1. Examining the mental health impacts of climate change on young people in Canada: a national cross-sectional survey

Abstract

Background: Climate change poses a substantial threat to the mental wellbeing of young people. Population-level research is urgently needed to help inform policies and interventions to ensure that young people are not burdened by long-term mental health impacts from climate change. We sought to identify the prevalence, distribution, and factors associated with climate change-related mental and emotional health outcomes among young people (aged 13–34 years) in Canada. Methods: This study is part of a larger cross-sectional survey, which examined mental and emotional health responses to climate change among individuals aged 13 years or older from across Canada. We used a multi-stage, multi-stratified random probability sampling procedure. Participants were randomly recruited through either an addressed letter or a telephone call. Online and telephone questionnaires were used to interview individuals in English, French, or Inuktitut between April 1, 2022, and March 31, 2023. Data were weighted by age and province using population estimates from Statistics Canada and analysed using descriptive statistics, factor analyses, and multivariable regression analyses. Findings: The full survey included 2476 participants, with a subgroup of 409 young people. Of the 401 respondents who provided their gender identity, 215 (54%) identified as cisgender women, 167 (42%) identified as cisgender men, and 19 (5%) identified as non-binary. Preliminary results suggest that young people in Canada experience a wide range of climate-related emotional and mental health outcomes. More than 70% of respondents in the young people subgroup reported having at least mild levels of sadness, anger, worry, anxiety, concern, helplessness, hopelessness, or powerlessness related to climate change. The severity of climate-related emotional responses differed by gender, with non-binary respondents and cisgender women reporting higher average levels of distress than cisgender men. Regional differences were also observed, with northern regions and urban locations reporting more severe reactions. Interpretation: This study builds on the understanding of the burden of climate change on the mental health of young people. If unaddressed, the impact of this burden could have long-standing and wide-reaching public health and related socioeconomic effects. Abstract

Abstract To most conservationists and many parents, it seems obvious that it is a good thing to teach children to value the natural world. Not only does connection with nature support their development and well‐being, but it also supports ongoing efforts by humans to sustain the natural world. However, there are incontrovertible trends towards a diminution of the state of nature as a consequence of human activities. In this context, as a thought experiment, we address a rather grim question: Should we still encourage children to be connected to nature, to care for it and be concerned about it? We first consider the meaning of connection to nature in the Anthropocene, and then turn to a consideration of several ethical dimensions of this problem, including the potential trade‐off between well‐known health benefits of time in nature and the long‐term psychological impacts of loss of nature (e.g., ecological grief and solastalgia). While there is no simple answer to our question, our analysis does highlight underappreciated ethical dilemmas of the Anthropocene as well as the value of the local, urban forms of nature to which children around the world are increasingly exposed and engaging with in unprecedented ways. Abstract

Climate change has severe and sweeping impacts on mental health. Although research is burgeoning on mental health impacts following climate and weather extremes, less is known about how common these impacts are outside of extreme events. Existing research exploring the prevalence of psychosocial responses to climate change primarily examines university students and uses non-random sampling methods. Herein, our protocol outlines an approach to data collection, processing, and analysis to estimate the population prevalence, magnitude, and distribution of mental health responses to climate change in Canada. A cross-sectional survey of youth and adults aged 13 years and older in Canada will be administered over the course of one year. The questionnaire will take approximately 10 minutes to complete orally and will be administered in English, French, and Inuktitut. The survey will consist of six sections: (1) self-reported past experiences of climate change; (2) self-reported climate-related emotions; (3) self-reported past and current impacts, anticipatory impacts, and vicarious experiences; (4) self-reported subclinical outcomes; (5) self-reported behavioural responses; and (6) demographics. A multi-stage, multi-stratified random probability sampling method will be used to obtain a sample representative of the Canadian population. We will use two different modes of recruitment: an addressed letter sent by postal mail or a telephone call (landlines and cellular). Population-weighted descriptive statistics, principal component analysis, and weighted multivariable regression will be used to analyse the data. The results of this survey will provide the first national prevalence estimates of subclinical mental health responses to climate change outcomes of people living in Canada.

Abstract

Introduction: Extreme weather events caused by climate change pose a risk to mental health. Illustrating this reality, this study estimates the impact of the 2021 Western North American heat dome (June 25th, 2021 – July 1st, 2021) on climate change anxiety among British Columbians. Methods: We conducted an online survey of British Columbians, aged ≥ 16. Participants were recruited using paid advertisements on social media. Data were collected pre- and post- heat dome between May 12th, 2021 – June 21st, 2021 (n = 439), and July 15th, 2021 – July 18th, 2021 (n = 420), respectively. A multivariable inverse Gaussian regression model tested differences in Climate Change Anxiety Scale (CCAS) scores pre- and post- heat dome. Potential confounders measured included age, gender, ethnicity, education, income, and political orientation. CCAS reliability was assessed using Cronbach's alpha. Results: Most participants indicated that they were much (40.1%) or somewhat (18.4%) more worried about climate change due to the heat dome. Mean CCAS scores increased from 1.66 (standard deviation [SD]= 0.80) to 1.87 (SD = 0.87) pre- and post- heat dome, respectively. Abstract

Background: Compared with other health areas, the mental health impacts of climate change have received less research attention. The literature on climate change and mental health is growing rapidly but is characterised by several limitations and research gaps. In a field where the need for designing evidence-based adaptation strategies is urgent, and research gaps are vast, implementing a broad, all-encompassing research agenda will require some strategic focus. Methods: We followed a structured approach to prioritise future climate change and mental health research. We consulted with experts working across mental health and climate change, both within and outside of research and working in high, middle, and low-income countries, to garner consensus about the future research priorities for mental health and climate change. Experts were identified based on whether they had published work on climate change and mental health, worked in governmental and non-governmental organisations on climate change and mental health, and from the professional networks of the authors who have been active in the mental health and climate change space. Results: Twenty-two experts participated from across low- and middle-income countries (n = 4) and high-income countries (n = 18). Our process identified ten key priorities for progressing research on mental health and climate change. Conclusion: While climate change is considered the biggest threat to global mental health in the coming century, tackling this threat could be the most significant opportunity to shape our mental health for centuries to come because of health co-benefits of transitioning to more sustainable ways of living. Research on the impacts of climate change on mental health and mental health-related systems will assist decision-makers to develop robust evidence-based mitigation and adaptation policies and plans with the potential for broad benefits to society and the environment.

6. Climate anxiety in children and young people and their beliefs about government responses to climate change: a global survey

Summary: Background: Climate change has important implications for the health and futures of children and young people, yet they have little power to limit its harm, making them vulnerable to climate anxiety. This is the first large-scale investigation of climate anxiety in children and young people globally and its relationship with perceived government response. Methods: We surveyed 10 000 children and young people (aged 16–25 years) in ten countries (Australia, Brazil, Finland, France, India, Nigeria, Philippines, Portugal, the UK, and the USA; 1000 participants per country). Invitations to complete the survey were sent via the platform Kantar between May 18 and June 7, 2021. Data were collected on participants’ thoughts and feelings about climate change, and government responses to climate change. Descriptive statistics were calculated for each aspect of climate anxiety, and Pearson's correlation analysis was done to evaluate whether climate-related distress, functioning, and negative beliefs about climate change were linked to thoughts and feelings about government response. Findings: Respondents across all countries were worried about climate change (59% were very or extremely worried and 84% were at least moderately worried). More than 50% reported each of the following emotions: sad, anxious, angry, powerless, helpless, and guilty. More than 45% of respondents said their feelings about climate change negatively affected their daily life and functioning, and many reported a high number of negative thoughts about climate change (eg, 75% said that they think the future is frightening and 83% said that they think people have failed to take care of the planet). Respondents rated governmental responses to climate change negatively and reported greater feelings of betrayal than of reassurance. Climate anxiety and distress were correlated with perceived inadequate government response and associated feelings of betrayal. Interpretation: Climate anxiety and dissatisfaction with government responses are widespread in children and young people in countries across the world and impact their daily functioning. A perceived failure by governments to respond to the climate crisis is associated with increased distress. There is an urgent need for further research into the emotional impact of climate change on children and young people and for governments to validate their distress by taking urgent action on climate change. Abstract

Addressing the biodiversity crisis requires renewed collaborative approaches. Large carnivores are ambassador species, and as such they can aid the protection of a wide range of species, including evolutionarily distinct and threatened ones, while being popular for conservation marketing. However, conflicts between carnivores and people present a considerable challenge to biodiversity conservation. Our cross disciplinary essay brings together original research to discuss key issues in the conservation of large carnivores as keystone species for biodiversity rich, healthy ecosystems. Our findings suggest the need to promote coexistence through challenging ‘wilderness’ myths; to consider coexistence/conflict as a continuum; to include varied interest groups in decision making; to address fear through positive mediated experiences, and to explore further partnerships with zoos. As wide-reaching institutions visited by over 700 million people/year worldwide, zoos combine knowledge, emotion and social context creating ideal conditions for the development of care towards nature, pro-environmental behaviors and long-term connections between visitors and carnivores. Based on current research, we provide evidence that large carnivores and zoos are both powerful catalysts for public engagement with biodiversity conservation, recognizing barriers and suggesting future ways to collaborate to address biodiversity loss.

Abstract

Psychology has an important part to play in helping society understand and adapt to the growing threat of climate change. Psychological research has explored people’s perceptions and beliefs about climate change, uncovering some of the important factors that inhibit or promote awareness. Researchers have also begun to explore the current and potential impacts of climate change on psychosocial wellbeing, including short-term impacts of natural disasters as well as more gradual, long-term impacts of a warming and less predictable climate. In addition to threatening mental health, evidence is accumulating that climate change may lead to increase suicide and social conflict. The distribution of impacts is certain to heighten social injustice and inequity. Research on behavioral change suggests ways of encouraging positive adaptations and more sustainable behavior. To maximize the effectiveness of their research, psychologists need to work collaboratively with people from other professions.

9. Emotions and Perceptions Surrounding Teaching Climate Change in the United States: Results from a Teacher Survey

Abstract

Children are worried about climate change. Educational settings provide an opportunity to help children cope with their worries, but teachers do not always feel comfortable doing so. One reason for hesitancy may be their own negative emotions. This paper describes findings from a survey of climate-aware teachers, looking at their perceptions and emotions about climate change, as well as their experiences in teaching climate-related topics. Teachers had both positive and negative responses to climate change; motivation and compassion along with frustration and sadness were the four most common emotional responses. They also generally felt supported in their teaching. Both positive and negative emotions inspired teachers to talk with their students about climate change, but feeling climate anxiety was associated with hesitancy. There was evidence that increased understanding of climate change was associated with lower climate anxiety. Teachers reported that they would value more support for teaching climate change.

10. Navigating Eco-Anxiety and Eco-Detachment: Educators' Strategies for Raising Environmental Awareness Given Students' Disconnection from Nature

Awareness of environmental problems such as climate change can motivate action, but educators debate whether to raise students' awareness given that it may provoke eco-anxiety. We have even less understanding of how these relationships are affected by young people's growing disconnection from nature. Through 28 semi-structured interviews in Canada and the United Kingdom, we explore how educators perceive students' nature connection and eco-anxiety and how they introduce discussion of environmental problems. Educators frequently observed experiential, cognitive, and emotional indicators of nature disconnection and eco-anxiety, although many (39%) reported rarely, if ever, witnessing such environmentally related distress. Educators prioritised improving nature connection over raising awareness of environmental problems. When they discuss these issues with students, they emphasise hope and encourage pro-environmental behaviours to avoid eliciting eco-anxiety for those not currently experiencing it, a strategy that is partially inconsistent with literature suggesting some eco-anxiety can nurture pro-environmental behaviour. Our findings provide new insights into the challenges that educators face in helping their students navigate current environmental trends.

Abstract

Background. Evacuation and sheltering are commonly used strategies for disaster risk reduction and climate change adaptation, but may negatively affect mental health of internally displaced persons (IDPs). Recently, Chinese governments have developed planned settlements providing integrated and intensive health services and environmental interventions to reduce immediate disastrous impacts and support the mental health of IDPs. Methods. Here we selected 69 planned shelters by stratified sampling to describe the implemented interventions conducted in Anhui Province of China after the 2016 severe floods, and we used standardized psychological scales to survey the intervention group (IDP who lived in these planned shelters) and the matched control group (victims living in their homes). Multivariable logistic regression was used to examine the association between social-demographic characteristics, flooding exposure, environmental conditions and the psychological diseases. Adjusted odds ratios (ORs) were calculated to compare their prevalence of psychological diseases, and to identify its influencing factors though comparing multiple interventions. Finally, the structural equation modeling was used to identify their influencing pathways. Results. Compared with the control group, the intervention group had a significantly lower risk of anxiety (OR = 0.36; 95% CI: 0.18–0.71), depression (OR = 0.36; 95% CI: 0.19–0.68) and post-traumatic stress disorder (OR = 0.29; 95% CI: 0.15–0.56). Environmental interventions providing clean water, safe food, environmental hygiene, risk communication and sufficient accommodation had a protective effect (standardized indirect effect = −0.153, p < 0.01) on the risk of psychological problems, mediating the negative effect caused by displacement and sheltering. Conclusions. How planned shelters were used to achieve better mental health outcomes in Anhui could inform other flood-prone areas to mitigate psychological vulnerability of IDPs.

Nineteenth century reeditions and reworkings of the Renaissance The Faerie Queene, can be equated with renewed interest in one of Spenser’s leading protagonists, Britomart, daughter of a knight, who herself passed as one. This Victorian interest in an example of transgenderism coincided with women’s demands for socio-cultural changes. We shall address the questions of links between gender and power, and consider how the past is used to consolidate the present.Our analysis starts by contextualising Victorian revivals of Britomart’ story, paying special attention to a prose adaptation by Mary MacLeod, then assessing these revivals in relation to women’s demands for change, bearing in mind that British suffragettes also looked across the Channel to another female knight-at-arms, Joan of Arc, for a model for their campaigns. Finally we will consider certain cultural implications when such a resurgence interacts with challenges to gender.

The intention of this report is to provide an overview of the intersection of climate change and children's and youth's mental health. The report aims to offer a synopsis of both impacts and responses, translating peer-reviewed research, agency reports, and other primary sources to help empower a wider audience with useful insights and guidance. It is intended for health and mental health professionals and institutions, climate and children's advocates and advocacy organizations, policymakers, youth, parents, educators, and other organizations or people interested in the topic. It is worth noting that there is a large body of literature in this field, particularly regarding child development, that goes beyond the scope of this report. While we dive into the impacts of climate change on children's and youth's mental health and describe potential responses, it's also important to acknowledge that the topic is complex, and more research needs to be done to fully understand this critical issue. The report's primary focus is the United States, but many findings will be relevant to other areas of the world. We hope this report adds to the work being done to bridge the gap between climate change and children's and youth's mental health and well-being, fostering a broader understanding and motivation to act.

The American Psychological Association and ecoAmerica are pleased to offer this update of our 2017 report, Mental Health and Our Changing Climate. Since that report appeared, concerns about the mental health impacts of climate change have grown among health professionals, policymakers, and the public. Research on climate and health has accelerated and many new findings have emerged. With this update, we aim to provide readers with the information and guidance they need to stay current in this field and take effective action.Indeed, now is the time to act. Climate change has become a crisis. As recent studies confirm, our society must begin now to adapt to the inevitable impacts of climate change on health and well-being and move toward reducing climate change. We know how to do this. What is needed is the commitment of individuals, communities, and nations to act.This report provides the latest information on the multiple effects of climate change on mental health, the structural inequities that lead to some populations bearing greater impacts, and how people think about and respond to climate change. It goes on to describe how individuals and communities can strengthen their resilience in the face of climate change and work for climate policies that will lead to a healthier and more just society.

Abstract

Conservation is predominantly an exercise in trying to change human behaviour – whether that of consumers whose choices drive unsustainable resource use, of land managers clearing natural habitats, or of policymakers failing to deliver on environmental commitments. Yet conservation research and practice have made only limited use of recent advances in behavioural science, including more novel behaviour change interventions. Instead conservationists mostly still rely on traditional behaviour change interventions – education, regulation and material incentivisation – largely without applying recent insights from behavioural science about how to improve such approaches. This paper explores how behavioural science could be more widely and powerfully applied in biodiversity conservation. We consider the diverse cast of actors involved in conservation problems and the resulting breadth of behaviour change that conservationists might want to achieve. Drawing on health research, we present a catalogue of types of interventions for changing behaviour, considering both novel, standalone interventions and the enhancement of more traditional conservation interventions. We outline a framework for setting priorities among interventions based on their likely impact, using ideas developed for climate change mitigation. We caution that, despite its promise, behavioural science is not a silver bullet for conservation. The effects of interventions aimed at changing behaviour can be modest, temporary, and context-dependent in ways that are as-yet poorly understood. We therefore close with a call for interventions to be tested and the findings widely disseminated to enable researchers and practitioners to build a much-needed evidence base on the effectiveness and limitations of these tools.

Abstract

Research indicates that beautiful nature can have positive impacts. Does polluted nature have a corresponding negative impact? This paper presents two experiments investigating the impact of viewing images of natural settings, on a college campus, that do or do not contain litter. The moderating role of environmental identity was also examined. Study 1 showed that landscapes with litter evoked more negative emotions among a sample of 332 U.S. residents on MTurk than did landscapes without litter. Surprisingly, natural landscapes did not have a more positive effect than images of buildings. In Study 2, using an MTurk sample of 310 U.S. residents, results were similar to Study 1 but were qualified by an interaction between condition and EID: those high in EID were more strongly affected by the images. These results suggest that viewing polluted landscapes can have a negative effect on emotions (hedonic wellbeing), and that these effects are stronger among those who have a stronger relationship with nature. There was no evidence for an impact on eudaimonic wellbeing as represented by a sense of meaning, efficacy, or ethicality. Given the continuing degradation of our natural environment, further research on the impacts of polluted landscapes is needed.

Abstract

Human activities are degrading ecosystems worldwide, posing existential threats for biodiversity and humankind. Slowing and reversing this degradation will require profound and widespread changes to human behaviour. Behavioural scientists are therefore well placed to contribute intellectual leadership in this area. This Perspective aims to stimulate a marked increase in the amount and breadth of behavioural research addressing this challenge. First, we describe the importance of the biodiversity crisis for human and non-human prosperity and the central role of human behaviour in reversing this decline. Next, we discuss key gaps in our understanding of how to achieve behaviour change for biodiversity conservation and suggest how to identify key behaviour changes and actors capable of improving biodiversity outcomes. Finally, we outline the core components for building a robust evidence base and suggest priority research questions for behavioural scientists to explore in opening a new frontier of behavioural science for the benefit of nature and human wellbeing.

Abstract

Background: Climate change has significant implications for the health and futures of children and young people, yet they have little power to limit its harm, making them vulnerable to increased climate anxiety. Qualitative studies show climate anxiety is associated with perceptions of inadequate action by adults and governments, feelings of betrayal, abandonment and moral injury. This study offers the first large-scale investigation of climate anxiety in children and young people globally and its relationship to government response. Methods: We surveyed 10,000 young people (aged 16-25 years) in ten countries. Data were collected on their thoughts and feelings about climate change, and government response. Findings: Respondents were worried about climate change (59% very or extremely worried, 84% at least moderately worried). Over 50% felt sad, anxious, angry, powerless, helpless, and guilty. Over 45% said their feelings about climate change negatively affected their daily life and functioning, and many reported a high number of negative thoughts about climate change. Respondents rated the governmental response to climate change negatively and reported greater feelings of betrayal than of reassurance. Correlations indicated that climate anxiety and distress were significantly related to perceived inadequate government response and associated feelings of betrayal. Interpretation: Climate change and inadequate governmental responses are associated with climate anxiety and distress in many children and young people globally. These psychological stressors threaten health and wellbeing, and could be construed as morally injurious and unjust. Abstract

International audience; Direct experiences of nature are increasingly being replaced by technologically-mediated ones, with unclear implications for people's attitudes toward nature and toward environmental conservation. In this essay, we argue that it is useful to think not simply about the extinction of nature-based experience, but about the specific ways in which experiences of nature are being transformed into different types of experience, in order to consider the possible effects. Two important characteristics of these transformed experiences are that they are standardized, and that they encompass less sensory richness; based on research on the positive effects of direct experience, these characteristics suggest a possible reduction in both human wellbeing and support for environmental conservation. Because the transformation of nature experience is unlikely to be reversed, we encourage formal and informal environmental education that mindfully teaches children how to think about the relationship between virtual and real environments, so that one does not completely supplant the other.

Abstract

The Intergovernmental Panel on Climate Change has encouraged psychologists to become part of the integrated scientific effort to support the achievement of climate change targets such as keeping within 1.5°C or 2°C of global warming. To date, the typical psychological approach has been to demonstrate that specific concepts and theories can predict behaviors that contribute to or mitigate climate change. Psychologists need to go further and, in particular, show that integrating psychological concepts into feasible interventions can reduce greenhouse gas emissions far more than would be achieved without such integration. While critiquing some aspects of current approaches, we describe psychological research that is pointing the way by distinguishing different types of behavior, acknowledging sociocultural context, and collaborating with other disciplines. Engaging this challenge offers psychologists new opportunities for promoting mitigation, advancing psychological understanding, and developing better interdisciplinary interactions. Effectively addressing environmental challenges such as climate change will require adopting policy measures that have some impact on collective human behavior. The present research examined attitudes toward different environmental policies, specifically focusing on the role of perceived justice. Justice was measured in two ways: as an assessment of the fairness of a particular policy and as a general tendency to endorse statements related to environmental justice. Because justice judgments can be context specific, policies were presented in four conditions, in a 2 × 2 design manipulating the type of impact described, ecological or societal, and the level of focus, individual or collective. The roles of political ideology and environmentalism were also investigated. Results from an online sample of 162 US residents showed that non-coercive policies, overall, were rated as more acceptable. Environmental justice statements were strongly endorsed, and justice in both its specific and general forms was a determinant of policy acceptance. In particular, ratings of the fairness of specific policies were a stronger determinant of acceptability than perceived effectiveness of the policy. Type of impact had little effect, but policies tended to be rated as more acceptable when they were framed in terms of the collective rather than the individual. Although a liberal ideology was associated with acceptance of environmental policies in general and with endorsement of environmental justice, controlling for endorsement of environmental justice eliminated the effect of political ideology in most, but not all, cases. Implications for policy support are discussed.

Abstract

As an organization that promotes socially relevant research, SPSSI encourages psychologists to engage in individual behaviors—their research activities—that are connected to issues of social interest. Thus, it can be argued that one of SPSSI's goals is to strengthen the connection between social issues and personal life, broadly defined. Some of the most urgent and salient social issues currently revolve around the natural environment, climate change in particular. This is an issue which many people do not connect to their personal lives. In this article, after briefly explaining why environmental issues should be considered social issues, I discuss the way in which environmental issues relate to personal life, and use environmental issues to explore the connection between social issues and personal life. I argue that the ability to make this connection involves cognitive processes such as systems thinking, and encourage more research in this area.

Abstract

Despite decades of awareness about the biodiversity crisis, it remains a wicked problem. Besides preservation and restoration strategies, one approach has fo-cused on increasing public concern about biodiversity issues by emphasizing opportunities for people to experience natural environments. In this article, we endeavor to complicate the understanding of these experiences of nature (EoN). Because EoN are embedded in social and cultural contexts, transforma-tive or new EoN are emerging in combination with societal changes in work, home, and technology. Policies that acknowledge and accept a diversity of culturally situated EoN, including negative EoN, could help people reconnect with the complexity and dynamics of biodiversity. A new conceptualization of EoN that encompasses diverse experiences and reflects the sociocultural context could help to stimulate a broader transformation in the relationship between society and nature, one that better integrates the two spheres. Such a transformation is necessary to more effectively address the biodiversity crisis.

The biodiversity crisis is not salient to many people. A zoo visit not only provides the opportunity to learn about the issue, but also provides direct experiences with animals that may increase public engagement. The present study used a nonequivalent pretest–posttest design to assess the impact of a zoo visit on conservation knowledge and engagement by comparing 88 visitors entering a zoo in Paris and 84 visitors on their way out. Those who had completed their visit scored higher on conservation knowledge, general concern about threats to biodiversity, and perceived self-efficacy to protect biodiversity. Notably, conservation knowledge was not highly correlated with the other dependent variables, but self-efficacy was significantly correlated with environmental concern, behaviour, and behavioural intent. We conclude that a zoo visit does have a positive impact on knowledge and concern, and by affecting self-efficacy, it has the potential to influence future behavior.

Abstract

Background: Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy.Methods: RESTART was a prospective, randomised, open-label, blinded-endpoint, parallel-group trial at 122 hospitals in the UK that assessed whether starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. For this prespecified subgroup analysis, consultant neuroradiologists masked to treatment allocation reviewed brain CT or MRI scans performed before randomisation to confirm participant eligibility and rate features of the intracerebral haemorrhage and surrounding brain. We followed participants for primary (recurrent symptomatic intracerebral haemorrhage) and secondary (ischaemic stroke) outcomes for up to 5 years (reported elsewhere). For this report, we analysed eligible participants with intracerebral haemorrhage according to their treatment allocation in primary subgroup analyses of cerebral microbleeds on MRI and in exploratory subgroup analyses of other features on CT or MRI. The trial is registered with the ISRCTN registry, number ISRCTN71907627.Findings: Between May 22, 2013, and May 31, 2018, 537 participants were enrolled, of whom 525 (98%) had intracerebral haemorrhage: 507 (97%) were diagnosed on CT (252 assigned to start antiplatelet therapy and 255 assigned to avoid antiplatelet therapy, of whom one withdrew and was not analysed) and 254 (48%) underwent the required brain MRI protocol (122 in the start antiplatelet therapy group and 132 in the avoid antiplatelet therapy group). There were no clinically or statistically significant hazards of antiplatelet therapy on recurrent intracerebral haemorrhage in primary subgroup analyses of cerebral microbleed presence (2 or more) versus absence (0 or 1) (adjusted hazard ratio [HR] 0·30 [95% CI 0·08–1·13] vs 0·77 [0·13–4·61]; pinteraction=0·41), cerebral microbleed number 0–1 versus 2–4 versus 5 or more (HR 0·77 [0·13–4·62] vs 0·32 [0·03–3·66] vs 0·33 [0·07–1·60]; pinteraction=0·75), or cerebral microbleed strictly lobar versus other location (HR 0·52 [0·004–6·79] vs 0·37 [0·09–1·28]; pinteraction=0·85). There was no evidence of heterogeneity in the effects of antiplatelet therapy in any exploratory subgroup analyses (all pinteraction>0·05).Interpretation: Our findings exclude all but a very modest harmful effect of antiplatelet therapy on recurrent intracerebral haemorrhage in the presence of cerebral microbleeds. Further randomised trials are needed to replicate these findings and investigate them with greater precision.Funding: British Heart Foundation.

Abstract

Background: Results of small trials indicate that fluoxetine might improve functional outcomes after stroke. The FOCUS trial aimed to provide a precise estimate of these effects.Methods: FOCUS was a pragmatic, multicentre, parallel group, double-blind, randomised, placebo-controlled trial done at 103 hospitals in the UK. Patients were eligible if they were aged 18 years or older, had a clinical stroke diagnosis, were enrolled and randomly assigned between 2 days and 15 days after onset, and had focal neurological deficits. Patients were randomly allocated fluoxetine 20 mg or matching placebo orally once daily for 6 months via a web-based system by use of a minimisation algorithm. The primary outcome was functional status, measured with the modified Rankin Scale (mRS), at 6 months. Patients, carers, health-care staff, and the trial team were masked to treatment allocation. Functional status was assessed at 6 months and 12 months after randomisation. Patients were analysed according to their treatment allocation. This trial is registered with the ISRCTN registry, number ISRCTN83290762.Findings: Between Sept 10, 2012, and March 31, 2017, 3127 patients were recruited. 1564 patients were allocated fluoxetine and 1563 allocated placebo. mRS data at 6 months were available for 1553 (99·3%) patients in each treatment group. The distribution across mRS categories at 6 months was similar in the fluoxetine and placebo groups (common odds ratio adjusted for minimisation variables 0·951 [95% CI 0·839–1·079]; p=0·439). Patients allocated fluoxetine were less likely than those allocated placebo to develop new depression by 6 months (210 [13·43%] patients vs 269 [17·21%]; difference 3·78% [95% CI 1·26–6·30]; p=0·0033), but they had more bone fractures (45 [2·88%] vs 23 [1·47%]; difference 1·41% [95% CI 0·38–2·43]; p=0·0070). There were no significant differences in any other event at 6 or 12 months.Interpretation: Fluoxetine 20 mg given daily for 6 months after acute stroke does not seem to improve functional outcomes. Although the treatment reduced the occurrence of depression, it increased the frequency of bone fractures. These results do not support the routine use of fluoxetine either for the prevention of post-stroke depression or to promote recovery of function.Funding: UK Stroke Association and NIHR Health Technology Assessment Programme.

Abstract

The environmental identity (EID) scale, first published in 2003, was developed to measure individual differences in a stable sense of interdependence and connectedness with nature. Since then, it has been reliably correlated with measures of environmental behavior and concern. However, the original scale was developed based on U.S. college students, raising questions about its validity for other types of populations. This study revised the EID scale and tested it in five countries (four continents) with a total sample size of 1717 participants. Results support strong internal consistency across all locations. Importantly, EID was significantly correlated with behavior and with environmental concern. This research gives us greater confidence that the EID construct is meaningful across different cultural contexts. Because the revised EID was designed to be relevant to a wider range of people and experiences, it is recommended as a replacement for the 2003 version.

Abstract

Climate change will affect psychological wellbeing. Substantial research has documented harmful impacts on physical health, mental health, and social relations from exposure to extreme weather events that are associated with climate change. Recently, attention has turned to the possible effects of climate change on mental health through emotional responses such as increased anxiety. This paper discusses the nature of climate anxiety and some evidence for its existence, and speculates about ways to address it. Although climate anxiety appears to be a real phenomenon that deserves clinical attention, it is important to distinguish between adaptive and maladaptive levels of anxiety. A focus on individual mental health should not distract attention from the societal response that is necessary to address climate change.

Abstract

There is increasing attention to the negative emotional responses associated with awareness of climate change. We present three studies developing a scale of climate change anxiety. In Study 1, the scale was developed and validated in an MTurk sample of 197. Exploratory factor analysis of our item pool revealed a four-factor structure, with cognitive-emotional impairment, functional impairment, behavioral engagement, and experience emerging as unique factors. Cognitive-emotional impairment and functional impairment were considered to constitute subscales for climate change anxiety; along with behavioral engagement, they were all related to experience as well as to negative emotions. Neither climate change anxiety nor general depression and anxiety were related to behavioral engagement. Study 2 replicated the factor structure as well as the pattern of correlations in a second MTurk sample of 199. Study 3 examined the relationship between climate change anxiety and adaptation responses in a sample of 217, and tested whether climate change anxiety scores would be affected by the framing of a climate change message. Overall, results suggest that climate change anxiety is not uncommon, especially among younger adults; that worry can be differentiated from a more serious impact on one's life; and that climate change anxiety is correlated with emotional but not behavioral responses to climate change.
