BackgroundThe importance of strategies and services by caregivers and family members substantially impact the psychological and emotional wellbeing of autistic children. The rapid research developments in clinical and non-clinical methods benefit the features of autistic children. Among various internal and external factors, the influence of the built environment also impacts the characteristics of autistic children. This study investigates primarily the psychological effect of light and colors on the mood and behavior of autistic children to identify the most favorable and preferred indoor lights and color shades.MethodsA questionnaire survey was conducted at an autism center among autistic children and their parents. This study included autistic children aged between 6 and 16 (45 males, 42 females, mean age 8.7 years, standard deviation 2.3). Eighty-seven participants were involved in the survey to determine the sensory perceptions, intolerance, preferences, and sensitivities of children with an autism spectrum disorder toward colors and lighting. The margin of error at the statistical analysis’s 95% confidence level is ± 0.481.ResultsAs per this case report, the children have various color preferences and respond differently to different shades. Different hues have varying effects on autistic children, with many neutral tones and mellow shades proven to be autistic-friendly with their calming and soothing effect, while bright, bold, and intense colors are refreshing and stimulating. The stimulus of bright-lighting causes behavioral changes in autistic children prone to light sensitivity.ConclusionThe insights gained from this interaction with parents and caretakers of autistic children could be helpful for designers to incorporate specific autistic-friendly design elements that make productive interior spaces. A complete understanding of the effect of factors like color and lighting on the learning ability and engagement of autistic children in an indoor environment is essential for designers and clinicians. The main findings of this study could be helpful for a designer and clinicians to address designing an autism-friendly built environment with a color palette and lighting scheme conducive to their wellbeing and to maximize their cognitive functioning.

Objective\ud \ud We aim to validate four-dimensional flow cardiovascular magnetic resonance (4D flow CMR) peak velocity tracking methods for measuring the peak velocity of mitral inflow against Doppler echocardiography.\ud \ud \ud \ud Method\ud \ud Fifty patients were recruited who had 4D flow CMR and Doppler Echocardiography. After transvalvular flow segmentation using established valve tracking methods, peak velocity was automatically derived using three-dimensional streamlines of transvalvular flow. In addition, a static-planar method was used at the tip of mitral valve to mimic Doppler technique.\ud \ud \ud \ud Results\ud \ud Peak E-wave mitral inflow velocity was comparable between TTE and the novel 4D flow automated dynamic method (0.9 ± 0.5 vs 0.94 ± 0.6 m/s; p = 0.29) however there was a statistically significant difference when compared with the static planar method (0.85 ± 0.5 m/s; p = 0.01). Median A-wave peak velocity was also comparable across TTE and the automated dynamic streamline (0.77 ± 0.4 vs 0.76 ± 0.4 m/s; p = 0.77). A significant difference was seen with the static planar method (0.68 ± 0.5 m/s; p = 0.04). E/A ratio was comparable between TTE and both the automated dynamic and static planar method (1.1 ± 0.7 vs 1.15 ± 0.5 m/s; p = 0.74 and 1.15 ± 0.5 m/s; p = 0.5 respectively). Both novel 4D flow methods showed good correlation with TTE for E-wave (dynamic method; r = 0.70; P < 0.001 and static-planar method; r = 0.67; P < 0.001) and A-wave velocity measurements (dynamic method; r = 0.83; P < 0.001 and static method; r = 0.71; P < 0.001). The automated dynamic method demonstrated excellent intra/inter-observer reproducibility for all parameters.\ud \ud \ud \ud Conclusion\ud \ud Automated dynamic peak velocity tracing method using 4D flow CMR is comparable to Doppler echocardiography for mitral inflow assessment and has excellent reproducibility for clinical use.

Sepsis is extremely common amongst critically ill patients and requires early diagnosis. Hence, identifying a biomarker that could acknowledge sepsis at its prior stage is of vital significance. Immature platelets are a percentage of circulating platelets that contain RNA and is a newer parameter that is measured using automated hematology analyzers in diagnosing sepsis. This review article discusses 10 articles that reveal the role of immature platelet fraction in predicting the onset of sepsis and its relationship with mortality in sepsis. Literature search was done using PubMed, Scopus and Google Scholar and words like platelet indices and immature platelet fraction were typed in the search bar. The aim of this review article is to present a precise form of data that talk about immature platelet fraction (IPF) and its association with the severity and mortality of sepsis. Five out of 10 articles suggest that immature platelet fraction can predict the onset of sepsis and eight out of 10 articles suggest that increased IPF values are associated with high mortality.

During pregnancy the maternal heart undergoes significant physiological change. Technological advancements in echocardiography (echo) have improved our ability to assess the right heart in detail and yet there is little research into adaptation during pregnancy. In this pilot study, we considered how three measures of right heart function changed in normal women from the 1st trimester through to the post-partum period. We observed a significant increase in tricuspid annulus planar systolic excursion (TAPSE) and right ventricular systolic velocity (RV S’) from the 1st to the 2nd trimester; both measures of longitudinal right ventricular function. There was no significant difference in right ventricular systolic area change, which is more difficult to accurately measure, and reflects radial function. TAPSE and RV S’ are easy to accurately measure with modern echo technology. This pilot study suggests that any reduction in these values from the 1st to the 2nd trimester should prompt more detailed assessment, and correlation with clinical symptoms. It would be valuable to undertake a comprehensive study to define normal ranges for common measures of right ventricular function, using echo, during pregnancy. The ability to assess the right heart, with an understanding of expected normal values, has the potential to significantly improve our understanding and management of maternal cardiovascular disease.

We report a systematic study on the magneto-structural transition in Mn-rich Fe-doped Mn-Fe-Ni-Sn(Sb/In) Heusler alloys by keeping the total valence electron concentration (e/a ratio) fixed. The martensitic transition (MT) temperature is found to shift by following a proportional relationship with the e/a ratio of the magnetic elements alone. The magnetic entropy change across MT for a selected sample (Mn49FeNi40Sn9In) has been estimated from three different measurement methods (isofield magnetization (M) vs temperature (T), isothermal M vs field (H) and heat capacity (HC) vs T). We observed that though the peak value of magnetic entropy change changes with the measurement methods, the broadened shape of the magnetic entropy change vs T curves and the corresponding cooling power (~140 Jkg-1) remains invariant. The equivalent adiabatic temperature change ~ -2.6 K has been obtained from indirect measurements of temperature change. Moreover, an exchange bias field ~ 783 Oe at 5 K and a magnetoresistance of -30% are also obtained in one of these materials., 21 pages, 8 figures, 3 tables

A 74-year-old male presented to the emergency department (ED) with complaints of high-grade fever, productive cough, and difficulty breathing for three weeks, and a significant weight loss in the past six months. Despite a detailed investigation, diagnosis could not be achieved. Multiple empiric regimens were tried but no response was observed. Bone marrow biopsy was conducted after suspicious peripheral film result, which concluded that the patient was having myelodysplastic syndrome. Although pyrexia of unknown origin has a diverse group of diseases to evaluate for, and rare entities are difficult to diagnose, meticulous evaluation may give significant evidence for targeted diagnostic workup.

The anomalous thermal expansion in a layered $3d\text{\ensuremath{-}}5d$ based triple perovskite iridate ${\mathrm{Ba}}_{3}{\mathrm{CoIr}}_{2}{\mathrm{O}}_{9}$ is investigated using high resolution synchrotron diffraction. Below the magnetostructural transition at 107 K, the onset of antiferromagnetic order is associated with a monoclinic distortion of the hexagonal structure. Deeper within the magnetically ordered state, a part of the monoclinic phase distorts even further, and both these structural phases co-exist down to the lowest measured temperatures. We observe negative thermal expansion in this phase co-existence regime, which appears to be intimately connected to the temperature driven relative fractions of these monoclinic phases. The significant NTE observed in this system could be driven by magnetic exchange striction and is of relevance to a number of systems with pronounced spin orbit interactions.

Fungal infections have been drastically increasing in incidence in recent years, preferentially affecting immunocompromised hosts and causing potentially fatal outcomes. One of the emerging fatal fungal pathogens is Trichosporon asahii, a non-Candida yeast that has been increasingly reported in recent years. Previous literature has described T. asahii as primarily affecting immunocompromised hosts, specifically those who are neutropenic, and causing fatal disseminated infections. Herein, we describe a case of an isolated subcutaneous abscess with T. asahii in an immunocompetent host without overlying skin manifestations or predisposing factors that resulted in complete mycotic cure when treated with voriconazole and terbinafine.

Purpose The COVID-19 surge required the deployment of large numbers of non-intensive care providers to assist in the management of the critically ill. Institutions took a variety of approaches to “uptraining” such providers though studies describing methods and effectiveness are lacking. Materials and methods One hundred and seventy-five providers underwent a 3 h simulation-based session focused on management of shock, mechanical ventilation, acute respiratory distress syndrome, and critical care ultrasound. All participants were sent surveys to assess their comfort with various aspects of critical care following return to their usual work environments. Results One hundred and eight providers of 175 (62%) completed the survey. Overall, 104/108 responders (96%) felt training either significantly or somewhat improved their knowledge in the management of ICU patients. Responders felt most comfortable in the management of hypoxemia in intubated patients and the management of ventilated patients with acute respiratory distress syndrome (93% strongly agree or agree, and 86% strongly agree or agree, respectively). Fewer responders felt more comfortable using focused echocardiography (70% strongly agree or agree) and lung ultrasonography in following progression of COVID-19 (76% strongly agree or agree). Conclusions Simulation-based training improved provider comfort in the management of critically ill patients with COVID-19., Highlights • Non-intensivist providers underwent simulation-based training to work in the COVID-19 ICU. • Simulation-based training improved comfort in management of critically ill patients. • Comfort in management of intubated patients was greatly improved.

We report a comprehensive investigation of the triple perovskite iridate ${\mathrm{Ba}}_{3}\mathrm{Co}{\mathrm{Ir}}_{2}{\mathrm{O}}_{9}$. Stabilizing in the hexagonal $P{6}_{3}/mmc$ symmetry at room temperature, this system transforms to a monoclinic $C2/c$ symmetry at the magnetic phase transition. On further reduction in temperature, the system partially distorts to an even lower symmetry ($P2/c$), with both these structurally disparate phases coexisting down to the lowest measured temperatures. The magnetic structure as determined from neutron diffraction data indicates a weakly canted antiferromagnetic structure, which is also supported by first-principles calculations. Theory indicates that the ${\mathrm{Ir}}^{5+}$ carries a finite magnetic moment, which is also consistent with the neutron data. This suggests that the putative $J=0$ state is avoided. Measurements of heat capacity, electrical resistance noise, and dielectric susceptibility all point toward the stabilization of a highly correlated ground state in the ${\mathrm{Ba}}_{3}\mathrm{Co}{\mathrm{Ir}}_{2}{\mathrm{O}}_{9}$ system.

We observe a complex interplay between lattice strain and structural phase co-existence, which is typically not observed in this family of compounds. The low temperature magnetic ground state is characterized by the absence of long-range magnetic order, and points towards the condensation of a cluster glass state from an extended regime of short range magnetic correlations.

We report temperature dependent magnetization and neutron diffraction measurements on the corundum related magnetoelectric antiferromagnet ${\mathrm{Mn}}_{4}{\mathrm{Ta}}_{2}{\mathrm{O}}_{9}$. Measurements performed on a single crystalline specimen reveal that the magnetization is anisotropic, and a weak ferromagnetic component emerges well within the antiferromagnetically ordered state. Powder neutron diffraction measurements indicate that the magnetic structure comprises of antiferromagnetically coupled ferromagnetic chains of ${\mathrm{Mn}}^{2+}$ spins aligned along the trigonal $c$ axis, in contrast to that reported in other isostructural members of this family. Magnetic measurements performed under a periodic electric field indicate that the magnetoelectric response is also anisotropic, with this coupling along the trigonal $c$ axis and that perpendicular to it having different signs.

We report a detailed investigation of the ${\mathrm{Ni}}_{2}\mathrm{Mn}\mathrm{Ga}$ shape memory alloy through magnetic, electronic, and thermal measurements. Our measurements of the anomalous Nernst effect (ANE) reveal that this technique is very sensitive to the onset of the premartensitic transition in sharp contrast to other transport measurements. With the ANE being sensitive to changes at the Fermi surface, we infer on the link between the structural modulations and the modulation of the Fermi surface via its nesting features, with the magnetic field induced strain being the mediating mechanism.

Cardiovascular Magnetic Resonance has been a powerful tool in the non-invasive assessment of inflammation in several conditions. More recently, Ultrasmall superparamagnetic particles of iron oxide have been successfully used to evaluate macrophage activity and subsequently inflammation on a cellular level. Current evidence from research studies provides encouraging data and confirms that this evolving method can potentially have a huge impact on clinical practice as it can be used in the diagnosis and management of very common conditions such as coronary artery disease, ischaemic and non-ischaemic cardiomyopathy, myocarditis and atherosclerosis. Another important emerging concept is that of myocardial energetics. With the use of phosphorus magnetic resonance spectroscopy, myocardial energetic compromise has been proved to be an important feature in the pathophysiological process of several conditions including diabetic cardiomyopathy, inherited cardiomyopathies, valvular heart disease and cardiac transplant rejection. This unique tool is therefore being utilized to assess metabolic alterations in a wide range of cardiovascular diseases. This review systematically examines these state-of-the-art methods in detail and provides an insight into the mechanisms of action and the clinical implications of their use.
