2. Die Greenlight-Laservaporisation der Prostata zur Therapie des benignen Prostatasyndroms

Abstract

Zusammenfassung: Die Greenlight-Laservaporisation hat in den letzten 10 Jahren breiten Einsatz in der Therapie des benignen Prostatasyndroms (BPS) gefunden. Neben der Steigerung der Effektivität des Lasers durch eine Erhöhung der maximalen Leistung auf zunächst 80 W und später 120 W führten auch die guten Ergebnisse der klinischen Evaluationen zu einer zunehmenden Akzeptanz dieses Lasersystems. In der vorliegenden Arbeit werden wesentliche Ergebnisse der klinischen Evaluation der Technik aus den letzten Jahren dargelegt, an denen Schweizer Kliniken maßgeblich beteiligt waren. Hier sind v. a. die geringe peri- und postoperative Morbidität sowie die bisher guten kurz- und mittelfristigen funktionellen Ergebnisse zu nennen. Neben diesen Vorteilen, die v. a. in der nahezu blutungsfreien Durchführung des Eingriffs begründet liegen, werden jedoch ebenso die Nachteile und Limitationen der Laservaporisation beleuchtet, die möglicherweise auch spezifische klinische Nebenwirkungen zur Folge haben. Der zukünftige Stellenwert der Technik wird daher auch vor dem Hintergrund der noch ausstehenden Langzeitdaten weiter evaluiert werden müssen.

Abstract

India has the second largest population in the world and is characterized by a broad diversity in climate, topography, flora, fauna, land use, and socioeconomic conditions. To help ensure food security in the future, agricultural systems will have to respond to global change drivers such as population growth, changing dietary habits, and climate change. However, alterations of how food is produced in the future may conflict with other UN Sustainable Development Goals (SDGs), such as the protection of land resources and climate change mitigation. It is crucial for decision‐makers to understand potential trade‐offs between these goals to find a balance of human needs and environmental impacts. In this paper, we analyze pathways of agricultural productivity, land use, and land‐cover changes in India until 2030 and their impacts on terrestrial biodiversity and carbon storage. The results show that in order to meet future food production demands, agricultural lands are likely to expand, and existing farmlands need to be intensified. However, both processes will result in biodiversity losses. At the same time, the projections reveal carbon stock increases due to intensification processes and decreases due to conversions of natural land into agriculture. On balance, we find that carbon stocks increase with the scenarios of future agricultural productivity as modeled here. In conclusion, we regard further agricultural intensification as a crucial element to help ensure food security and to slow down the expansion of cropland and pasture. At the same time, policies are required to implement this intensification in a way that minimizes biodiversity losses.

Abstract

When modelling medium and long-term challenges we need a reference path of economic development (the so-called baseline). Because sectoral models often offer a more fundamental understanding of future developments for specific sectors, many CGE modeling teams have adopted different kinds of linking approaches to sectoral models to generate baselines. We systematically compare and discuss approaches of linking CGE models for the baseline calibration procedure, and discuss best practices and pitfalls. We identify different types of linking approaches which we divide into a) links with partial equilibrium models, and b) links with non-economic models. These two types of linking approaches are then analyzed with respect to e.g. the degree of linkage, information exchanged, as well as compromises in aggregations and definitions. Based on this, we discuss potential drawbacks and conclude with suggestions for best practices and research recommendations.

37. Lowering the PSA threshold for prostate biopsy from 4 to 2.5 ng/ml : influence on cancer characteristics and number of men needed to biopt

Abstract

Objective: In 1999 we lowered the prostate-specific antigen (PSA) threshold for prostate biopsy at our institution from 4 to 2.5 ng/ml. The aim of this study was to compare the differences in tumor characteristics of the detected prostate cancers (PCAs) and the detection rate for the two different PSA thresholds and to evaluate if lowering the threshold was justified by any of the detected differences. Patients and Methods: We retrospectively analyzed the records of all patients who underwent an 8-core prostate biopsy between January 1999 and December 2004 and had a PSA between 2.5 and 10 ng/ml. Patients with a PSA between 2.5 and 4 ng/ml (group 1, n = 214, mean age 62.0 years) were compared to patients whose PSA was between 4 and 10 ng/ml (group 2, n = 292, mean age 63.2 years). Patients who were older than 75 years or had a suspicious rectal examination were excluded from this study. Results: Overall, we detected 120 can-cers in 506 patients (cancer yield 23.7%). The cancer yield in group 1 was significantly lower than in group 2 (17 vs. 28%, p < 0.01). In group 1 significantly less Gleason score ≧7 (p = 0.04) and significantly more potentially insignificant cancers (p = 0.03) were identified. In 80 patients who subsequently underwent radical prostatectomy, final pathology revealed no significant differences between the two PSA groups with regard to high pT stages, Gleason score ≧7 PCA or positive surgical margins, respectively. The difference in the absolute risk of being diagnosed with high-grade PCA between a PSA threshold of 2.5 ng/ml and a PSA threshold of 4 ng/ml was 1%. Conclusion: Lowering the PSA threshold for prostate biopsy from 4 to 2.5 ng/ml results in a substantial increase in the number of men who undergo biopsy and may result in an increased detection of potentially insignificant cancers. If total PSA alone is used to determine the need for prostate biopsy, the disadvantages of this lower threshold probably outweigh its potential benefits.
