17. Improvement of Nutritional Status of Elderly Patients with Severe Obstruction Esophageal Carcinoma by Image-guided Photodynamic Therapy

Abstract

Background Esophageal cancer is one of the most aggressive gastrointestinal tumors. Advanced esophageal carcinoma is mainly associated with dysphagia. Most elderly patients with severe obstruction esophageal carcinomacannot tolerate anesthesia and invasive treatment due to comorbidities, while the failure to improve dysphagia in the short term will seriously affect the nutritional status, life quality and prognosis of patients. Objective To explore the safety and efficacy of image-guided photodynamic therapy (IGPDT) under local anesthesia for short-term improvement of obstruction and nutritional status in elderly patients with severe obstruction esophageal carcinoma. Methods A total of 24 elderly patients with severe obstruction esophageal carcinoma admitted to Hebei General Hospital from March 2020 to December 2021 were selected for IGPDT in the prospective, single-arm, self-control study. The upper boundary of the lesion was located by endoscopy and marked with metal tissue clips, the lower boundary of the lesion was located by CT and esophagography before treatment. During the treatment, the fiber of laser treatment was delivered to the lesion site under the guidance of X-ray fluoroscopy during treatment. The Stooler dysphagia score was evaluated before, 1 week and 1 month after operation. The nutritional status of patients was evaluated by nutritional risk screening 2002 (NRS 2002) score, hemoglobin, BMI, albumin and prealbumin before and 2 months after operation. The swallowing quality of life scale (SWAL-QOL) was used to evaluate the quality of life in patients. Results All patients achieved partial response (PR) at 1 month postoperative efficacy evaluation. Abstract

In the process of development and transformation of vocational education, it is difficult for traditional methods to carry out a digitalized and accurate description of it. Aiming at the construction of the discipline of vocational education, this paper innovates the teaching method by constructing a knowledge graph and uses TF-IDF as the keyword extraction algorithm for entity extraction. To ensure that entity words are recognized accurately, LSTM neural units are utilized to eliminate problems related to gradient vanishing and explosion. On this basis, knowledge graphs are used to integrate heterogeneous data from multiple sources to apply knowledge graphs to the recommender system to create a personalized teaching model to provide schools with a digital transformation method for vocational education. Two different vocational schools tested the model design after it was completed. The analysis of the data revealed that knowledge points experienced an average decrease in error rate of 10-20%. The error rate of knowledge points 10, 11, and 12 did not change much, but the rate of student guessing decreased to 0. The percentage of users who improved their grades in the experimental class using the system by more than 0% was 5.90% higher than that of the normal class, the percentage of those who improved by more than 5% was 5.28% higher, and the percentage of those who had great improvement was 1.90%.

Abstract

It is particularly appealing for efficient active terahertz (THz) modulators using photonic structures to enhance light–matter interaction. Here, an optical controlled THz modulator is proposed that combines lead sulfide (PbS) quantum dots with subwavelength metallic split-ring resonators (SRRs) for providing field enhancement. The modulation depth reaches 60.3%, which is approximately 3 times larger than the PbS quantum dots film without SRRs (as reference) in the frequency range of 0.1–1.1 THz. Such significant enhanced THz modulation is mainly due to the local THz field enhancement caused by the SRRs, which is consistent with the simulation result.
