Abstract

: Introduction: Evidence from low‐resource settings indicates that economic insecurity is a major barrier to HIV treatment adherence. Economic empowerment (EE) interventions have the potential to improve adherence outcomes among adolescents living with HIV (ALWHIV) by mitigating the effects of poverty. This study aims to assess the efficacy and cost‐effectiveness of a savings‐led family‐based EE intervention, Suubi + Adherence, aimed at improving antiretroviral therapy (ART) adherence outcomes ALWHIV in Uganda. Methods: Adolescents (mean age 12 years at enrolment; 56% female) receiving ART for HIV at 39 health centres were randomized to Suubi + Adherence intervention (n = 358) or bolstered standard of care (BSOC; n = 344). A difference‐in‐differences analysis was employed to assess the change in the proportion of virally suppressed adolescents (HIV RNA viral load Results: At 24 months, the intervention was associated with an 8.85‐percentage point [95% confidence interval (CI) 0.80 to 16.90 percentage points] increase in the proportion of virally suppressed adolescents between the study arms (p = 0.032). Per‐participant costs were US$177 and US$263 for the BSOC and intervention groups respectively. The incremental cost of virally suppressing one additional adolescent was estimated at US$970 [95% CI, US$508 to 10,725] over two years. Conclusions: Our results support the integration of family‐based EE interventions into adherence‐support strategies as part of routine HIV care in low‐resource settings to address the underlying economic drivers of poor ART adherence among ALWHIV. Moreover, per‐participant costs to achieve viral suppression do not seem prohibitive compared to other community‐based adherence interventions targeted at ALWHIV in low‐resource settings. Abstract

Sliding bearings are the main kinematic pair of a Stirling engine, and one of the main sources of friction power loss and excitation force. There is a coupling effect between the lubrication state and the excitation force. In this paper, the dynamic model of a Sapphire bearing fatigue test bed, which includes thermo-elasto-hydrodynamic (EHD) property, is established and the accuracy of the model is verified by the experimental data. Based on the model, the effects of the load, the oil supply temperature, the oil supply pressure, the bearing clearance on the tribological and dynamic characteristics are studied. With an increase in the load, the orbital path of the journal moves downward, the lubrication state changes from the full hydrodynamic lubrication to the boundary lubrication, the friction power loss increases, and the bearing excitation frequency increases. Due to the decrease of the oil viscosity, the hydraulic friction power loss decreases, but the friction power loss derived from the asperity contact increase, so does the total friction power loss. With the increase of the radial clearance, the shape of the orbital path and the minimum oil film thickness remain unchanged. The increase of the bearing clearance results in the collisions between the journal and bearing bush, thus the amplitude of the frequency multiplication component in the bearing load increases, and the asperity and the total friction power loss increase.

Abstract

With the increase of extreme natural disasters and the frequent occurrence of man-made attacks, resilience studies of power grids have attracted much attention, among which resilience assessment reflects the resistance and resilience of power systems to cope with extreme disasters. To improve the resilience of distribution grids under extreme weather conditions, this paper proposes a resilience assessment framework for distribution grids under typhoon disasters. First, a probabilistic generation model of typhoon is established. Second, a spatiotemporal vulnerability model of the distribution grid lines to quantify the spatiotemporal impacts of typhoon. Third, a breadth-first search algorithm is used to island the distribution grid, and the amount of load shedding of the islanded microgrid is calculated. Meanwhile, the resilience of the distribution grid was quantitatively assessed according to the proposed new resilience index. Finally, the feasibility of the proposed resilience assessment method is verified in the IEEE 33-bus test system, and the results show that the proposed method can accurately account for the impact of typhoon on the distribution grid and provides a quantitative reference basis for later power system planning and scheduling.
