Introduction: WHO has declared COVID-19 as a global pandemic on 11th March 2020. The Corona Virus Disease 2019 (COVID19) has rapidly spread worldwide. First case of COVID-19 was reported on 30th January 2020 in Kerala state, India. The first COVID-19 case was reported to Government Allopathic medical College Banda on 1st April 2020, and thereafter many COVID-19 cases were admitted and treated. This study was conducted to find the epidemiological variables affection mortality among hospitalised COVID-19 patients. Materials and Methods: Study design: This study was a hospital based cross-sectional study. The outbreak of second wave started and the period was 1st April 2021 to 30th June 2021. Socio-demographic data, clinical signs and symptoms present, history of co-morbidities and vaccination history etc. were taken at the time of admission. Adjusted odds ratio with a 95% confidence interval was estimated for all the potential covariates associated with death. The p-value less than 0.05 considered statistically significant. Results: Maximum number of cases was found in the age group of 41-60 years (463) followed by 21-40 year age group (379). Out of 1226 cases, 795 (64.8%) cases were males respectively. The covariates marital status, sore throat, breathlessness and vaccination status were found to be significantly associated with mortality. Conclusion: Breathlessness, cough, sore throat and fever were the important clinical findings of this study. Also comorbidity was one of the significant risk factors. The COVID death can be minimized by vaccination;therefore there is a need of awareness and health education about COVID vaccine in community. Background: The COVID-19 pandemic is a global emergency. AEFIs reporting and assessment should be done with high priority as they could change the benefit-risk profile of the vaccine. Objective: To assess adverse effects after COVID-19 vaccination. Methods: This cross-sectional study conducted among 605 heath functionaries taken part from January 2021 to March 2021. Results: The most common AEFI reported was pain at injection site (51.9%). Gender, previous COVID status and perception before vaccination were significantly associated with AEFI. Conclusion: There was no serious adverse event after vaccination. COVID vaccine has a good safety profile. © 2021, Indian Association of Preventive and Social Medicine. All rights reserved.

Background: As per the Child Mortality Report 2018 by UNICEF the current neonatal mortality rate in India stands at 24 per 1000 live births with it being more than the national average for Uttar Pradesh. The neonatal morbidity and outcome pattern in terms of discharge or otherwise shows considerable national, state, district and sub- district variations due to a multitude of diverse factors contributing to it at various levels. Aim & Objective: The aim of the study was to explore the mortality rate and outcome in NICU setting, and factors influencing the outcomes. Methods and Material: This government NICU based follow-up study comprised of data analysed from 450 neonates through a semi-structured questionnaire using face to face interview technique. Data was evaluated in SPSS and; independent t test and chi square test were applied. Statistical analysis used: Data was evaluated in SPSS and; independent t test and chi square test were applied. Results: For morbidity low birth weight (LBW) (14.4%) and for mortality LBW/prematurity (56%) were the most common causes. The discharge rate was 64.4%. Good outcome in the form of successful discharge was statistically significant with birth weight (p=0.000), gestational age (0.001), length of stay at NICU (p=0.003) male sex (p=0.003) and feeding of newborn (p=0.002) Conclusions: The study enunciates a high discharge rate in tertiary care government NICU with a mixed morbidity and mortality profile. The causes are mainly preventable and hence can be largely mitigated through dedicated ante natal, intra natal and post-natal care.

Background: With increasing life expectancy people are living longer but they are not necessarily healthier than before. Nearly a quarter (23%) of the overall global burden of death and illness is in people aged over 60, and much of this burden is attributable to long-term illness like non-communicable diseases and mental health problems. Aims & Objectives: Aim of the study the magnitude of depression among elderly population of Agra. Objectives To estimate the prevalence of depression in elderly population of Agra. 2. To study the factors associated with depression among elderly population of Agra. Material and Methods: This study was conducted among 355 elderly in urban Agra. Data was collected by house to house visit using pre-designed and pre-tested schedule. Geriatric depression scale (GDS-30) was used to assess the burden of depression in study population. Results: The mean age of the study participants was 68.05 years. Depression was found in 50.1% among study population and 15.5% had severe depression. Depression was more among the Illiterates, Muslims, financial dependents, those who were suffering from chronic disease/s, encountered a stressor in the last 6 months, less religious, not living with partner or suffered elderly abuse. Conclusion: High burden of depression was found among the victims of elderly abuse. Stressful life events and absence of spouse were found to be significant risk factors for depression. To deal with these factors we suggest a link worker Geriatric Age Social Health Agent (G-ASHA) to act as a link between family, community and authorities.

Background: TB is one of the top 10 causes of death worldwide. The risk of TB is high among populations living in poverty, low socioeconomic groups, low income, immune-suppressed (including AIDS), and extreme age (old age and children) groups, etc. Methods: The study was conducted in TB and chest department of Sarojini Naidu medical college Agra city. The total number of patients registered during the period of July to September 2017 was 518; out of these 288 were new pulmonary and extrapulmonary patients. Out of 288 subjects, 123 were pulmonary patients. All the 123 new pulmonary tuberculosis patients (both smear +ve and smear -ve) were included in the study. 18 patients were loss to follow up after registration thus final effective sample size was 105 for further follow-up study. Results: Overwhelming (84.76%) patients belonged to a lower socioeconomic class. The maximum number (85.33%) of the study subjects among upper lower socioeconomic status had no knowledge regarding causative microbes of the disease (p=0.001). Conclusions: The majority of patients belong to illiterate, lower socioeconomic group, married and other than general cast, living in a joint, overcrowded family with more than four family members. About half of the subjects not knowing the method of prevention & consequence of treatment interruption. Most of them felt that quality of life affected after disease and about 1/4th of them felt hurtful behaviour of family members.

Background: Health services are designed to meet the health needs of the community through the use of available knowledge and resources. Secondary Level of Health Care essentially including Community health centers (CHCs) constituting the First Referral Units (FRUs) and the Sub-district and district hospital, designed to provide referral health services. First Referral Units provide optimal specialized care to the community to achieve and maintain acceptable quality care. Objective: To assess the available resources and minimum services at FRUs as per IPHS Standards in District Agra. Material & Methods: A Cross- sectional study was conducted in Four FRUs of Agra District, from June 2015 – August 2016. All four FRUs were evaluated for infrastructure, drugs and consumables, equipment, manpower, as per IPHS standards and minimum services provided by first referral units (FRUs). Status of FRUs as against revised (2012) IPHS Standard. Results: All the four FRUs (100%) have their own government building. Availability of drugs and consumable, equipment, were good which was 100% while 25% of FRUs had blood storage facility.50% of FRUs had general surgeon, dental surgeon, gynecologist and anesthetist while 100% of FRUs had medical superintendent, medical officer and availability of staff nurse and pharmacist were 75% and 91.7% respectively among all four FRUs. None of FRUs fulfills the minimum services available as per IPHS norms i.e. emergency obstetric care, blood bank/ blood storage facility, except 24-hourly delivery services which was available at all four FRUs (100%) Conclusion: Physical infra-structure, drugs and consumables, equipment, availability found adequate in all FRUs.Availability of specialist doctor markedly lacking in all FRUs however Paramedical staffs position was better, none of the FRUs meets the minimum criteria for service available at FRUs as set by IPHS standard.

Background: Corona viruses signify a most important group of viruses mostly affecting human beings. It is a respiratory infection with common signs and symptoms of fever, cough, sore throat, headache, and loss of taste, loss of smell, respiratory symptoms. In India till 31st December 2020, the total number of confirmed cases were 1,02,86,310; with active number of cases were 2,52,699 number of cases recovered were 98,81,732 while number of deaths were 1,49,018. Objective of the study was to find the quadratic and cubic model of COVID-19 positive cases at tertiary care hospital of Bundelkhand region of Uttar Pradesh.Methods: A hospital based study was carried out with confirmed covid-19 cases admitted to Government Medical College Banda, UP. 1486 cases have been taken from the period of 1st April 2020 up to 31st December 2020.Results: In this study maximum cases (30.14%) belongs to the age group of 30-45 years. Background: Chronic non-communicable diseases (NCD) are increasing among the adult population in both developed and developing countries, resulting diabetes, cardiovascular diseases with high morbidity and mortality. Due to increase in life expectancy and change in life style and behavior of people and clustering of metabolic abnormalities in individuals leads to development of metabolic syndrome. To estimate the prevalence of metabolic syndrome and their associated risks factors belonging the age group of 18 years and above of urban Agra population. Methods: Cross sectional study was carried out among 18 years and above age group of urban population of Agra, Uttar Pradesh, India. A multi stage simple random sampling technique is used to reach the subjects. Results: Prevalence of metabolic syndrome was found 37.1% in study subjects, more common in females (65.9%). The metabolic syndrome was also found higher in the educated group between class 8 to class 12 and among socio economic status III. Higher risk showed by serum triglyceride component of metabolic syndrome among study group. Conclusions: The prevalence of metabolic syndrome in female study population is higher as compared to the male study population in every selected bio social characteristics.

Background: Fertility is only way for biological replacement of human being in order to continue its existence on earth. The fertility behavior of any population is influenced by various factors. Objective of the study is to study the fertility perception and behavior among urban women in Agra and understand the social factors on perception of women regarding fertility behavior among study group women. Methods: Community based, Cross-sectional study was done among 280 married women of reproductive age group residing in Agra City, using a standardized questionnaire to assess their fertility perception and behavior. Results: Mean age of marriage was 19.01 years. About 70% of women married before the age of 20. Mean gap between marriage and first child was 1.57 years. The mean least birth interval was 1.61 years. Less than one third of women desired for more than two children. Level of education, transition from lower to high SE status, significantly shifts the preferred family size. Conclusion: Majority of women considered appropriate age of marriage 18 year and above, gap between marriage and first child two years and above, wishes for two child norm. However, two third had confessed the necessity of mail child. Level of education and working status of women had positively effect.

Janani Suraksha Yojana (JSY) was launched in April 2005, to promote institutional deliveries through provision of cash assistance, transport, escort, and referral services. An observational cross-sectional study was conducted to assess the antenatal registrations, postnatal checkups, and institutional deliveries, and to compare the various social groups. Married women of the reproductive age group, having at least two children, were interviewed regarding antenatal care, delivery, and postnatal care in both pregnancies, latest as well as previous. Post JSY implementation, antenatal registrations increased from 61.79 to 96.34%, Deliveries at the Government Health Facility increased from 25.20 to 53.25% and postnatal check-ups increased from 45.93 to 69.51%. In the post-JSY-implementation phase, the Government Health Facility was preferred more by Scheduled Castes (SC), Scheduled Tribes (ST), Other Backward Classes (OBC) (SC/ST = 56.87%, OBC = 60.2%, and general = 43.68%), educated (Illiterate = 17.39%, Primary = 88.14, and Middle or above = 81.94%) and the lower socioeconomic classes (Lower SEC 71.83% and Upper lower and above = 45.71%) for their deliveries. It appears that the socially backward groups have benefited more from JSY.

Background: Pregnancy and child birth are normal event in the life of women. Most pregnancy results in normal birth but any pregnancy can develop complication at any stage so timely provision of obstetric services is extremely important and to the care by a skill birth attendant. So knowledge, skills and practices of these are very important. In light of above the study was carried out with objective to assess the knowledge and practices of medical officer and paramedical staffs (staff nurses and ANM) regarding antenatal services, intra-natal services, postnatal services.Methods: A cross sectional study was conducted at first referral units (FRUs) of Agra district of Uttar Pradesh, India from June 2015–August 2016. Structured open ended interview schedule were use d to collect the desired information. Before commencing the interview, informed written consent were taken from respondents. All medical officer and all staff nurse, public health nurse and auxiliary nurse midwives (ANM) were included as study subjects. Data were collected by first author himself. A total of 25 medical officers and 30 paramedical staff were interview for the study.Results: Almost all the medical staffs (100%) have knowledge regarding antenatal care except minimum ANC visit (80%) as practice of ANC services concerned less than half of them doing it as routine, nearly 1/3rd of medical staff (32.0%) conducted or assists delivery (i.e. only LSCS) in routine practices.Conclusions: Knowledge about maternal health services was more among medical staffs than paramedical staffs. Paramedical staffs have less knowledge as well as practice about neonatal resuscitation and infection control measure. Only less than 1/3rd of medical staffs conducting or assisting deliveries.

Background: Janani Suraksha Yojana (JSY) is a maternal protection scheme that promotes institutional delivery by providing cash incentive to the mothers who deliver their babies in a health facility. With the purpose of improving maternal and neo-natal mortality and morbidity indicators, the investment and emphasis on JSY is continued. Utilization pattern and perception in the community regarding a particular health program is important to study for assessing the success or failure of the program. Objectives: To assess the utilization of JSY and its perception among mothers and health care providers. Materials and Methods: A descriptive, cross-sectional, community based study was Conducted at Agra district of Uttar Pradesh, among 246 married women of reproductive age group who had childbirth after JSY implementation for assessing their utilization pattern and perception regarding JSY services. Perception regarding the strengths and weaknesses of JSY among the health care providers was also studied. Results: Nearly half (53.25%) of the mothers studied had an institutional delivery and were eligible for the JSY benefits. Postnatal home visits by Accredited Social Health Activists were done in 48% of home and 100% of institutional deliveries. Nearly half (48.09%) of the beneficiaries were benefited by free transport facility under JSY. Although all of the health care providers perceived JSY as beneficial for improving maternal health, 44% of them had the notion that cash incentives under JSY can have a negative effect on family planning practices. Conclusion: The utilization rate of the JSY services was found to be low in certain aspects like institutional deliveries, transport facility etc., and there is scope for improvement.
