Abstract

The length of hospital stay after birth should be individualized to meet the needs of mother and baby, and a plan for appropriate follow-up should be in place before discharge from the hospital. The Maternal and Child Health Bureau (MCHB) of the Health Resources and Services Administration studied postpartum discharge policies because evidence suggested that discharge before 48 hours may present physical and psychological hazards to mothers and babies. However, the problem appears to be not simply the length of stay. The MCHB recommended that discharge should occur when mother and child are medically stable, newborn screening tests have been accomplished, and appropriate education of the mother has occurred. Abstract

The purpose of this study is to examine the trend of usual source care (USC) rates and the discipline of choice among rural and urban older adults. Data used in this study were obtained from 2004 to 2010 Medical Expenditure Panel Survey. The yearly percentages of having a specific discipline as the USC were demonstrated from 2004 to 2010. The association of residence with likelihoods of having any of these three physician categories as USCs was assessed holding other individual characteristics constant. The overall sample size is 24,834, of which 20.0% of older adults resided in rural areas. A similar percentage of older adults living in urban areas had USCs than that of rural areas (93.69% vs. 93.46%). Overall, family practitioners were the most common USC noted in this survey while geriatricians are the least. The urban older adults are more likely than the rural ones to have geriatricians as their USC. Geriatrician discipline receives the most intensive education and training to care for older adults. The growing aging population makes it imperative to address the crisis of geriatrician shortage. [ABSTRACT FROM AUTHOR]

Abstract

The article "Reconsidering Community-Based Health Promotion: Promise, Performance and Potential," by Merzel and D' Afflictti in the issue of the journal makes a valuable contribution to the literature on community approaches to health promotion. The breadth of studies covered in this review article, combined with the prominence the Journal is giving to the subject in this issue, suggests how far the field has come in its understanding of the links between public health and communities. The authors summarize in any of the community-based studies since 1980 end draw useful conclusions for strengthening community based efforts at improving the health of the US population. Moreover, by drawing from the lessons learned from human immunodeliriency virus prevention programs, they provide significant recommendations for improving the potential of community-based strategies. However, one would like to draw the readers' attention to some of the substantive issues involved in reviewing such a diverse literature including a number raised by Merzel and D' Afftitti.

41. Changing Presentation of Herpes Simplex Virus Infection in Neonates.

Abstract

We compared the clinical presentation of 95 newborns with herpes simplex virus (HSV) infection from 1973 through 1981 (first period) with data from 196 newborns evaluated from 1982 through 1987(second period). There was a significant change in the presentation of infection in these infants. From the first to the second period, the frequency of disseminated disease decreased from 50.5% to 22.9%, whereas the frequency of skin, eye, and mouth (SEM) diseases increased from 17.9% to 43.4% (P < .001). The frequency of infants with central nervous system (CNS) disease remained relatively unchanged - 31.6% versus 33.7%. We also compared the demographic and clinical characteristics of the infants and their mothers. For neonates with CNS or disseminated infection, disease duration and frequency of prematurity were significantly decreased in the second period, as was the frequency of skin vesicles for newborns with SEM or disseminated infection. These changes are most likely the consequence of recognizing and treating SEM infection before its progression to more-severe disease. [ABSTRACT FROM PUBLISHER]

45. Recovery of Anaerobic Bacteria From Pediatric Patients: A One-Year Experience

Abstract

• During 1975, 115 specimens from the soft tissues, body fluids, and purulent sites in 105 pediatric ward and nursery patients were submitted to the clinical laboratory for anaerobic study, and 75% of the cultures were positive. In most instances specimens were collected in anaerobic transport tubes containing peptone yeast extract and glucose. In the laboratory, samples were processed by the anaerobic holding jar method. Nineteen different anaerobic organisms were isolated; the average number of species per specimen was 1.8. Bacteroides sp were the most common isolates (45 strains). Other isolates included 44 anaerobic cocci (15 of which were Peptostreptococcus sp), 22 Propionibacterium sp, ten Fusobacterium sp, nine Clostridium sp, eight Veillonella sp, six Bifidobacterium sp, and four Eubacterium sp. The main sources of anaerobic isolates included 52 gastric aspirates, 46 soft tissue sites, 14 blood specimens, 12 CSF samples, and 12 samples from the external ear canals. Forty-one (43%) of the 86 specimens that contained anaerobic bacteria also had aerobic organisms present.(Am J Dis Child 133:1020-1024, 1979)

Abstract

The main objective of this study was to define the role of EBV in the progressive evolution of AIDS by means of a prospective evaluation of persons with HIV infections. Detailed EBV antibody responses and virologic studies to determine the EBV load in body fluids and EBV strains were performed serially. Current findings from this study indicate that reactivated type EBV infections are particularly prominent in the initial stages of the underlying HIV-1 infection. However, serologic response to EBV remains quite intense and abnormal through-out the early and late stages of the HIV-1 infection. The serologic findings are reflective of the abnormally elevated burden of transforming EBV present in oropharyngeal secretions and peripheral blood lymphocytes of patients with HIV-1 infection in all WR classes. Interestingly, the replicating form of the virus remains at highly elevated stages, i.e. Walter Reed classes 5 and 6. These high levels of EBV should be directly responsible for the emergence of EBV-related manifestations such as lymphoproliferative lesions and the increasing number of B cell lymphomas being reported in HIV-1 infected individuals. The tongue lesions, oral hairy leukoplakia, which may portend a poorer prognosis of the underlying HIV infection, represent a recently defined example of the EBV-related lesion containing an extremely high level of replicating EBV. The mechanism(s)responsible for the immunodysregulation of EBV in patients with HIV infections that permit the development of these EBV-related lesions is being investigated. (SDW)

The consequences of this highly increased burden of EBV in HIV patients are not well understood but could support the possibility that EBV may be a stimulus to produce tumors or other lymphoproliferative manifestations. The subgroup of patients with oral hairy leukoplakia (OHL) had an overall body burden of EBV, including a high density of replicating virus in the oral lesions, and more abnormal EBV antibody responses than HIV-infected personnel lacking OHL. This finding further strengthened the speculation that EBV is not well immunoregulated in HIV-infected patients and can lead to various lesions, some as lymphomas (reported by others) being quite deleterious. No correlation was noted between specific EBV genotypes and progression of the HIV infection, but the numbers tested (and the changes in disease progression) were small at this point in the study.
