Background and Aims: Caudal block is more frequently used in children for postoperative analgesia. However, its disadvantage is its short duration. Erector spinae plane block (ESPB) at the sacral level can potentially block the pudendal nerve. It may prove an alternative to caudal block for hypospadias repair regarding time to first rescue analgesia. Methods: Fifty children of 2–7 years of age were included. After induction of general anaesthesia, Group I (n = 25) was given ultrasound-guided sacral ESPB with 1 ml/kg of 0.25% bupivacaine and Group II (n = 25) was given caudal block with 0.5 ml/kg of 0.25% bupivacaine. Postoperatively at face, leg, activity, cry, consolability (FLACC) score ≥4, rescue analgesia was given using intravenous 15 mg/kg paracetamol. The primary outcome was to compare time to first rescue analgesia, and secondary outcomes were intraoperative haemodynamic parameters, fentanyl consumption, postoperative FLACC score and analgesic consumption in 24 h. Continuous variables were compared using the independent sample t-test or Mann–Whitney test, and categorical variables were compared using the Chi-square test. Results: The mean time to first rescue analgesia was 21.30 (standard deviation [SD]: 3.06) h in Group I and 9.36 (SD: 1.71) h in Group II (P < 0.001) (mean difference −11.94 [95% CI: −13.39, −10.48]). The FLACC score was significantly higher (P < 0.05) postoperatively at 8, 10, 12 and 18 h in Group II. Mean postoperative analgesic consumption was 310.5 (SD: 72.69) mg in Group I and 615.6 (SD: 137.51) mg in Group II (P < 0.001) (mean difference 30.5 [95% CI: 236.41, 373.78]). Conclusion: Sacral ESPB is better regarding time to first rescue analgesia than caudal block in paediatric patients undergoing hypospadias repair.

Background and Aims: Both laryngoscopy and endotracheal intubation individually contribute to hemodynamic pressor response following endotracheal intubation. Parker Flex-Tip tracheal tube has a flexible, curved, smooth, centered, and tapered distal tip that is designed to facilitate rapid, easy, and nontraumatic intubation. The present study was conducted to compare Parker Flex-Tip and polyvinyl chloride (PVC) endotracheal tube (ETT) for hemodynamic response during intubation. Materials and Methods: Sixty patients of either sex of age 18–50 years, belonging to the American Society of Anesthesiologists (ASA) physical Status I or II scheduled for elective surgery under general anesthesia requiring endotracheal intubation were included. In Group I (n = 30), Parker Flex-Tip tube was used and in Group II (n = 30), PVC ETT was used. Heart rate (HR), systolic blood pressure, diastolic blood pressure (DBP), and mean arterial pressure were compared at various time intervals. Quantitative variables were compared using unpaired t-test and qualitative variables were compared using Chi-square test. P ≤0.05 was considered statistically significant. Results: The mean basal value of HR was 81.33 ± 13.78 beats/min in Group I and 87.36 ± 13.34 beats/min in Group II (P = 0.09). The difference between both the groups was found to be comparable at baseline, prelaryngoscopy, and just after laryngoscopy (AL) but this difference was found to be statistically significant just after intubation (AI) and at 1 min AI. In both groups, DBP started increasing just AL but the difference was not significant. This difference between the two groups was observed to be statistically significant at 1 min AI (81.73 ± 13.29 in Group I vs. 88.96 ± 10.27 in Group II, [P = 0.02]). Conclusion: Parker Flex-Tip tube is beneficial in controlling the hemodynamic response during laryngoscopy and intubation. This effect is significantly evident in HR and DBP.

Introduction: Comforting manoeuvres like hand holding and calming conversation with the parturients may decrease anxiety among them by establishing a rapport and building confidence and trust to alleviate fear. Aim: Evaluation of role and effectiveness of comforting manoeuvres (hand holding and calming conversation) in relieving patient’s anxiety and subjective satisfaction undergoing caesarean section in regional anaesthesia. Materials and Methods: This was a single centre randomised control trial conducted in the Department of Anaesthesiology and Critical Care, Post Graduate Institute of Medical Sciences, Rohtak, Haryana, India, from February 2022 to June 2022. There were 144 parturients, in the age group 18-40 years, American Society of Anaesthesiology (ASA) II, and admitted for Caesarean section under regional anaesthesia. Patients were randomised into four groups- control group (C)- group 1, only calming conversation (CC)- group 2, only Hand Holding (HH)- group 3 and Hand Holding and Calming Conversation (CH)- group 4. Demographic details of the patient, Visual Analogue Scale (VAS) for anxiety, Blood Pressure (BP), Heart Rate (HR), Respiratory Rate (RR) and patient satisfaction score on a 1-5 Likert scale were noted preoperatively. VAS was noted preoperatively and postoperatively for all groups. Patient Satisfication Score (PSS) was noted postoperatively. The HR, Mean Arterial Pressure (MAP), and RR were noted every 5 minutes for 20 minutes, then every 10 minutes till the end of the surgery, and 30 minutes after surgery. Any anxiolytic medication used intraoperatively was documented for all groups. Results: Total of 144 participants were analysed with 36 patients in each of four groups. Mean age (years) for group 1 was 23.36, group 2 was 23.25, group 3 was 23.17, and group 4 was 23.22. Baseline VAS was similar in all the groups. Post-operative VAS differed significantly - group 1 was 3.33±0.926, group 2 was 1.53±0.845, group 3 was1.47±0.845 and group 4 was 1.11±0.708. PSS also differed significantly - for group 1 was 2.42±0.732, group 2 was 3.50±0.697, group 3 was 3.67±0.717 and group 4 was 3.92±0.692. Stabilisation of hemodynamics in terms of BP, HR and RR was significantly better in all three interventional groups as compared to the control group. Conclusion: All three manoeuvres (hand holding, calming conversation alone and in combination) were equally effective in reducing perioperative anxiety, stabilising the hemodynamics and improving patient satisfaction in parturients undergoing caesarean section under regional anaesthesia. Comforting manoeuvres are simple, easy to practice and without any financial implication.

Background and Aims: Airtraq has been found to be useful in improving the view of the glottis. However, directing the tube tip into the glottis can be challenging during nasotracheal intubation. This problem gets resolved if the bougie is placed first. The present study was conducted for the evaluation of nasotracheal intubation using a nasal Airtraq laryngoscope with and without a bougie. Methods: Fifty patients of either gender, aged between 18 and 60 years, who belonged to the American Society of Anesthesiologists physical status (PS) I or II, requiring nasotracheal intubation were included. In group I (number (n) = 25), nasotracheal intubation was performed with a bougie, and in group II (n = 25), it was performed without a bougie. The primary objective was a comparison of the time taken to achieve successful nasotracheal intubation. Secondary objectives were ease of intubation and additional manoeuvres required for intubation. Results: The mean (± standard deviation) for time for intubation in group I was 59.24 ± 9.98 s and that in group II was 41.00 ± 4.23 s (P = 0.001). Two patients (8%) in group I and ten patients (40%) in group II required additional manoeuvres for intubation (P = 0.008). Twenty-three patients (92%) in group I and 15 patients (60%) in group II had easy intubation (P = 0.030). In group I, no patient had trauma, whereas, in group II, four patients (16%) had trauma (P = 0.030) during intubation. Conclusion: The time taken for nasotracheal intubation using an Airtraq laryngoscope was more with the use of a bougie as compared to the non-bougie technique. However, bougie-guided intubation was easier with less requirement of additional manoeuvres. In addition, trauma was also significantly less with the bougie technique.

Introduction: Patients with oropharyngeal masses pose a challenge for an anesthetist in terms of ventilation and tracheal intubation. Thus, preoperative assessment and preparation become an integral part of the management of such anticipated difficult airway cases.Case report: A 45-year-old female presented with growth in the oropharynx causing dysphagia and hoarseness of voice. Clinical examination and investigations predicted a difficult airway. It was managed with fibreoptic nasotracheal intubation with successful perioperative outcomes. Tracheostomy was kept as plan B in case of the CVCI situation.Conclusion: Careful preoperative examination and assessment are required to prepare oneself for a difficult airway. Fibreoptic bronchoscope-guided nasotracheal intubation in a spontaneously breathing patient is a safe and successful technique of airway management in difficult airway cases.

Background and Aims: Though manufacturer recommendations suggest use of specific endotracheal tube (ETT) with intubating laryngeal mask airway (ILMA) and air-Q intubating laryngeal airway (ILA), Parker Flex Tip tube introduced by J D Parker has certain advantages and is also cost-effective. This study was conducted to compare ILMA and air-Q ILA for intubation using Parker Flex Tip tube. Methods: Patients of either gender, aged 18–60 years, scheduled for elective surgery requiring endotracheal intubation were included in this study. In group A (n = 55), blind intubation was done through ILMA using Parker Flex Tip tube and in group B (n = 55), blind intubation was done through air-Q ILA using Parker Flex Tip tube. Success rate, number of attempts, ease and a total time of intubation were recorded. Results: Intubation was successful in 54 patients (98.2%) in group A and in 46 patients (85.2%) in group B (P = 0.026). Intubation was significantly easy with ILMA (P = 0.048). Manoeuvres for intubation were used in 10.9% patients in group A while it was used in 27.8% patients in group B. Significantly, more manoeuvres were required with air-Q ILA for intubation (P = 0.026). Number of attempts for ETT placement (P = 0.092), insertion time of ETT (TT) (P = 0.472) and total time taken for successful intubation (P = 0.526) were comparable in both the groups. Conclusion: The intubating laryngeal mask airway was superior to the air-Q intubating laryngeal airway for blind intubation using Parker Flex Tip tube.

Amniotic fluid embolism (AFE) is one of the catastrophic complications of pregnancy in which amniotic fluid, fetal cells, hair, or other debris enters into the maternal pulmonary circulation, causing cardiovascular collapse. Etiology largely remains unknown, but may occur in healthy women during labour, during cesarean section, after abnormal vaginal delivery, or during the second trimester of pregnancy. It may also occur up to 48 hours post-delivery. It can also occur during abortion, after abdominal trauma, and during amnio-infusion. The pathophysiology of AFE is not completely understood. Possible historical cause is that any breach of the barrier between maternal blood and amniotic fluid forces the entry of amniotic fluid into the systemic circulation and results in a physical obstruction of the pulmonary circulation. The presenting signs and symptoms of AFE involve many organ systems. Clinical signs and symptoms are acute dyspnea, cough, hypotension, cyanosis, fetal bradycardia, encephalopathy, acute pulmonary hypertension, coagulopathy etc. Besides basic investigations lung scan, serum tryptase levels, serum levels of C3 and C4 complements, zinc coproporphyrin, serum sialyl Tn etc are helpful in establishing the diagnosis. Treatment is mainly supportive, but exchange transfusion, extracorporeal membrane oxygenation, and uterine artery embolization have been tried from time to time. The maternal prognosis after amniotic fluid embolism is very poor though infant survival rate is around 70%.

Background and Objective: There are various techniques to identify epidural space but superiority of one technique over other has not been adequately studied. We conducted a study to Compare and evaluate the three techniques for epidural space localization that is, loss of resistance (LOR) syringe technique, balloon technique and drip infusion technique. Materials and Methods: Seventy-five patients of either sex, belonging to American Society of Anesthesiologists physical status Class 1 or 2, between 20 and 50 years of age, scheduled to undergo lower abdominal and lower limb surgeries were randomly allocated to one of the three groups (n = 25 each) depending upon epidural space localization. In Group I, epidural space localization was done with LOR syringe technique. In Group II Balloon technique and in Group III drip infusion technique was used. Distance of the epidural space from skin, number of attempts, time taken for epidural space localization and quality of the block were the parameter recorded during the study. Results: First attempt success rate for epidural space localization was highest in Group III (100%). The mean time taken for epidural space localization was least in Group III, and when compared with other groups it was found to be statistically significant with P = 0.016. Number of attempt for space localization and success rate of the block was better in the majority of patients of Group III, but the difference was found to be statistically nonsignificant. Complication rate was almost negligible in all three techniques. Conclusion: We conclude that the time taken to localize the epidural space was least in drip infusion technique. As for number of attempts, quality of the block and complications is concerned, all the three techniques are comparable.

Background and Aims: Infraclavicular (IC) approach of subclavian vein (SCV) catheterisation is widely used as compared to supraclavicular (SC) approach. The aim of the study was to compare the ease of catheterisation of SCV using SC versus IC approach and also record the incidence of complications related to either approach, if any. Methods: In the study, 60 patients enrolled were randomly divided into two groups of 30 patients each. In Gp. SC right SCV catheterisation was performed using SC approach and in Gp. IC catheterisation was performed using IC approach. Access time, success rate of cannulation, number of attempts to cannulate vein, ease of guidewire and catheter insertion and length of catheter inserted and any associated complications were recorded. Results: The mean access time in group SC for SCV catheterisation was 4.30 ± 1.02 min compared to 6.07 ± 2.14 min in group IC. The overall success rate in catheterisation of the right SCV using SC approach (29 out of 30) was better as compared with group IC (27 out of 30) using IC approach. First attempt success in the SC group was 75.6% as compared with 59.25% in the IC group. All successful subclavian vein catheterisations in SC group and IC group were associated with smooth insertion of guidewire following subclavian venipuncture. Conclusion: The SC approach of SCV catheterisation is comparable to IC approach in terms of landmarks accessibility, success rate and rate of complications.

Abstract  Voluntary sexual intercourse in an adult woman with normal vagina is not a known cause of vesicovaginal fistula (VVF), though sexual abuse or sexual intercourse with young girls before reaching physical maturity can result in VVF. Postcoital VVF in an adult woman without these predisposing factors is very infrequent. A nulliparous woman who had VVF after first intercourse is presented, as it is unusual by virtue of etiology.
