10. Early identification of postoperative remission for thyrotropin‐secreting adenomas.

Abstract

Objective: Thyrotropin‐secreting adenoma (TSHoma) is a rare type of pituitary adenoma, occurring in one per million people. Little is known about TSHoma. We summarized the demographic, clinical and hormonal characteristics of TSHoma based on a single‐centre experience. Moreover, we explored the predictive value of postoperative thyroid function for long‐term remission. Design, Patients and Measurements: We retrospectively analysed 63 patients who were diagnosed as TSHoma and surgically treated at our hospital from January 2015 to June 2021. The preoperative clinical characteristics were analysed and compared between remission and nonremission groups. Thyroid function was measured at 1 day, 1 month, 3 months, 6 months, 12 months and over 12 months after surgery to determine whether they could predict long‐term remission. Results: The male to female ratio for TSHoma was 1.25. The mean age at diagnosis was 45 ± 12 years. Clinical presentation was varied, presenting with hyperthyroidism (68.25%), space‐occupying effect (15.87%), amenorrhea (7.14% of female patients) and nonsymptoms (22.22%). 88.14% of patients achieved postoperative endocrinological remission. Larger tumour size and tumour invasion into cavernous sinus and suprasellar with chiasmal compression were strong predictors of lower rates of endocrinological remission. Postoperative thyroid function at 3 months was a viable diagnostic predictor for postoperative remission, especially for FT4 level with a 20.65 pmol/L cutoff. Conclusions: Tumour size and extent are major prognostic factors for remission. Abstract

Using No.26 paraffin oil as the base oil raw material, the base oil was emulsified by a compound emulsifier to pre pare a stable and non-layered paraffin oil emulsion. The effects of four different emulsifier compounding ratios, including dehydrated sorbitanoleate (Span80), polyoxyethylenesorbitan monooleate (Twen-80), isotridecyl alcohol polyoxyethylene ether (TO-5) and fatty al cohol polyoxyethylene ether (AEO-3),as well as the amount of emulsifier, emulsifier promoter, paraffin oil, emulsifying temperature, emulsifying time and emulsifying speed on the stability of release agent were investigated. The results show that when the hydrophilic-hydrophobic balance (HLB) of the composite emulsifier is about 10.5, it is equivalent to the HLB value of paraffin oil. When the amount of emulsifier is 25% of the mass of paraffin oil, the amount of emulsifier promoter SG-6 is 8% of the mass of composite emulsifier, the amount of paraffin oil is 20% of the mass of release agent, the paraffin oil content is 20%,the emulsification temperature is 80 t, the emulsification time is 25min,and the emulsification speed is 2000r/min, the storage stability of the water-based mold release agent at this time is the best, the film release effect is better, and it can meet the use requirements of the engineering project. Abstract

Aims To determine the relationships of sudomotor functions, nerve conductions and self-reported depressive symptoms in Chinese type 2 diabetes (T2DM) patients. Methods T2DM patients in a single community health center were included in this study. Demographic, medical and laboratory data were collected. Michigan Neuropathy Screening Instrument (MNSI) examination was conducted in all patients. SUDOSCAN test and Point-of-care Nerve Conduction Device (DPN-check) were conducted and all the patients finished the Patient Health Questionnaire-9 (PHQ-9). Results A total of 162 T2DM patients (74 males and 88 females) were included. The mean age was 69.0±7.2 years and the mean course of diabetes was 10.5±8.0 years with a mean HbA1c level of 7.3±1.4%. Thirty of them (18.5%) had self-reported depressive symptoms (PHQ-9 scores≥5). Diabetic peripheral neuropathy (DPN) was diagnosed according to the MSNI examination in 74 patients. Electrochemical skin conductance (ESC) values of both hands and feet were significantly lower in patients with depressive symptoms (Hands ESC: 60.63±18.92 vs. 67.64±16.02 μS, p<0.05; Feet ESC: 59.60 ± 15.19 vs. 66.19±14.99 μS, p<0.05). The proportion of patients with moderate to severe depressive symptoms were significantly higher in those with ESC values<60 μS (13.33% vs. 3.846%, p<0.05). Hands ESC values were negatively related to PHQ-9 scores (r =− 0.168, p<0.05). After adjusting for variables, hands and feet ESC values remained negatively related with depressive symptoms (β =− 0.036 and−0.038, p<0.05). Female were positively related to depressive symptoms with odds ratio 3.4 (95%CI 1.1–10.5, p<0.05). Abstract

Background: Autonomic dysfunction (AD) accompanying with chronic liver disorders led to an increased risk of mortality. However, researches that investigated the association between non-alcoholic fatty liver disease (NAFLD) and AD were insufficient. Aims: To study the association of NAFLD with AD in middle-aged and elderly Chinese adults. Design: Four thousand nine hundred seventy-four adults aged 40 years or older were enrolled in this cross-sectional study. NAFLD was diagnosed by hepatic B-mode ultrasonography. Autonomic function was assessed using a simple EZSCAN test by measuring sudomotor function, with an AD index > 50% defined as a manifestation of AD. Methods: Pearson correlation, multiple stepwise linear regression, univariate and multivariate logistic regression was employed to examine the relationship between NAFLD and AD, controlling for potential confounders. Results: The prevalence of AD was significantly higher in participants with NAFLD than those without (40.75 vs. 26.86%, P < 0.0001). Age, body mass index, status of diabetes, sex, diastolic blood pressure and prevalent NAFLD, were positively correlated with AD index in multiple stepwise linear regression analysis (all P < 0.05), whereas total cholesterol was negatively related to it ( P = 0.0043). Compared with the participants without NAFLD, those with NAFLD had an increased odds of the prevalent AD (odds ratio 1.38; 95% confidence interval 1.15–1.64; P = 0.0004) after controlling for multiple confounders. Conclusions: The presence of NAFLD was significantly associated with AD, as indicated by abnormal sudomotor function. The association was independent from various conventional risk factors.
