- Abstract
Background: Congenital pulmonary malformations (CPM) are a group of rare abnormal lung development lesions that can have various presentations. The aim of this study was to define the differences in the clinical presentations of CPM in neonates, infants, and children, and to review the outcomes., Methods: A retrospective study was conducted at a tertiary care hospital in southern Thailand between 1992 and 2016., Results: Fifty-four patients were diagnosed with CPM, and the median age at onset was 1.7 months (IQR, 0.03-10 months). There were 33 cases (61.1%) of congenital pulmonary airway malformations, two (3.7%) of bronchogenic cyst, eight of (14.8%) congenital lobar emphysema, seven of (13.0%) pulmonary sequestrations, and four of (7.4%) congenital lung cysts. Twenty patients under 1 month old and 16 patients who were 1-12 months old had symptoms of respiratory distress. In contrast, 13 patients >1 year old had symptoms of pulmonary infection. There were significant differences in the numbers of patients who had cyanosis (P = 0.006), cough (P < 0.001), and fever (P < 0.001) between the three age groups. Thirty-eight patients (70%) required surgical treatment involving lobectomy (78.9%). Median follow-up duration was 28.1 months (IQR, 3.7-9.4 months). Nine of 10 patients had abnormal lung function tests, and 80.6% of patients had no subsequent limitations in physical activities., Conclusions: Respiratory distress is the important clinical feature in neonates and infants, whereas the signs of pulmonary infection usually occur in children >1 year old. 4. Etiology of Malocclusion and Dominant Orthodontic Problems in Mixed Dentition: A Cross-sectional Study in a Group of Thai Children Aged 8-9 Years.

- Abstract
Background: Etiology of malocclusion can be the cause of deviation in the skeleton, dental, and soft tissue development in children. Identifying etiology of malocclusion and dominant orthodontic problems as well as early detection could help in future effective treatment, management, and public health planning., Materials and Methods: A cross-sectional study was performed among 202 children. Consensus process was carried out between experienced orthodontic experts in etiology of malocclusion, dominant orthodontic problems, type of early treatment, and malocclusion severity. Fisher's exact test and descriptive statistics were used to explain the study results., Results: Etiology of malocclusion was detected in both congenital and acquired etiology (64.3%), followed by acquired etiology only (29.7%). The top three dominant orthodontic problems were caries (22.5%), early loss of primary tooth (15.6%), and tendency of crowding in permanent dentition (14.6%). Nearly all the children needed restoration (86.4%) and interceptive orthodontic treatment (69.3%), whereas severe malocclusion level was found in one-fourth of the children (26.0%). Statistical significance was found between type of early treatment and malocclusion severity ( P < 0.001)., Conclusion: Acquired etiology was very high. Caries and early loss of primary teeth were dominant orthodontic problems. An early treatment, especially preventive orthodontic treatment, was needed in almost all children. Malocclusion severity higher than moderate level was found in more than half of the children., Competing Interests: There are no conflicts of interest.

- Abstract
Tuberculosis of the thymus is very uncommon. Most previously reported cases have involved adolescents or adults. Herein, we report a case of tuberculosis of the thymus diagnosed in a 6-month-old infant, presenting with progressive dyspnea and a mediastinal mass. Both clinical and radiologic findings were nonspecific, while pathologic findings confirmed thymic tissue with massive caseous granulomas consistent with tuberculosis.

- Abstract
Background: Beta(2) agonist administered via a nebulizer is the standard treatment for acute asthma exacerbation. There are some limitations for the use of nebulization. We conducted a study to determine the efficacy of salbutamol administered via the pMDI with Volumatic spacer and the Easyhaler (DPI) compared to nebulization in mild to moderate asthma exacerbations in children., Methods: A multicenter, randomized, controlled study was conducted in children between 5 and 18 years of age who presented at an emergency or outpatient department. They were randomized to receive either 6 puffs of salbutamol via the pMDI with Volumatic spacer, or via the Easyhaler, or 0.15 mg/kg of salbutamol nebulized via oxygen (or compressed air). The primary outcome was the clinical response which was assessed using the modified Wood's asthma score. The secondary outcomes were: hospitalization, asthma revisit within 3 days, systemic corticosteroid use and adverse events. The clinical score, oxygen saturation, PR, RR, BP and adverse events were recorded at time 0 (before treatment) and 20, 40 and 60 minutes after drug administration., Results: There were no statistically significant differences in the clinical response between the three groups at the 1st, 2nd or 3rd dose or for the SpO(2) or the respiratory rate while the children in the Easyhaler group had significantly less tachycardia after the 2nd dose. No significant adverse events were noted among the three groups., Conclusions: Salbutamol administered via pMDI with Volumatic spacer or DPI (Easyhaler) are as effective as salbutamol given via a nebulizer in providing effective relief of mild to moderate severity acute asthma exacerbation in children between 5 and 18 years of age.

- Abstract
In 1999, a survey was carried out in 1,008 Thai children aged 7 years, which found that 85 (8.5%) children were habitual snorers, and 7 (0.69%) children had mild obstructive sleep apnea syndrome (OSAS). Since the natural history of snoring and untreated mild OSAS is still largely unknown, this study was undertaken in 2002 in the same group of children to determine the natural history of snoring and OSAS. Questionnaires, consisting of questions about snoring, were sent to the parents of the 1,008 children. Polysomnography was performed in 1) the 7 children who had OSAS in the previous survey, and 2) other habitual snorers who had sleep-related symptoms in this survey. Seventy-five percent of the questionnaires were returned. The prevalence of habitual snoring had decreased slightly, from 8.5% in 1999 to 6.9% in 2002. Sixty-five percent of the children who had snored habitually in the previous survey no longer did so, whereas 4.5% of the children who previously never snored or snored sometimes had become habitual snorers. Of the 7 children who had OSAS previously, 5 had persistent snoring, and polysomnographic studies revealed more severe OSAS, with an apnea-hypopnea index (AHI) of 1.5-9.2 per hour of sleep. Five children were newly diagnosed with OSAS in this survey, with an AHI of 1.5-7.5. The overall prevalence of OSAS in this survey was 10/755 (1.3%). In conclusion, 65% of children who snored habitually no longer did so when they got older, while 9% of children had developed OSAS. We suggest that regular follow-up in children with habitual snoring may be needed, and additional research is required to determine the indications for polysomnography and neurobehavioral and cardiovascular assessment. We also showed that children with mild OSAS could develop more severe disease if left untreated, suggesting that deferment of treatment may have negative consequences.

- Abstract
A case of initial esophageal atresia and tracheoesophageal fistula in a female newborn, later complicated by pneumonia and a second bronchoesophageal fistula, is reported. She was treated surgically by closure of the tracheoesophageal fistula and by end-to-end esophago-esophageal anastomosis. An esophagram at 1 month of age was normal. Three months later she developed severe, persistent right lower lobe pneumonia that required intensive antibiotic therapy and respiratory support. Esophagography was repeated and revealed a second fistula between the right main-stem bronchus and the lower esophagus. The bronchoesophageal fistula was repaired, and a right lower lobectomy was performed. Postoperative recovery was uncomplicated. Histologic examination indicated that the fistula was congenital in origin. To the best of our knowledge, this is the first reported case of a congenital bronchoesophageal fistula coexisting with a tracheoesophageal fistula and esophageal atresia., (Copyright 2002 Wiley-Liss, Inc.)

- Abstract
The clinical presentation of TSH-secreting pituitary adenoma in an 11-year-old boy is reported. Hyperthyroidism, signs and symptoms of increased intracranial pressure with the elevation of TSH, T3, T4 were the clues for suspecting this disorder. Bilateral carotid angiogram was performed which disclosed a suprasellar mass, and pituitary surgery was performed. Electron microscopic examination confirmed the diagnosis of TSH secreting pituitary tumor.
