has increased due to livestock activities. Common vampire bat population management has been implemented across Mexico due to the economic losses to livestock production, inflicted by vampire bat attacks and rabies transmission. Yucatan is one of the seven most impacted states in Mexico by the number of cattle rabies cases per year. However, there is little research on D. rotundus populations such as the frequency and attack patterns to cattle. This study's objective was to analyze the relationship between D. rotundus abundance and number of bovines attacked in livestock landscapes in Yucatan. The study used data gathered by the State Committee for Protection and Promotion of Livestock in Yucatan through the National Campaign for Common Vampire Bat Population Control. Data collected from January 2014 to December 2017 was analyzed using Pearson correlation. Distribution maps on Desmodus rotundus abundance and number of bovines attacked were also created. Higher abundance of Desmodus rotundus and number of cattle attacks were observed in the central region of Yucatan, particularly in Izamal municipality. Positive correlations were found between (1) abundance of Desmodus rotundus and number of cattle in the region, (2) total number of cattle and number of cattle attacked, and (3) abundance of Desmodus rotundus and number of cattle attacked. We can conclude that there is a relationship between Desmodus rotundus abundance and frequency of cattle attacks in most municipalities across Yucatan. Some outstanding exceptions were observed, which require further detailed investigations.

Abstract BackgroundThe European Foundation for the Care of Newborn Infants (EFCNI) has promoted the importance of parental involvement in the care of children. ObjectiveThe study aimed to examine how the time required by parents to achieve autonomy in the care of their very low–birth weight newborn infants was modified during the implementation of a training program. MethodsThis was an observational prospective study in the context of a quality improvement initiative. The Cuídame (meaning “Take Care of Me” in English) program was aimed at achieving parental autonomy. It was implemented over 2 periods: period 1, from September 1, 2020, to June 15, 2021; and period 2, from July 15, 2021, to May 31, 2022. The days required by parents to achieve autonomy in several areas of care were collected from the electronic health system. ResultsA total of 54 and 43 families with newborn infants were recruited in periods 1 and 2, respectively. Less time was required to acheive autonomy in period 2 for participation in clinical rounds (median 10.5, IQR 5‐20 vs 7, IQR 4‐10.5 d; PPPPPP ConclusionsParents required less time to achieve autonomy for participation in clinical rounds, feeding, and observation of neurobehavior during the implementation of the training program. Nevertheless, they required more time to achieve autonomy for kangaroo mother care, diaper change, and infection prevention.

Abstract

Background Hyperthermic intraperitoneal chemotherapy (HIPEC) with gemcitabine, after cytoreductive surgery, may reduce the tumor progression of pancreatic cancer through the reduction of the neoplastic volume and the subpopulation of residual pancreatic cancer stem cells, improving the survival of patients with pancreatic cancer and decreasing the recurrence of the disease. Methods A pilot study is performed with the first ten patients in the experimental group. A randomized study (phase II-III clinical trial) that requires a population of 42 patients, with 21 patients in each group. All patients have a diagnosis of ductal adenocarcinoma of the pancreas, which will be surgically resected with curative intention. (I) Group I: after an R0 resection, patients receive individualized adjuvant treatments. (II) Group II: after an R0 resection, HIPEC is performed with gemcitabine (120 mg/m2 for 30 min), and they also receive individualized adjuvant treatments. To analyze the safety of the procedure, the main variables measured were as follows: grades of complications by means of the Clavien-Dindo system: pancreas surgery complications (e.g., pancreatic fistula, perioperative hemorrhage, delayed gastric emptying, biliary fistula), operative mortality, and laboratory parameters to control system functions. Values were measured three times: preoperatively, twenty-four hours after surgery, and on the 7th postoperative day. Results From 2018 to 2019, 31 patients were recruited for our clinical trial. Fifteen patients were excluded because of intraoperative unresectability or a different intraoperative histologic diagnosis. Ten patients were included in the experimental group (resection plus HIPEC gemcitabine). The mean age was 65±7 years, and six patients were female (60%). We confirmed the histologic diagnosis of ductal pancreatic adenocarcinoma in all patients prior to HIPEC. Total pancreatectomy was performed in five patients. The surgical median time was 360 min, and the hospital stay was 11 days. Four patients showed complications classified as Clavien-Dindo type II and one showing type I. Six patients were classified as having stage III tumors. To date, no hospital mortality, locoregional recurrence, or differences between the two groups in terms of perioperative complications, biochemical and gasometric values, or Clavien-Dindo complication grades were observed. Conclusions Our clinical pilot study demonstrated a similar perioperative outcome that allows the trial until main objectives are achieved.

Abstract

Background While multidimensional and interdisciplinary assessment of older adult patients improves their short-term outcomes after evaluation in the emergency department (ED), this assessment is time-consuming and ill-suited for the busy environment. Thus, identifying patients who will benefit from this strategy is challenging. Therefore, this study aimed to identify older adult patients suitable for a different ED approach as well as independent variables associated with poor short-term clinical outcomes. Methods We included all patients ≥65 years attending 52 EDs in Spain over 7 days. Sociodemographic, comorbidity, and baseline functional status data were collected. The outcomes were 30-day mortality, re-presentation, hospital readmission, and the composite of all outcomes. Results During the study among 96,014 patients evaluated in the ED, we included 23,338 patients ≥65 years—mean age, 78.4±8.1 years; 12,626 (54.1%) women. During follow-up, 5,776 patients (24.75%) had poor outcomes after evaluation in the ED: 1,140 (4.88%) died, 4,640 (20.51) returned to the ED, and 1,739 (7.69%) were readmitted 30 days after discharge following the index visit. A model including male sex, age ≥75 years, arrival by ambulance, Charlson Comorbidity Index ≥3, and functional impairment had a C-index of 0.81 (95% confidence interval, 0.80–0.82) for 30-day mortality. Conclusion Male sex, age ≥75 years, arrival by ambulance, functional impairment, or severe comorbidity are features of patients who could benefit from approaches in the ED different from the common triage to improve the poor short-term outcomes of this population.

Abstract

We have explored the properties of a peculiar object detected in deep optical imaging and located at the tip of an H » I tail emerging from Hickson Compact Group 16. Using multiband photometry from infrared to ultraviolet, we were able to constrain its stellar age to 58-9+22 Myr with a rather high metallicity of [Fe/H] = -0.16-0.41+0.43 for its stellar mass of M- = 4.2 × 106 Mpdbl, a typical signature of tidal dwarf galaxies. The structural properties of this object are similar to those of diffuse galaxies, with a round and featureless morphology, a large effective radius (reff = 1.5 kpc), and a low surface brightness (μg-eff = 25.6 mag arcsec-2). Assuming that the object is dynamically stable and able to survive in the future, its fading in time via the aging of its stellar component will make it undetectable in optical observations in just ∼2 Gyr of evolution, even in the deepest current or future optical surveys. Its high H » I mass, M(HI) = 3.9 × 108 Mpdbl, and future undetectable stellar component will make the object match the observational properties of dark galaxies, that is, dark matter halos that failed to turn gas into stars. Abstract

SUMMARYIn human–robot comanipulation, virtual guides are an important tool used to assist the human worker as they constrain the movement of the robot to improve the task accuracy and to avoid undesirable effects, such as collisions with the environment. Consequently, the physical effort and cognitive overload are reduced during accomplishment of comanipulative tasks. However, the construction of virtual guides often requires expert knowledge and modeling of the task, which restricts the usefulness of virtual guides to scenarios with fixed constraints. Moreover, few approaches have addressed the implementation of virtual guides enforcing orientation constraints and, when done, these approaches have treated translation and orientation separately, and consequently there is no synchronization of the translational and rotational motions. To overcome these challenges and enhance the programming flexibility of virtual guides, we present a new framework that allows the user to create 6D virtual guides through XSplines which we define as a combination of Akima splines for the translation component and spherical cubic interpolation of quaternions for the orientation component. For complex tasks, the user is able to initially define a 3D virtual guide and then use this assistance in translational motion to concentrate only on defining the orientations along the path. It is also possible for the user to modify a particular point or portion of a guide while being assisted by it. We demonstrate in an industrial scenario that these innovations provide an intuitive solution to extend the use of virtual guides to 6 degrees of freedom and increase the human worker’s comfort during the programming phase of these guides in an assisted human–robot comanipulation context.

El impacto del aislamiento social en relación con el mundo real es amplio y es posible que se deba a los cambios sociales y antropológicos provocados por el uso del móvil. La necesidad de compartir información de manera inmediata no se limita a una cuestión de edad ni está restringida a un contexto específico. Puede manifestarse en cualquier entorno, ya sea escolar, familiar, social, profesional, cultural o incluso en las relaciones personales que puede desencadenar en una adicción actualmente no regulada. El objetivo de este estudio es identificar si existe relación entre el uso del móvil y la posibilidad de desarrollar una adicción en base a variables sociodemográficas, así como estudiar los posibles deterioros en el correcto desarrollo de las Funciones Ejecutivas. Para ello se ha estudiado la influencia de diferentes variables como edad y sexo de los participantes, su localización geográfica, así como su formación académica o situación laboral mediante un diseño descriptivo transversal. En este diseño transversal se estudia las diferencias entre la variable independiente (uso problemático del móvil) en función de las variables dependientes (género, edad, nivel de estudios, situación actual laboral y localidad). La muestra para este estudio está constituida por 196 participantes y es de tipo no probabilístico e incidental y fue evaluada mediante un cuestionario ad-hoc, el cual está compuesto por 10 ítems que incluyen variables sociodemográficas, uso del móvil, horas de su utilización, frecuencia de apagado y encendido de este por la noche, para qué se utiliza con mayor frecuencia, y por la escala MOBILE PHONE USE SCALE de López-Fernández et al. (2012), compuesta por 26 ítems, que tienen por objetivo explorar la existencia o ausencia de un uso excesivo o problemático del móvil. Los resultados indican que la variable edad y la variable sexo influyen significativamente en el riesgo del desarrollo del uso excesivo del móvil, siendo las demás variables sociodemográficas no relevantes para un uso problemático del móvil. En base a ello se proponen unas pautas de intervención psicológicas para el correcto desarrollo de las funciones ejecutivas en riesgo de desarrollar una posible adicción o uso excesivo del móvil.

Abstract

Multidisciplinary strategies have transformed the management of advanced ovarian cancer. We aimed to evaluate the effectiveness of paclitaxel in hyperthermic intraperitoneal chemotherapy (HIPEC) following surgical cytoreduction for ovarian peritoneal metastases in a randomized phase III trial conducted between August 2012 and December 2019. Seventy-six patients were randomized to either the HIPEC or no HIPEC group. Although median values for the primary endpoints (recurrence-free survival (RFS) and overall survival (OS)) revealed superior outcomes for the HIPEC (RFS: 23 months, OS: 48 months) over the control group (RFS: 19 months, OS: 46 months), these differences were not statistically significant (p = 0.22 and p = 0.579). Notably, the HIPEC group demonstrated significantly higher 5-year OS and 3-year RFS rates (47.2% and 47.5%) compared to patients without HIPEC (34.5% and 21.3%). Stratification according to Peritoneal Surface Disease Severity Score (PSDSS) showed improved OS and RFS for patients with lower PSDSS (I–II) in the HIPEC-treated group (p = 0.033 and p = 0.042, respectively). The Clavien–Dindo classification of adverse event grades revealed no significant differences between HIPEC and controls (p = 0.482). While overall results were not statistically significant, our long-term follow-up emphasized the potential benefit of HIPEC-associated cytoreduction with paclitaxel, particularly in selected ovarian cancer patients with lower PSDSS indices.

Abstract

Introduction: Parental satisfaction is rarely measured in the neonatal intensive care unit due to a lack of specific assessment tools. The Empowerment of Parents in The Intensive Care-Neonatology (EMPATHIC-N) questionnaire is an instrument to assess satisfaction in relation to family-centred care that has been validated in several countries, but not Spain. Objectives: To perform the translation and cultural adaptation of the EMPATHIC-N to Spanish followed by its validation for the purpose of assessing satisfaction in parents with children admitted to the neonatal intensive care unit. Material and methods: The questionnaire first underwent forward and backward translation and transcultural adaptation by a panel of experts through a standardized process based on the Delphi method, followed by a pilot study in 8 parents and then a cross-sectional study in the neonatal intensive care unit of a tertiary care hospital to assess the reliability and converging validity of the Spanish version. Results: The study proved the comprehensibility, validity, feasibility, applicability and usefulness of the Spanish version of the EMPATHIC-N in the field of paediatric health after being evaluated by 19 professionals and 60 parents. The content validity was found to be excellent (0.93). The reliability and convergent validity of the Spanish version of the EMPHATIC-N was analysed in a sample of 65 completed questionnaires. The Cronbach α for each domain was greater than 0.7, indicating a high internal consistency. We assessed validity by analysing the correlation of the 5 domains with the with the 4 general satisfaction items. The validity was found to be adequate (rs, 0.4−0.76; P
