Introduction: Suicidal ideation and behavior (SIB) are serious problems in people with schizophrenia spectrum disorders (SSD). Nevertheless, relatively little is known about the circuitry underlying SIB in SSD. Recently, we showed that elevated emotional impulsivity (urgency) was associated with SIB in SSD. Here we examined brain activity in people with SSD and elevated SIB. Methods: We tested 16 people with SSD who had low SIB and 14 people with high SIB on a task in which emotion regulation in response to affective pictures was implicitly manipulated using spoken sentences. Thus, there were neutral pictures preceded by neutral statements (NeutNeut condition), as well as negative pictures preceded by either negative (NegNeg) or neutral (NeutNeg) statements. After each picture, participants rated how unpleasant each picture was for them. The latter two conditions were compared to the NeutNeut condition. We compared the emotion-regulated condition (NeutNeg) to the unregulated condition (NeutNeut). Statistics were threshold using threshold free cluster enhancement (TFCE). Results: People in the low SIB group showed higher activation in this contrast in medial frontal gyrus, right rostral anterior cingulate, bilateral superior frontal gyrus/DLPFC, and right middle cingulate gyrus, as well as right superior temporal gyrus. Introduction: Pregnancy‐associated gynecological cancer (PAGC) refers to cancers of the ovary, uterus, fallopian tube, cervix, vagina, and vulva diagnosed during pregnancy or within 12 months postpartum. We aimed to describe the incidence of, and perinatal outcomes associated with, invasive pregnancy‐associated gynecological cancer. Material and methods: We conducted a population‐based historical cohort study using linked data from New South Wales, Australia. We included all women who gave birth between 1994 and 2013, with a follow‐up period extending to September 30, 2018. Three groups were analyzed: a gestational PAGC group (women diagnosed during pregnancy), a postpartum PAGC group (women diagnosed within 1 year of giving birth), and a control group (women with control diagnosis during pregnancy or within 1 year of giving birth). We used generalized estimation equations to compare perinatal outcomes between study groups. Results: There were 1 786 137 deliveries during the study period; 70 women were diagnosed with gestational PAGC and 191 with postpartum PAGC. The incidence of PAGC was 14.6/100 000 deliveries and did not change during the study period. Women with gestational PAGC (adjusted odds ratio [aAOR] 6.81, 95% confidence interval [CI] 2.97–15.62) and with postpartum PAGC (aOR 2.65, 95% CI 1.25–5.61) had significantly increased odds of a severe maternal morbidity outcome compared with the control group. Babies born to women with gestational PAGC were more likely to be born preterm (aOR 3.11, 95% CI 1.47–6.59) and were at increased odds of severe neonatal complications (aOR 3.47, 95% CI 1.45–8.31) compared with babies born to women without PAC. Conclusions: The incidence of PAGC has not increased over time perhaps reflecting, in part, the effectiveness of cervical screening and early impacts of human papillomavirus vaccination programs in Australia. Gender-based fatal violence (femicide) is a preventable cause of premature death. The Counting Dead Women Australia (CDWA) campaign is a femicide census counting violent deaths of women in Australia from 2014. We conducted a cross-sectional in-depth review of CDWA cases Jan-Dec 2014 to establish evidence of antecedent factors and describe femicide in Australia. Victim (n = 81) and perpetrator (n = 83) data were extracted from the CDWA register, law databases and coronial reports. Mixed methods triangulation of socio-demographic and incident characteristics. Women ranged in age from 20 to 82 years of age (44 ± 15.4). There were 83 perpetrators, of which 13 were unknown (not yet apprehended). Known perpetrators (n = 70) ranged in age from 16 to 72 years of age (40 ± 12.7) and 89% were male (62/70). The location of the crime was most frequently the victim's home (49/70). In cases where the relationship between the victim and perpetrator was known (n = 59), over half of femicides were committed by intimate-partners (33/59). Intimate-partner perpetrators were more likely to have a history of violence and commit murder-suicide than other perpetrators. Femicide is overwhelmingly perpetrated by males, with women most vulnerable in their own home and with their intimate partners. Furthermore, intimate-partner femicide is associated with modifiable risk factors, including previous violence and mental health issues, which represents opportunities for early intervention within healthcare settings as practitioners are well-placed to identify risk and provide support. Background and Aims: Connections is a voluntary health program that facilitates access to opioid agonist treatment (OAT) and social services for people with opioid use exiting prison. This study aimed to measure the effectiveness of Connections in reducing recidivism and improving health outcomes for people with a history of opioid use on leaving prison. Design: Retrospective cohort study with quasi‐random allocation to the program. Setting: Public adult prisons in New South Wales, Australia, 2008–2015. Participants: Adults released from custody with a history of opioid use. Of 5549 eligible releasees, 3973 were allocated to Connections and 1576 to treatment‐as‐usual. Measurements Outcomes were return‐to‐custody, all‐cause mortality, and OAT participation. Findings Regression analyses on an intention‐to‐treat basis, and adjusting for baseline propensity scores, comparing patients allocated to Connections versus treatment‐as‐usual showed no difference in rates of return‐to‐custody within 2 years (hazard ratio [HR]: 1.01; 95% confidence interval [CI]: 0.92 –1.12). Patients allocated to the Connections program were more likely to access OAT (odds ratio [OR]: 1.21; 95% CI: 1.06–1.39) and had lower mortality within 28 days of release (0.25% vs. 0.66%; HR: 0.38; 95% CI: 0.14–1.03). Differences in mortality did not persist beyond 28 days. Subgroup analyses showed that allocation to Connections was associated with higher risk of return‐to‐custody within 28 days for Aboriginal and/or Torres Strait Islander (Indigenous) and female releasees. Conclusions: The Connections program for people with opioid use exiting prison did not reduce the likelihood of return‐to‐custody but did facilitate opioid agonist treatment participation on release from prison. Purpose: Patient self-advocacy is valued and promoted; however, it may not be readily accessible to all. This analysis examines the experiences of women in Australia who had cardiac disease in pregnancy or the first year postpartum through the lenses of self-advocacy and gender, specifically seeking to elaborate on the contexts, impacts, barriers, and women's responses to the barriers to self-advocacy. Method: A qualitative study design was used. Twenty-five women participated in semistructured in-depth interviews. Data were analysed using thematic analysis. Results: Analysis of findings generated the following themes: 1) Silent dream scream, 2) Easier said than done, 3) Crazy-making, and 4) Concentric circles of advocacy. Regardless of women's personal attributes, knowledge and experience, self-advocating for their health was complex and difficult and had negative cardiac and psychological outcomes. Conclusion: While the women encountered significant barriers to self-advocating, they were resilient and ultimately developed strategies to be heard and to advocate on their own behalf and that of other women. Findings can be used to identify ways to support women to selfadvocate and to provide adequately resourced and culturally safe environments to enable healthcare professionals to provide person-centred care.
